Disasters are often tragic outcomes of high-risk technologies such as mines. In Australia, more mines are being developed every day, and the risk of disasters is ever increasing. No matter how effective our conventional safety devices are, there is a form of accident that is inevitable. These relate to accidents that result from æinteracting failuresÆ in a way that could not be foreseen by the designers. In so-called ætightly coupled production systemsÆ (processes that happen very fast, such as on a high producing mine) the risk is even higher and our risk controls mostly introduce some sort of a technological fix. While we are at the one hand attempting to control the risk, we are also introducing another level of complexity. Are we really controlling the risk? This is one of the fundamental questions that will be addressed in this paper: Do we have the ability within the mining industry and other industries to effectively prevent these catastrophes, or are we, on the contrary, faced with an increasing risk as a result of increasing complexities of our technology, management systems and practices. This paper will contend that perceived improvement in risk control is an illusion of activity, and that the likelihood of mining catastrophes may be exponentially increasing. Although the paper focuses on the mining sector many aspects may be applicable across a range of industries. This paper will further contend that we are applying the right solutions to the wrong problems. Our focus is technological and procedural, while our problem is one of production cultures ripe for error and failure.

This paper considers a recently completed hoist project for IAMGOLD. Called the Westwood project, this mine utilises some of the largest drum hoists supplied to the North American Market in recent times. This project is an example of where many of the design features discussed the associated paper, (Robotham, 2011) were incorporated. Director of sales and marketing, Gordon Scott has worked at DavyMarkham for over 30 years in a variety of roles starting as a mechanical design engineer, moving to project management before joining the sales department. Objective To reduce dust generated from soiled floors within enclosed operator cabs. Background Enclosed operator cabs are widely used on mobile excavation equipment in the mining and construction industries. They protect the operator from bad weather, noise, and airborne dusts. The inside cab environment usually must be kept at very low dust concentrations because of quartz dust generated from the excavation of silica-bearing rock. For cabs to be effective in controlling airborne respirable dust, they must have an efficient air filtration system, while providing a positive pressurization of the cab interior. Ideally, the recirculated and exterior makeup air filtration should provide at least 99% capture efficiency for dust particles as small as 0.3-µmaerodynamic diameters. Many of these enclosed cabs use heating units mounted or directed along the floor (see figure 1), which can cause dust entrainment problems. Prior studies by the National Institute for Occupational Safety and Health (NIOSH) of enclosed cabs have shown that these floor heaters can generate notable amounts of dust within the cab enclosure (see Technology News 486). Multiple-shift dust levels measured inside a drill cab increased on average from0.04 mg/m3 during the summer months to 0.68 mg/m3 during the winter months. Additional dust level measurements, made with optical particle counters during a nondrilling time period, verified that dust levels inside the cab increased from 0.03 to 0.26 mg/m3when the floor heater/fan was turned on. Dust levels within the cab also became greater than the ambient air dust levels outside of the cab when the floor heater was turned on during the nondrilling time period. This shows that the floor heater was a notable dust source problem. Cab floors are commonly soiled from operators tracking dirt inside the cab upon entering from the mine or construction site. Interior cab dust levels can be increased by (1) airflow disturbance of the soiled floor or (2) operator disturbance of the soiled floor. Approach During prior NIOSH surveys of surface coal mine drills, several drills with very low cab dust levels (#0.1 mg/m3) were observed to have floor sweeping compound spread out over the cab floor during the working shift. This initial observation led to a more controlled A-B study of the effect of the sweeping compound on the interior cab dust levels. This study involved dust sampling with and without sweeping compound inside the cabs of a bulldozer and two drills. Sweeping compounds commonly use sawdust or cellulose as the main bulk material, with oil or wax added for dust adhesion purposes. Sand is sometimes added to increase bulk density. Sweeping compounds with sand are commonly used to sweep up concrete floors. Those without sand (usually referred to as "gritless") are used to sweep up smooth finished floors. Most of the oils and waxes used for the adhesive ingredient are petroleum-base and can have an irritating odor. People sensitized to petroleum distillates could have allergic reactions to these sweeping compounds. A few companies offer non petroleum-base sweeping compounds, using either a natural oil or chemical additive for dust adhesion. During this study, it was decided that only the gritless sweeping compound (without sand) would be used, eliminating the addition of another potential silica source inside the cab. Since the sweeping compound would be applied on the enclosed cab floor for the duration of the shift, it was decided to use a canola oil-base sweeping compound to eliminate irritating odor from the petroleum-base sweeping compounds. The natural canola oil-base sweeping compound had a slight woody scent. The three pieces of equipment used in this study had notably different cab layouts. The bulldozer had a small cab with vents discharging air-conditioned or heated air from adjustable louvers at operator waist level and floor level. The bulldozer also had a cloth fabric seat and a carpeted floor. Drill A had a large cab with the heat directed from adjustable louvers at operator knee level and the air-conditioned air directed from cabin ceiling louvers. This drill had a vinyl fabric seat and a steel plate floor. Drill B had a small cabin with the heat directed across the floor (see figure 1) and the air-conditioned air directed down from the ceiling. It had a vinyl fabric seat and a worn rubber mat on a steel floor.

