"Spring Creek Mine uses high pressure water to extract the thick Main seam in the Upper Rewanuicoal measures. The extraction method results in the creation of large voids which collapsespontaneously. This can result in the displacement of large volumes of methane, which may create a signifi cant hazard if not correctly managed. A system of automatic dilution doors is employed to mitigate this risk. This paper describes the evolution of the system and gives some examples of its eff ectiveness."

Boliden Tara Mines Ltd, located in County Meath Ireland, produce in excess of 2.5 million tonnes per annum of ore from the underground resource located 40 miles north west of Dublin. The zinc-lead ore is mined using long hole open stoping techniques, with ore treated on site. The operation is continually searching for new ways to improve productivities at the mine, and have employed a detailed data acquisition and reporting system on site to provide feedback around performance. One area where both Boliden Tara Mines Ltd and Orica Mining Services believed improvements could be made was in the application of bulk emulsion technology through an Orica supplied but Tara operated Maxipump Delivery System for all production and development blasting at the mine. Both parties committed to the evaluation of this technology at the mine, however the capital cost and technical and economic changes meant the project needed to be diligently investigated and any improvements documented. The first stage of this project was for Orica Mining Services personnel to visit the operation, and conduct a detailed analysis of the operationÆs performance over the past 12 months. The next step in moving forward was then to commission and utilise the customer operated emulsion system at the mine. This was then followed with the collation and analysis of data from the demonstration period to examine and document the benefits of the system. This paper is a summary of the observations made during the site inspection and the data analysed to date, and has been used in determining operational and financial improvements in the comparison between a Maxipump Emulsion Service and the current ANFO loading systems.

The investigations of the Leningrad ruining lilstitute on this problem are carrying out in the following main directions: 1) The analysis and study of contaminants migration at particular sites of mining development; 2) The working out of theoretical and me- thodical basic principles of aquifers migration tests; 3) The work- ing out of methods of the forcasting of groundwater pollution pro- cesses; 4) The theoretical and uethodical substantiation of hydro- geological observations of groundwater pollution processes; 5) The wort ing out of the measures at the groundwater control and their rational use at the particular sites of mining development. The research that have been carried out by the present moment have covered the wide range of wining and geological conditions -- as from the point of view of specific genetic types of water-be- aring rocks (porous, fissured and fissured-porous), as well as from the point of view of pollution sources (surface waste water basins and underground basins of non-standard water) and the cha- racter of the latter's migration (tracers, physically neutral ef- fluents, heavy or light ones). The combination of specialized mi- gration tests and observations carryed out at uoayi, sites, as well as the following ar~,alysis o:i the data of field study allowed to get useful theoretical, methodical and practical results on the all directions above mentioned, ns the summing up of these results the author proposed the concept of controlled ground :ester polluti on. Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

The stratiform and vein-type pyritic silver-Iead-zinc-copper sulphide deposits of Sunny Corner, NSW are hosted by acid volcanic and clastic sedimentary rocks of Middle Silurian to Early Devonian age. The rocks have been subjected to low-grade regional metamorphism (prehnite-pumpellyite facies) and three phases of folding and four faulting events are recognised. Pyrite is intensely fractured, indicating brittle deformation. Chalcopyrite and galena exhibit deformation twins and slip planes indicating ductile deformation; sphalerite and chalcopyrite show straight and curved grain boundaries and dihedral angles of 120° as evidence of annealing recryslallisation. Sulphur isotope ratios for 22 samples from five deposits range from 1.7 to 10.7 per mil and average 7.4 per mil. Sulphur isotope data are similar to those from other stratiform sulphide deposits in acid volcanic rocks of Late Silurian age in the eastern Lachlan Fold Belt, especially Woodlawn and Captains Flat and together with other features of the mineralisation, suggest a volcanic exhalative origin for the Sunny Corner deposits.

A recent review of resource reports lodged to statutory bodies found geological models with oversmoothed or unrealistic lithological boundaries, many of which do not represent geological appearance in the field, pit or underground drive. The underlying data is often too complex or messy to be incorporated into the resource model and is thus a simplification of reality, to the detriment of the mining operation and its shareholders. Volumes can be over- or underestimated and subsequent grade predictions can be misplaced, combining to cause over- or understatement in resource reporting to stakeholders and statutory authorities. Domain models can be improved with new modelling techniques such as machine learning. However, with poor data or improper use of machine learning techniques, the conclusions can be just as misleading as with current techniques. Machine learning rarely offers a single ‘right’ outcome, rather a range of possible outcomes from the data provided. But importantly it can take advantage of the full richness of data for more informed models. Machine learning also provides confidence in its predictions so that uncertainty in the resultant models can be quantified and passed through for risk assessment during mine planning and operation. This paper will discuss the factors accentuating complexity in deposit modelling: data diversity, structural controls, chemistry, data volumes, process workflows and external non-geological constraints. A case study illustrates the risk of reducing or ignoring complexity, which can result in an overly simplified geological model.

