"The Little Lake Harris (LLH) Bridge was constructed in 1951 to replace the eastwest County Road 48 Bridge connecting the towns of Howey-in-the-Hills and Astatula in Lake County, Florida, United States. The 78-span bridge carries two lanes of traffic, is 3,130 feet long, and 36.1 feet wide. The concrete deck is supported by steel I-girders carried by one row of 18-inch square precast concrete driven pile. In the late 1980's, slight settlements were observed in several bents. In 1990, Florida Department of Transportation (FDOT) initiated annual survey of deck elevations during bridge inspections to monitor settlement. Due to the slow nature and magnitude of settlement, substructure instability was not a major concern to the department. However, public concern over the settlement and rough ride of the bridge continued to rise. FDOT contracted Kisinger Campo & Associates Corp. (KCA) to provide the needed design to correct the dips in the riding surface and minimize future settlement. KCA's initial design was based on the concept of ""helper bents"" at the settled bents in order to restore the bridge to original elevation. Estimated construction cost for seven helper bents was about $3.3M. FDOT requested evaluating other alternatives to reduce the cost of the bridge restoration. KCA involved the geotechnical consultant, Terracon Consultants, Inc. (Terracon), to perform a peer review and provide an economical alternative solution. Jacking and shimming the bridge was determined to be both technically viable and economical with a total cost of less than $300K. The design was completed in March 2010 and construction was completed in February 2011. This paper provides an overview of the LLH Bridge history, summarizes the evolution of the design methodology including the findings of the geotechnical study and presents lessons learned during the design and construction of the project.INTRODUCTIONThe Safety and Preservation of the State Highway System is the top priority of the FDOT. Therefore, FDOT initiated a bridge management program that starts with conducting biennial safety inspections performed by Certified Bridge Inspectors (CBI). The inspection reports together with the CBI’s recommendations are reviewed by a registered Professional Engineer (PE) and work orders are issued with priority ranking depending on the severity of the bridge deficiencies. If the scope of work is considered beyond the in-house crews’ capability, the work is then included in a contract for and bid. LLH Bridge was a little different. It was initiated in direct response to number of complaints the FDOT District 5 Structures Maintenance Office (D5SMO) received regarding settlement of the Little Lake Harris Bridge."

This publication by the Federal Bureau of Mines contains the free-swelling indexes for 2,812 samples of coal received from January 26, 1948, to July 15, 1957, and the Hardgrove grindability indexes for 2,339 samples of coal received from June 23, 1947, to July 15, 1957. These data consist of the origin of the sample, bed name, and analysis. Summaries show the high, low, and average values for each bed. The free-swelling index is a relative measure of the caking properties of a coal when burned on a grate, and the Hardgrove grindability index is used to estimate the relative ease of grinding coals. Development and standardization of these test procedures were at a much later date than those for the more familiar tests, such as proximate and ultimate analyses and calorific value determinations. The newer tests are valuable in supplying supplemental information, particularly for assisting in evaluating coals used in certain types of combustion processes.4/ Data from such tests are important to designers of burning equipment, operating engineers, and purchasing agents. Data from these newer tests are scarce and scattered through a number of publications. Special projects were started to determine the free-swelling and grindability indexes, in addition to the regular analyses on the tipple samples collected by Bureau engineers. Since the start of these special projects, all the coal fields of commercial importance have been adequately covered in the sampling program.

