This surveillance report summarizes data on work-related fatal and nonfatal injuries, illnesses, and hazardous exposures in the mining industry for the 10-year period 1986-1995. The term "surveillance," as used in public health, may be new to many readers of this report. With regard to occupational safety and health, the goal of surveillance is to describe the occurrence of work-related injuries, illnesses, and known hazardous exposures; to identify new hazards that may occur due to the introduction of new technology or other factors; and to assess the effects of preventive measures designed to improve worker safety and health. Surveillance provides an overall picture, which can be used to focus resources on areas most in need of prevention programs or further research. Surveillance generally requires the use of multiple sources of data, all of which have their own strengths and limitations. Surveillance in the U. S. mining industry is complicated by the fact that U.S. national data systems that provide information on mining are based on two different definitions of the mining industry. One is established by the Mine Safety and Health Administration (MSHA), the other by the Standard Industrial Classification (SIC) System [Office of Management and Budget 1987] and the 1980 U. S. Bureau of the Census (BOC) Classification System [U.S. Bureau of the Census 1982]. The mining industry as defined by MSHA is based on the regulatory jurisdiction of that agency. Reporting requirements for injuries, illnesses, and workplace exposures are stipulated under the Federal Coal Mine Health and Safety Act of 1969 and the Federal Mine Safety and Health Amendments Act of 1977. Of note is that the oil and gas extraction industry falls outside of MSHA jurisdiction. The Occupational Safety and Health Administration (OSHA) was charged with enforcement in the oil and gas industry under the Occupational Safety and Health Act of 1970. Under the SIC, the definition of mining covers the extraction of naturally occurring minerals, including liquids (petroleum), gases (natural gas) and solids (coal, metal, and nonmetallic minerals). Exploration and development of mineral properties are also included in the SIC definition of the mining industry. The SIC was developed by the Office of Management and Budget for use in classifying establishments based on the type of economic activity in which they are engaged and serves as the industry classification standard for all establishment based Federal economic statistics. This classification is important for occupational safety and health surveillance because it also serves as the industry classification system for all data on work-related injuries and illnesses collected by the U.S. Bureau of Labor Statistics (BLS). The U.S. Bureau of the Census uses an industry coding scheme similar to that used by the SIC. For the mining industry, the categorization is identical in both systems. Death certificate data made available by the National Center for Health Statistics (NCHS) are categorized under this system. Death certificate data are particularly important for surveillance of mortality from work-related illnesses. Key differences between the SIC/BOC categorization system and the MSHA system can be summarized as follows: • MSHA excludes the oil and gas industry, which is classified as part of the mining industry under SIC/BOC. • MSHA excludes work that is done off of mine property; work off of mine property is included under SIC/BOC if it is performed by an establishment falling into one of the SIC/BOC mining industry codes. • MSHA includes mine-associated mills and processing plants; these are classified under the manufacturing industry, rather than mining, according to the SIC/BOC coding structure. Since the data collected by MSHA and the data collected by BLS and NCHS using the SIC/BOC classification system are both important sources of information on the mining industry and in many cases serve different purposes, data from both systems are presented in this report. In general, data using the SIC/BOC system are comparable across U.S. industries and should be used when comparing mining to other U.S. industries. Much of the data in this report are presented in the form of frequencies or rates. A frequency provides only a count of the number of cases without providing any information about the risk of occurrence. Rates provide an estimate of the risk of injury or illness. The reader is cautioned, however, that rates based on small numbers can be unstable; thus, any inferences should be drawn with care. In this report, no rates are presented for groups in which there were fewer than three cases. The incidence of most injury and illness conditions differs by age, sex, and race. For this reason, data in many surveillance reports are adjusted for these demographic factors. It was not possible to do this for most of the rates in this report because information on demographics was not available. With the exception of the proportionate mortality ratios (PMRs), all data presented are unadjusted. A detailed account of the sources of data is in appendix A. The methods used in data selection and analysis are described in appendix B.

