Mineworkers face a challenging and dynamic work environment every workday. To maintain a safe workplace, mineworkers must be able to recognize worksite hazards while they perform their jobs. Though hazard recognition is a critical skill, recent research from the National Institute for Occupational Safety and Health (NIOSH) indicates that mineworkers fail to identify a significant number of hazards. To further the understanding of mineworkers’ hazard recognition ability and to begin to address hazard recognition performance, NIOSH researchers analyzed data collected during a laboratory research study to determine the effect of individual mineworker factors including risk attitude, work experience, and safety training on hazard recognition accuracy. The results of this study show that mineworker risk attitude and safety-specific work experience affect hazard recognition performance while hazard-specific safety training does not. These results suggest that some of these individual factors can be overcome through experience and training. Potential strategies that can be used to address these factors are also discussed.

"INTRODUCTION The future of oil production in the part of the Appalachian region included in southwestern Pennsylvania; West Virginia, and southeastern Ohio is directly dependent on the maintenance of existing wells. This report presents an analytical study of the methods used in oil-well reconditioning in this area. The study is part of a research program designed to evaluate those means of stimulating oil production that can effectively and economically maintain present oil-production rates and make it possible to keep existing wells in operation. Well-reconditioning work was curtailed during the war; therefore, an abnormally large number of wells in the Appalachian Region is now in need of repair. Because the lubricant content of Appalachian crude: oils is high and the refined lubricating oils an; particularly suited to use in high speed, high-temperature engines, the demand for these crude oils during the war exceeds, the maximum rate of production obtainable under wartime operating conditions. Increases in operating, expenses, inability to meet war-time reconditioning costs, and inflexible price structures caused the abandonment of an alarming number of wells during the past 4 or 5 years.As a means of increasing regional oil-production rates, new drilling in the older fields for primary production generally is uneconomical because reservoir pressures in most of the fields have virtually been depleted and the production rates of newly drilled wells are small."

[The Bureau of Mines selected Anahuac for a petroleum-engineering study because it is one of the notable fields of the Gulf Coast region. It is outstanding because the producing formation is very thick; ever modern technique of drilling' and development has been applied; complete records of all operations,, including detailed subsurface information have been kept; the field has ,been developed systematically; and every effort has been made to utilize the reservoir energy efficiently. Data and information pertaining to the development anal operation were collected, compiled, and studied by the writers as if the field were under single ownership. The chief aim of the study was to increase the accuracy of the existing methods of estimating reserves by applying all the data collected during the modern development of an oil field, These scientific aids include coring; electric logging; micropaleontology determinations of porosity, permeability of oil-bearing sands, and their connate water content; subsurface sampling of the reservoir oil; and depth-pressure measurements. Because the reservoir structure in the Anahuac field is extremely complex, special attention was given to methods of calculating the reservoir volume.]

Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are improving but more slowly and yet, serious accidents and fatality rates are not: in fact, these rates of more serious incidents have plateaued or deteriorating. In more dramatic cases, organizations that had ?exemplary? safety statistics, suddenly had a catastrophic or multi-fatality event occurring. Popular books such as In Search of Excellence of Peters and Waterman or Good to Great of Jim Collins created many delusional ?secrets? or recipes, to achieve great performance and yet, most of those ?great? or ?excellent? companies have now declined in their performance, some dramatically and some have fallen from grace, like Fanny Mae, a company Jim Collins identified as a ?great? one, that had to be bailed out in the latest financial crisis. These organizations share several ?delusions? about managing a business, and often, these delusions are created by academics, consultants and authors of popular books about (safety) management. More damaging however is that safety management is defined by a set of key practices or axioms: a focus on human error, compliance to rules and regulations, creating consistency of systems and actions, reacting to accidents and incidents and on prevention of those, measuring safety with performance metrics and rewarding employees on those measurements. All of these axioms contain the seeds of destruction, and they create conditions and dynamics that precede the disaster - and ironically, they are all 'good things' we do in safety management. For example, a mere focus on safety in the business, and the measurement of it, create a culture of repression of mistakes and incidents, leading to a situation where the organization knows incrementally less and less of risks and risktaking behaviour - until the day it all coincides destructively! This paper discusses seven such safety delusions and how they impact on the views and safety management approaches. They are: ??The delusion of (risk) control, that risks can actually be identified, analyzed and mitigated. ??The delusion of compliance, that mere compliance to rules will safeguard the business ??The delusion of consistency, that standard operating procedures will ensure safe operations ??The delusion of predictability, that accidents and trends tell us everything about the risks of the future ??The delusion of human error, that people are the weakest link and that it can actually be dealt with through discipline and enforcement. ??The delusion of statistical trends, that safety is measurable with statistics ??The delusion of invulnerability, that we have achieved high levels integrity and capability to deal wit risks. Organizations that suffered catastrophes all share some or all of these delusions and this paper traces how these played a role in many disasters, ranging from the Titanic, Challenger, Piper Alpha, Bhopal, BP Deepwater Horizon and many more.

