Safety performance in many companies and even industries has stalled in the last decade. Accidents rates are improving but more slowly and yet, serious accidents and fatality rates are not: in fact, these rates of more serious incidents have plateaued or deteriorating. In more dramatic cases, organizations that had ?exemplary? safety statistics, suddenly had a catastrophic or multi-fatality event occurring. Popular books such as In Search of Excellence of Peters and Waterman or Good to Great of Jim Collins created many delusional ?secrets? or recipes, to achieve great performance and yet, most of those ?great? or ?excellent? companies have now declined in their performance, some dramatically and some have fallen from grace, like Fanny Mae, a company Jim Collins identified as a ?great? one, that had to be bailed out in the latest financial crisis. These organizations share several ?delusions? about managing a business, and often, these delusions are created by academics, consultants and authors of popular books about (safety) management. More damaging however is that safety management is defined by a set of key practices or axioms: a focus on human error, compliance to rules and regulations, creating consistency of systems and actions, reacting to accidents and incidents and on prevention of those, measuring safety with performance metrics and rewarding employees on those measurements. All of these axioms contain the seeds of destruction, and they create conditions and dynamics that precede the disaster - and ironically, they are all 'good things' we do in safety management. For example, a mere focus on safety in the business, and the measurement of it, create a culture of repression of mistakes and incidents, leading to a situation where the organization knows incrementally less and less of risks and risktaking behaviour - until the day it all coincides destructively! This paper discusses seven such safety delusions and how they impact on the views and safety management approaches. They are: ??The delusion of (risk) control, that risks can actually be identified, analyzed and mitigated. ??The delusion of compliance, that mere compliance to rules will safeguard the business ??The delusion of consistency, that standard operating procedures will ensure safe operations ??The delusion of predictability, that accidents and trends tell us everything about the risks of the future ??The delusion of human error, that people are the weakest link and that it can actually be dealt with through discipline and enforcement. ??The delusion of statistical trends, that safety is measurable with statistics ??The delusion of invulnerability, that we have achieved high levels integrity and capability to deal wit risks. Organizations that suffered catastrophes all share some or all of these delusions and this paper traces how these played a role in many disasters, ranging from the Titanic, Challenger, Piper Alpha, Bhopal, BP Deepwater Horizon and many more.

Tunnels in deep mines are subjected to high stresses. Fracture and failure of the rock mass around an opening can occur when mining-induced stresses are high. In burst-prone grounds, mining-induced seismicity can cause additional dynamic loading which can further increase the stress around the tunnels. When the dynamic stress reaches the rock mass strength, fracturing can occur and rockburst may happen (depending on loading stiffness around the failed rock mass). Rock support installed in the tunnel must be capable of dissipating dynamic energy and holding the failed rocks. Hence, it is important to estimate the depth of failure for rock support design. This paper focuses on how to model depth of failure under dynamic loading. In the modeling, two scenarios of rock failure are considered. In the first scenario, rock failure occurs under static loading and subsequent dynamic loading further increases the depth of failure. In the second scenario, no failure occurs under static loading but in the subsequent dynamic loading, failure around the tunnel is created.

Backfill is increasingly used in underground mines, as it can help reduce the environmental impact from mining operations. However, the primary objective of backfilling is to provide safe working space for miners, by improving ground stability conditions. As cement is often added to the backfill, a critical issue is the determination of the required backfill strength (cohesion) due to the binder. This value is usually obtained from a solution proposed by Mitchell and co-workers, in the early 1980?s, for stopes with an exposed backfill face. This paper presents a brief review of the Mitchell et al. solution, with some of its main limitations. Some of the recently developed solutions for backfill design are then summarized and discussed.

