Bow-tie analysis is a commonly used tool in the Major Hazard Facility sector to identify, display and control risks that have the potential to pose serious harm to personnel. It is a simple but powerful risk management tool that displays traditional risk assessment outputs as easily interpreted ‘bow-tie diagrams’. These are an effective and versatile communication format that can be applied to any type of risk (eg safety, environmental, assets etc) and as such show great benefit in managing operational hazards in mining.

The need for formalised geotechnical reporting has not been fully explored at many operations. With the continuing need to provide safer and more efficient workplaces, the requirement for good geotechnical reporting systems (as with other reporting systems) has become of greater importance.Historically, the risk from rockfalls has been the largest cause of fatal injuries in the underground mining industry. The response of the ground to mining is generally poorly documented at many mines. At Olympic Dam, it was found that the existing corporate hazard and incident reporting (HIR) procedure did not record enough specifics about geotechnical issues. A separate and more specific geotechnical reporting process was set up to address this problem.This paper examines the role that geotechnical reporting plays at the WMC Resources Olympic Dam Mine; from reporting and rectifying day-to-day geotechnical issues that affect the operation to documenting and analysing longer term trends.

There has been a reduction of coal production in the United States due to environmental pressure and the increased use of natural gas. The process of excavating and mining coal causes fractures in the overlying and underlying strata. The fractures provide pathways for gas released from coal seams and other gas-bearing strata to flow into the mine workings. Coal seam gas typically contains high concentrations of methane with lesser concentrations of carbon dioxide. Low concentrations of other gases such as nitrogen, hydrogen sulfide and ethane may also be present in some coal seams. In certain geological settings, particularly in close proximity to geological structures, such as faults and dykes, the concentration of gases contained in a coal seam can vary significantly between methane-rich to carbon dioxide-rich.In mine design and planning it is vital that mining engineers gather sufficient information to accurately determine the content and composition of the gas present in the coal seam and develop an understanding of changes in these parameters within the planned mining area.The longwall method of mining coal causes fracturing of the overlying and underlying strata behind the retreating longwall face. Where coal seams and other gas-bearing strata are present within the caving zone, gas will be released from the strata leading to contamination of the mine workings. If the rate of gas emission exceeds the diluting capacity of the mine ventilation, the concentration of gas in the mine may exceed the statutory limit, resulting in production delays and potentially unsafe conditions.CITATION:Black, D J, 2016. Overcoring is a common technique to measure in situ rock stress using the concept of stress relief. The strains measured during overcoring are used to calculate the stress tensor assuming continuous, homogeneous, isotropic and linear elastic behaviour. This approach does not recognise any effect of the thickness of the rock annulus produced during overcoring instead assuming that the stresses are entirely a function of the difference in strain between the start of the overcoring and the end with no regard to the transient strains produced. Three-dimensional numerical modelling was used to simulate the overcoring process and investigate the impact of core diameter on the transient stresses and strains during overcoring. This study showed that in thicker core, maximum transient axial stresses and strains occur when the drill bit approaches the measuring point and greater tensile axial stresses were produced when the annulus was thinner. There is an increasing trend in the extraction and development of copper resources from complex deposits. The complex deposits pose mineral processing challenges, as they often contain low grade copper ore, relatively large quantity of clay minerals, complex texture, very fine disseminated ore, high amount of pyrite, and high level of impurity elements such as arsenic. With the amount of arsenic in concentrates increasing and with environment, occupational health and safety regulations becoming more restrict, there is a need for a processing method that results in no arsenic emissions and captures all arsenic while immobilizing it in a compound that is stable under a wide range of conditions, so it is suitable for long-term disposal. Some development work has been undertaken to examine the most effective arsenic treatment option for complex orebodies that contain arsenic, but a technically robust and economically viable option has not yet been developed. Previous research has demonstrated the possibility of separating arsenic-copper sulphides from other copper minerals by controlling the potential of the flotation pulp in the single mineral systems. The selective flotation of enargite from copper sulphides in a real complex ore system was studied by Tayebi-Khorami (2016), which is summarised in this paper. CITATION: Tayebi-Khorami, M, 2018. The Macraes gold mine in Otago, New Zealand, is developed along the central portion of the Hyde-Macraes Shear Zone (HMSZ), which dips gently to the NE, and is traceable over 30 km of strike, before disappearing under tertiary cover. Mineralisation is most intensely developed along the hanging wall shear, which defines the upper limit of the shear zone, although significant packages of quartz vein arrays and mineralised shears occur sporadically beneath the hanging wall shear.The Macraes gold mine is now in its 25th year of operation, well beyond the projected mine life at the time of commissioning in 1990, and currently contains over triple the measured and indicated resource inventory that was defined at start up. The extended mine life and resource growth has been driven by exploration and resource development success, efficient low-cost mining and processing, improving gold price, as well as an ethos of continuous improvement.The emphasis of Oceanagold’s exploration and resource development has shifted over time; from hanging-wall-focused mapping, soil sampling and step-out drilling, through exploitation of previously drilled stockwork mineralisation, to the pursuit of blind, higher-grade underground targets.Mineralisation within the HMSZ remains open at depth, and the regional potential has not been fully tested. A critical step in open cut coal mining is clean removal of the overburden. This is usually achieved by drilling, blasting and digging. A key part of this process being successful is to stop drilling at the correct blast ‘stand-off distance’. The stand-off distance is the distance between the base of the drilled blasthole and the top of the coal horizon to be mined. Having this correct provides a protective cap of rock above the coal horizon so that the blast will fracture all the overburden without damaging the coal horizon or mixing it with the overburden. Drilling beyond the correct stand-off distance causes the blast to continue into the coal horizon, damaging it and mixing it with the overburden, leading to significant losses of coal product. Conversely, drilling short of the correct stand-off distance results in additional work to remove the intact overburden.

