Best practice standards generally recommend assessment of the environmental and societal risks posed by tailings storage facilities, but they rarely specify how the analysis should be made. As a case study, this paper describes a safety assessment of the Zelazny Most tailings pond operated by the Hydrotechnical Unit of KGHM Polska Miedz SA in Poland. The complete assessment includes a failure mode effect criticality analysis (FMECA) that considers all conceivable failure modes, but this paper focuses on one key mode, the loss of stability during the planned raising of the crest above its current level. Reliability against static dam failure considers uncertainty on the safety factor in a limit equilibrium approach. The threat of dynamic (earthquake induced) instability is assessed by developing a model of future seismicity as a function of the mining plan and considering uncertainty on the induced permanent displacements along critical sliding surfaces. We quantify the additional level of protection against outflow provided by the beach using an event tree with expert-assigned probabilities. Continuous monitoring of the dam for premonitory signs of instability and a set of loss-prevention plans in the case of a spill provide additional layers of protection; their effectiveness is quantified by estimating the fraction of the population that could be evacuated as a function of warning time. The probabilistic analysis indicates that the safety targets are met and produces practical recommendations on how to maintain and improve the safety level while the dam crest is raised above its current level.CITATION:Van Dyck, J, Pytel, W, Veneziano, D, Snow, R and Zanoli, O, 2015. Bauxite was discovered in the Kimberley in 1965 by an Amax field crew. Reserves of 230 million tons, of bauxite. averaging 47% total alumina on Mitchell Plateau and 980 million tons averaging 36% total alumina on Cape Bougainville have been indicated by the results of drilling and bulk sampling. Laterite which contains the bauxite deposits caps basalt and interbedded sandstone and siltstone of the Carson Volcanics formation. Bauxite on Mitchell Plateau is 3.2 metres thick on average and is underlain successively by nodular ironstone, clay and basalt. The major components of the bauxite are gibbsite,boehmite, hematite and goethite containing some alumina, anatase and clay minerals. Quartz and vanadium oxide are minor constituents. Boehmite and hematite are concentrated near the surface, whereas gibbsite and goethite predominate at depth. The bauxite profile consists of ferru- ginous, pisolitic and gibbsitic zones with increasing depth. Cape Bougainville bauxite is 8.6 metres thick on average and of tubular structure; its mineralogy is relatively uniform..

Ilmenite (FeTiO3) is the primary global source of titaniumdioxide which is commonly used as a whitening agent in papermanufacture and a pigment in paint manufacture. In WesternAustralia ilmenite is processed by the Becher process (Becher,1963, Becher et al, 1965) where ilmenite particles are feddirectly into the furnace with coal and heated to form acomposite of elemental iron and TiO2. The mechanism of thecarbothermic reduction of ilmenite has been extensively studied(Grey, Jones and Reid, 1973; Jones, 1973; Grey and Reid, 1974;El-Tawil, Morse and Francis, 1993) and is reasonably wellaccepted.

It is generally acknowledged that the head grade of the world’s copper resources is declining. Accordingly, there is an increase in the volume of resource material being mined and processed to maintain copper production. This has led to increases in cost base accompanied by reductions in Life of Mine (LOM) Net Present Value (NPV). The higher tonnages of material mined are driven by increased quantities of lower grade material required to satisfy the short term mine model. In turn, this has brought an increased focus on sampling and assay methods, since any error associated with estimating the grade of each block and determining its correct destination serves to magnify the losses that will occur. Presently most open-pit copper mining operations use blasthole sample assays to feed their shortterm model. The problematic nature of blasthole sampling is well documented (Engström, 2017), particularly the poor timeliness of the data and the significant errors that are inherent to the technique. Methods to improve grade control, such as drilling a separate grade control grid, have been proposed but come with their own cost, logistical, and accuracy shortcomings. A recent development in grade estimation is the Pulsed Fast and Thermal Neutron Activation (PFTNA) downhole logging probe. Deployed on an efficient semi-autonomous platform, PFTNA is being aggressively adopted in the Australian iron ore industry. The technology provides in-situ elemental analysis of the geological formation, delivering high accuracy, high efficiency, near realtime data from a sizeable measurement volume. In this paper we consider the application of the technology to copper. Whilst there are several operational components to such a value proposition, we focus on the benefits of improved gradeestimation. A simulated copper mine is used to model the quantum of improvement expected from a reduced grade-estimation error, which reduces the likelihood of misallocating material to the wrong destination, which in turn increases NPV.

