The principal causes of unstable boreholes* have been known for many years. For example, in a paper published in 1938, Halbouty and Kaldenbachl listed nearly all the classes of troublesome shales that we know today. Much progress has been made since then, but we still have difficulty in identifying the cause of hole problems and consequently in selecting the right technique for combatting them. This subject was discussed at length by Kelly2 in a recent paper and much valuable information was provided. In the Shell Laboratory it was felt that there was a need for a better understanding of the basic mechanisms underlying the various forms of hole instability, and accordingly, a study was undertaken. This entailed reviewing the physics and chemistry of shales, developing tests for characterizing shales, and constructing a model to study borehole stability. Full simulation of underground conditions was not attempted; for example, we used reconstituted shale specimens, and often simple solutions or suspensions rather than practical drilling muds. Such procedures are necessary in order to establish mechanisms, but caution must be exercised in applying the results to the more complex conditions in the field. Part I — Compaction Swelling and Dispersion of Shales Compaction of Sediments Shales are formed by the compaction of sediments. Water is expressed as sediments are buried by subsequent layers, and the degree of compaction is proportional to the depth of burial, provided the water is able to escape easily to permeable formations. The younger sediments, when recovered at the surface, soften and disperse when mixed with water; the older shales have undergone diagenesis (alteration of the clay minerals, secondary cementation, etc.), and remain hard and unaffected by water. In this paper we shall apply the term "shale", as the drilling engineer does, to everything from clays, which are highly reactive to water, to completely lithified materials such as claystones and slates, which are completely inert. However, because these materials behave quite differently when encountered in the drilling well, it is desirable to develop a simple means of characterizing them. The feature that distinguishes the various shales is their dispersibility in water —soft clays disperse readily, harder shales disperse slowly when agitated, the lithified materials will not disperse at all unless milled. In the dispersibility test used in our study, a weighed amount of dry shale particles in the 12-to-100-mesh-size range was placed in a tube filled with distilled water and allowed to soak for 16 hours. The South 180th Street Grade Separation underpass in Tukwila and Renton, Washington, USA, permits vehicular traffic to pass beneath Burlington Northern Santa Fe and Union Pacific railroad tracks. The railroad tracks were temporarily relocated onto a shoofly crossing while the permanent bridges were constructed. The maximum underpass depth is approximately 25 feet (7.6 meters [m]). The majority of the sidewalls are supported using alternating reinforced and unreinforced secant pile walls. Temporary and permanent struts brace the top of the excavation. Because the groundwater is about 15 feet (4.6 m) above the base of the excavation and the subsurface materials are loose to medium dense fill, alluvium, and soft flood plain organics, the base of the excavation was strengthened and sealed using wet cement deep soil mixing (CDSM) methods. During construction, CDSM-treated ground temporarily acted as part of the railroad shoofly shoring while the remainder of the underpass was excavated on either side of active railroad tracks. The combination of secant pile sidewalls and CDSM bottom seal allowed for easy excavation, even during wet conditions. Using a 35-foot-thick (maximum) (10.7-m) CDSM layer both to act as a bottom seal and to resist uplift pressures is a unique, dual-purpose application of CDSM technology in the United States.

"Milling operations in northern Saskatchewan’s Athabasca Basin have produced uranium continuously since 1975. Over that period, the Athabasca Basin has been one of the world’s premier uranium camps, yielding more than 660 million lb of U3O8. In this paper, the author describes former and current milling operations and discusses why and how the milling process has changed and developed over the past 35 years. The author’s considerations for the future of uranium extraction in the Athabasca Basin close out the paper.INTRODUCTIONThe roughly oval Athabasca Basin in northern Saskatchewan occupies nearly one third of the surface area of the Saskatchewan Shield. Its sandstone-like deposits lie unconformably on the older basement. The basin hosts many high-grade unconformity-type uranium deposits(Saskatchewan Energy and Resources, 2010) for which the Athabasca Basin is world famous. Uranium production in the Athabasca Basin has come from four mills. In the order of startup these are Rabbit Lake, Cluff Lake, Key Lake and McClean Lake. General data (AREVA, 2010; Cameco, 2010; Denison 2010) for each mill are shown in Table 1."