The spatial distribution of copper grades at the Mansa Mina project is highly variable due to the presence of high grade copper/silver/arsenic breccia bodies. Copper grades within the breccia bodies tend to average about 4.5% copper while grades in the surrounding stockwork average about 1.0%. The breccia is known to occur in the vicinity of significant controlling structures, however, the outline and therefore the thickness of the bodies are far from regular. Due to the high grade of these bodies a significant proportion of the value of the project is derived from the breccia tonnages. The irregularity in the outlines/thickness translates into an uncertainty in the breccia tonnage and therefore in the value of the project. Acknowledging the economic importance of the breccia bodies and the risk associated with the breccia tonnage uncertainty, significant additional drilling has been proposed to both understand the spatial distribution of the breccias and reduce project risk. The costs associated with this additional drilling are not inconsequential; thus, a reasonable question is ‘what value is derived from this additional drilling’? To obtain an answer to this important question, a simulation approach is applied. First the deposit is simulated using the existing drilling and the uncertainty in the resource is defined. Next the proposed drilling program is simulated and the simulated data are added to the existing data. Using this expanded data set a second simulation of the deposit is created and once again the uncertainty in the resource is defined. Using this approach, the reduction in the uncertainty in the resource can be determined. To convert this reduction in uncertainty (or reduction in risk) in the stated resource into a dollar value, a method of defining the increase in the project rate of return necessary to account for risk is presented. Using the two different rates of return, the difference in project NPV and hence the value of additional drilling is determined.

South Africa?s Northern Cape Province houses a variety of gemstone minerals. Diamonds remain the primary gemstone in the province, but there are also tiger?s eye, sugilite, rose quartz, jasper, amethyst, amazonite, tourmaline, and topaz deposits. This paper gives an overview of the tiger?s eye (TE) mining sector in the Northern Cape, with the objective of assessing the potential of TE to contribute to job creation and poverty alleviation in the province. Mining of TE started in 1803, but gained momentum during the 1960s. Mining is conducted on a small-scale basis, and in most cases without any legal framework. The majority of the residents are not benefiting from TE. It has been reported that an estimated R700 000 to R800 000 worth of TE is leaving the country every week through illegal trading. The potential for TE mining to contribute to socioeconomic development is hindered by illegal operations, lack of technical skills and knowledge, exploitation by dealers, lack of market information, community rivalries, and absence of value addition. A multi-stakeholder approach is needed to formalize the sector and reap the benefits. Mintek is spearheading a TE project in the Prieska area of the Northern Cape, which aims to establish a gemmological centre where TE can be beneficiated into saleable and high-value products..

Since 1988, there have been thirteen documented incidents in the United States and Canada in which carbon monoxide (CO) is suspected to have migrated through near surface strata into structures or other enclosed spaces as a result of nearby trench blasting or surface mine blasting. From these incidents, there are thirty suspected or medically verified CO poisonings and one fatality associated with these incidents. To better understand the variables contributing to this hazard, the National Institute for Occupational Safety and Health (NIOSH) carried out a small- scale field study to identify key factors that may contribute to the migration of CO through the ground. NIOSH conducted single- hole heavily confined experimental test shots to measure the radial pressure pulse as it propagated through surrounding monitoring holes to determine if a relationship existed between the pressure pulse and resulting CO measurements afterwards. NIOSH also monitored blasts at an airport construction site. Following the detonations, the concentrations of CO, nitric oxide (NO), and nitrogen dioxide (NO2) were monitored for several days in the surrounding satellite holes. The peak CO measurements relative to distances from the shots were compared. Pressure histories and peak particle velocities (PPV) were measured at monitoring boreholes. It was found that the differential pressures recorded within the boreholes give some indication as to the likelihood of CO appearing, whereas the PPV measurements do not seem to be as good an indicator for potential CO migration. In this limited study, changes in barometric pressure had very little influence on removing the trapped detonation products, whereas applying a vacuum to the monitoring holes was effective in reducing the entrapped CO concentration.

From site demolition, to design, to construction, Detroit's new Tiger Stadium project is required to be completed within a short 2 ½ years from award of the design-build contract. Such extreme schedule requirements together with somewhat difficult ground conditions at the site, resulted in several challenges in the design and construction of the deep foundation system for the stadium. During the geotechnical investigation, an intermittent water bearing sand layer was encountered overlying the hardpan at the site. As a result, some concern developed regarding the potential for collapse of caisson bells within this layer, as well as the potential for flooding of the caissons during drilling. Although sand and/or ground water was encountered within about half of the caissons, standard drilling and belling methods were used, and adverse conditions were controlled in almost all cases through expeditious drilling, inspection and concreting of the caissons. This paper will present a summary of the ground conditions, design and specification requirements, and construction methods used to successfully install the caissons for this project.

