The following charts were presented at the Randol Gold & Silver Forum 2000, held at the Hotel Vancouver (Vancouver, B.C.) from April 18-21, 2000. It must be borne in mind that the "facts" stated and con­clusions drawn by the author were not as the result of a rig­orous technical investigation, but rather as a member of a journalistic team seeking a rapid view for presentation as news to a TV audience. Since the author's visit, some retrenchment on the part of the operators on estimating the cyanide content of the spill seems to have occurred. This does NOT alter the conclusions that (a) the spill was an intolerable embarrassment for the world mining industry, nor (b) the potential damage to future mining investments, should prompt action not be taken: 1. to understand the reasons for the dam failure, and 2. to draw up firm concepts of how to avoid future fail­ures of dams at mining properties containing materials that are toxic, environmentally unfriendly, or with as highly dis­comfiting a profile as cyanide.

"In recent years the gold mining industry has been challenged by declining metal prices as well as having to treat ores that have low grades and are refractory in nature. Many factors must be considered when metallurgical studies are designed to develop process design criteria for these type of ores. These factors are mostly related to the mineralogical and chemical compositions of the ores, the geological setting of the deposit and how the ore/waste will be mined, such as whether openpit or underground, the mine schedule, and the mine fleet type and capacity. The purpose of this paper is to provide an experience-based practical approach to how to select and characterize test samples as well as devise process development metallurgical studies for both oxide and refractory gold ores, and to provide insight into some of the matters that might go wrong during the course of the studies. This paper is intended as both a guide for someone new to the field and also as a reminder for those familiar with the field. IntroductionThe recent decline in ore grades and metal prices have challenged mining companies to reduce capital and operating costs associated with developing new mineral resources. In some cases, metallurgical studies have been sacrificed to prevent cost overruns resulting in inadequate mineralogical, chemical and metallurgical characterization. Consequences have included longer construction schedules, higher initial and working capital, higher operating costs and less than desirable plant performance (Whincup, 2010; Torres, Chaves and Meech, 1999).Mineralogy, geology, mining type and sequence, and the metallurgy of an orebody will determine the process development metallurgical study requirements and eventual process plant design. Therefore, once the geologist has an early indication of a prospective orebody, geologists, metallurgists, mining engineers, and social and environmental responsibility scientists should convene and devise a plan on how to exploit the orebody economically (Lunt, Ritchie and Fleay, 1997).Geologists may find a prospective orebody solely based on grade. However, if the orebody cannot be mined and processed economically, then it is deemed not to be a deposit. An example of this could be a refractory gold deposit, either sulfidic, carbonaceous or siliceous refractory or any combination of those, that may not be mined and processed economically due to reasons such as the physical location of the deposit, depth of the ore lode, or social and environmental concerns. Therefore, an early collaboration between all disciplines must be established.Each orebody may have its own unique mineralogical, geological and chemical characteristics (Guresin et al., 2012). These unique characteristics should be effectively sampled to reduce risks associated with process design and engineering efforts for the process development metallurgical studies. If the sample or samples are not representative of these unique characteristics, then the studies performed will be pointless. The project metallurgist in charge of conducting metallurgical studies should also possess general geology, mineralogy and mine engineering knowledge. Recommended readings are a book on understanding mineral deposits by Misra (2000) and articles on heap leach development by Scheffel (2014a and 2014b)."

The design of a preparation plant for minimum maintenance and clean up requires both the skills of the Engineer and the practical experience of the Operator. From the initial flow sheet and preliminary design until the plant is completed and operating, each phase of the work must be thoroughly explored for simplicity, workability and high efficiency. It is the responsibility of the Designing Engineer to develop the plant around the basic flow sheet, selecting equipment and materials not only with regards to Initial cost but also required upkeep and repair.

