Qualitative risk assessment methods are not well suited to considering complex systems and potential interactions of new technology. As part of supporting tier one mining organisations and international industry groups, the author and colleagues have innovated processes to help provide better solutions to sites/companies considering a move to implementing proximity detection and collision avoidance systems. The process identifies required operating states for the system. By applying a combination of research and team-based analyses, a rigorous identification of credible failure modes which could cause these states to be lost, is identified. Finally, required business inputs, expressed in practical terms are distilled from regulator, industry, organisation and site in a way which clearly addresses the failure modes and so retain the required operating states. These business inputs identify improvements for site arrangements (EMESRT levels 1 to 7) before identifying functional requirements for technological collision avoidance solutions (levels 8 for advisory and 9 for autonomous). The paper presents a case study synthesised from multiple sites analyses and will provide guidance on where to find additional resources to assist in making the best possible transition to better equipment and road network solutions.

This study considers realistic data for a planned open pit iron ore mine, but is applicable to any open pit situation. By interpolating drill hole data, grades are generated for a rectangular block model. Each blockÆs grade designation vector has components for each analyte (chemical element or compound) influencing ore value. Interpolation by means of geostatistical kriging leads to each block being assigned its expected value, and thus underestimates the overall grade variability. Alternatively, interpolation by means of conditional simulation is a method that implements random sampling from an infinite population of solutions. Each conditional simulation has appropriate overall grade variability, but estimating any blockÆs mean and variance requires sampling from multiple conditional simulations. For a block model, an ore/waste selection criterion maximises the expected tonnage at a target grade. This criterion is a linear composite of the grade components, with positive coefficients for the beneficial analyte (iron) and negative coefficients for the deleterious analytes (such as silica, alumina and phosphorus).Although conditional simulation gives the same expected grade as kriging for each block, the expected maximum tonnage of ore selectable at a target grade may differ from that obtainable from the kriged solution. We apply the linear composite selection criterion to each of 25 conditional simulations, as well as to the kriged block model. The resulting distribution of product tonnage is confirmed to have an expected tonnage that is over 20 per cent greater than that of the kriged model. The method also enables a selection probability to be computed for each block, and thus a probabilistic pit boundary distribution to be identified and used in mine planning. Proposed extensions will consider risk-based scheduling of the multiple selection solutions through minimisation of a derived æstress factorÆ and treating the mining process as an iterative system with actual or artificial ædepletionsÆ modelled in line with the mine plan, using the updated state (with new information) to re-evaluate the mine plan for subsequent periods.CITATION:Everett, J E and Grobler, F, 2013. Many organisations struggle to yield meaningful outcomes from incident investigations. This paper will demonstrate that we can discover systemic issues by rethinking the ‘Why’ and ‘How’ of investigations, fostering genuine risk reduction. Key points: 1. Shift from ‘prevent recurrence’ to ‘learn and reduce risk’: conventional investigations focus narrowly on recurrence prevention, resulting in narrowly focused investigations with limited ability to identify and reduce organisational risk. Redirecting our objective to learning and improvement opens avenues for broader risk mitigation. 2. Understanding daily work for risk insights: recognise that work is not always completed in full compliance with procedures. Workers will adapt how they complete tasks influenced by the conditions and management systems that exist every day. Understanding the conditions and systems affecting everyday work is vital for uncovering systemic risks. 3. Unearth systemic issues through in-depth exploration: many investigations address surface concerns such as lack of training, incorrect procedures, equipment not being maintained, and missing overarching organisational risks. Systemic, organisation-wide risks are not identified. To discover these systemic issues and create shared learnings, investigators must question management systems, including resourcing, planning and risk management. 4. Design actions for safe failure: common actions like discipline and training fall short and do not reduce risk. Resilience emerges from well-designed controls. Incorporate planning, better resources, interlocks, and guarding to bridge the gap between task execution and risk. 5. Investigation Reports as a Business Case: Investigation reports should not simply state historical information regarding an incident. They are to facilitate decision-making and be presented as a business case to drive change. Organisations can move beyond localised risk and ineffective administrative actions by changing investigation objectives and methods. They can facilitate learning, understand daily work intricacies, identify systemic issues, and enhance the capacity for safety, driving lasting risk reduction.