The Gallinas Mountains are in northern Lincoln County and southern Torrance County, New Mexico, USA where a series of alkaline igneous bodies, including porphyritic latite, trachyte/syenite, andesite, and rhyolite laccoliths, flows, dikes, and plugs, have intruded Permian sedimentary rocks belonging to the Permian Abo, Yeso, and Glorieta Formations. A small amount of bastnaesite, a REE mineral, was recovered during processing for fluorite from the Gallinas Mountains. Four types of deposits are found in the Gallinas Mountains: epithermal REE-F veins, Cu-REE-F veins, REE-F breccia pipes and iron skarn deposits; all are associated with Tertiary alkaline to alkalic-calcic igneous rocks. Alteration includes brecciation, silicification, chloritization, and fenitization. District zonation is defined by Cu-REE-F (±Pb, Zn, Ag) veins that form center of the district, surrounded by REE-F veins. Carbonatites are inferred at depth by the presence of fenitization, carbonatization of the breccias, presence of REE and similarity of the intrusive rocks and mineralization to areas with known carbonatites.

The axial capacity of step tapered bored piles in sand was examined by field tests at one site in Kuwait. Two straight piles, and two step tapered piles were load tested in compression to failure. All four piles were 3 m deep. The straight piles were 0.2 m in diameter. The step tapered piles had a diameter of 0.3 m for the upper segment which extended up to 40% of the pile length changing to 0.2 m diameter for the lower section. The soil profile consists of medium to dense sand with minor cementation and no groundwater was encountered. Both the ultimate bearing capacity and the settlement at working loads were examined. The axial capacity of the step tapered piles was greater, and the settlement at working loads was less, compared with the straight shaft piles. The increase in capacity is mainly attributed to the added side friction along the larger diameter top segment of the shaft.

Deformed regions of the Hamersley Province, Western Australia, contain several major iron ore deposits where banded iron-formation (BIF) of the Dales Gorge Member was converted to martite-microplaty hematite. The genesis of these ores remains controversial, in part because no study has systematically documented how stratigraphically equivalent rocks change chemically from undeformed regions into the deposits. Profound chemical and mineralogical differences do occur between rocks in the undeformed north and mineralised south. BIF and shale can be classified into three types: BIF, altered BIF and iron ore, and black shale, altered black shale and red shale. A series of low temperature (20 to 250¦C) chemical reactions can explain the observed differences in chemistry and mineralogy. The inferred reaction paths indicate that no single process could have produced all of the altered rocks at Mt Whaleback. Recently described carbonate-rich, silica-poor rocks at Mt Tom Price (Taylor et al, 2001) suggest this alteration sequence is even more complex. Our favoured explanation for the genesis of hematite ores is one that can explain deposits at Mt Whaleback and Mt Tom Price: following regional metamorphism, hydrothermal fluids locally formed carbonate-rich, silica-poor BIF with magnetite and carbonate-rich altered black shale. Subsequent surface derived fluids oxidised this magnetite, crystallised microplaty hematite (mpl) and eventually dissolved carbonates and silicates leaving high-grade ore. Either during or sometime after mineralisation, acidic, oxygenated fluids converted altered black shale to red shale and unaltered BIF to martite-rich BIF without carbonates across the southeast of the province.