INTRODUCTION In the USA regulatory agencies have adopted standards which limit the allowable concentration of ore dust in underground uranium mines. The American Conference of Governmental Industrial Hygienists has recommended a threshold limit value for silica which has been incorporated by reference in federal regulations (30 CFR 57, 1980). The Nuclear Regulatory Commission (NRC) imposes a limit for unprocessed uranium ore dust for reducing the exposure to radioactive materials from inhalation (10 CFR 20, 1980). We have been unable to trace the origin of the NRC standard. Recently the ICRP has published recommendations concerning limits for intakes of radionuclides by workers (ICRP, 1979). We have used the ICRP methodology to compute the dose equivalent commitment to lung and bone from inhalation of insoluble ore dust particles. These values were used to derive air concentrations which would provide an acceptable risk for induction of cancer from occupational exposure. Results indicate that the new ICRP recommendations are more restrictive than the present NRC standard for radioactivity. The maximum allowable dust load in a uranium mine will depend on both the concentration of silica (% Si02) and the amount of radioactivity (% U308). We have combined these standards assuming complete independence of biological hazard to establish boundary conditions for which either silica or radioactivity is the dominating factor. Based on these results we present suggestions for analysis of ore dust to insure compliance with regulatory agencies. SILICA STANDARDS Silicosis, fibrotic lung disease produced by crystalline silica, was one of the earliest forms of occupational disease to be recognized. The pulmonary lesions caused by silica, silicotic nodules, consist of concentrically arranged bundles of collagen fibers. Fusion of these nodules results in progressive massive fibrosis causing alveolar changes which decrease ventillation and blood flow in the lungs. The current Threshold Limit Value (TLV) for mineral dusts is based on the concentration of free silica. The standard is expressed in three forms: (1) Particle count standard: TLV (mppcf) = 300/(% Si02 + 10) (mppcf = million particles per cubic foot) (2) Respirable dust mass standard: TLV (mg/m3 ) = 10/(% respirable Si02 + 2) (3) Total dust standard: TLV (mg/m3 ) = 30/(% Si02 + 3) The most commonly used form is the respirable mass standard. These curent values for silica are based primarily on epidemilogical studies of Vermont granite workers and other occupationally exposed workers. In the Vermont study no cases of silicosis were seen in individuals employed after dust control measures were initiated and whose subsequent exposure averaged less than 5 mppcf. The free silica concentration in the airborne dust to which these workers were exposed averaged 25%. A concentration of 10 mppcf of granite dust is considered equivalent to 0.1 mg/m3 quartz. The current respirable mass standard allows a maximum silica concentration of 0.1 mg/m3. NIOSH (1974) has recommended a reduction in the current TLV for respirable silica to 0.05 mg/m3 Si02. As with radiation standards, the TLV for mineral dusts is an upper limit for time weighted average exposure. It is recommended that dust concentrations be maintained as far below the TLV as current practices permit. RADIATION STANDARDS The present concentration limit for airborne natural uranium in ore dust prior to chemical separation is 75 µg of uranium per m3 of air ( 10 CFR 20, 1980) . This corresponds to 1.85 Bq/m3 (50 pCi/m3 ) of natural uranium, U-238, U-235 and U-234 and is equivalent to 3.7 Bq/m3 (100 pCi/m3 ) of gross alpha activity with radioactive equilibrium between the long-lived alpha progeny through Ra-226. The origin or basis for this standard is not known, but it presumably includes both chemical toxicity and risk from somatic radiation injury. Recently the International Commission on Radiological Protection (ICRP, 1977) has published recommendations concerning the objectives and criteria for limiting radiation exposures. This was followed b y a revision of the limits for intake of radionuclides by workers (ICRP, 1979; ICRP, 1980). The previous compilation of limits for internal emitters was published by ICRP Committee II in 1959 (ICRP, 1959). As a result of the enactment of the Federal. Coal Mine Health and Safety Act of 1969 1, the Department of Health, Education, and Welfare (HEW) has been carrying out research in the "field of coal mine health". The National Institute (formerly Bureau) for Occupational Safety and Health (NIOSH) was designated the agency within HEW to be responsible for such research activities. One of the early programs sponsored by NIOSH was undertaken by Eastern Associated Coal Corp., with the Harvard University School of Public Health participating as a subcontractor to Eastern. This program on Coal Mine Respiratory Protective Devices, which started on June 30, 1970, had five major objectives as follows: a. to determine current status of respirator usage with respect to duration and frequency, types. and maintenance b. to determine acceptability ( and reasons therefor) of currently used units c. to determine protection factors provided by respirators worn by working miners d. to make recommendations on ways to improve existing units, or on research needed to develop new types of respiratory protective devices for coal miners e. to provide recommendations to mine management on more effective use of respiratory protective devices