The authors were engaged to optimise a traditional rebar cage reinforced concrete segmental lining by means of finite element analysis (FEA) using macro synthetic fibres for the Shanghai metro extension. The aim of this study was to reduce or entirely replace the original steel reinforcement of the segmental lining, in order to make the precast production process faster and more economic. The outer diameter of the tunnel was 6200 mm, the thickness of the segments was 350 mm.The numerical calculations were performed by Atena Finite Element software, which is based on the combination of Menétrey-William and Rankine failure surfaces on the compression and tension sides respectively. The effect of the macro synthetic fibres was modelled as tension stiffening on the tension surface.A one-to-one full scale model of an assembled tunnel ring using the original reinforcement cages was tested at Tongji University in Shanghai, July 2013. The ring was tested to differing load levels where load and displacement were measured until failure. The author’s first step was to calculate the real displacement characteristics of the original reinforced concrete (RC) ring, then calculate the numerically modelled version, so as to compare them. The material parameters of the concrete and rebar used had been determined through basic material tests conducted at the Tongji University. (Bi, Liu and Wang, in press).The numerical modelling undertaken has shown good correlation with the full-scale physical test results. Optimisation of the lining reinforcement was achieved through the addition of fibres, resulting in a substantial reduction of the steel rebar cages. The approach to the disposal and storage of mine tailings is slowly evolving. Tailings disposal has been based on minimising short-term capital and operating costs, with rehabilitation costs discounted by the application of a high discount factor in a net present value approach to accounting. This has led to the widespread adoption of surface tailings storage facilities (TSFs) to store slurried tailings delivered by robust and cheap centrifugal pumps and pipelines. Depending on the site climate, rate of rise of the tailings, nature of the tailings and the extent to which supernatant water is removed from the surface TSF, the tailings may or may not dewater, densify and gain strength. High rates of rise of dilute tailings to a facility used to store both tailings and supernatant water results in a wet, low-density, soft tailings deposit that not only occupies a large storage volume but also presents rehabilitation difficulties. A low tailings density results in high containment wall costs, very high rehabilitation costs and a low potential for future land use or ecological function. Overall, the cost of slurried tailings disposal, thought to be low, is actually high. However, low rates of rise, even of dilute tailings, in a dry climate with efficient supernatant water removal can produce a dewatered, high-density, stiff tailings deposit that may occupy a large footprint but to shallow depth that is relatively easy to rehabilitate. There are advantages in dewatering tailings in the processing plant to recover process water and chemicals, facilitate tailings disposal in a more solid form and facilitate rehabilitation. However, the cost of dewatering in-plant can be high, and the process water may not lend itself to reuse. There are also advantages in removing supernatant water to facilitate solar and wind drying of exposed tailings, although water is lost to evaporation and the dried tailings may best be harvested prior to inundation by fresh tailings or rainfall run-off. A major Ventilation Upgrade Project commenced at the Fosterville Gold Mine (FGM) in 2017 to service the ongoing ventilation requirements of the underground mining operation. The project consists of three large diameter ventilation raises totalling 1100 metres, and new axial primary ventilation fans to upgrade the mine ventilation system. With a mining depth of 1200 metres and a thermal gradient of approximately 2.7 °C per 100 metres of depth, the completion of the Ventilation Upgrade Project is key to the ongoing success at FGM. The Ventilation Upgrade Project will allow continued mining, including high-grade gold deposits, lower in the mine where heat is increasing. Figure 1 below illustrates the underground workings at FGM, and provides a general view of the ventilation upgrade works to establish the new exhaust system from the lower workings to surface. Whilst raisebore pilot drilling the H4760 Return Air Rise (RAR) from surface to 395 metres, the raisebore drill string became stuck in the raisebore pilot hole at approximately 300 metres down hole, presenting a significant technical problem and delays to the project. This Case Study provides an overview of the recovery process undertaken at FGM including the recovery of the stuck drill string. The value of the stuck in-hole equipment, the investment in the shaft site preparation, the collar stabilisation, the underground development to the base of the ventilation raise, the surface footprint prepared for the Primary Fans construction and the limited available space on the site were all motivations to recover the raisebore drill sting and ultimately recover the ventilation raise location. More importantly, the long lead-time required should a new shaft location be the final solution, and the delay this would incur to the ventilation upgrade was unacceptable. Recovery of the drill string was not possible using the capability of the Contractor’s Raise Drill. An alternative method for freeing the drill string was required. A significant amount of engagement with industry was undertaken to seek out potential solutions and draw from the experience of others. All methodologies available were considered and assessed. Ultimately, after several attempts at freeing the raisebore drill string using various methodologies, Adams Drillers and Clear Cut Interventions were engaged to provide a turnkey recovery solution.

A new high intensity flotation process FASTFLOT was deployed to recover residual zinc from the zinc circuit tailings at Pasminco Mining's Concentrator, Broken Hill. Performance of the FASTFLOT process is discussed with respect to various size fractions. Results suggest that the process has an affinity not only for fine particles but also for +200 ¦m particles which the conventional flotation cells failed to recover. Various strategies for FASTFLOT deployment are also discussed.

