In 1990, Freeport Exploration began regional reconnaissance exploration programs in Irian Jaya, the easternmost province of the republic of Indonesia (Fig. 1). Irian Jaya occupies the western half of the island of New Guinea. Freeport was awarded an initial contract of work that totaled 2.6 million hm2 (6.4 million acres). The company was subsequently awarded additional concessions that added 1 million hm2 (2.5 million acres). The concessions require work commitments and periodic relinquishments. Due to these relinquishments, Freeport’s current land position totals 1.9 million hm2 (4.7 million acres). Freeport began the exploration program to locate additional Grasberg deposits, the largest deposit in the Ertsberg district. But the company recognized that other types of deposits could be present in the largely unexplored area of Irian Jaya.

With 13.7km of TBM tunneling in Atlanta’s hard gneiss well under way, drill and shoot work continues on several sites throughout the city. In order for the City of Atlanta to meet the consent decree imposed by the federal government, an aggressive construction schedule was a necessity. Construction consists of two deep, large diameter tunnels, four construction shafts, a pumping station, one overflow shaft, three large intake structures, and two connecting tunnels. Due to the multitude of blasting insensitive urban environments, a high standard of seismic and air-overpressure criteria, coupled with time restrictions, was adopted for the project. At any given time, drill and blast operations were under way at four of the seven sites requiring a certain amount of coordination between the TBM excavation, surface work, and the forces of Mother Nature. This paper addresses the challenges faced and the techniques used to safely and successfully complete the rock excavation of the West Area CSO Tunnel and Pumping Station project.

BACKGROUND This paper discusses work performed as part the rehabilitation of the gravity flow section of the New Croton Aqueduct (NCA) during an approximately two year period between 2004 and 2006. The goal of the project was to restore the gravity flow section of the NCA to optimal operating condition, extending the lifespan of this significant water supply for the New York City metropolitan area. Rehabilitation of the NCA was purposely scheduled before the completion of the new NCA Water Treatment Plant now under construction (Figure 1). The NCA is a 3.73 to 4.34 m (12.25 to 14.25 ft) diameter brick-lined circular andhorseshoe shaped water tunnel constructed between 1885 and 1891. The NCA conveys water for approximately 50 km (31 miles) by gravity and pressurized flow from the New Croton Reservoir in Westchester County, NY to distribution and reservoir points along its alignment, terminating at a distribution station at 135th Street and Convent Avenue in Manhattan, NY. Prominent features of the NCA include a large intake facility at New Croton Reservoir, 38.6 km (24 miles) of gravity flow horseshoe shaped aqueduct, 11.3 km (7 miles) of pressurized flow circular aqueduct, 2 siphons, and 36 open shafts and headhouse chambers which are used as sampling, access and blow-off locations. Until a partial inspection in 1982, the NCA had flowed continuously since its completion nearly 100 years prior. Flow tests have confirmed that the NCA can convey up to 12.7 m3/s (290 mgd) (Figure 2). Cover depths of NCA vary considerably, ranging from a few to hundreds of meters, typically of hard rock. Primarily mined using means conventional for the late1800s, construction of the NCA also employed cut and cover methods in several low-lying sections of the alignment, totaling approximately 1.6 km (1 mile). The NCA crosses through numerous lithologic changes, fault/shear zones and under several significant water bodies including the Pocantico, Saw Mill and Harlem Rivers and the Tarrytown Reservoir. The NCA passes under the Harlem River as a siphon at a depth of approximately 122 m (400 ft) below grade (Figure 3). Soon after construction the Aqueduct Commission Reports on the New Croton Aqueduct documented prominent defects such as large voids behind the lining. More recently, large-scale inspections of the gravity flow section performed during the 1990sand a 2004 inspection of portions of the pressurized sections of the NCA have revealed additional defects such as open and/or deteriorated masonry joints/cracks, leaks ranging from trickles to several liters per minute into and out of the NCA, missing bricks, formed openings, and in one place, a rupture through the liner with discernable