The planning, construction, and monitoring of underground storage facilities is a key factor in meeting upcoming challenges as part of the energy transition from fossil fuels to renewables. Therefore, the many years of knowledge and expertise in ground control gained from coal mining are highly valuable for new mining and storage operations. For such applications, our previous work examined cavity registration, combining structure from motion and LiDAR through a case study in a former lead ore mine. Building on this, the results of this recent case study are discussed regarding the possible application in sites for nuclear waste disposal in Germany including a view on the situation in the United States. Photogrammetric methods are particularly suitable as part of long-term monitoring, which relies on both precise survey results and a large variety of detailed visualization. In order to gain a better understanding of the resulting application potential for holistic monitoring, both existing repositories sites in Germany and ones yet to be found are examined. This search and selection process for high-level nuclear waste disposal is studied in detail together with rules resulting from the (German) mining law, regulations, and other guidelines. Hereby, key requirements of photogrammetric products like oriented images, point clouds, and three-dimensional (3- D) models are identified for different case studies. The findings on accuracy and integrity of the generated models are not only of interest for future repository sites or underground pumped storage power plants, but also offers various possibilities for underground mining of coal and other commodities.

INTRODUCTION Tunnel construction has always been an extremely high risk form of contracting primarily due to the inaccessability of information concerning the ground conditions to be encountered during excavation. We certainly make every effort to overcome this problem through borings, geological research, site visits, etc.; but at best owners, engineers and contractors alike are only able to interpolate a relatively small amount of factual data from isolated sections of the project into a forecast of ground conditions for the entire project. In essence this becomes an "educated guessing game" with risk to reward ratios (and subsequently the bid price) in direct relationship to the contractor's confidence in the quality of the guess. In many instances it is economically or physically infeasible to obtain information and of course the total picture is never known until the project is complete. This problem effects the contract format as well as price. The contractor's ability to handle unforseen problems on the project become extremely important as to the project's overall success. The following pages contain an actual case history of a project which affords an excellent example of the entire spectrum of situations resulting from tunnel construction in difficult ground conditions.

Lafarge Corporation's Texas Quarry, in Baltimore MD, achieved a substantial expansion of pit reserves by removing an inactive underground mine. The day lighting program, designed, and completed in 1997, open pit mined more than 400 meters of room and pillar workings which were originally developed to recover high brightness Calcium Carbonate. The project proved to be a challenge throughout its course as the pit plan was modified and successful daylighting techniques were developed to remove the mine without adversely affecting the safety of the operation, quality of the stone recovered, or negatively impacting the surrounding community. The design parameters and techniques developed and implemented at Texas may prove of interest to other operators as they are faced with daylighting opportunities. This article will detail the challenges we faced and the solutions which made the project a success.