The ferrite-pearlite microstructure is the most popular microstructure for alloys used in structural applications, including railway wagon wheels. These alloys have been designed through alloying and thermo-mechanical processing to have a refined microstructure. Ferrite-pearlite alloys are low cost, weldable, have good fabricability, and are reliable under extreme conditions. Given these performance attributes, it seems unlikely then that their dominant position as structural steels would ever be challenged by alternative microstructures. One major achievement in the development of ferrite-pearlite steels has been in the refinement of their interlamellar spacing to very fine distances of the order of < 0.3 um. A refined microstructure increases the hardness of the alloy, thus increasing its life under wear conditions. The interlamellar spacing in pearlitic steels has, however, been refined almost to its theoretical limit. The increasing demand for speed and increased axle loading on railway wagons requires the use of stronger, tougher, and more durable materials. This has opened the window for the development of novel bainitic steels. Bainitic alloys have a higher level of microstructural refinement than pearlitic ones. They have shown to have good wear resistance and rolling-contact fatigue resistance, and high toughness. This paper will discuss the progress to date on the development of bainitic railway wheel alloys. Four alloy chemistries have been chosen for possible further development.

The reversal of smoke products-of-combustion (POC) from a mine fire was determined in a mine section with the airway connectivity of an electrical Wheatstone bridge. Four diesel-fuel fire experiments with fire heat-release rates between 504 and 771 kW were conducted in the Safety Research Coal Mine (SRCM) located at the National Institute for Occupational Safety and Health’s (NIOSH) Pittsburgh Research Laboratory (PRL). Smoke reversal propagated upwind from the fire with significant leakage into the upwind diagonal airway and without causing a complete reversal of airflow in the diagonal airway. A control measure consisting of a brattice suspended half entry height from the entry roof was determined to abate the smoke rollback. Computational fluid dynamics (CFD) analysis of the smoke movement agreed with the measurements.

Rock mass classification systems are extensively used in rock engineering design work, and mine design is no exception. Among the systems most widely used for mining-related design work are the NGI 0-system (Barton et al., 1974), the RMR system (Bieniawski, 1976), the MRMR system (Laubscher and Taylor, 1976) and, more recently, the GSI system (Hoek et al., 1998). Classifying the rock mass is widely seen as being the fieldwork required to characterize the rock mass and enable the application of empirical design methods associated with the different classification systems. This paper argues that it is fundamentally important to recognize the distinction between rock mass characterization and rock mass classification. These two processes should, in most cases, be separated from each other. Rock mass characterization should be used to determine the intrinsic properties of the rock mass independently of the application; i.e. independent from the infrastructure to be designed, the size, shape, and orientation of the excavation(s) or pillar(s), etc. Rock mass characterization should also be compatible with most classification systems and empirical design methods to be used. Rock mass characterization is the background fieldwork required to perform rock mass classification and/or engineering design work. Rock mass classification is the subsequent step to the characterization, and an integral part of the design process. Parameters that vary according to the design, such as the relative orientation of geological structures compared to the opening or the mine-induced stresses, should be calculated as part of the rock mass classification and design process, rather than during the rock mass characterization process. The failure to distinguish between rock mass characterization and rock mass classification can lead to major design errors and poor results.

This paper looks at recent advances in the United Kingdom (UK) tunnelling industry with particular reference to the A3 Hindhead Project. A 1.8 km long twin bore tunnel is the centrepiece of the A3 Hindhead scheme, which commenced construction in 2007 and is being delivered under a UK Highways Agency early contractor involvement (ECI) contract. The proposed construction methodology is progressive mechanical excavation of a horseshoe-shaped tunnel with shotcrete installed close behind the face using robotic spraying equipment. The tunnelling medium is a æsoft rockÆ sandstone for which there was little previous tunnelling experience, requiring extensive geotechnical investigations and an innovative approach to lining design. A 240 m long section though sandy material utilising a steel pipe umbrella is also required. This paper describes the development of the tunnel planning and design including the most recent developments in UK road tunnels. The innovative approach to the design of the following is discussed:the æpermanentÆ primary lining,an observational technique allowing optimisation of the steel pipe umbrella design,a plain concrete secondary lining designed for resistance to a hydrocarbon fire, anda spray-applied membrane.