Utah produces a wide variety of industrial rock and mineral commodities, valued in 2005 at a record $758 million (21 percent of the total non-fuel mineral production of Utah). Major commodities produced in 2005 include salts (halite, potash, and magnesium salts), Portland cement, sand and gravel, carbonates (lime, limestone, and dolomite), phosphate, crushed stone, clays, lightweight aggregate (expanded shale and perlite), gypsum, silica, sulfur and sulfuric acid, and building stone. Commodities produced in smaller amounts include diatomite, pumice, gemstones, ornamental stone, fluorspar, and mineral and fossil specimens. Utah is in a period of rapid growth in industrial rock and mineral production; however, some commodities have grown faster than others. The robust western economy has stimulated production of construction-related commodities like sand and gravel, Portland cement raw materials, lime, and building stone, resulting in exploration for raw materials, development of new quarries, and expansion of existing plants

The Red Dog zinc-lead-silver deposit is located in the DeLong Mountains of the western Brooks Range in northwest Alaska. The region is located 145 km(90 miles) north of the village of Kotzebue (Fig. 1).Red Dog’s current resource base is 143 Mt (157 million st) averaging 16.1% zinc, 4.3% lead and 83g/t (2.4 oz/st) silver. It is contained in four structurally separate deposits— the Hilltop, Main Deposit, Aqqaluk and the newly discovered Paalaaq deposit. Recent exploration for the Aqqaluk and the Paalaaq deposits has contributed to Cominco American’s understanding of the detailed structural geology of the district and its role in the placement of individual ore bodies.

Tectonic stress is an important parameter for understanding the dynamics of underground engineering and for the exploitation of coalbed methane. Multiple-period structural features usually have complex spatial relationships in tectonic superposition areas, which provides evidence for identification of the sequence of the tectonic stress period. Conjugated joints and slickensides are used to reconstruct the tectonic stress of the No. 8 Mine in Pingdingshan. Based on the geological unit division and field measurements, data on hundreds of joints and slickensides are collected from 33 stations. The tectonic stress results indicated by the conjugated joints show that there are mainly four directions of tectonic stress: NEE-SWW, NW-SE, NE-SW, and NNESSW. The tectonic stress results of the slickensides indicate that there are mainly four directions of tectonic stress: NNE-SSW, NNW-SSE, NEE-SWW, and NW-SE. According to the systematic analysis of the joints and slickensides, this paper separates the superposed joint and slickenside sets and identifies the tectonic stress periods with their abutting relationships. Three tectonic stress periods are determined and the sequence of the tectonic stress periods is NE-SW, NW-SE, and NEE-SWW. The results are consistent with other tectonic stress measurements for both the region and the locality.

Each year, hundreds of mine workers are involved in machinery-related accidents. Many of these accidents involve inadequate or improper use of lockout/tagout (LOTO) procedures. To mitigate the occurrence of these accidents, new safety methods are needed to monitor access to hazardous areas around operating machinery, improve documentation/monitoring of maintenance that requires shutdown of the machinery, and prevent unexpected startup or movement during machine maintenance activities. The National Institute for Occupational Safety and Health (NIOSH) is currently researching the application of Internet of Things (IoT) technologies to provide intelligent machine monitoring as part of a comprehensive LOTO program. This paper introduces NIOSH’s two phase implementation of an IoT-based intelligent machine monitoring system. Phase one is the installation of a proof-of-concept system at a concrete batch plant, while phase two involves scaling up the system to include additional sensors, more detailed safety/performance metrics, proximity detection, and predictive failure analysis.