A general discussion of the structure of the junk automobile recycling system shows that the generation of stripped automobile hulks and scrapped automobiles is price inelastic. As a result, it is extremely difficult for the price mechanism to automatically achieve an equilibrium that will prevent the accumulation of excessive automobile wrecker inventories and vehicle abandonment by the last owners of scrapped cars. The new shredder processing technology and the new portable flattener technology have altered the structure of the junk automobile recycling system. The shredder technology has increased the demand for processed automobile scrap. This increased demand is reflected in higher prices paid for hulks by scrap processors. The portable flattener has reduced the transportation cost of shipping hulks to processors. The two new technologies have eliminated, at least potentially, the net cost of shipping stripped hulks so frequently incurred by wreckers in the past. The elimination of this cost eliminates the economic incentive for excessive wrecker inventories. Unfortunately, the incentive for last owners to abandon their cars does not appear to have been eliminated by the new technologies. The last owner of an automobile with no spare parts value is still likely to incur a cost by delivering his automobile to the recycling system. Although a subsidy payment is probably unnecessary to reduce excessive wrecker inventories, such a payment might be one solution to the problem of automobile abandonment. However, there are a wide variety of subsidy programs that could be introduced. Before any Federal subsidy program is introduced to reduce automobile abandonment, it is recommended that the program be tested on a limited basis in many different parts of the country.

In order to produce a more saleable product and to improve operating and maintenance conditions, it is necessary to depress the water table below the mining level. Considerable success has been achieved by drilling deep wells in strategic locations and installing vertical tur-bine pumps at depths of up to 400 feet. A case study is presented of the mechanical behaviour of fractures in the Westphalian A and B siliciclastic succession of mud-, silt- and sandstones with intercalating coal layers in the Campine basin of northeast Belgium. Mining ceased in the area in 1992 and a pilot well was drilled to assess the feasibility of methane extraction by hydraulic fracturing. Work was carried out to study the influence of types of cement fill in fractures on the shear behaviour of the rocks by comparison with that of the coal seams. X-ray computerised tomography was used for visual appraisal and selection of jointed rocks before destructive shear testing was carried out. Two major type of veins were found to be present, one cemented with carbonate and the other with kaolinite or kaolinite/carbonate. Uniaxial compressive strength and shear tests showed that differentiation of the two types of veins was vital in evaluating the strength of the sandstones, as cementing of the fractures by rigid carbonate minerals can reinstate the strength of the rock masses to that of the intact material. Kaolinite was more susceptible to plastic deformation than carbonates

"The Cortez SAG mill liner design was recently modified to improve the mill operating energy efficiency. In addition to the expected benefits, the design change unexpectedly played a significant role of verifying the proposed grinding circuit expansion for the Cortez Hills project as well as illustrating current SAG mill bottlenecks. This paper will discuss the evaluation that addressed operator and maintenance observations, historical PLC data, and “crash stop” observations to calculate charge motion, packing between lifters, and slurry transport through grate and pulp lifters. The performance of the new shell and discharge end liner design will also be discussed as well as their role in the possible grinding circuit expansion.INTRODUCTIONCortez Gold Mines commenced operation in 1969 with its Mill #1 rated at a nominal 2,000 tons per day. Mill #1 was a conventional rod mill/ball mill circuit followed by an 8-stage CIP circuit. In the early 1990s the Pipeline deposit was discovered and subsequently Mill #2 was built to process the increased ore volume. Mill #2, commissioned by mid-1997, was a nominal 10,000 tons per day SABC circuit followed by an 8-stage CIL circuit for oxide gold ore. Operating and commissioning details of this plant have been published elsewhere (Powell, 1998). By late 1998, there was not enough ore volume to support both mills so the smaller, higher operating cost Mill #1 was put under its current care-and-maintenance status. A simplified flowsheet of the current mill is shown in Figure 1."

