Fortunately, few mining professionals have dealt with what all mining professionals fear---a mine disaster. Unfortunately, many have dealt with ?accidents? which may or may not claim lives but nevertheless rise to the level of a ?disaster? given the fallout of liability and litigation in today?s society.1 The undersigned authors published an article some ten years ago regarding crisis management2 based upon their collective first-hand experiences in actual disasters to that point in history. Unfortunately, disasters have occurred since that time, prompting legislation, and a fresh analysis and expansion of those basic principles is certainly in order. Without question, no two disasters are ever alike, yet general guidelines for forming an effective plan of action can be formulated?and if a mining operation is to have any chance of survival after a disaster, such a plan is vital. The primary concern as any disaster evolves remains as stated a 2003 SME article3--the protection of human life. From that point, the focus moves to any assistance that may be needed given the magnitude of the situation. Permeating throughout this process is "damage control." The first step in dealing with a disaster, however, begins long before any threat of danger arises?formulation and implementation of an Emergency Response Plan.

Kaolin clays from five different commercial deposits in Georgia were studied to understand the variations of zeta potential and related fundamental properties. The results suggest that higher negative zeta potential is related to exchangeable or soluble calcium ions in the clays.

The environmental liabilities of coal waste disposal sites can have a major impact on a mining project's economic viability. Developing a waste disposal site is no longer simply an exercise in completing paperwork to obtain a permit, with the proliferation of environmental regulations in recent years, managing disposal sites now requires design creativity, short term and long term planning and detailed monitoring to maximize the benefits to the mining company. Operating costs and future capital costs for expansion of existing sites, new site development and regulatory compliance should be reviewed periodically and considered in the overall management plan. This paper discusses various management methods that are being employed in the industry to improve waste disposal operations both environmentally and profitably.

"The DC Clean Rivers (DCCR) Project is a 25 year, $2.6 Billion court ordered project to reduce combined sewer overflows (CSOs) discharging into the Potomac and Anacostia Rivers and Rock Creek, and also reduce flooding in the northern areas of the District of Columbia. Like all massive infrastructure programs, DCCR faced technical challenges, management challenges, risks of cost and schedule overruns, procurement challenges, and financing challenges. While DC Water is no stranger to large capital improvement projects (CIP), this CSO Program represents a once in a lifetime event for the Owner requiring the assembly of a single purpose team to implement. Some of the DCCR Project team’s most successful accomplishments to date are attributable to its introduction of early contractor involvement (ECI) to thoroughly vet the work from the perspective of the future constructors, development of functional baselines to share risks that are difficult to price and reduce reliance on geotechnical baseline interpretation during tunnel excavation, and attainment of financing through Municipal Century Green Bonds based on using design criteria of a minimum of 100 year life for the structures to establish intergenerational equity and fairness. This paper will discuss these innovations in detail and provide some sets of recommendations for the underground industry on when and how to use different approaches to better manage projects of this magnitude. INTRODUCTION The District of Columbia Water and Sewer Authority (DC Water) is an independent multi-jurisdictional authority that was established in 1996. Its primary mission is to provide water distribution services to more than 640,000 residents in the District of Columbia (District), and sewer treatment and disposal to 1.6 million residents of the District, Montgomery and Prince George’s counties in Maryland and Fairfax and Loudoun counties in Virginia."

An architect representing the Atlanta office of the Brick Institute of America periodically presents a slide lecture to interested groups entitled "The First 10,000 Years of Brick". The title signifies the fact that brick, man's oldest manufactured building material, has been around for quite some time and will continue to be utilized by architects and builders in the foreseeable future. This is not to say that the brick industry can rest on its laurels and ignore the competition presented by newer more innovative products. The industry will have to be equally inovative and market oriented in order to maintain its market share of the predicted construction boom between now and the year 2000. The implication is made, however, that brick can be produced under rudimentary conditions and that the desire to use the product has been long standing. In addition, history has sham that there has been plentiful clay available to produce the product. This paper is an attempt to assess the climate in which the brick industry will operate during the next 26 years. An analysis of regulatory conditions affecting the manufacture of the product will be followed by an assessment of the materials environment in which the industry will be operating. These factors are then reconciled with a projected demand for the product to determine the ability of the industry to satisfy future demand for brick. From a mineral point of view this paper will address itself generally to the availability of and demand for clay. However, other than a brief overview of the uses of other specific types of clay, it will concentrate on the clays utilized in the production of structural clay products, specifically brick.