5. General Escapeway Requirements 5.1 Escape Routes Present escapeway regulations state "Every mine shall have two separate properly maintained escapeways to the surface which are so positioned that damage to one shall not lessen the effectiveness of the other, or a method of refuge shall be provided when only one opening to the surface is possible." "Escape routes shall be a. Inspected at regular intervals and maintained in safe, travellable condition, b. Marked with conspicuous and easily read direction signs that clearly indicate the ways of escape." "Application. "Regular" intervals means at intervals no greater than monthly where conditions are stable and oftener where conditions may change." "Designated escapeways inclined more than 30* from the horizontal shall be equipped with stairways, ladders, cleated walkways, or emergency hoisting facilities." As can be noted in present and proposed regulations there must be two separate properly maintained escapeways. In most operations, the escapeways will consist of one route that is the normal travelway used in day to day operations. The second route is usually a return airway through old work areas. Based on this generally accepted practice, we will designate the two types as primary and secondary.

Sphalerite crystals occur sparingly in minor faults and pockets in a coarsely crystalline dolomite on the Brown-Tipton farm near Greenville in Greene County, Tenn. After several visits by U. S. Bureau of Mines engineers and U.S. Geological Survey geologists, a survey was r: the in the fall of 1944. Upon their recommendations, trenches on the Brown farm under Bureau of Mines supervision. In each case, the bed most favorable for zinc deposition was exposed. In the finished trench bottoms no zinc minerals are visible.

Powered haulage has been, and continues to be, a major source of severe accidents and fatalities at metal/nonmetal surface mines. Between 1986 and 1997, truck drivers accounted for63% of the lost-time injuries in surface haulage. This project was undertaken to reduce the number and severity of lost-time injuries among operators of these trucks. Work involved measuring shock acceleration at a western surface mine during representative work cycles on two types of trucks and collecting data from cab floors using a triaxial accelerometer and from operator seats using a seat pad accelerometer. NIOSH researchers also used the mine’s GPS systems to locate shock events on a mine map in real time. Shock tests were also run at Caterpillar, Inc.’s, proving grounds in Green Valley, AZ, to determine the magnitude of shocks resulting from controlled rock drops onto the bed of a haulage truck.

To minimize the equipment operator?s respirable dust exposure in enclosed cabs of mobile equipment by install-ing cab filtration and pressurization systems and by keep-ing doors and windows closed. Background Enclosed cabs are used on mobile equipment to protect operators from health and safety hazards. The primary health concern is overexposure to respirable dust. Examples of such equipment in mining include drills, dozers, haul trucks, and other equipment. When equipment is new, the operator?s dust exposure is normally at acceptable levels. As the equipment ages and gaskets and seals deteriorate, the air quality inside an enclosed cab can reach a point where it is no longer at acceptable levels for the equipment operators. In an effort to improve the air quality in enclosed cabs of older mining equipment, NIOSH has been performing research to retrofit cabs with new filtration and pressurization systems. Over the past few years, this research has shown that retrofit systems can substantially improve the cab?s air quality back to safe and acceptable levels. How-ever, this is only possible when a positive cab pressure is achieved and maintained. Thus, when a cab door or window is opened, the cab pressure drops to zero and the cab filtration and pressurization system is rendered ineffective. During a recent field study, the magnitude of this impact was noted when a drill operator repeatedly opened the cab door throughout the course of the drill cycle. Upon closer examination, it was determined that the cab door was being opened to manually guide the next drill steel into place each time an additional section of steel was needed to drill deeper holes.

