This report (Independent Functional Safety Assessment 4.0) is the fifth in a nine-part series of recommendations addressing the functional safety of processor-controlled mining equipment. It is part of a risk-based system safety process encompassing hardware, software, humans, and the operating environment for the equipment’s life cycle. Figure 1 shows a safety framework containing these recommendations. The reports in this series address the various life cycle stages of inception, design, approval and certification, commissioning, operation, maintenance, and decommissioning. These recommendations were developed as a joint project between the National Institute for Occupational Safety and Health and the Mine Safety and Health Administration. They are intended for use by mining companies, original equipment manufacturers, and aftermarket suppliers to these mining companies. Users of these reports are expected to consider the set in total during the design cycle. • 1.0 Safety Introduction.—This is an introductory report for the general mining industry. It provides basic system/software safety concepts, discusses the need for mining to address the functional safety of programmable electronics (PE), and includes the benefits of implementing system/software safety program. • 2.1 System Safety and 2.2 Software Safety.—These reports draw heavily from International Electrotechnical Commission (IEC) standard IEC 61508 [IEC 1998a,b,c,d,e,f,g] and other standards. The scope is “surface and underground safety-related mining systems employing embedded, networked, and nonnetworked programmable electronics.” System safety seeks to design safety into all phases of the entire system. Software is a subsystem; thus, software safety is a part of the system’s safety. • 3.0 Safety File.—This report contains the documentation that demonstrates the level of safety built into the system and identifies limitations for the system’s use and operation. In essence, it is a “proof of safety” that the system and its operation meets the appropriate level of safety for the intended application. It starts from the beginning of the design, is maintained during the full lifecycle of the system, and provides administrative support for the safety program of the full system. • 4.0 Safety Assessment.—The independent assessment of the safety file is addressed. It establishes consistent methods to determine the completeness and suitability of safety evidence and justifications. This assessment could be conducted by an independent third party. • 5.0 Safety Framework Guidance.—It is intended to supplement the safety framework reports with guidance providing users with additional information. The purpose is to assist users in applying the concepts presented. In other words, the safety framework is what needs to be done and the guidance is the how it can be done. The guidance information reinforces the concepts, describes various methodologies that can be used, and gives examples and references. It also gives information on the benefits and drawbacks of various methodologies. The guidance reports are not intended to promote a single methodology or to be an exhaustive treatment of the subject material. They provide information and references so that the user can more intelligently choose and implement the appropriate methodologies given the user’s application and capabilities. [ ]

Avoid errors in determining the amount of trace elements in overburden when core drilling samples, particularly as required by PL 95-87, the Surface Mining Control and Reclamation Act of 1977. Reason Metal additives present in certain drill stem joint lubricants appear to contaminate overburden samples. If these lubricants are used in assembling the drill string, they can interfere with the analytical methods used to establish concentration of trace metals. Prior to 1854 the provincial legislatures in Austria enacted their own mining laws. The confusion that resulted from the existence of different codes when mining began to be conducted by modern methods forced the drafting of a law to cover all mining operations of the country. Such a general law (allegemines österreichisches Berggesetz) came into effect May 23, 1854, in the old monarchy, with the exception of Hungary, which had its own special laws. This general law of 1854 is still the basic mining law for Austria, although it has been amended and supplemented by a mumber of acts.

The Crow Indian Reservation is located in the southeastern part of Montana, stretching from near Billings, in the center of the state, to the Wyoming border in the south. The Office of Surface Mining's Casper, Wyoming, Field Office is responsible for regulating mining and reclamation on Crow tribal land. Federal legislation enacted July 11, 1987, authorized the Indian tribes to implement approved Abandoned Mine Land Reclamation Programs. In early 1988 the Crow elected to develop an Abandoned Mine Land Program under the Title IV provisions of SMCRA. The tribe's plan received final approval on January 4, 1989.

In addition to developing various types of coal-interface detection system, the US Bureau of Miner is actively in involved in developing a coal rib-thickness monitoring system for highwall mining applications. One particular system that shows promise in a ground-pentetrating radar system to detect the coal-air boundaries of coal rifts. Based on preliminary laboratory and field tests results indicate that this method can be applied successfully in highwall mines to maintain constant rib thickness. The coal seam should be relatively clean in the vicinity of the antenna (e.g., have minimal partings or dirt bands) for best results.

