"Sub-standard practices and their adverse impact on safety performance remain a challenge in the South African gold mining industry. The purpose of this study was to investigate the root causes of sub-standard practices and the effect on safety performance in South African gold mines. The focus of the study was on the underground production operations at a gold mine in the Free State. The study consisted of three parts: a personal investigation into the causes of sub-standard practices at the mine, a comparison with the results of a cultural study performed at the West Wits mines, and a behavioural survey.The studies described in this article yielded similar results in terms of the key drivers behind sub-standard practices. It was found that substandard practices had a number of causes, and the origin lies within the habits, attitude, and behaviour of employees. Current methods of addressing sub-standard practices and improving safety performance were found to discourage employees from performing work safely and according to procedure. It was concluded that the lack of critical behavioural habits for the given work environment and job title could possibly be the primary reason for the occurrence of sub-standard practices. IntroductionSince the implementation of the Mine Health and Safety Act, 1996 (Act 29 of 1996) and the Mine Health and Safety Regulations, the South African mining industry has been striving to improve health and safety performance and has placed great emphasis on adherence to mine standards.The Act requires employers in the mining industry to report accidents, incidents, and dangerous situations to the Regional Principal Inspector of Mines in their area. The data provided by employers is captured and analysed by the South African Mines Reportable Accident Statistical System (SAMRASS) and made available for public viewing (Department of Mineral Resources, 2011).A review of the national mine-related accident statistics shows clearly that the South African gold mining industry has improved its safety performance markedly over the past decade, but is still facing major challenges (Figure 1). "The development of on-stream analyzers (OSA), such as those based on X-ray fluorescence (XRF), has been key to improve metallurgical performance of flotation processes. However, since these analyzers are shared between several streams, they typically provide grade measurements every about 10-15 minutes. In general, this frequency is too long compared to process dynamics and so, implementing an efficient feedback control strategy based on OSA can be difficult. In addition, due to the limited capacity of OSA systems, several important intermediate streams are not analysed, but could provide relevant information about the state of a flotation circuit.To overcome this problem, vision systems have been developed in the past to predict concentrate grade based on froth color – this could be very useful to control flotation performance between OSA measurements. However, applications reported in the literature typically use univariate summary statistics of the full red, blue, and green color distribution of froth images, while RGB images are truly multivariate. Although, in some cases, grade prediction was achieved with reasonable accuracy, one may, by using only summary statistics, miss important information about the state of the flotation process.This paper investigates the application of Multivariate Image Analysis (MIA) concepts to flotation froth images. These techniques are based on multivariate statistical methods such as PCA and PLS, which have the ability to analyze large databases (such as digital images) and summarize the relevant information content of the data into a small number of latent variables, capturing the most important features of the images for concentrate grade prediction. The potential of application of these methods in the mineral processing industry is illustrated using a few industrial case studies."

