Organisations’ incident classification processes are plagued by the overclassification of events, often leading to excessive workloads and suboptimal outcomes. This presentation addresses the pressing need to rethink how incidents are classified to maximise organisational learning and minimise systemic risk. A major issue in the current incident classification landscape is the tendency to classify events at high levels, overwhelming organisations with investigations they cannot efficiently handle. This results in inadequate risk reduction and learning opportunities. The link between injury classification and investigation methods further compounds these problems, creating unnecessary administrative clutter. Challenging the misconception that every event, no matter how minor, requires a comprehensive investigation, this presentation advocates for a shift in perspective. The key recommendation is to decouple injury classification and risk matrices from the decision-making process regarding what and how to investigate. Instead, the emphasis should be placed on identifying opportunities for organisational learning and risk reduction. Organisations can streamline their incident management processes by adopting a more pragmatic approach that aligns incident classification with the organisation’s investigation capacity. This transformation focuses on achieving meaningful outcomes, enhancing safety and improving performance. This presentation provides an innovative process for organisations to revamp their incident classification practices, enabling them to harness the full potential of incident data for learning and risk mitigation. It highlights the importance of aligning incident classification with the opportunity for organisational learning, ultimately leading to more effective incident management and enhanced safety outcomes.

The Hazelwood Mine (HM) commenced operations in the 1950s and was originally operated by the State Electricity Commission of Victoria (SECV) prior to it becoming privatised. ENGIE, the current owners of the Hazelwood Mine ceased operations in 2017 and have since transitioned the site into the rehabilitation and closure phase, which is to encompass a full pit lake as part of the final landform. A key aspect of HM closure is the maintenance of long-term stability of the pit slopes to deliver a final landform which is safe and sustainable, particularly given its proximity to critical public receptors. The Princes Freeway, which is the primary access route to the Latrobe Valley (LV), and the Morwell Township are both located immediately to the north of the HM. As mines get deeper, the critical underground temperature limits will be exceeded, and an increasing number of mines will need to implement, expand or modify cooling systems. Surface cooling plants have many advantages over underground cooling such as efficient heat rejection, and lower relative capital cost; however the delivery of cooling through long underground distances can result in lost refrigeration capacity before the chilled air reaches the areas it is needed most. Refrigeration plants are therefore often overdesigned in size and capacity to compensate for underground losses.More mines are now utilising real-time monitoring and automated control of ventilation airflow and fan systems to ensure the conditions underground are safe and productive, and power consumption is efficiently managed. There may be significant opportunities to extend ventilation monitoring and control into the refrigeration delivery and control system.Energy reduction is an effective way to reduce costs and CO2 emissions, which is an increasingly important part of corporate governance. More efficient refrigeration control is a further step toward reducing energy consumption. This paper looks at an added step in how real-time monitoring can assist in controlling the output and chilled air delivery of a cooling plant. The requirements, implementation, benefits and challenges will be discussed.

Over the longer term, the adoption of new technology is an essential component in the endless tasks of reducing mining costs, improving safety and improving environmental standards. However the choice of which of the many available new technologies to adopt and which not to adopt is not easy and the price of mistakes can be very high. The most important issue for mine developers faced with making such choices is knowing whether or not a new technology has been developed to the stage where the timing is right for its adoption. A system for categorising new mining technologies is provided. It provides a basis for more easily reaching common understandings about various new technologies and the background information supplied provides an understanding of what it takes to get new technologies to the proven stage and the risks of never getting there. Wherever possible, new full-scale mine developments should not rely on unproven technologies. Approaches to handling situations where a proposed new mine development is not feasible on the basis of proven technologies are also provided. If any commitment to the use of unproven technologies is made, a guarantee of high quality development support from the supplier of that technology is essential. Several case studies of successful and unsuccessful new technology development programs at BHP are Provided.