One of the major problems encountered with recirculatory breathing apparatus presently used by mine rescue teams is that the inspired air becomes uncomfortably, and often dangerously, hot. This is also true with self-rescuers. It is well known that man can tolerate very high dry-bulb temperatures if the wet-bulb temperature is low, but not vice- versa. The maximum tolerable, wet-bulb temperature is about 42 °C (108 °F). A continuously inhaled wet-bulb temperature above this value can cause irreversible damage to the throat and lung tissue, and wet-bulb temperatures close to it cause extreme discomfort (1) and seriously retard the capability of the man to perform useful work. A theoretical analysis, combined with experimental results, has shown that it is the carbon dioxide absorbent that is the major cause of high wet-bulb temperatures of the inspired air in the Siebe Gorman Proto 4, and in the Draeger BG 174 apparatus, both of which are used by rescue teams on South African mines (2). In the Proto 4 apparatus, the absorbent for the carbon dioxide is soda lime (calcium hydroxide). The German Draeger BG 174 also uses this absorbent but under arduous conditions such as in hot environments (50 °C, 122 °F), the use of an alkali absorbent cartridge is recommended which consists mainly of caustic soda (sodium hydroxide). Lithium hydroxide is used in some apparatus in the United States. It is clear that different absorbents will produce different quantities of heat and moisture for a given rate of absorption of carbon dioxide. The proportion of this heat and moisture which is transferred to the user depends upon how much can be removed from the recirculating air between the absorbent and the mouthpiece. This paper discusses the present methods of heat removal and describes the design and test results of a prototype evaporative cooler.

Introduction During the past summer, detailed geological mapping was extended to cover the entire group of 31 claims owned by Waite-Ackerman-Montgomery Mines, Ltd., and considerable new information was revealed by development and exploratory work underground. These new facts provide a more complete view of the lithology and structure of the area than has heretofore been available. They also necessitate revision of certain interpretations presented last year by Peale (l), and have served to straighten out some of the problems suggested by Porritt (2). The purpose of this paper is to outline in a broad way the relations as now understood and to discuss in detail certain points which should be of interest to members of the Institute. Maps and records by the mine staff have been freely used in arriving at the interpretation presented in the following pages. The writers are especially indebted to Mr. R. V. Porritt, the mine manager, with whom many of the problems touched upon were discussed on the ground and whose intimate knowledge of the geology of the underground workings aided greatly in testing various hypotheses. The writers have profited greatly, also, from close association with Dr. W. F. James during the course of the work. Permission to publish the included data has very kindly been given .by the management.