Development during 1942 led to the discovery of 26 new fields in the Texas Gulf Coast, as compared with 27 during 1941. Of these, 19 are classed as oil fields, 4 distillate fields, and 3 gas fields. Production from the new fields amounted to 614,467 bbl. to October 1942. Producing Sections.—Discoveries in I942, compared with the previous two years, classified by producing formations, are shown in the following table: It is interesting to note the increase in Wilcox discoveries due to the extensive geophysical campaign that has been con. ducted since 1940. Drilling.—During the year 688 wells were drilled, of which 574 produced oil or gas and 114 were dry. This represents a considerable decline in activity over the previous year, when 1405 wells were drilled in the area, of which 1081 were producers and 324 failures. Jackson County continued to hold the lead, with 222 wells completed. Production.—-Despite curtailed drilling, production amounted to 118,009,451 bbl. for 1942, indicating a slight increase over the 113,726,893 bbl. produced during 1941. The statistics in Tables I and 2 include condensate and distillate as oil production, because many Gulf Coast fields produce oils with a wide variation of specific gravities, all of which are commingled in pipe lines. Table 3 shows by counties most of the activities referred to. The number of wells drilled includes only field wells; not wildcats. Outstanding Fields of 1942 The Texas Gulf Coast covered in this report has 270 more or less active oil and gas fields, including the new discoveries of 1942. Of these the most outstanding from the standpoint of production are listed in Table 4. Each of these fields produced one million barrels or more during 1942. Some of the fields where new extensions are occasionally referred to as separate fields, such as West Columbia and New West Columbia, or Thompson and South Thompson, are combined to give a more accurate outline. Two interesting facts may be noted from this list of some 26 oil fields: (I) that the Lolita field, Jackson County, is the only discovery since 1938 to produce more than one million barrels annually, although there have been 79 discoveries since Jan. The purposes of the research were to assess the applicability of new accident investigation methods to mining accidents, and to adapt those found to be applicable in the development of a new methodology for use in mining. The development included field testing, evaluation and modification of the new methodology. The report is in two volumes. This, volume one, discusses primarily the Phase I work: background research, design of a methodology for field evaluation, and preliminary field tests in Utah mines. Phase I was completed in June, 1980. Also discussed are some activities to prepare for a field evaluation during the period May 1980 -October 1981. Volume 2 discusses the subsequent field tests, modifications of the initial methodology and the results of use of the methodology in two mining companies for a period of approximately eighteen months, and in other companies for selected loss incidents during that period.

This study provides recommendations for improved safety in underground blasting procedures based on in-mine observations of blasting practices in 30 underground metal, non-metal, and coal mines, and the analysis of blasting accidents since 1970 and existing state and federal regulations pertaining to the storage and use of explosives. Results of these analyses and observations indicate that 1) blasting accidents have declined but are an important contributor to the hazards of underground mining; 2) existing regulations are essentially adequate; 3) greater emphases should be placed on human errors and unsafe practices and the enforcement of existing regulations; and 4) future revisions of these regulations and standards should be organized, and their intent coordinated to eliminate ambiguities.

The studies previously made on Smelting in the Lead Blast Furnace have dealt with the custom smelter of the International Smelting Co. at Tooele, Utah, and on the smelter of the Bunker Hill & Sullivan Mining & Smelting Co. at Kellogg, Idaho. The International plant may be considered representative of the traditional custom smelter. The lead smelter of the future seemingly will have to treat increasingly richer lead charges. This feature was then stressed in describing the Bunker Hill plant.

Until recently, the fire protection systems used to protect mining equipment were based on generically designed, off the- shelf concepts. These systems were reliant on single agent approaches and were generally comprised of a series of small twenty or thirty pound chemical canisters that would discharge their contents when triggered by thermal detection means. As the equipment grew in capacity, the logic used to increase the protection was to add more chemical canisters to the system. Hydraulic excavators and large-scale mining trucks grew dramatically in size into the 1990?s and beyond; with this growth came the increase in major losses due to fire. This was evidenced throughout this period, and continues to be of great concern today. During this presentation you will learn of the significant changes that have come about as a result of this history. Today, dual-agent products, infrared detection and unit-specific, engineered systems are the primary phrases used when discussing how to best protect this equipment from fire loss. This session is a must for anybody looking to optimally protect their equipment assets and for those looking to renew and/or secure new insurance for high value equipment in the future.