Fibre reinforced shotcrete has long been accepted as temporary ground support in both mining and civil tunnel linings. Greater knowledge and understanding of shotcrete as an engineering material and advances in its quality have enabled designers to eliminate the requirement of a second pass lining. The initial shotcrete can be used as the permanent tunnel lining or be incorporated into the final lining design, rather than ignored and considered as sacrificial.For the initial shotcrete lining to become a suitable permanent tunnel lining it must be able to establish and maintain stability over time when subjected to all possible loads. To ensure this, best engineering practices are required during the design phase of the project to analyse; ground conditions including rock mass properties, ground and ground water quality, rock mass loadings, excavation sequencing, and construction methodologies.The increased durability of steel fibre reinforced shotcrete over traditional tunnel support, in the right ground conditions, makes it possible to achieve a serviceability design life of greater than 100 years (as required in most infrastructure projects). By specifying a good quality shotcrete mix and ensuring adequate quality control for batching, fibre dosing, preproduction testing, excavation sequencing, and application control, it is possible to eliminate the requirement of a secondary lining. The properties of a suitable permanent lining must be well established using suffi cient preproduction testing in combination with post application monitoring.A major benefit of steel fibre reinforced shotcrete in ground support applications is the removal of any requirement for workers to work under unsupported ground. Its use can also significantly reduce, if not eliminate, the manual handling associated with placing traditional reinforcement such as lattice girders and mesh. Both of these benefi ts are significant with the ever increasing focus on health and safety.This paper highlights factors that must be considered in the design and specifications to enable an initial shotcrete tunnel lining to become the permanent shotcrete tunnel lining.

Xstrata Copper is extending the Ernest Henry Mine with a 6 Mtpa sub-level cave (SLC) mine being constructed under the pit. The SLC mine will maintain the EHM operation until 2024 after the pit is completed in August 2011. In February 2008, the first cut of the new underground mine at EHM was fired with the start of an exploration decline. Since then Xstrata Copper has continued the development and construction of a SLC mine. During this process a number of challenges arose, some of which had not been anticipated in the planning stages, that in turn has been addressed to ensure the development continued. The challenges have been many and varied and range from setting up the portal area and commencing the exploration decline development in an active large open pit, to management of underground water. Further challenges were presented by the effects of the GFC on investment recommendations, with the scale of the mine varying from a shaft mine to a smaller scale trucking only mine in mid-2009 and back to the full shaft mine in late 2009. The effect was the parallel development of two separate mining scenarios, with the challenge of maintaining commonality for as long as possible to keep both options alive until the final board approval was obtained. In 2010 the RSPT caused further impacts. This paper will describe the current progress of mine development as well as how the various challenges have been met and some key lessons learnt from these experiences.

This paper describes the design of the precast concrete segmental lining for the 13.1 m internal diameter and 450 mm thick, 2.4 km long mainline tunnels for the Waterview Connection project in Auckland for the New Zealand Transport Agency.The design and detailing of this segmental lining presented some noteworthy challenges, including an alignment that has sections of low cover in soft ground. Key points of interest are that this precast segmental lining is the largest to ever include steel fibres as the primary method of reinforcement and the lining includes cams on circumferential joints to control ring deformation.The paper describes some of the key design decisions that were made in a collaborative manner with the construction team, tunnel boring machine manufacturer, segment manufacturer and the designers.CITATION:Mahajan, R, Kuras, A and Pradhan, S, 2014. Gold mineralisation at Macraes occurs as late quartz-carbonate veins within a body of sheared and hydrothermally altered Otago Schist, the Intrashear Schist. The alteration is characterised by the presence of rutile and the absence of epidote and titanite, indicative of an alteration fluid containing >10 mol per cent CO2. In areas of high strain, a fabric of black microshears develops, gradational to mylonitic cataclasite in the high strain zone adjacent to the hanging wall of the duplex shear zone. The microshears contain hydrothermal graphite, inferred to have been deposited from a methane-rich fluid. The mylonitic cataclasite is characterised by abundant pyrite, the absence of albite and extreme grain size reduction. Gold recovery in the CIL processing plant at Macraes can drop to 50 per cent or lower for some unfavourable ore types. Preg-robbing by a component in the ore, possibly graphite, was suspected as the cause. Mylonitic cataclasite was identified as the lithology responsible for the poor recoveries, with the capacity to bind about 30 ppm of gold in the leach tanks. A program of detailed geochemistry and quantitative grain-size measurement using Coulter analysis was undertaken to identify specific mineralogical factors associated with preg-robbing. The results indicate that pyrite, graphite, and extreme grain-size reduction are the factors associated with highly preg-robbing ores at Macraes. The evolution of the mylonitic cataclasite is accompanied by strong potassium enrichment, as muscovite, and depletion in sodium as albite is removed. This suggests that a simple powder XRD ratio of the albite to illite peaks would suffice to distinguish the preg-robbing ore for grade-control purposes.