"The SUSOP« Framework is a new technique that helps incorporate sustainable development principles and objectives into the design and operation of businesses. Developed through the Co-operative Research Centre for Sustainable Resource Processing, it brings a design methodology to sustainability, similar to what hazard and risk techniques such as HAZOP did for safety.Metallica Minerals Limited (æMetallicaÆ) adopted the SUSOP« framework to help identify opportunities and risks that could directly improve or affect its flagship SCONI (nickel-cobalt and scandium) project. This project has the potential to become the worldÆs largest producer of scandium oxide (æscandiaÆ) which is used in solid oxide fuel cells and strengthens aluminium alloys. Located northwest of Townsville in Queensland, the plant site will be located within the previously mined and partially rehabilitated Greenvale Nickel Mine. By applying the SUSOP« framework through a series of multi-disciplinary workshops and offline analysis, Metallica was able to identify numerous sustainability opportunities and risks. Opportunities were clustered together to form concepts to develop a more holistic understanding of the projectÆs overall potential contribution to sustainable development as well as determine where key risks could be mitigated. Four key concepts were identified: Waste and Water Workforce Community and Enterprise Development Energy and Greenhouse Gases (GHG). Metallica was able to include a range of initiatives that could deliver enhanced environmental, social and community outcomes into their mine development plan. These included measures to reduce reliance on water, reduce the volume of waste, provide local opportunities for training and skilled employment and create support for local enterprise development. These measures also resulted in a significant reduction in the capital estimate of the project.This paper provides an overview of the SUSOP« framework, details its application to the SCONI project and discusses the outcomes that will help establish the projectÆs strong sustainable development credentials and improved business outcomes.CITATION:Corder, G, Currey, N and Becker, G, 2013. History The Cortez Mining District has a long history of mining activity dating back to 1862. More recently, Cortez Gold Mines had operated a cyanide leach, counter current decantation (CCD) recovery gold mill from January 1969 to February 1976. This type of plant has been standard in the US for treating higher grade, finely disseminated gold in Carlin type ores. The mill was initially operated from 1969 to 1973 at 1.5 kt/d (1650 stpd) on oxidized ore from the Cortez ore body. In 1973, the mill was operated at 2 kt/d (2200 stpd) on softer, less oxidized, high clay ore from Gold Acres, located 13 km (8 miles) west of Cortez, NV. In February 1976, the oxidized portion of the ore body at Gold Acres was depleted and the mill ceased operations. Successful heap leaching of the lower grade portion of the ores was practiced at both Cortez and Gold Acres to supplement the milling operations. Valuable experience was gained in the operation of these two heap leaching programs (Duncan and Smolik, 1977). The ore heaps at Gold Acres and Cortez were releached with a limited crew from 1976 to 1979. The surge in the price of gold in late 1979 sparked an interest to incorporate new techniques into treating lower grade and less oxidized carbonaceous ores at Cortez. In early 1980, heap leaching was started on low grade oxidized dump material left from the earlier Cortez mining operation. By June of 1980, testwork was completed on the London Extension Dump, a low grade, mildly carbonaceous dump at Gold Acres. It was decided to convert the mill at Cortez from the conventional cyanide leach CCD recovery plant to a carbon-in-leach (CIL) recovery system. In May 1983, ore processing from the Horse Canyon deposit began. The Horse Canyon ore body contains both oxide and carbonaceous ores and is located 22.5 km (14 miles) from the Cortez mill. The Cortez CIL circuit was expanded to handle a blend of oxide and carbonaceous ores from Horse Canyon. Ore selection In the last quarter 1979, gold prices were increasing to a point where restarting the Cortez mill, idled since 1976, appeared imminent. Several previously explored geologic reserves at various stages of development were available as possible sources of feed for the mill. Initial metallurgical testing of dumps and tailings from earlier operations was completed, and previously uneconomic deposits were explored. It was decided to pursue sources of ore with the least lead time to production and those with the least capital cost requirements. A master plan was developed to bring the four profitable ore resources into production. Equipment and techniques were developed with Bureau of Mines facilities for measurement in the laboratory of sonic shear wave velocity in variously shaped rock specimens in a wide range of sizes. A variety of rock types was tested. The shear wave front is produced by electronically excited, piezo-electric ceramic) disk-shaped transducers. Transmitter and receiver are interchangeable. Special equipment components were Bureau produced; other components were commercially available. The equipment permits quick measurement of shear wave velocity on variously shaped specimens with a minimum of specimen preparation. The technique, under favorable conditions, allows for determination of the moment of arrival of the shear wave when it is superimposed on a weak longitudinal wave.

