An understanding of how health and safety management systems (HSMS) reduce worksite injuries, illnesses and fatalities may be gained in studying the behaviors of health and safety leaders. These leaders bear the accountability for identifying, understanding and managing the risks of a mining operation. More importantly, they have to transfer this knowledge of perception, recognition and response to risks in the mining environment to their workers. The leaders’ efforts to build and maintain a mining operation’s workforce that consistently executes safe work practices may be captured through more than just lagging indicators of health and safety performance. This exploratory study interviewed six leaders in occupations such as site-level safety supervisors, mine superintendents and/or general managers at surface and underground stone, sand and gravel and metal/nonmetal mine sites throughout the United States, with employee populations ranging from 40 to 175. In exploring leaders’ perspectives on how they systematically manage health and safety, examples such as approaches to task training, handling near-miss incidents, identifying future leaders and providing workers with feedback offer insights into how leaders translate their knowledge and management of site-level risks to others.

In the constant search for ways and means of improving the quality of coke and thus making its use at the blast-fornace more satisfactory and economical, it has become increasingly evident that the present laboratory tests do not show the full picture of a coke's value as a blast-furnace fuel. This fact was recognized by Dr. A. C. Pieldner thirty-five years ago: (Ref. 1) here is perhaps no phase of fuel technicology which has aroused more discussion or regarding which there is more difference of opinion than the properties of coke which make it most suitable for blast-furnaces and other uses." Dr. Fieldner followed this assertion with a statement to the effect that men who write on this subject today (1923) have, because of conflicting experimental evidence brought forth in recent years, far less positive opinions on the matter than did writers of some years past. One is led to wonder if these statements are not just as true now as they were when first made.

The 1889 Johnstown Flood, which killed 2,209 people, was a preventable disaster stemming from systemic decisionmaking failures. This paper applies modern Risk-Informed Decision Making (RIDM) frameworks to analyze the historical context and explore alternative outcomes through hypothetical multiverse scenarios. By reconstructing the risk landscape of the South Fork Dam, we demonstrate how structured hazard identification, risk assessment, and stakeholder communication could have mitigated or prevented the catastrophe. Three counterfactual scenarios—proactive maintenance, community engagement, and regulatory oversight—are evaluated to quantify potential lives saved and economic benefits. The study acknowledges historical barriers, including technological limitations, lack of governance, and socioeconomic disparities, which constrained risk-aware decisions in the 19th century. The analysis draws critical parallels to contemporary engineering challenges, particularly in tailings storage facility (TSF) management, where similar risk blind spots persist. The paper concludes that RIDM is not merely a technical tool but a governance imperative, essential for ethical and sustainable engineering practice. By learning from historical near-misses and failures, the modern engineering community can strengthen resilience and accountability. This work contributes to the growing body of retrospective risk analysis, offering a methodological framework for applying RIDM to historical disasters to inform future safety standards.

With the evolving landscape of gold mineral resource development and utilization, the easily treatable gold resources are continuously decreasing, while refractory gold minerals are becoming the most important resources in the field of gold extraction. In these original ores, most of the gold is encapsulated in the lattice of pyrite and arsenopyrite, resulting in difficulty of gold transfer into the solution during the leaching process. The most effective method to solve the problem of insufficient contact between gold particles and leaching reagent is oxidation pretreatment before leaching. Different from traditional high-temperature roasting, concentrated sulfuric acid was used as both an oxidant and leaching agent in a sulfideencapsulated refractory gold ore to decompose the pyrite in the curing environment to expose the gold and improve the leaching efficiency. The experimental results showed that under the conditions of liquid solid ratio 3:1, curing temperature 240 °C and the reaction time 1.5 h, the weight loss rate and iron removal rate of the ore sample reached the highest value of 60.01% and 99.68%, respectively. The main phase of curing residue was Fe2( SO4)3, and silica was nearly the only phase after washing. At the same time, by combining the sodium thiosulfate system gold leaching method, the gold leaching rate increased from 3.67% to 81.30% before and after pretreatment with concentrated sulfuric acid.

Upgrading of low-concentration ores is of growing importance today. The Heavy Medium Cyclone Process, thanks to strong centrifugal forces, effectively separates particles varying only slightly in specific gravity but widely in size. The paper discusses the separation efficiency, the method of assessing that efficiency, and the influence of such factors as medium solids. The operating results of nine heavy medium cyclone plants handling following minerals are reported: barite, wolfram, iron, magnesite, diamond, lead-zinc, fluorspar, rocksalt potash. Highlighted are an improved recovery system for heavy medium and some other developments, such as techniques for separation down to 0.1mm and the application in the field of recycling.