A greenfield open pit gold mine in Latin America suspended operations less than three years into an eight-year mine plan after a waste rock dump and heap leach facility sustained substantial damage from a deep-seated landslide. No injuries to employees were recorded because of the slow-moving failure. Due to early detection by the operations team, the owner had sufficient notice of the potential failure and successfully rinsed the heap leach facility prior to the failure event. Engineers estimated approximately 35 million cubic meters of mass movement below the waste rock and heap leach facilities. The original eight-year mine plan produced approximately 60,000 ounces of gold per year. After the failure, the owner terminated gold production, re-configured the mine site to a safe, standby condition and launched a detailed investigation into the root-cause of the failure. Approximately two years after the shutdown, legal interests filed a statement of claim and named three engineering design firms and individuals involved in the project. Several years of litigation, including discovery and deposition proceedings, followed as the legal process considered evidence spanning a 20-year timeline. Changes in project ownership and consulting firms during the development and operations phase complicated the process. In 2017, the plaintiff and last defendant settled. The author was involved as the corporate representative for one of the engineering design firms.

Model pillars of limestone, marble, sandstone, and granite, with length-to-diameter ratios of 3, 2, 1, 0.5, and 0.25 (0.286 for granite), were broken in axial compression to determine to what extent an increase in end constraint increased compressive strength. Radial end constraints of 13 to 23 percent of the average axial stress in the pillar, produced by solid steel rings bonded with epoxy to the ends of dogbone-shaped specimens, increased compressive strength somewhat above that of cylindrical pillars without ring constraint. However, when the results were compared with those obtained by other investigators for straight specimens of several rock types taken collectively, with L/D ratios greater than 0.5, the resulting strengths were not significantly different. Thus, the amount of end constraint produced by the solid steel rings was about the same as that produced by the friction from the steel end plates. In other tests a radial prestress of 3,000 psi or 5,000 psi was applied prior to axial loading by adjustable hardened steel rings to increase the constraint above that obtained for the solid rings. The average radial constraint stress, expressed as a percentage of the average axial pillar stress at failure for the 3,000 psi prestress, was 54.3 percent for limestone, 40.3 percent for marble, 44.7 percent for sandstone, and 23.4 percent for granite. The average radial constraint stress, expressed as a percentage of the average axial pillar stress at failure for the 5,000 psi prestress, was 74.2 percent for limestone, 51.2 percent for marble, 61.6 percent for sandstone, and 29.7 percent for granite. These constraints increased the compressive strength significantly above the strength of straight specimens and solid-ring constrained specimens. These results suggest that large horizontal stresses in orebodies mined by the room and pillar method should increase the strength of the pillars and allow an increase in ore recovery by a reduction of pillar size when major structural defects are absent.

Flotation of rutile with N-benzoyl-N-phenyl-hydroxylamine (N-BPHA) has been investigated. Initially, floatability tests were performed in a Hallimond tube. The results indicated that rutile was floatable in the pH range from 2 to 4 at N-BPHA concentration between 2x10-3 M and 2x10-2 M. Selectivity against hematite was recognized when operating at pH values below 2. Following the hypothesis of the formation of a surface chelate between N-BPHA and titanium ion, this compound was precipitated from a solution of Ti(SO4) to which N-BPHA was added. I.R. spectrometry oft the rutile before and after treatment with N-BPHA solution confirmed the presence on the rutile surface of the same compound. The absorption isotherm and kinetics were determined by analyzing N-BPHA in solution with an U.V. technique. A hypothesis for the mechanism of adsorption of N-BPHA on rutile is suggested.

When coal in an oil/water slurry is ground to minus-200-mesh in a ball mill, some of the mill energy, normally wasted as heat, can be usefully recovered to help emulsify the oil. If the discharge flows onto a suitable sized trommel screen, water and ash material are rejected through the screen while coal and oil ball together to give a clean coal product. A small (6-lb/hr) continuous plant has demonstrated the feasibility and the limitations of this process for New Mexico coals.