Responsible mining requires stakeholder engagement throughout all operational stages and after mine closure. By assessing the current and anticipated future socio-economic and environmental impacts, a company can maximise the positive impacts they can have on a community, whilst minimising any negative impacts. Despite the current emphasis on companies maintaining effective stakeholder relations to gain their ‘social license’ to operate, findings of the present study reveal a wide variation in community expectations, with different degrees of ‘acceptability’ evident across seven sites in Romania (Rosia Montana), Sweden (Kristineberg), the United Kingdom (Cornwall), Bosnia Herzegovina (Vihovici) and Russia (Karabash, Gay and Mednogorsk). These variations reflect the current status of mining within a community, as well as its socio-economic background and the nature of existing engagement with mining companies already operating in the region.

Recently the operating philosophy at concentrators processing platinum bearing ores has started to move from a recovery dominated system to one in which the production of high grade concentrates at the expense of some recovery would provide greater benefits in the further upgrading of the platinum group minerals (PGM). The low grades usually obtained are associated with the presence of naturally floatable talcaceous gangue minerals in all PGM ores. Polysaccharide depressants such as guar gum and carboxymethyl cellulose (CMC) are used to improve the grades of the concentrates. Recent testwork has shown that the use of high depressant concentrations can prevent any naturally floatable gangue from reporting to the concentrate. The only gangue present in the concentrates would be due to its entrainment in the froth. Under these conditions much higher grades can be achieved. The sulphide minerals themselves, however, become susceptible to depression at high depressant concentrations. This work examines how the recovery of sulphide minerals in two Merensky ores was affected by the type and concentration of the collector in the presence of high depressant concentration (500 g/t) and that the response of the sulphide minerals was also dependent on the type of depressant used (guar or CMC).

The process for bio-remediation of sulphidic slag (BROSS) operates at 45 degrees C in a neutral (pH 7), aerobic aqueous environment in which sulphide in the slag is converted to sulphate. A two-stage version has been developed that involves quantitative chemical pre-oxidation of total sulphide to thiosulphate, followed by complete biooxidation of thiosulphate to sulphate. Ferric hydroxide, also produced, acts as a flocculant in the bioreactor effluent, concentrating heavy metal precipitates in the thickener as a sludge which can be returned to the smelter. The process has a high sulphate production rate and low acid and nutrient consumptions. Microorganism growth is the rate-limiting stage, requiring a minimum liquid residence time in the bioreactor of about 7 h. Although large quantities of oxygen are needed, sufficient gas mass transfer can be attained in an air-lift reactor. Products are an aqueous stream containing sodium sulphate with only trace levels of toxic heavy metals and a benign, toxic metal-containing sludge

