This paper examines the interrelationship of conducting full risk assessments in the context of the coal mining industry and under the framework of the Health's Safety in Employment Act 1992. In the context of Occupational Health's Safety, Risk Management is a holistic management process that can be used by individuals working in all organisations and from all industries. The Australian New Zealand Standard 4360: 2004 defines a Risk as being; The chance of something happening that will have an impact on objectives. This definition is followed by several notes that include: a risk is often specified in terms of an event or circumstance and the consequences that may flow from it, risk is measured in terms of a combination of the consequences of an event and their likelihood, and risk may have a positive or negative impact. For the purposes of this paper, the above definition will be adopted. This paper examines the newly expanded approach of the fully documented risk management process in the New Zealand coal mining industry. This is done in the context of the coal mining regulations but more importantly by using the framework of the Health & Safety in Employment Act 1992. This legislation under Section 6 requires all employers to, Take all Practicable Steps to prevent harm coming to employees while at work and to others who might be at or in the vicinity of a workplace. When the term 'all practicable steps' is studied, one can clearly see that such steps should be the same steps taken that a prudent and reasonable person would have taken given the same type of circumstance. Due to the fact that in New Zealand parliament made statutes is administered by a Government Department, means that their breaches will be heard before a judge in the District Court. Under the Westminster System of Justice, a basic dictum of that court in matters relating the occupational health and safety is that a breach needs to be satisfied to a specific standard. That standard is, 'Beyond a reasonable Doubt'. One of the primary benefits of adopting a proactive approach to risk management is that if the industry were to adopt similar approaches, then this mythology could be seen as an industry standard. As such, the approach carries the same status in a court of law, as that of an Approved Code of Practice.

Aluminium in Australia, and in fact all round the world, is made virtually exclusively by the Hall-Heroult process, whereby the metal is derived from the electrolysis of alumina dissolved in an electrolyte comprising fluoride salts. It is the distribution and control of these inorganic fluorides during and after the reduction process which go to make up the bulk of environmental problems associated with aluminium reduction smelters. The emission characteristics of fluorides from the process, the occurrence and effects of fluorides in the environment, and the control techniques wnich are applied in industry, all need to be taken into account in assessing the overall environmental aspects of aluminium Production.

Large scale cut and fill mining in narrow silver-lead-zinc orebodies began at Mount Isa in 1964. Initially each orebody was one stope and access was via raises from the level above. Upholing was the principal mining method, although flatbacking was employed where ground conditions were more difficult. Developments since have included access via footwall inclines and connection of all stopes with major crosscuts. Flatbacking is the principal method. The most recent cut and fill operation is now fully mechanised using drilling jumbos, rockbolting jumbos and load-haul-dump units. Improved ground support and drilling and blasting practices have enhanced safety, recovery and productivity in all cut and fill operations.

The Kwinana Nickel Refinery began operations in 1970 processing sulphide concentrates from Kambalda using the Sherritt Gordon pressure leach and hydrogen reduction process to produce nickel metal as powder and briquette. Over the years a number of changes have seen the refinery capacity increased from the original design capacity of 15 000 tpa nickel on an all concentrate feed to its current design capacity of 42 000 tpa nickel on an all matte feed. This paper outlines the changes in plant equipment and process for the most recent refinery expansion from 35 000 tpa to 42 000 tpa which have been implemented since 1992.

A proved routine mapping practice is described involving detailed observations on grade and structure. The equipment and methods employed are giving good service under the conditions encountered. A well-maintained and low cost mining operation today requires the use of condition monitoring techniques. At ERA - Ranger Mine oil analysis, vibration monitoring, infra-red themtography, and performance analysis have been successfully applied over the last four years to lower operating and maintenance costs. All condition monitoring is scheduled through a comprehensive in-house developed maintenance management system known as the Engineering Information System (EIS). (EIS) is used extensively at Ranger to ensure consistent frequency and recording .of all condition monitoring data. This paper provides an insight into the benefits of condition monitoring and explores the future direction for condition monitoring techniques at Ranger.