"SUMMARYWaste management in general, and the water treatment of mine and mill effluents, has become an ever increasing burden for mining operations. A reduction in these expenditures would undoubtedly be desirable if this could in fact be achieved within effluent guidelines and monitoring requirements.A brief summary is presented of the research which has as its aim the development of methods which would reduce such costs and lead to solutions for environmentally acceptable close-out conditions for acid generating mine waters. The basic concepts of Ecological Engineering and Biological Polishing are presented, together with an indication of the present state of these research efforts. Details are provided of the progress of various experiments which aim to promote reducing conditions, organic matter production and the establishment of biological filters which remove metals from the water which are then retained within the waste management areas. These conditions should lead to a curtailment of acid drainage and an improvement in effluent quality.A description is provided of the first pilot demonstration using this approach to achieve a walk-away condition for the close-out of a massive sulphide tailings resulting from 10 years of operating a copper/zinc concentrator in Northern Ontario.1.0 INTRODUCTION1.1 Waste management and close-outIt is recognized that the achievement of a cost-effective and competitive mining operation, from the commencement of exploration for ore bodies through to the day-to-day efficient operation of the mine, requires a certain expertise and sophistication. CaF2is often used as a fluoride addition to the chloride salt flux in the melting of aluminium scrap. The solubility of CaF2in the chloride salt system is limited. Dissolved CaF2is considered to improve the properties of salt fluxes. Experimental investigations indicated that the solubility ofCaF2in 70 wt % NaCl/30 wt % KCl, German salt flux used in aluminium scrap melting, might be lower than expected according to literature data. The pseudo binary section CaF2-(70 wt % NaCl-30 wt % KCl) was investigated by differential thermal analysis (DTA). The determined solubility of CaF2appeared to be considerably lower than presented in the literature. The pseudo binary section calculated using thermodynamic data partially confirmed the results. DTA allowed us alsoto determine the crystallization temperature interval of the two phase eutectic consisting of the solid solution NaCl-KCl and CaF2. "The South African Code for the Reporting of Exploration Results, Mineral Resources and Mineral Reserves (the SAMREC Code or the Code) sets out minimum standards, recommendations and guidelines for public reporting of exploration results, mineral resources and mineral reserves in South Africa. The Code is being rewritten and updated, with work on-going since 2013. The revision is being undertaken to ensure the Code remains relevant, incorporates developments within the international mining industry and keeps abreast of technology. Some of the changes and their reasons are discussed. INTRODUCTION The South African Code for the Reporting of Exploration Results, Mineral Resources and Mineral Reserves (the SAMREC Code or the Code) sets out minimum standards, recommendations and guidelines for public reporting of Exploration Results, Mineral Resources and Mineral Rreserves in South Africa. The Code is being rewritten and updated, with work on-going since 2013. The proposed 2016 update is not significantly different from previous versions of the Code. The revision is being undertaken to ensure the Code remains relevant, incorporates developments within the international mining industry and keeps abreast of technology. The Code sets out minimum standards, recommendations and guidelines for public reporting. However, reporting may be unclear, ambiguous, selective, obscure, distorted or unintelligible. Part of the purpose of the update is to provide better guidelines to Competent Persons (CPs) to ensure that the material information is being reported and that reporting is transparent in accordance with the guidelines of the Code. The information provided should, as required by the Code, be adequate to inform the “reader/investor or potential investor to make a reasonable and balanced assessment of the significance of the information”. The Committee for Mineral Reserves International Reporting Standards (CRIRSCO) highlights the importance of the reporting Codes, as provided from a quote from their website (www.crirsco.com): The mining industry is a vital contributor to national and global economies. It is a truly international business that depends on the trust and confidence of investors and other stakeholders for its financial and operational well-being. Unlike many other industries, it is based on depleting mineral assets, the knowledge of which is important prior to the commencement of extraction. It is therefore essential that the industry communicates the risks associated with investment effectively and transparently in order to earn the level of trust necessary to underpin its activities."

The use of augered cast-in-place piles has seen tremendous growth under commerical structures in the past twenty years. Problems with vibration, soil densification and/or heave associated with driven piles do not occur with properly installed augered piles. Augered cast-in-place piles were originally constructed (1950s) with diameters of .31 m (12 in.) and lengths up to 6.1 m (20 ft); however with the advent of better drilling equipment, diameters varying from .41m(16 in.)to .61 m (24 in.) and typical depths ranging from 18.3 m (60 ft) to 24.4 m (80 ft) are achievable. Reinforcement may vary from a single high strength rebar (Grade 60) at the top of the pile to a continuous steel cage depending on loading (compression, tension-uplift, and lateral). Typical axial design loads (compression) for a single augered cast-in-place pile are in the 445 kN (50 tons) to 890 kN (100 tons) range.

In the summer of 1937 the authors began a series of time studies of the various processes employed in' the production of crashed stone. These studies, made at quarries situated in the eastern and southeastern parts of the United States, have not been completed, but to make results available to the industry as early as possible, it was decided to publish summarizes of the studies made to January 1, 1939, as a series of progress reports. The first of these dealt with power-shovel loading and was issued as Report of Investigations 3461 in September 1939. The second, issued in September 1939 as Report of Investigations 3467, dealt with quarry haulage. The present report discusses analyses of time studies of primary drilling a t the same group of quarries. OBJECT OF STUDY Pressure of business problems frequently requires the entire attention of quarry management, and in consequence internal plant economies often are neglected. Also, operating officials absorbed in their local problems are prone to overlook possible economies that would attract attention immediately were they critically examining another plant. The authors felt that technical studies at a number of plants, made by impartial engineers, might disclose opportunities to reduce costs or improve efficiency in internal plant processes.

