Long Island Rail Road’s (LIRR) service to New York City is limited to Penn Station on the West Side of Manhattan, which is operating at its practical capacity for the commuter railroad. This situation imposes serious considerations for future growth and limits the destination of many Long Island commuters. The East Side Access Project will provide a connection of the LIRR to the landmark Grand Central Terminal (GCT)on the East Side of Manhattan. The construction of the connection will require significant tunneling and underground work utilizing a variety of construction techniques. In Queens, the connection will require five soft ground tunnels using pressurized face tunnel boring machine. In Manhattan, the connection will require a series of rock tunnels and caverns connecting to two deep large station caverns under GCT to accommodate eight tracks with four platforms in a stacked configuration. The caverns housing train rooms are connected to the lower level of GCT where a large concourse will be provided. With a budget of $4.3 Billion, The East Side Access Project is the largest transportation project ever undertaken in New York City. This paper provides an overview of the project, discusses its environmental and the engineering challenges, and provides an insight into the design development to date.

A conventional mechanical anchor bolt consists of a smooth bar threaded at the anchor end, with a conical wedge, and a mechanical shell anchor. When the smooth bar is torqued, the conical wedge pushes the shell anchor against the borehole wall. Coal mines use mechanical bolts in addition to other types of bolts to control the deformation and stabilize the yielded coal ribs. Limited research has been conducted to understand how a mechanical anchor performs in a coal rib. Researchers from the National Institute for Occupational Safety and Health (NIOSH) conducted the study described in this paper to define the loading characteristics of mechanical bolts (stiffness and capacity) installed in coal ribs at five underground coal mines. Standard pull-out tests were performed in this study to define the loading characteristics of a mechanical rib bolt. Based on the strength of the coal seam and the conditions of the coal mines, the bolts were installed at different amounts of torque. A typical tri-linear load-deformation response was obtained from these tests. It was found that the anchorage capacity depends on the coal strength, and the anchorage stiffness depends on the installation torque. The outcome of this research provides essential data for rib support design. INTRODUCTION Rib falls are a serious hazard in underground coal mines. In the last 10 years, rib failures have resulted in 17 fatalities, representing 52% of the ground-fall fatalities in underground coal mines in the United States (MSHA, 2018). Although resin bolts has been used increasingly in the last few years to control coal ribs, mechanical bolting (mainly expansion-shell bolts) is still used in many underground coal mines. Recently, NIOSH researchers surveyed 90 sites in 14 underground coal mines in the Eastern United States and found that 79% of the mines determine their rib support requirement based on the depth, mining height, and the loading conditions, of which 35% use the conventional mechanical bolts. Conventional mechanical bolts are preferable for rib support because they do not require the miners to be too close to the rib during rib bolting. Hence, using the conventional mechanical rib bolts can reduce the risk of injuries and fatalities during the installation process. The mechanical bolt or the point-anchored bolt is the oldest type of rock bolt that is still in use today. The point-anchored bolt is a tensioned bolt anchored down the hole with a mechanical expansion shell or a grouted medium (Peng and Tang, 1984). Figure 1 shows the components of the mechanical anchor bolt. The mechanical anchor bolt consists of a plug, a shell anchor, a headed smooth bar with the top end threaded, a bearing plate, a hardened washer, and sometimes a friction-reducing washer. The shell anchor can be either standard or bail type. The shell of the standard type is held in place by a nut, while the bail anchor uses a strap to hold the shell in place (Peng, 2008). The diameter of the smooth bar is generally 5/8 in for the #5 bolt or 6/8 in for the #6 bolt. The diameter of the hole that the bolt is inserted into must be carefully controlled since an oversized hole can result in poor anchorage (Moss, 1971). The anchorage is obtained by applying torque to the bolt head, which in turn pulls the wedge down in the shell and expands the serrated leaves against the sides of the hole-wall. In the resin-grouted anchor, the resin replaces the mechanical expansion shell at the end of the hole. The anchorage is achieved by bonding between the resin, bolt, and the hole-wall.