In the drive for faster development advance rates, railbound systems can be advantageous, in the right application. Skanska BS, the contractor to mine-owner VSK Mining, is driving an adit access deep into the Volouské Urchy Mountain in Slovakia to access the Gemerská Poloma talc deposit, some 70 km (43 miles) west of Ko?ice, the economic and cultural center of eastern Slovakia. Skanska began driving the Stolna Elisabeth drift in February 2007, extending 25 kg/m (50 lb/yd) rails (with 750 mm or 30 in. between sleepers) into the mountain to provide the main haulage to bring talc ore and waste out of the new mine once it gets into production. It will also be used for personnel transport, and is now almost complete. As a second egress and for ventilation, a 2.5-m- (8.2-ft-) diameter shaft of about 300-m (984-ft) depth will be sunk from surface down into the orebody. Stolna Elisabeth is a 4.2-km- (2.6-mile-) long drift mined at a cross section of some 11 m2 (118 sq ft). The finished profile is 3.2-m- (10.5-ft-) wide by 3.6-m- (11.8-ft-) high. It has been developed on a very slight incline so that water runs down it to the portal. The portal is 450 m (1,476 ft) above sea level and the end of the tunnel is 475 m (1,558 ft) above sea level. Much of the rock through which it has been developed is magnesite with a compressive strength of 150 MPA. The drilling jumbo is an Atlas Copco twin-boom Boomer 282 Raildrill drilling 42- to 45-mm- (1.6- to 1.7-in.-) diameter holes to a maximum length of 2.5 m (8.2 ft), depending on ground conditions. This is a drill jumbo that Skanska has used successfully on a number of projects during its life. The condition of the rock through which this drift has been driven is extremely variable. Five classifications of A1 and A2 and B1, B2 and [ ] B3, where A1 is the most competent rock and B3 is the worst, have been established. The amount of support used, of which more later is determined by the classification of ground conditions. At the time of the author?s visit, the project it was about 40 days ahead of schedule, thanks to high advance rates. For instance 6 m/day (20 ft/d) was the plan for the advance to be achieved in B1 rock, but sometimes the crews have achieved 8 m/day (26 ft/d) ? which is the average for the whole project ? in those conditions. In A1 the best advance rate achieved is 12 m/day (39 ft/d). The loading unit is a Häggloader 9HR-B on crawler tracks with a pony track to run on the rails. This allows the unit to be towed up and down the drift along the rails. To load from the face, the Häggloader lowers the foot of its onboard conveyor system to the floor and the backhoe pushes material on to it. This digging arm-conveyor combination produces uninterrupted, optimal volume loading of the haulage unit, without spill ? a technique offering load capacities as high as 180 m3/h (6,356 cu ft/hour). INTRODUCTION If a uranium mill worker is involved in an accident, analysis of excreta for uranium content and[in vivo] counting methods are used to estimate the extent of the possible exposure and to predict the pattern of uranium distribution among tissues (Spitz, [et al]., 1980). Interpretations of urinary excretion data are based on studies of human exposures to single uranium compounds including U02, U308, U03, UF4, U02F2 and UF6 (Alexander, 1974). Refined uranium ore (yellowcake) is a variable mixture of U03-NH3-H20 adducts (ammonium diuranate, ADU) and their thermal conversion product, U308. The variability in composition of yellowcake (Kalkwarf, 1979; Eidson and Mewhinney, 1980; Dennis, Blauer and Kent, 1981) complicates the interpretation of bioassay data from individuals that might have inhaled any single yellowcake material. A test system would be useful to estimate the dissolution behavior of a particular yellowcake aerosol inhaled by a worker and aid in the interpretation of bioassay data collected before and after an accident. A series of experiments was conducted for this purpose. The objectives of these studies were to: 1) develop specific criteria for comparing [in vitro] dissolution systems and 2) use the criteria to evaluate solvents, methods and experimental temperature and pH conditions used in previous studies. Dissolution experiments were conducted using different combinations of solvents, conditions and methods with a common yellowcake sample. Because little information is available from human yellowcake inhalation exposures to evaluate the [in vitro] results, criteria for evaluation of different tests were derived from a model of yellowcake dissolution in lung constructed from a number of cases in which humans were exposed to single uranium compounds known to be in yellowcake (Mercer, 1975). MATERIALS AND METHODS Materials The powder used in the tests was taken from one lot of yellowcake obtained from a commercial uranium mill. The same powder was previously designated as Mill D in Eidson and Mewhinney, 1980 and Mill 2 in Kalkwarf, 1979. The ammonium diuranate content was measured by infrared absorption to be 25% ± 1% (Eidson and Mewhinney, 1980). Solvents The compositions of the two biological fluid simulants used are compared in Table 1. Simulated lung fluid (SLF) (Kalkwarf, 1979; Dennis, Blauer and Kent, 1981) was chosen to simulate the composition [ ] of lung fluid as closely as practical (Moss, 1979). Simulated blood serum ultrafiltrate (SUF) (Eidson and Mewhinney, 1980) was chosen because of previous successful efforts to model human urinary excretion of inhaled plutonium aerosols (Kanapilly Raabe and Boyd, 1974) and the dissolution rate of 241Am02 in Beagle dogs (Mewhinney and Griffith, 1981). Major differences are that simulated lung fluid contains Mg2+ and a higher Ca2+ concentration than SUF and that SUF contains ammonium ion. Both solvents include protein substitutes; acetate is used in SLF, while amino acids are used in SUF. Both solvents include phosphate but the chelating agent diethylenetriaminepentaacetic acid (DTPA) is included in SUF to prevent possible formation of insoluble uranyl phosphate precipitates that would result in an underestimation of solubility. In addition to the above solvents, modified solvent compositions were prepared to investigate the role of specific salts in yellowcake dissolution studies. Solutions of SUF (Table 1) were prepared without DTPA (SUF-DTPA) and without amino acids (SUF-amino acids). No solution excluded both DTPA and amino acids. The methodology of quarrying changed significantly in the Indiana limestone industry in the 60 years between Sept. 11, 1941 and Sept. 11, 2001. The former was the date of the Pentagon groundbreaking ceremony, while the latter was when the building was damaged by American Airlines Flight 77. So too, though to a lesser degree, did the machinery and methods used to fabricate the stone. The rededication ceremony on Sept. 11, 2002 marked the end of the Phoenix Project, as the rebuilding effort was named. The story of this remarkable project illustrates what the American construction(and Indiana limestone) industry can accomplish when working toward a common goal.