Reduce potential for steering errors during tramming of articulated vehicles, particularly when the operator is seated to the rear of the machine pivot point and is tramming in reverse. Approach Position-sensing and feedback cylinders were applied to the steering cylinders of two S&S cleanup tractors to cause the steering-control lever to match the orientation of the vehicle sections. The vehicles were tested in two working mines. How It Works A position-sensing cylinder (see Figure 1, Item A) is attached to one of the steering cylinders in the cleanup tractor steering system. As the steering cylinder piston rod is actuated by movement of the control lever, hydraulic oil flows from the position sensing cylinder to the feedbck-cylinder (B), causing the feedback-cylinder piston and rod to move. The outside end of the rod repositions the the steering-control lever until its angular position corresponds to the orientation of the vehicle sections.

This trainer’s guide is designed as a reference manual. The purpose is to offer information and examples to skills trainers to assist them in structuring training for new operators of Walk-Thru roof bolting machines. Trainers can use this manual as a resource for helping trainees learn, understand, and apply knowledge and skills. In this sense, the guide is not prescriptive. It is designed to offer information and trigger ideas on what might be done to accelerate learning to those who are new to the roof-bolting task. On-site trainers can modify this guide to fit their conditions, machines and equipment, and work procedures. They can integrate roof control plans, company policies and procedures, and operators’ manuals from the manufacturers of the original equipment into this guide. Every trainee is different and will come to the job with varying levels of relevant knowledge and skills. The trainer’s decisions on where to start, how to organize the on-the-job and classroom training portions, and when and how to offer different aspects of the training will be based on (1) a pretraining assessment of the trainee’s knowledge and skills and (2) the trainer’s experience in conducting skills training. The “Skill Check” section might be useful for deciding where training should start among miners who have underground experience but limited knowledge of roof-bolting techniques. Every job carries a learning curve and likely has a number of “teachable moments” where significantly new knowledge and skills can be learned. The videos on the DVD that accompany this guide can be shown at the beginning of training to give trainees an idea of what the job entails and related requirements. They can also be used within the training cycle (after the trainee has some experience in the job) to reinforce key points or as a trigger for eliciting questions about particularly difficult parts of the job. The video segments are meant to be visual models of good performance. They are designed to elicit discussion on good (lower risk) ways to do the job. In this context, they can be used very creatively at different times within the training cycle. The section on “Talking Points” offers a number of questions that can be used in the classroom to support and elaborate on certain aspects of the job duties. If the trainee has a fair amount of underground experience, some of the questions can also be used to support the pretraining assessment (i.e., skill check) of knowledge and skills. Likewise, some of the questions can also be used while teaching on the job to reinforce key points affecting safety, production, or maintenance. The section dealing with a mine’s roof control plan should be used at various times in the training cycle to make sure the trainee understands the plan and knows how to apply it in performing the job. The supplemental materials offer additional resources that can be used to support initial job training or follow-up. Learning continues well after an individual has been trained. The supplemental materials can help reinforce good operating skills and provide an opportunity for further discussion on better ways to do the job.

Ferromanganese continues to be an indispensable requirement in steel manufacture. Approximately 470,000 of the 80 per cent ferro-alloy was consumed in 1929 and increasing amounts will be required annually by the steel industry unless some satisfactory substitute is found. An average of 14 pounds of metallic manganese is used for each ton of steel produced, The principal use of manganese is for de-oxidation of the metal in the steel refining processes, although a considerable amount is also used for the production of special manganese steels. Unfortunately, domestic production of manganese ore is inadequate to meet normal peace-time requirements, not to mention increased abnormal war-time demands. The rather large quantities of low-grade manganiferous iron ores which occur in the Coruna Range in Minnesota have for a number of years greatly appealed to those interested in the development of a domestic source of the supply. The North Central Experiment station of the Bureau of Mines has made an elaborate study of the possibilities of the pyrometallurgical treatment of ores from these deposits and much of the valuable information secured by them has already been published.

