The relative infrequency of fatalities and other serious events can give them an appearance of being random, of being beyond any reasonable degree of anticipation and prevention. In fact, the vast majority of these events result from exposures that are identifiable, measurable, and manageable. The lessons of prominent incidents, such as the Space Shuttle Columbia, OxyÆs Piper Alpha, Esso Longford, BP Texas City, Massey Mine, as well as lessons from single fatality events, is that alongside the proximate causes of each incident, there also coexisted an underlying fabric of systems, mechanisms, and culture that allowed risk in the workplace to persist and often to become normalised. All of these ingredients of the organisationÆs safety fabric lend themselves to the influence and intervention of senior leaders. The difficulty is that traditional injury prevention paradigms are not sufficient for addressing the precursors that lead up to catastrophic and fatal events. A recent study of such events, led by Dr Thomas Krause concluded that the descriptive aspect of HeinrichÆs safety triangle is useful, but that the predictive aspect is not. The study surfaced reasons why a reduction in less serious injuries does not necessarily correspond to a proportionate reduction in catastrophic and fatal risks. This presentation expands on the findings to include the concept of exposure to the event potential linked to precursor events and the framework for leading an organisation to be both cognisant and effective in eliminating such events.CITATION:Russell, R, 2012. Since 2019, the mine planning team at Olympic Dam has been establishing and maturing a shift from deterministic to probabilistic scheduling techniques. This paper outlines this journey undertaken by the mine planning team, key learnings from implementation and further work required to increase the level of planning maturity. Olympic Dam (OD) is a highly variable, polymetallic orebody with a vertically integrated value chain from operations to final product. The processing plant has a tight set of quality constraints which are managed through timing of stopes from varying parts of the orebody. Value is eroded for the operation when forecasted quality of the mine feed encounters disruptions from unplanned changes to the timing of stope starts. As production has moved from the historic Northern Mining Area into the more geologically complex Southern Mining Area, average stope size has reduced, resulting in increased mining fronts required to meet production targets. This has created a problem of increased schedule complexity and difficulties in identifying and communicating resource priorities within a large set of individual stope critical paths. The problem was approached by considering each stope as an individual project constructed by teams working across multiple projects at any one time. The operation could then be considered a multi-project environment with shared resources and enables the application of Critical Chain Project Management (CCPM) to the mine plan. Key to CCPM is the addition of time buffers to protect the critical path of a project form upstream or downstream processes. Implementation of time buffers protects the stope start date against compliance to plan variance. The monitoring of each time buffer consumption, as a proportion of total construction activities completed enables proactive prioritisation of resources. This resulted in an increase in the number of stopes starting in accordance with the mine plan, simplified communication of priorities to all levels of the operation, increased compliance to plan and a clear measure of schedule risk during planning routines.

The paper deals with the problem of managing the supplies of consumables (energy resources, lubricants, explosives, explosive supplies, tires, spare parts, etc) in mines. After the introductory remarks and comments, a general stochastic model of supply, relating in general to the dynamic programming method is presented. A selection of stochastic supply model for experiments and research by the authors of this paper is not a coincidence, based on of their long experience, showing that this type of model is the most suitable to provide support for decision-making and management of supplies of consumables in mines.In order for the problem?of?consumables?supply?management to be reviewed, in whole and objectively from the aspect of risk, experimental tests of the stochastic model in real conditions were performed at the limestone open pit mine æZelenikovecÆ near Skopje, the capital of Macedonia. The scope of the experimental research enabled reliable analysis. The results obtained confirm the efficiency and reliability of the stochastic model and two separate manipulative-operational positions regarding its application, which may have an important influence on the optimisation of supplies.