The Kidston gold discoveries were originally made about 1907, and by 1934, 1.76 Mg (62,000 oz) of gold had been produced from placer and lode mining of higher grade, most¬ly oxidized gold ore. As mining in the lode ore penetrated deeper, costs increased and the world gold price stagnated. The lode or underground production averaged close to 6.9 g/t (0.2 oz per ton) Au for the 201 kt mined. No mining or development took place from 1934 until 1965. Kidston's high grade ores were exhausted by 1934. From 1965 to 1976, exploration had developed 3 Mt of about 3 g/t (0.88 oz per ton) Au. Placer Pacific acquired rights to the property in 1976 and began a geologic and engineering study of the district. It was clear that the deposit had the potential for becom¬ing a large-tonnage, low-grade producer. However, explora¬tion had to discover the tonnage and grade and engineering needed to solve the obstacles of production costs. Bulk material handling problems were addressed by Placer's wide experience in open pit mining, and the increase in work index accompanying deeper ores was handled by Placer's metallurgists. The matter of power for the operation was solved by engineering studies. These showed that power could be generated and brought in economically. The price of gold also increased after 1976, and this helped in the economic analysis. The Kidston discovery since 1976 is really a combination of good exploration geology and careful engineering plan¬ning that overcame the obstacles presented by ore characteris¬tics and the location of the deposit. INTRODUCTION During the past decade, significant pro¬gress has been made in applying rock mechanics principles to mine design problems by a great number of researchers. Some of these new techniques have found their way into the engineering and planning offices of the operating mine. In spite of these advances, however, the application of rock mechanics in the day-to-day mine planning and design of our domestic coal mines is rela¬tively rare. There are a number of reasons responsi¬ble for this relatively slow progress, not the least of which is the question of how to measure the success or cost effectiveness of a sizable rock mechanics effort in an opera¬ting mine. Firstly, the properties of geologic materials generally vary over a wide range and it is, therefore, difficult to assess whether an improvement in the mining conditions is due to a design change or due to a change in the geologic conditions. A second reason for the slow acceptance of rock mechanics in engineering thinking is the fact that some of the concepts are quite difficult and that physical evidence of their validity to mine design is still rare. Thirdly, com¬munication between research, engineering and applications oriented people appears to function only intermittently. In our view, however, the most important impediment lies in the "state-of-the-art." Exactly how much can rock mechanics help the mine operator? The best that rock mechanics can do at present is determine whether one approach is better than another when weighed against some design objective (stress, strain, deformation). We cannot determine the abso¬lute best ground control design -- only what is better. If ground control is lost, the statement that "it's the best one we looked at" means very little to the harried operator. While the "state-of-the art" has advanced a great deal, it has certainly not been reduced to an engineering matter in that design hand¬books are available -- they are not. The success of a rock mechanics program, there¬fore, can be determined only over a long period of time of continuous effort.

In 1992, United States mine production of barite decreased 8.5%. Barite consumption, sold or used by grinding plants, decreased by 36.9% and imports decreased by 61.6% vs. 1991. World mine production decreased 1.5%, according to the US Bureau of Mines. The value of domestically produced barite, f.o.b. mine, decreased 11.9% according to the Bureau. The declared value, c.i.f. United States port, of all imported crude barite during 1992 decreased from $42.35/t ($38.42/st) in 1991 to $40.09/t ($36.37/st). Nevada continued to be the leading producer of barite with 88% of the total, followed by Georgia and Missouri. It is estimated that 80% of the domestically mined barite was used as a weighting agent in drilling fluids. The remaining 20% was used in barium chemicals, glass or as a filler. Most of the production from Missouri, Georgia and Ten¬nessee was used in the non-oilfield sector. Of the total barite used by grinding plants and chemical manufacturers, it is estimated that 90% was used in drilling fluids and 10% in the non-oilfield sector. This included barium chemicals and fillers in paper, plastics and glass. The tonnage consumed in the drilling fluids market decreased due to the lower average rig count during 1992. The average number of rotary rigs operating (717) was 16.6% less than the 1991 average of 860 in the United States. Crude barite imports were 518 kt (571,000 st), 61.6% less than in 1991. Imports accounted for 44.7% of the barite consumed (apparent) in the United States. During the year, China continued to be the leading exporter of crude barite ore to the United States with 67.2% of total crude ore imports. India followed with 31 %, and others with 1.8%. With the construction management team established and a full funding agreement secured the Washington Metropolitan Area Transit Authority (WMATA) now has the manpower and the financial resources to continue Metrorail construction into the 1990's. This paper describes WMATA's new role as construction manager after the transition from Bechtel Associates and gaining the full funding agreement with the Urban Mass Transit Administration (UMTA) and regional governments to complete 84.7 Metrorail miles (136km). The authors review the most recent station and line tunneling projects. WMATA's contractors have employed the state of the art soft ground and hard rock tunneling equipment and methods - an earth pressure balance TBM, a closed face TBM, and the New Austrian Tunneling Method (NATM). In final, the paper previews future underground work to be let under the funding agreement.