"Point cloud technology is now an indispensable tool in geological and geotechnical data collection, interpretation and analysis. Open-pit and underground mines of all sizes are regularly collecting point cloud data as laser scanning (LiDAR) and photogrammetry surveying devices have become affordable and the workflows fast and efficient. Past challenges of large data processing and manipulation are gone with recent technological advancements in computer hardware and software. These innovations reduce file sizes, ease rendering requirements and allow for the implementation of real-time 3D viewers and surface meshing tools. With its ability to remotely and safely, rapidly and accurately extract large quantities of georeferenced and 3D-oriented data, point cloud technology provides numerous applications to the geomechanical field. The list of uses is continuously growing. This article specifically focuses on digital outcrop modeling and digital terrain engineering. Point clouds. The high-resolution and increasing availability of point cloud technology is rapidly improving open-pit and underground mines’ investigation, interpretation and characterization of site geology and rock mass properties (Kemeny et al., 2015; Lyons-Baral, 2012). LiDAR and photogrammetry are common remote sensing methods employed to measure the coordinates of surfaces in three dimensions (3D), providing the X, Y and Z locations and typically the reflective intensity and/or the visible three-band color of each point (Fig. 1).LiDAR uses either a time-of-flight or phase-shift measurement of a laser beam reflected off targeted surfaces and received back into the device.Photogrammetry generates point clouds by merging multiple overlapping photographs and uses stereographic plotting to define the locations of object points in 3D space. These methods can be deployed from airplanes, unpiloted aerial systems (UAS or drones), tripods, land vehicles and even in a mobile backpack shown in Fig. 2. RADAR, Total Station-Prism and other remote sensing systems also offer very precise “point clouds,” but this article does not address these due to their higher cost and lower versatility and mobility."

EPA's Mining Waste Section within the Office of Solid Waste is continuing to collect and analyze information on mining waste management practices. Starting in 1991, the Mining Waste Section has continued to conduct site visits and prepare case study reports on the waste generation and management practices of operating mines. In choosing sites to study, a reasonable attempt was made to provide diversity in mining commodity types, geographic regions, geology, climate, and environment. To date, EPA has visited the following mines: Asarco Troy (copper) in Montana, Newmont Rain in Nevada (gold), Cyprus Thompson Creek (molybdenum) in Idaho, LTV Steel (iron) in Minnesota, Phelps Dodge Morenci (copper) in Arizona, Doe Run Fletcher (lead) in Missouri, Brewer (gold) in South Carolina, IMC Four Corners (phosphate) .in Florida, Pennzoil Culberson (sulfur) in Texas, Nerco Cripple Creek (gold) in Colorado, Copper Range White Pine (copper) in Michigan, Magma San Manuel (copper) in Arizona, Colosseum (gold) in California, and several gold placer mines in Alaska, including the Valdez Creek Mine. EPA has prepared a case study report on each facility. The mine operators were provided an opportunity to comment on their respective site visit report and their comments have been included in' each respective report. These site visit reports have in turn been incorporated into technical resource documents which provide a detailed description of each mining sector. EPA is currently reviewing the technical resource documents and plans to release the documents in the Spring of 1994.

In the hard-rock mining industry, the generation of fines during blasting is an important issue because it has a significant effect on production processes. Although various approaches have been used to estimate fines generation in recent decades, as a practical matter, measuring the amount of fines is nearly impossible given the technical limitations and the scale of mine blasts. In this study, a dynamic PPV damagemodel was developed and applied to predict fines generation as a result of blasting. This modified version of the crushed zone model (CZM) considers dynamic rock properties and peak particle velocity (PPV), which represents the induced stress in blasting. Dynamic rock-fracturing tests were conducted using a split Hopkinson pressure bar (SHPB) to determine the parameters that represent the relationships between rock strength and strain rate as well as the relationships between fines amount and strain rate in the model. A case study was conducted that entailed determining the radius of the crushed zone for given blast designs and rock properties and then measuring the fines amount within this crushed zone. This case study indicated that the dynamic PPV model can provide a rough estimate of fines generation for given dynamic rock properties and blast design parameters and that this method has the potential to be a useful indicator, or index, of fines generation.