In the course of a study of the formation of external deposits on tubes in water-tube boilers it became desirable to determine the sulfur dioxide (SO2) and sulfur trioxide (SO3) in the Due gases. Determinations made by the A.S.M.E. method for SO2 and SO3 in gaseous products of combustion did not give accurate results. It was found that minute quantities of copper, present in the flue gases from the coal in use, caused oxidation of sulfite in the absorption bottle and that the carbon dioxide (CO2) in the flue gases caused high values for SO2. The A.S.M.E. test procedure was revised to prevent oxidation of sulfite in the absorption bottle and to allow for the effect of CO2; subsequent analyses of flue gases produced reliable results. The revised procedure is described.

During 1981, the U.S. Bureau of Mines and U.S. Geological Survey conducted a mineral resource survey of the 6,400 acre (15,800 ha) Lincoln Creek RARE II area (No. 5983) in the Toiyabe National Forest (fig. 1). Samples of outcrops and stream channels contained no significant amounts of economic minerals. A radioactive deposit in a pegmatite dike occurs 1.25 mi (2.02 kin) southeast of the study area, hut similiar deposits have not been discovered within the area boundary. No coal or deposits were found.

Groundfalls are much more likely to occur in coal mine intersections than in entries. NIOSH is using the experience of U.S. coal mines to determine the factors which influence intersection instability and provide guidelines for the safe excavation and support of intersections. Detailed field investigations have resulted in a database of U.S. coal mines containing 12 mines and 639 roof falls so far. By using the roof fall rate as the outcome variable, correlations between roof geology (CMRR), intersection span, and roof support have been established. Case studies have indicated that replacing 3- way intersections with 4-way intersections may not reduce the total number of roof falls. Additionally, the size of intersection spans tend to decrease with lower (weaker) CMRR. Protocols have been established for the collections of roof bolt parameters. The performance of individual roof bolts can now be tracked with roof fall rate.

This pamphlet and the enclosed video describe a safe, effective and economical method for removing dust from work clothes. Background Information Dusty work clothes can increase a worker’s exposure to respirable dust. Respirable dust is usually considered to be particles having an aerodynamic diameter of 10 microns or less and able to enter and deposit in the gas-exchange region of the human lung. A former U.S. Bureau of Mines report documented a 10-fold increase in worker dust exposure on a number of separate occasions from dusty work clothes.* It was found that once clothes become contaminated, they are a continual dust source until cleaned. The only MSHA approved method to clean work clothes requires a HEPA-filter vacuuming system which is very time consuming and difficult to perform effectively. Therefore, most workers use a single air hose (not MSHA approved) which can elevate dust levels for the worker, co-workers, and the work environment. A substantial amount of engineering technology has been developed that, when applied, can significantly lower respirable dust exposures of workers at mineral processing plants. But even with these improvements, some workers continue to exceed their permissible exposure limit. Elevated exposures increase the potential for developing debilitating or fatal lung diseases. ?e clothes cleaning method described within is an example of a continuing effort to reduce the potential for developing these lung diseases. The Bureau of Mines Information and Analysis Directorate collects worldwide data on virtually every commercially important nonfuel mineral commodity. These data form the base for tracking and assessing the health of the minerals sector of the U.S. economy. The Bureau's data collection activity was instituted by the 47th Congress in an appropriations act of August 7, 1882 (22 Stat. 329), which placed the collection of mineral statistics on an annual basis. The most recent authority fur the Bureau of Mines survey activity is the National Materials and Minerals Policy, Research and Development Act of 1980 (Public Law 96479, 96th Congress), which strengthens protection fur proprietary data provided to the U.S. Department of the Interior by persons or firms engaged in any phase of mineral or mineral-material production or consumption.