The NS9 Drop Shaft contract formed part of the Waste Pollution Abatement Program (WPAP) conducted by the Milwaukee Metropolitan Sewerage District (MMSD). The contract involved the construction of two shafts, a deaeration chamber and connecting tunnel, and ancillary facilities. Portions of the construction were located in saturated overburden where dewatering was prohibited. Ground freezing was utilized for temporary support of the excavation. This paper concentrates on the freezing system and the excavation of frozen ground, with particular emphasis on the use of mechanical cutting equipment, including a raiseborer and a roadheader.

This paper describes and demonstrates the application of systems analysis and mathematical modeling techniques in the planning of actual tunnel projects and research and development to improve tunneling techniques. A recently completed tunnel project located in Utah is used as an example. Although cost estimating computer programs are used in tunnel project planning, the introduction of performance modeling, simulating the time to excavate, has added advantages that are explored here. The performance/cost tunneling model (TM) described herein mathematically simulates on a high-speed digital computer the important activities of a number of tunneling techniques accounting for activity interactions, variable external environments, mechanical failure, and other uncertainties. A cost accounting segment is included to facilitate cost-effectiveness comparisons. It is shown that, when combined with informed judgment and appreciation of limitations, the TM can be a useful tool that may help improve tunneling operations and cost estimation, for example, by achieving more balanced system designs or bounding the performance/cost implications of geologic and other uncertainties. Ultimately, this might lead to more competitive bids and greater profits. The need for systems analysis and modeling in tunneling research and development (R&D) is well recognized and documented.(1-5) In R&D one must seek broadly useful solutions to the basic problems of face fragmentation, materials handling, ground support, and environmental control. These solutions must take account of the interdependence of these elements and, for a variety of ground conditions, the limitations or constraints that any innovation in one element may impose on total system performance and cost. The most perplexing problem that has confronted mining engineers since the dawn of mining has been strata control. For the purposes of this paper, strata control will be defined as control of ground in the immediate area surrounding the, mine opening. To prevent movement of the surrounding strata into underground mine openings, miners have traditionally relied upon some type of artificial supports placed within the openings and located in a manner determined by trial and error from previous experience. Without the means to measure changes of stress in the supports and within the mine structure itself, an evaluation of the efficiency of the supports and mining system was impossible. Strata control thus remained an art with very little application of science. During recent years a revolution has occurred in mining. The modern mining engineer is contented no longer-with trial and error methods but is utilizing all the technological information at his command to obtain a solution to this age old problem. However, serious problems have arisen due to the lack of certain basic knowledge in three important areas: 1, The original stress distribution within the earth strata prior to mining. 2, Redistribution and concentration of the original stresses with mining activity. 3. The properties of rock that are significant in failure and its prevention.

The paper gives a short resumé of the findings of the joint US - Swedish "Mechanical Boring (MB) or Drill and Blast Tunneling (DB) ? Workshop" in Stockholm December 1976. New developments and trends in both MB and DB techniques in Europe since 1976 are described, and information is given on some interesting new projects. Differences in contracting principles between the US and Europe lead to different practices for preinvestigations, design, excavation and arbitration and legal affairs. It is shown that this often leads to a higher cost in the US than in Europe for similar excavation jobs. A typical US tunnel job also has more men on the site than a similar job in Europe. This adds to the cost difference and is only partly balanced by a slightly higher rate of excavation in the US. A comparison is made between two quantitative methods of predicting MB penetration rates. These are in reasonable agreement. A probable future development in machine design is an increase in cutter load towards perhaps 25 tons per disc where the tunnel diameter is large enough to house the machine size dictated by the size of the very large electric motors required. Recent European developments in drill and blast technique include improved methods for cautious blasting, precision hydraulic drilling of small diameter holes, and non-electric interval blasting techniques. These developments still keep the drill and blast method ahead of mechanical boring costwise for tunnels in igneous rock whereas for most sedimentary materials mechanical boring has a lead, particularly for long tunnels.