"Diesel particulate matter (DPM), in particular, has been likened in a somewhat inflammatory manner to be the ‘next asbestos’.From the business change perspective, there are three areas holding the industry back from fully engaging with the issue:There is no real feedback loop in any operational sense to assess the impact of investment or application of controls to manage diesel emissions.DPM are getting ever smaller and more numerous, but there is no practical way of measuring them to regulate them in the field. Mass, the current basis of regulation, is becoming less and less relevant.Diesel emissions management is generally wholly viewed as a cost, yet there are significant areas of benefit available from good management.This paper discusses a feedback approach to address these three areas to move the industry forward. The six main areas of benefit from providing a feedback loop by continuously monitoring diesel emissions have been identified: Condition-based maintenance. Emissions change instantaneously if engine condition changes. Operator performance. An operator can use a lot more fuel for little incremental work output through poor technique or discipline. Vehicle utilisation. Operating hours achieved and ratios of idling to under power affect the proportion of emissions produced with no economic value. Fuel efficiency. This allows visibility into other contributing configuration and environmental factors for the vehicle. Emission rates. This allows scope to directly address the required ratio of ventilation to diesel emissions. Total carbon emissions. Long-term water management is typically ignored in mine closure planning. A review of 73 mine closure plans dating from 2007 to 2013 shows that, on average, relinquishment is proposed to be achieved in 11 years. A separate review of 57 mines in actual closure shows that only five have so far achieved relinquishment with an average closure period of 14 years after production cessation. The remaining 91 per cent of sites in closure have an average closure duration to date of 21 years - and still going.A common issue among those sites in closure is water management, with a further common issue being acid and metaliferous drainage (AMD). Of the closure plans that were reviewed only one site had allowance for long-term (>50 years) of water treatment. Of the sites in actual closure 25 per cent have reported that they expect such long-term water treatment, with many acknowledging that in perpetuity treatment is probable.Clearly there is a gap between what is being allowed in closure plans and the reality of closure. There are a number of factors leading to the optimism of the closure plans, including unrealistic assumptions, pressures to reduce closure cost provisions and the technical complexity of many AMD issues.Long-term water management is a reality that needs to be acknowledged and addressed. This paper proposes a new multiresource multistage scheduling problem for optimising the open pit drilling, blasting and excavating operations under equipment capacity constraints. The flow process is analysed based on the real-life data from an Australian iron ore mine site. The objective of the model is to maximise the throughput and minimise the total idle times of equipment at each stage. The following comprehensive mining attributes and constraints have been considered: types of equipment; operating capacities of equipment; ready times of equipment; speeds of equipment; block-sequence-dependent movement times of equipment; equipment-assignment-dependent operation times of blocks; distances between each pair of blocks; due windows of blocks; material properties of blocks; swell factors of blocks; and slope requirements of blocks. It is formulated by mixed integer programming and solved by ILOG-CPLEX optimiser. The proposed model is validated with extensive computational experiments to improve mine production efficiency at the operational level. Coal mining is recognised globally as a hazardous activity, and as a result operates under high levels of regulatory and public scrutiny while managing a workforce operating in a high-risk environment. Mining companies have developed sophisticated risk assessment procedures, management systems and personnel competency requirements that facilitate the safety and health of employees, and Australia is a world leader in coal mining safety and health.RISKGATE is an Australian Coal Association Research Program funded online platform that delivers broad cross-industry knowledge about current practice in the identification, assessment and management of coal mining risk. Centennial Coal has engaged with the RISKGATE program, managed by the University of Queensland’s Minerals Industry Safety and Health Centre, since its inception in 2010, and contributed topic experts to many modules, including underground topics within the geological and geotechnical domain (strata underground, outburst, coal bumps and bursts, and inrush).In 2013, Centennial Coal experienced significant unexpected strata failure at the Mandalong mine without any personal injury or fatality. This paper presents a case study in the application of RISKGATE to management of this unwanted event, the steps that were implemented, the information that was sourced and the contribution that RISKGATE made to the final outcomes. Strategic mine scheduling is an industry best practice for increasing the long-term value of mining operations and supply chains. The application of optimisation technology such as mixed integer linear programming (MILP) is able to achieve a mathematically optimal result for a given scheduling model. Such a model will typically define the available source material, processing options and desired constraints for accessing and processing the material. Most strategic scheduling models, however, do not include a sufficiently detailed model of capital expenditure options related to capacity expansion.Typical strategic schedule models do not include important capital expenditure features such as capacity decay or expiration (limited lifespan of trucks), precedence relationships over time (rail construction before a new plant or non-linear expansions due to economies of scale) or mutually exclusive option sets (choices of plant size). By not including these features, a strategic mine scheduling model for a capital-sensitive operation is unable to be optimised to deliver maximum value or profitability.This paper presents several results of MILP optimisation using advanced capital expenditure modelling for mining operations and supply chains. Results of the study demonstrate that optimising capital expenditure at the same time as optimising the mining schedule allows for minimisation of capital purchasing, optimal timing for those purchases and optimal scheduling around them. A material aim when feeding ore blends into a processing facility is to reduce the variability of the properties of the feed material and to maximise the output, or throughput. At the Mining Geology conference of 2019, we presented a paper on optimal blending of coking coal given geological variability. The paper introduced integer-programming techniques to both maximise value and minimise feed or product variability. We discussed an application where the geological uncertainty is characterised using a normal distribution and exact solutions are readily obtainable using second order conic constraints in the mathematical model. Geological uncertainty is not always normally distributed, many properties will be characterised as log normal or some other skewed distribution. For example, copper can be some form of lognormal and Fe in Iron ore has a left skewed distribution. In this paper, we discuss some distribution free techniques to optimise the blends of plant feed or output given a quantified geological uncertainty. We discuss the sample average approximation method and how to set-up and apply the method to an ore blending problem. Evaluating a case study demonstrates that the explicit use of non-normally distributed geological uncertainty can better optimise feed into an ore processing facility and potentially add significant value. The approach is equally applicable to all mining and processing operations that blend material into a processing facility or directly into a saleable product.