The goal of this project has been to digitise and use drill parameter data from rotary drilling (measurement while drilling; MWD) for a more effective planning of the blasting process in surface mines. A more accurate and reliable digital drilling and automatic hardness identification technology was developed, which includes both a hardware and software system. In the aspect of hardware, we designed a positioning, navigation and hardness perception terminal system which consists of a global positioning system original equipment manufacturer board, programmable logic controller and sensors of measuring major parameters, and a wireless data transmission system which combines digital blasting platform and wireless transmission facilities. In the aspect of software, we developed the navigation software of locating blastholes using LabVIEW and C++, and the blasthole design software using AutoCAD ActiveX (C#) technologies. Northern Star’s Thunderbox Open Pit C-Zone cutback was designed to maximise extraction in a short life excavation; In fresh rock on the western wall this included 20 m to 30 m bench heights with 6.5 m to 10 m berm widths and a 90° pre-split angle. Rock mass quality in this area is generally fair to good, however, there are sections where the dominant foliation runs parallel with the designed pit wall, and more closely spaced, altered, foliation is present: these areas are difficult to predict and prone to failure. Stringent controls and monitoring were required in order to safely access below these areas, in particular the use of slope stability monitoring radar, Trigger Action Response Plans (TARPs), re-entry procedures, visual inspections, drone inspections and day-to-day personnel awareness of the hazard. Robust and comprehensively planned remediation was also required after two significant slope failure events where long reach equipment and rope access techniques were utilised to remediate the areas in question safely, but also in a timely manner, such that expected production targets were impacted as little as reasonably practicable. This combination of rigorous risk management and well planned remediation resulted in a successful and complete extraction of the C-Zone cutback with no impact to operational safety.

The study commenced in October 2012, to understand the basic ecology of the spotted-tailed quoll (Dasyurus maculatus) and to confirm whether the species utilise previously mined land on the Mt Owen mine site that was rehabilitated in 1996 to 2011, as part of their home range. The spotted-tailed quoll is a widely distributed marsupial carnivore that is listed as rare or threatened in every state in which it occurs. The species is a forest-dependent marsupial carnivore that occupies a wide range of habitat types (Long and Nelson, 2004).The first part of the study involved trapping a spotted-tailed quoll and fitting the individual with a combined global positioning system / ultra high frequency (GPS/UHF) radio-transmitter collar (Telemetry Solution – Quantum 4000E). The benefit of the GPS/UHF collar is that it records locations (or fixes) at intervals programmed by the user. The UHF component allows the researcher to locate the individual at a frequent interval, and the Quantum 4000E also has a remote download function, enabling the GPS data stored in the collar to be wirelessly transmitted between the collar and researcher. The benefit of this new technology is that the animal does not need to be repeatedly captured to retrieve the GPS data.Daytime searches are also undertaken using the UHF receiver to locate the individual and locations of daytime denning and latrine sites. The location of each den site is recorded using a hand-held GPS. Data downloaded from the GPS collar is then imported and mapped using computer software program ARCGIS (geographic information system), to overlay the data points onto aerial photography.Results demonstrate that threatened species will utilise rehabilitation areas post mining. The data presented will be used to develop a greater understanding of threatened species ecology in and around rehabilitated areas, which will assist in the planning and design of native forest rehabilitation areas.CITATION:York, P, 2014. Poorly judged or under-appreciated change plays a role in many major accidents. In particular, operational changes related to continuous improvement, maintenance and introduction of new technology can lead to unintended safety (and other) consequences. This paper provides an overview of the role of management of change in preventing unwanted impacts on safety, community relationships and the environment. It draws on a range of incidents covering mining, minerals processing, refining and smelting where inadequately assessed or ineffectively implemented change led to significant safety, environmental or community consequences. The paper will also highlight cases of well-managed complex change and the underlying model of decision-making in place to make those decisions. All these examples are drawn from Atturra’s experience in response to accidents or working with sites to improve their management systems. The paper will report on the challenges of applying conventional risk assessment approaches to complex or technically significant change. These typically result in either onerous and lengthy change procedures that are prone to workarounds or poor implementation. Conversely, ‘light touch’ or efficiency driven change processes present the risk of intended consequences by failing to bring appropriate expertise to bear in decision-making. Such balances can be resolved by considering the designation of ‘technical authority’ with appropriate expertise to consider the change and deconflict any production, cost or schedule trade-offs. Finally, the paper will reflect on the key components of management of change systems in the context of major accidents, highlighting good practice in how they are implemented. This will consider the integration of existing corporate processes, risk management and organisational structures into management of change systems, not just the content of change forms and procedures.