The concept of using specially designed slot blasts to mine through stress abutments that intersect faults, while controlling fault-slip seismicity, has come of age at Mt Charlotte Mine. Due to the high and adverse stress field and the presence of major faults, the mine is vulnerable to significant fault-slip seismicity. As mining proceeded, advancing stress abutments around various mined areas encroached on several major subvertical faults. If the sequence formed a diminishing pillar between stope and fault, the removal or failure of that pillar daylighted the fault, and often resulted in significant fault-slip seismicity. A new strategy called ‘Tight Slot Blasting’ (TSB) was developed to restrain or dampen this style of fault-slip movement. The TSB has geometric similarities to a conventional cut-off slot used to create stress shadows, but its function is different. Its objective is to trigger and control a sudden shift of the stress state on a fault from an initial stable state, through an unstable transition, to a second stable state. A TSB firing advances a long tall narrow slot through a stressed abutment between a stope void and a nearby fault, using a rapid blast with a low swell ratio, so that the slot substantially and rapidly fills with blasted rock fragments. During the blast process, the stressed rock is rapidly removed and transformed into the fragmented rock fill in the slot. The strategy deliberately allows a fault-slip episode to manifest at a chosen time and place. The tight-packed broken rock in the slot performs an essential shock-absorbing function by controlling the slip amount and rate on the fault, while at the same time the fill becomes compacted by the movement. This controls the dissipation of fault-slip energy, and greatly reduces the seismic response in time and space. A small closure across the slot occurs, with consequent relaxation and destressing of the adjacent rock mass. Stress monitoring data shows that slot creation lowers the abutment stress in the nearby rock mass, while the newly placed fragmented rock fill acts as a shock absorber, providing considerable resistance to substantial fault-slip movement that otherwise could deliver a significant seismic event. First used in 2007, the strategy has been refined to become a routine specialised practice for management of seismically hazardous situations at Mt Charlotte, with 12 successful TSBs conducted to date. Despite mining through multiple high stress abutments, seismic reactions have not exceeded ML 2.1 with any TSB firing, all remaining below the site tolerable maximum of ML 2.3. This paper presents an overview of the strategy, and five case examples of the strategy in critical stressed areas in July 2016, Sept 2017, Dec 2017 and two in Oct 2020. The strategy has successfully managed the seismicity and is expected to remain an essential part of the seismic management plan at Mt Charlotte Mine.

The character of alluvial gold deposits presents severe difficulties, and sometimes limitations, on the extent to which metallurgical efficiency can be measured. Very little monitoring of metallurgical performance is practised. An indication of performance is often extracted from the comparison of production to the estimate. This is a dubious practice. The recovery efficiency of conventional practices, viz sluices, tables and jigs, falls off sharply below 0.2 mm. Field evaluation procedures tend to simulate these conventional practices. Thus the presence of fine gold may not be detected, particularly if obscured by clay. There is increasing evidence that fine gold could, in places, be present in sufficient concentrations to be economic. Most gold in paleoplacers is finer than 0.1 mm. There are established practices elsewhere in the mineral industry that may physically recover gold down to 5 microns in size and which could find application for fine alluvial gold.