"The Cerro Corona mine consists of a Cu-Au porphyry deposit located in the Andes of northern Peru. The deposit comprises a typical Cu-Au multiphase porphyry mineralisation that has been overprinted by an epithermal event. This has generated a complex pattern of argillic alteration facies, which was accompanied by a locally structural-controlled Au-Cu mineralisation with the presence of gold and limited Cu sulfides and sulfosalts. These features, along with the porphyry-style mineralisation, produced a multifaceted occurrence of gold.Since the beginning of operations at Cerro Corona in 2008 there has been a continuous improvement of the geological and mineralogical knowledge. All mineralogical studies have mainly focused on host rock types and, to a lesser extent, alteration minerals, but little work has been done on the mineralisation itself. As a consequence, limited mineralogical information was available for use in the process optimisation initiatives.In the last two years, a series of mineralogical characterisation studies were undertaken with two main goals. First, to gain an understanding of the mineralisation genesis and second, to obtain a detailed characterisation of copper and gold minerals to support ongoing process optimisation. The first goal was achieved by conventional optical microscopy while the second required electronic microscopy and other microanalyses due to the necessity of quantifying several minerals of interest for the processing.The results were used to refine pre-existing geometallurgical domains, explain copper and gold losses in the flotation process and to some extent, improve copper and gold recoveries. The mineralogical characterisation is an ongoing work. "One of the contributing factors to the extreme cost escalation in the first decade of the 21st century was the increase in the amount and cost of engineering work that went into studies and projects. Various reasons for this were identified, including unsatisfactory productivity due to a shortage of experienced/qualified engineers, the increased complexity and infrastructure requirements of many projects due to location, metallurgy that is more challenging and increasing environmental and social requirements. These conditions often led to unsatisfactory project results in terms of cost and schedule blowouts and created a loss of confidence – both internally and externally – in the industry’s ability to complete projects on time and on budget.Technical fundamentals aside, the experience and effectiveness of project teams has had, and continues to have, a major impact on project cost, schedule and ramp-up to nameplate capacity and operating parameters. There are unfortunately too many examples of where project teams have not been as effective as they could or should have been, all to the ultimate detriment of the project.Using the authors’ extensive experience on both owner’s and engineer’s teams, this paper discusses some of the main reasons for suboptimal team, and hence project, performance and offers thoughts on how best to build and maintain project teams. The analysis and conclusions drawn are built around the Tuckman model of group development known as ‘forming-storming-norming-performing’, which was first proposed some 50 years ago. The methods used and some of the results obtained from a major geotechnical study of the Ok Tedi mine area, Papua New Guinea, are described. The study was carried out at both regional and local scales and extended over a two-year period. A team of individual engineers and scientists from a variety of disciplines carried out the work. This study may provide a model for other investigations in similar environments.

Maintaining catch berm capacity in open pit mines through controlled blasting prevents production delays associated with managing risks associated with wall instability. A method has been developed to quantify the drill hole deviation of presplit holes at Century Mine. Different controlled blasting techniques are used to control wall quality and decrease the risk. The performance of presplit drilling, in terms of deviation from design has been investigated. A methodology has been developed to quantify the drill hole deviation in three dimensions. This methodology was converted into a series of algorithms capable of handling an extensive range of data. The methodology was applied to a case study at Century Mine to evaluate the performance of drilling on two presplit blasts. Overall, 67 drill holes were investigated in the case study. The drill hole deviation was quantified in terms of three parameters: skewness, linearity and drill hole deviation parameter. Methods for quantifying each parameter were developed. Skewness, a measure of the amount of curvature and linearity, was defined as the measure of the consistency of the gradient of the drill hole. Drill hole deviation parameter is defined as the average measured deviation of the surveyed drill hole position from the design position. Acceptable drill hole deviation tolerances for each of the defined parameters were developed by analysing drill hole deviation parameters with respect to implications to catch berm capacity. Verification of the methodology was confirmed through the comparing of the completed drill hole deviation results using both visual inspection and cross analysis for each of the three methods. Holes with an R squared value of less than 0.97 for skewed deviation were classified as having skewed deviation, while holes with a difference in gradient of 3.5 were classified as having linear deviation. The results show that 76 per cent of the drill holes had deviation. Of this, 19 per cent were skewed, 57 per cent had linearity and 12 per cent contained both skewed and linearity deviation.

Net solar radiation is one of the key factors affecting evaporation from soil covers on waste rock dumps. While net solar radiation is normally taken as a uniform parameter over a site for modelling purposes, there can be significant variation in the radiation actually received on various portions of the dump. These variations are a function of the difference in the cover slope and direction of exposure. A predictive model has been developed to calculate the actual radiation received on the dump surfaces, based on the net radiation data measured at a weather station on a horizontal surface. The model has been designed for application at any mine site in the world, and can be used in conjunction with evaporative flux models to help estimate the effect of three-dimensional variations in the cover surface exposure. The waste rock dump at Equity Silver site (located in British Columbia, Canada) has a barrier-type soil cover system to limit oxygen entry and water infiltration. This cover has been monitored for the past nine years. The new net solar radiation model has been applied to this site and the predicted net radiation was compared to the values of actual net radiation measured on the site. The predicted variations in net radiation over the three-dimensional surface of the waste rock dump have been applied to calculate spatial variations in potential and actual evaporation (or evapotranspiration) from the site. Preliminary analyses indicate actual evaporation from a site at this latitude could vary by a factor of two for exposed versus shaded slopes. The implications of these variations for the site are considered.