The Bureau of Mines, U.S. Department of the Interior, studied the use of manganese sea nodules for catalytic oxidation in air of 1 pct carbon monoxide, 1 pct propane, and 1 pct methane. Catalysts were prepared from both Pacific Ocean and Atlantic Ocean nodules and compared with a platinum catalyst using a fixed-bed, steady-state microreactor with gas chromatographic analysis. Combustion temperatures with manganese nodules were 50° to 100° C lower than those with the platinum catalyst. Ion-exchange addition of rare-earth promotors to the nodule catalyst decreased combustion temperatures an additional 5° to 45° C.

Slope stability accidents are one of the leading causes of fatalities at U.S. surface mining operations. The Spokane Research Laboratory of the National Institute for Occupational Safety and Health (NIOSH) is currently conducting research to reduce the fatalities associated with slope failures. The purpose of this paper to discuss some of this research and to present potential new technologies for slope monitoring and design. The paper also briefly discusses various warning signs of slope instability, introduces the most common slope monitoring methods, describes the limitations of various slope monitoring systems, and presents some field results using some of this new technology.

A 180° magnetic deflection residual gas analyzer mass spectrometer with a mass range of 2 to 200 amu was used to study the nature of gases evolved during arc and electron-beam melting of certain reactive and refractory metals. The partial pressure of residual gases over molten Mo, Hf, Ti, and Fe, was found to be similar: Water and hydrogen were evolved from the samples and from the furnace components, carbon dioxide and carbon monoxide resulted from deoxidation of some of the melts. Argon and acetone were residuals in the system, and methane was a decomposed organic compound from the oil diffusion pump.

The high price of pollucite ore, its limited availability, and the need to provide stack removal of S02 present formidable obstacles to the use of pure cesium as a seeding material in MHD power generation. Cost estimates indicate that mixed potassium-cesium seeding is preferable to the use of either pure cesium or pure potassium. Cheaper electricity and higher open-cycle MHD operating efficiencies result from the use of mixed seed. Savings of $3,000,000 per year in power costs for a 1,000-MWE open-cycle MHD power-plant are envisioned if the mixed seeding technique is adopted.

Florida has accumulated about 400 million tons of phosphogypsum, a mineral process waste generated in the production of fertilizer, and is adding 30 million tons to this annually. To minimize storage of this waste and reduce potential environmental effects, the Bureau of Mines conducted research into utilizing the phosphogypsum, by identifying and developing high-volume uses for this material. Admixtures of phosphogypsum, fly ash, and lime that could produce a coarse aggregate suitable for road construction were investigated. Using phosphogypsum with asphalt, cement, cement-kiln dust, silica, clays, and oxychlorides to pro-duce aggregate was also examined.

"The Phosphate Rock Market:Indications, in the fertilizer market, point to a prosperous condition, caused chiefly by the growing demand for all classes of fertilizer material in spite of a shortage of supplies. Both of these conditions have been caused by the war. The shortage in phosphate rock is being relieved as production increases, but the supply has not caught up with the demand for this spring's trade, at least. Reports from Florida are to the effect that more than half the necessary quota of laborers are at work. This may not appear to be a very good showing, but even this situation is en¬couraging because there is a larger force now at work than has been the case for some time, and it is steadily, though slowly, increasing. Some of the producing companies are conducting advertising campaigns to get laborers back to the phosphate fields and attractions in the way of better housing and living conditions are being offered.Producers are far more confident about the situation than they were two months ago, and though the shortage will not be met before the end of 1920, theme is a surplus accumulating over what is going out on existing contracts.Quotations are now being offered on all Florida rock excepting land pebble and this, it is expected, will be soon quoted.Quotations on Florida rock per ton are as follows:"

A method of determining the strength of a mine roof layer in place is described in this report. The method was tested on a limestone roof layer 50 feet wide, 100 feet long, and 20 inches thick. In place measurements were accomplished by introducing air under pressure into a separation between the layer and the overlying roof, and as pressure was increased, the corresponding strain and deflection of the roof layer were measured until the roof failed. The ultimate strength of the roof agreed with the tensile strength of the roof rock (modulus of rupture) as determined in the laboratory; the modulus of elasticity as measured in place did not agree with the laboratory-determined values; the deflection and tensile strain in the layer in place showed good agreement with the theory of a body-loaded beam clamped at both ends. This procedure for determining the strength of a mine-roof layer is safer and less costly than successively widening a roof to failure. For example, it would have been necessary to mine the room to a width of 114 feet and a length of 228 feet to produce failure in this roof layer. The method is not necessarily limited to limestone; any thinly bedded rock type can be tested by this procedure when there is an open parting over the mined area.