IN 1904-5 diamond drilling operations in the Mount Morgan mine proved the presence of some millions of tons of rich copper-gold ore, averaging about 45%to 50%silica and 22%Fe as sulphide. To smelt this ore in blast furnaces, without previous concentration, large quantities of basic flux were required. Unlimited quantities of cheap limestone were available at Marmor, on the GladstoneRockhampton railway, 50 miles from Mount Morgan, but the supply of iron flux was a more difficult problem. Smelting Operations started with one furnace in January, 1906, using as flux low-grade sulphide ore from the mine, containing very little silica. When the second furnace was blown in this was supplemented by ironstone from small deposits in the immediate district. It was recognized that these sources of supply would be insufficient to keep the full plant going after the third furnace had been blown in. Accordingly, the deposits at Kabra and Mount Etna were opened, neither of which proved capable of supplying the quantity required-viz., 1000 tons per week. Negotiations were meanwhile being conducted with the Marble Island Iron and Limestone Co. to obtain the right to mine at Iron Island, where a sufficiently large deposit was known to exist. The particular underground operation relating to this paper is a gold producer and has an orebody predominately consisting of rhyolite and marcasites pyrite. The ore has been identified as being reactive to nitrates.Operations currently utilise two twin boom Tamrock™ Super-Drill Jumbos for development mining. Capital (waste) headings have a nominal size of 7 m wide × 6 m high, while ore headings are designed at 5 m wide × 5 m high, with the lower reaches of the mine narrowing down to a 4.5 m × 5 m design. Production drilling is currently undertaken with an Atlas-Copco L6 Solo. Due to the reactivity of the ore, inhibited emulsion phase bulk explosives are used for all blasting activities, with electronic detonators utilised in the production sequence.Sublevel shrinkage stoping is utilised when the orebody is stronger than the host rock. After blasting the ore, and during extraction, the surrounding host rock cannot maintain integrity and it ‘collapses’ on top of the ore. Gravity forces this collapsing rock to follow the blasted ore through the drawpoints.The highest risk involved with sublevel shrinkage stoping is the necessity for charge crews to work upon ‘rills’ at the drawpoints, which are occasionally unstable, to charge the next blast ring to be fired. To overcome this issue a loading technique called ‘precharging’ is used whereby two to three rings are charged, but only the first ring is fired. This technique can minimise risks by taking the charge crew away from the rill and brow.As this mine contains hot and reactive ground, strict guidelines from the Australian Explosives Industry and Safety Group Inc (AEISG) are adhered to. Concerns with crest losses have seen changes to a number of design and operating parameters to retain the integrity of the wall and the berms of the Kidston Gold MinesÆ Eldridge Pit. Until trials began in 1999 most final wall designs utilised large diameter blastholes up to the presplit row to achieve the wall angle. The berm crests were being lost, wall support requirements were increasing and affecting mining schedules and several fault areas were putting pit access at risk. Standard designs had been determined during the early mining years and while periodic modifications had been made there was a need for major blast pattern revision. This paper traces the development of wall control blasting programs over the years, relating designs to the geology and the evidence in the walls with the theories of the day and includes the recent trials that were conducted in what is a most unforgiving geology. Trials and improvements to ensure the walls were safe were conducted even as the mine neared its economic life. Instrumented monitoring of trials blasts has greatly increased the understanding of the damage criteria and enabled rapid revision of designs. Recent changes included reduction in presplit energy, charge redistribution and use of smaller blast holes for the trim patterns. Determination of a minimum proximity-to-final wall limit for production blastholes, establishment of the correct standoff to berm crests and walls and selective use of presplitting techniques for protecting crests have resulted in cost effectiveness, improved bench crest retention and an overall reduction in visible wall damage.