This presentation is a 20 minute snippet of my professional career, and my progression in the mining industry, from a lazy uni student to an up-and-coming blast engineer in the middle of a mining boom.My name is Daniel Hepburn and I am a blasting engineer for Orica Mining Services. I graduated from Royal Melbourne Institute of Technology (RMIT), in Melbourne in 2003. I completed both of my final year projects with Orica Quarry Services and was offered an assistant shotfiring job, which I accepted and worked for eight months. Having missed the cut-off date for the Orica graduate program, I planned to work with Quarry Services until the following year where I hoped my marks and the experiences I had gained would secure a graduate position. I didnÆt have to wait long, as in August of that year, I was offered (and I accepted) a graduate position in Perth as a technical services engineer for Orica Mining Services. The Orica graduate program is based over two years, and consists of real roles from day one; however, there is also opportunity to fast track the program for high performers. There is a structured rotation, from site to site, interstate or even international if you are so inclined. This includes the backpacker program, which allows graduates to experience the Orica culture in countries of their choice. There are also opportunities to try cross-functional roles in the different Market segments within Orica. I was lucky in that I knew what I wanted to do and where I wanted to work, so I stayed in Perth for my entire graduate program. I was also lucky enough to be fast tracked and finished my graduate program in 15 months. At the end of 2005 I accepted a position as an open cut blasting engineer based in Perth and even in such a short period of time I have already completed a number of major accomplishments. This paper discusses the implementation and potential limitations of the method for estimating sampling and analytical errors of multiple source mine output blended into a shipping product, described by Everett and Ziegelaar (2010). The practice of quality measurement varies between iron ore miners. The common objective is to monitor effective resource utilisation and control of product quality to customers. A flow diagram is given presenting the various options used by iron ore mine operators and the merits of the different options are discussed.