The networks of interdependencies between elements in building projects, referred to as the project topology, are often complex and difficult for project teams to comprehend. It is hypothesized that a graphbased model of the project topology can be based on existing data in Building Information Models (BIM), facilitating the application of graph-theoretic algorithms to analyze this topology. The objective of this study is to propose an outline for the synthesis of BIM and graph theory. This synthesis is expected to contribute to the development of new tools for representing, managing and analyzing the design of building projects.

Building Information Modeling (BIM) is an increasingly popular method for generating and managing facility information during the life cycle of a building, ranging from facility conceptualization, through design, construction and its operational life. Organizations involved in Facility Management (FM) have the opportunity to use BIM as a knowledge repository to document evolving facility information and to support decisions made by the facility managers during the operational life of a facility. This paper demonstrates the potential of using BIM to develop algorithms that automate decision making for FM applications. The potential of utilizing BIM as an analysis tool is demonstrated through the scenario of HVAC (Heating, Ventilation, and Air Conditioning) system failure in an operating facility. In case of a typical HVAC malfunction today, facility occupants record complaints in a ticketing database maintained by the FM organization. Upon receiving notification of HVAC system failure, facility inspectors visit the location to confirm the reported failure. Upon confirmation, facility managers review building plans and specifications to develop a detailed plan of action to repair any HVAC components suspected of damage. Based on the plan of action, inspectors visit the facility to inspect and, in case of damage, repair the appropriate HVAC system components. These FM practices ? as currently implemented across the industry ? are labor intensive, time consuming, and often rely on unreliable and outdated information. To address these shortcomings, the authors propose an alternative methodology of HVAC fault detection in operational buildings. The authors implement an algorithm that leverages complaint ticket data and automates BIM to determine potentially damaged HVAC system components. Based on the list of HVAC components suspected of damage, the algorithm develops a plan of action for the facility inspectors. Finally, the authors discuss the advantages of the proposed method as well as the challenges of implementing automated BIM-enabled decision making processes in the FM industry.