This paper presents the results of a comprehensive pre-production pile testing programme for the US$ 800 million 1400MW Coal-Fired Jimah Power Plant Project in Negeri Sembilan, Malaysia. The testing programme included high-strain dynamic testing and fully instrumented static axial and lateral load testing of driven prestressed concrete piles and bored cast-in-place piles. A highlight of the testing programme was the pioneering use of the Global Strain Extensometer (GloStrExt) Method, a deformation monitoring system that uses advanced pneumatically- or hydraulically-anchored extensometers and a novel analytical technique to monitor loads and displacements down the shaft and at the toe of driven prestressed concrete piles. The experience with the testing programme for the Jimah Project also demonstrates the possibility of achieving notable breakthroughs, not only in the monitoring, evaluation and reporting of pile foundation behaviour dynamics but also in our understanding of the mechanisms that underpin such behaviour. These significant gains are undeniably a direct result of the committed and co-operative relationship among the project owner, design/builder, specialist piling subcontractors, pile suppliers and pile instrumentation and testing specialists.

Although the pattern of metamorphic zones south of the Thackaringa-Pinnacles Fault is much obscured by polymetamorphism, displacement of the orthopyroxene isograd by the fault agrees with the displacement of a horizon of &quot;banded aplite&quot; mapped by Andrews (1922). A dextral transcurrent component of 20,000 ft is indicated.INTRODUCTIONThe east-west Thackaringa-Pinnacles Fault (Andrews, 1922; King and Thomson, 1953), one of the major geological structures in the Broken Hill district, New South Wales, cuts across the general north-easterly strike of outcrops and fold structures of the Willyama Complex. The degree of shearing and recrystallization in its unusually wide crush zone and the manner in which adjacent rock units and structures have been dragged into conformity with its strike denote considerable movement on the fault. Information concerning the nature of this movement is of critical importance in the search for possible continuations or repetitions of lead-zinc mineralization south-west of Broken Hill.A transcurrent movement of 20,000 ft, with the southern block moving west, was estimated by Andrews (1922), who correlated prominent outcrops of &quot;banded aplite&quot; at The Pinnacles (north of the fault) and Big Hill (south of the fault). Displacement of this order, however, does not seem sufficient to explain the extent and character of the fault zone. Recent chemical and petrological studies of the banded aplites at The Pinnacles and Big Hill by Vernon (19B1) confirmed their similarity, but indicated an origin by metasomatic replacement of pre-existing folded and metamorphosed rocks. After carrying out a brief review of the existing safety assessment criteria for the spontaneous detonation of explosives in the mining of sulphide ore deposits in China, and giving a detailed description of the dynamic process and mechanism of the spontaneous detonation of explosives and the relative factors, the latest research results of three kinds of explosives reacting with a group of sulphide ores are presented, based on a reliable experimental method. Afterwards, a complete safety criterion for spontaneous detonation assessment, which depends on the hole temperature, the water solution Fe2+ and Fe3+ and the pH value, is established to determine (he safe charging time of explosives in variable conditions. The criterion is verified as correct by a large-scale field test.

Today?s modern mining operations have literally dozens of applications and systems generating data whose information very often, are not crossed or centralized for operational decisions. In remote locations, the information and situation awareness is typically limited to the location itself. In addition, access to that information is limited to a small, select group of personnel that have received training on how to interpret the results. When the situation affects another area of the mine then information is relayed to the applicable areas involved. Few workers have time to analyze the situation thoroughly, to see how it affects the mines symbiotic relationship within all areas of production. If the capability to correlate and measure events and situations that affect your overall key performance indicators are not in place, the opportunity to identify process improvement becomes more difficult.