"Coal mining is an age-old method of power production in the world. It is also widely known for the associated risks during mining. Although the number of mishaps has dropped dramatically over the years, the number of fatal accidents is still a concern, and any aid in reducing these figures would be important to industry workers. Therefore, data from accident reports during the past seven years were collected and analyzed. Qualitative data from reports and articles provide a deeper understanding of the incidents, thus affording the possibility of identifying patterns and relationships between the accidents. For this study, a total of 119 fatality accidents occurring in U.S. coal mining operations were analyzed using advanced text mining techniques. The fatality data were obtained from U.S. Mine Safety and Health Admininstration fatalgram reports. This analysis is primarily exploratory in nature and uses term frequencyinverse document frequency (TF-IDF) methodologies along with correlation network plots. An interesting word relation pattern was obtained that identified “positioning” as the common cause of 12.6 percent of accidents, and found that 29.4 percent of accidents can be categorized as vehicle-related. IntroductionIn the mining industry, accidents are situations that employees and employers wish to avoid. They are devastating experiences to the victims and their coworkers alike. Even worse, accidents that result in fatalities create trauma and undesirable repercussions. Several measures have been taken throughout the industry to prevent such mishaps, and the U.S. Mine Safety and Health Administration (MSHA) provides continuous monitoring for miners’ safety by guiding organizations through best practices in every job role. As a result, according to MSHA, there has been a steady decline in the average number of deaths due to accidents over time. Nonetheless, any opportunity to avert one more such casualty is of paramount importance, and that is the goal of our analysis. Our objective is to identify opportunities resulting from previously unexplored directions in order to provide additional insights into potential safety recommendations. Until now, most of the studies in this area had been conducted using structured and tabulated data with a constant number of categorical and numerical variables. Unstructured text data mining is an emerging area that is still insufficiently researched and has a feasible scope for important results, as a vast majority of human communication and cognizance is comprised of words. The aim of undertaking this project by capitalizing on unstructured data is to analyze the text reports to discover potential relationships that explain the causes of accidents in new light in order to mitigate the occurrence of fatalities. The analysis in this project is based on textual data obtained from the MSHA website (2017). Individual accident reports are organized according to the date and year of the fatality. Each report includes details with images of the incident. For convenience, a simplified summary of the report is also available. Data collection consisted of gathering formal documentation of fatal accidents in the coal mining industry during the past seven years. These reports were extracted from the website in text format."