The National Mining Hall of Fame and Museum (NMHF&M) will induct the 2013 National Mining Hall of Fame class on Sept. 14, 2013. The ceremony will take place at the Brown Palace Hotel in Denver, CO. Representing the domestic and worldwide metal production, education, and health and safety sectors of the industry, this year?s five inductees cover a diverse cross-section of the mining industry. The inductees are: Charles F. Barber, Donald W. Gentry, Patrick H. O?Neill, Milton H. Ward and David A. Zegeer. They will join 218 other mining industry pioneers when they are formally inducted into the National Mining Hall of Fame at the 26th Annual Induction Banquet and Ceremony. They were selected for being visionaries, leaders and ambassadors, both within their own sectors and across the industry at large. Barber was a top executive at leading U.S. metals producer Asarco from 1956 to 1982. During his tenure, Asarco responded successfully to the demanding requirements of new environmental legislation in the United States and to changing concepts of foreign investment in mining, especially in Latin America. Gentry?s career took him from being a working mining engineer, to renowned mining educator, to president and chief executive officer of a mining company. As an educator, he had a positive impact upon hundreds, if not thousands, of new mining engineers and many others who took his short courses in mine economics. During his years in South America, O?Neill did everything from supervising operations of 15-bucket ladder gold dredges to becoming president of International Mining Corp. As an executive, he emphasized provision of education and training to local people working at mine sites for which he had responsibility. Ward?s experience covered the world and spectrum of metals, minerals and ores, ranging from A to Z (antimony to zinc and Australia to Zambia). He was president, chairman and chief executive officer of Cyprus Amax Minerals from 1992 to 1999, and he was president and chief operating officer of Freeport-McMoRan from 1983 to 1992, during which time he was instrumental in developing Freeport?s world-class Indonesian operations. Zegeer dedicated his mining career to protecting the health and safety of miners as a corporate executive, head of the U.S. Mine Safety and Health Administration (MSHA) and community leader. His tenure at MSHA coincided with a period of remarkable progress in U.S. mining safety .NMHF&M Board of Directors Chairman Paul C. Jones said: ?The five outstanding gentlemen being honored this year as inductees into the National Mining Hall of Fame are classic examples of the leadership and work ethic that has made America?s mining industry the world leader in technically sound, environmentally and socially correct, and economical mineral production that sets the standards for mining worldwide. We are very proud to add these five legendary gentlemen, who have demonstrated their sound technical and management skills to advance society on a worldwide basis, to our current list of 218 inductees being honored by the National Mining Hall of Fame.? 2013 Prazen Award winner ? The design of hydrostatic liner for a deep potash mine shaft in Saskatchewan poses tremendous technological challenges. Technology has evolved to a point where one could consider a new innovative approach to hydrostatic liner design. But there is still a place for conventional technologies such as cast iron tubbings. As will be discussed in this paper, improvement to manufacturing process, installation procedures and even new materials can bring this technology to an excellent level of performance. With the spur of new projects in the potash industry of Saskatchewan, the authors worked in various shaft liner designs from the conceptual stage through to the construction phase. They were either directly leading the design work on some of these projects or acting as an external reviewer on others. Most of these shaft liner designs involved cast iron tubbing technology and ground freezing. The major challenge in sinking potash shafts in Saskatchewan is to go through a water-bearing ground formation under high pressure, known as the Blairmore or Manville fm. The only proven stabilizing technology for these formations is ground freezing. For a potash shaft, dry shaft conditions are highly desired, so a fully waterproof hydrostatic liner is required. All the existing potash mine shafts in Saskatchewan, except for two with composite steelconcrete lining, used the cast iron tubbing technology. Two new shaft tubbing liners have been constructed recently, one in the Athabasca Basin and one for a potash mine in Saskatchewan. When the authors began working on these new projects, what became quickly evident is that the technological knowhow for designing and constructing tubbing liners was no longer available within Canada and that the past construction methods used to sink tubbing lined shafts was obsolete. The tubbing liner for the shafts sunk in the sixties were designed either by German or British companies. In both of these countries only few tubbing lined shafts were sunk in last 40 years and the technology used in the sixties was not improved. There are only few countries where tubbing is routinely produced and installed: China, Russia, Ukraine and Poland. Out of these countries Poland is the one where this technology is the most advanced. The authors, working in close collaboration with engineers from Poland, attempted to rebuild this knowledge for the mining industry in Canada. This process involved thorough analysis of old technical reports, literature review and study of current polish design and construction practices as well as site visits of existing shafts. Through actual construction experience, some lessons were learned as well as ideas for improvements. In this paper, the authors will present a succinct review of design and procurement process. But, the practical aspects of the construction and installation methodology will be treated with more emphasis, more specifically the practice leading to good liner performance. Some of the poorly understood technical aspects of tubbing installation will be discussed more thoroughly and some currently accepted myths dispelled.

Transportation of fluid and slurry (fluid and solid particle mixtures) in a pipe and annulus space has been the focus of numerous studies. There are different parameters to be considered when studying slurry transportation. These include slurry velocity or flow rates; fluid properties such as density and rheology; and solid particle properties including concentration, density, shape and size. Also the angle of the flow conduit, rotation of the pipe and possible eccentricity of the annulus are other factors which influence slurry transport characteristics. Although a number of analytical, numerical and empirical equations as well as numerical simulations have been developed for studying the flow and slurry transport characteristics, the results need to be validated against either field or lab data. As performing field tests is costly and time consuming, conducting simulations at the laboratory scale appears as a good alternative. Different flow loops have been designed to study slurry transport behaviour in different science and engineering disciplines including oil and gas and mining. However, few of these consider, in particular, cuttings transportation in small size annulus space. The flow characteristics appear to be very different when slurry travels within a small size annulus, in particular when the fluid velocity is high. In this study, a review of some of the existing slurry flow loops will be conducted. Then the details of a slurry loop, which has been designed and commissioned for the purpose of studying cuttings transport in a small size annulus space for applications in drilling mineral exploration wells using coil tube technology, will be presented.

