"Sub-standard practices and their adverse impact on safety performance remain a challenge in the South African gold mining industry. The purpose of this study was to investigate the root causes of sub-standard practices and the effect on safety performance in South African gold mines. The focus of the study was on the underground production operations at a gold mine in the Free State. The study consisted of three parts: a personal investigation into the causes of sub-standard practices at the mine, a comparison with the results of a cultural study performed at the West Wits mines, and a behavioural survey.The studies described in this article yielded similar results in terms of the key drivers behind sub-standard practices. It was found that substandard practices had a number of causes, and the origin lies within the habits, attitude, and behaviour of employees. Current methods of addressing sub-standard practices and improving safety performance were found to discourage employees from performing work safely and according to procedure. It was concluded that the lack of critical behavioural habits for the given work environment and job title could possibly be the primary reason for the occurrence of sub-standard practices. IntroductionSince the implementation of the Mine Health and Safety Act, 1996 (Act 29 of 1996) and the Mine Health and Safety Regulations, the South African mining industry has been striving to improve health and safety performance and has placed great emphasis on adherence to mine standards.The Act requires employers in the mining industry to report accidents, incidents, and dangerous situations to the Regional Principal Inspector of Mines in their area. The data provided by employers is captured and analysed by the South African Mines Reportable Accident Statistical System (SAMRASS) and made available for public viewing (Department of Mineral Resources, 2011).A review of the national mine-related accident statistics shows clearly that the South African gold mining industry has improved its safety performance markedly over the past decade, but is still facing major challenges (Figure 1). These include meeting the Mine Health and Safety Milestones (Department of Mineral Resources, 2011)."

Insurers are hesitant to provide coverage for Open cast Pit side wall collapse, and even the consequent damage to plant and equipment in the pit as well as the resulting interference with the mining operations. After a general introduction to relevant principles of insurance, this paper addresses some of the major reasons for that reluctance based upon the authors? extensive claims experience, examines what limited coverage can still be obtained and looks to the future possibilities. Confidentiality of sources and examples referred to in this presentation is a natural imperative so that a bibliography is not provided but reference is made to relevant sample insurance policy wordings.

Anglo American Kumba Iron Ore (Kumba) continues to implement technologies focussed on improving safety and productivity; this forms part of the groups FutureSmart Mining initiative. The paper provides insights into the implementation of three technology solutions that improves the productivity of heavy mining equipment (HME). Enhancements to the short term mine planning (STMP) process, the effective use of a fleet management system (FMS) and the deployment of drones or Remotely Piloted Aircraft Systems (RPAS) are discussed. The implementation of these and other technologies contributed to a 13% and 22% increase in the overall equipment efficiency (OEE) of the primary HME at Kumba’s Sishen and Kolomela mines respectively. Four key success factors (KSF) are highlighted namely that Kumba’s technology implementation drive is a key enabler to the company’s strategy, has a strong focus on value-add, is underpinned by a holistic and integrated implementation approach and is people-centric.

In multivariate geostatistics, it is common to have different types of complexities between variables of interest. In this context, an inequality constraint is an example of complex bivariate relationships. Unfortunately, traditional co-kriging and co-simulation algorithms cannot reproduce this type of bivariate complexity, leading to the overestimation of disturbing elements. This paper proposes a new algorithm based on a hierarchical sequential Gaussian co-simulation framework, integrated with inverse transform sampling, to model inequality constraints between variables. First, the proposed methodology's validity was evaluated by applying it to a real case study from an iron deposit, with an inequality constraint between iron and aluminum oxide. Then the simulated results were compared with a conventional hierarchical co-simulation algorithm to investigate the effect of inverse transform sampling on the quality of the co-simulation. The results showed that the proposed algorithm can reproduce an inequality constraint between variables.

