Safety and health at work is about people. It is about people who plan for work, organise work and perform work. It is about those who come in contact with the workplace and those who are responsible for the workplace. There are wider ranging safety and health obligations that are imposed on the industry stakeholders that span all phases of the production process. In all cases, those responsible for discharging the obligations must ask the questions: Is what we propose safe?, can it be done safer? and are we sure? What we learn from recent legal cases before the courts is that it is essential that all stakeholders work together in a collaborative way when discharging these obligations. Safety and health at work cannot become an issue that is viewed only through the eyes of a single stakeholder group. The reason for this is quite simple. The interrelatedness of the responsibilities that are imposed on each of the stakeholder groups, means that the performance based legislation simply wont work, where an active model of communication and consultation is not in place.

This paper examines the problem of optimising the life-of-mine costs of an underground mine, focussing particularly on the costs associated with the underground development needed to provide access to and haulage from the ore zones. This mine optimisation problem is modelled as a network and a solution technique is outlined. The effectiveness of this solution technique is then demonstrated by applying it to a prototype test example and some actual case studies. Finally, extensions of this technique to other underground mining problems are discussed.

Incidental to the progress of a geological survey of the Kalgoorlie goldfield during 1927 - 1928 a number of specimens of telluride minerals were obtained, chiefly with the help of Mr F G Brinsden, general manager of the South Kalgoorlie mine, and Mr Ernest Williams, general manager of the Boulder Perseverance mine. This collection was supplemented by a number of small specimens, including the rare copper telluride, which were collected and preserved from the Kalgoorlie mine by Mr A E Paton, and also by a few miscellaneous specimens from the collection of MrS F C Cook, of the Palace Hotel, Kalgoorlie.An extensive knowledge of the rare tellurium-bearing minerals was compiled during the last century from occurrences in Transylvania and Colorado. The recognition of different species has been based on chemical analyses and crystallographic measurements. In a few species there has been an absence of crystalline form, and these, together with the discoveries in this century of rickardite and 'weissite', muthmannite and empressite, have depended chiefly on the chemical analysis for identification. On the other hand, the crystalline form of nagyagite and stiitzite has been described in detail, while their precise chemical formulae were doubtfully defined.The announcement of the occurrence of tellurides at Kalgoorlie by A G Holroyd in the Kalgoorlie Mine on 29 May 1896, followed by their discovery in comparative abundance, stimulated the further study of these minerals. In this locality there is a general absence of well-formed crystals, and the knowledge gained has been confined to the record and description of previously known, but exceedingly rare, species.No new species has been recorded, but the intimate association of various species resulted in early description of two supposedly new species, kalgoorlite and coolgardite. These have been discredited since the investigation by L J Spencer in 1903, based on a collection of specimens donated to the British Museum.Spencer's list of species at Kalgoorlie comprises calaverite, sylvanite (or krennnerite), petzite, coloradoite, 'and altaite, and does not include hessite, previously mentioned by Krusch. In 1912 melonite and a copper telluride were added by J A Thomson and the presence of hessite confirmed. In 1912 there also appeared a full and detailed account by E S Simpson on behalf of the WA Geological Survey, which embraced his earlier notes on the tellurides, and which represents the state of our knowledge of the Kalgoorlie minerals, practically as far as can be attained without the use of the reflecting microscope.The position has been stationary for about 18 years, and it was thought. that the best use to which the present collection of tellurides could be devoted was an attempt by mineragraphic methods to study the mineral relationships, not only of the tellurides among themselves, but also to gold and other associated minerals. The precise mineral content and relationships become a factor of added importance with the development of flotation methods of treatment at Kalgoorlie.Moreover, the study would supplement the description of the ore deposits of the Boulder Belt that has recently been published.