The aim of this study is to examine the volumetric and mass movement characteristics of load and haul operations using statistical modelling of relatively large samples of bucket loads that are transported as small samples in the form of truck payloads. Generally, the factors influencing bucket load variability are random, difficult to predict and to control. In the process of loading trucks the underlying variability of individual bucket loads will determine the variability of truck payloads. Bucket loads of material prepared by drilling and blasting for loading into trucks are fundamental quantities in open pit mine production. A study of payload data from a large surface mining operation indicates that bucket loads and truck payloads that are a small sample of bucket loads are normally distributed. Truck payloads have a reduced dispersion compared with bucket loads that is an inverse 0.5 power function of the number of passes per truck payload. Comparison of statistical modelling results with the æ10/10/20Æ policy promoted by Caterpillar Global Mining Inc, indicate that compliance with the æ10/10Æ part of the policy, applying to truck overloading is facilitated, as the coefficient of variation of truck payloads decreases, as a result of the increasing number of loading passes. Compliance with the more stringent æ20Æ part of the policy, relating to safe steering and braking is also facilitated, albeit at lower coefficient of variation for similar reasons.

In situations where foundation piles are located in proximity to existing or future tunnels, it is typical for the tunnel owner to require that sleeving of the piles be carried out to at least the invert of the tunnels to minimise any load transfer to the tunnel lining. In competent rock, this practice may be sometimes unnecessary, thus uneconomical. This paper presents an assessment of the effect of pile loading from one of the multi-storey buildings at Barangaroo South in Sydney on the adjacent future Sydney Metro tunnels. It was originally required by Transport for NSW (TfNSW) that the piles be sleeved below the tunnel invert level; however, the building developer wanted to reduce the pile sleeving depth to achieve a more economical solution due to the large number of load transfer piles being required for the project. To assess this feasibility, a full 3D commercial numerical code, FLAC3D, was used to carry out this complex soil-structure interaction analyses. Based on these assessments, it was demonstrated successfully to the TfNSW that the pile sleeving may be terminated at 2 m above the tunnel invert level. For this arrangement, the investigations found that the loading on the piles should have negligible adverse impact on the tunnel lining design. As a result of such an assessment, the proposed pile sleeving level was approved by TfNSW, thus offering an economical foundation solution for the project.CITATION:Azad, F M, Oliveira, D A F and Wong, P K, 2014. Research cruises dedicated to seafloor hydrothermal activity along the Kermadec arc first began with the Sonne-135 expedition in 1998. This was followed by the worldÆs first systematic survey for venting along an arc during the 1999 NZAPLUME I expedition. Since then, cruises dedicated to mapping plumes along the Kermadec arc include NZAPLUME II (2002), NZAPLUME III (2004) and ROVARK (2007). In addition, the manned submersible cruises SWEEP Vents (2004) and NZASRoF (2005) utilised the Shinaki 6500 and Pisces V, respectively, to sample in detail select hydrothermalsystems for mineralised samples, rocks and vent fluids. The ROVARK (2007) cruise was the first to deploy an autonomous underwater vehicle (ABE) along the arc which surveyed Brothers volcano at a far greater resolution than had been done previously by ships on the sea surface. While these expeditions have largely been led by GNS Science, together with its collaborators from National Oceanic and Atmospheric Administration, Pacific Marine Environmental Laboratory (NOAA/PMEL), significant contributions have been made by other groups from various nations, exemplifying the global interest in seafloor hydrothermal activity associated with intraoceanic arcs. The iron ore district of Gol-e-Gohar (GEG) is located 60 km south-west of the city of Sirjan, in the Kerman Province of the Islamic Republic of Iran. Six exploration targets have been identified and iron ore has been mined from one area with dry and wet concentrate products sold. Prior to mining commencing in 1999, the proven reserves were estimated at 265 million tonnes (Mt) in 2004 and 40 Mt of 100 Mt overburden has since been mined. Of the six exploration targets, the largest anomaly in the GEG district is Deposit No 3. This paper discusses economic considerations focusing on mine extraction and construction of a concentration and pelletising plant to process ore from Deposit No 3 of GEG. Planned capacity of 10 Mt product (concentrate and pellet) per year is targeted to commence in 2011, and nominal capacity by 2014. The economical evaluation and analysis is one of the most important stages in the feasibility study of mine development. Financial calculations including the provision of financial resources, operation period revenue, loss and profit statement, cash flow, net cash flow, internal rate of return and net present value have been undertaken as part of the feasibility study. The feasibility study operating plan assumes a large open pit mine containing proven reserves of approximately 250 Mt. The feasibility study assumed an economic return of $80 per tonne iron pellet. The economic study determined that Deposit No 3 of GEG can be economically developed by open pit mining followed by magnetite separation and flotation to generate iron concentrate and pellets.