Longwall mining has existed in Utah for more than half a century. Much of this mining occurred at depths of cover that significantly exceed those encountered by most other U.S. longwall operations. Deep cover causes high ground stress, which can combine with geology to create a coal burst hazard. Nearly every longwall mine operating within the Utah’s Book Cliffs coalfield has been affected by coal bursts.

Concrete is one of the decisive construction materials, also for the AlpTransit Gotthard Tunnel. The client has many requirements, and many are the solution possibilities that concrete systems can offer. Cement producers and concrete admixture manufacturers have been challenged to design and test jointly concrete systems for cast and sprayed concrete. The testing operation followed the scenario laid out by the Gotthard AlpTransit Project management?s instructions ?Testing system for concrete mixes.? Various supplier teams have applied for the concrete systems for sections Amsteg/Erstfeld, section Sedrun and/or sections Bodio/Faido and have also followed through with the tests. As was to be expected, the rate of failed mixes matched the level of the requirements. Thanks to this system, it was nevertheless possible to test complete concrete systems for all sections and to get them approved. The joint ventures tendering for the contracts were allowed to include only approved mix designs in their bids. Workability and durability are of paramount importance for cast and sprayed concrete mixes. Durability is influenced by the chosen mix design where binder concept and well-defined water content, i.e., impermeability, are determinant. Aggregates for AlpTransit are to be considered as given. The concrete mix design also determines workability. Because ambient conditions and also requirements with respect to workability are subjected to change, it is necessary that the mix designs include control measures. The choice and number of components for the concrete systems (cement, additives and admixtures) allow adaptation to changing conditions. For AlpTransit such adaptive measures must meet very strict requirements.

Despite low commodity prices and mining industry cutbacks throughout the world, Peru remains one of the industry’s bright spots. The country’s remarkable economic turnaround during the 1990s remains a model for other developing nations to follow. And the Peruvian government has not forgotten the industry that was instrumental in that turnaround. True, the country has not been spared the effects of the mining industry’s retrenchment that has occurred in other parts of the world. However, foreign mining companies remain committed to doing business in Peru. And the government remains committed to attracting foreign investment and maintaining a strong mining industry. This commitment was evident in September 1999during the 24th Convention of the Institutos de Ingenieros de Minas del Peru in Arequipa, Peru. More than 3,000 mining industry professionals from around the world attended the convention. And Extemin, the accompanying exhibit, featured more than 450 companies, many from the United States, Canada, Australia, South Africa and other countries.