Historically bauxite-based refractories have been used in applications where andalusite-based refractories could work. Bauxite-based refractories were chosen over andalusite-based refractories mainly because of the availability of low-cost Chinese bauxite and also because many furnaces were designed by international companies that cannot easily access high-quality products. Currently, the availability of low-cost bauxite is under threat as a result of high export duties and tariffs as well as restrictions on the amount of material that China allows to be exported. South Africa is a major producer of andalusite, and this guarantees stability with respect to both availability and price. Refractories based on andalusite have an added economic advantage over bauxite-based refractories in that unlike bauxite, which requires high-temperature calcination before use, andalusite is used in its raw state. This paper constitutes a literature study of the merits, both technical and economic, of using refractories based on andalusite compared to bauxite-based refractories. The deep-hole surface blasting technique is a prime method of iron ore mining. Blasting fragmentation is one of the important economic indicators that measures the blast method to obtain the iron ore. To improve the efficiency of the blasting method and further reduce the cost of the subsequent processing of iron ore, this paper summarizes measured data gained from a certain amount of blasting test conducted in an iron ore mine in Guangdong province, China. According to the data analysis, there are differences on iron ore blasting fragmentation by different indicators of deep-hole blasting, such as decoupling coefficient of charge, contact state between charge and borehole wall, and change of detonation position in charge. These factors were compared in terms of relevance and impact. Research shows: under certain rock conditions, the decoupling coefficient of the charge has a best range related to blasting fragmentation. Since the detonation position in charge determines the direction of propagation of the explosion stress wave through ore and rock, bottom initiation and mid initiation will help to improve the iron ore blasting fragmentation in joints fractured rock and ore. The research conclusions have important application value for program design of iron ore surface blasting and to guide other types of rock surface blasting.

MOIL Limited (formerly Manganese Ore India Limited), a Government Enterprise, under the Ministry of Steel, Government of India, produces about 1.1 million tonnes (T) of manganese ore annually from three opencast and seven underground mines. The underground operations are mining using an overhand cut and fill method. The orebody is too narrow for the use of larger equipment and the poor to fair rock mass quality limits the size of stable excavations. Prior to the 1990s underground support consisted primarily of timber square sets in tandem with timber chocks and props. As mine production requirements increased throughout the 1990s led to larger opening sizes and more modern support practices. Research was conducted on ground support design based on rock classification and empirical techniques for designing ground support. Based on the findings and field trials, the Company adopted full column cement grouted cable bolts for back support. Instrumented field trials, using extensometers and load cells on the cable support confirmed the support effectiveness. Stope design was based on two and three dimensional stress modeling which resulted in a significant increase in stope dimensions. The rock mechanics studies for safe stope design were carried out using numerical modeling tool in two and three dimensions for the analysis of stresses. Based on the findings, the safe stope geometry with new stoping parameters was introduced. The earlier stope length of 30 mtrs were changed to 60 mtrs with in situ rib pillar at the cross cuts and in situ post pillars to be left-out in the stope. This leads the formation of of panel comprising of three stopes, for production, supporting and filling. For mechanical handling of ROM in stopes Side Discharge Loaders (SDL) were introduced. The above changes have improved the face productivity from 2?3.5 T with manual operation and 9 T with SDL. The above rapid mining operations has improved the stope productivity but reduced the stope life from 6 to 3 years. For the improvement of the stope availability, further studies were conducted by numerical modeling of stress analysis. The related studies confirmed that with the existing supporting system and proper instrumentation, level interval could be increased up to 45 mtrs from the existing 30 mtrs at deeper levels. At present below 12th Level interval is 45 m at Balaghat Mine. For future mechanization program for introduction of Single Boomers, LPDT, Cutting Machines etc for the development headings rock mechanics studies are underway. The paper provides details of rock mechanics studies that were carried out to study efficacy of the cable bolt support, safe stoping parameters and increased level interval for the improvements in safety, production and productivity at Balaghat Mine.