"Predicting how coal seams will be beneficiated through the coal handling processing plant (CHPP) has long been challenging within BHP Coal. Various coal seams from different areas are mined and stockpiled separately, often depending upon a deleterious coal quality parameter; high/low trace elements for example. These stockpiles are then fed in different ratios into the CHPP to produce the desired product/s. In understanding the washability characteristics of the feed source, mine geologists and process engineers work together to improve the beneficiation of different coal materials through the CHPP.For the majority of the BHP Coal deposits, the washability data is available for the coal seams across the deposits. However, no previous standardised modelling and prediction technique existed that could ensure the correct information was available for forecasting and refining CHPP processes. This paper will discuss the steps undertaken to implement a geometallurgical prediction tool that simplified and streamlined the washability modelling and yield estimation process. The value realised to the BHP Coal assets resulting from this process was: improved compliance to plan increased throughput increased CHPP recovery through improved efficiencies and increased revenues.CITATION:Handley, P J, 2017. Precast concrete segmental linings have enjoyed widespread use around the world, and are the lining type of choice for tunnels constructed using a shielded tunnel boring machine. However, there is significant variation in the design of such linings from one country to the next in such areas as materials, geometry, fixtures and fittings, waterproofing and reinforcement to name but a few. This arises because the speed at which the technology has developed within some countries and regions has sometimes outstripped the transfer of such technologies to the rest of the world.This paper provides some insights into the variations in lining design in different parts of the world with the aim of providing designers and constructors alike a reference for the different methods adopted. It provides a context for these developments by briefly summarising historical reviews of this subject before looking at some of the developments around the world over the last 15–20 years. The developments are presented with some reference to the reasons for implementation, and the advantages and limitations commented on. Low-grade manganese ore from Sandur, India containing 28.2 per cent manganese and 20.9 per cent iron was roasted with sulphating agents, viz ammonium sulphate and sulphuric acid. The effects of temperature of roasting (300 - 800°C), time (15 - 120 minutes) and amount of sulphating agent (0.5 to 2.5 times stoichiometric amount) were studied. Under optimum conditions, it was possible to extract 92 per cent manganese with 0.5 per cent iron at a temperature of 700°C for 90 minutes duration with stoichiometric ammonium sulphate. Further increase in quantity of ammonium sulphate has marginal effect on manganese extraction.Similar studies were carried out with sulphuric acid, and an extraction of about 87 per cent manganese with similar iron contamination was achieved at a temperature of 700°C for 60 minutes duration with stoichiometric quantity of the reagent. Selective extraction of manganese with respect to iron is discussed.

Research into the dynamic properties of ground support elements continues to grow at a global level, given the increasing importance of this field of application within underground hard rock mining. Measuring the mechanical response of such elements under the rapid onset of simulated rock burst loads forms the basis for this experimental work, typically utilising gravitational drop testing methods. This field of research is fundamentally driven by an increasing need for dynamic performance data, given the constant evolution of rock bolting product designs, materials selection, combination testing and specific application needs based on ground types and loading scenarios. A smaller number of globally recognised dynamic test facilities have predominantly lead research within this arena, typically in association with academic institutes, larger industry groups and government research institutes. However, due to the increasing requirement for dynamic performance data, additional facilities have now emerged in order to satisfy the overall demand for test work. These newer facilities are more typically associated with suppliers to the underground hard rock industry, with experimental designs looking to replicate, at least in part, the work of the primary facilities. For standardisation, repeatability and calibration purposes, the cross validation of test data between primary and secondary facilities becomes an obvious and important consideration across this field of research. This paper outlines the development of a new dynamic drop test facility for testing rock bolts, including the structural design and associated high speed data capture system. Based on the utilisation of identical bolt types, material grades, anchoring systems and decoupled ‘free-lengths’, comparative data has been generated to provide cross validation of the new test facility against data generated by a globally recognised primary facility. The cross validation of data provides increasing confidence in the repeatability and accuracy of the new facility as activity continues towards the provision of further experimental test work.

This paper outlines the journey of cultural maturation and management system development at the Mt Rawdon (MRO) gold mine from its acquisition by Evolution Mining Limited (Evolution) in late 2011 to 2016.Evolution embarked upon a series of projects that aimed to improve the operational consistency and implementation of change on-site, while leveraging off the existing strengths of the operation. This document describes the gaps that were identified, the processes used to close them, and the resulting cultural change and production outcomes, with a specific focus on how to optimise the culture at any mining operation.MRO had a history of four owners in less than three years when it became a key asset of Evolution. The site leadership team (SLT) decided to influence the culture at MRO by reviewing and upgrading the existing management processes.The first priority was to improve transparency and collaboration at the SLT level, followed by improving competency in project management. The next phase was to strengthen the internal communication using a deliberately designed network of meetings, communication screens and other initiatives. Once there was some traction, the focus moved toward increasing production volume and decreasing cost per unit by improving short-term planning and execution capability. Together with other initiatives, this resulted in improved collaboration between all functions, which in turn netted over 37 per cent mining production uplift.Following solid success, a process of prioritisation and alignment management was implemented. This supported the ‘Mine to Mill one operation’ ideal and allowed the SLT to decide month-by-month what actions were to be prioritised. These priorities incorporated production realities and longer term imperatives of the balanced business plan.Much of this work was completed during a period of declining gold prices and a relentlessly strong Australian dollar, making it difficult to justify spending money on improvement projects. The importance of proper planning, preparation, and execution of a steady state sampling campaign for metallurgical plant assessment must not be underestimated. Careful planning can increase the amount and reliability of information from an optimum number of samples and fully utilize available plant data. It requires specification of the data required, determination of run duration sampling frequency, the selection, modification and testing of sample points and cutters, as well as organisation and briefing of plant and sampling personnel. Preparation immediately prior to sampling, the conduct of the sampling campaign itself and procedures to handle the collected samples and information are discussed. Cutter design, fabrication and use are described in detail, and typical sampling situations, for ball mills, classifiers, pipes, launders, flotation cells and thickeners are considered individually. Measurement of pulp flow rate can often resolve difficult mass balance problems, and discussion of convenient techniques is included.