For metallurgical plant design, it is necessary to progress through successive phases of test work, piloting and process development before involving all the other engineering disciplines and “freezing” the process design criteria and flow sheet. This process usually relies upon the experience, skills and personal preferences of various technology providers, study and engineering entities, test work and metallurgical laboratories and, most importantly, the owner’s team and its operators. Current industry trends include lower grades, more complex orebodies, geometallurgy in defining orebody variation and design envelopes, more common “hybrid” plant processes and the market demands for more reliable, timely and predictable early plant performance.This paper will discuss interaction of these trends and provide guidelines for the level of detail and technical complexity required for a number of different “typical” examples. The paper will pay particular attention to the study definition level translated into the required detail and certainty needed by the overall process “owner” and original equipment manufacturer (OEM) equipment suppliers to ensure the best real plant performance. Some recent case studies will be discussed. In many projects, at a late stage of project definition, it has become common practice to undertake a “value engineering” phase. Although it could be argued that this should be undertaken earlier, it is largely driven by an overall need to reduce capex and “optimise” the perceived “fat” built into the project during the detailed design process. This value engineering can be very counterproductive to project timing and operational performance and warrants discussion.The paper is intended to promote discussion on a topic of wide interest. The total number of tailings accidents has reduced from over 50/decade in the 1960s-1980s to less than 30/decade in the 2000s to 2010s. However, the rate of significant tailings dam failures has not been reduced. The significant tailings dam failures the mining industry experienced in the recent history include: Merriespruit, South Africa, 1994; Los Frails, Spain, 1998; Kolontár, Hungry, 2010; Mount Polley, Canada, 2014; and Samarco, Brazil, 2015. The conventional Factor of Safety approach itself may not be enough to further reduce the rate of significant tailings failures due to its limitations such as uncertainty. For example, the same factor of safety could have a vast range of annual probability of failure for different tailings dams, and insufficient consideration of failure consequence. This paper presents a risk based approach for design with the objective being to minimise tailings dam failures, especially those significant tailings failures with high consequence and low probability. This approach includes risk based design and risk management using bowtie tools. In the risk based design, quantitative analysis will be used to estimate the annual probability of failure and the tolerable failure criteria is linked to the consequence category. Bowtie analyses will be used to understand hazards and risks and identify and select independent controls that are effective in preventing and mitigating catastrophic failures. As part of this work the nature and extent of many of the alluvial deposits have been resolved and the ages established with varying precision. Ausmelt is supplying commercial quantities of an alkyl hydroxamate flotation collector AM28 for flotation of oxide and supergene base metal ores and tailings. AM28 is a non-hazardous product used as a ten per cent to 50 per cent solution in dilute potassium hydroxide. It is a selective flotation collector for oxide or oxidised base metal minerals and replaces conventional sulfidisation for oxide mineral flotation. At Radio Hill, the AM28 is used in conjunction with traditional sulfide collectors, which allows simultaneous flotation of oxide and sulfide copper minerals. The Fox Resources Radio Hill copper concentrator near Karratha, Western Australia, is currently configured to treat supergene copper ore sourced from the West Whundo open pit. This Archean VMS type copper deposit has been deformed and remobilised with uplift, erosion and weathering to form a significant supergene zone. The sulfide copper minerals present in the upper zone are digenite and chalcocite with lesser chalcopyrite and covellite. Oxide copper minerals present include cuprite, with lesser malachite and azurite. Gangue minerals include quartz, talc and chlorite. Plant trials at Radio Hill are reported for supergene West Whundo ore, where the AM28 was stage added into the circuit at nominally 80 g/t. Plant data over three months shows the copper recovery increased consistently from around 50 - 60 per cent to above 70 per cent when AM28 was added. This has been proven statistically. The grade of the concentrate was consistent with and without AM28, at nominally 22 - 24 per cent copper.

A high-nugget effect and presence of coarse gold characterises many styles of gold mineralisation. The effective sampling of these deposits is often difficult because of the relatively low concentrations involved and erratic nature of the gold or other minerals. Geometric and/or metallurgical complexities often compound this effect. Underground development, in-fill drilling and bulk sampling/trial mining are generally required to delineate Ore Reserves. Similarly expensive definition strategies often apply to surface mining in complex orebodies of gold and other minerals, including precious stones. Development, particularly winzing is a historical technique that was used to gather geological, grade and metallurgical data during underground and near-surface exploration. It permits access to the mineralised structure below the surface datum to determine viability. Winzing has always been considered an arduous and dangerous task. Modern winzing technology has led to the development of the Mobile Supersucker – essentially a giant vacuum cleaner with underground and surface capabilities. By the elimination of shovel-loaded blasted winze material into a kibble and hoisting, this system permits safe and highly efficient winzing activities, and allows the collection of bulk samples for grade and metallurgical evaluation, as well as access to the mineralisation for geological and geotechnical mapping. Bulk samples from horizontal development have long been collected to help define mineable grade via pilot processing. However, when defining blocks below current development, there has been over-reliance on ineffective ‘small sample’ drill data. The Mobile Supersucker permits the development of winzes and off-winze sub-levels. Daunia and Caval Ridge coal mines, owned and operated by BHP, have been co-disposing rejects and dewatered tailings within spoil dumps from the beginning of their operations in 2013 and 2014, with the purpose of minimising environmental impact and geotechnical risk. Following the recent large failures of tailings dams around the world, BHP Coal, through its Coal Tailings Program, plans to transition other coal mines to deliver “dry” tailings and produce mixed plant rejects (MPR) for inpit co-disposal. This will materially reduce future tailings dam risk exposure. Previous laboratory shear strength testing was conducted on rejects with a maximum particles size of 20 mm, and some concerns were raised that this might not represent the mechanical properties of the actual full particle size distribution (PSD) rejects. BHP was also aware that using different technologies to dewater the tailings, such as belt press filters (BPF) and solid bowl centrifuge (SBC), could produce thickened tailings which could significantly modify the physical behaviours of the MPRs, particularly the bonding between particles. Therefore, laboratory testing on both coarse rejects and mixtures of the coarse rejects and dewatered tailings (MPRs), with full PSDs, was carried out. Testing was performed using the large direct shear machine developed by the Australian Coal Association Research Program (ACARP) project C20019 in 2017, as well as a 300 mm diameter triaxial apparatus at the University of Newcastle (UoN). This paper presents details of the laboratory testing program and results for the rejects from the first studied mine of the BHP Coal Tailings Program. It compares the shear strengths measured at full scale with those tested on modified (scalped or crushed) specimens in the past; and discusses the tailings dewatering technique selection from geotechnical aspects based on the testing results, and finally presents some dump and tip head stability analysis results using the testing results.