"A wind tunnel was designed to examine air flow, heat transfer and layering near an active fire at a working face. This tunnel simulates an active entry in an underground coal mine including an attached cross-cut of similar dimensions. A steel rectangular duct 1 ft. (0.305 m) tall, 3 ft. (0.914 m) wide and 10 ft. (3.05 m) long composes the main body of the tunnel and represents a mine entry. Air flow is generated by an exhausting centrifugal fan attached to one end of the crosscut. This fan was sized to supply air velocities based on Froude scaling. A 6 in. (15.25 cm) propane burner is used to simulate fires of various heat release rates at the working face. Thermocouples are used to map heat layering in the tunnel. Ports are installed for the introduction of tracer gases into and from sampling the system. Barometric, differential pressure, and temperature sensors are also included to monitor environmental conditions. Details about the design and testing of the system are presented including an overview of how the tunnel was scaled to accurately represent fire and flow phenomena. Finally, the proposed use of the tunnel for better understanding of mine fire phenomena and prevention is given. INTRODUCTION Fires in underground mines have been a concern since the dawn of mining. While technological advancements, regulations and best practices have improved both fire detection and firefighting techniques, mine fires continue to represent high risk events. From 2006 through 2015, fires accounted for 1.06% of fatalities in underground coal mines and 0.85 percent of fatalities in all underground mines in the United States (U.S. Department of Labor, 2006 to 2015). In addition to the risk of loss of life, mine fires can cause millions of dollars in equipment damages and lost production revenue. Extensive research has been conducted on how fires occur and spread in underground mines. In addition to testing conducted by government agencies such as the Mine Safety and Health Administration (MSHA) and the National Institute for Safety and Health (NIOSH) (e.g.(Liming, 2006), researchers like Verakis and Dalzell (Verakis, 1988) and Lowndes, et. al. (Lowndes, 2004) have furthered this field by studying ignitions and propagation of conveyor belt fires. While the studies of belt fires in mines are extensive, studies focused on fires at the working face are few in number. The particularly confined space and limited access to a working face, coupled with less robust, temporary ventilation controls create a unique environment that warrants special attention. Separate from studies on mine fires, the use of tracer gases to characterize flow patterns in normal mining ventilation is well established. In 1982, Timko and Thimons (Timko, 1982) outlined current and future applications of sulfur hexafluoride (SF6). However, little experimentation has been conducted to study specifically how tracer gases might be used to evaluate active mine fires and the fires’ effect on the surrounding mine environment (e.g.(Lindsay, 2014). In order to better understand the dynamics of smoke layering from a fire in an enclosed space, a small-scale apparatus was designed to map smoke layering and to examine other aspects of fire behavior at a working face. In addition to studying smoke flow around an active fire, experiments will be conducted using tracer gases in order to determine how they might be used to evaluate an underground fire."

