Many organisations struggle to yield meaningful outcomes from incident investigations. This paper will demonstrate that we can discover systemic issues by rethinking the ‘Why’ and ‘How’ of investigations, fostering genuine risk reduction. Key points: 1. Shift from ‘prevent recurrence’ to ‘learn and reduce risk’: conventional investigations focus narrowly on recurrence prevention, resulting in narrowly focused investigations with limited ability to identify and reduce organisational risk. Redirecting our objective to learning and improvement opens avenues for broader risk mitigation. 2. Understanding daily work for risk insights: recognise that work is not always completed in full compliance with procedures. Workers will adapt how they complete tasks influenced by the conditions and management systems that exist every day. Understanding the conditions and systems affecting everyday work is vital for uncovering systemic risks. 3. Unearth systemic issues through in-depth exploration: many investigations address surface concerns such as lack of training, incorrect procedures, equipment not being maintained, and missing overarching organisational risks. Systemic, organisation-wide risks are not identified. To discover these systemic issues and create shared learnings, investigators must question management systems, including resourcing, planning and risk management. 4. Design actions for safe failure: common actions like discipline and training fall short and do not reduce risk. Resilience emerges from well-designed controls. Incorporate planning, better resources, interlocks, and guarding to bridge the gap between task execution and risk. 5. Investigation Reports as a Business Case: Investigation reports should not simply state historical information regarding an incident. They are to facilitate decision-making and be presented as a business case to drive change. Organisations can move beyond localised risk and ineffective administrative actions by changing investigation objectives and methods. They can facilitate learning, understand daily work intricacies, identify systemic issues, and enhance the capacity for safety, driving lasting risk reduction.

The soil-water characteristic curve (SWCC) is one of the most essential soil properties in unsaturated soil mechanics. The water flow and storage characteristics of an unsaturated soil are closely related to the amount of water contained in the pores and the negative pore-water pressure in the soil. These are both in turn, related to the particle-size distribution and the plasticity of the soil. The SWCC is obtained by laboratory testing which is difficult to perform, costly and time consuming. There is a significant advantage in establishing typical SWCC envelopes for different soil types from a geotechnical design and analysis viewpoint. This paper presents some of the research work undertaken by UWA and NTU to develop SWCC envelopes for soil types classified in terms of the ASTM D2487 Unified Soil Classification System (USCS). This was done by establishing a large database of soils from across the world, classifying them according to ASTM D2487 and undertaking SWCC testing for each soil. Reconciliation involves the collection of tonnage, grade (quality) and contained metal (product) data from disparate and hopefully independent sources. Examples are exploration data, production sampling data from blastholes or drawpoints, and process plant data. These data may be compared by means of ratios (factors). The F1 factor usually relates short-term (ore control) model tonnages, grades and metal content to ore reserves depleted. The F2 factor usually relates received-at-mill tonnages, grades and metal content to delivered-to-mill = production tonnages, grade and metal content. The F3 factor is F1 × F2 and enables a comparison of a mineÆs ability to recover the tonnage, grade and metal content estimated in ore reserves. The F1 factor measures the accuracy of orebody knowledge in the ore reserves to the demarcation of ore and waste by ore control (short-term model). The F1 factor may be used to check and calibrate the selectivity of mineral resource models and/or planned dilution assumed in transfer from mineral resources to ore reserves. The F2 factor enables a check on unplanned dilution entering the ore stream between ore control and the mill. Using the factors it is possible to calculate a monetary value on improvements in the accuracy of orebody knowledge, selectivity and the effects of dilution and ore loss. Reconciliation should be an implicit part of the mining process, and reconciliation targets should be a key performance indicator for well-run mines.