A full-length paper was not prepared for this presentation.The Halasu porphyry copper belt, located in the East Junggar basin bounded by Chinese Altay to the north and the Tianshan mountains to the south in the Central Asian Orogenic Belt, is one of the major porphyry copper belts in Xinjiang province, north-west China. Combined with previous studies in this area, the magmatism can be divided into three periods: pre-mineralisation activity of 390 Ma syn-mineralisation activity of 380–370 Ma post-mineralisation activity of 360–330 Ma.Through the comparison of zircon chemistry from ore-bearing and barren syn-mineralisation intrusions, negative correlation was identified between Ti-in-zircon temperatures and oxygen fugacity, corresponding to the fact that late-stage syn-mineralisation granitoids contain high water content, high oxygen fugacity and low temperatures with higher mineralisation potential. In addition, the relative low initial 87Sr/86Sr ratios, high eHf(t) and eNd(t) values and mantle Pb isotope characteristics in this study support that the magma forming these intrusions was probably derived from juvenile crust generated during subduction-accretionary processes.CITATION:Wu, C and Chen, H, 2015. THE Company's smelting equipment consists of three blast furnaces, supplied with blast from two sets of vertical piston blowers, each capable of delivering 8000 cub. ft. of air per minute at a pressure of 60 oz. per sq. in. Each blower is belt-driven by a 1:30 h.p. motor. The maximum speed of the blowing engine is 35 r.p.m.No. 1, the largest of the three blast furnaces, has a crucible area of 49.5 sq. ft., and measures 11 ft. 6 in. x 5 ft. at the tuyeres.From tapping floor to feed floor measures 35 ft. 9 in., and holds, when full, 100 tons of charge. Blast enters through thirteen 3-m. tuyeres at 60-oz. pressure. Working normally, this furnace is capable of treating :300 tons of charge, exclusive of returned slag and coke, per 24: hours, with an output of 100 tons of lead. Tapped slag flows into a C.l. forehearth mounted on wheels, where any lead and matte is caught, and the slag overflows into a 3-ton cast-iron pot, in which it is drawn away by an electric locomotive and poured over the dump.

Post a serious heating event underground, and at the direction of the State Mining Safety Authority, a mining company needed to demonstrate the use of cutting-edge technology to mitigate future events. Yokogawa were contracted to provide a turnkey solution to help the mine achieve this objective. The solution was to use a fibre optic-based Heat Detector that will allow entry into and around the mine considering the potential volatility of the environment, also the need to redeploy as mining activities moved underground and more boreholes are added. Industrial strength fibre optic cable and a robust connector system was chosen to achieve this goal and give the customer the ability to send cable down boreholes of up to +1 km and continuously monitor the temperature ofunderground roadways. Being able to re-deploy the cables into different boreholes and link borehole temperature measurements gave insight into all areas on the mine. The machines deployed on-site were the Yokogawa DTSX1 heat detector incorporating the Ramen Backscatter principle. Being of a robust construction and a low power consumption device, it was easily included into the portable monitoring trailers designed by the customer and built locally.

Mechanical sampling provides a definitive value for the product sampled but the results are not instantaneous, meaning that decisions on process control can only be made hours after the sample is taken. Chemicals are added, tailings are sent to waste, stockyards are laid and the data for the decision-making is historical. Online analysers provide instantaneous information, allowing operators to make immediate and cost-saving decisions on the process. However, pending the specific online analysis application, there is a need for reference sampling for dynamic calibration/bias updating of the analysers so that they can deliver the desired precision and accuracy. What would be the economic justification for combining mechanical sampling with online analysis to reduce laboratory costs and provide immediate process control with certainty of accuracy? A better understanding of the detonation performance of an explosive charge can be gained by directly measuring pressure, temperature and velocity of detonation (VOD). This is particularly important with explosives used in the mining industry because their performance is directly influenced by the degree of confinement given by the borehole diameter and the surrounding rock mass. A project funded by the Australian Coal Association Research Program (ACARP) was initiated in early 2009 with the view to design and build cost-effective prototype instrumentation to measure the relative differences in detonation pressures and temperatures of commercial mining explosives. The project’s primary focus was on low-density explosives. Low-density (also referred to as low shock) explosives have been available in various forms for nearly 20 years. There have been significant advances in the availability, reliability and flexibility of these explosives, which now offer a range of densities and degrees of water resistance. Their detonation performance is not well understood and cannot be accurately predicted with current ideal and non-ideal detonation codes. It was therefore viewed as important to be able to directly measure pressure and temperature during the detonation process of these complex mixtures in production blastholes. Identifying the pressure release patterns of both conventional and low-density explosives under different geotechnical conditions should provide the necessary information to both validate detonation codes and better define input parameters for breakage and fragmentation models. This paper gives a general description of the prototype instrumentation developed and reports on the results obtained to date in full-scale conditions.