Noise control is an important consideration in the design and operation of coal preparation plants. It is essential to understand and control noise sources to provide a safe environment for operating personnel. The average noise exposure levels in a typical coal preparation plant generally range between 95 and 100 db. An operator should not be exposed to noise levels of this magnitude for more than 4 and 2 hr, respectively, according to MSHA (Mine Safety and Health Administration) regulations. The major noise sources in a coal preparation plant include noise from impacting the inside surfaces of steel chutes, vibrating screens, coal crushers, pumps, motors, fans, and blowers. The magnitude of noise varies considerably from one situation to another. NOISE EXPOSURE IN COAL PREPARATION PLANTS Noise regulations established by the Occupational Safety and Health and- ministration (OSHA) are accepted by MSHA relative to coal preparation plants. Fig. 14-43 shows a graph of the time of noise exposure in hours per day to which a worker can be exposed for each A-weighted sound level. According to this graph, a worker is permitted to spend 8 hr in a 90-db noise environment, but that time must be halved when the A-weighted sound level increases 5 db. The noise dosage a worker receives is determined by the ratio of the length of time the worker spends in a particular noise environment divided by the length of time he is permitted to spend in that environment. If the worker is exposed to several different sound levels, his total dosage would be the sum of each of the individual dosages. The equation for determining the dosage is: where C is the actual duration of exposure at a given steady state noise level and T is the noise exposure limit for the level present during the time C According to MSHA regulations, the dosage, D, should not exceed one (1) for any worker for a full day of work. As an example of typical plant noise levels, vibratory screens and crushers emanate noise levels in excess of 90 db at a distance of 3 ft (0.9 m) with no material flow. he selection of mill design parameters to suit the applica­tion requirements can result in quite different operating parame­ters for each generic type of application, even though the basic mechanical layout of the mills may remain similar. The selection of the type and size of grinding media is very important to efficient and cost-effective operation, and to ensuring that the milled product achieves the required properties. Accordingly, the grinding media selection is seen as an integral part of the mill design. Consideration of the requirement for classification is important in order to achieve the optimum mill and circuit performance. This can lead to the circuit feed not being classified for a steady feed with minimal slimes and a low size reduction ratio. Feed containing on-size particles would be deslimed by open circuit classification. Closed circuit classification is performed where the feed fluctuates. The rheological characteris­tics of the circuit feed slurry can have an influence on mill performance, and needs to be allowed for. A common misconception frequently encountered is the belief that "a mill is just a mill". This is not the case with stirred types of mills, since the operating parameters selected for the mill can have a large effect on the product characteristics. For example, horizontal shaft high speed stirred mills have a com­pletely different grinding action to vertical shaft low to medium speed stirred mills, which results in large differences between the surface area produced at a given product size (Liddell, 1986). Hence, it can be misleading to apply test results or design procedures obtained from one type of mill to other types for scale-up. The Metprotech stirred mill, as applied in the mining and minerals industries, is a low to medium speed vertical shaft mill. The basic design has been discussed previously (Liddell, 1989). Currently, the largest mills that have been manufactured have an installed power of 400 kW. The largest chamber volume at this power is 6000 litres, containing IO t of grinding media. It is intended that the installed power will increase up to lMW as applications requiring larger sizes arise.

"A paper called “Global Experience with Soft Ground and Weak Rock Tunneling under Very High Groundwater Heads” was presented at the North American Tunneling 2006 conference (Holzhäuser et.al 2006). It summarized global experience with earth pressure balance (EPB) and slurry shield tunneling on nine projects that encountered groundwater heads ranging from 6 to 11 bar. Since then, additional projects have been completed with groundwater heads ranging from 7 to 15 bar. Of these, the high pressure tunneling experience on the Lake Mead Intake No. 3 project in Nevada and on the Eurasia Tunnel (Istanbul Strait Crossing) had the greatest lengths of tunneling at face pressure over 7 bar. Experience with sustained pressurized face tunneling at groundwater heads over 7 bar and risk management observations are the focus of this paper. INTRODUCTION High groundwater head, particularly at 7 bar or more, is a major challenge when tunnelling in high permeability ground and on drives long enough to require cutter changes. It has a strong impact on design and operation of Tunnel Boring Machines (TBMs) in order to protect the main bearing from contamination, prevent excessive groundwater inflow, ensure face stability and enable access to the cutterhead and excavation chamber for maintenance. During the past 20 years, TBM manufacturers and suppliers have made tremendous improvements that allow tunneling at groundwater pressures to 17 bar or more. The most significant risk with high pressure tunneling is generally that of excavation chamber access and potentially the cutterhead for cutter changes, cutterhead repairs and TBM maintenance. Hyperbaric interventions are possible and safety variances have been approved for saturation dive interventions at pressures as high as 17 bar (Lake Mead Intake No. 3). While high pressure hyperbaric intervention equipment is available and safety approvals can be obtained at many locations, few contractors consider the cost and risk of hyperbaric interventions over 7 bar to be acceptable as the primary means of excavation chamber access. Experience with high pressure tunneling prior to 2006 was summarized in a paper entitled “Global Experience with Soft Ground and Weak Rock Tunneling under Very High Groundwater Heads” (Holzhäuser et.al 2006). That paper provided information on 9 projects: South Bay Ocean Outfall, San Diego; Channel Tunnel (French Side), France; Storebaelt Tunnel, Denmark; 4th Elbe Tunnel, Germany; Wesertunnel, Germany; Westerschelde Tunnel, Netherlands; St. Petersburg Red Line, Russia; Nara Prefecture Water Conveyance Tunnel, Japan; and Tokyo Wan Aqua Line Tunnel, Japan – see Table 1."