Two plate load tests of 1 m2 area were per- formed in a pressure tunnel at the Rocky Mountain Pumped Hydro Project. Reaction for the 250 tons loading in this 10.5 meter diameter tunnel was provided by deeply anchored, diverging rock bolts, eight per load frame. Pressure build up to 2.5 MPa was provided by circular flatjacks. Rock deformation was recorded at depths of approximately 0.3, 0.9, 1.2, 1.8 and 3.3 meters using axially located multiple position borehole extensometers. The tests were performed in representative units of massive shale and thin to medium bedded siltstone. Stress was built up in four equal steps to 2.5 MPa, held constant for one week for creep monitoring, then unloaded, reloaded and unloaded over a period of one day. The most deformable unit was the massive shale which exhibited 1.3 mm of permanent deformation at the surface after the first loading and unloading cycle, which included 0.36 mm of creep. After the second loading-unloading cycle, the permanent deformation had increased to 2.6 mm, following a maximum deformation at the surface of approximately 3.6 mm. At the maximum stress of 2.5 MPa, the tangent modulus averaged 0.4 GPa when loading, and 0.3 GPa when unloading. The thin to medium bedded siltstone exhibited only 0.17 mm of permanent deformation at the surface after the first loading-unloading cycle, which included only 0.05 mm of creep. After the second loading-unloading cycle, the permanent deformation had increased to 0.27 mm, following a maximum deformation at the surface of approximately 0.53 mm. At the maximum stress of 2.5 MPa, the tangent modulus averaged 6.2 GPa when loading and 21.9 GPa when unloading.

The pollution of our water supply, Lake Michigan, is a continuing threat so long as our overflows from combined sewers exist. The problems have become even more acute during recent years, due to the increase of paved area- which accelerates and enlarges the flows of storm water. After each storm, mixtures of raw sewage and storm water spill into our waterways. If the storm is too large for the drainage systems, the mixture backs up into our streets and basements. Under severe conditions, it may even become necessary to open the locks, and discharge the polluted water into Lake Michigan. Consider the scope of the problem, as shown in Figure 1. The area in which combined sewers exist is 375 square miles, located in the central parts of Chicago and 49 suburbs. There are an estimated 640 overflow outlets to the waterways, each requiring attention. The capacity of outlets tributary to the waterways has increased dramatically over the years. Each time a pocket of upstream flooding is drained into the channels, the problem of its rapid disposition increases. Another difficulty is that the bottom of our waterways is covered with a blanket of accumulated sludge, ranging in depth up to 3-feet. The planning and operation of ventilation network systems for uranium mines require special considerations since ventilation is the primary technique of controlling ambient concentrations of radon progeny. The major environmental challenge facing the ventilation engineer is the requirement to dilute radon progeny to below the Working Level Month per year of worker exposure required by law. The application of large airflow volumes and the utilization of low residence times constitute some of the factors considered by the design engineer. Good mine planning and sequencing are also critical for protecting the health of miners. The mine ventilation must have such characteristics as flexibility, high air distribution efficiency, rapid air transit times, zero recirculation and no short-circuiting. A well established maintenance program for all ventilation appliances must be in place to guarantee control of the distribution system. Real-time monitoring systems permit assurance the adequate quality of air is delivered to working places. A case study for an operating mine is used to demonstrate how the ventilation system is successfully managed and operated using the above concepts.

There was a time when an occasional failure of a tailings embankment was considered an acceptable risk associated with mining activities. This is not the case anymore. A major tailings failure today could result in the indictment of mine officials, huge fines and a possible site closure due to the loss of operating permits. As a result of this changed operational environment, tailings disposal facilities must be operated with these potential consequences in mind. Physical controls for tailings embankments are prohibitively expensive. However, the nuclear industry has developed a system of administrative controls as an alternative to physical controls. Administrative controls may be defined as control of actions, through organizations and management, using written procedures, record keeping, review and audit to assure operation of a facility in a safe manner (10 CFR 50). Administrative controls can reduce the cost of operating tailings impoundments, as well as increase the level of confidence in these facilities.

Long term contracts to supply approximately 90 million tonnes of coal to National Power and PowerGen to the period of 3 March 1998 ?Approximately 7,600 employees and 4,500 mining contractual labour. Up to the acquisition RJB Mining had the capacity to produce 14 million tonnes of saleable output with an approximate 1,500 strong workforce.