KiwiRail has a goal of significantly improving passenger rail services to west Auckland, as part of its DART (Developing Auckland’s Rail Transport) suite of projects. The New Lynn Rail Trench (NLRT) project was a significant component of this work. The NLRT project facilitates essential doubletracking of the Western Auckland line, and provides a new commuter transportation hub within a busy town centre.The 1.5 km stretch of track through the New Lynn town centre has bisected the township for over 80 years. Double-tracking the line provided the opportunity to grade-separate road and rail, which would help to re-connect both sides of the township and enhance public amenity and safety.KiwiRail employed an Early Contractor Involvement (ECI) procurement model engaging a consortium comprising Fletcher Construction, Beca Infrastructure and Synergine to develop a Design and Construct solution.With weak soil conditions and high groundwater levels, a diaphragm wall construction method to form the trench walls was selected as the best solution. Further challenges to construction included working within a narrow rail corridor constrained by buildings along much of the boundary, and a requirement to keep the existing line open for rail services. Detailed construction planning was required to ensure rail and construction safety in a constrained space whilst building the trench walls, propping these walls during excavation, and constructing the base slab in the most efficient way possible.This paper will detail the geotechnical conditions and the additional constraints that made this rail trench a challenging structure to design. It will also describe the fi nal design and construction methodologies that were employed, that allowed very successful completion of the project.Additionally, this paper will review the design and construction lessons applicable to the current Victoria Park Tunnel project, and indeed for all future cut and cover tunnel projects in the Auckland isthmus.

Geometallurgy is often a misunderstood and poorly defined concept. The first thing the paper will attempt is to provide a definition of geometallurgy. Ultimately this paper has been written to provide guidance to geologists, mining engineers and processing professionals as to what geometallurgy means in the crucial early phases of a potential mining project between the time of discovery and the time of commencing construction, should this occur. The paper covers the options available for conducting a geometallurgical study of a resource or orebody, appropriate to the level of definition required in the projectÆs development cycle. The paper will then describe how the geometallurgical information generated is subsequently used in calculations, planning and preparation of reports designed to inform and reassure stakeholders, such as the market. Ultimately in a successful project, appropriate options adopted in the development phase will form the basis of an ongoing geometallurgical program that links the major site disciplines for the good of the operation.CITATION:David, D, 2013. The Grasberg Mine is a copper and gold open pit mine operated by PT Freeport Indonesia (PTFI) and located in the high equatorial mountains of the Indonesian province of Papua. The climate is alpine/subalpine with little seasonal variation in temperature or rainfall. The daily average temperature ranges from about 2¦C to 14¦C and the annual rainfall in the vicinity of the mine and overburden stockpiles is about 4000 mm to 5000 mm. Mining of the Grasberg open pit will continue until 2014 at which time approximately 2750 Mt of overburden will have been produced. Current overburden production rates are 200 Mt per annum. The current mill rate is 230 000 t/day from the Grasberg pit (190 000 t/day) and the underground operations (40 000 t/day). The acid rock drainage (ARD) issues associated with overburden from the Grasberg operations were identified early in the mining operation. Almost all overburden placed to date has been Potentially Acid Forming (PAF). Laboratory testing has identified four main overburden types: limestone and non-acid forming (Green waste), PAF low capacity (Blue waste) and PAF high capacity (Red waste), and Heavy Sulfide Zone (HSZ), which contains greater than 20 per cent pyrite. Waste type block modelling indicates that at least 30 per cent of the overburden production from 2003 to the end of Grasberg pit operations will be limestone. Strategies are being developed to maximise the beneficial use of limestone through strategic blending and placement for ARD mitigation and control.

Land surface deformation can be caused by underground mining activities. The maximum subsidence normally occurs directly above the location of the excavation of the coal seam. In the case where the longwall mining technique is applied, the magnitude and shape of the subsidence bowl are constrained by the longwall structure (pillar). This paper describes the use of satellite radar interferometry for monitoring surface subsidence due to underground mining, as a cost-effective geodetic technique and complementary to conventional ground surveying techniques. This paper uses data acquired by the current generation of SAR satellites û the European ENVISAT and the Japanese ALOS û to monitor ground subsidence in the southern highland coalfield in New South Wales, Australia. An accumulated DInSAR subsidence mine map is produced using six successive ALOS acquisitions. The results are post-analysed with the aid of geographic information systems (GIS) and validated using ground survey data.