As part of a continuing project to develop refractory metal alloys for high-temperature structural applications, the Bureau of Mines applied solid solution and precipitation strengthening techniques to improve the properties of columbium alloys. The purpose was to investigate the effects of hafnium, tungsten, and boron on the formability, strength, and oxidation resistance of columbium. The experiment was of factorial design with two replications which called for 72 ingots of 36 basic compositions, weighing 150 grams each. Hafnium, tungsten, and boron were evaluated at 3, 3, and 4 levels, respectively. Values of alloy content are all in atomic percent. The highest strength observed was 29,000 psi at 1,200° C for alloys Cb-15Hf (No. 3), Cb-7.5Hf-5W-2B (No. 14), and Cb-15Hf-5W-2B (No. 15). At 1,200° C, the ternary alloy Cb-15Hf-5W (No. 6) which has shown good properties in previous Bureau of Mines investigations was the most resistant to oxidation For boron additions, the alloys oxidized more readily with increasing amounts of this element. The fabricability of the alloys high in boron became increasingly difficult, resulting in an inadequate number of alloys required for a complete statistical analysis for all the tests. As a result of the decrease in ductility, an insufficient quantity of tensile samples were available for a proper analysis for the high-temperature strength data.

It is with considerable trepidation that I set out to defend "litigation" to Design Professionals (DPs), as a better method of resolving disputes than arbitration, in the light of my personal feeling that an increasing amount of America's productive process is being squandered in litigation. Newer methods of dispute resolution, particularly arbitration, are fashionable, and opponents of these schemes risk being viewed as cynical or self-serving. My quandary is heightened since for some smaller disputes, arbitration seems clearly the better choice. I must concede, for example, that when the cost of dispute resolution--i.e., the cost of the process (such as arbitrators fees, attorneys fees, etc.), plus the cost impact of the non-productivity of involved employees--approximates the amount of the dispute, controlling that cost may be as important as a correct resolution. This principle is particularly true for small disputes arising during the progress of a project; it is probably more cost-effective for all parties to resolve their disputes quickly than permitting the dispute to drag out.

Viscosities of quasi-ternary CaO-SiO2-CrOxmelts in contact with metallic chromium were measured by the rotating cylinder method up to 1750ºC. Twelve slag compositions were selected in the composition range 20–55 wt% SiO2, 10–45 wt% CaO and 10–60 wt% CrOx. Estimation of the amounts of CrO and Cr2O3was made by regression analysis based on the earlier data. Professor Iida’s (Osaka University) viscosity model was applied and model parameters (0CrO,CrOand Cr2O3) were evaluated. The Iida model gave good consistency with the measured viscosity values.

Walls consisting of CFA-piles are often used for vertical cut offs for building pits. Several methods of installation and geometry may be used and sizes, depths and strengths of walls necessary vary widely. Based on experience with secant CFA-pile walls in The Netherlands, requirements for this type of wall can be set with a risk management view to it. This paper describes those requirements, based on specific local conditions.

Blast-induced rock mass displacement can have a significant impact on ore loss & dilution. The mischaracterisation of the grade boundaries both prior to, as well as a result of blasting can lead to significant financial losses on account of ore loss and dilution. Ore dilution occurs when waste material is miscategorised as ore and sent for processing or stockpiling, diluting the run of mine head grade and recovery. Ore loss takes place when valuable mineral is miscategorised as waste and sent to the waste dumps, thus never being processed and recovered. Extensive research has been conducted at the University of Queensland over the last fifteen years to understand the impact of blast movement on ore loss and dilution. Recent outcomes of this research have developed innovative tools and techniques to measure and model blast movement. These techniques have been implemented at Ahafo gold mine in Ghana to understand blast movement dynamics and to implement solutions to reduce blast-induced ore loss and dilution. These solutions are validated through controlled trials and have been incorporated into site standard operating procedures to sustain the benefits.

Disasters are often tragic outcomes of high-risk technologies such as mines. In Australia, more mines are being developed every day, and the risk of disasters is ever increasing. No matter how effective our conventional safety devices are, there is a form of accident that is inevitable. These relate to accidents that result from æinteracting failuresÆ in a way that could not be foreseen by the designers. In so-called ætightly coupled production systemsÆ (processes that happen very fast, such as on a high producing mine) the risk is even higher and our risk controls mostly introduce some sort of a technological fix. While we are at the one hand attempting to control the risk, we are also introducing another level of complexity. Are we really controlling the risk? This is one of the fundamental questions that will be addressed in this paper: Do we have the ability within the mining industry and other industries to effectively prevent these catastrophes, or are we, on the contrary, faced with an increasing risk as a result of increasing complexities of our technology, management systems and practices. This paper will contend that perceived improvement in risk control is an illusion of activity, and that the likelihood of mining catastrophes may be exponentially increasing. Although the paper focuses on the mining sector many aspects may be applicable across a range of industries. This paper will further contend that we are applying the right solutions to the wrong problems. Our focus is technological and procedural, while our problem is one of production cultures ripe for error and failure.