A calculation programme of finite element method has been developed in order to study the behavior of excavation with sheet piling in soft clay. Based on a series of isotropically consolidated-undrained tests (CIU) on Hangzhou clay a linear relationship between the normalized initial tangent modulus number, E1, and stress-path parameter, a, is found for four different stress paths, and stress-path dependent modulus equations derived. Meanwhile, several influence factors, such as embedment depth and stiffness of the wall and soil conditions, were taken into consideration in the programme. Some valuable conclusions were obtained. Finally, a case history was studied by the proposed method of calculation. The result of calculation and the field observation are in good agreement.

INTRODUCTION Mica Dam is located on the Columbia River just downstream of the "big bend" of the Columbia, where it leaves the Rocky Mountain Trench and begins to flow southwards, approximately 85 miles north of Revelstoke, B.C. (Fig.l). The dam was built as one of three Columbia River Treaty dams in British Columbia which regulate the flow of the river and provide flood control downstream in Washington and Oregon. In addition, the three Canadian dams make possible greatly increased power generation at existing power plants in the United States and further development along the Columbia and its tributaries in British Columbia. History Mica Dam, constructed during the period 1967 to 1973, is a zoned earthfill embankment 650 feet high, which has 12,000,000 acre feet of live storage and an available head of 550 feet. Power intakes which would serve either a six-unit surface or underground plant were incorporated into the design and construction of the right abutment of the dam (Fig. 2). Prior to 1971 alternative 2500 MW capacity surface and underground powerplant arrangements were studied. Cost estimates for the two alternatives were the same within the limits of estimating procedures. However, it was believed there was a certain continuing risk in locating a surface plant at the toe of a steep canyon wall. Mineral exploration activity in the US suffered through another depressed year in 1984. And continued strength in precious metals (and possibly industrial minerals) exploration failed to make up for declines in other minerals. 1984 offered little solace for the explorationist. But some new data on mineral exploration provided better statistics for analysts of US mineral exploration trends. The Society of Economic Geologists (SEG) published data from its survey of 1980,1981, and 1982 exploration by US and Canadian companies - 96 companies responded (Barber and Muessig, 1984). In addition, a Mining Engineering (April 1984 p. 345) article by Emerson and Ivosevic presented their data on annual exploration expenditures in the US for gold and for all minerals from 1961 through 1983. A review of 1984 domestic exploration and development activities reported in several industry journals indicates that almost 90% of those projects were directed at gold and silver deposits. Statistics on 1984 exploration expenditures are not available. But some industry observers believe that precious metals accounted for an even larger share of industry's exploration budget in 1984 than in 1983. Federal standards for refuge alternatives (RAs) mandate that they not exceed a Steadman apparent temperature (AT) of 95 °F (35 °C) at an assumedmetabolic rate of 325W, a limit that appears to be arbitrary. Occupants in an RA spend most of their time at rest (a metabolic rate less than 325 W), and thermal equilibrium can likely be maintained at an AT > 35 °C. The purpose of this study was to examine the upper limit of sustainable heat stress during 4- and 8-h exposures at rest. Five men underwent five 4-h trials (phase 1: AT range 39 to 49 °C, 90% rh) and five 8-h trials (phase 2: AT range 46 to 56 °C, 90% rh) in a semi-recumbent position. Descriptive statistics for gastrointestinal temperature (Tgi), heart rate (HR), systolic and diastolic blood pressure (SBP and DBP), sweat rate, fluid intake, urine color, urine specific gravity (USG), and changes in body mass are reported. Mean Tgi for all trials ranged from 36.9 to 37.3 °C, and mean HR for all trials ranged from 61 to 75 bpm. To determine the effects of increased heat exposure, differences in heat strain between 4- and 8-h trials were examined via repeated measures analysis of variance (ANOVA). As AT increased, there were no significant changes in ?Tgi and ?HR from 1 to 4 h in the 4-h and 8-h trials. During the 8-h trials, as AT increased, there was a difference in ?Tgi between 4 and 8 h. In conclusion, there was support for sustainable exposures resulting in no significant increase in physiological strain at an AT greater than 35 °C AT, with a sustainable limit (no increases in Tgi due to increases in AT) below 46 °C AT. Although the participants were not representative of coal miners, the results provide support for sustainable exposures greater than 35 °C AT at an average metabolic rate of 155 W.

