The most frequent causes of accidental injury and fatality are carelessness and ignorance; and the most common accidents are haulage accidents, fall, drowning, and fire. However, in this modern day and age, severe injury has become the dire consequence of an ever-increasing number of industrial and mining accidents as well as sport-, military- , etc. Mine accidents have declined dramatically in number and severity through decades of research, technology, and preventive programs. However, preventing recurrence of disasters like those of the past remains a top priority requiring constant vigilance by management, labor, and government. The goal of the present research is to investigate and identify root causes associated with types of mining accidents (victim being caught by an equipment, bodily motion, over exertion, etc) and nature of injuries such as (sprain and strains, fracture, dislocation, amputation, etc). Further, we hope to discover the impetus of the root causes occurring more and more often in this modern day of technology and quality awareness. With the different dimensions of accidents and injuries brought into the fold, there is the need for researchers and mining operators to comprehend and better still visualize significant patterns of associations among these variables of interest. We assume that, there is a high correlation between many dimensions of nature of injuries and types of accidents. This suggested a principal component analysis to reduce the dimensions for easy and amenable interpretation.

"Investigations of flotation characteristics of Lumwana and Kansanshi ores using formulated frothers were carried out. Of the six frothers tested, two came from the alcohol family, two from the glycol family and last two were formulated products. The initial work involved determining the optimum mesh of grind and frother dosage for Lumwana and Kansanshi ores. Further work involved carrying out surface tension, bubble size and froth stability measurements. Finally, water recovery tests were conducted at Lumwana and Kansanshi mine sites. For froth stability tests, six frothers were tested on a two-phase (water–air) system using the Bikerman column flotation cell, while water recovery tests were conducted using a total of seven frothers, including the standards for each plant. The results of the testwork showed that (i) although a stable froth is essential for optimum recovery of valuable minerals, a higher molecular weight frother, like Dowfroth 250, caused poor selectivity when used on fine ores and (ii) a lower molecular weight frother was suited for Lumwana ore, while a mid to higher molecular weight formulated product was suitable for Kansanshi sulphide ore. Firstly, froth stability tests were conducted using six frothers, two from the glycol family (Dowfroth 200 and Dowfroth 250), two from alcohol family (MIBC and pine oil) and other two from the formulated family (Betafroth 20 and Betafroth 245), at the University of Cape Town’s Centre for Mineral Research (CMR) laboratories. Your Excellency, on behalf of the Council of the Australasian Institute of Mining and Metallurgy, and, indeed, on behalf of everyone present, I extend to you a welcome, and our thanks for your gracious acceptance of the invitation to be here to-day and, later on, to present Honorary Membership Certificates. I also welcome the several official representatives of our Allies, of Great Britain and our sister Dominions.To Messrs. E. C. Driffield and E. J. Rigby, two of the original Foundation Members, who are with us to-day, I extend greetings.The present occasion is opportune to express, on behalf of a representative body of professional men, deep appreciation of the sacrifice which His Majesty the King has made, and will continue to make, by appointing His Royal Highness the Duke of Gloucester as Governor General of Australia and, therefore, His Majesty's senior personal representative in this country.I suppose, that as the primary reason for our assembly to-day is to celebrate the Fiftieth Year of the Institute's existence, I should properly on this occasion, give an account of its history and development. However, as this subject was ably covered by Mr. L. V. Waterhouse, in his Presidential Address in 1938, I trust I may be excused for selecting some other subject. I shall, therefore, exercise my privilege as President and give you a short history of the Mining and Metallurgical Industry of this country with special reference to its effect on the development of the Commonwealth and on the national economy. I apologize in advance for any statistics which I inflict upon you; some of these will be three or four years out of date, because of censorship restrictions. I shall as far as possible refer firstly to the industry prior to and including 1893, the Institute's foundation year, and then to subsequent development.Australia, is pre-eminently a pastoral and agricultural country and it leads the world in the production of wool. Nevertheless, it has an international reputation as a producer of gold and various metals and of minetal products.The development of gold mining in the decade 1840-50 was an epoch-making event in Australia's history, because, to quote one writer's apt phrase, "it precipitated Australia into nationhood." It was certainly responsible for a rapid increase in population. Thus, during the period 1841-1861 the population of Australia increased fourfold, from 221,000 to 1,170,000.The effect of the discovery of gold in Western Australia was even more striking. The innovative Soil-Cement Mixing Wall Technique, developed independently in Japan, consists of mixing in-place soils with cement grout using multi-axis augers and mixing paddles to construct overlapping soil-cement columns. The applications of overlapped soil-cement columns include cutoff walls for groundwater control and structural diaphragm walls for excavation support. The special features of this technique include water tightness, versatile strength design, and efficient installation in subsurface materials varying from clayey soils to cobbles or soft rocks. Through the introduction of two recent subway projects in Japan, this paper presents the design parameters, construction procedure, quality control and performance of soil-cement wall for excavation support and groundwater control.