Overexposure to noise remains a widespread, serious health hazard in the US mining and other industries despite 25 years of regulation. Most categories of illnesses and injuries associated with mining have improved, with the exception of hearing loss. The drilling of rock in a confined work environment contributes to high levels of noise exposure in mining. The National Institute for Occupational Safety and Health (NIOSH) is conducting research to reduce the noise exposures of jackleg drill operators and to prevent additional cases of noise induced hearing loss (NIHL) by developing and evaluating low-cost retrofit noise controls for equipment. This report describes the procedure for the measurement and reporting of noise from portable pneumatic tools such as jackleg drills. The technique used in this research allows for the determination of the source A-weighted sound power levels and the radiation patterns in octave and 1/3 octave bands. Overall sound power level is also determined. This paper also reports the results obtained by using this procedure to evaluate a SECO S215 standard production drill and a CSIR Miningtek prototype rock drill incorporating engineering noise control measures. It was found that by using the manufacture?s recommended operating pressure of 496 kPa (72 psi) that the CSIR prototype?s sound power was 10 dB(A) less then that of the SECO S215.

The Bureau of Mines conducted corrosion tests of 18 commercial alloys in H3 P04 -phosphate rock or A1,03 slurry, in 10 pct HF, and in 25 pct H2 SiF6 to simulate three environments anticipated in a Bureau process to extract fluoride values from phosphate rock. Small alloy coupons were rotated in duplicate tests at 4.4 to 21 fps in the slurry at 170° C to determine rates of erosion-corrosion. At 4.4 fps the lowest rates, in mils penetration per year (mpy), were measured for Hastelloy C-276 (<20), Hastelloy B (30), and Inconel 625 (150);the rates increased at 21 fps to 55 mpy, 45 mpy, and 1,000 mpy, respectively. The corrosion rate for all alloys exceeded 40 mpy in either the equilibrium vapor or liquid phases of aqueous solutions containing 10 pct HF or 25 pct H2SiF6 at 100° C. Uniform corrosion in 10 pct HF at 50° C was observed only for Inconel 625 (22 mpy), Hastelloy C-276 (43 mpy), and Illium 98 (17 mpy). Inconel 625 (0.75 mpy), Hastelloy C-276 (1.3 mpy), Illium 98 (1.7 mpy), and Incoloy 825 (0.45 mpy) were attacked uniformly in 25 pct H2SiF6 at 50° C.

INTRODUCTION Present-day Mexican mining legislation involves, not only the latest minim enactment (August 1, 1026), but many other enactments and regulations, principal among which are the Constitution, the Civil Code, the Code of Commerce, the Foreign Law, the Mining Tax Law, and. the Civil Jurisprudence of Mexico--the last-mentioned being based in Dart on Supreme Court decisions. Ere Mexican Federal Government is supreme in mining legislation because, among other reasons, a large part of the revenue of the Government must come from the fiscal control of the mines. All circulars, decrees, and rulings that affect the mines not only have their origin in the Central Government but are administered thereby. The uses of talc are many but are not, in general well known or fully developed within the last decade several companies have realized the possibilities of greatly increased use and have intelligently proceeded to develop some of the fields which seemed most promising. This has had a tendency to lift talc from the obscurity in which it had existed for years, but the work is only started and much remains to be done. Considerably more attention has been paid to the technology and utilization of talc in Germany and Austria than in this country, despite the fact that the United States produces 65 of the world's talc and Germany and Austria combined but 5.4%. In the "Bericht,uber die tatigkeit des verbends der talcum-interessenten in Oesterreich-Ungarn" (Proceedings of the Austro-Hungarian Talc Association), for the years 1912 and 1913, many notes upon new uses appeared, together with suggestions as to further development of old uses. Aside from a few notes on the use of tar in paper, little has been published in the English language upon this subject. By recent action of the Talc and Soapstone Producers Association (U.S.A.) funds for research into the properties and uses of talc have been made Available and it is hoped that markets will be materially broadened a a result of this work.