Horizontal stresses are caused by global plate tectonic forces. During 1995 alone, they were largely responsible for closing two U.S. longwall mines. This paper presents six case histories from Pennsylvania. West Virginia, Kentucky and Alabama. In each case history, a mine encountered roof falls or difficult ground conditions in the headgate, caused by horizontal stress concentrations. The problems are detailed and the control measures adopted are described. In most cases, nearby longwall panels without stress concentrations were trouble-free. The paper also discusses detailed measurements made at two adjacent Pennsylvania longwalls. One headgate was oriented to avoid a horizontal stress concentration, and the other was not. Eliminating the stress concentration dramatically reduced roof support loads and roof deformation. The paper concludes that proper panel orientation and sequence is the key to maintaining headgate ground control. The optimum orientation is not parallel with the maximum horizontal stress, as previously thought but rather 200 in the stress shadow of the gob. Other stress control techniques, including artificial support, are briefly discussed.

High horizontal stress and its adverse effects on longwall gate entries have been witnessed in many US coal mines for the past years. It can cause cutter roofs and leads to roof falls either in the headgate or tailgate entries depending on the relative orientation of the panel to the maximum principal horizontal stress. Underground observations have shown that if the maximum principal horizontal stress is perpendicular to the panel entries, the roof is in the worst condition and if the maximum principal horizontal stress is parallel to the entries, the entry is in the best condition. However, little has been done about the high horizontal stress effects on panel entries subject to different orientations of high horizontal stress. In order to design a longwall panel and control the roof effectively, the redistribution and concentration of high horizontal stress field must be fully understood. Using 3-D finite element analysis, this paper presents the high horizontal stress redistribution and concentration for a retreat longwall face with different orientations of high horizontal stress. Based on the results, possible roof failures for different orientations of high horizontal stress are analyzed. A panel design criterion is recommended.

Ausmelt top entry submerged lance technology offers a cost-effective means of carrying out a range of base metal smelting operations with minimum impact on the environment. The system is capable of using a range of fuels depending upon local costs. Due to the intense, localised reaction zone, high energy efficiency is obtained and the physical size of the plant is relatively small compared to that of most competing technologies. Capital cost can thereby be minimised. The furnace is well enclosed, such that fugitive emissions and ingress of diluting air to the gas stream are both minimised. The oxygen potential in the slag can be varied over a wide range such that the system can be applied in both oxidising and reducing duties. Following a general description of the technology, the paper discusses Ausmelt plants already in operation. The application of the technology for the provision of additional smelting capacity, for the improvement of converting operations and for slag cleaning is then described. A brief explanation of how these processes could be applied at southern African smelters is given and the paper closes with an assessment of novel Ausmelt applications for this geographical region, such as zinc recovery from sulphides.

The telephone rings at 3:20 a.m. An exasperated voice on the other end says, “There has been a mine explosion at Mine No. 5 and we have not heard from three units of men.” Twenty-three minutes later, the phone rings again and another panicked voice says, “There is something wrong at the slurry impoundment, the pool level has just dropped five feet and we are trying to figure out what is wrong” Thirty minutes later, the mobile phone rings. An excited security guard says, “We have reports of thick black water running down the right fork and left fork of Straight Creek and both seem to be rising in the banks.” If this is not enough excitement, the mobile phone rings again. A stressed company official says, “I just heard a report that black slurry is running out of the portals of XYZ Co. No. 2 Mine, which is adjacent to our No. 5 Mine.” Is this all some kind of dream or nightmare? More importantly, though, if this happens, what do you do? Most people think: “It can’t happen to me.” But mine explosions do happen, slurry impoundments can rupture and creeks can flood with black water causing damage to others, including competitors. A mine’s response must be quick, cool-headed, calculated and effective. This article examines the issues, concerns and ramifications of this hypothetical disaster. The authors then present a systematic crisis-management response.