This paper will address the feasibility of using low-density cellular concrete (LDCC) for an annular fill application where project conditions required pumping the LDCC material for distances of up to 16,000 feet. These conditions occurred during construction of the Kaneohe-Kailua Sewer Tunnel in Oahu, Hawaii, between 2014 and 2018. Located on the island of Oahu, Hawaii, this 13-foot-diameter rock tunnel extends for approximately three miles from the Kaneohe Pre-Treatment Facility to the Kailua Regional Wastewater Treatment Plant. The tunnel was built to convey wastewater and provide temporary storage for peak wet weather flows entering the collection system, thereby reducing the potential for uncontrolled discharges. LDCC was chosen for this project partly due to its environmental benefits, which were essential for an application contiguous to the inland marine and coastal waters of the Pacific Ocean. In order to ensure optimal performance characteristics—compressive strength (with a requirement of 300 psi at 56 days), flowability, and light weight—were achieved, extensive testing of various LDCC products and mix designs was completed. The testing measured the initial set time of the LDCC as well as the compressive strength needed to reduce the quantity of grout lifts required for the project.

Rosebud Mining Co. (RMC) is based in Kittanning, PA. As of October 2013, it operated 24 active underground mines in Pennsylvania and Ohio. RMC?s Tracy Lynne Mine is located approximately 5 km (3 miles) east of Apollo, PA, (approximately 48 km or 30 miles northeast of Pittsburgh, PA) and mines the Lower Kittanning coal seam. The coal seam averages approximately 1.1 m (45 in.) thick and is of steam coal market quality. The mine is a single unit operation. Coal is mined using a continuous mining machine and is transported from the face to the conveyor belt using a continuous haulage system. Typical ventilation in the mine consists of an exhausting fan that draws air through isolated intake, belt and return air courses (Fig. 1).

Frothers play a crucial role in sulphide and PGM ore flotation recovery, and AECI Mining Chemicals studies on PGM ores have shown significant flotation kinetic increases with the optimal frother-collector combinations. Using Senfroth 200 as the industry baseline, remarkable kinetic increases were observed with AECI Mining Chemicals developed frothers in the first concentrate. Even in the worst-case scenarios, 4E PGM recovery increased by 5% at a slightly higher concentrate grade. It is important not to limit frother dosage to a fixed value. Instead, enough frother should be added to maintain a stable bench float, measuring the dosages by frother volume before and after tests. Frothers can be specialized blends and their importance should not be underestimated. The results produced by formulating and innovating frothers to address specific industry challenges have been encouraging. Testing a frother on a plant scale does not have to be as daunting as with collectors, enabling the identification of the best frother-collector combinations at an industrial scale. AECI Mining Chemicals bench scale testwork has motivated plant-scale trials that have successfully transitioned to commercialization. This paper compares the results from metallurgical and froth stability tests and shows that occasionally frothers that have a less stable froth can have improved valuable mineral recoveries, while still exhibiting better drainage for reduced entrainment and mass pull. It was shown that caution should be used when relying only on simplified froth stability tests in the evaluation of new frothers.

The Magnetometsic Resistivity (MMR) method is based on the measurement of the low-level, low-frequency magnetic fields associated with non-inductive current flow in the earth. The traditional resistivity method maps the electrical properties of the earth by measuring the electric field caused by galvanic current flow between a pair of current electrodes. The MMR method differs from it in that the potential electrodes are replaced by a sensitive coil or magnetometer and a component of the magnetic field due to the current flow is recorded. The presence of an inhomogeneity redistributes the current flow which results in a perturbation of the electric field and, usually, of the magnetic field. The electric field immediately above a relatively conductive inhomogeneity must, by Ohm's Law, be smaller than that expected for a uniform earth. The current density in this region is also reduced but the current density within the inhomogeneity itself is enhanced so that the horizontal component of the magnetic field measured above the inhomogeneity is usually larger than that expected for a uniform earth. A method of interpretation using the magnetic anomaly was patented by Jakoskyl in 1933 but it met with little success at that time. Part of the problem was undoubtedly with the instrumentation, and only recently have robust instruments of sufficient sensitivity been developed2 and field data of good quality obtained. There are many common configurations of the potential and current electrodes in the resistivity method. Similarly, in the MMR method one or both current electrodes may be moved with the magnetometer, either horizontally or vertically ( in a bore-hole), or the current electrodes may be maintained in fixed positions and the magnetometer alone moved. Numerical simulations have been used to investigate roof fall in the past decades. Comparing to the continuum based methods, the discrete element method has been well recognized for its excellent performance in simulating large strain problems and fracturing processes in the rock mass. However, the discrete element method has its limitations. When simulating a large-scale problem, a large number of particles are required, which consumes significant computational resources. In the present study, a 3D numerical model coupled with the finite-difference method (FDM) and the discreteelement method (DEM) was created to simulate the laminated roof failure in an eastern coal mine in the US. In the FDM-DEM coupled model, the laminated roof of the entry is represented by an assembly of bonded particles using PFC3D. The laminated roof is simulated by adding parallel discontinuities. The discontinuities are given certain stiffness and strength in the model. The ribs, floor, and far-field surrounding rock are represented by continuum zones using FLAC3D. The strength of the discontinuities is varied sequentially and the fracturing of the laminated roof was analyzed with respect to entry advance. The results showed that the laminated roof fractures along the discontinuities while an intact roof fractures in a dome shape. Increasing the strength of discontinuity could enlarge the tensile zone inside the laminated roof and hence change the crack distribution. In addition, the crack quantity in the laminated roof behind the advancing face decreases with the increment of discontinuity strength while this trend is not occurring ahead of the advancing face. The simulation demonstrated that coupled method using PFC3D/FLAC3D can reproduce the laminated roof failure in underground coal mines.