The Mariana arc is an ~1,400-km-long intraoceanic arc that marks the southern extension of the Izu-Bonin arc, and is host to 62 ?volcanic centers? of which at least 27 (21 submarine) are hydrothermally, or volcanically active. A surface ship cruise in early 2003 mapped the distribution of hydrothermal plumes emanating from these centers, followed by remotely operated vehicle (ROV) cruises with ROPOS in 2004 and Jason II in 2006. With the exception of one volcano (East Diamante), none of the vent sites visited by the ROVs were host to sulfide mineralization. Rather, they were characterized by high sulfidation mineral assemblages dominated by native sulfur. The sulfur was seen marking the vicinity of vent sites, or as sulfur ?chimneys? (e.g., NW Eifuku and Nikko), or more spectacularly as ?lakes? of molten sulfur, either occupying the summit crater of the volcano (e.g., Nikko), or perched on the flanks of the volcano, near the summit (e.g., Daikoku). Gas (H2S and CO2) was discharging through the molten sulfur. Evidence for these lakes to have either been substantially ?deeper? in their recent past (e.g., Nikko), or existing at other volcanoes (e.g., Seamount ?X?) is given by remnant ?pillars? of native sulfur that are reminiscent of rock pillars in lava lakes found along mid-ocean ridges.

Acid mine drainage, discharging from coal beds, has polluted our streams and rivers since early time. These pollutants affect water quality by lowering the pH, reducing natural alkalinity, increasing total hardness, and adding undesirable amounts of iron, manganese, aluminum and sulfates, The tangible damages are the costs involved in replacing equipment corroded by the acid water, additional treatment costs at municipal and industrial water treatment plants, and damages resulting from corrosion of steel culverts, bridge piers, locks, boat hulls, steel barges, pumps, and condensers. Intangible damages, which are real and important, include destruction of biological life of the stream, reduced property values, and streams rendered undesirable for recreational uses.1 The major problems of mine drainage occur in the anthracite and bituminous coal regions in Appalachia. However, many of the western mining states have significant mine drainage problems in specific areas, but the overall problem is not as great as in the eastern states.

INTRODUCTION There are currently around fifteen companies in the United States and Europe manufacturing hard rock tunneling machines. In recent years upwards of 200 hard rock tunnels of lengths up to 12 miles and diameters to the order of 30 ft have and are being successfully driven by over 100 different machines. It has been predicted that within the next decade over 2,000 miles of tunnels will be driven for transportation purposes in the United States alone (OECD Advisory Conference on Tunneling, Washington, 1970) and it is expected that eventually most tunnels including mine drivages will be excavated by machines. All tunneling machines have certain common basic functions; thrust is applied to drive or hold the rock cutting tools into the face, and torque is applied to rotate the cutting tools over the face so that they can continuously break out rock. The speed of head rotation, together with the torque requirement, determines the cutter head power. (The power required to push the head forward is usually negligible.) There are many factors which determine how successfully a given tunneling machine will perform in a given rock but one of the most important is effective utilization of cutter head power. This paper is a review of the specifications and performance of hard rock tunneling machines, obtained from manufacturers' catalogs, published literature, talks with manufacturers and visits to operating machines throughout the United States and Europe.

Introduction Lyon, with 1.1 million inhabitants, is the third largest city in France after Paris and Marseille. It is a lively industrial centre. Lyon lies at the intersection of many transport routes and at the fluence of Rhone and Saone. Like all large cities, Lyon is attempting to alleviate inner city traffic problems by constructing a metro. Metro construction in Lyon began with tunnels built by cut and cover methods along the streets. Deep diaphram walls, which were back anchored and toed into imperveous strata, formed the excavation walls. It is a very economic and simple method. However during the construction period, it has the disavantage of strangling urban life for several years. It is therefore increasingly being replaced by subsurface tunnelling methods. As it was decided to build the metro tunnel between the Guillotiere and St. Jean stations, and thus cross under the Rhone and Saone, the client, Semaly, put the contract out for a competitive tender in which the general parameters were defined. Line, level and construction method were left to the contractor. There was an extensive examination of the tenders, during which reference projects from competitors in Japan were visited, as well as in Germany. Finally the consortium of Dragages, Campenon Bernard, Colas and Hochtief was awarded the contract to drive this tunnel section with an hydro-shield and an extruded steel fibre concrete lining, a Hochtief design.

