"“Safety Pays in Mining” web application developed by the U.S. National Institute for Occupational Safety and Health Mining Program, helps mines determine the potential costs associated with mining injuries. This web app groups injuries by type, either by the cause of the injury or by the nature of the injury. When the user selects one of more than 30 common types of mining injuries, the app provides information on the distribution of costs of workers’ compensation claims for that type of injury. Based on other user inputs, the app will estimate the total costs of the selected injuries, including an estimate of additional indirect costs, the estimated impact of total injury costs on mining company profits, and provide some examples of other items, such as services or equipment, on which companies could spend the savings that result from the prevention of injuries. This paper reviews the app by discussing its development, how it is used to show the true costs of mining injuries, and how mines can benefit from using it.IntroductionInjuries on the job cause pain and suffering. They can also profoundly affect profits and daily operations. In addition to paying direct costs or increased premiums for workers’ compensation insurance, a company might need to pay overtime for other workers to fill an injured worker’s job role, cover training costs for a replacement worker or divert administrative resources in the wake of an injury. “Safety Pays in Mining” is a web app that estimates the distribution of these injury costs and assesses the impact that occupational injuries have on a company’s profits.The costs of specific types of occupational injuries in mining are not readily available as mining and insurance companies do not usually share this information. As a consequence, companies only have cost information based on previous experience with their own employees. Therefore, if a mine never experienced a finger amputation for one of its miners, for example, it would not be aware of the possible costs associated with this type of injury. In addition, injury costs are unique in that the cost distribution is so wide that just using the average cost of a specific injury type does not provide adequate information. Some injuries involve immensely high costs, and even though the risk of these high-cost injuries occurring is low, mines need to be aware of the potential impact on their company’s financial health."

To accommodate larger double-stack rail cars through a tunnel, the existing active tracks were to be lowered. The tunnel is an elliptical, horseshoe-shape with concrete base support, removal of which would jeopardize the tunnel integrity, particularly since the soils beneath part of the tunnel floor included soft, saturated silt overlying rock. Grouted bar anchors combined with a SuperJet soilcrete strut replaced the existing support in situ prior to floor lowering. Close cooperation and detailed timely engineering support provided a quality project. Project design and construction, ground improvement methods, and the performance of the work are described.

The purpose of this paper is to outline briefly some basic principles of ground-water hydrology that have application in mine hydrology and to suggest that an understanding of these principles can help to forestall problems that are internal as regards mining operations and external as regards damage or harm to surrounding areas. The development of a mine below the water table pleads for accurate values to answers of hydrologic problems. Such internal problems involve both an inflow of water without adequate drainage facilities and a deficiency of water for the required mining or milling use. Methods which give acceptable quantitative values for the solution of many ground-water problems depend on rather simple geologic structures, characterized chiefly by homogeneous rocks of considerable areal extent. Unfortunately, mining operations are likely to be centered where rocks are not homogeneous. Consequently, methods that attempt to show values of the storage and transmitting capacity of rocks in mining areas must be applied with caution. In spite of complex geology and apparent uniqueness of individual mines and quarries, adherence to some concepts of ground-water occurrence and movement should be helpful to mine operators. Mining operations that extend below the water table disturb the natural hydrologic regimen. Prior to development, the water table slopes away from the site of a mine in at least one direction, but as soon as development begins and water is removed from the mine, the mine becomes the apex of a depressed cone in the water table, causing a convergence in the flow of water toward the mine, The behavior of the cone of depression, as it tends to deepen and to increase in areal extent when the mine is deepened, is related to the rate of withdrawal of water necessary to keep the mine dry. The significance and nature of the cone of depression in the vicinity of pumped wells has been studied by ground-water hydrologists for years, but the cone caused by pumping water from mines has generally been less systematically appraised. Repeated noise exposure and occupational hearing loss are common health problems across industries and especially within the mining industry. Large mechanized processes, blasting, grinding, drilling, and work that is often in close quarters put many miners at an increased risk of noise overexposure. In stone, sand, and gravel mining, noise is generated from a variety of sources, depending on the type of ore being mined as well as the final consumer product provided by that mine. Depending on the source of noise generation, different strategies to reduce and avoid that noise should be implemented. The National Institute for Occupational Safety and Health (NIOSH) has evaluated the noise profile at three operational surface stone, sand, and gravel mines. A-weighted sound level meter data as well as phase array beamforming data were collected throughout the mines in areas with high noise exposure or high personnel foot or vehicle traffic. Sound level meter data collected on a grid pattern was used to develop sound profiles of the working areas. These sound contour maps as well as phase array beamforming plots were provided to the mines as well as guidance to modify work areas or personnel traffic to reduce noise exposure.