INTRODUCTION Controlled blasting is used at some time during most civil engineering projects where there is rock excavation. Controlled blasting is often necessary because the blasting must take place in the near proximity of adjacent structures. In other instances, the blasting is controlled because a smooth perimeter is desired to minimize rock support and overbreak. The construction of tunnels and shafts for subways, rock excavations for foundations and the excavation of road cuts for interstate highways, sometimes require blasting operations which must be conducted in an urban environment. Unfortunately, blasting operations produce unwanted sounds and vibration along with their beneficial effects. Thus, in the writing of specifications for blasting in an urban environment, potential damage to adjacent structures by ground vibrations must be considered, possible damage to windows from airblasts must be considered, potential damage from fly rock must be considered, and the discomfort to people from ground vibrations, airblast, and noise must be taken into account. The contractor or the owner's engineer is faced with determining the maximum weight of explosives which may be detonated without damage to structures on adjacent property. If the weight of explosive is overestimated, the resulting damage to adjacent structures may result in costly losses. But, if the engineer or contractor is too conservative, and the weight of explosive becomes too restrictive, progress of the project may be curtailed and the cost of excavation will increase accordingly. In tunneling through water-bearing formations, the expense of controlling the water can have a major impact on overall project cost. The author has seen the cost of an eight-foot, finished sewer tunnel rise from under $500 per foot to over $1,000 when a water problem, anticipated as modest, became severe. There is need for greater professionalism in the approach to groundwater control. The days of the forked stick should be behind us. The basic technology exists. Its application is not yet widespread. This paper discusses predrainage; that is, the lowering of water in advance of tunnel excavation. Both free air and compressed air tunnels are considered. Examples from recent tunnel projects are described. SPECIAL NATURE OF TUNNEL DEWATERING Tunneling is unlike other excavations below the water table in a number of significant ways. The tunnel excavation is always on the critical path; if production slows or stops, there is no other work for the heading crews, no other work to carry the cost of service crews and job overhead. The tests were performed in exploratory tunnels constructed for site characterization purposes of an underground hydro-electric scheme. Due to the large number of data (41 petite sismique measurements, 45 Goodman jack tests and 19 flat jack measurements), it was possible to evaluate thoroughly the testing techniques employed. The purpose of this paper is to indicate the problems encountered in the interpretation of the results and to offer suggestions for selecting in situ tests. INTRODUCTION In situ tests are expensive and time consuming but they are an important aid in tunneling applications since they can provide deformability data of rock masses in contrast to rock material characteristics obtainable from small laboratory specimens. Some in situ tests are relatively easy to perform but their reliability may be in question unless a comparison of the data obtained by several different test methods is possible. Although many in situ techniques are available for determination of rock mass deformability (1), those more convenient to use are the Goodman jack, flat jacks and 'petite sismique. These methods are considered convenient because the Goodman jack is a borehole device; flat jacks can provide not only deformability data but also information about the stress field around excavations; while 'petite sismique' is based on seismic refraction surveys. Accordingly, these three methods have been selected for comparison.

INTRODUCTION For many years, in Europe and in North America, pre-formed metallic and concrete segmental linings in tunnels have been used extensively as temporary and permanent ground support systems. In the United Kingdom the use of cast iron as a means of ground support in mining shafts dates from the first decade of the 19th century. Although at that time wood tubbing was extensively employed, cast iron was used in shafts at Wallsend and Percy Main collieries in the North East of England. The original form of cast iron tubbing consisted of cylinders which were lowered bodily into the pit and were built from the bottom up, but subsequently they were subdivided into segments which involved the making of vertical as well as horizontal joints. Principally, the practice of providing immediate pre-formed support developed because of the incompetent state of the surrounding ground and, although this still remains the over-riding consideration nowadays, several other factors are examined before finally deciding what type of lining to use. Other branches of sub-terranean technology such as tunnel thrusting or immersed tubes (both of which involve pre-formed linings) can provide alternative solutions to problems hitherto thought better solved by bored tunnelling methods. However, this paper will confine itself to those linings developed for the more traditional tunnelling methods. Usually the shape of segmentally lined tunnels is circular because this profile is the most suitable for bad ground and many tunnelling machines.
