Entrapments are a huge risk in underground mines. This happens when miners (or any other persons who have entered underground) survive the initial event of an accident, become trapped or lost in unknown and life-threatening locations after evacuation, and remain unaccounted for. At this point, every second counts, and emergency response is highly desired for any chances of saving survivors. The survival chances of the missing persons depend on how quickly they can be located. Rescue teams are often deployed to search for the missing persons underground. However, the rescue teams are forced to search in random and presumed areas of the mine. This is due to the unavailability of information pertaining to the whereabouts of the missing persons. Not only the provision of accurate locations, but various other types of information such as post-accident two-way communication can enhance the survival chances of the missing persons. This information can be provided by a fit-for-purpose missing person locator system. A wide variety of missing person locator systems are already commercially available, some are in testing phases and others in prototype and development phases. This includes tracking, tracing, location detectors and post-accident two-way communication systems. These systems have unique functionalities, capabilities and limitations; making it difficult for mines to select the most effective, applicable and suitable system specific to their mine. In this study, the user requirements for a fit-for-purpose missing person locator system were developed. The purpose of the user requirements was to provide a guideline to facilitate the system selection process. The user requirements can also be used as a tool to measure and evaluate the performance of the selected missing person locator system. The user requirements were also used to predict the success of the selected system under different missing person incident scenarios. The development of the user requirements showed a potential for many other benefits for missing person locator systems.

In the Rocky Mountain region, variations in Tertiary mineralization correlate with changes in tectonic setting and associated igneous activity that can largely be related to tectonics along the west boundary of the American plate. Successive settings include compressional tectonics and associated continental-arc igneous activity; transitional igneous types associated with intra- and back-arc extension; and basaltic or bimodal basalt-rhyolite assemblages associated with Basin and Range extension in- ward of a growing transform boundary between Pacific and American plates.

The People's Republic of China has abundant coal resources with proven reserves amounting to 582 Gt (641 billion st). Recognizing coal as a major energy source on which to build the country's industrial and economic base, there has been a rapid increase in production since the founding of the People's Republic. In 1949, only 29.4 Mt (32.4 million st) of coal was mined. Production has increased to 576.5 Mt (635.5 million st), exclusive of Taiwan, at present, making China the third largest coal producing country in the world. Plans call for a 4-5% increase in annual production for the next 20 years. Coal constitutes 70% of the primary energy consumption in China. The remaining 30% is divided between petroleum and natural gas with 26% and hydropower with 4%. Coal use and consumption is divided as follows. General industrial use comprised 48.9%, power generation uses 20.1%, and coking accounts for 10.5% (6.3% for metallurgy and 4.2% for chemical industry). Domestic use such as home heating and cooking makes up 17.8% and coal-fired steam engines for railways uses 4.7%. Coal Districts Coal underlies about 1/18 of China's land area. It occurs in 30 provinces, autonomous regions, and municipalities. In addition, the western interior region, which has not been explored, may contain sizable reserves. Based on geology, geography, and administration, the coal fields can be grouped into five districts as shown in Fig. 1. The north China district, the heart of the industrial base for the country, has the most extensive and largest deposits of the five districts. This amounts to almost 40% of the country's total reserves. The largest coal basin with proven reserves of 181.4 Gt (200 billion st) and the most exploitable seams is located some 300 km (186 miles) southwest of Beijing (Peking) in Shanxi Province. Centered around Taiyuan, the compound field extends from Datong in the north to Xian in the south. Its western extension is not known fully. The Northwest and Northeast districts contain the second and third largest deposits. The biggest coal field in South China is located in the Sichuan and Guizhou Provinces near Chongqing. At present, it is not known if sizable coal measures exist in the Tibetan district. Coal Types China's coal varies widely both in age and rank. Geologically, coal measures were deposited from the lower Carboniferous (Pennsylvanian of the US) and Permian of late Paleozoic, mainly during the Jurassic of the Mesozoic and peri¬odically throughout the Cenozoic. Sequential and evolutionary changes through time are recognizable in many, perhaps most major families of ore deposits and their geological environments. These changes are additional useful indicators of the evolutionary history of the earth and its tectonic processes. They are important in mineral exploration because they involve consideration of time as a fourth dimension, thereby indicating the most favorable ages, rather than types of rock for investigation. Many of the changes are analogous to those recognized in faunal evolution. Some primitive, unspecialized types of ore deposits occur nearly unchanged through rocks of widely differing age. Others diversify into several, more specialized variants, whereas still others become extinct. The place of these is taken by new, younger types, some of which undergo sudden proliferations and later diversifications of their own. In Archean greenstone belts irrlportant iron, nickel, gold, and base metal deposits occur co-regionally, but in different stratigraphic positions in the same, thick volcano-sedimentary successions. Here their close temporal and spatial associations suggest common genetic exhalative hydrothermal processes. The massive base metal sulfide family of ore deposits provides an excellent example of evolutionary changes, exhibiting distinct variations through both time and space. Deposits belonging to this family span the entire range of time, occurring in rocks of ancient Archean to Recent age, and specific types within the family also span the entire range of spatial environments from ensimatic to ensialic. Moreover there are possible evolutionary metallogenic links between certain types of massive base metal sulfide deposits and other important base metal ores, noteably Mississippi Valley-type and porphyry copper deposits.