In the construction of recent subsurface projects, particularly mined openings and transit systems, it has been apparent that final costs were often greater than first expected. Because many overruns result from alleged differing site conditions, there was reason to suspect that pre-bid geotechnical investigations were inadequate for predicting subsurface geology. In 1982, nine federal agencies involved in constructing underground facilities funded the National Academy of Sciences' U.S. National Committee on Tunneling Technology (USNC/TT) in an effort to evaluate and clarify the relationship between geological knowledge and construction costs. That committee formed the special Subcommittee on Geological Site Investigations, which spent more than two years studying underground projects by examining contract documents and geologic reports, making site visits, and interviewing owners, designers and contractors. The study ultimately encompassed 84 mined tunnels and chambers and three deep shafts in the United States and eastern Canada. Data collected on geologic exploration, design and construction were analyzed and conclusions reached concerning whether and how site investigations need to be improved. The results of the study are published in two volumes by the National Academy of Sciences. Due to the recent completion date of the main report, it was not possible to prepare a summary for inclusion in the RETC Proceedings. "On October 11, 2000, an estimated 306 million gallons of water and coal waste slurry drained from an impoundment in Martin County, eastern Kentucky into an adjacent underground mine. Approximately 230 million gallons of the water and slurry discharged from two underground mine portals and affected over 75 miles of streams in Kentucky and West Virginia. In response to this and several other similar events, the U.S. Office of Surface Mining Reclamation and Enforcement (OSMRE) commissioned an oversight study that evaluates how well state and federal regulatory programs established under the Surface Mining Control and Enforcement Act are ensuring that impoundment operators are minimizing the potential for impoundment-basin breakthroughs. The study focuses on slurry impoundments constructed in the hollows of Appalachia where numerous coal seams and steep topography combine to result in a large number of mined seams intersecting and underlying impounding facilities. Factors under consideration include: (1) full accounting of all mineable coal seams intersecting and underlying the impoundment; (2) identification and accurate location of underground mines close enough to the impoundment to potentially affect its stability; (3) assessment of the stability of coal barriers between the impoundment basin and adjacent mines, and stability of roof rock and pillars in mines subjacent to the structure; (4) determination of the flowability of the impounded slurry when expanding or undermining the facility are being considered; and (5) measures undertaken to reduce breakthrough potential when necessary. The oversight study began in 2012 after program-evaluation criteria were defined in an OSMRE technical position paper. The study is scheduled to be completed in 2017. BACKGROUND The Surface Mining Control and Reclamation Act (SMCRA) authorizes the disposal of coal mine waste by constructing a coarse coal mine waste embankment across a valley and pumping the fine coal mine waste (slurry) into the resulting basin (Figure 1). A concern shared by many engineers, geologists, and mine inspectors familiar with coal mine waste slurry impoundments is related to the common occurrence of underground mine workings adjacent to or beneath the impoundment: the potential for slurry “breakthroughs” into mine works and subsequent breakouts into the surface waterways (Figure 2). On October 11, 2000 a combination of coal refuse slurry and water from the Big Branch impoundment in Martin County, Kentucky broke through into an underground mine and subsequently discharged into the receiving streams. The location of the point of breakthrough, pathways of slurry/water flow within the mine workings, and two points where the liquid exited the mine are shown in Figure 3. An estimated 306 million gallons of water and coal refuse slurry drained from the impoundment into the adjacent underground mine. Approximately 230 million gallons subsequently discharged from the underground mine at two portals."