The 2,250-MW, coal-fired Navajo Generating Station located in Page, AZ draws water from Lake Powell using submersible pumps installed in five inclined shafts. Drought threatens to lower the reservoir to below the existing intakes. A system of new steel-lined intake shafts located on the same site was required for uninterrupted plant operation. The small size of the site, and the need to keep the existing system in operation, required detailed designs and placed significant limitations on construction equipment and activities. The Navajo Sandstone that forms the near vertical shoreline of the reservoir is 98 percent very fine grained quartz, highly abrasive and contains highly fractured intervals that presented numerous challenges to drilling the new 152-m (500-ft) deep, 109-cm (43-in.) diameter intake shafts inclined 23° to 26° from vertical. Spatial constraints, submersible pump design and operational criteria required that each shaft have a unique inclination and orientation and hit a small breakout target located 76 m (250 ft) under water. Sophisticated drilling equipment and techniques as well as state of the practice downhole survey technology and methods were used to make adjustments and maintain drilling accuracy with less than 1 percent deviation. Environmental requirements mandated water as the drilling fluid and grout was not allowed in to the lake under any circumstances. The shafts were videotaped and the breakout locations were examined using an underwater remotely operated vehicle (ROV) to verify shaft locations and rock conditions, and to guide placement of pneumatic packers to ensure grout did not transmit to the lake during the steel liner installation. In anticipation of the invasion of quagga mussels, the shaft design included an allowance for a chemical dosing system and a copper-rich alloy at the exposed portions of the steel liner.

"Australia’s largest rail freight network operator, the Australian Rail Track Corporation (ARTC), has selected GE Transportation’s Movement Planner solution as the key technology enabler for its Hunter Valley-focused network control optimization program.ARTC’s Newcastle-based network control, is the nerve center of Hunter Valley rail operations, where locally based train control professionals make thousands of decisions every day to safely support the annual movement of more than 24 kt/h (26,400 stph) of freight, and tens of thousands of coal, freight, grain and passenger trains each year.ARTC plans to transform its train control function by integrating and then building on Movement Planner, an advanced software solution by GE Transportation that provides real-time rail traffic planning and optimization to enable freight to move more efficiently. “The Hunter Valley rail network is one of the busiest and most complex in the country, supporting the largest coal export port in the world,” ARTC managing director and chief executive officer John Fullerton said.“As bulk freight demands grow, rail operators like ARTC and supply chains like the Hunter Valley coal network need to be creative in the way they respond so the Australian economy doesn’t miss out. That’s why we have conducted a worldwide search to find globally proven solutions that will improve our network productivity and efficiency,” Fullerton said.The Movement Planner solution considers multiple factors including train schedules, trafficcontrol systems and train movements relative to each other and then develops an optimized traffic plan for the trains throughout the network. Existing GE customers have benefited from increased visibility of network operations, higher velocity, less dwell times, and improved on-time performance across their networks."