Methods for reducing the cyclic nature of drill and blast tunneling operations have been long needed. A miniature blasting system using 10 mm (.375 in.) holes, 0.91 m (36 in.) deep, and blasting only one hole at a time makes it practical for men to work behind a shield, remaining at the face during the entire process. A prototype automated module was developed to drill, load the hole and initiate the detonation without moving from the hole. A unique cost effective gell explosive and a novel, low cost initiation method were developed.

The backfill system at Cleveland Potash?s Boulby Mine places waste filter cake underground in the form of a high concentration slurry in worked out areas of the mine reducing the mine?s environmental impact. The backfill system delivers slurry approximately 1100 m down the mine and up to 11 000 m horizontally underground. The Boulby backfill system is unique in that the placed backfill waste is not used for the usual purposes such as support, pillar extraction or reducing ventilation requirements. It was specifically implemented for environmental reasons. This influenced the design and resulted in some unique features of the system. A previous paper (Wilkins et al., 2004) described the design and commissioning of the backfill system. This paper describes the operational experience with the backfill system and backfill placement since commissioning in May 2003 and looks at lessons that can be applied in other backfill designs.

Critical path modeling (CPM) has been used widely as a tool to manage large-scale projects having many activities which are conducted in series or parallel. The construction industry is a regular user of critical path modeling as a planning tool. In this context, the method is used to provide an orderly schedule for all activities to forecast delays, slippages, and problem areas and to identify critical activities where resources should be directed. It also identifies others which have slack time, and provides information on the degrees of freedom and flexibility available in completing the project. The purpose of applying critical path modeling to an underground mining system is not primarily for scheduling, but as an analysis tool. In observing underground mining systems, it Is clear that repeated sequences of activities must take place for mining to proceed efficiently. Some of these activities must be sequential, while others be carried out in parallel. The fundamental difference between critical path modeling applied to mining versus a large-scale construction project are: (1) The activities in a mining system are cyclic in nature. (2) The location of the activity varies, and the entire process is not thought of as co-located, as is the case in a construction project. Another important aspect of the network development for mining systems is the degree of detail that is desired. One can be concerned with very macroscopic networks which represent, for example, an entire mine life cycle. Networks representing this problem would be more closely analogous to those of a large construction project. On the other hand, one might be interested in evaluating alternative ways of developing a property or of mining in a given section. More detailed networks representing the section activities are necessary to evaluate system parameters and their impact on long-term production.

"The New Mexico potash exploration described herein was begun in February- 1944 by the Federal Bureau of Mines with the object of determining the amount of potash ore in the United States Potash Reserve in Eddy County, N. Mex., by exploring the deposit from the surface with vertical drill holes. The work was completed in October of the same year after 16 holes had been drilled on the reserve and in the immediate vicinity.The Potash Reserve is located in secs. 26, 27, 34, and 35, T. 20 S., R. 29 E., Eddy County, N. Mex., about 14 miles northeast of the city of Carlsbad. Drilling was done in secs. 26 and 35 of the reserve, in sec. 14 to the north, and secs. 25 and 36 to the east. All work was done on Government-owned, unleased land.The reserve may be reached from Carlsbad by traveling northeast by automobile for about 14 miles on paved U. S. Highway 62. This highway passes through the southern part of the reserve. Other parts may be reached by a State highway which passes along the eastern side of the area, then by poor dirt roads to various drilling sites. A branch of the Atchison, Topeka and Santa Fe Railroad also passes across the southern part of the reserve en route from Carlsbad to the plant and mine of the Potash Co. of America, a few miles northeast of the Project and 22 miles from Carlsbad.Work on the project was greatly facilitated by the help of Ralph H. King, associate geologist, R. K. Bailey, geochemist, H. C. Spicer, geophysicist, Hope Bunker, junior geologist, and R. H Allport, district mining supervisor, all of the Federal Geological Survey; and Charles H. Johnson, who at that time was district engineer for New Mexico for the Mining Branch of the Bureau of Mines."