The Ernest Henry Copper gold mine situated in North West Queensland, Australia, is owned and operated by Xstrata Copper. Currently the open pit produces 11 Mtpa of ore. With open pit mining ceasing in mid 2011 development has begun on an underground sublevel cave mine to extend the life of the operation by another 12 years. When completed the open pit will be 530 m deep and 1.5 km in diameter. The orebody dips at approximately 45° and has been outlined a further 450 m below the base of the pit. This provides some challenges for the Ernest Henry underground mine which is designed to initially truck development and production ore from the end of the pit until a hoisting shaft is commissioned in 2013. Production will then increase to 5.0 Mtpa on its way to an ultimate rate of 6.0 Mtpa in 2016.This paper will focus on the capital development and infrastructure requirements for the underground mine. Areas covered include mine accesses, ore handling system, accommodations for the dip of the orebody, primary ventilation design, dewatering systems and selection of an underground workshop over surface only maintenance.Discussions include the complications arising due to the mine being located directly below a large open pit and the combination of first principle cost analysis and risk assessment which has been used to safely optimise the feasibility study designs.

Energy Resources of Australia Ltd - Ranger Mine is located about 255 kilometres east of Darwin in the Alligator Rivers Region of the Northern Territory. The Ranger project is surrounded by Kakadu National Park which is on the UNESCO World Heritage list. . Ranger Mine operates the largest uranium mine and processing plant in Australia. It is designed to produce 3000 tonnes of uranium oxide per year, but presently is producing a little less than one half that figure due to the depressed uranium market. The safety performance of the employees and Ranger as a whole continues to improve. The lost time injuries (LTI) have significantly reduced from 74 in 1985 to five in 1994. The total days lost due to LTIs have gone from 1543 to 14. These significant improvements have reflected directly on workers' compensation premiums in which a four-fold reduction has occurred. It is estimated that the actual monetary saving to the company is about $1.3 million per year and directly reports to the profit of the company. There is also an obvious and significant reduction in the less quantifiable costs such as the suffering of the injured employees and cost to society. The paper will review the various initiatives introduced and assess their effectiveness. Performance improvement is primarily dependent on the commitment, introduction of various initiatives and the subsequent change in workplace culture.

"Premature failure of rock bolts has been recorded at twelve underground coalmines over the past 15 years in Australia. Over the last four years, UNSW Australia has been conducting an Australian Research Council (ARC) and Industry funded linkage research project into premature failure of rock bolts. The extent of the problem was investigated by underground surveys of mines with known prematurely failed bolts using visual mapping, non-destructive load and ultrasound testing. A major component of this project was to define the environmental factors responsible for corrosion of rock bolts. The environment to which a rock bolt is exposed will include groundwater chemistry, the mineralogy of the strata, atmosphere within the drill hole and even microbial activity. This paper focuses on the groundwater survey and analysis findings to date along with in-hole bolt corrosion coupons installed into field sites. Corrosivity classification systems exist for general corrosion but do not apply to the rock bolt stress corrosion cracking (SCC) problem found in Australian underground coal. The method of in situ investigations is leading towards the building of a corrosivity classification model to assist mines in prediction of premature rock bolt failure. New Zealand has an established history of gold production beginning with the gold rushes of the 19th century in the Coromandel, Nelson/Marlborough, West Coast and Otago regions. Despite this, the number of significant modern hard rock gold operations has been limited in recent years due to a lack of sustained exploration capital and not limited geological prospectivity.The New Zealand Government is actively encouraging explorers to invest in New Zealand through a series of targeted promotional visits, more importantly through the acquisition of precompetitive regional geophysical data. Data collection has been completed over the prospective Northland epithermal district and a large portion of the South IslandÆs west coast that is prospective for both orogenic gold and intrusive related gold.Analysis of new data has been an important component in aiding the generation of exploration targets from prospectivity modelling. Determining the prospectivity of an area involves reviewing all the available data and analysing it with respect to the most up-to-date mineral system model for the mineralisation style of interest. Using the weights of evidence modelling approach, the most prospective areas for epithermal gold-silver, orogenic gold and intrusion-related gold have been identified. The key exploration parameters relevant to each mineral system are first represented spatially and then statistically combined into a single prospectivity map. New potentially economic deposits could be found by focusing