"BACKGROUND As of December 2015, there are roughly 475 road tunnels within the United States according to the most recent national tunnel inventory data compiled by the Federal Highway Administration (FHWA); whereas, there are more than 615,000 highway bridges on public roads. The FHWA estimates that tunnels represent nearly 100 miles of Interstates, State routes, and local routes. Tunnels are part of the national transportation infrastructure and represent a significant financial investment for the owner and, hence, the tax payers. Tunnels, that are not properly maintained, usually require more extensive repairs and consume more resources. Tunnels must provide safe, reliable, and effective transportation even as these structures age and deteriorate. Furthermore, tunnels generally remain in service for extended periods of time due to their very high construction costs. For example, many tunnels were constructed decades ago during two historic periods of highway system expansion; the first period correlated with the era of public works programs linked to the Great Depression of the 1930-40s; and the second period of expansion occurred with the building of the interstate highway system in the 1950-60s. As such, these tunnels are fast approaching 60 to 80 years old. Congress enacted the Moving Ahead for Progress in the 21st Century Act (MAP-21), which was signed by the President on July 6, 2012. This legislation followed a fatal tunnel incident on July 10, 2006 and a National Transportation Safety Board (NTSB) report1 that noted the importance of conducting regular tunnel inspections. MAP-21 authorized the Federal Highway Administration (FHWA) to develop a mandatory inspection program for all tunnels on all public roadways (Bergeson and Ernst 2015): To help safeguard tunnel users and to ensure reliable levels of service, the FHWA developed the National Tunnel Inspection Program (NTIP), which is comprised of the National Tunnel Inspection Standards (NTIS), the Tunnel Operations Maintenance Inspection and Evaluation (TOMIE) Manual, the Specifications for National Tunnel Inventory (SNTI), and the Manual for Bridge Evaluation (MBE). The data collected from the program will be stored in a National Tunnel Inventory (NTI) database. A comprehensive and refresher training program was also developed to help qualify and certify key tunnel inspection personnel to include the program manager and team leader."

"INTRODUCTION An inherent aspect of any tunnelling project is ground risk: how it is identified, quantified and managed throughout the various stages of a project. Early identification of these risks, such as adverse ground conditions or potential obstructions is crucial; however specific identification of all risks will inevitably be limited by the extent of the desk study, ground investigation and accuracy of as-built information available, particularly in a densely populated urban environment such as London with a long and varied history of development. It is therefore crucial to have suitable plans and procedures in place for dealing with potential ground risk scenarios if they occur during construction work to ensure the safety of the workforce and to protect nearby critical assets. A number of experiences of potential ground risk were encountered during the Sprayed Concrete Lining (SCL) works on Crossrail contract C300/410 which was constructed by a joint venture of BAM Ferrovial Kier (BFK). The C300/410 works, undertaken in the heart of London, comprised of over 2.5km of SCL tunnels for Bond Street and Tottenham Court Road Stations and the Fisher Street crossover, as well as two 6.2km TBM tunnel drives between Royal Oak and Farringdon. BFK appointed Donaldson Associates as their SCL designers for the works; together they were responsible for excavation stability up to the point of ring closure. Despite the majority of SCL tunnelling works at Bond Street and Tottenham Court Road being through the favourable medium of London clay, there remained areas of local geotechnical challenges. Some of these were expected from the outset, some became apparent as works progressed from records of other nearby excavation works, and some were only encountered during tunnel excavation. Being located in the centre of London meant that there always remained a possibility of meeting some form of obstruction from previous structures. However, identification of all potential issues prior to the commencement of the works was challenging and proved to not be entirely possible. Through a series of case studies, this paper documents incidents where obstructions or adverse ground conditions arose. These scenarios all had the potential to cause safety risks, impact the tunnel design and/or construction methods and ultimately add time and cost to the works. The paper discusses how these challenges were overcome which utilised in part, a progressive risk reduction cycle, feeding back knowledge gained at all stages of construction into the approach for undertaking the works. It also examines what could be done in future to further reduce the ground risk through early identification, communication, education and appropriate contingency measures."