"It was possible to beneficiate a low-grade iron ore fines sample from Bellary-Hospet, Karnataka, India assaying 56% Fe, 9.1% SiO2 and 5.2% Al2O3 and produce two concentrates, namely, a sinter-grade fines concentrate (43.7% by weight), assaying 63.6% Fe, 2.6% SiO2 and 1.7% Al2O3, and a pellet-grade concentrate (30.7% by weight) assaying 64.7% Fe, 2.9% SiO2 and 1.7%Al2O3. Rejects (25.6% by weight) assayed 32.5% Fe, 27.9% SiO2 and 15.4% Al2O3. The mineralogical characterization indicated that the sample consisted of 69.5% ironbearing minerals (56% hematite, 7.5% martite and 6% goethite-limonite), 12.5% quartz and 15.2% clay, and the rest were minor minerals. Liberation size was found to be in the size range of 25 to 100 µm. The process flow sheet consists of jigs, spirals, grinding and low- and high-intensity magnetic separation of the deslimed feed. IntroductionJSW Steel Ltd is a 10-Mt/a integrated steel plant. To produce 10 Mt/a hot metal requires nearly 16 to 20 Mt/a iron ore fines. The fast depletion of highgrade iron ore fines, increased industrial demand and an increase in iron ore prices have necessitated the exploitation and use of low-grade iron ore fines in the steel-making process after beneficiation. Thus, the beneficiation of low-grade iron ores by removing the associated gangue minerals and enhancing their grade is essential. The use of these iron ore fines in sinter and pellet making has prompted the undertaking of systematic studies on low-grade iron ores.Finely disseminated low-grade iron ores with siliceous impurities have been upgraded by gravity, magnetic and flotation-based processes (Burt, 1994; Svoboda, 1987; Houot, 1983). The processing of low-grade Indian hematite ore containing high levels of alumina-bearing minerals as impurities present separation challenges and pose special problems in martite and magnetite, in association with quartz and clay as the gangue-forming minerals. Alumina is mainly contributed by clay (kaolinite, montmorillonite, illite, alunite), lateritic material and gibbsite; some alumina occurs as solids in iron oxide minerals, namely, goethite and limonite. "Testing was conducted to determine the ability of foam to maintain roof coverage in a simulated longwall mining environment. Approximately 27% of the respirable coal mine dust can be attributed to longwall shield movement. Developing controls for this dust source has been difficult. The application of foam is a possible dust control method for this source. Laboratory testing of two foam agents was conducted to determine their ability of foam to adhere to a simulated longwall face roof surface. Two different foam generation methods were used; compressed air (CA) and blower air (BA). Using a new imaging technology, image processing and analysis utilizing ImageJ software produced quantifiable results of foam roof coverage. For CA foam in 3.3 m/s (3.3 m/s (650 fpm)) ventilation at 3 minutes after application, 98% of agent A was intact, while 95% of agent B was intact on the roof. At 30 minutes after application 94% of agent A was intact while only 20% of agent B remained. For BA in 3.3 m/s (3.3 m/s (650 fpm)) ventilation, the results were dependent upon nozzle type. Three different nozzles were tested. At 30 minutes after application 74-92% of foam agent A remained, while 3-50% of foam agent B remained. CA foam seems to remain intact for longer durations and is easier to apply than BA foam. However, more water drained from the foam when using CA foam which demonstrates that BA foam retains more water at the roof surface. Agent A seemed to be the better performer as far as roof application is concerned. This testing demonstrates that roof application of foam is feasible and is able withstand a typical face ventilation velocity, establishing this technique’s potential for longwall shield dust control. INTRODUCTION NIOSH personnel have visited many longwall operations at various geographical locations in the U.S., conducting benchmark surveys to characterize current operating practices and dust control measures in use. Sources of respirable dust generated from the longwall face have been identified as: intake (10%), stage loader (20%), shields (27%), and shearer (43%) [1]. Dust generated from the shearer, armored face conveyor, and the stage loader can mainly be controlled by water sprays and increases in ventilation air quantities, however, research has demonstrated limited effective controls for dusts generated by shield movements [2]. One possible approach to mitigating shield dust generation is to apply a layer of water or foam to the shield-roof interface. Once this interface is blanketed with foam or water, dust will be less likely to become airborne and contaminate the airways over the personnel walkways. Though water is an abundant and common dust control measure, roof applications have limited success because water is unable to remain in-place in adequate quantities for the necessary duration between application and shield advance. Past and current testing of foam for material properties has shown that it has longer applicability times, ranging from 10 to 60 minutes [3] and has shown better performance for dust control, ranging from 19-96% improved efficiency over water [4, 5, 6, 7, 8]. The majority of this foam dust control research has focused on dust control efficiency at the longwall shearer or continuous miner cutting drum."