Introduction Zircon is a natural occurring mineral and may exhibit natural low level radioactivity. It contains trace levels (90 to120 pCi/g and 110 to 115 pCi/g respectively or 3.3 to 4.4Bq/g and 4.1 to 4.3 Bq/g) of naturally occurring radioactive uranium and thorium. This mineral is often found in a suiteof minerals (rutile, ilmenite and monazite) Due to the overlap in physical properties of monazite (containing 4–7% thorium) with some of the zircon (slight magnetic susceptibility) it often contaminates the zircon, adding to the levels of thorium in the product. Hazards associated with the products from zircon and its processing is inhalation of respirable dust (generated by milling) containing radioactive uranium and thorium which may cause cancer of the lungs. Legislature regulates the transport, use, workplace environment exposure and storage of radioactive material. In America the general regulation for the unlicensed use of low levels of radioactive materials limits the concentration to 500 ppm (U plus Th). Europe (IAEA) specifies a limit of10 Bg/g (Bequerels per gram) on the parents for transport only which is a higher level of U+Th. Companies selling into these markets need to comply with these regulations and certification is therefore required, not only of the major constituents determining the price, but of the U and Th levels as well. Exceeding the limits for U and Th may result in material being returned to the country of origin. However, for the analyst the requirement is to analyse for these elements throughout the production process as well as the analysis of the ‘tails’ that is returned to the mine area. This requires a wide range of concentration to be analysed. Methods of analysis A wide variety of methods exist for the analysis of U and Th in a range of matrices. (Table I) All these methods have been used with varying degrees of success in terms of accuracy. Overall precision for the different techniques varies from 5 to 25% relative. Time to produce results from these methods vary from a few minutes to several days. The Production Chemist is expected to produce between5 and 10 analyses per hour for process control purposes. A suitable technique that fits this requirement is X-Ray Fluorescence. Both Wavelength Dispersive (WDXRF) and Energy Dispersive X-Ray (EDX) instruments are suitable. X-Ray analysis in general has a few major advantages over competitive techniques. •It is fairly simple to set up and interferences are known and easy to correct •It has multi-element analysis capabilities •Clean spectral lines for U and Th (in zircon specifically) •Uncomplicated and easy to measure background •Low detection limits (3 to 5 ppm) •A very high degree of precision •Sample preparation is uncomplicated and fast (pressed powders and fused beads). In Zircon (ZrSiO4) processed sands the matrix is constant(>99% ZrSiO4) and the background is thus essentially constant. A typical set up for the analysis of U and Th by WDXRFis given below: X-Ray Tube Tungsten (W) Target, 2.5 to 3kW operated at 50mA/50kV Crystal LiF200 (or LIF220 for higher resolution) Analytical Lines (2?)U La1,226.14Th La1,227.47 Detector Scintillation Counter Analysis time, depending on the concentration can vary from as little as 20 seconds up to 5 minutes. Standards As with any other comparative technique XRF analysis relies heavily on the quality of the standards used. Assuming proper set-up and operation of the instrumentation as well as consistent sample preparation, accuracy is determined by the quality of the initial calibration standards. Pt-based alloys are being developed for high-temperature applications with the aim of replacing some of the currently used Ni-based superalloys (NBSAs) in the highest temperature applications. The Pt-based alloys have a similar structure to the NBSAs, and since Pt is more chemically inert than nickel and has a higher melting point, they can potentially be used at higher temperatures, up to 1300°C, and in more aggressive environments. Several experimental Pt-based alloys were studied at Mintek, and an optimum composition was found to be Pt84:Ahi:Ru2:Cr3 (at.%). On the basis of hardness and microstructure, a later study identified a new optimum: Ptr8:A111:Ru5:Cr6 (at.%). There are at least two Pt3A1 allotropes, and the high-temperature cubic structure has better properties than the lower temperature tetragonal form, and so needs to be stabilized. This work describes the latest results obtained in transmission electron microscopy (TEM) studies of the quaternary Pt-based superalloys. These results are both interesting and important, because the samples have a higher precipitate density compared to those from earlier work. The precipitate morphology is mainly cubic, with minor areas having irregular-shaped precipitates. The high volume fraction of the precipitates is a major breakthrough, since the objective of this work is to improve the alloys. A prior disadvantage was that the proportion of the precipitates was too low in samples before this work, especially compared with the work from Germany on Pt-Al-Cr-Ni¬based alloys as well as the NBSAs. TEM -Pt3A1 diffraction patterns were studied, and for each diffraction pattern, many possible lattice point combinations were tried, with the angle between the lattice spots as well as interplanar spacings being calculated for each phase (cubic or tetragonal) to match the measured results. An overall analysis of the diffraction results indicates that the cubic phase fitted the experimental lattice points with much lower errors compared to the tetragonal phase. Thus, with the close match achieved with the cubic structure, the structure of -Pt3A1 precipitates is likely to be cubic. X-ray diffraction has been carried out on selected samples, and the results confirmed the presence of cubic -Pt3A1 and (Pt). Different X-ray diffractometers were used to further verify the results, and the results were also compared with those from TEM.

Underground mining in high rock-stress environments can induce unanticipated dynamic rockmass deformation, posing a serious risk to the workforce. Data collected by seismic networks are imperative for the design-as-you-mine process. Often an inappropriate assessment of the seismic hazard is done when unnecessary data are included in analytical techniques, particularly short-term time frames. Depending on the methodology or application of data analysis and interpretation, some of the information may not be relevant and even detract from the objective. By using the spatial event clusters associated with active workplaces to define the shape and size of the polygons for data selection for subsequent analysis, a problematic subjective component can be eliminated. The mining of tabular reefs in South Africa, be they gold- or platinum group metals, is very extensive, and underground workplaces are often widespread. Production stoppages are very costly, especially for marginal mines, so there is just cause not to evacuate an entire operation’s workforce following the occurrence of a large mining-induced seismic event. A rationale based on the damaging peak particle velocities (PPV) of the past seismicity can exclude the unaffected underground personnel and allow a quick orderly withdrawal where required. Micro-deformation may precede rockbursting, which can occur on-shift close to the workface. A mine-wide array of geophones suitable for the accurate location and analysis of seismicity may not be able to detect these higher-frequency emissions. Inexpensive but fit-for-purpose accelerometers, providing real-time, continuous measurement of acoustic rockmass deformation near the workplace, integrated through underground and surface communications networks, are shown to record ‘rock-talk’, clearly distinguishable from the instrument background noise, which can be in the order of hours leading up to an imminent damaging seismic event.