A better understanding of the detonation performance of an explosive charge can be gained by directly measuring pressure, temperature and velocity of detonation (VOD). This is particularly important with explosives used in the mining industry because their performance is directly influenced by the degree of confinement given by the borehole diameter and the surrounding rock mass. A project funded by the Australian Coal Association Research Program (ACARP) was initiated in early 2009 with the view to design and build cost-effective prototype instrumentation to measure the relative differences in detonation pressures and temperatures of commercial mining explosives. The project’s primary focus was on low-density explosives. Low-density (also referred to as low shock) explosives have been available in various forms for nearly 20 years. There have been significant advances in the availability, reliability and flexibility of these explosives, which now offer a range of densities and degrees of water resistance. Their detonation performance is not well understood and cannot be accurately predicted with current ideal and non-ideal detonation codes. It was therefore viewed as important to be able to directly measure pressure and temperature during the detonation process of these complex mixtures in production blastholes. Identifying the pressure release patterns of both conventional and low-density explosives under different geotechnical conditions should provide the necessary information to both validate detonation codes and better define input parameters for breakage and fragmentation models. This paper gives a general description of the prototype instrumentation developed and reports on the results obtained to date in full-scale conditions.

"Phytostabilisation with tolerant plant species has been advocated as a cost-effective solution for sustainable cover of tailings from metal mines and bauxite refinery. The development of sustainable plant ecosystems on such wastes requires not only tolerant plant species compatible with local climatic conditions, but also the formation of stable and functional root zones through systematic processes to develop functional soil matrix and horizons (eg A-B-C). However, the process requirements for reconstructing functional root zones in tailings are poorly understood. Based on a review of current literature and industry experience in tailings revegetation, the present paper conceptualises three key phases in the process of root zone development in amended tailings: hydro-geochemical stabilisation resulting from the interactions among tailings characteristics, amendment measures and local climatic conditions the rehabilitation of soil biological capacity the establishment of ecological (carbon and nutrient) linkages between the rehabilitated root zones and the above ground plant communities.Key research is required on: the relationship between hydro-geochemical stability and mineralogy and geochemistry of tailings, in response to rehabilitation treatments under local climatic conditions the relationship between soil biological capacity and hydro-geochemical stability in the root zone the transition of soil microbial communities specific to exogenous organic matters into those required for mineralising litters of in situ plant communities.CITATION:Huang, L, Baumgartl, T, Edraki, M and Mulligan, D R, 2012. The recycling of process water comes with increased electrolyte concentrations that affect water quality, which may in turn affect the flotation performance of valuable minerals (Hirajima et al, 2016). Electrolytes such as Ca2+, SO4 2- , Mg2+ and S2O3 2- have been surmised to have an impact on the action of flotation reagents such as collectors (Ikumapayi et al, 2012). Xanthate collectors are known to play a significant role in imparting hydrophobicity on mineral surfaces to enhance their flotation. The interaction between minerals and xanthates is well known to be electrochemical in nature. Such redox processes may be elucidated by electrochemical techniques. Since the mechanisms involving the interactions between electrolytes in process water with sulfide minerals and the subsequent adsorption kinetics of collectors are not well understood, this study therefore aims to provide fundamental knowledge to unpack the mechanisms involved when electrolytes interact with Platinum Group Minerals (PGMs) and how these interactions may affect xanthate adsorption. The interaction of sodium iso-butyl xanthate (SIBX) in the presence of electrolytes at increasing ionic strength was investigated by rest potential measurements, to explain the impact of electrolytes on the adsorption of SIBX on PGMs in the flotation process. Rest potential measurements for PdTe2 and PdS were conducted in an air saturated 0.05 M di-sodium tetraborate decahydrate solution, in the absence and presence of SIBX and electrolytes. It was found that the existence of sulfide and telluride on palladium affects the mechanisms in which electrolytes interact with Pd mineral surfaces. An increase in the ionic strength of Na2S2O3, CaCl2 and MgCl2, increased the interaction between the salts and PdS, thus decreasing the extent to which SIBX interacted with PdS. An increase in ionic strength for Na2S2O3 increased the interaction between Na2S2O3 and PdTe2 and therefore decreased the extent to which SIBX interacted with PdTe2. Other salts investigated in this study increased the rate of oxidation of PdTe2 with an increase in ionic strength thereby enhancing the interaction of SIBX and PdTe2. This study has shown that mineral type, electrolyte type and concentration all have an impact on the mechanisms that result in the adsorption of SIBX on PGMs.