All underground operations have limiting factors that influence overall profitability. Hoisting, hauling, available headings, labor, milling capacity and other factors limit how much material can be removed from an underground mine. Consequently, when mines use limited labor to handle the additional rock from overbreak and to perform extra ground support tasks, productive activity is replaced by non-productive work. Mining economics are further penalized when overbreak causes dilution of ore grades. This paper reviews organizational systems that mines have used to create environments which foster the creation of successful overbreak control programs. Actual case histories document the various quality improvement techniques, worker incentives, and practical project management systems used by these mines. A brief review of controlled blasting technology sources and overbreak measurement tools is also included.

The following report details the successful construction of Contract 2A of the Three Rivers Water Quality Management Program, Intrenchment Creek Combined Sewer Overflow Conveyance System, in DeKalb County for the City of Atlanta, Georgia. The contract required construction of approximately 2878 m (9443 ft) of tunnel bored to an initial diameter of 7.95 m (26 ft 1 in), with provisions to concrete line those portions designated by the engineer to a finished internal diameter of 6.91 m (22 ft 8 in). Flanking the tunnel are two shafts. The upstream shaft was excavated to a nominal diameter of 4.87 m (16 ft) to accommodate a finished internal diameter of 3.66 m (12 ft). This shaft required a transition boot fully concrete lined to final dimensions of 6.91 m (22 ft 8 in) wide, 13.72 m (45 ft) long, and varying in height from 6.91 m (22 ft 8 in) to 12.80 m (42 ft). Associated work necessary to complete the contractual obligations included installation of 22.86 m (75 ft) of cut-and-cover box culvert, 484 m (1589 ft) of open-cut sewer installation in sizes ranging from 0.30 m (12 in) to 1.68 m (5 ft 6 in), one river crossing, 137.46 m (451 ft) of monolithic concrete trapezoidal channel, and installation of 19125 cu m (25000 cu yd) of select fill material compacted and graded to finished contours. The downstream shaft was excavated to a minimum diameter of 12.04 m (39 ft 6 in) and concrete lined to a finished internal diameter of 10.67 m (35 ft). Requirements here included installation of a complete pumping station with all mechanical, electrical, and structural elements necessary for dewatering the tunnel through the controlled use of four 250 hp submersible pumps discharging into the facilities of the Intrenchment Creek Water Pollution Plant. Free cyanide has been successfully determined in ground and surface water by isolation using a microdiffusion technique followed by a pyridine-barbituric acid photometric finish. The determinable free cyanide includes hydrocyanic acid, ionic cyanide, and the cyanide of weak complexes such as cadmium, lead, manganese, and zinc cyanide. Complexes of copper, iron, nickel, and silver are sufficiently strong so the dissociation into free cyanide does not occur at the microdiffusion pH 7. Cyanate, sulfide, and thiocyanate do not interfere. Samples containing as little as 0.005 mg/L are successfully analyzed. The relative standard deviation is <10%. Samples stored at pH >I2 in the dark are stable at least three weeks.

"Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are at a “plateau” and yet, serious accidents and fatality rates are not. In more dramatic cases, such as in the BP Texas Refinery disaster, organizations that have “exemplary” safety statistics, suddenly have a catastrophic or multi-fatality event occurring. Classic examples are the Piper Alpha disaster and NASA’s Challenger and Columbia disasters, the BP’s Deepwater Horizon Rig disaster in the Gulf of Mexico. Looking into the causes of these accidents provides an insight into the events and deficiencies that led up to the accident, but what are the common features in the organization’s mindset, or its culture? What characterizes these organizations’ decision-making, their approach to safety and to risk and are there features that can be delineated? The research and review presented in this paper covers a period since1995, which started in the Australian resource industry and since then covered international events. The features of these organizations are summarized as the seven deadly delusions of near zero organizations (NZO) that suffered unexpected disasters, based on extensive research by the author. More recently, the massive West Fertilizer plant explosion in Texas, and the Train Disaster in Lac-Mégantic, Quebec, Canada underlined the same trends. Organizations may experience calamities not because they are bad, or unsafe, but because they have become “cult-like”. INTRODUCTION The train disaster of 6 July 2013, in the small town of Lac-Mégantic in Quebec, Canada, in which 47 people were killed, had many strange pre-cursorsso-called “black swan” events that have occurred in many different industries. Black swan events are so rare, that there is almost zero chance that they will ever occur again, but they are so calamitous that the responses to these are emotional and irrational, (but necessary) and in those responses, the seeds for other, different black swan events are put into place. It is not what we don’t do, or do wrong, that causes modern disasters, it is what we do. The same things in the culture that make us successful, may also cause the disasters. We slowly, incrementally and insidiously ‘float’ into disaster. One such Black Swan event: The Piper Alpha Oil Rig disaster, killing 167 men in the North Sea on the 6 September 1988, was one of the most pivotal events in safety around the world. It changed the thinking and focus of governments, whole industries and led to numerous books and papers. It resulted in new legislation, textbooks and a critical self-examination by the oil and gas industry."

Cathodic Protection using impressed current was employed to reduce the wear of the mill wall during ball mill grinding. Wear rates were determined from weight-loss measurements made on four, I-inch diameter, mild steel coupons installed flush with the interior surface of the mill wall. Grinding tests were conducted on quartz, pyrite and quartz-pyrite mixtures at pH values of 4.6, 7.1 and 9.2. Cathodic protection was shown to reduce mill wear by 40-60010. An examination of the coupon surfaces using scanning electron microscopy showed that cathodic protection greatly reduced pitting corrosion. The presence of a sulfide mineral, such as pyrite, was found to increase the wear rate significantly, possibly due to galvanic interactions. It is also found that the wear rate is significantly higher than the sum of abrasive wear and the corrosive wear, due to a synergistic effect.