To effectively plan for closure, we must have an end-point in mind. To undertake meaningful rehabilitation, a post-mine land use (or uses) must be known. However, it is becoming more apparent that the end-point or “goal-posts” are shifting - and may continue to do so throughout the life of a mine. As well as being safe, stable and not causing environmental harm, post-mine land uses should always be sustainable, achievable within the given land capability, and they should involve stakeholders in their design and development.The majority of mines in Australia, and around the World, were historically expected to return disturbed land to either native ecosystem or agriculture/forestry practices. However, examples of highly successful final land uses suggest that today’s focus on closure includes not only minimising future liability and impact but also – and importantly – a move towards the creation of economic opportunity (Murphy and Heyes, 2016).CITATION:Durbin, J, 2018. A central aim of rehabilitation is to ensure that disturbed land is returned to a post-mine condition that is safe, stable and self-sustaining. This paper describes initial results from a grazing trial at the New Acland open-cut coal mine in southern Queensland (-27.27 lat, 151.72 long) that aims to assess the sustainability and viability of cattle production from rehabilitated mined land compared to production from surrounding unmined lands (Newsome 2014). The grazing trial was established in 2013 by the Acland Pastoral Company, a subsidiary of New Hope Coal, on lands previously used for dairying, beef and crops before open cut coal mining of the Walloon coal measures began in 2002. The area has a mean annual rainfall of 640 mm. Situated in north-west Queensland, Australia, the Ernest Henry Mine (EHM) has been producing copper and gold since 1996. Owned and operated by Glencore Xstrata, ore is currently sourced from a newly developed underground sublevel cave (SLC) mine below the old EHM open pit (the pit). Ore feed to the concentrator is supplemented by satellite open pits comprising the Mount Margaret operation.The transition from open pit to a producing underground mine involved a complex extraction sequence for the first two production sublevels on RL1650 and RL1625. Underground mining during cave initiation was further complicated when south wall instability prevented completion of the final open pit shell. The transition mining phase included mass firing of an inclined crown pillar between the initial SLC void and the pit followed by a combination of transverse and longitudinal SLC mining.The inclined crown pillar mass firing was the first significant milestone in the extraction of the transition ore block. Comprising approximately 890 000 t of in situ material; 70 000 m of drilling was charged with 351 t of emulsion and 6026 electronic detonators to initiate the single mass firing. The firing contained five direction changes over 19 seconds making it one of the largest and most complex underground blasts ever conducted in Australia.Risk management was integrated into every facet of the technical and operational planning and execution of the transition mining phase, whilst maintaining daily mine construction and production commitments. Consumable materials used in mines are often transported to the working areas down shallow angle decline shafts, typically at anywhere between 10¦ and 30¦ to the horizontal. The winding equipment is usually a relatively small single-drum winder and the material is transported in one or more cars, running on rails in the decline. The winding rope is attached directly to the cars, which are mechanically coupled together. For small rock hoisting declines, the cars are replaced with hoppers or skips. The winder is normally of basic design, fitted with only the minimum required safety devices and very elementary brake control. Since these winders are not ælicensedÆ and are usually located in remote areas, the standard of maintenance on the installations is often poor, leading to frequent accidents, resulting in runaways, derailments and broken ropes. These problems can be overcome easily with the application of the rope haulage principle. When large volumes of ore have to be transported to surface, belt conveyors are used on shallow angle inclines, or high-capacity friction winders are used in vertical shafts. Conveyors are limited to inclines no steeper than 14¦ and vertical shafts with large friction or drum type winders are extremely capital-intensive. Rope haulage systems can be used on decline shafts where the inclination is too steep for belt conveyors and too shallow for conventional drum winders. Rope haulage systems also offer a variety of safety, technical and cost advantages over other winding and transport systems. This paper describes the adaptation of the rope haulage principle to modern requirements and, combined with 21st century electronic technology, explains how the principle can be used as an efficient, safe and practical solution to some problems associated with the transport of materials, personnel and ore in mines. The authors describe the designs, methods of operation, safety aspects, automated control, loading and unloading facilities of modern rope haulage systems used for transporting personnel, material and ore. This paper considers a variety of systems already designed for different applications, from small material-only installations to multi-purpose and large rock-hoisting systems with payloads as much as 180 t. It also describes in more detail, a material transporting system recently commissioned in South Africa.