Batch centrifugal gravity concentrators have become standard equipment in gold grinding circuits for recovery of gravity recoverable gold (GRG). The downstream benefits of installing a gravity circuit are well understood, and in general, if there is a reasonable amount of GRG in the ore, a gravity circuit is usually installed. The benefits of gold gravity recovery have also been well documented, with a large database of successful case studies. As well, predicting production scale gravity recovery results ahead of installing gravity has become a common practice. The missing link between lab and full scale has always been the ability to predict the recovery benefit of installing gravity ahead of actually doing so. Being able to predict the recovery benefit in advance would help an operation in assessing the economic benefit and help justify the capital costs of the gravity circuit with much less uncertainty.

How aqueous fluids move through rocks and minerals is a question of possible pathways and mechanisms. Existing pathways such as fractures and grain or mineral boundaries provide obvious channels for fluid movement. We show how minerals reactive in the presence of an interfacial fluid can be replaced by new product phases through a process of interface-coupled dissolution-reprecipitation. A resulting porosity in the new phase provides a new pathway for fluid movement and therefore mass transport through the mineral, which progressively reacts at a moving interface. Eventually fluids of new and changing composition can pass through originally solid minerals and move on further, taking remobilised elements with them, to be reprecipitated whenever the chemical and/or physical conditions allow. One such example is albitisation of rocks associated with mineralisation.

Multotec Process Equipment and Dominique Frantois-Bongarton have a patent for a mineral sampling device, which they call a æTrue-Belt SamplerÆ. This uses a plough, which is rotated about an axis parallel to a conveyor belt to throw material off the belt. A fixed sample cutter takes a sample from the material thrown off the belt.Discrete element modelling has been used to consider a range of plough designs, plough speeds, positions for the cutter relative to the plough, cutter apertures, cutter designs and frictional properties, prior to the building of a physical prototype. A full-length paper was not prepared for this presentation.The Halasu porphyry copper belt, located in the East Junggar basin bounded by Chinese Altay to the north and the Tianshan mountains to the south in the Central Asian Orogenic Belt, is one of the major porphyry copper belts in Xinjiang province, north-west China. Combined with previous studies in this area, the magmatism can be divided into three periods: pre-mineralisation activity of 390 Ma syn-mineralisation activity of 380–370 Ma post-mineralisation activity of 360–330 Ma.Through the comparison of zircon chemistry from ore-bearing and barren syn-mineralisation intrusions, negative correlation was identified between Ti-in-zircon temperatures and oxygen fugacity, corresponding to the fact that late-stage syn-mineralisation granitoids contain high water content, high oxygen fugacity and low temperatures with higher mineralisation potential. In addition, the relative low initial 87Sr/86Sr ratios, high eHf(t) and eNd(t) values and mantle Pb isotope characteristics in this study support that the magma forming these intrusions was probably derived from juvenile crust generated during subduction-accretionary processes.CITATION:Wu, C and Chen, H, 2015. Open pit/underground mining and mineral processing can generate large amounts of waste that contains sulfide-type minerals such as pyrite (FeS2), and it can produce acid mine drainage (AMD) when exposed to oxygen and water. This highly acidic wastewater contains heavy metals and this is one of the most serious environmental issues after mining operations taking several decades to completely remediate with high costs. For green mining, the present study focused on the development of a recovery technique for gold from an eco-friendly solvent, thiosulfate, by utilising mine wastes, tailings. First, gold recovery experiments in ammonium thiosulfate system were carried out using model samples representatively present in tailings (ie SiO2, Al2O3, FeS2, Fe2O3, and Fe3O4). The results showed that gold recoveries were negligible when using Al2O3, SiO2, Fe2O3, and Fe3O4 while pyrite shows a recovery efficiency three-fold as high as the maximum recovery efficiency when using others. Subsequently, the study also addressed the identification of the recovery mechanisms and post-recover process. The results indicate that tailings can be sufficiently utilised for gold recovery in the hydrometallurgy process and a further sustainable process can be established.