Global mining company achieves significant productivity gains with three-dimensional (3D) mine models developed with Abaqus finite element analysis software. In a steep change beyond traditional processes, Abaqus finite element analysis (FEA) software from Simulia (the Dassault Systèmes? brand for realistic simulation), is being used to enhance mine design and engineering simulation at a number of major mines around the world. In North and South America, Africa and Australia some of the world?s biggest mining companies are applying FEA technology to evaluate safety and improve design planning, implementation and operations. Beck Arndt Engineering (BAE), a Sydney-based international consultancy, is a pioneer in the commercial development of engineering solutions for the mining industry. The consultancy has worked closely with engineers at Simulia Australia to expand the use of Abaqus FEA simulation software for mining applications. Among the early adoptors of mine-ready FEA technology is the world?s largest miner, BHP Billiton. With BAE?s help, BHP has already applied this technology to evaluate mines in Canada and Australia. At the BHP Billiton Nickel West Perserverance Deeps Project in Western Australia, Abaqus FEA software is now being used to help engineer the safety and productivity of planned deep-mining operations. To achieve this goal in the deep-mining environment significant technological innovation is required. Using measurements of site deformation and seismicity, Abaqus FEA models have been calibrated and, in a single day, used to simulate a full, 3D, inelastic analysis of a mine?s life cycle. In recent years, similar applications at Debswana?s Jwaneng Mine in Botswana, the Newcrest Mining Ridge-way Deeps Project in New South Wales, Australia and Rio Tinto?s Argyle Diamond mine in Western Australia have also established Abaqus FEA analysis as the lead-ing technology for multi scale, simulation-aided mining engineering. Joop Nagtegaal, a pioneer of FEA and a Dassault Systèmes Corporate Fellow, said that Abaqus FEA software is unique in its capabilities to enable mining engineers to investigate design innovations from the drawing board to full production. ?In the design stage, Abaqus models, which include rockmass volumes spanning several kilo-meters around the ore body and down to excavations just a few meters across, are used to compare and optimize engineering options,? Nagtegaal said. ?Then, as the mine goes into production, large volumes of data from the field are incorporated with the analysis models to allow them to be calibrated to a precision not previously available to the mining industry.?

The pelletizing process for low grade iron ores has undergone continual changes since its inception over twenty years ago. These changes have necessitated filtration equipment changes in order to produce a product suitable for balling. With modern techniques it is now possible to produce pellets of 68% Fe from ores containing as low as 26% Fe. Low grade ores require fine grinding to liberate impurities This shift to finer grinds over the years has necessitated major changes in the filtration steps. Techniques are now available for producing suitable product from ores with Blaines as high as 3600 cm2/gram. This has been made possible by the development of high performance filters. Recent developments have reduced maintenance costs and produced higher production reliability. The paper discusses iron concentrate filtration from its beginning to present day design. It will be shown that the dewatering step prior to balling has gone through four generations of filters. Particular emphasis is placed upon hydraulic studies which have allowed today&apos;s optimum designs.

"Building and operating tunnels has been a highstakes endeavor in terms of costs, complexity, safety and impact on contractors, owners and the public for a long time. There are many variables that need to be closely monitored and managed to construct a tunnel on-time, on-budget and safely. Once built, there are also many elements that need to be monitored and managed to operate a tunnel safely and efficiently.Fundamentally, the risks to the safe and efficient construction and operation of a tunnel are a function of the degree of harm and the likelihood of the harm occurring. From a cyber security perspective, risk assessments used to inform decision makers and support risk responses are typically based on identification of relevant threats; internal and external vulnerabilities; the impact that may occur given the potential for threats to exploit vulnerabilities and the likelihood that harm will occur. This article focuses on creating awareness of vulnerabilities associated with technology used in tunnel construction and operation and some of the challenges associated with remediation of these vulnerabilities. As we all know, technology has been evolving at an accelerating pace and infiltrating every aspect of our lives in new and often exciting ways. In the world of tunneling, sensors and systems that provide position and time have become accurate, rugged and cost-effective enough to come out of the research labs and into the equipment and systems used every day. There are many opportunities to employ connected computing devices and sensors to costeffectively capture, integrate and analyze key data with minimal latency to manage risk better, improve safety and avoid surprises."