We discuss the stability problem of a piston of the reversible axial-piston pneumatic motor called DAR-14M. We performed the full analysis of stress and deformation characteristics of the piston near a resonant frequency with the help of FEM ANSYS. We estimated the own frequency of the piston analytically for the given motor, this estimate was confirmed numerically in the model analysis FEM ANSYS. It was also shown in harmonic analysis that loaded piston end behaves very different in comparison to the unloaded end. As expected, maximum deformation was found in the area of the holes of the bearing shaft, indicating an optimization necessary in that region of the motor.

The integration of three-dimensional (3D) data with infrared thermography has received much attention for its potential use as input data for building energy simulations. Therefore, researchers have proposed methods to integrate 3D data with infrared thermography obtained by different sensors. These studies show the potential of the integration of 3D data and infrared thermography using various systems. However, these studies have ignored property aspects of infrared thermography, such as the low resolution of infrared thermography and external environmental influences. Thus, their proposed methods to integrate 3D data and infrared thermography are inadequate because these problems can affect the resulting building energy simulation. This research proposes a framework that integrates infrared thermography and 3D data while accounting for the resolution and shadow effect of infrared thermography. The proposed framework consists of three steps. In the resolution enhancement step, bi-cubic interpolation is proposed. In thermal data correction from the shadow effect, the shadow area is detected from the visible image, and then the shadow effect is removed from the registered infrared thermography. Finally, enhanced infrared thermography is mapped to the 3D data obtained by a laser scanner. The performance of the proposed framework was evaluated using test images obtained from a building in operation. The experimental results show that the proposed framework provides a 3D thermal model that is useful and accurate in simulating building energy. It is expected that the 3D thermal model can be used for accurate energy simulation of existing buildings. In addition, the proposed framework can be applied in defect detection for buildings. Furthermore, this novel approach enhances infrared thermography using visible images.

En consecuencia, el carbón activado puede requerir de altas temperaturas para la elución, así como una reactivación térmica.

Commercial deployment of Carbon Capture Utilization and Storage (CCUS) requires field testing of a scale that can stress the geologic reservoirs. One such reservoir of interest consists of unminable coal seams that exhibit favorable characteristics and depositional environments and lower pressure and temperature than other, deeper, reservoirs. Such conditions can reduce compression costs while utilizing the action of adsorption that offers a more effective carbon dioxide (CO2) bonding than free storage or solution. To ensure the success of such tests, a number of parameters should be accurately determined, such as the seam geometry and stratigraphy, coal porosity and permeability parameters as well as optimum injection conditions. In essence the CO2 injection model should be calibrated for the reservoir characteristics using all available data. History matching calibrations are based on gas and water production from existing wells prior to injection. This paper will present reservoir models that were developed for a pilot--scale 907 tonnes (1000 tons) CO2 injection test that was performed in 2009 through one legacy coalbed methane production well in Russell County, Virginia, USA. The model incorporates a number of individual coal seams about 0.3 m (1ft) in thickness located at depths ranging from 300 to 700 m (1,000 to 2,200 feet). Model calibration was performed through history matching to prior production data, and subsequently the model was utilized to develop different injection scenarios. The developed model was used after injection to calculate CO2 plume distribution patterns that were monitored at the injection test. The paper will present model data and assumptions with a special emphasis on the effect of hydraulic fractures and the skin factor to the coalbed methane (CBM) production model.