"Phosphate" is a misnomer as often used. The word is increasingly used to refer to any material containing phosphorus - an element essential to any form of life. Phosphate rock, a mineral - fluorapitate - is recovered from Florida's sedimentary deposits in particle sires ranging from 0.1 to 13.0 millimeter. This product of mining, washing, screening, and flotation is commonly called "phosphate". Its phosphorus content, measured and reported as P2O5, varies from 29% to 35%, and is essentially insoluble. "Phosphate" is also used to describe products containing soluble phosphorus obtained from acidulation of phosphate rock with mineral acids; primarily, sulfuric acid. Many miners have nearby chemical plants wherein phosphate rock is converted into upgraded products such as phosphoric acid, superphosohates, and ammonium phosphates with 45% to 70% P205, soluble and suitable for use as fertilizer. The rock and upgraded products are all commonly called "phosphate"; the plants, "phosphate plants' and the producers collectively are known as the "phosphate industry". The public in Central Florida generally loses identification of the individual operators that include major corporations such as IMC. Williams Companies, W.R. Grace, Mobil, Embark, U.S. Steel, Borden, American Cyanamid and others. They all become just a "phosphate company".

An ore zoning technique developed by Climax geologists during the past forty years has been used to contour drill-hole assay values on cross-sections for fourteen porphyry molybdenum deposits in western North America, including the Climax and Henderson orebodies. Detailed geological studies of these large, complex deposits are essential in determining ore controls, which are the factors most critical to the zoning of MoS2 assays and to conceptualizing geologic models. The technique provides a detailed representation of the size, shape, grade distribution, and geologic continuity of the individual deposits. It demands a disciplined approach and adheres to rigorous rules while maintaining flexibility for adaptation to widely varying geologic conditions. By defining the limits that guide and control manual, computer-assisted conventional, and geostatistical reserve estimates, it exerts a strong geologic influence on the estimation process. A combination of ore zoning and geostatistical analysis of molybdenum deposits produces a significantly more accurate, precise, and defendable geologic reserve estimate than other methods.

The reduction roast-ammonium carbonate leach-hydrogen reduction process for laterites has been under development by Sherritt Gordon for the past 20 years. The process can treat high iron or low iron laterites, with some process modification, and gives extractions of 88-93% Ni or 82-88% Ni, respectively. Five campaigns on a 1 tonne per day Pilot Plant and four campaigns on a 35 t/d Demonstration Plant have been carried out on laterite from three different deposits, and many other laterites have been investigated in the laboratory. The Marinduque Surigao Nickel Refinery in the Philippines was designed on the basis of this work. The process comprises roasting to selectively reduce the nickel and cobalt in the laterite, leaching the calcine in ammonium carbonate to dissolve the metals, recovery of cobalt by precipitation of a mixed metal sulphide, concentration of nickel from the carbonate solution by basic nickel carbonate precipitation and recovery of high purity nickel by hydrogen reduction of nickel from ammonium sulphate solution. Ion exchange has been developed as an alternative to BNC and for recovery of trace nickel from BNC barren liquor. Details of the development of some of the process steps through the laboratory to commercial plant are discussed. The process metallurgy has been confirmed in the commercial plant.

Predictions of delayed environmental impacts from mining—degradation that won’t begin for hundreds of years, but will then continue for thousands—present new permitting challenges. Examples include evaporative concentration in pit lakes, which can eventually cause metals concentrations to exceed human and wildlife toxicity standards, and delayed degradation of groundwater by slow moisture percolation through acid-generating tailings and waste rock. Given the time-value of money, the present worth of such delayed impacts is insignificant, yet they are unacceptable undercurrent laws and cultural values. Long-term financial set-asides offer a possible solution, first funding infrequent, long-term monitoring, followed by more expensive mitigation when the set-aside has matured.