After a short presentation of the Division of Mining at the Royal Insti- tute of Technology, Stockholm, Sweden, the author presents some of-the fill research carried out at the Division. Literature surveys and testing methods and equipment are described. The use of flocculants' in hydraulic filling has been tested both in laboratory and in mines and the results are very promi- sing. Finally compressibility investi- gations and some finite element calcu- lations are presented.

Consent to develop and operate a new coal mine in New South Wales (NSW) will be conditional upon a number of environmental controls. In NSW, applications for any new major development associated with coal mining are determined by the Minister for Planning and Environment. The environ- mental conditions placed on the development consent, the coal mining lease and the approvals, and licences under the pollution control legislation amount to a substantial component of any new project's commissioning considerations. The nature, scale and location of projects and the principles of pollution control on which conditions of State Pollution Control Commission (SPCC) approvals are based have a considerable influence on project planning. The design, construction and operation of coal mines in NSW has been progressively modified over the past 15 years, to incorporate increasingly stringent requirements for environmental management. These requirements are in turn, a reflection of a greater community demand for environmental protection.

Equipment wear is strongly related to the performance of grinding circuits. In particular, SAG mills are subjected to the wear of their shell liners and grates. Changes in lifter height and face angle impact mill performance, while classification at the mill discharge is also affected as grates wear out. Moreover, liner mass loss caused by wear affects how operators interpret readings of sensors such as load cells or bearings pressure. Automating and optimising grinding circuits under these constrain becomes challenging due to the nonlinear and dynamic nature of the system. The operation of SAG mill grinding circuits remains largely dependent on human intervention, which frequently results in inconsistencies in performance across shifts, reflecting the operator’s personal preferences, experience, and other human factors. The influence of human factors can be reduced by careful training of operators as well as by automation using advanced supervisory control. The latter approaches could benefit from the development of dynamic process models that consider the effects of wear on the performance of equipment over time. This paper presents detailed industrial data showing the effects of wear on SAG mill performance. Additionally, a dynamic SAG mill model that accounts for these phenomena is proposed and demonstrated in a simulated environment. This model is shown to mimic the effect of wear on the performance of industrial scale mills and the operational constraints they are subject to, thus extending traditional SAG mill models. Incorporating wear into modelling and simulation platforms can improve the understanding of the effects that wear can have on the performance of key assets in comminution circuits and may allow the development of more robust model-based advanced process control strategies.

This paper describes the design, manufacture and site services associated with the supply of mine hoists for the new mine under construction by Falconbridge Limited in Ontario, Canada. Two new shafts are being sunk, and the hoisting equipment at each shaft can be summarised as follows: Ventilation shaft:4.66 m diameter double-drum second-hand hoist, originally in operation at San Manuel Mine, Arizona, to be used as the shaft sinking hoist. The second-hand hoist presented a challenge in that during its previous life, the hoist had been intensively operated during service at San Manuel Mine, and this paper describes the scope of refurbishment work undertaken, both electrically and mechanically, to ensure future reliable operation, together with the requirement to meet Canadian and Ontario regulations and codes of practice. 3.05 m diameter new single drum auxiliary hoist to be used as a second means of egress. Main shaft:4.57 m diameter new double-drum hoist to be used initially as the shaft sinking hoist, and subsequently to be converted to the equivalent of a single-drum multi-rope hoist for service duties during the production phase; 5.49 m diameter new double-drum hoist to be used for skip hoisting during the production phase; and 3.05 m diameter new single drum auxiliary hoist.The new hoists are all designed to comply with SABS requirements such that hoisting with reduced rope safety factors will be possible in the future. This paper describes the principles involved both mechanically and electrically. Particular attention is paid to the conversion of the main shaft service hoist from a double-drum sinking hoist to a multi-rope hoist.This paper describes the ac electrical powertrains (drives and motors) used that eliminate the need for harmonic filters and reactive power compensation equipment since they exhibit no negative effects on the utility electrical network system. This paper describes the logistical challenges of installing, commissioning and setting to work a series of five hoists in a short time frame.