As part of a $2.2 Billion bridge project that will link Indiana and Kentucky, an interchange project northeast of Louisville will require the mining of a 6-lane tunnel system about 2,000 ft long in order to cross beneath the historic Drumanard Estate. This paper will discuss preliminary engineering that has been undertaken to compare two-bore and three-bore layouts, the planning of an exploratory tunnel facility, and the potential for including a test chamber to investigate the ability of high capacity roadheaders to mine the 22,000 psi limestones.

"The solvent extraction of cerium(III) and samarium(III) from mixed solutions of all rare earth elements (REEs) except promethium using PC88A (2-ethyl hexyl phosphonic acid mono-2-ethyl hexyl ester), an acidic extractant, was investigated. The effects of aqueous equilibrium pH, various acids (sulfuric acid (H2 SO4 ), hydrochloric acid (HCl) and nitric acid (HNO3 )), initial metal ion concentration and extractant concentration on the solvent extraction of Ce(III) and Sm(III) were systematically studied. From the experimental results, HNO3 and HCl were found to be the preferred aqueous media for the solvent extraction of both Sm(III) and Ce(III). The most promising experimental conditions of pH, PC88A concentration and metal ion concentration were identified for the extraction of Ce(III) and Sm(III). Separation results showed that H2 SO4 at pH 2 provided the best separation between Sm(III) and Ce(III). IntroductionIn recent years, demand for rare earth elements (REEs) has increased significantly because of their many commercial applications. REEs have especially important high-tech applications owing to their unique physical and chemical properties, and they are used in a wide range of areas such as catalysts, ceramics, magnets, lasers, glass, neutron absorption and alloys. As REEs have very similar physicochemical properties, their separation and purification from each other is challenging and gaining significant interest. Samarium (Sm) and cerium (Ce) are found in different REE minerals, including bastnäsite ((Ce,La,Nd(CO3)F), xenotime (YPO4) and monazite ((Ce,La,Nd,Th)PO4) at major production sites in the United States and China, as listed in Table 1 (Bleiwas and Gambogi, 2013). The most common acidic organophosphorous extractants used for REEs extraction and separation are DEHPA (di-(2-ethylhexyl)phosphoric acid), PC88A (2-ethyl hexyl phosphonic acid mono-2-ethyl hexyl ester), Cyanex 272 (bis-2,4,4-trimethylpentyl phosphinic acid) and Cyanex 301 (bis(2,4,4-trimethylpentyl)dithiophosphinic acid) (Sarangi, Reddy and Das, 1999; Surampally et al., 2012; Tian, Jia and Liao, 2013). Out of these extractants, PC88A is known for providing higher separation factors (Acharya, Mishra and Misra, 2015). El-Hefny et al. (2010) investigated the kinetics of the solvent extraction of Sm from chloride media using sodium salt of Cyanex 272 and reported the extracted species to be Sm(OH)A2•2HA. Lee et al. (2005) investigated the solvent extraction of Sm(III) with PC88A and saponified PC88A from chloride solutions and found the stoichiometry of the extraction reactions to be 1.5 with PC88A and 3 with saponified PC88A."

The paper shares programs that presently use the CDC PLATO System to train individuals in industry. A short history of PLATO development; as a training system is presented with reflection on the major training trends being used to improve training effectiveness. A discussion is offered to describe the benefits of using a system for individualizing instruction. An introduction; is offered showing how one can recognize' individual differences, with an explanation of how the strengths of a computer are used to deliver and manage the training. Simulations of hazardous processes are shown to be one of the most effective uses of the computer for training.

Last month's engineering fundamentals article discussed some general considerations of underground mine ventilation. This month's article takes a look at auxiliary ventilation-brattice, fans, and ducting-in underground mines. The need for good, efficient auxiliary ventilation increases as mines get deeper and move farther from ventilation shafts. New mining methods and equipment resulted in faster work area advances, liberating more dust, gas, and heat. As a result, worker health and safety requirements in the face area have become more stringent. For all these reasons the safety and efficiency of any underground mine depends heavily on its auxiliary ventilation. The purpose of such ventilation is to supply sufficient quantities of clean air to the face of the drift and to maintain air quality within desired limits until expelled from the mine.