Introduction Rock bolting is a general term that encompasses pinning, roof bolting, rib bolting, and cable bolting. Rock bolting contributes the highest nonproductive cost and is the most significant safety risk in mining today. Total installed bolt costs are now more than $20 per bolt. Rebolting or repair work doubles this. And bolting accidents account for about 30% of all fatal and nonfatal lost time accidents. It is only logical, then, for the mine operator to install the highest quality anchors possible. Every bolt on the market - tensioned, untensioned, full contact, or point anchor - has designed capabilities in axial, shear, and fluctuating ground conditions. Bolts will provide a designed support if material specifications and installation procedures fall within the manufacturer's recommendations. This also assumes that there are no unexpected geological or operational changes. Proper material specifications and installation procedures should guarantee the mine operator a quality anchor. To help assure this, the Mine Safety and Health Administration (MSHA), the US Bureau of Mines (USBM), and the AMC Manufacturers group developed stringent ASTM materials standards. These govern mechanical point anchor bolts and bolts using threaded and nonthreaded rebar. There are also minimum standards that suppliers must comply with in the manufacture of frictional bolts. However, most frictional bolt standards are based on the manufacturer's own specifications. It would be impossible to set uniform regulations on frictional bolts. Each bolt provides support in a slightly different manner and thereby requires different material needs. Quality Control In any case, material quality can generally be assured through tight order specifications and periodic quality control checks. The problem with installing quality anchors does not solely lie with the material. It includes all the events that occur from the time a bolt leaves a manufacturer to the time it is installed. The anchoring ability of any major type bolt can be affected by transportation, storage, mechanical performance of installation equipment, installation demands of the operator, proper drilling accessories, and local geology. Table 1 and Fig. 1 document the negative events that can affect each bolt and the degree to which it can be affected. Drilling a hole of oversized diameter, for example, is an operator controlled factor. It will adversely affect most bolts. An over-sized diameter reduces, or even eliminates, the anchorage capability of a mechanical shell. It will reduce the support capacity of a split set to nearly nothing. And it will cause a partial or incomplete grouting around the resin bolt. Frequently, due to bit wear, holes are drilled undersized in diameter and may not be properly cleaned out. In this case, it will be difficult for the operator to push the mechanical shell into the hole, thereby causing him to drill another hole. In cases where the bolt is obviously not providing anchorage, operators will probably drill another hole and place another bolt. Rock Bolt Regulations It is the situations where a bolt is improperly installed in the hole, but gives the appearance of a good bolt, that are frightening. In fact, some safety officials and engineers believe that only 50% to 60% of installed bolts are working. These bolts cause irregular ground load transfers in other bolts and overload them. This causes hazardous situations and overstressing. It is these bolts that fail prematurely 200 to 255 mm (8 to 10 in.) behind the face and possibly contribute to the high accident rate. Paper presented at Mineralisation in the Caledonides, the Mike Gallagher memorial meeting held in Edinburgh, 27-28 June 1996. Oxygen isotope analysis indicates that quartz, carbonates and muscovite could all have formed in near-isotopic equilibrium with the mildly saline H2O-CO2 ore fluid at 330 degrees C. Calculated values for O, C and S in the ore fluids imply derivation from a deep, homogeneous source region and are consistent with formation either as a direct or an indirect result of igneous processes. Values for delta-O18 plot a little outside the accepted magmatic range, but have almost certainly been modified by exchange reactions with wall rocks. These reactions were also the most likely cause of deposition of the ore constituents. The chemical and isotopic character of the fluid was similar to that in many Archaean and later mesothermal auriferous vein systems, especially the Mother Lode region of California. The purpose of the cut is to provide a free face to which the remainder of a development or breasting round can break. The results of field testing in drifts and at a quarry face were used as a basis for a description of the rock breakage and flushing mechanism in the cut. It was concluded that the important features of a cut design are proper hole spacing, sufficient relief volume, drilling accuracy and relief holes slightly longer than the planned length of the round. The void space provided by the relief holes should be about 15% of the cut volume, with more required in weak, highly fissured rocks. The importance of drilling accuracy increases with round length, an error being magnified by the small blasthole spacing. The first hole pulls only about 20% of the round length, 4-6 holes having to be fired before the cut is pulled close to its design depth. The first four blastholes should be fired with a minimum inter-hole delay of 100 ms to provide progressive relief of burden and avoid the superimposition of ground vibrations. The earliest-firing charge should be closest to the relief area and act laterally, while the first of the vertically-fired charges to be detonated should be the uppermost. The Bureau of Mines has undertaken investigations to examine selected deposits of beryllium throughout the United States. The beryllium deposit about 6 miles southeast of Lone Pine in the Owens Valley region of Inyo County, Calif., was among those selected for study. In 1960, field work was conducted on the Inyo Beryl claim to determine the extent and character of the beryl-bearing deposit. This report describes the work that was done. In the area investigated, the beryl occurs disseminated in granite and limestone and as stringers. Apparently, the stringers are related to nearby pegmatite dikes, inasmuch as some appear to contain fine-Grained pegmatite. Granitic rocks enclose most of the stringers, which occur as fracture fillings, ranging from thin seams to 4 inches in thickness. The stringers are of muscovite, quartz, albite, and beryl. Some also include epidote and fluorite. For the most part, they are widely spaced. Therefore, mining of substantial tonnages averaging better than 0.2 percent beryl does not appear feasible.