MOIL Limited is operating 7 underground and 3 opencast mines in central India and producing annually more than 1.2 million tonnes of various grades of manganese ore. The underground mines are operating at shallow - 90 m at Munsar mine to moderate - 353 m at Balaghat mine below the surface with horizontal cut and fill (HCF) or its variant method of stoping with post filling by hydraulic sand stowing. The Munsar mine is being worked underground since 1903. The opencast mining is presently stopped. The ore body below the opencast quarry is geological continuity of the area excavated in past by opencast method of mining. The underground mining is done through the adit in three levels i.e. 270’L, 220’L, 170’L and by incline in western part of the property at 70’L. The horizontal drift development has been developed in the manganese ore body with sill drive of 5 m above the sill pillar in lower level at 70’L and barrier pillar of 5 m thickness is left for protection of upper level at 170’L. Valuable mineral has been locked in sill pillar and therefore rock mechanics investigations have been carried out. On the basis of study now the drift development has been carried out in footwall rock at 70’L, (-) 30’L and below levels and it has improved the mineable manganese ore by about 20%. In this modified HCF stope design, an alternative fill material in place of sand, which has been developed in house by use of overburden (OB) material after treatment, has been used for hydraulic stowing. The experimental trials find out that the alternative fill material of OB is more compact and forming a non expansion floor for the men and machines in the stope. This will certainly help in the introduction of mobile underground mining equipment for drilling in the stopes and mechanical mucking, transportation and loading of the ROM. The paper presents rock mechanics investigations and future use of alternative fill material of OB for better safety and productivity at Munsar Mine of MOIL. A thousand writers have harped upon the wondrous nature of the inventive faculty. They realise that man is a creator, and that capital and labour alike are, and will always be impotent without machinery and processes that is invention. What Bacon has said is echoed by them all, "the introduction of new inventions seemeth to be the very chiefest of human actions. The benefit of new inventions may extend to all mankind-they make all men happy without injury to 'any' they are new creations and imitations of God's own works." My remarks will occupy another field I will refer to some of those features of the Patent Laws of different countries, which prevent the use of new inventions, oppress the development or inventive' genius, and diminish human happiness and progress' Wherever Patent Laws are framed and administered upon wrong principles, these results are inevitably found. A concise, complete, common-sense comparison of the chief Patent Laws of the world cannot easily be found but to avoid seeing scrappy, misleading references to Patent Law in the press particularly the English-would be equally difficult. Writers who are considered authorities so frequently make minor issues prominent and so uniformly fail to give weight to fundamental principles that the effect on the public is to mislead.

Anglo Platinum recognised with its expanded operations the need to change its evaluation strategy due to increased sample volumes and the pressure it created at laboratories. The sender/receiver analytical scenarios had to change in order to curb costs and to prevent substantial investments in new sampling and analytical facilities. Given the high cost associated with correct sampling systems and sampling protocols and the shortage of technical resources a strategy of single point sampling and analysis was introduced. The analysis was then done at an ISO17025 accredited laboratory with ten per cent checks, subjected to careful statistical scrutiny. Logistical problems increased with respect to weighbridge congestion, maintaining sampling quality, throughput of samples in the sample preparation areas, manual quality control systems in place, audit trails and analytical capacity throughout all operations. This required the expedited improvement of existing sampling practices and the development of new sampling methods and equipment. Anglo Platinum is classifying and standardising sampling equipment, sample preparation and analytical procedures for all process streams with the view to minimise overall uncertainties. Major investments were made into a customised LIMS and automated equipment for data management, reduction of manual transcription errors and streamlining of sample processing. Anglo Platinum is committed in rolling out the enhancements to all sites as priority dictates.

The failure of roof of thin bedrock working face under loose aquifer in a coalmine can lead to water-sand inrush disaster. Based on the accident of water-sand inrush in 30108 working face of a mine in Shaanxi province, China, the mechanism of water-sand inrush is theoretically analysed. The process and critical conditions of water-sand inrush in working face are reproduced by numerical simulation where the bedrock thickness is 20 m, the clay layer thickness is 40 m and the mining height is 6.5 m. The study found that under the condition of thin bedrock and thick clay layer, the characteristics of the clay layer have a significant inhibitory effect on the development of the fractured zone, and the development of the water-conducting fractured zone is incomplete and insufficient. However, in the process of continuous mining of multiple adjacent working faces, the strength of the clay is gradually reduced by the influence of mining and the erosion of water, and the height of the fractured zone is gradually increased. In the subsequent mining process of the working face, the roof is more likely to be cut-off and lead to water-sand inrush. From the initial failure of the clay to the final crack directly through the surface, the shear strength of the clay is reduced by about 52 per cent. Further simulation found that in addition to the characteristics of the clay layer itself, the main factors affecting the water-sand inrush in the working face caused by overburden failure also include the difference in bedrock thickness and strength, and the difference in coal seam mining height. With the increase of bedrock thickness, the development height of the fractured zone increases gradually with increases of bedrock thickness when the cutting failure of overburden rock no longer occurs. With the increase of mining height, the height of water flowing fractured zone also increases, but the cracking ratio gradually decreases. Therefore, it’s of great practical significance to systematically study the mechanism and critical conditions of water and sand inrush in mines and the main influencing factors for preventing similar accidents in mines with similar conditions.