The technology employed in the processing of nickel- and cobalt¬bearing ores is a continually developing process for utilizing the natu¬ral resources available at a given time. Most of the nickel-bearing ores of commercial importance fall under the category of sulfide or lateritic ores. While the occurrences of both types of ore have been known for over a century, until 1960 sulfide ores largely from Cana¬dian mines have constituted the chief source of production for com¬mercial nickel in the Free World. Sulfide ores are readily amenable to some physical means of concentration, but the lateritic ores are not readily concentrated, and to compete economically with the sulfide ores only the high-grade garnierite ores (a type of lateritic ore) found in New Caledonia were processed to any great extent up to the time of World War II. Around 1960, a gradual shift in emphasis towards latetitic ores as a commercial source of nickel became evident. See Fig. 1. New discoveries of sulfide ores, usually located underground, had not kept pace with the steadily increasing demand for nickel. The lateritic ores, found near the surface of the earth, could be readily surveyed and mined. Lower cost processes were being developed for the extrac¬tion of nickel from lateritic ores and for a time it appeared that, based on pre-OPEC economics, the lower grade lateritic ores-down to 1.2% nickel-could satisfy an increasing share of the world demand for nickel. The sharp increase in the price of crude oil in the 1970s, however, slapped an economic damper on the lateritic ores. Processes for treat¬ing lateritic ores consume large amounts of energy; as the nickel industry entered its deep recessionary period in 1981-83, nickel pro¬duction from lateritic ores became less and less profitable than from sulfide ores. At the same time, new sulfide ores were discovered and brought into production, notably in Australia and South Africa. The chapters in Section 17 were prepared in 1973. Recent improvements in mineral dressing techniques since then include Inco's pyrrhotite separation process which can remove additional pyrrhotite from nickel concentrates, resulting in reduced sulfur dioxide emission from the Sudbury smelter, by some 25%. The chapters in this section dealing with nickel are grouped in line with the two broad categories of nickel ores. Starting with the description of nickel and cobalt ores in Chapter 2, Chapter 3 deals with the processing of sulfide ores, and Chapter 4 deals largely with lateritic ores. The intermediate steps to produce a suitable feed for extraction plants and refineries are similar in principle within the same categories, although dissimilar between sulfide and lateritic ores. Thus Inco in Canada employs flotation and so does Western Mining Corp. in Australia, although Inco's plants were started before World War II, and that of Western Mining only recently in the late 1960s. Treatment subsequent to the ore dressing step, however, shows some interesting differences. "In recent years the gold mining industry has been challenged by declining metal prices as well as having to treat ores that have low grades and are refractory in nature. Many factors must be considered when metallurgical studies are designed to develop process design criteria for these type of ores. These factors are mostly related to the mineralogical and chemical compositions of the ores, the geological setting of the deposit and how the ore/waste will be mined, such as whether openpit or underground, the mine schedule, and the mine fleet type and capacity. The purpose of this paper is to provide an experience-based practical approach to how to select and characterize test samples as well as devise process development metallurgical studies for both oxide and refractory gold ores, and to provide insight into some of the matters that might go wrong during the course of the studies. This paper is intended as both a guide for someone new to the field and also as a reminder for those familiar with the field. IntroductionThe recent decline in ore grades and metal prices have challenged mining companies to reduce capital and operating costs associated with developing new mineral resources. In some cases, metallurgical studies have been sacrificed to prevent cost overruns resulting in inadequate mineralogical, chemical and metallurgical characterization. Consequences have included longer construction schedules, higher initial and working capital, higher operating costs and less than desirable plant performance (Whincup, 2010; Torres, Chaves and Meech, 1999).Mineralogy, geology, mining type and sequence, and the metallurgy of an orebody will determine the process development metallurgical study requirements and eventual process plant design. Therefore, once the geologist has an early indication of a prospective orebody, geologists, metallurgists, mining engineers, and social and environmental responsibility scientists should convene and devise a plan on how to exploit the orebody economically (Lunt, Ritchie and Fleay, 1997).Geologists may find a prospective orebody solely based on grade. However, if the orebody cannot be mined and processed economically, then it is deemed not to be a deposit. An example of this could be a refractory gold deposit, either sulfidic, carbonaceous or siliceous refractory or any combination of those, that may not be mined and processed economically due to reasons such as the physical location of the deposit, depth of the ore lode, or social and environmental concerns. Therefore, an early collaboration between all disciplines must be established.Each orebody may have its own unique mineralogical, geological and chemical characteristics (Guresin et al., 2012). These unique characteristics should be effectively sampled to reduce risks associated with process design and engineering efforts for the process development metallurgical studies. If the sample or samples are not representative of these unique characteristics, then the studies performed will be pointless. The project metallurgist in charge of conducting metallurgical studies should also possess general geology, mineralogy and mine engineering knowledge. Recommended readings are a book on understanding mineral deposits by Misra (2000) and articles on heap leach development by Scheffel (2014a and 2014b)."