The Henderson/ML King CSO Project is a major combined sewer overflow (CSO)reduction project for the Department of Natural Resources and Parks of King County, Washington. The central element of the project is the 3100 lineal feet long, 14'-8"diameter 42nd Ave S Storage/Treatment Tunnel. During the course of the project, the owner and consultants evaluated numerous elements of the tunnel including the alignment, geotechnical conditions, tunnel liner, and settlement monitoring requirements. The contractor utilized innovative shaft construction methods for the tunnel portals, and adhered to strict contract requirements in successfully constructing the tunnel. This paper provides an overview of the design and construction elements of the 42nd Ave S Storage/Treatment Tunnel that led to the successful completion of the project.

Exploration activities in 1980 felt the effects of unstable economic conditions and oscillating metal prices. In the cyclical, boom-or-bust precious metals and uranium markets, precious metals activity flourished and uranium was definitely in a slump. By November last year, the price for vellowcake was down nearly $20 from two years earlier. Many mining and milling operations closed, particularly in Wyoming, New Mexico, and Colorado. Decreasing emphasis on nuclear power continued to impact the market, as did the Three Mile Island accident of 1979. Earlier forecasts estimated that 10 to 12 reactors would be ordered per year by US utilities throughout the 1980s, but current forecasts suggest that it may be 1983 or 1984, or even later, before any new US plants are ordered, according to Thomas A. Boyden, vice president of Nuclear Exchange Corp. "Without those new plants, the consumption loss to industry is 4 to 5 million pounds (1.8-2.3 kt) of uranium per year. The cumulative loss to the industry is currently estimated at 25 to 30 million pounds (11.3-13.6 kt)," Boyden said in a May 1980 presenta¬tion. Despite the plunging market, there were a few notable uranium explora¬tion programs: •Exxon expanded uranium exploration in New Mexico, where the company reportedly took preliminary steps toward acquiring mineral rights to a 22-km2 prospect, 19 km south of Santa Fe. • Mountain States Resources began the second phase of an exploratory drilling program on its Sinbad/ Solo prospect in Utah. • Also in Utah, Cotter Corp. drilled its first exploratory hole in the Dirty Devil River district. Exploration for precious metals in 1980, however, appeared to tend toward the opposite extreme. High prices sparked near feverish searches in Alaska, California, Colorado, and Nevada. Numerous mines abandoned years ago were revived with some signs of success. Significant gold finds were reported in California and Nevada: • Homestake Mining Co. announced a gold discovery in California estimated to contain over 5 Mt with an average grade of about 5.8 g/t. Because the deposit consists of finely disseminated particles not visible to the eye, many speculate that prospectors bypassed the area during the 1849 California gold rush. • Newmont Mining Corp. discovered two gold ore bodies-Maggie Creek and Gold Quarry. Maggie Creek reportedly contains an estimated 13.7 t of gold in 4.3 Mt of ore, and Gold Quarry contains about 31.1 t of gold in 9 Mt of low grade ore and 2.3 Mt of higher grade ore. Highlights of this year's exploration review include articles on the modest growth recorded in geophysical activity, technical developments in exploration geochemistry, the variety of new instruments being tested or made available for exploration, and a state-by-state exploration review for the year.

Mountaintop removal (MTR) mining started in West Virginia nearly 30 years ago. This surface mining method can extract multiple coal seams as thin as a few inches in whole and minimum rock dilution in very steep mountainous terrain. MTR is highly efficient, even if coal seams are in close proximity and small in a real extent. Consequently, MTR has been widely used in the Central Appalachian coalfield (mainly Kentucky, Tennessee, Virginia and West Virginia) with great success. In that respect, there has been no demand for research and little scientific research has been performed to investigate its impact on the surrounding area. During the past two years, however, the debate over MTR mining has grown quickly and has become very intense. This article describes chronologically issues raised by various parties, legal actions, impacts on mining permit applications and research projects developed to address the issues.

The Bureau of Mines has studied 260 underground coal mine fires which occurred from 1970 through 1989. Every fire had the capacity to be hazardous to life and property, however upon closer examination some were found to be inherently more dangerous than others. Our research involved reviewing and recording specific information contained in published Mine Safety and Health Administration fire accident reports. The objective was to determine if degrees of hazard relative to fire source or location could be construed from information contained in the reports. We specifically looked at the propensity for fires to cause injuries and fatalities as well as the need to seal the area in order to extinguish the fire. Relative to all underground locations, this review found that fires occurring in main haulage-ways were most likely to cause injuries and fatalities as well as require sealing of at least the immediate area to extinguish.