Mark Westfield, writing in The Australian on 16 May 1994 stated: The role of the independent expert is crucial to Australia's investment system. When it falls down, there can be very dramatic consequences. The scope of these consequences was dramatically illustrated in the decision of Rogers J. at first instance in Cambridge Credit Corporation v. Hutcheson (1983) 8 ACLR 123 when the auditors of the failed company were found liable for breach of contract and faced a damages award of $145 million, well in excess of their insurance cover. No comfort should be sought in the fact that this decision was overturned on appeal, for the result of the appeal rested on whether there had been a causal connection between the breach of contract which had occurred and the loss suffered, rather than on the fundamental principle of auditors' liability. Thus: Those who prepare experts' reports in company cases carry a heavy moral responsibility, whatever their legal duties may be... Unless high standards are observed by those who prepare these reports, there is a danger that systems established for the protection of the investing public will, in fact, operate to their detriment through reliance placed on those reports and on the reputations of those who furnish them. PT Nusa Halmahera Minerals (PT NHM) is an Indonesian joint venture company owned by Newcrest Mining Limited (75 per cent) and PT Aneka Tambang (Persero) (25 per cent). PT NHM operates two underground gold mines, Kencana and Toguraci at the Gosowong mine site in North Halmahera District, North Maluku Province, Indonesia.On 8 February 2016, a major ground movement occurred in the K-Link orebody of the Kencana underground mine, effectively blocking the only two access ways to the stoping area. Following mine clearance, it was found that a Solo production drill rig operator, who had been drilling in the Level 10 Extraction Drive was still underground, cut off from the rest of the mine by the ground movement.Communication, nutritional and logistic supply to the trapped operator was established with the breakthrough of two diamond drill holes. A range of technical expertise and opinions from both on and off-site were sought and two options were chosen for implementation of the rescue operation. Lihir operations of Newcrest Mining Limited (NML) are on Lihir Island (PNG) which is located approximately 900 km north-north-east of the national capital Port Moresby. The mine is located within the Luise Caldera of the Luise Volcano which is located on the east coast of the island. The caldera is an extinct volcanic crater that is geothermally active. Volcanic activity on Lihir Island is limited to remnant hydrothermal venting in the Luise Caldera in the form of hot springs and fumaroles. Steam and gas (including H2S) naturally discharge within the pit area and along the Kapit beach and near shore region. The hydrothermal reservoir temperatures can reach 100°C at the water table and exceed 200°C at depth. Newcrest maintains a Geo-hazard Management Plan to identify and manage the various geotechnical and geothermal hazards on-site. The major geothermal hazard to mining at Lihir is the occurrence of geothermal outbursts, which have occurred in the past with varying intensities and caused two ‘near misses’, production delays and material losses.

Safe work Australia (SWA) has a number of proposed amendments to existing Workplace Exposure Standard (WES) values that may dramatically change how mining workplaces using personal gas monitoring instruments will operate. In consultation with leading gas monitor manufacturers and distributors, an assessment of common Australian alarm settings values and analysis of potential frequency of increased alarm likelihoods has been conducted. This assessment explores the relationships between current monitoring practices, using instruments as health and not just safety tools and identify what will be needed to minimise worker ‘alarm fatigue’ and for common tasks to continue safely. This paper provides the necessary information to aid industry and hygienists to be ‘future ready’ given the likely impact that order of magnitude reductions in time weighted average (TWA) values for NO2, SO2, H2S and other common gases will have on current working practices. Reductions to gas alarm values will provide significant challenges to existing work methods and will require, practical application of the pending future values. Improvements and awareness of instrument capabilities by safety and hygiene professionals will better utilise gas monitors real-time data monitoring capacity and no longer be just ‘safe or not-safe’ instruments. The presentation seeks to promote the practical steps required in setting alarms based on risk consequence and likelihood, increase general awareness of the tools and data available in modern instruments and will enable industry professionals to better manage the challenges of any potential future changes.