Open cut coal-mining has been conducted in the Bowen Basin region of central Queensland for almost five decades, and has disturbed an estimated area of 140 000 ha of agricultural land until 2011. Approximately one-third of the area disturbed by mining has been rehabilitated to pastures or bushland. However, none of the rehabilitated land has yet been assigned a specific end-use. This research project develops an approach for assessing end-use risks for the regionÆs mined land, and develops risk assessment models for selected end-uses. First, a web-based survey of stakeholders was used to identify risks to be assessed. The risks identified were surface erosion, subsurface erosion, bushfires, weeds and feral animals. The survey also identified possible end-uses of bushland and grazing. Second, conceptual risk models were developed based on the risk assessment concepts of likelihood and consequence specified in the Australian and New Zealand risk assessment standards. Likelihood was modelled using site characteristics and management factors that influence the occurrence probability of risks (eg surface erosion), while consequence was modelled using a set of site condition indicators and condition thresholds (changes in root-zone water-holding capacity, soil erodibility, vegetation ground cover and soil organic matter and transition probability to a non-preferred ecosystem type).The factors influencing likelihood and consequence for each risk have been integrated using Bayesian networks. The next step in model development is to parameterise the Bayesian networks using existing equations, empirical datasets, or expert opinion where data are not available. The parameterised models will be used to assess grazing and bushland end-uses against erosion, bushfire, weed and feral animal risks for rehabilitated mined land sites, with the purpose of identifying the relative risks associated with each end-use, and the landform design specifications and land management practices under which end-use risks are minimised.CITATION:Maczkowiack, R I, Smith, C and Erskine, P D, 2012. Constraints on the mineral industry are best treated as issues that have to be understood and managed in partnership rather than in conflict with communities. The Zapopan development at Mt Todd in Australia and the Martha Hill development at Waihi in New Zealand are but two of many recent successful examples at the local level. However, at the national level, the issues are more complex and more difficult to resolve because the most influential stakeholders are now the urban electorates and governments defer to their perceptions when making decisions and policy.

Cannington Mine is an underground silver lead zinc operation located in north-west Queensland which has been in production since 1997. Ore is primarily moved through an underground material handling system comprised of a crusher, conveyor and hoisting shaft. The Fowler Shaft was constructed in 1997 using a strip and line method with the intent for it to provide a means to hoist ore from the loading station 600 m below surface. The shaft also serves as the route of key services including high voltage power, mine dewatering and fresh air. Despite the adherence to a mining exclusion zone, large-scale change in geotechnical conditions around the shaft were first observed in 2015. Ongoing ground movement around the shaft lead to cracking and delamination of the concrete liner. Shaft management required the on-site team to measure deformation rates and magnitudes, forecast change, manage rockfall risk and deforming pipework, and ultimately, hoisting skip clearance. Monitoring techniques to observe and measure deformation were required to evolve with the magnitude of movement. Some techniques leveraged off modern technology while some of the most useful techniques were based on logic and first principles. All shaft deformation management strategies relied on the co-operation and alignment between Cannington’s Infrastructure Maintenance, Survey and Geotechnical Departments.

The present study attempts to critically review the pillar-design as required by Coal Mines Regulations of India vis-a-vis latest rockmechanics formulae. Equivalent material modelling was the approach adopted for testing the safety and stability of the pillar sizes predicted by various formulae. Two parameters, considered here for calculating pillar sizes was the stresses acting over the pillars and the strength of the pillars. Stresses acting on the pillars were calculated by the tributary area method. Based on the studies of Coates and Hustrulid, the stress obtained was reduced by 40 percent. For calculating the strength of the pillars, three formulae, namely Holland and Gaddy, Salamon and Munro and Central Mining Research Station, India, were used. The pillar widths were then determined by equating the product of recommended factor of safety and pillar stress with the pillar Strength.