The concept of Big Data refers to data sets that are so large, variable and/or complex that traditional data processing applications struggle to integrate the data for effective investigation, analysis and value generation. With advances in sensor technology and smarter equipment, the mining industry is experiencing Big Data, and in a different way to how the intelligence and marketing communities experience Big Data. Future mining will bring larger, more complicated and diverse data sets increasing the strain on traditional data integration and investigation techniques. Block caving is an important mining method for the extraction of massive, low-grade ore deposits due to its low operating cost and high production potential, given the right orebody geometry and ground conditions. This paper focuses on block caving due to its increasing popularity and importance in the future economic extraction of multiple commodities. The block cave mining system (BCMS) is a complex interrelation of factors, and due to the reliance on natural processes within the system, it is one of the least understood mining methods. Obtaining information on the factors within the BCMS generates large, complex data sets on various time scales. Each sensor network within the BCMS combines spatial and temporal information with tens to hundreds of recorded parameters. Data sets are quickly moving from multidimensional to n-dimensional. Optimisation of the BCMS is only possible through a holistic analysis approach, thus involving the integration of multiple, diverse, n-dimensional data sets (Big Data). This paper presents a current example BCMS Big Data situation involving 12 sources of information with differences in volume, data format and frequency of data generation. This suitability of virtual reality scientific visualisation (VRSV) as a solution to BCMS Big Data interpretation difficulties is then discussed. These 12 data sets can be effectively integrated using VRSV for intuitive and accelerated investigation of the BCMS. Potential future Big Data within the BCMS is presented involving 20 sources of information resulting from our increased need for monitoring and advancements in technology. Current methods of defining queries may not be suitable within these future complex, multidimensional data sets, and the potential of artificial intelligence algorithms within this system is discussed. These advancements in technology and machine learning lead to the requirement of live data acquisition and analysis. It is estimated that in less than a decade, high-grade iron ore reserves in South Africa will be depleted, leaving only low-grade ores with a dominant lithology of banded iron formation (BIF) hematite. BIF is a complex ore reserve as it is typically liberated below 1 mm, resulting in high energy costs. However, due to its abundance, its beneficiation potential should be evaluated to maximise recovery at an acceptable product grade. To develop a beneficiation program for BIF hematite, the first step is to determine the effect of two laboratory crushing techniques on mineralogical characteristics, specifically liberation and shape. For this study, the cone crusher and high pressure grinding rolls (HPGR) were used. The Electron MicroProbe Analysis (EMPA) and Mineral Liberation Analyser (MLA) were used to compare the mineralogical characteristics. The liberation characteristics for hematite are similar for both crushing techniques; however, HPGR preferentially liberates quartz. HPGR achieved 8– 12 per cent additional quartz liberation in the greater than 80 per cent and 100 per cent liberation classes respectively and specifically in +75 μm size range. Improved quartz liberation is associated with lower energy requirements for downstream milling and improved separation efficiencies for beneficiation units that operate in this size range. Both samples displayed similar shape characteristics; most particles were elongated smooth and the rest were elongated angular. Good hematite liberation is expected at grinds of 212 μm to 150 μm. Further consideration should be given to throughputs, energy efficiency and downstream equipment selection prior to selecting a crushing technique.