The challenges of increasingly scarce water resources and the risk of dam failures has seen the mining industry turn to sustainable tailings management solutions such as dewatering and dry stacking. Filtering tailings has been seen in the past as a cost-prohibitive process for larger throughput mines. However, as larger pressure filters are developed, the economic case for filtered tailings has gained traction.Focus has largely been placed on the correct design and operation of the filters. It is critical, however, that the material handling portion of the disposal system is also properly designed and operated. Thorough testing at laboratory and pilot scale to determine the conveying properties of the filtered material needs to be conducted. This testing will insure the material handling system meets the important availability needs of the mine even during periods of variability in the upstream processes and mine plan.The control system must also be robust and flexible in design to compensate for this upstream process variability. Acidic and metal-rich waters have been discharging from Sullivan Mine, West Coast, New Zealand since the closure of the mine during the early- 1990s. This has resulted in a decrease in pH from a background of 4.5 to 2.8 and an increase in metallic elements in nearby Rapid Stream. In addition to the chemical effects on Rapid Stream, a study of freshwater macro-invertebrates revealed the absence of most biota downstream from the discharge. An attempt is being made to use the Sullivan Mine as a case study of problem and remediation analysis.

"This paper was primarily put together in order to demonstrate what are believed to be acceptablestandards, in relation to ventilation surveys and the subsequent model assembly process. Thiswas brought about by the fact that in many parts of the world it is regarded as acceptable to carryout ventilation surveys and then build and supply the client with a æstick fi gureÆ type ventilationmodel. Such stick models have arguably litt le use and a very short life span in the real world and isalmost impossible for site staff to maintain. Thankfully most underground coal mines in Australia strive to maintain a true replica model of their ventilation circuits and to keep them in a relatively good state. Given that these mines own and maintain the models, it is often seen as necessary to update or rebuild them. The level of survey detail required to do so is totally dependent on the end use of the data and the criticality of the accuracy of the data set, to the ongoing safety and viability of the business. Mine technical personnel should think clearly about the use and importance of the ventilation survey and subsequent report outputs required that will help them bett er understand their mines and bett er prepare them to manage their ventilation circuits, on an ongoing basis. This paper provides some guidance for Australian ventilation engineers so they may plan and execute this work æin houseÆ, should they rightly choose to do so. To this end, methods of data collection and circuit analysis required for surveys of a high order of accuracy are detailed. Also, some of the issues which are important when formulating a strategy to carry out the appropriate ventilation survey for the required purpose are summarised. As well, the major details that should be furnished within any survey and some survey/report outputs and/or omissions, which are clearly not acceptable, are highlighted."