O2, NaCl03, H202, NaOCI and Ce+4 have been examined as oxidants in an alkaline carbonate system for uranium in situ leaching. The South Texas and New Mexico ores contained up to 2.6 meq/g of reducing compounds that consume oxidants in the leaching operation. Leaching rates did not increase linearly with the oxidation potential of the oxidants. A mild oxidant can be used to maximize uranium oxidation selectivity and oxidant efficiency. 02 can oxidize most of the uranium mineral with an adequate rate and high oxidant efficiency.

[The Mining and Metallurgical Divisions of the Bureau of Mines, Region V, conducted an investigation in 1950-51 of the iron sulfide deposits in Aitkin and Carlton Counties, Minn. Twelve diamond-core-drill holes, aggregating 2,347 feet, were drilled in secs. 14 and 15, T. 46 N., R. 25 W., Aitkin County, to increase reserves of iron sulfides known to exist 2 miles southwest in secs. 28 and 29, T. 46 N., R. 25 W. Three diamond-core-drill hobs, totaling 561 feet, were drilled in Canton County, sec. 7, T. 46 N., R. 2 0 W., to check reports of the presence of a large sulfide deposit in that area. Mineral-dressing tests were made on cores from 10 holes drilled in Aitkin County. These included core from hole 21 drilled in sec. 28 and submitted by the Glen Development Co.]

"""Rock-dust barriers""5 as they are termed in this country, are for the purpose of confining a coal-dust mine explosion to the limited area in which it originates. These barriers were first tested in 1910-1911, with favorable results under the conditions of test, at the French official gallery which was then at Lieven, Department of Pas-de-Calais. Shortly afterwards the Bureau of Mines in the course of its investigation of all practical explosion-prevention methods tried barriers in the Experimental Mine near Bruceton, using the primary form of simple fixed cross shelves 20 inches wide extending over the passageway and laden with rock-dust. The Bureau termed them “Taffanel barriers"" in honor of the director of the Lieven testing station. The trials were made in conjunction with tests of explosibility of different coal-dusts.Experience showed that although the principle of barriers had merit when they were used for secondary defence, the primary form was uncertain in effectiveness. This led the senior author to design mechanically discharging barriers to be operated by the compression air waves which precede the flame of an explosion. For the better understanding of the principal involved, it is pointed out that the advance air waves are what make a coal-dust explosion possible by stirring up the coal-dust from the walls and floor of the passageway. If a barrier can throw inert dust into the air to form a cloud as dense or denser than that formed by the coal-dust, it will neutralize the explosiveness of the coal-dust, and the flame of the explosion will be extinguished. When the flame ceases, the pressure dies away in a short distance if the 'area that had been traversed by the explosion is not large."

"Sillimanite, kyanite, and andalusite are aluminum silicates having the same chemical composition (Al2Si05) and are used in the ceramic industry for the manufacture of high-grade refractories. ""Natural sillimanite with added corundum or other alumina to just short of mullite 2SiO2) proportions is unequalled for jobs where strength at high temperatures, resistance to spalling, and resistance to slagging by acid slags is desired. High-grade porcelain ware also requires these minerals.""At the same cost, sillimanite would be preferred for most uses, but the lack of any large source of sillimanite has given economic preference to kyanite or andalusite for special refractories and high-grade porcelain ware.""3/The possible economic value of the sillimanite schist in the piedmont Plateau was first recognized by Dr. L. L. Smith, of the University of South Carolina, who published an article in the Engineering and Mining Journal, August 1943, on an occurrence of sillimanite schist in South Carolina.The Bureau of Mines was interested in developing a large source of sillimanite to relieve the shortage of high-grade refractories, and initiated a project to locate the ore-bearing areas in Georgia and South Carolina and to develop the most favorable ones.Two hundred and seventy-eight outcrops of sillimanite-bearing schists were found by a Bureau of Mines engineer.4/ The schist occurs irregularly throughout a belt extending from Talbot County, Georgia; to Spartanburg County, South Carolina, about 220•miIes long and approximately 15 miles in maximum width. The Bureau drilled four areas of the more coarsely crystallized schist in Georgia and South Carolina:In its program of exploration of mineral deposits, the Bureau of Mines has as its primary objective the more effective utilization of our mineral resources to the end that they make the greatest possible contribution to national security and economy. It is the policy of the Bureau to publish the facts developed by each exploratory project as soon as practicable after its conclusion. The Mining Branch, Lowell B. Moon; chief; conducts preliminary examinations, performs the actual exploratory work; and prepares the final report. The Metallurgical Branch;.R. G. Knickerbocker; chief, analyzes samples and performs beneficiation tests. Both these branches are under the supervision of Dr. R. S. Dean, assistant director."