exploration on targets identified from these models. Rock burst is common in deep underground excavations and is characterised by a violent ejection of block rocks from excavation walls. It is critical to understand the phenomenon of rock burst and particularly the failure mechanism. Laboratory experiments are one of the ways. For that purpose, a rock burst laboratory test system was developed at the State Key Laboratory for Geomechanics and Deep Underground Engineering at the China University of Mining and Technology, Beijing. A true triaxial test system, designated the Deep Underground Rock Burst Analogue Test Machine, was developed, which can transfer a stress state from three-direction-compression on six surfaces to the compression on five surfaces with one set free. The rock burst experimental system comprises an acquisition of acoustic emission signals, a high-speed recording in order to accurately record the kinetic characteristics of rock fragments ejected during a rock burst event and the possibility of infrared thermography showing the surface temperature of the samples. In this paper, analyses of typical results from rock burst tests are presented with a description of the obtained results. The mechanisms of rock burst are illustrated, and a rock burst classification is proposed based on these laboratory tests. For the control of rock burst, the constant resistance, large deformation (CRLD) bolt or anchor is presented for accommodating the large displacement of surrounding rock masses and absorbing the impact of the sudden release of rock burst while outputting a constant resistant force in response to the external load. The CRLD bolt or anchor has had practical applications in underground mines. Mining companies are increasingly faced with the challenge of reducing operating costs while maximising throughput and profitability. At the same time, the grade and quality of treated ores are becoming poorer and more variable, resulting in suboptimal process performance.It is now widely accepted that strategies to drive operational goals must revolve around improving plant availability, reliability and process efficiency. In particular, progress in plant automation and control systems has seen a trend towards unified frameworks for control and optimisation of mineral processing plants. These frameworks seek to seamlessly integrate different roles and tasks involved in process operations - IT, automation, plant maintenance and operations. An integrated solution framework for reducing process variability, increasing process efficiencies and maximising plant availability and reliability is discussed within the context of a typical milling circuit. The framework exploits a real-time analysis ægapÆ evident in production operations that prevents timeous identification of equipment and process failures. This is achieved by leveraging predictive analytics for equipment health monitoring, control loop monitoring and advanced regulatory control solutions towards enhanced process performance, improved asset management and regulatory compliance. The framework provides for early and actionable warnings on equipment and process degradation using real-time operational data.Resolving these problems can potentially avoid unplanned downtimes, process instabilities and other operational surprises, thus ensuring optimal plant throughput and optimal process behaviour for improved recoveries. Integrating control systems and plant automation with advanced analytics to analyse and optimise processes enables operations to achieve reductions in the variability of key process parameters. In addition, early detection of equipment malfunction or suboptimal process conditions leads to improved equipment availability and reliability. Ultimately, these operational gains reflect in profitability because of plant throughput and recovery gains. The Filblast Cyanidation Process incorporates the advantages of intense high shear mixing, high dissolved oxygen concentration and high pressure to achieve extremely rapid gold dissolution rates. This is made possible by the unique design of the Filblast gas shear reactor. The reactor is a rugged and compact in-line device. The outer casing is protected by a polyurethane coating and the internal parts are of wear resistant polymer. The largest unit built to-date has overall dimensions of 1200 mm length by 300 mm diameter and has a capacity of 150 dry tonnes per hour at 40 - 45 per cent solids. Service life at this throughput is at least three months. The process may further be applied to high temperature leaching of refractory gold ores or other metals. Wear resistant metal or ceramic materials would be employed for this duty. Four mines are currently employing the Filblast Process for treating a variety of ore types ranging from highly reactive, almost impossible to treat, pyrrhotite/arsenopyrite to deeply weathered clay ore which forms a highly viscous pulp. The effect of shear thinning has resulted in improved adsorption kinetics resulting in higher carbon loading and reduced soluble gold loss. Total tonnage treated is approximately four million. Operating costs which include leasing and maintenance charges for the Filblast and pump power costs are about 16 cents per tonne. This is equivalent to about ten per cent of savings. Laboratory scale units having capacities up to 30 litres/min are available for test work but laboratory testing techniques so far developed do not accurately reflect the oxygen and cyanide savings which have been achieved in the plant.