Coal ribs present a major safety hazard in underground mines, and there is currently no unified approach for their design in the United States. To tackle this issue, this study investigated the stability of underground coal pillar ribs under development load. Utilizing over a thousand 3DEC models, a framework for calculating coal pillar rib rating (CPRR) was developed. A detailed step-by-step method was outlined for computing the CPRR based on a 3DEC analysis, which was then validated with various 3DEC models, demonstrating a strong correlation between the two models. The results demonstrated that the rib factor of safety (RibFOS) under development load is directly proportional to CPRR, coal mine roof rating (CMRR), and floor strength, and inversely proportional to the overburden depth and rib height. Moreover, a regression equation was formulated to predict the RibFOS based on the aforementioned parameters. Furthermore, empirical relationships were developed between the RibFOS and the applied primary rib support density (PRSD) in U.S. coal mines. To ease the calculations, a stand-alone application, Design of Rib Support (DORS), was developed. This application can be used to assess rib stability and easily calculate CPRR, RibFOS, and PRSD.

Blasting is the main method of production in many non-coal underground mining operations and produces multiple toxic gases as a result. The Mine Safety and Health Administration (MSHA) requires mine operators to measure the level of toxic gases in mines as frequently as necessary to ensure they are below regulatory safety limits. The current practice uses portable gas monitors to check the concentrations of toxic gases after a fixed post-blast time. This paper studies the application of a gas monitoring system in the return entry of a limestone mine to determine a safe re-entry time. The National Institute for Occupational Safety and Health (NIOSH) conducted such a monitoring program in a limestone mine from September 2016 through May 2018. NIOSH/PMRD (Pittsburgh Mining Research Division) is endeavoring to develop workplace solutions to improve detection of and reduce the risk of hazardous conditions. This study showed that the use of gas monitoring in the return air can be a useful tool at the mine operator’s disposal to detect and reduce the risk of hazardous conditions and also to reliably estimate the re-entry time. INTRODUCTION Underground miners are at risk of overexposure to toxic gases from various sources. Production blasting operations using explosives in underground mines are a source of multiple toxic gases that could be harmful and even fatal to humans if prolonged exposure occurs beyond the safe limits. According to Code of Federal Regulations, Title 30, Part 57.5002 [1], metal and nonmetal mine operators must conduct gas, mist, and fume measurements as frequently as necessary to determine the adequacy of control measures to protect miners from overexposure to harmful gases. MSHA will inspect mine operators’ activities to verify the evidence of such measurements [2,3]. Continuous gas monitoring in the return air is an effective way to make sure that toxic gas concentrations at the working areas are kept below the regulatory limits before the return of miners. Toxic gases can enter the mine atmosphere from different operations, such as welding, combustion, and blasting. The main gases produced from blasting are carbon monoxide (CO), carbon dioxide (CO2), nitric oxide (NO), and nitrogen dioxide (NO2). Other gases are produced in small amounts and, therefore, were not considered for this study. Related regulatory and recommended exposure limits of CO, NO, and NO2 gases are summarized in Table 1 [4].

Since its discovery, Cuba has been called the “Pearl of the Antilles.” The country remains that, albeit a little tarnished these days. In Havana, many of the buildings are being refurbished, while some new ones are under construction. Since 1991, the laws have been relaxed so that Cuba is developing a mixed economy of central control with free market. Joint venture agreements have been signed with businesses from many countries. This does not include the United States due to its embargo. But even these regulations have been relaxed some.