In blasting sensitive areas, the use of traditional blasting for rock fracture is restricted. Soundless cracking technology has been presented to achieve rock fracture instead of blasting. However, when the diameter of a borehole is larger than 50 mm, it is very dangerous that the soundless cracking agent (SCA) will have a tendency to gush from the borehole, which also leads to cracking failure. In this paper, a new large scale borehole soundless cracking technology is presented, using a new enlarged drill bit and mechanical plugging, which could solve the hole gush problem. Experiments were made to measure the expansive pressure of the SCA working in boreholes of F40 mm and F100 mm. The difference between two sizes of cracking phenomenon was compared and analyzed to show that large scale borehole soundless cracking has larger expansion pressure and faster reaction speed than traditional soundless cracking. Based on the experimental data, finite element analysis was utilized to study the stress concentration and stress distribution between two neighboring large boreholes in several media during the cracking process. Using the data from experiments with finite element numerical simulation, the brittle material fracture condition was analyzed to design the parameters of the borehole mesh. With the theoretical analysis, the technological process and reference parameters of large scale borehole soundless cracking were proposed, including the borehole spacing, burden distance, depth of the borehole and so on. Then a single rock was cracked with the new enlarged drill bit and new plugging, and the results proved the feasibility of the new large scale borehole soundless cracking technology, which achieved the expected goal of improving the cracking ability and reducing the cracking time. This paper presents a new efficient rock cracking method for blasting sensitive areas, and which could apply to mining engineering, demolition engineering, excavation engineering and other engineering applications where there is a demand soundless and non-vibration fracturing of rock.

This paper presents the results of research aimed at proposing new solutions in the area of belt conveyor elements. Many factors, such as energy consumption costs, and operating costs of belt conveyor elements, including belts, idlers and drives have an impact on the operational costs of conveyors. The paper presents special tests of new materials for the bottom cover of an energy-saving belt conveyor, which decrease rolling resistance. It also presents large-scale in-situ tests of a high-output belt conveyor for the purpose of simultaneous measurements of various types of conveyor belts (standard vs several energysaving types) under typical operating conditions of a lignite surface mine. Improvements in materials used for the bottom belt covers, supported by specially developed comprehensive tests of the new types of belts have provided valuable results of various proposed solutions. Since the tests of belts were done at exactly the same time and in the same operating conditions, direct comparison of the results is valid and should lead to a substantial decrease in the conveyors? drives? energy consumption, which will improve the effectiveness of belt transport and minimize its negative influence on the environment.

In the construction industry, project changes constitute a major cause of delay, disruption, cost increases, poor quality and unsatisfying performance. They are also known as a main factor of litigation between the owners and constructors. Construction projects are essentially process-based, and with the critical role of Information Technology (IT) in this industry, projects, especially mega projects, are being managed remotely. The volume of data and documents, such as Requests For Information (RFIs), different types of Change Requests (CRs) and Project Change Notices (PCNs), being transferred and exchanged amongst the stakeholders of a project is considerable. The circulation of these documents amongst project stakeholders still heavily relies on two methods or levels for management of change process; Conventional (fully paper-based, Faxes, Snail mails) and Electronic or semi-automated (Email, Internet, PDF files). These two methods are dependent on human discipline to follow specified processes which often break down because of human nature. This creates serious compliance problems. It is hypothesized here that fully that automated process-based management of change can lead to better compliance, more efficient management of change, and reduction of time and cost of data exchange. An approach of doing this is presented in this paper. Ultimately, its impact should improve project performance. Conclusions are presented on time savings and improved compliance.

A trend in mining exploration is to increase the number of geophysical parameters collected and interpreted for geological features in a given area. Airborne technology now allows us to simultaneously combine electromagnetic, gravity, aeromagnetic and radiometric surveys. We report the results of a multiparameter survey conducted in the area of Kamiskotia, Ontario. This area is located near and on the West side of the multi-metalliferous Kidd Creek deposit. Various metallic occurrences are reported in the studied area. Integrating multiple parameters in the interpretation produces a more detailed pseudogeological map than what has already been published. 3D interpretation has been undertaken using some of these parameters and suggests that future work is required in 3D earth model solutions in order to fully use all parameters.