The explosives demolition of a post-stressed, steel reinforced, concrete road bridge in Australia’s capital city, Canberra. The bridge was 50 m from the new replacement bridge, 320 m from community structures and within five kilometres of the national parliament buildings. The job required multiple overlays of risk assessment, public relations, safe work procedures, specialised blast design, environmental predictions and flyrock controls. Project stakeholders included Worksafe, Roads ACT, bridge owners, consulting structural engineers, community groups and road users. The bridge precincts, including a major road underneath, had to remain operational to peak traffic in the days leading up to the demolition. The break-up and removal of debris had to be executed in just two days from Friday to have the road beneath re-opened for Monday morning city-bound traffic.In Canberra, all explosives demolitions are now viewed with particular caution after a demolition in 1997, during which a young girl was killed. Any approval requires that Explosives Regulations, AS2187 2006 and Worksafe processes are satisfied, and a convincing case presented for blasting as a safe and viable option.Our design used specialised shaped cutting and blasting charges to sever in situ concrete beams and tendons and to shatter components of the stressed concrete bridge assembly. We include comments on structural engineering reports dealing with pre-weakening mechanisms. The benefit of pre-weakening is that it reduces the amount of explosives needed to safely complete the job and facilitates quick removal of debris after the shot. The complementary skills of the team, each one an expert in his field, resulted in a highly successful demolition, praised by authorities as an outstanding example of controlled use of explosives.

The Albany Conglomerate is a widespread geological unit found in northern areas of Auckland, New Zealand. It has previously been described as part of the early Miocene Waitemata Group which underlies most of Auckland. Much of the Waitemata Group comprises weak interbedded sandstones and mudstones, but also includes conspicuous conglomeratic units such as the Albany Conglomerate. The Albany Conglomerate comprises a well-cemented mixture of hard pebbles, cobbles and boulders of igneous and metamorphic derivation in lenticular beds. A number of tunnelling projects are planned for the near future in Auckland. Recent experience has shown that the variable features associated with the Albany Conglomerate can make selecting appropriate construction methods very challenging. This paper explores the engineering geological and sedimentological features of the Albany Conglomerate with respect to other conglomerates within the Waitemata Group and the potential implications on tunnelling projects in Auckland. The Kwinana (Western Australia) and Gladstone (Queensland) industrial areas are among the few dozens of heavy industrial areas globally that demonstrate significant levels of regional resource synergies. Such synergies are also referred to as æindustrial ecologyÆ or æindustrial symbiosisÆ. Fundamentally they involve the exchange of by-products or sharing of utilities between companies in close geographic proximity to achieve competitive advantage and environmental benefit. A concerted research effort is currently underway to foster greater realisation of regional resource synergies in minerals processing intensive regions, in Australia and internationally. This paper summarises current insights from the foundation research into engineering tools and technologies and enabling mechanisms for regional resource synergy developments. Regional resource synergy developments were reviewed for 16 international and two Australian regions. It was found that Kwinana and Gladstone compare favourably with the well-regarded international examples of regional synergy development. Given the fairly strong evidence that self-organisation is critical for regional synergy developments, an implementation model has been proposed that captures entry points for synergy developments at the level of platform (industry collaboration), process (developing synergy opportunities) and project (assessing and implementing specific synergies). It was also found that regional synergies have so far developed in the absence of specific methods for identification and screening of synergy opportunities. Moreover, very limited attention has been paid to the associated technological and engineering challenges. The current technical research effort therefore targets: the development of a toolset for the identification and development of potential synergy opportunities; and assessment of technology needs and opportunities for regional synergies involving water, energy and materials recovery and reuse.

OneSteel Whyalla’s grate-kiln-cooler pelletising plant was commissioned in 1968 processing haematite feed from the local South Middleback Ranges mining area. The existence of a magnetite resource beneath the haematite ore at Iron Duke had been known for some time. In 2004, a decision was made to covert the primary iron source for the Whyalla steelworks from haematite to magnetite – known as project magnet. The intent of the project was to convert the feed to the steelworks from haematite to magnetite and to free up the remaining haematite resource for export. The pelletising plant was cut over to a magnetite feed in 2007.The thermochemical differences of magnetite compared to haematite are threefold:1. the introduction of an exothermic reaction during conversion of magnetite to haematite,2. the absence of bound water which existed within the haematite, and3. lower gangue content requiring lower fluxing levels and therefore lower calcination energy demand.The combination of these differences results in a significantly different induration requirement for magnetite as compared to haematite. The change therefore required extensive research and test work to design modifications to the furnace and to ensure appropriate risk mitigation for the Whyalla steelworks.Preliminary pot-grate test work was performed to define the pellet chemistry and confirm the suitability of the magnetite to produce pellets suitable for the OneSteel blast furnace. A heating curve was developed by use of computer-based heat and mass balance modelling. Extensive pilot scale grate-kiln-cooler test work was then performed to confirm the heating curve and to verify and fine-tune the pellet properties. Computation fluid dynamics was used to design the physical changes to the furnace required to achieve the proposed heating curve.Following installation of an additional preheat zone, including a wall and dilution air fans, into the preheating furnace, and installation of an additional wall into the cooler, the modified furnace was commissioning in December 2007. The plant performed as expected during commissioning with the actual heating curve matching the proposed curve and the produced pellets within blast furnace specifications.