Surface displacements are routinely monitored at multiple points during slope excavation. The monitoring data reveals the safety status of the slope under current loading conditions. They can be used to enhance the prediction of the future performance of the slope by reducing the uncertainties of the prediction model and its model parameters. In a multistage excavation, the observed displacements in the current stage can be used to update the prediction of the displacements in subsequent stages. Besides, due to the spatial correlation of the slope parameters, the prediction of the displacement at one location can also be updated using the monitored displacements at adjacent locations. This study aims to develop a multistep updating method to enhance the prediction of the future performance of the slope using time-series monitoring data at multiple points. This method integrates the theoretical computational model with observational evidence. It considers the inherent uncertainty of the slope, model uncertainty and measurement uncertainty. With more monitoring information being incorporated, the prediction becomes closer to the actual performance of the slope. It is well known that the high pressure grinding rolls (HPGR) can only be operated efficiently under choke feeding conditions in which a material bed is built up between the rollers to ensure interparticle breakage. By increasing the fines amount in the material bed up to a specific level with the recycling load, a uniform material bed is created and higher pressure gradient along the roller width is obtained which results in compact flakes including more fines. Extensive test work has been carried out with an industrial HPGR unit grinding cement clinker in a cement plant in Turkey. During the test work the recycling load of the circuit was changed to achieve different ratios to the fresh feed rate in order to identify the effect of the recycling load on the circuit performance. Six different tests were performed with the circuit at steady state. For each test, samples were collected from HPGR fresh feed and discharge, and total feed, then the particle size distributions of the samples were determined and mass balances around the circuit were calculated. The conclusion was that the reduction ratio of the HPGR increased with increase in recycling load. The flake densities showed the same trend with more compacted flakes being produced for higher recycling loads.

This paper examines the application of Lean Manufacturing in a mining environment, based on experience within the Rio Tinto entities: Rio Tinto Aluminium, Northparkes Mines and Hunter Valley Operations. The three applications have tested the flexibility of the Lean methodology. Within Rio Tinto, Rio Tinto Aluminium was the first group to introduce Lean. The rollout began in late 2004. It was introduced firstly at Boyne Smelters, Gladstone, as a model site with the initial application in the Carbon Bake Furnace area. It was later extended to other sites including the Weipa Bauxite Mine. LeanÆs successful application within Rio Tinto Aluminium was in part responsible for its recent adoption by other Australian-based Rio Tinto business units including Northparkes Mines, a copper mining operation in Central New South Wales, and Hunter Valley Operations, a group of four coal mines in the Hunter Valley region of New South Wales. Lean is also being adopted by Rio Tinto Iron Ore. Rio Tinto Aluminium introduced Lean Manufacturing to complement an existing Six Sigma business improvement program and to achieve continuous improvement activities at a workplace level. Lean provides a similar level of rigour and discipline to Six Sigma but its application is everyday and for everyone, everywhere in the workplace. It is a valuable management tool. In a short time, Lean has achieved good and sometimes spectacular results improving productivity and efficiency at all sites, including our mining site. Within Rio Tinto Aluminium, it has also had beneficial impacts on our ability to develop and retain employees.

Sustainability is one of those fuzzy words like innovation: everybody has an idea what it means, but if one starts asking for concise definitions then it becomes clear that everybody puts their own nuance on the meaning of these words. It’s not because these are difficult words per se, but rather because our understanding of them is highly dependent on the environment we work, live and play in. To remove any ambiguity and aligned with the risks that are faced by the human population today, the definition for sustainability that will apply for this paper is: generating value in the short term without destroying value in the long term.CITATION:Scholtz, P, 2018. Scheelite was floated from a low-grade (0.28 per cent W03) calcitic (96.30 per cent CaCO3) scheelite ore using oleoyl sarcosine as collector and alkylated 8-hydroxyquinoline as modifier. Compared to oleoyl sarcosine alone, the grade and the recovery of scheelite concentrate were increased from 8.07 per cent to 33.54 per cent and from 55.1 per cent to 73.4 per cent, respectively. Infrared studies with pure calcite and scheelite minerals showed that while alkyl oxine is adsorbed on the scheelite surface, it is not adsorbed on the calcite.