[This is the sixth in a series of reports on properties of commercial motor fuels, made in accordance with a cooperative Agreement between the Cooperative Fuel Research (C.F.R.) Committee and the Bureau of tines of the Department of the Interior. The first report gave data on motor gasolines sold during the winter of 1935.-36.3/ The present report cover brands of gasoline represented by 1,490 samples plus thus: from the Pacific Coat, which ;sere averaged by the Pacific Coast Technical Group before being submitted and are published exactly as they were submitted. In some areas reports were received from 8 to 10 companies on the same brands of gasoline, Whereas in other areas only one Company reported on a brand. Thus, it can be seen that soma of the data are more representative than others. The inclusion of data from several cities within some of the districts has improved the report by reducing the number of brands reported by only one company. Severe changes have been made in the districts. A new district, Memphis, Tenn., has been formed to include Arkansas any western Tennessee. The Rocky Mountain area, have been divided into three sections. Salt Lake City and Denver form the Middle Rocky Mountain district; Laramie, Wyo., Idaho, Montana, and. Spokane, Wash., are included in the North Rocky Mountain district, and Arizona, New Mexico , and El Paso, Tex., have been combined to represent the South Rock Mountain district. The Cleveland district has been expanded to include Ohio ant. Indiana. Eastern Pennsylvania, Maryland, and northern Virginia are included in the Washington (D. C.) district. Vertical expansion of existing structures frequently requires foundation retrofitting to enhance the capacity of existing foundations for supporting additional loads. Owing to their small diameter, a variety of suitable drilling methods available for installation, and smaller installation equipment capable of operating under limited headroom conditions, micropiles are often preferred for foundation retrofitting. This paper presents a case history demonstrating application of micropiles to underpin the existing foundation of the Washington D.C. Children?s National Medical Center (CNMC) for its future expansion. Located at 111 Michigan Avenue in northwest Washington, DC, portions of the hospital currently extend five stories above grade and three levels below-grade as parking garages. The hospital intends to expand the surgical wing up to the current build out (five stories) above the remaining portion of the existing parking garage. The original foundations consist of 80-ton Raymond Step-Tapered piles. New loading on the foundations required each Raymond pile to be capable of supporting 150 tons. Results from quick load tests on existing piles indicated that two entire column lines (Column Line A and B) would experience loads beyond their capacity, requiring enhancement of the existing foundation at these locations. The design and construction of the foundation upgrading work was particularly challenging due to various project constraints related to a high groundwater table, micropile installation under limited head room, and ensuring that the parking garage remained fully operational for the entire duration of construction. The paper presents the challenges faced during testing of existing foundations amid a high groundwater table, design developments of underpinning options to support the additional column loads, difficulties and challenges encountered during installation of the micropiles and construction of the micropile pilecaps, and value engineering options developed during various phases of the project. Understanding that micropiles are often a preferred option for foundation retrofitting, the case history is expected to provide a baseline design approach and innovative solutions to typical challenges faced during construction, thereby reducing potential cost overruns during design development and construction of similar future projects.

"SynopsisRock mass classification systems constitute an integral part of empirical underground excavation design. One system that is commonly used to classify rock mass is the Rock Mass Rating, or RMR, system. The RMR system has evolved over time to better reflect the perceived influence of various rock mass factors on excavation stability. While the introduced modifications have enhanced the applicability of this classification system, there are still areas that cause potential ambiguity. The RMR system cannot deal with input data ambiguities, uncertainties, and vagueness. To deal with the uncertainty, a Fuzzy Rock Mass Rating (FRMR) has been developed. This study aims to describe the main concept of the fuzzy approach, which is implemented in the FRMR to classify rock mass. The FRMR considers the uncertainty associated with the input data (uncertain variables) used in rock mass classification. The Gol-e-Gohar (GEG) area 3 iron ore deposits in the south of Iran were chosen as the case study for investigating the applicability of FRMR. The results of this study show that the FRMR software tool can easily be used for rock mass classification, even if uncertain input parameters have been used. FRMR consists of three parts, each of which is suitable for certain conditions.IntroductionNowadays, rock engineering classification systems, which constitute the backbone of the empirical design approach, are widely employed in both civil engineering and mining engineering. Rock mass classifications have recently been quite popular and are being used mostly for the preliminary design and planning purposes of projects. According to Bieniawski (1989), a rock mass classification scheme is intended to classify the rock masses, provide a basis for estimating deformation and strength properties, supply quantitative data for support estimation, and present a platform for communication between the exploration, design, and construction groups."