The prediction of how waste materials will evolve geochemically within waste storage facilities has been the subject of hundreds of thousands of hours researched by geoscientists globally. Despite the quantity of research there are a number of significant areas of uncertainty that remain as partially resolved issues. One of the most significant issues is the applicability of scaling laboratory test results to field conditions, and the use of laboratory data for predictive calculations to determine geochemical evolution of waste materials in the field. This issue has arisen predominantly as a result of the relative paucity of field monitoring data from waste rock storage facilities (WRSFs) and the fact that laboratory test methods are not typically linked to specific field conditions; rather they are based on generic standardised methods and designed primarily to simulate accelerated weathering conditions that do not relate to field conditions in a linear manner.To address these specific issues the authors have in the last two years made significant progress in the development and deployment of advanced field monitoring equipment within numerous WRSFs. O’Kane Consultants (OKC) has previously reported the findings from data collected from these monitoring installations in a number of papers presented throughout 2013–2014. This valuable site data has subsequently been used by OKC as the basis to design and engineer advanced customisable site-specific kinetic leach columns at OKC’s geochemistry research laboratory. These columns represent an important advancement in kinetic column assessment methodology as they have been designed to replicate the field internal conditions within waste facilities.Key features of the columns include their capacity, size and technical specifications; including fully programmable solenoid-controlled air supply with variable flow and pressure control and in-line heating option, automated temperature control and monitoring, automated oxygen consumption and carbon dioxide production monitoring, and soil moisture/matric suction monitoring.This flexibility of the design not only allows for more accurate field-based assessment of pyrite oxidation rates (POR) but also assessment of the potential effectiveness of waste management options such as deposition within a reduced oxygen environment, for example, as simulations of reduced net percolation rates or oxygen ingress rates can be accommodated.CITATION:Pearce, S, Pearce, J and Barteaux, M, 2015. Friction rock stabilisers, commonly termed ‘Split Sets’, friction rock bolts, or continuous friction coupled bolts are the most widely used rock bolt in the Australian underground metalliferous industry. Historically, they were used for temporary reinforcement, mainly within ore development. However, their use as primary reinforcement has grown with the demand for higher development rates at lower costs. The introduction of galvanisation has increased their longevity and pattern bolting in conjunction with weld mesh, specifically made for use in development headings, has allowed the use of friction rock stabilisers to increase rapidly with development jumbo’s undertaking the installation. The installation of friction rock stabilisers appears to be deceptively simple, particularly when compared to resin encapsulated bolts. The friction rock stabilisers capacity is a function of the borehole parameters, rock mass quality/strength, the bolt dimensions and steel quality, and lastly and perhaps the most important the operator skill levels. However, as in industry we are still seeing common occurrences of friction rock stabilisers not reaching their design pull test capacity. Pull testing the friction rock stabilisers is used to verify the installation of the design capacity. It involves loading the collar of the bolt with a hydraulic ram until the design load is reached or the bolt slips. Pull tests often show failure rates of 20 per cent or higher. This is despite over 40 years of use and with millions of units installed annually. Such high failure rates would suggest there remain issues with the quality of installation. Ground support quality assurance and quality control (QAQC) programs are needed in combination with operator training to ensure that pull test failure rates are kept as low as practical. However, there are times when despite the QAQC being completed the pull test failure rates increase quite suddenly. When this occurs, it is important to conduct a thorough investigation. These investigations often show there are several factors that are not obvious to personnel installing the bolts. To complete a useful investigation, it must also be done methodically, working through the entire process that includes less obvious factors such as steel quality and the calibration of the hydraulic pull test kit. The aim of the paper is to give mine practitioners an understanding of the complexities of friction rock stabilisers, what QAQC requirements they should be doing on-site, what can go wrong during installation and, if necessary, a process of how to conduct investigations into high friction rock stabiliser pull test failure rates.

Field trials at three New South Wales and one Queensland coal mines were carried out to gather data on oxygen "run out" times of Self-Contained Self Rescuers (SCSRs), time taken to escape from the mine, distances travelled and the average heart rate of subjects wearing SCSRs. The study has led to a method of predicting the duration of oxygen supply from a SCSR as a function of the wearer's body weight, physical fitness and the prevailing environmental conditions. Escapeway design, planning for emergencies, familiarity with SCSRs and experiential escape training are critical to control panic and maximise the likelihood of survival of a person attempting to escape in an emergency involving fire or explosion.