The Goonyella Riverside Mine is located 30 km north of Moranbah in Central Queensland, Australia. Recent mining activities have exposed the north–south striking Kenya and Zimbabwe thrust Faults. These faults vertically offset coal seams by up to 25 m and locally thicken coal packages through imbrication of coal seams. The presence of back-thrusts add to the geotechnical complexity of maintaining highwall stability. The Kenya and Zimbabwe Faults have provided multiple challenges to the mining operation, with an unplanned additional 240 kt of coal tonnes being mined from imbrication of coals seams or from the refinement to local scale geological models. These additional coal tonnes have been either identified during the mining of the coal seam or just prior to overburden removal above the coal seam. These faults can also negatively impacted highwall stability with highwall failures, lowering the overall slope angle for pit designs and increasing stand-off distances for hazardous highwall conditions. Unmanned Aerial Vehicles are used to scan exposed highwalls and enable geological mapping via high-quality 3D images. Measure while drill data from drill and blast rigs can provide local-scale intersections of the faults and coal seams. Mine Geologists utilise these and other data sets to create localised geological models refined position of the fault planes and coal seams. These models are utilised by relevant mine site teams to ensure the safe and efficient mining of coal around these faults.

Steel wire weld mesh is a common industry standard surface support used in underground mining, traditionally installed with mining drills during the bolting cycle. As underground mining depth and stress increase, mining methods and their sequences at these depths place challenges on conventional surface support options. These challenges have provided a need for high capacity surface support options that improve development efficiency, reduce the need for secondary ground support installation and is easily installed with current mining equipment. A 5m2 mesh testing machine was built, suspending a 9500kg concrete slab, designed with multiple hold down points to test varying bolting patterns. Mesh load and displacement is accurately measured using a data acquisition system which pushes against the restrained mesh module with an available 200kN load and 1200mm displacement capacity. A testing program comparing conventional surface support, straps and combinations of both were tested for baseline results. New module designs incorporating existing mesh types and new design concepts were manufactured to allow for single pass installation during a bolting cycle. Nine mesh types, mesh/strap combinations and new mesh module designs were compared using the same test method. Based on these results, two high capacity mesh module types are produced. This paper will detail the test program, load and displacement results and underground mesh module performance, describing the two high capacity mesh modules function within the development cycle and manual handling. The Olympic Dam copper/uranium deposit was discovered in 1975 and subsequently commenced production in 1988 at a rate of 1.5 Mtpa using the sublevel open stoping mining method. In the following five years the operation underwent two optimisation programs that have increased mine output to the current rate of 3.4 Mtpa of ore. The pre-expansion requirements were annual development of approximately 10 000 m and the placement of around 500 000 m 3 /yr of cemented backfill. Previous to 1995 all hoisting was carried out by the Whenan shaft. The Robinson shaft was commissioned during that year to hoist increasing development mullock tonnages and to allow for increased ore hoist via the Whenan system. In 1996 WMC committed to expanding the output of the operation from 85 000 tpa (1996/97 budget) copper metal to 200 000tpa. The corresponding increase in mine ore production requires expansion to a nominal rate of 9.0 Mtpa. To achieve these increased tonnages, major hardware installations will include sinking of a third shaft capable of hoisting in excess of 9.5 Mtpa, a rail haulage horizon utilising automatic trains, expansion of the existing backfill plant to a production capacity of over 1.8 million m 3 /yr, increasing long term development rates to 15 000 m/yr and increasing the production drilling and blasting rates in line with the higher stope production output. This paper provides detail into the intended hardware (shafts, equipment, etc) of the mining system, and outlines the intended long-term operation and management of the operation. Of particular note is the background requirement of ensuring existing production targets are met to provide critical cashflows into the project during the period of expansion.