The Ranger Uranium Mine is located in the wet-dry monsoonal tropics approximately 250 km east of Darwin in the Northern Territory of Australia (Figure 1). Mining of open-cut ore bodies commenced in 1980 (Pit 1) and ceased in 2012 (Pit 3) and milling and production are scheduled to cease by 2021. At the conclusion of mining and milling operations, mine tailings will be returned to the mined out pits 1 and 3, which must then be physically isolated from the environment for at least 10,000 years (Australian Government 1999). The final landform also needs to have sufficient mitigation measures in place to ensure that areas downstream are not impacted by erosion products from the mine site. The proposed post-mining rehabilitated landform will cover approximately 847 hectares (8.5 km2) which represents an area that has the potential to supply sediment to catchments and streams downstream. One method of reducing runoff and associated sediment is the construction of rip lines along the contour to slow down the runoff velocity and to also trap fine sediment and nutrients in the depressions to assist vegetation growth.CITATION:Saynor, M J and Lowry, J B C, 2018. Mining education in Australia has undergone another cyclical downturn over the past five years and several departments have been discontinued while others are still teetering on the brink of destruction. During this downturn Curtin UniversityÆs WA School of Mines (WASM) has managed to stay alive and well, but this has not been by accident. This paper outlines some of the strategies and experiences that have kept WASM in a strong position and offers hope to other departments and schools elsewhere that are still experiencing difficulty. This paper is also directed at all mining departments and schools that are certainly going to see it all happen again after the next mining boom turns to bust.

The Northside Storage Tunnel is a TBM driven rock tunnel system designed to collect and store sewage from four major storm overflow points along the existing Northern Suburbs Ocean Outfall Sewer. At present, sewage overflows during wet weather are the main source of pollution for Sydney Harbour. The tunnel system comprises a 16 km long by 3.8 m to 6.5 m diameter tunnel from Lane Cove to the North Head Sewage Treatment Plant and a 4 km long by 6.3 m diameter branch tunnel from West Chatswood. The tunnel will have a typical rock cover of about 50 m to 150 m. Most of the tunnel alignment is contained within the Hawkesbury Sandstone Formation which is generally an excellent tunnelling stratum, and offers few geotechnical constraints. However, the tunnel is required to pass beneath incised palaeochannels at Middle Harbour and Manly with each palaeochannel containing about 70 m of sediment.Innovative geotechnical and geophysical investigations were undertaken to examine the tunnel route and characterise the rock beneath the palaeochannels. These investigations have located zones of poor quality rock beneath one palaeochannel in particular, suggesting significant stress relief had taken place in the rock at times of lower sealevel producing structural features indicative of valley bulging. This paper presents the geotechnical model developed for the tunnel, and the models and constraints for tunnelling beneath the palaeochannels.

The Nickel Rim Depth Cu-Ni-PGE deposit, discovered by Falconbridge Limited in 1993, illustrates several trends in exploration in the Sudbury area. Recent discoveries tend to be deep, high grade, footwall hosted, close to or straddle property boundaries and made using a multi-disciplinarian exploration approach that integrates economic considerations with geology, metal geochemistry and geophysics (borehole EM). The Nickel Rim deposit occurs down dip of Inco's Victor Deep deposit (4.2 million tonnes at 2.25 per cent Ni, 7.30 per cent Cu) in the eastern margin of the Sudbury Igneous Complex (SIC) at a depth of 2650 m. Before commencing drilling an economic evaluation was done to determine the minimum size needed to be economic at the expected depths and determine the expected value of an exploration venture. The subsequent surface exploration program discovered a drill indicated, undiluted mineral inventory of 1.6 million tonnes of 1.58 per cent Ni, 10.13 per cent Cu, 4.17g/t Pt and 3.50 g/t Pd including 519 000 tonnes of 4.13 per cent Ni, 25.53 per cent Cu, 9.76 g/t Pt and 8.42 g/t Pd. The deposit is hosted by gneiss, migmatite, gabbro and pseudotachylite (Sudbury Breccia) 100 m below the base of the SIC.

The colliery is owned by the BHP Collieries Proprietary Ltd., the principal shareholders being the Broken Hill Pty. Company Ltd. and Hebburn Limited.It is situated due west of Newcastle and 6 south-east of Cessnock, N.S.W., and is conneeted by 36 miles of railway to the port of Neweastle. Of this mileage, 3 miles are owned by the Company, 3 by Hebburn Limited, 9 by the South Maitland Railways Ltd., and 21miles by the Government Railways.The seam worked is known as the Main Greta Seam of the Greta Coal Measures and is on the southern slope of the Lochinvar anticline.At the shaft, the seam is 1190 ft. below the surface, and the section of the coal measures is:-Upper or Main Seam, . 24 ft. 0 in.Conglomerate and shale 70 ft. 0 in.'Middle Seam 8 in.Conglomerate 25 ft. 0 in,Bottom Seam 1 ft. 2 in.These lie between two marine measures, and are the lower coal series of the permo-carboniferous period.Only the upper seam is workable in this area, the lower seams being too thin for eompetitive working. The section of the workable seam at the shafts is as shown on Fig. 1.Unfortunately, only the lower 12 ft. 5 in. of the seam is saleable.