A study of the accessory opaque iron-titanium oxide minerals of the Oiles Complex, central Australia, was an Important complement to an investigation of the magnetic properties of the Complex. These minerals constitute no more than 10 per cent of the rocks sampled, and generally less than 1 per cent of the ultramafic rocks ("magnetite" concentrations were not sampled for magnetic investigation).Titanomagnetite is the main opaque mineral present, although ilmenite exsolution has been widely developed. High-temperature oxidation of these produced mainly titanohematite, pseudobrookite and rutile. Both exsolution and high-temperature oxidation developed along specific crystallographic directions (along {111}), and in Irregular patches. Low-temperature oxidation is generally not well-developed, indicating the small degree of weathering in the rocks sampled.Although the Oiles Complex is layered, no apparent overall preferred orientation of the iron-titanium oxide minerals was observed, other than crystallographically controlled alignment of very fine grains within pyroxene crystals. Other textural features of the opaque oxides are restricted to symplectic intergrowths mainly with the ferromagnesian silicates.

The estimation of the spatial variation of the b-value of the Gutenberg- Richter relationship is important for both the general interpretation of the mechanism of rock mass response and seismic hazard assessment. The interpretation of b-value as a parameter of rock mass response is discussed in this paper. The methods applied to evaluate the spatial variation of b-value and the algorithm for obtaining the magnitude of completeness and b-value for subsets of data are presented with some verification analyses. The algorithms presented enable the automation of a spatial evaluation of b-value.

This specification, along with the drawings, encompasses all designs, materials, products, accessories, tools, equipment, services, transportation, labor and supervision, installation techniques, and testing procedures required for the installation of soil nail excavation support or earth retention systems. This is a Guide Specification designed to assist in the development of project specifications for soil nail excavation support and earth retention systems. This Model Specification is based on generally accepted industry standards but may require modification by the specification writer to accommodate unusual and/or unforeseen site and subsurface conditions and the particular circumstances of the project. Care must be taken to avoid internal inconsistencies. It contains language allowing for a Contractor-Designed Soil Nail Wall, which would be removed if the design is provided by the Owner (e.g. See Sections 1.2 and 4.1).

As placas de circuito impresso (PCI) são materiais heterogêneos e sua composição pode variar em função do ano de fabricação, tecnologia empregada, propósito de utilização, legislações vigentes e país de origem, entre outros. Essas características interferem diretamente no valor econômico agregado a estes resíduos, pois dependem da composição dos mesmos, de tal maneira que as empresas recicladoras tomam como base essas informações no momento da aquisição da sucata. Assim, a classificação e caracterização de PCIs é de suma importância para o mercado de reciclagem de resíduos eletroeletrônicos. A fim de alcançar este objetivo foi realizada cominuição em moinho de facas utilizando grelhas com abertura de 9, 6 e 3mm. A etapa de caracterização incluiu ensaios de digestão em água régia, perda ao fogo, análises química quantitativa por ICP-OES e MEV/EDS. Verificou-se que o lote de PCI estudado possui 39,5% de material cerâmico, 34,5% polimérico e 26 % metálico. A porcentagem em massa de cobre na placa é de 14,28%, de ferro 3,73%, de ouro 0,012% e de estanho 1,29%. O lote não é lead free e é multilayer com quatro camadas de cobre.