Physical separation of bitumen from low-grade Utah tar sand deposits containing a relatively high viscosity bitumen phase (Sunnyside and Tar Sand Triangle deposits) has been accomplished by traditional size reduction and froth flotation techniques. At appropriate experimental conditions more than 90 percent of the bitumen can be recovered in a concentrate, containing more than 20 weight percent bitumen, which should be a suitable feed material for subsequent hot water or thermal processing. The efficiency of bitumen recovery depends on the extent of size reduction, promoter and dispersant addition. Rejection of greater than 60 percent of the sand at ambient temperature and ease of water removal from the concentrate make such a process strategy both energy and cost effective. The energy required to achieve effective separation by the ambient temperature process is significantly less than the energy required for the recently developed hot water process. The flotation behavior of the tar sand has been analysed with respect to flotation pH, contact angle measurements and the apparent iso- electric point of the bitumen. The best flotation response at pH 7.8 to 9.0 can be correlated with the maximum in contact angle developed between the air bubble and bitumen surface and with the apparent iso-electric point of the bitumen (-pH 8.0).

Precious metals are commonly believed to be recovered along with sulfide minerals in flotation, but a few neutron activation analyses showed that the recoveries of gold and silver were unexpectedly low. Recoveries of platinum group metals remained unresolved due to interferences in the analyses. Electron micro-probe, scanning electron microscope and scanning auger microprobe analyses have identified nickel arsenide minerals in the ore and confirmed a close association of platinum group metals with the arsenide minerals as minute inclusions. These results indicated a need for re-evaluation of flotation procedures which have so far ignored special problems related to arsenide minerals. The electrochemical and flotation behaviors of nickel arsenide and their implications to the flotation recovery of arsenide minerals from copper-nickel bearing Duluth gabbro are presented.

Canada produces close to 42 million tonnes of coal per annum of which some 38 million tonnes is produced by surface mining methods. The two Western Canadian provinces, Alberta and British Columbia, produce 31 million tonnes of coal by surface mining. These two provinces provide the bulk of the metallurgical coal for export. The geological conditions in the Western provinces particularly in the mountain region are complex. The coals are friable and the seams are steeply inclined, sheared, intensely folded and faulted. There may be as many as 15 to 20 coal seams present with true thickness ranging from 2.5 m to over 8 m. This paper reviews the surface mining methods used to extract such seams. The difficulties associated with steep seam and multiple seam mining of friable coal resulting in lower coal recovery, high dilution and the problem of cleaning fine coal in the wash plant have been highlighted.