The recovery of copper from a flotation concentrate using ferric chloride leaching has been investigated. The concentrate from the Horseshoe Lights Mine near Meekatharra, Western Australia, contained mainly digenite, pyrite and talc minerals. Over 95 per cent of the copper was dissolved in an acidified ferric chloride solution in one hour at 70¦C and ambient pressure. The pyrite and gangue minerals did not react. The leaching process was found to consist of four separate reactions and the shrinking core model was used to describe the process. The effects of particle size, temperature, concentration of FeCls, stirring speed and leaching time on metal dissolution were investigated. The apparent activation energies in the second, third and fourth reactions of leaching of copper concentrate were 28.8, 11.9 and 53.9 kJ/mol, respectively. The optimum conditions for the leaching of copper from the concentrate, within the range of conditions studied were: particle size 63 per cent minus 38 ¦m, a ferric chloride concentration of 1.48 M, a temperature of 70¦C, and a stirring speed of 800 rpm.

The modern workplace has changed dramatically in recent years with an increased emphasis on the psychological well-being of employees. While adhering to a certain standard of psychosocial safety at work is now not only a moral, but legal obligation in Australia, it begs the question: how can organisations begin to ensure psychosocial safety? A positive psychosocial safety climate is one where employees feel they are psychologically safe, feel supported in their work and are not exposed to work-related stressors that can negatively affect their mental health. Such a climate can help boost productivity and create a more positive working environment. This presentation will provide a twopronged approach to understanding and managing psychosocial safety: pairing a traditional risk assessment with climate measurement. Risk assessments can provide several benefits, including identifying and characterising work-related hazards, identifying the nature and severity of the risks to employees and prioritising necessary interventions for reducing or preventing psychosocial harm in the workplace. However, these assessments can have certain limitations, especially when it comes to capturing some of the less obvious causes of psychosocial harm. Climate measurement can provide valuable insights into employees’ perceptions of the work environment, identify areas of concern that may not have been identified in the risk assessment and help to guide organisational changes that will enhance employees’ psychosocial well-being over time. It will also be argued that climate should be assessed from a broad perspective exploring general, physical, social and psychological aspects of safety climate. This presentation will explore why conducting risk assessments in conjunction with detailed climate measurements is integral to protecting the psychosocial safety of employees in the workplace and demonstrate why employers should view the two approaches as complementary and interconnected. Best practice recommendations will be provided for the risk assessment, climate measurement and psychosocial interventions

Following an underground coal mine explosion, it is important that ventilation is quickly restored to ventilate the mine and affect a mine rescue. To achieve this, surface ventilation fans must be suitably protected from explosion damage, but still able to be quickly restored.A review of all Australian coal mine regulations was carried out, revealing that Australia has no prescriptive regulations that specify the design of explosion relief panels and the protection of ventilation fans in the event of an underground explosion.To better define the level of protection required, this paper reviews and extracts details from relevant major international regulations and standards on the subject.A suitable design methodology for the protection of ventilation fans is presented that could be applied in Australia, using a combination of explosion relief panels, appropriate isolation of the fan, and ventilation shaft/mine design.CITATION:Rafferty, T and Bourcier, R, 2013. The introduction of large diameter drill holes and mass firings in underground metalliferous mines has led to an increase in the number of reported sulfide dust explosions in Australia and overseas. Adaptation of the coal mining technique of using limestone to inhibit or prevent dust explosions has not been entirely successful as the development and inhibition of a sulfide dust explosion is somewhat different from that of a coal dust explosion. Tests were carried out on two pyritic ore samples, sulfur contents of 41 and 36 per cent, in 8 and 20 litre explosion chambers to evaluate the effect of the addition of limestone on the maximum explosive pressures developed and rates of pressure rise. Significant values were obtained in both chambers with dusts containing 60 per cent ore and 40 per cent limestone. The percentages of limestone required to inhibit the pyritic dusts were considerably higher than those values generally accepted by the mining industry and the current methods used to inhibit sulfide dust explosions should be re-evaluated.