"Pareto analysis was used to investigate the failures database of a Hitachi EX1900 hydraulic shovel over one year of continuous operation. The hydraulic system and hydraulic links, hoses and piping were identified as the most critical system and subsystem, respectively. A three-parameter gamma distribution function was identified as the best fit for a probabilistic modeling of the time between failures (TBFs). This distribution was used in a Monte Carlo reliability analysis of the hydraulic system. Autoregressive integrated moving average (ARIMA), within the context of a time series analysis of the TBFs, was proposed as a viable alternative to the traditional technique of distribution fitting. The mean absolute percentage error (MAPE) metric was used to compare the two models. The MAPE value that resulted from the ARIMA was 66 percent smaller than that from the traditional technique in predicting the TBFs.Resumen ¦ Se usó análisis Pareto para investigar la base de datos de fallas de una pala hidráulica Hitachi EX1900 durante un periodo de un año de operación continua. Se identificó al sistema hidráulico y a los enlaces hidráulicos, mangueras y tuberías como el sistema y subsistema más importantes, respectivamente. Se identificó una función de distribución de tres parámetros gamma como el más apto para un modelado de probabilidades del tiempo transcurrido entre las fallas (Tabes por sus siglas en inglés). Esta distribución fue usada en un análisis de confiabilidad Monte Carlo para un sistema hidráulico. The Bardon Trimount, Inc. stone quarry in Ashland, Massachusetts was the recent subject of a quarry closure review report. The quarry produces aggregate using standard drilling and blasting techniques. Stone is processed at an on-site plant. Noise monitoring data collected during the closure review indicate that the quarry generally operates within the quiet range. There is a decrease in noise while the quarry is not operating. Levels of dust associated with the quarry are low. Unpaved roadways are the greatest contributors of fugitive dust associated with the mining operation. The quarry cannot be seen from any homes within the town of Ashland, except during the winter months. The visual impact of the quarry on surrounding properties is minimal. Further quarry development is likely to reduce visibility of the quarry from some areas.

"U.S. Mine Safety and Health Administration (MSHA) regulations require underground coal mines to use refuge alternatives (RAs) to provide a breathable air environment for 96 hours. One of the main concerns with the use of mobile RAs is heat and humidity buildup inside the RA. The accumulation of heat and humidity can result in miners suffering heat stress or even death. MSHA regulations require that the apparent temperature in an occupied RA not exceed 95 °F. To investigate this, the U.S. National Institute for Occupational Safety and Health (NIOSH) conducted testing on a 23-person tent-type RA in its Experimental Mine in a test area that was isolated from the mine ventilation system. The test results showed that the average measured air temperature within the RA increased by 9.4 °C (17 °F) and the relative humidity approached 94 percent at the end of a 96-hour test. The test results were used to benchmark a thermal simulation model of the tested RA. The validated thermal simulation model predicted the volume-weighted average air temperature inside the RA tent at the end of 96 hours to within 0.06 °C (0.1 °F) of the average measured air temperature. IntroductionIf an accident occurs in an underground coal mine, miners who fail to escape from the mine can enter a refuge alternative (RA) for protection from adverse conditions, such as high carbon monoxide levels. One of the main concerns with the use of mobile RAs is the potentially adverse thermal environment inside an RA from the metabolic heat of the occupants and the heat released by the carbon dioxide (CO2) scrubbing system. Moreover, the humidity within the RA will increase through occupants’ respiration and perspiration, and from the chemical reaction within the CO2 scrubbing system. The accumulation of heat and humidity can result in miners suffering heat stress, heat stroke or even death."

Foskor operates a mine in the Northern Province of the Republic of South Africa exploiting the igneous complex at Phalaborwa. It produces about 3 million tons of phosphate concentrate per year by means of froth flotation from 26 million tons of ore of which 16 million is mined and milled on site. The remaining 10 million tons originate at the Palabora Mining Company and is pumped as a slurry to Foskor. Two types of ore are mined and milled namely Foskorite and Pyroxenite. (The main mineral content of this ore is Apatite, Phlogopite and Diopside.) After three stages of crushing the ore is milled using 24 rod mills and 5 verti mills. The apatite is then recovered using four stages of froth flotation. The stocks of Foskorite ore are rapidly being depleted and the PMC tailings will also fall away by the year 2002 so that the whole mining operation will then be centered around the Pyroxenite ore. This will lead to substantial increases in production costs. The recoveries achieved in the flotation plant are largely dependent on the successful operation of the comminution section. Parts of this plant are now 35 years old and the advantages of re-engineering, new technology and automation can only be applied to a limited extent in an effort to reduce costs. It was then decided to investigate alternative milling methods, both wet and dry.

This paper presents tunnel design considerations for the SR75/282 Transportation Corridor Project in Coronado, California. The project is managed by the City of Coronado in cooperation with Caltrans and the US Navy and may include a one-mile long twin-tunnel section with two options: a 36.5 foot diameter twin-bored tunnel or a twin cut-and-cover box. The alignment crosses the Coronado Fault, and design provisions were needed at the fault crossing to maintain life safety and prevent rapid inundation of the tunnel in the event of fault rupture. These provisions are discussed along with considerations related to liquefaction and ground shaking. The project is currently in the planning stage with construction to start around 2012.