Freeport-McMoRan Copper & Gold, Inc. (FMI) will soon begin copper production from the Dos Pobres and San Juan deposits near Safford, Arizona. Ore reserves total 615 M tons grading 0.36% copper, predominantly as chrysocolla. Copper extraction will be by leaching with sulfuric acid. Fluor Canada Ltd. provided EPCM services to design an ore handling and conventional SWEW facility capable of producing a nominal 240 million pounds per year of cathode copper. The ore handling facility will process 114,000 tons per day of ore through three-stage, open-circuit crushing followed by agglomeration and product placement on the permanent leach pad. Stacking of ore on the leach pad will utilize Terra Nova Technologies' "Super Portable" mobile stacking system. The stacking system fleet consists of 13 self- propelled, crawler-mounted mobile conveyor units, each with 72-inch-wide belting and a capacity of 6,800 tons per hour, including ten 250-foot-long portable conveyors, a horizontal feed conveyor, a 285-foot-long horizontal conveyor and a 220-foot-long radial stacker. PROJECT BACKGROUND Project History The Safford Leach Project is located in the Safford mining district in Graham County, Arizona, approximately eight miles northeast of the town of Safford and 170 miles east of Phoenix. The 3,360-acre property is located in the Gila River Valley, immediately south of the Gila Mountains, at elevations between approximately 3,000 and 5,400 feet above sea level. The Safford Leach Project is comprised of the Dos Pobres and San Juan deposits, two of five known copper ore deposits in the Safford mining district.

Economical dewatering of ultra-fine (minus 74 m) clean coal slurry produced in column flotation to a 20 percent or lower moisture level will be an important step in successful implementation of the advanced fine coal cleaning processes. The main objective of the present study was to evaluate efficiency and effectiveness of three dewatering techniques, namely, hyperbaric, vacuum and screen bowl centrifuge, in dewatering of ultra-fine (D50 -25 m) clean coal slurry produced by the 'Ken-Flote' columns operating at the Powell Mountain Coal Company, St. Charles, Virginia. he hyperbaric filter using 3 bar pressure and 1650 cake formation angle, provided a filter cake with 23.6 percent moisture with a throughput of 800 Kg/m2-h and air consumption of 868 Nm3/t (460 scfm/t). The vacuum filter with 30 percent submergence provided a filter cake with 27.8 percent moisture with a throughput of 122 Kg/m2-h. The screen bowl centrifuge provided a dewatered product containing 32.9 percent moisture at a feed rate of 450 Kg/hr, however, the solids capture was only 65 percent. Addition of 1 Kg/t of a cationic surfactant provided filter cakes with 19.1 percent, 23 percent, and 34 percent moisture using hyperbaric, vacuum, and centrifuge, respectively. Similarly, addition of 0.5 Kg/ton of copper chloride lowered filter cake moisture to 20.7 percent and 24.8 percent, using hyperbaric and vacuum. With the centrifuge no improvement in filter cake moisture was noted. However, use of 80 Kg/ton of 'orimulsion' in the centrifuge provided dewatered product containing 26 percent moisture, and the solids capture increased from 65 to 95 percent. The dewatered product appeared dry and was easily handleable.

The Grand Forks Energy Research Center and Commonwealth Edison of Chicago jointly conducted tests in which 400 tons each of subbituminous coal and lignite were dried in a commercial scale dryer, oil sprayed and cooled, then shipped in open-top rail cars from Pekin, IL to Grand Forks, ND and stockpiled. Cars containing raw coal and dried coal that had not been oil sprayed were also transported for comparative purposes. The tests were conducted in August and November of 1974. The subbituminous coal was dried from 26 to 16 pct moisture and the heating value was upgraded from 8,420 to 9,650 Btu/lb. The lignite was dried from 39 to 22 pct moisture and its heating value was increased from 6,420 to 8,300 Btu/lb. Before loading subbituminous coal was cooled to 115° F and sprayed with oil at a rate of from 2 to 6 gal/ton. Similarly, lignite was cooled to 85° F and oil sprayed at rates of from 1 to 2 gal/ton. Both shipments arrived in Grand Forks with a minimum of difficulty. The subbituminous coal was subjected to 2 inches of rain during transit with no change in average moisture content. The cars containing dried and oil treated coal had less material lost to wind than either cars containing raw or untreated dried coal. With both dried lignite and subbitnminous shipments, a moderate increase in temperature was measured during transit. With the subbituminous shipments, ignition occurred near poorly fitted bottom dump doors, but was limited to a very small area. The dried lignite was shipped when ambient temperatures were below freezing, and frozen coal was noted around the edge of the car. At Grand Forks, the coal was stockpiled using proven techniques and has remained stable to this date.