The Bureau of Mines investigated the flotation responses of two copper-nickel ore samples from the Duluth Complex with the objective of recovering bulk sulfide concentrates. One of the ores studied was taken from a test pit; the other was taken from a test shaft. The samples were quite similar except that the pit sample analyzed 0.35 pct Cu and 0.11 pct Ni and the shaft sample analyzed 0.69 pct Cu and 0.14 pct Ni. Flotation responses were studied in both laboratory and pilot plant tests. Pilot plant flotation responses were similar for both samples resulting in grades of 12.2 pct Cu and 2.5 pct Ni for the pit sample and 12.2 pct Cu and 2.1 pct Ni for the shaft sample. Weight recoveries were 2.5 pct for the pit sample and 5.2 pct for the shaft sample. Copper and nickel recoveries for the pit sample were 87 and 62 pct, respectively, and 92 and 73 pct for the shaft sample. Both concentrates contained small but significant values of cobalt and precious metals. Cobalt recoveries were low--less than 40 pct for both concentrates. Precious metals contents of the concentrates were 1.59 oz/ton for the pit sample and 0.99 oz/ton for the shaft sample. Silver was the predominant precious metal by weight, but platinum and palladium represented the highest values.

High-temperature enthalpy and X-ray powder diffraction studies were con-ducted on aluminum sulfide (Al2S3) as part of the Bureau of Mines effort to provide new data for the advancement of mineral technology consistent with environmental preservation and energy conservation. Relative enthalpies were measured with a copper-block calorimeter from 298.15 to 879 K. Tabulated values are listed for heat capacity, entropy, Gibbs energy functions, and relative enthalpies between 298.15 and 900 K. Enthalpies were also given in the form of an equation and combined with published data to calculate standard enthalpies of formation and Gibbs energies of formation for reactions of crystalline aluminum with crystalline, liquid, and gaseous diatomic sulfur. The hexagonal, rhombohedral, and cubic phases of Al2S3 were identified by X-ray powder diffraction studies, and the indexed powder diffraction patterns of these phases are given. The hexagonal phase was stable to about 852 K and was partially converted to the rhombohedral form at 879 K.

Provide an operator cab to assure protection, comfortable use of controls, and good visibility. Approach A cab which raises and lowers by pivoting about d horizontal axis was Installed on two S&S cleanup tractors and tested in two underground working mines. How It Works Hydraulic cylinders actuated by a lever inside the cab cause the cab to pivot about a horizontal axis. This action raises or lowers the cab height. The total cab excursion is 14 inches. A release valve in the hydraulic system allows the cab to lower automatically if struck by a roof load or overhead obstruction. The pivoting action of the cab is engineered so that the operator maintains comfortable use of all controls at all cab heights. The movement is such that the angular relationship of the operator's ankles With the tram and brake foot controls IS consistently within 90 to 100 degrees. Figure 1 outlines the approximate position of the operator at three cab elevations.

The Bureau of Mines conducted research into the dispersion strengthening of lead by the coprecipitation method. Commercially available reagents in water solutions were reacted to form homogeneous precipitates of lead and aluminum compounds. The resulting precipitates were roasted to oxides and reduced in hydrogen to form mixtures of lead powder and alumina. The powders were isostatically pressed into billets which were extruded into rods. The rods were rolled into strip to provide material for testing. Alloys containing 0.5, 1, 2, and 3 nominal volume-percent A1203 were tested in short-time tensile tests and in tensile creep tests. An alloy of Pb-3 volume-percent A1203 exhibited an average tensile strength of 8,570 psi after rolling to 75-percent reduction at 125° C but had no measurable elongation. Alloys of lower A1203 content provided a better compromise between tensile strength and ductility. For example, an alloy of Pb-0.5 volume-percent A1203 had an average tensile strength of 6,590 psi with 9-percent elongation after rolling to 75-percent reduction at 125° C. Creep rates of dispersion-strengthened Pb-A1203 alloys were below 1 percent in 10,000 hours at stresses of 2,000 to 4,000 psi.