Provide the technology to remotely control a thin¬-seam continuous mining machine from a safe location hundreds of feet outby the working face in an underground room-and-pillar mine. Background and Approach Hard-wired or radio remote control of continuous mining machine has been commercially available since the early 1970&apos;s. This type of remote control, in low and high coal seams, situates the machine operator offboard, but within direct view of the underground coal face, Although positioned under supported roof, the operator can still be exposed to face area hazards that include roof-rib falls, dust, methane explosions, and equipment congestion. The U.S. Bureau of Mines approach has been to put the operator into a protected compartment at a location away from the working face. Since 1979, the Bureau has investigated innovative methods to better protect face area workers in under¬ground room-and-pillar mines. In 1989, the Bureau demonstrated an offboard, computer-based remote con¬trol system in a West Virginia highwall mining operation that stationed the operator hundreds of feet away from the working face. In late 1992, much of this technology, plus improvements, Was used to demonstrate, on the surface, how a thin-scam continuous mining machine could be remotely controlled 200 feet from the machine in a 36-inch working height.

The U.S., Australian, and United Kingdom coal and the Canadian hard rock mining industries have long recognized the significance of high horizontal stresses as a factor affecting the stability of roof and rib conditions in underground mines. Recently, a growing segment of the U.S. underground stone mining industry has also begun to recognize that horizontal stresses occur in some of its more than 90 mines. Considering the typically high strength and massive nature of limestone, this fact is a revelation in itself. High horizontal stresses produce extensive and sudden rock failures and, in some cases, resulted in injuries to mine workers. Through the years diverse control strategies have been proposed and experimented with. Reorientation of mine entries to reduce stress concentrations have proven successful and are widely accepted in practice. Other solutions, like rock reinforcement, are poorly understood and less accepted in practice. It is the purpose of this National Institute for Occupational Safety and Health (NIOSH) study to develop a better fundamental understanding of these ground control strategies under high horizontal stress conditions through a series of field and laboratory studies. To this end a design technique is presented which provides stone miners with a method for making stability assessments. The consequences of widening rooms, changing geology and horizontal stresses, and different rock bolts on roof beam failures are discussed.

Montana&apos;s coal reserves are estimated to be more than 120 billion tons, 24.6 percent of U.S. reserves. Montana&apos;s reserves are the largest in the nation. Coal fields are located throughout the state, primarily east of the Continental Divide. Three of the state&apos;s 17 coal fields have producing mines. Coal resources range from lignite to high volatile bituminous; however, most of the coal currently mined is subbituminous. Although Lewis and Clark mentioned the presence of coal along the banks of the Missouri River in Montana in 1805, the first major mine was not opened until 1867, at Chestnut, Montana, near Bozeman. The mine at Chestnut served Fort Ellis until the mine was bought by the Northern Pacific Railroad in 1883. By 1900, underground coal mining had developed throughout the state, primarily to supply fuel to the railroads. In the 1920s, large-scale surface mining began in Colstrip to fuel railroad steam engines. Currently, all coal mined in the state is produced from surface mines. The Office of Surface Mining granted the Montana regulatory program conditional approval April 1, 1980, followed by final approval February 1 1,1982. Montana&apos;s Abandoned Mine Lands Program was approved November24,1980, and its emergency program was approved August 18, 1983. A Federal Lands Cooperative Agreement was approved May 8, 1981.

Network analysis of mine ventilation systems is not new, having been traced back to 1854. A Committee Report (1970) has also reported the extensive work done in this respect in the United Kingdom, Japan, U.S.S.R., South Africa, United States, and other countries. Wang, et al (1970) developed a computer program which is an improvement over the earlier one developed by Wang et al (1967). The program has been recently modified and extended to include methane emission features by Owili-Eger (1973). This computer program incorporates all the novel features of the earlier programs. These include the capabilities of mesh selection, free splitting, internal or external fans, natural ventilation pressure and fixed quantity branches. The program is written in Fortran IV and, in effect, is an experimental mine in which many ideas can be tried out. The computer produces, besides the input data and certain computational data, the quantities and head losses in the branches, the head losses to be adjusted in the fixed quantity branches and the fan operating point. The last named program, hereafter to be referred to as the PSU/MVS, is presented in Appendix I. The application of this program is demonstrated in this manual through a series of selected examples. Application of the methane generator program is not included here as a detailed report on the subject is available elsewhere (Ramani et al, 1975).