Over the last three years there has been an increase in mining congress and conferences focusing on water management. Water in mining is a key concern for mine managers and operations, and we are now seeing this operational dilemma escalated to the level it deserves. According to Frost & Sullivan research, mining water and wastewater treatment and its related technologies will be a U$3.6 Billion global market in 2016, which translates the importance of this issue. Looking at the current mining trends, some factors increased the importance of water in mining operations, including: ? Future mining investments will be located in geographical locations where water is scarce (water stressed areas) ? A decline in grade will increase the intensity of mineral processes and therefore increase the use of the water ? Increased sustainability and regulations will extend the control/monitoring of water during the entire mine life cycle representation of the mine water flow system. A water balance has several benefits/objectives in a mine operation: ? Mine planning/simulation of mining scenarios/climate change Production optimization ? Minimizing strategic and operational risks ? Social license to operate ? Water-smart mine planning ? Target: water neutral mines ? Benchmarking within and between sites ? All mine operators using explosives want to maximize explosive performance while minimizing blasting costs. Use of bulk explosive products such as AN/FO, heavy AN/FO, slurries and emulsions has greatly improved blasting efficiency over the years. Many operators have improved energy output from these products by adding aluminum. A major problem facing them is the fluctuation in aluminum price. The question therefore becomes, when is it economically advantageous to use aluminum in bulk products? This paper answers this question by first reviewing the energy output from the main types of aluminized and non aluminized bulk mixed explosives and then by showing the relationship between the different products on a cost and energy basis. The paper finally presents examples of charts and tables which will aid mine operators to conveniently select the most economically correct bulk explosive, including the level of aluminum addition.

Methane has been a hazard since the beginning of coal mining and has influenced, more or less, every aspect of coal mine design. It is receiving increased attention lately because the rate of methane emission in coal mines is increasing as increasing demand for coal leads to exploitation of deeper seams and the use of higher productivity equipment. Consequently, the government and private industries have directed a great deal of effort towards understanding and solving the problem of methane emission in coal mines. In the United States, Consolidation Coal Company has pioneered research in this area since the early 1950's.1 Currently this work is being expanded by Consol through the Mining Research Division of the parent company, Continental Oil (Conoca), to find new techniques for degasification of virgin areas as well as control of methane in gob areas. Basic reservoir properties of coal seams, such as gas content, reservoir pres-sure, diffusivity, porosity and permeability are better understood today largely through the efforts of the U. S. Bureau of Minese2,3,4 It is possible now to identify and model the basic mechanism of gas emission from coal and to forecast steady as well as unsteady state emission of methane in coal mines with some degree of reliability. Combining this knowledge with any ventilation simulator should often identify problem areas in advance and permit adequate methane control measures to be planned.5 Another recent innovation in methane control is the horizontal hole drilling technology. The U. S. Bureau of 1-1ines and the Mining Research Division of Conoco have independently developed techniques for drilling horizontal holes 300 to 600 m deep6 The Bureau's method employs a rotary drill string while Conoco uses a non-rotating string with a down hole drill motor. It is the authors' view that the latter provides more positive control on bore hole deviation both vertically and laterally and appears to be faster, though somewhat more difficult to operate. Butler Taconite, operated by the Hanna Mining Company, is an Iron ore pelletizing operation that is located on the Western end of the Mesabi Iron Range in Northeastern Minnesota. The operation mines low-grade taconite ore and through primary crushing, two stage grinding, magnetic separation, agglomerating and indurating, produces 2.6 million annual tons of high quality, 66% iron pellets. Mining and stripping is done with 13 - M100 trucks and 5 - 12 cu. yd. shovels. The operating processes in the concentrator and pellet plant are primarily con¬trolled through a centralized control room and monitored by a PDP l140 computer. During 1969 and 1970, a re-organization of the Company took place. Through this re-organization, each mine unit was analyzed and studied to determine the proper staffing of the internal departments. At this time, it was decided to continue with further detailed studies of the Maintenance Department with the intent to reduce maintenance costs. Maintenance costs at the time of the re-organization were 38.6% of the total mine operating cost. The preliminary study indicated that the Maintenance Department had a system of maintenance, as a matter of fact, it had a separate system in each sub-maintenance division and in some of the sub-divisions each foreman had a system of his own. Therefore, from the data collected, it was decided to develop one system of maintenance that could be utilized throughout the entire operation. To determine the type of system suitable to all phases of the operation, detailed time and method studies were conducted. All types of maintenance tasks performed at the operation were studied. The data collected was analyzed and the recommended improved methods were reviewed with the Maintenance Superintendent and his staff. The new improved methods that were accepted as feasible, were introduced into the system immediately.