An inusual job in controlling a gas well was executed on No. 1 well, Inland Oil and Refining Co., situated on the Alkali Butte structure, Freemont County, Wyoming. The work was done by the company in cooperation with the . S. Bureau of Minos, and resulted in a fuel saving of $5,000 in one season and of a repair job estimated at $15,000, or a total of $21,000. The woll had an open flow of 6,000,000 cubic feet of gas per day with a rock pressure of 560 pounds per square inch. A string of 6-1/4 inch casing with a toothed shoe was set on the cap rock above the gas sand at 2181 feet. Then the well was shut in, the gas worked out around both the 6-1/4 inch and 6-1/4 inch strings, breaking through the ground surface at several points under the derrick. Casing troubles encountered while drilling made it unsafe to attempt Coving the 6-1/4 inch string. Moreover, it was important from the standpoint of good management to make the gas from this well immediately available for fuel in other operations and, in addition, to save the gas from wasting. An ordinary mudding and cementing job would have necessitated sitting down this well for probably two months out of a very short operating season. For these rousons it was determined to mud the well, if possible, between the 6 and 8 inch strings by pumping mud fluid through the connection from the gas clamps. The gas clamps used wore of ordinary construction with a side outlet, making them equivalent to a Braden-hoad. When the mud fluid was pumped through the gas clamp into the space between the two strings, it began to appear inside the 6-1/4 inch string. It apparently entered the well either around the toothed shoe or through a "line sawed" leak in the string and was blown out of the well as fast as it was pumped in. In order to stop the leaks, about a half a gunny sack of finely chopped bull ropa was fed with the mud between the casings as before. The hemp gathered around the valves of the mud pump to some extent, and it also stuck in the casing-head fittings a few times. An obstruction in the casing- head would be removed by permitting the woll to "blow back" for a few seconds, and one in the pump valves, by removing the valves when necessary. In two days' time sufficient hemp and mud had been pumped into these strings to kill external and internal pressure. The wall was shut in and allowed to stand over night. The production of harmful pollutants from coal mines and/or from coal and associated strata has been a recognizable fact in the United States for over two hundred and seventy years. In 1698, Gabriel Thomas observed(l) that the colored water flowing from streams in this country was similar to that which flowed from the coal mines in Wales. Hence, water pollutants such as acid were being produced before any known coal mines were operating in this country. The coal mining industry has contributed to the increase of pollution by exposing large amounts of sulfide materials that enable the reaction of water, oxygen and sulfur containing materials to form acid. A map of the United States coal fields can be seen on Figure 1. The anthracite regions of Pennsylvania, although small when compared to the total reserves, have produced some of the country's major water problems. The map indicates the extent of bituminous reserves of the Appalachian and midwest regions. Also evident are the bituminous, subbituminous and lignite coal reserves in the west which have recently come into greater demand. Acid mine pollution is encountered in numerous states, but the major problem is found in the Ohio, Susquehanna, and Potomac River basins.(2) Most coal beds contain some sulfur which may be in the inorganic, sulfate, or sulfide form, and thus have the potential to produce sulfuric acid. However, much of the strata that overlays the coal beds also contains acid neutralizing materials. Therefore, the amount of acid produced depends upon the amount of both acid and alkali materials that dissolve in the water in any given mine. The common sources of sulfurous material in the seam or surrounding rock are generally called sulfur balls or "nigger heads" by the miners. Sulfur balls that have been observed(3) seem to be equally divided between those containing sulphuritic iron capable of producing sulfuric acid upon oxidation and those that are primarily calcium carbonate. It has been observed in many instances that two types of water, acid and alkaline, are being produced in the same mine. A study of the geological strata and a survey of subsidence, faults and sulfur balls will usually give some indication of why this happens. The reaction of pyrite (FeS2) with oxygen and water to form ferrous sulfate (FeSO4) and sulfuric acid (H2SO4) by a series of complex chemical interactions is well documented but not well understood.(4,5) However, it is well known that the reactions to form the acid cannot occur unless all three constituents are present. Since pyrite is inherent in most coal seams and air is needed to sustain the miner, the most logical answer is to eliminate water infiltration into the mine, or to remove it from the mine before acids can form.