"This paper investigates the conditions leading to the indefinite termination of production in four critical primary panels at an underground coal mining operation, the observed shortcomings in the mining approach, and the proposed strategy to mine through the affected panels. Initial assessment of the abandoned panel conditions indicated time-dependent strata failure,(i.e. bolted roof failure overrunning intersections), which occurred from mere minutes to up to four weeks post-production, with and without prior warning of failure. This prompted the constant re-supporting of back areas, which raised safety and productivity concerns. Investigation of the initial mining conditions revealed that the failures were due to a critical combination of factors, the chief of which was isolated horizontal stress. Other factors that were initially overlooked by the mine (i.e. influence of hydraulic stress, misinterpretation of borehole data), resulted in the conditions being described as abnormal. Remedial actions were determined, and in so doing, a new strategic approach was formulated to thoroughly address all failure concerns. The four panels were explicitly planned to serve as the main intake and return airways for the recently commissioned secondary ventilation shaft, as well as providing access to millions of tons in proven coal reserves. It is thus imperative to mine the panels. A feasibility study showed that the proposed strategy set for implementation would be financially viable. The object of this paper is to give a description of the new office and changehouse building and equipment at the Zinc Corporation Limited, the office section of which has now been in full use for approximately three years, and the changehouse section for approximately two years. As the expectations of the design have been fully realized, it is thought that this description will be of interest to members and possibly of some use to others faced with similar problems.GENERALIn the centralization of the surface plant around the new main shaft the combined office and changehouse building forms a prominent feature, being located between the shaft and the mill and facing the workshops on the northern side.The scheme for housing all employees in one building was proposed by the Manager, Mr. A. J. Keast, who was familiar with a similar successful structure built at the McIntyre Porcupine Mines Ltd. in Ontario, Canada.The principal features of this building were adopted as a basis for design, and the present layout evolved to suit the particular requirements of this mine.Some of the advantages of the design are:(a) Economy in first cost and space occupied when compared with plants having separate office and changehouse buildings.(b) Easier and more constant supervision of employees and of the plant by the senior staff members.(c) Close contact between officials of all departments.(d) Close covered connection between changehouse and shaft.(e) Avoidance of duplication of ,essential services such as electric power, water, heating plant, sewerage, etc.The Managing Director, Mr. W. S. Robinson, further instructed that the following requirements be adhered to in the new building:(a) Ample accommodation to be provided for office staff and other employees.(b) Special attention to be paid to lighting, both natural and artificial.(c) Working temperatures to be controlled to facilitate comfort and efficiency.(d) The buildings and equipment' to be of a permanent nature with furnishings and fittings of modern design.

Zoning is the legal means by which land use can be controlled. Although zoning laws have historically been quick to prevent quarrying, one is hard pressed to find zoning ordinances which specifically state that land should be used (or held in reserve) for quarrying and prevent other uses such as recreation, housing, etc. This situation is especially true when the mineral to be quarried has a low intrinsic value such as sand, gravel, and crushed stone. The chance of a gold mine surviving urban sprawl is probably considerably greater than that of a sand and gravel pit, even though it is possible that the dollar value of the sand and gravel' would exceed that of the gold mine. The research reported herein was concerned with two major topics: to delineate the real costs associated with the non-protection of our construction aggregate resources (growth minerals) and to develop a computer simulation model which would aid planners in making economically sound decisions. This paper describes the problems faced by the mineral aggregate industry and discusses the role of computer simulation in planning problems. It briefly describes a simulation model which has been developed and previously reported12 and discusses the results of a pilot study conducted on the Tri-City area (Albany, Schenectady, and Troy, New York). 3

There has been a continuous interest in chemical aids for processing Canadian oil sands by hot water extraction process since the first commercial industry operation in 1967. Several inorganic and organic chemical systems have been introduced, of which sodium hydroxide is the most widely used extraction process aid by major producers. Use of NaOH, however, results in accumulation of sodium ions in recycled water, causing higher clays dispersion and producing tailings with poor geotechnical properties that turn into mature fine tailings. This is especially true for low grade and oxidized ores, which present the greatest challenges in bitumen recovery and produce the major portion of fine tailings. In this paper, we are presenting a novel method based on use of inorganic polymers as process aids for various low grade oil sands ores. Denver flotation cell and a laboratory scale hydrotransport loop were utilized to evaluate the effect of chemical dosage and operating parameters on the processability of oil sands samples from Athabasca region. Process tailings were examined for properties such as water chemistry, suspended solids, settling rate and the ease of tailing treatments. Our results demonstrate that the use of inorganic polymers during the froth process lead to a significant improvement in bitumen recovery from low grade oil sands and can improve further tailing treatments. This offers a viable solution that can be easily incorporated into current processing facilities and deliver several operational and economical benefits. Advantages in use of inorganic polymers versus sodium hydroxide are presented in details.