Mina Santa Rosa, in west central Panama, is the first large-scale gold mine in Panama and will soon be the largest single-mine gold producer in central America (Fig. 1). At full production, the mine will process 1.8 Mt/a (2 million stpy) of ore and produce more than 1.8 t/a (60,000 oz/year) of gold and 9.3 t/a(300,000 oz/year) of silver. Mina Santa Rosa is a subsidiary of Greenstone Resources of Toronto, Ontario. Freeport-McMoRan geologists first visited the area in 1985. Their rock chip sampling adjacent to old shafts and adits confirmed widespread gold and trace element geochemistry anomalies. Surface mineralization appeared to be localized in and adjacent to narrow veinlets. However, they recognized astructural setting similar to the Nevada Tertiary volcanic-hosted epithermal gold systems.

INFACON conjures up many things for many people. For the fortunate few who have been actively involved with this event since its inception, it brings back memories of far places, stimulating debates and above all, the people who have over the years contributed to the outstanding success of the Congress and who have added to the wealth of knowledge on all aspects of ferroalloys. The International Ferroalloy Congress (INFACON) has been held every three years since 1974. INFACON X will be held in Cape Town in February 2004. Simple arithmetic shows that next year?s Congress should in fact be INFACON XI. Thus, one Congress was skipped. INFACON 74, as the name implies, was held in 1974, in Johannesburg, South Africa. The event was sponsored by the Ferro Alloy Producers? Association of South Africa (FAPA), the National Institute for Metallurgy (NIM - Mintek as it is known today) and the South African Institute of Mining and Metallurgy (SAIMM). You will see some of the same sponsors involved in INFACON X. Dr. Robbie Robinson, renowned academic and past president of the SAIMM, was the Chairman of the Organising Committee. The editor was Dr. Helen Glen. Anyone fortunate enough to possess a copy of the proceedings of this first Congress will agree that from a literary and technical perspective, it is still the benchmark for subsequent events. A unique feature of INFACON 74 was the use of rapporteurs (my first duty) to ensure that the discussions following the presentations were recorded (in direct speech) as accurately as possible. This involved time-consuming correspondence (can you imagine life before e-mail?) to all parts of the world between the author and the person posing the question, with the rapporteurs acting as go-betweens. It is understandable that the much-awaited proceedings only saw the light in 1975. The names recorded in the proceedings as the members of all six committees read like the who?s-who of the South African ferroalloy industry at the time: Dr. Peter Jochens, Mr. Harvey Reid, Mr. Piet Streicher, Prof David Howat, Mr. Sigurd Selmer-Olsen - to name but a few. The Congress was held in the new, very fashionable and upmarket Carlton Hotel in the CBD. In the Foreword of the proceedings, Dr. Robinson provides an insight into the event. Three hundred and thirty six delegates from twenty two countries attended the Congress, which was opened by the Honourable P.G.J. Koornhof, Minister of Mines. Again, the opening address makes fascinating reading. During the Congress an ad hoc committee was convened as the first International Committee on the Organisation of Ferroalloys Congresses, with representation from the main ferroalloy associations of the world. The following decisions were recorded: ? The Congress would be continued at three-yearly intervals under the name of INFACON ? Every third Congress would be held in South Africa ? NIM (Mintek) would provide the permanent secretariat. Six technical sessions were held, each preceded by a plenary address with a closing address by the session chairman. In all, thirty technical papers were presented. Prior to the closing address, a panel discussion on factors affecting the future of the ferrochromium industry provided stimulating debate in the form of a question-and-answer session. The hosting country for the next INFACON would be announced. Eventually, word got round that the next INFACON would be held in Japan. However, due to unforeseen circumstances, the event was cancelled ? thus no INFACON 77. INFACON 80 was subsequently held in Lausanne, Switzerland from 12th to 16th October 1980. The event was organised by IFPEO, Institut des Producteurs de Ferro-Alliages d?Europe Occidentale. The Congress was officially opened by the President of IPFEO, Mr. R. Björklund, who was also President of the Organising Committee. The technical sessions were held in the Palais de Beaulieu. The Congress was well attended by 400 participants from 28 countries as well as 100 accompanying ladies. A total of four plenary addresses and 18 technical papers were presented. As with the first event, an international panel discussion (questions and answers) preceded the closing address. In his closing address, Dr. Louw Alberts, Chairman of the International Committee on Ferroalloys and President of NIM, presented an insight into the factors affecting growth in the industry and where growth was likely to take place. Twelve exhibitors had stands at the Congress. Attendees could choose from four post-event tours to Belguim/France (SADACEM/Compagnie Universelle d?Acetylene et d?Electro-Metallurgie), France (CUAEM), Finland (Outokumpu Oy) and Norway (Elkem Fiskaa plant).