"The chromium (Cr) content of stainless steel originates from recycled scrap and/or ferrochrome (FeCr), which is produced mainly by the carbothermic reduction of chromite ore. The oxidative sintered pellet production process is one of the most widely applied FeCr processes. The supplier of this technology specifies that recycling of chromite-containing dust collected from the pellet sintering off-gas and fines screened out from the sintered pellets (collectively referred to as pre-oxidized chromite fines) should be limited to a maximum of 4 wt% of the total pellet composition. However, the results presented in this paper suggest that recycling of such fines up to a limit of 32 wt% of the total pellet composition may improve the compressive and abrasion strengths of the cured pellet. In addition, electron microprobe and quantitative X-ray diffraction (XRD) analyses demonstrate that chromite grains present in the pre-oxidized chromite fines consist, at least partially, of crystalline phases/compounds that will improve the metallurgical efficiency and specific electricity consumption (i.e. MWh/ton FeCr produced) of the smelting process. Introduction Stainless steel is a vital modern-day alloy that is well-known for its corrosion resistance, which is mainly due to the inclusion of chromium (Cr) (ICDA, 2013). Stainless steel is mostly produced from recycled scrap and ferrochrome (FeCr), a relatively crude alloy of Cr and iron (Fe). FeCr is predominantly produced by the carbothermic reduction of chromite, a mineral belonging to the spinel group characterized by the unit formula [(Mg,Fe2+)(Al,Cr,Fe3+)2O4] (Haggerty, 1991; Paktunc and Cabri, 1995; Tathavakar, Antony, and Jha, 2005). Although Cr can occur in 82 different minerals, chromite is the only source of new Cr units that can be exploited in commercial volumes (Motzer and Engineers, 2004). Approximately 95% of mined chromite is used in the production of various FeCr grades, of which high-carbon and charge grade FeCr are the most common (ICDA, 2013)."

The goal of the study described here is to develop objective. quantitative methods for estimating the outcome probabilities of exploratory oil wells. The study employs the 're-experience' approach, in which historical data are used in a manner which attempts to recreate the past. Our past experience, of course, is the principal guide to the future. To appraise present-day oil prospects, however, we must analyze prospects that have been drilled in terms of how these former prospects were interpreted before they were drilled. An analysis of how they are interpreted after they were drilled is an inadequate guide to the future because our hindsight knowledge is superior to our foresight knowledge. Objective calculation of future outcome probabilities, then, requires that we attempt to treat past information in a manner analogous to that in which we must treat present information in forecasting future outcomes.

In particular, this model allows calculating metal extraction efficiency as a function of pH, solvent flowrate, stripping flowrate and aqueous feed flowrate in the mixers-settlers. To reduce the frequency of mining incidents and improve safety and production efficiency, it is important to develop technology for the autonomous positioning and navigation of mining equipment, without the assistance of satellite navigation signals. In this study we review the current technological environment for positioning and navigation underground and present several integrated navigation strategies. Existing underground positioning and navigation technology is described, and the performances of different methods based on this technology analysed and compared. The combination types and positioning effects of underground positioning and navigation strategies are discussed. Recent research directions for filtering methods and positioning estimation algorithms for underground positioning and navigation are reviewed. Finally, we summarize the future direction of underground positioning and navigation technology and related algorithms.

More than half of global cobalt reserves are located in the Southern African Copperbelt, stretching over the northern tip of Zambia and southern area of the Democratic Republic of Congo. In these areas, cobalt is not typically produced as a primary product but as a byproduct of copper production. Ores from this region often contain uranium, which is readily leached under typical copper leach conditions and enters the process. Uranium does not pose a challenge to copper production, but it becomes a significant problem when it follows cobalt to the cobalt precipitation process and results in unacceptably high levels of U in the final Co product. This paper evaluates the existing process technologies that can be used to remove uranium from cobalt-bearing process streams, including phosphate precipitation and ion exchange. Advantages and disadvantages of the uranium removal processes are discussed, and important design considerations are highlighted.