The Buick Concentrator is a highly automated operation which utilizes a process control system for continuous monitoring and control. The process control system development, installation, and application to the concentrator grinding and flotation circuits are presented in this paper. The design and implementation of analog field sensing instrumentation, a continuous on-stream X-ray analyzer, grinding and classfication circuit controls, and a recent digital process control system utilizing a centralized computer are discussed. Emphasis is placed on the project planning and installation of the digital control system including a presentation of future plans for continuing application of computer control.

"There is certainly no argument that LED lighting systems have taken a strong foothold in the lighting marketplace over the last several years. This has even proven true in the world of transportation and transit lighting, for which new LED cobra head style luminaires are replacing traditional HID cobra head luminaires on North America’s roadwaysand linear LED light fixtures are replacing conventional linear fluorescent light fixtures in North American rail transit facilities. However, LED technology has taken a while longer to penetrate the very specific market of vehicular tunnel lighting in this continent. Though both the New York/New Jersey Holland and Lincoln Tunnels have recently been retrofitted with LED lighting, and an LED Lighting RFP is currently out for Boston’s Central Artery Tunnels, very few vehicular tunnels in North America to date have been illuminated with LED light fixtures. This is largely because cost effective LED lighting, that met our typically stringent tunnel lighting standards, was until recently simply unattainable. However, with the rapid advances in LED technology over recent years, owners and specifiers of North American tunnel lighting projects have finally begun to adopt LED lighting systems as the norm. In fact, a recent Lux Fit/Yole Development “LED in Road and Street Lighting Report” stated that “LED luminaire growth will be driven firstly by tunnel lighting, and then relayed into highway, road, residential and amenity lighting applications starting in 2014.” 1 Our highways are continually becoming more congested. As a result, Provincial and State Departments of Transportation (DOTs) are constantly looking for innovative ways to improve traffic flow. One way DOTs are now helping to ease congestion is by expanding the use of tunnels and long underpasses in large interchange reconstruction projects. With less available right-of-way to work with, and the high costs of maintaining elevated bridge structures over the years, it often makes more sense to “go under” (rather than over) the traffic congestion points. With the addition of these new tunnels and long underpasses comes the issue of proper illumination within these structures, for a safe and efficient driving task. DOTs use recommended lighting practices and guidelines for highway lighting, which include specific criteria for tunnels, set by either the Illuminating Engineering Society of North America (IESNA) or the American Association of State Highway and Transportation Officials (AASHTO). However, both IESNA and AASHTO point out that these recommended practices and guidelines are not a mandate, and DOTs are free to make their own decisions about lighting criteria. Accordingly, effectively illuminating tunnels is often times a challenge. This paper will examine the key considerations of the tunnel lighting application in generalas well as what LEDs now bring to the table and how, accordingly, traditional lighting technologies will be phased out in the future illumination of tunnels."

During 1975, the Reclasource Corporation constructed and commissioned a plant for the recovery of mill scale, coke breeze, blast furnace dust, blast furnace sludge and basic oxygen furnace dust at the site of Crucible Alloy Division, Midland, Pennsylvania. The Reclaform Process was employed as the criteria for plant design. This Process takes the solid in-plant fines as listed above, commonly found in integrated steel mills, sizes and forms them through the use of a carbonaceous binder briquetting system and cures the binder through a final heat-treating step. 15,000 tons of Reclaform briquets have been tested in the Crucible blast furnace in several campaigns structured to assess material handling properties, burden feasibility and finally culminating in an iron and carbon assessment campaign. Data are presented from the records of the blast furnace operation and conclusions are drawn that both iron and carbon utilization have been demonstrated.