William Frederick McBryde Broun, a graduate of the Royal School of Mines, was employed on the staff of the Mount Morgan Gold Mining Company for some years prior to 1912, and will perhaps be remembered by some of those who attended the Mount Morgan meeting of the Institute in 1910, as an Associate Member present on that occasion. He severed his connection with the Mount Morgan Company in 1912, and was returning to England via the East Indies, when he contracted typhoid atSingapore and died at Colombo.Years later some old papers were being turned out in the office where he had worked, when a set of M.S. notes in his unmistakeable handwriting came to light. On examination these appeared to be the rough draft of a paper he must have beenpreparing for the local branch of the Institute, which he had not been able to complete. The notes were very rough. Apparently it was the author's intention to fill in numerous gaps with extracts from sundry published articles on Cornish mining; theextracts were missing and only the written portions linking them together were there. They give a sketch of Cornish mining practice which those who wish may supplement (as the author would have done) from Le Neve Foster and other well-knownsources. The fragmental notes which have been pieced together as carefully as possible by Mr B G Patterson, Hon Secretary Mount Morgan Branch; are as follows :-The first mine the author worked at was Carn Brea, in 1904."This is one of the largest tin mines in Cornwall, and belongs to the Carn Brea and Tincroft Mining Co, which also owns Cooks Kitchen. The only reported mineral production in 1977 was small output of alluvial gold, valued at an estimated $54,320.2 In addition, marine shells are gathered for use in lime manufacture, although output was not reported. Proposed development of a bauxite mine and smelter, involving Rennell and Vaghena Islands, was deferred in June. Conzinc Riotinto of Australia Ltd. (CRA), in partnership with Mitsui Mining & Smelting Co. Ltd., and the Solomon Government, with- drew from the project, and Mitsui, which conducted a feasibility study and metallurgical testing of the bauxite, decided to defer further work because of slack demand for aluminum in Japan and difficulties in arranging financing. The Mitsui feasibility studies reportedly indicated that the project could be viable despite technical difficulties and relatively high initial investment costs. The project was to produce 1.5 million tons of bauxite and 600,000 tons of alumina per year from reserves totaling 60 million tans at 45% to 50% A1203.

It is said by some that the occurrence of a deposit of sulphide of antimony in Southwestern Arkansas has been known for fifteen or twenty years. Whether or not such is the case I am not prepared to say, but I am not able to find any record of such a discovery, or any analysis of antimony ore obtained from that section, published at so early a period. The present discoveries, that seem to be attracting some attention, date back to the winter of 1873-74. At that time a vein of ore was found by Mr. Robert Wolf, in honor of whom the vein was named, and still is called, "the Wolf lode." At the time of the discovery the ore was unknown to the prospector, and its composition was not thoroughly understood in that locality until samples were sent to several analytical chemists, who pronounced it sulphide of antimony—stibnite. Since the discovery of this ore there have been three mines opened, and a number of interesting minerals of the antimonial series have been found. I have had an unusually good opportunity to collect specimens from these mines; and it is owing to the constant inquiry concerning the character of these ores that I have collected for publication the results of quite a number of analyses, sufficient I trust to clearly point out their composition. This is one of the few localities in the United States where antimony ore is found in workable quantities ; I say workable quantities, because two different shipments of ore have been made to English reduction works, and in both cases excellent returns were made. The discovery of these deposits will, it is hoped, give rise to a prosperous and remunerative industry at no distant day, and when fully developed they may yield ore in sufficient quantity to supply the regulus for the home demand, thus adding wealth not only to those interested, but also to the State of Arkansas. The three mines are situated in the northern part of Sevier County, and may be reached by the St. Louis, Iron Mountain and Southern Railway to Fulton, thence by wagon-road to Columbus, Nashville, Centre Point, and to the mines. Previous analyses of small samples of mining projects have found that feasibility studies tend to well under-estimate the actual capital costs of the project. Our review of 63 mining and smelting projects completed over 4 decades confirms that as-built capital costs are, on average, 25% higher than as estimated at the bankable feasibility study stage. There appears to be little attenuation over time of this bias in capital cost estimation, at first appearing to reflect an absence of learning on the part of the consulting engineer. We instead argue that this persistence of bias is intentional, driven by a scarcity of project financing and the need to inflate the project economics in a bid to secure financing. Around this intentional bias is a considerable amount of scatter. Roughly half of all projects’ as-built capita costs fall outside of the expected ±15% of the feasibility study capital cost estimate, even after allowing for intentional estimation bias. Cost overruns of 100% or more happen in roughly 1 out of 13 projects. The data reveal that a shifted lognormal probability distribution should be used when modeling mining project capital costs in a Monte Carlo analysis.