There is currently limited knowledge concerning South African coal deposits as hosts for rare earth elements (REE). This project aims to determine the concentration of REE, including yttrium and scandium (REY+Sc) in various coal seams and adjacent sediments in borehole cores from the Ermelo, Witbank, and Waterberg coalfields in South Africa. Coal petrography (maceral count and vitrinite reflectance), XRD, XRF, sequential chemical extraction procedure (SCEP) before ICPMS, and mineral liberation analysis (MLA) were conducted on the coal and associated sediment samples. The coals were inertinite-rich, medium-rank bituminous coal, with moderate to high ash content. The two dominant minerals in all the coalfields are kaolinite and quartz, except for sample ZBS2M, where dolomite was dominant. The dominant major oxides are Al2O3, SiO2, Fe2O3, and CaO, which concur with the XRD results. The ICP-MS results following SCEP indicated that both coal and the associated sediments were light REY+Sc dominant. There was also an elevation of medium REY+Sc in the Witbank coal’s associated sediment samples. Notably, the samples with the highest REY+Sc results were the sediment samples. The Waterberg samples had the lowest REY+Sc from both the coal (less than 2 μg/g to 7 μg/g) and associated sediments (2 μg/g to 15 μg/g). While the highest REY+Sc was yielded from the Witbank-associated sediments (500 μg/g to 2,500 μg/g). The carbonate fraction step proved to be the most successful leaching step, as most REY+Sc was recovered in that step for all coalfields. In addition to the carbonate fraction, the silicates and sulfide fraction in the Witbank samples, and the ion-exchangeable fraction for the Waterbergassociated sediments samples proved successful in leaching out the REY+Sc. MLA determined that monazite and xenotime in all the coal samples were the REY+Sc-bearing minerals.

The mining industry, both here and internationally, is continually developing increasingly sophisticated software to compute mineral resource estimates, but these processes cannot compensate for a flawed geological interpretation of the geology of the economic commodity(s) concentrations. Quality assurance and quality control (QA/QC) is becoming increasingly common in association with the sampling and assaying practices within the industry. However, it is rare in the case of a geological interpretation associated with mineral resource estimation. Why? In the context of this paper, QA/QC relates to the use of statistical and graphical tools to show that the geological economic commodity concentrations have been appropriately domained. Domaining is essential to improve the chance of getting a mineral resource estimate right. Commodity value and acceptable concentration variation within a mineral resource can be ingredients that make mineral resource estimation difficult to get right. But it is possible to get it right a very high proportion of the time. It is more likely to occur when inappropriate data consolidation is minimised. In any case, inappropriate data consolidation is significantly reduced when the geological interpretation is strongly consistent with the data and it is demonstrable. To achieve quality global estimates of geological domain concentrations, it is essential that the geology of the prospect is understood sufficiently that the deposit is appropriately domained. Appropriate domaining occurs when sample populations are separated in such a way that only data in harmony with each other within the population contributes to the domain's concentration estimate. Anglo American Platinum Limited is constantly striving to improve safety and productivity. There are many different initiatives taking place within the organization in order to ensure continued safe operation and profitable ounces for the future. One of the opportunities and potential growth areas that has been identified and is being explored is that of mechanization. In order to do this, the approach that has been adopted by Anglo American Platinum is to fully understand and analyse the current mechanized mining methods. Once each configuration is fully understood, the method can then be optimized and new mining technologies and systems can be developed to overcome the current shortfalls. This paper will discuss a methodology that was developed by Anglo American Platinum (in conjunction with Cyest Corporation) and adopted to optimize the mechanized mining layout and fleet configuration, and will also present the final findings of the study. Due to the impracticability of piloting every possible option in the workplace, extensive modelling and simulation exercises were conducted to determine realistic parameters for productivity and value. The study resulted in a full understanding of mechanized mining systems and their associated layout. From the results of the study a mining / logistic layout was developed which highlighted the following opportunities: 1. Create safe operations through the reduction of personnel in the ?high-risk zone? of stopes 2. Separate machines and personnel by use of remotely operated machines 3. Create focused mining on primary development from the production stoping, thus having dedicated teams on development and stoping. 4. Develop primary development ahead of the stoping activity. This enables: a) Understanding of the geology and subsequent implementation of the correct b) technology (?fit-for-purpose?) required to mine that area based upon this geological knowledge c) Ability to install tipping points ahead of stoping, creating ?immediate stoping reserves?, thus increasing flexibility and reducing production risk d) Improved productivity by having adequate faces available and reduction in re-development to establish faces. 5. Quicker return on Investment due to recovering revenue from ?on-reef development? 6. The layout allows flexibility between stope sections enabling better ?resource sharing? 7. Optimize the long-term ratio of machine makeup of the fleet in relation to the production outputs 8. Creates an agile mining layout allowing rapid response to market pressures. The findings and work presented in this paper is currently being implemented at different test sites within Anglo American Platinum and will demonstrate that improved safety and productivity can be achieved through the adoption of mechanization.