The relative infrequency of fatalities and other serious events can give them an appearance of being random, of being beyond any reasonable degree of anticipation and prevention. In fact, the vast majority of these events result from exposures that are identifiable, measurable, and manageable. The lessons of prominent incidents, such as the Space Shuttle Columbia, OxyÆs Piper Alpha, Esso Longford, BP Texas City, Massey Mine, as well as lessons from single fatality events, is that alongside the proximate causes of each incident, there also coexisted an underlying fabric of systems, mechanisms, and culture that allowed risk in the workplace to persist and often to become normalised. All of these ingredients of the organisationÆs safety fabric lend themselves to the influence and intervention of senior leaders. The difficulty is that traditional injury prevention paradigms are not sufficient for addressing the precursors that lead up to catastrophic and fatal events. A recent study of such events, led by Dr Thomas Krause concluded that the descriptive aspect of HeinrichÆs safety triangle is useful, but that the predictive aspect is not. The study surfaced reasons why a reduction in less serious injuries does not necessarily correspond to a proportionate reduction in catastrophic and fatal risks. This presentation expands on the findings to include the concept of exposure to the event potential linked to precursor events and the framework for leading an organisation to be both cognisant and effective in eliminating such events.CITATION:Russell, R, 2012. Anglo Coal Australia (ACA) takes a proactive position with regards to attracting and retaining employees at a graduate level. Currently, ACA has graduates in its program across all disciplines: Mining Engineering, Mechanical Engineering, Electrical Engineering, Commercial, Occupational Health and Safety, Environmental Engineering, Human Resources, Safety Engineering and Processing Engineering. The current program is three years in duration and involves two (generally) equal periods at different ACA mine sites or Brisbane Corporate Office. ACA takes an active role in promoting its status at the university level by maintaining a scholarship program. Six undergraduate scholarships are offered each year across all disciplines. Associated Vacation Employment is offered to each scholarship holder during the period of his or her university study. This commitment to education is regarded highly by future graduates. ACAÆs active role in sponsoring student bodies is also known and appreciated amongst the university population. ACAÆs graduate program has been a successful training program for future leaders of the coal mining industry. The company is committed to making the program more suited to the needs of the graduates across all disciplines and as a result the program is not stagnant but is constantly changing and evolving. Recently the æGraduate PassportÆ has been drafted with an aim to keep each graduateÆs needs in relation to experience and training firmly in focus for the duration of the three-year program. Appropriate training for each graduateÆs position is supported by the company; for instance, support through funding and time for a commercial graduateÆs CPA qualifications is provided.

Mt Keith concentrator treats low grade disseminated nickel sulfide ore supplied from the adjoining open pit mine. It is a traditional crushing, two-staged closed circuit grinding circuit (SAG & Ball Mills), followed by a complex split-size rougher, scavenger and cleaner flotation circuit. In 2016, 10.5 Mt was milled, at an average head grade of 0.54% Ni, yielding 35 000 nickel metal tonnes in concentrate.Historical analysis of the plant data indicated that of all the nickel lost in the fines rougher circuit tails stream, 25.6% was recoverable in an eight minute laboratory scale flotation test. These results implied that this fine nickel was readily available for recovery in the plant, either through operational improvements or additional flotation capacity. With operation optimisation ongoing, and capital within the business focused elsewhere, plant personnel were challenged to think differently about improving recovery.In 2012, a separate project to improve plant recovery when treating high talc ore was completed. Amongst other vessels, this project saw the installation of two 150 m3 Outotec Cells to the scavenger section of the fines rougher, middlings and scavengers (RMS 1) circuit. Since installation, the operation of these cells resulted in difficulties immediately upstream with instability in the scavenger cells. It was therefore decided that these cells would be better suited to treating the fines rougher circuit tails.The alterations required to divert these flows were minimal, with the capital project totalling $220 000.In the two years since installation, the fines rougher tailings nickel grade has dropped by 9.9%. The scavenger circuit has also seen a 2.8% reduction in tailings nickel grade, and instability issues were immediately resolved. This project serves as a lesson in how existing plant infrastructure may be better utilised to maximise recovery. Although this is especially applicable when capital constrained, such re-appropriation can be considered at any point to maximise return on existing assets. CITATION:Hussey, L, 2017. The link between geostatistics and Theory of Sampling (TOS) is examined again in view of new findings made while trying to compare predicted sampling variances and estimated grades (using geostatistics) on the same material, and this provides an important consistency requirement whenever calculations are made that involve both theories in a comparative way.