"The Northeast Ohio Regional Sewer District (NEORSD) was required under the Federal Clean Water Act and U.S. Environmental Protection Agency’s (U.S. EPA) Combined Sewer Overflow (CSO) Control Policy to develop a Long Term Control Plan (LTCP) to reduce or eliminate overflows from its permitted CSO outfalls. The LTCP was completed in 2002. Key components of the plan include: 26 miles (42 km) of deep storage and conveyance tunnels providing a total CSO storage volume of 325 million gallons (MG) (1.2 billion liters); over 30 miles (48 km) of consolidation sewers to route CSO control flows to the tunnels; and associated diversion/inflow control structures and drop shafts. Final design and construction of the initial phases of the LTCP are completed or underway. Advanced planning of the final phases began in April 2013. This paper summarizes this advanced planning process and, specifically, how hydraulic performance and tunnel design and construction considerations were balanced to eliminate unnecessary projects and shafts, reduce implementation risks, and reduce program costs by approximately $650 million. BACKGROUND The Northeast Ohio Regional Sewer District (District) serves an area of approximately 350 square miles (SM) (907 km2), including combined (81 SM; [210 km2]) and separate (269 SM [697 km2]) sewer systems that discharge to three wastewater treatment plants in the greater Cleveland area. The service area, shown in Figure 1, is divided into three districts—Easterly, Westerly, and Southerly—each having its own interceptor system and treatment plant, ranging in wet weather treatment capacities from 100 to 735 million gallons per day (MGD) (378.5 million to 2.8 billion L/day). The system includes 124 permitted combined sewer overflow (CSO) outfalls overflowing into the region’s tributary streams, Cuyahoga River, and Lake Erie."

Rockburst is defined as a sudden and violent explosion of rock on the surface of a rock excavation. Rockburst has been regarded as the least understood and most feared hazard problem for underground rock excavations. Traditionally, rockbursts were classified into the strain, pillar, and fault-slip categories. However, rockbursts are also known to occur at stress levels well below the rock yield strength, in mines where the rock is far from brittle, or without the presence of geological discontinuities. A plausible explanation of rockbursts is buckling, which has been identified as one of the main mechanisms for rockbursts. It is observed that rockbursts are often induced by dynamic loadings (pulses) from adjacent blasting. Therefore, the present paper studies the effect of pulse shape and amplitude on pillar rockbursts induced by dynamic loadings from blasting using the method of numerical analysis. The governing equation of a pillar’s motion is set up and the mechanism of pillar rockbursts is investigated through a practical example. The results show that it is not the shape of a pulse but the impact energy to decide the occurrence of pillar rockbursts. If all other parameters remain the same, pillar rockbursts are more likely to occur when the pulse duration is longer. The parametric resonance and the perturbation of dynamic loadings are also found to play an important role in the occurrence of rockbursts. These insights would provide invaluable clues about the mechanism and for better control of pillar rockbursts.