Closure options under consideration at some sites include backfilling mined-out pits with waste rock. Backfill may include mineralised and non-mineralised waste rock. Following groundwater rebound, flow-through conditions may develop in the pit post-closure. The backfill located in the saturated zone below the water table may release solutes under such conditions. It is therefore important to understand the potential impacts that backfilled pits could have on postclosure groundwater quality.Geochemical characterisation programs are the foundation for assessing the potential chemistry of water that has contacted mined wastes. Such programs must be designed to obtain data relevant to the closure options under consideration. The purpose of this paper is to therefore investigate the hydrogeological characteristics of tailings, coarse and mixed rejects from coal processing to enable better understanding of the potential for undrained behaviours in coal mine spoil piles. This is achieved by analysing the results from permeability and oedometer testing of tailings, coarse rejects and mixtures thereof. Focus is given to the effect that tailings content has on the behaviour of mixed rejects, the transition point between drained and undrained material behaviours, and the evolution of properties with increasing stress that would occur with increased depth of burial in a mine spoil pile. The implications of the findings are discussed with respect to spoil pile stability.

The application of tunnel boring machines (TBM) for reef mining is a unique practice with benefits over current mining methods given the right geology and geometry of the orebody. The Stillwater Mining Company (SMC) of Nye, Montana, USA operates two mines for obtaining platinum metals. The operation is currently using a 5.5 m diameter open-type machine for further mine development, as well as another TBM that they own.The latest 7.1 km bored tunnel, known as the Blitz Tunnel, will be routed parallel to the ore deposit in igneous mafic intrusive rock, and will be used for the life of the mine operation. It will be used to map the ore deposit, and provide a rail haulage route.Detecting the reef in relation to the TBM requires careful analysis. Diamond core drilling occurs ahead, above, and alongside the machine at every 150 m. The core is drilled, logged, and interpreted on the spot, concurrent with boring. "This paper is focused on blasting activities undertaken as part of the Pacific Highway Upgrade – Tintenbar to Ewingsdale – project (T2E). This is a ‘priority 1’ Pacific Highway road construction project in New South Wales (NSW), to be completed by mid-2015 (weather permitting). T2E will provide approximately 17 km of dual carriageway road starting at the northern end of the Ballina bypass and extending to the Ewingsdale interchange, and will complete the existing dual carriageway road between Ballina and the NSW/Queensland border.Roads and Maritime Services (RMS) (formerly the Roads and Traffic Authority) is delivering the project on behalf of the NSW state and federal governments. In October 2011, RMS awarded a contract to Baulderstone Pty Ltd (now Lend Lease Engineering Pty Ltd – Lend Lease) to design and construct T2E. One of the key elements in this project is the St Helena Tunnel.In order to avoid the steep grades of St Helena Hill, eliminate a well-known accident black spot and maintain the aesthetics of the Byron hinterland escarpment, RMS and Lend Lease prepared designs to construct a 434 m long dual carriageway tunnel, 46 m below the ridge line of St Helena Hill. Lend Lease elected, where geotechnical conditions permitted, to use drill and blast methods to excavate the full tunnel profile, instead of traditional mechanical excavation techniques.Since all blasting locations (both inside and outside the tunnel) were in close proximity to nearby residents, the NSW Department of Planning and Infrastructure imposed strict air blast overpressure (airblast), ground vibration (vibration) limits and limited working hours. Both RMS and Lend Lease recognised that these restrictions meant that nearby residents may be exposed to blasting activities for longer than was necessary. As a result, after close consultation and agreement with the nearby residents, RMS applied successfully to the NSW Department of Planning to increase the airblast and vibration limits, as well as the working hours.Working under these new conditions, Lend Lease employed a range of best practice drill and blast technologies provided by both Orica Australia Pty Ltd and Atlas Copco Mining and Rock Excavation to improve the safety and efficiency of blasting activities and to minimise the potential negative impacts of the blasting works on affected residents. These technologies included: the use of a continuous environmental monitoring system to record the results of the blast events and provide quick and easy reporting to key stakeholders to address RMS specification requirements electronic detonators to enable accurate hole detonation timing for vibration compliance and to give project engineers the flexibility and precision to alter each design to suit the geological profile of each face the use of two software programs and advanced drill rigs to enable the efficient design of blasts, electronic transfer of designs to the drill rigs, as well as in-hole monitoring and recording of drilling performance the use of a specialised bulk emulsion system to deliver fully coupled bulk explosives into face blastholes to maximise fragmentation, and decoupled bulk explosives into perimeter blastholes, to reduce overbreak and deliver a cleaner profile.By taking a progressive approach towards the environmental restrictions and the work hours imposed, as well as utilising best practice blasting techniques, RMS and Lend Lease were able to blast with confidence and achieve their production deadlines and reduce the total cost of the project.Most importantly, RMS and Lend Lease were able to meet their environmental obligations and work constructively with the nearby residents to ensure that the blasting was undertaken as safely and efficiently as possible without delay or negative impact.CITATION:Dowling, M, Domotor, R and Miller, D, 2014.