This chapter summarizes data on crude mineral production for the United States, its island possessions, and the Common-wealth of Puerto Rico. Included also are tables that show the principal mineral commodities exported from and imported into the United States, and that compare world and U.S. mineral production. Except for the mineral fuel commodities, the detailed data from which these tables were derived are contained in the individual commodity chapters of volume I and in the State chapters of volume II of this edition of the Minerals Yearbook. In October 1977, responsibility for the collection of data on mineral fuels-coal, petroleum, natural gas, and natural gas liquids-was transferred to the newly created Department of Energy. Although chapters covering these commodities will no longer appear in Volume I of the Minerals Yearbook, quantity and value data have been included in most of these summary tables to maintain statistical consistency in State and national value of overall crude mineral output. This shift in responsibility has also resulted in changes to the composition of the "Nonmetals" and "Mineral Fuels" groups in table 1 and table 2. The commodities-asphalt and related bitumens (natural), carbon dioxide (natural), helium, and peat-have been removed from the "Mineral Fuels" group, and are now included under the "Nonmetals" group. There has been no change in the components of the "Metals" group.

It is with great pleasure that we announce the availability of this fourth issue of the IPMI Journal, a publication of the International Precious Metals Educational and Scientific Foundation. This digital peer reviewed publication has been created as a vehicle for the sharing of technical information related to the science and technology of precious metals. Since 1976, the International Precious Metals Institute has championed the exchange of information of critical importance to the precious metals scientific and commercial communities through its various communications vehicles. This has included instructional videos, annual conference symposia, special topic committee meetings, regional seminars, and professional and student achievement awards. Due to the breadth of important topics related to all aspects of precious metals, it has been the vision of the IPMI to provide a publication available to the general public that would become a key source of pertinent topics containing high quality publications by experts in their fields of study. Historically, the IPMI has attracted world renowned experts in many fields (including Nobel Laureates Henry Taube, Ei-ishi Negishi, Robert Grubs, Professor Ben Feringa, and David MacMillan) in the dissemination of discoveries, inventions, and industry proven practices. Such information has a total value greater than the sum of its parts as seemingly unrelated innovations from distinct focus areas can be adapted to solve problems. Because of this important characteristic of information exchange, we have decided to create a digital journal, available to the general public, that will not only communicate state of the art discoveries and sound practices, but will also review historical communications from the IPMI’s archives that have value and use even in today’s environment. On behalf of the members and leaders of the IPMI, we hope you will find value in this new publication.

The Birimian rocks of the region are gold-bearing and show many similarities to those of Burkina Faso. All three greenstone belts (Goroual, Tera and Sirba) host gold-bearing quartz veins. It appears that, contrary to previous misconceptions, lateritic terrain is not an unfavourable environment for exploration geochemistry. This is demonstrated by the results for the Koma Bangou area, where soil and termite-mound geochemistry have been successfully used in the search for gold. Soil provides an effective sampling medium where it is shallower than about 2 m; there is evidence that some soil anomalies are enhanced by the presence of secondary (botryoidal) gold. In areas of thicker, allochthonous soil cover, termite-mound sampling is more effective. At Koma Bangou an identified resource of some 11 100 000 tonnes with an average gold grade of 2.44 g/t has been reported. The gold mineralisation occurs in quartz veins and stockworks aand in the wallrocks of shear and fault zones that trend N, NW and NE. The host rocks are a granodiorite-syenite porphyry and intermediate metavolcanic rocks and tuffs

Controlled graphitization has become known as a practical method for improvement of wear resistance and machining properties in steels. In this paper, the effect of heat treatment on microstructure of CK45 steel has been investigated. Austenitising was carried out at 920°C for 5 hours. Besides, isothermal transformation was conducted at 750°C in the time range of 1-20 hours. The microstructure of the steel considerably changes by this heat treatment process which exhibits the effects of temperature, appropriate austenitising duration and isothermal transformation. Conducted experiments show a suitable distribution of semi-spherical graphite particles especially on the surface of the steel. Also, analyses demonstrate that the amount of formed graphite in the austenitising temperature 920°C is more than graphite in single heat treatment temperature of 750°C.