NATURE OF ORETHE Edna May ore consists mainly of clean quartz of a moderately hard and tough nature. The gold content is &quot;free&quot; and fairly coarse, so that a large percentage is recovered by amalgamation. The ore is practically free from metallic minerals.GENERAL OUTLINE OF PLANT AND TREATMENTThe ore, received from underground in trucks, is broken by a jaw breaker, then stamp-milled, follow~d by partial filie-grinding in pans. Mercury is used both in the mill boxes and pans. The pulp, on leaving the pans, passes over amalgamating tables and is then elevated by tailings pulp to cone classifiers and separated into a sand and slime product, each product receiving a separate cyanide treatment-sand by leaching in vats and slime by agitation and decantation in a &quot;Trent&quot; plant. Gold solutions from both sand and slime are precipitated together by zinc shavings in the usual type of precipitation boxes. Accumulated slime is trucked from the dam to a vortex mixer and from this point receives with the cement slime the same treatment.ORE BREAKING AND TRANSPORTThe ore is trucked from the brace of the shaft along an overhead line to the breaker station. BM37 is a bonanza-grade (&gt;30 ppm Au) ore shoot of the Maria Lode of the Karangahake adularia-sericite epithermal deposit in the southern Hauraki Goldfield. Samples from BM37 contain up to weight percent gold. The ore shoot is a vein that pinches and swells, but is typically tens of cm thick, and locally exceeds 0.5 m in true width. Gold occurs as irregular anhedral electrum grains that range up to 300 microns across and contain 38 to 59 wt&nbsp;per cent&nbsp;Au (ave. 52 wt per cent&nbsp;Au). Electrum occurs in sulphide-rich bands in association with pyrite, chalcopyrite, acanthite, galena, sphalerite, and local covellite. Lower grade samples contain local to rare mckinstryite [(Ag, Cu) 2S] and polybasite [(Ag, Cu) 16 (As, Sb) 2S11]. Pyrite from BM37 is nearly stoichiometric; it lacks the As that commonly occurs at up to percent levels in pyrite from other veins in the goldfield. Acanthite also contains low quantities of trace metals, and typically has less than 2.5 per cent&nbsp;Se. The vein shows complex textures. Bonanza grade sulphide-rich ginguro bands form discrete layers adjacent to the vein margins, and occur as breccia fragments encased by later massive to comb quartz with local quartz after platy calcite. Both the breccia fragments and ginguro bands are cross-cut by later quartz veins. Textures thus indicate that formation of the high-grade portions of BM37 resulted from one or more discrete events that differed markedly from the events that formed the bulk of the vein.

Compared to conventional fully ducted axial and centrifugal fan systems, jet fans can offer a number of advantages for control of smoke during fire emergencies and dilution of diesel engine exhaust products in vehicular and rail tunnels. The success of jet fan applications is the result of a balance between the temperature de-rating of the fan, buoyancy forces and the installation momentum transfer efficiency. This paper provides a case study of a unique application of vertically-mounted jet fans in ventilation shafts used to provide both emergency ventilation and pollution control for an overbuild created by the construction of a series of commercial high-rise residential buildings over a portion of an existing passenger rail line. A detailed evaluation of a set of shafts for one building is presented, including a discussion of the calculation methods used to estimate the required jet fan capacity and the results of the subsequent numerical analysis using Computational Fluid Dynamics to simulate the performance of the jet fans during train car fire emergencies.

Earlier this year, Simplot Phosphates achieved a safety milestone at its Vernal, UT phosphate operations. In March, the entire operation reached achieved seven years without a lost time accident. And the mine operations at Vernal reached the 1 million hours benchmark without a lost time accident. This million hour achievement has been 19 years in the making. This achievement reflects the mine’s strong attitude towards safety, the company said. Safety is an integral part of its culture and requires diligence and considerable work. Attention to even the smallest of details cannot be allowed to slip. This is a record Simplot Phosphates said it hopes to improve on every day.

Due to the international depletion of reserves of traditional high-grade iron ore, the ore resources available throughout the world, especially in steel mills in East Asia, have changed drastically from hard hematitic to more goethitic ore types, such as pisolitic and Marra Mamba ores. The level of impurities and loss on ignition in iron ore has also been slowly increasing. Finally, the proportion of fine material in the available products has increased considerably for most iron ore fines. This has been found to have significant impacts on sinter quality and sintering performance. Magnetite concentrate is typically high in Fe grade and releases extra heat when oxidised to hematite. The amount of magnetite concentrates is expected to increase due to the recent development of a number of magnetite projects. Some of these magnetite concentrates will end in sinter blends. This will not only improve the sinter quality and but also have a beneficial impact on the fuel consumption of the sintering process. However, most of the concentrates available are very fine, which will negatively impact the green bed permeability and consequently the productivity of the sintering process.
