During the course of evaluations by the Mine Safety and Health Administration’s Roof Control Division, the controls on ground instability were investigated at surface and underground limestone mines throughout the eastern and southern United States. Structural geologic features, including joints and faults, were identified by basic geologic mapping as primary controls on highwall and back instability in surface and underground limestone mines. By comparing the spatial orientation of discontinuities and internal angle of friction with the orientation of highwalls on a stereonet, the potential for sliding, toppling, and raveling failures was evaluated in a semi-quantitative fashion. Structural geologic mapping identified faults and joints, which dismembered the back and were associated with ground instability, in underground mines. This paper documents the importance of recognizing structural geologic controls on ground stability in limestone mines, advocates basic geologic mapping as a critical component of ground control programs at surface and underground limestone mines, and provides examples of simple stability evaluations using the stereonet method.

The methodology used to obtain fly ash subsamples of different sizes is generally based on wet or dry sieving methods. However, the worth of such methods is not certain if the methodology applied is not mentioned in the analytical procedure. After performing a fly ash mechanical dry sieving, the authors compared those results with the ones obtained by laser diffraction on the same samples and found unacceptable discrepancies. A preliminary study of a wet sieving analysis carried out on an economizer fly ash sample showed that this method was more effective than the dry sieving. The importance of standardizing the way samples are handled, pretreated and presented to the instrument of analysis are suggested and interlaboratory reproducibility trials are needed to create a common standard methodology to obtain large amounts of fly ash size fraction subsamples.

Two 610 m (2000 ft) long tunnels were driven through water bearing silts, sands and clays beneath the Sacramento River using a 4.59 m (15.1 ft) diameter Earth Pressure Balance Tunnel Boring Machine (EPBM). Four sheet pile shored shafts were sunk with and without dewatering. The pre-cast concrete segmentally lined and grouted tunnels were driven down at 6% grades beneath the river flood protection levees, buildings, roads, a railroad and adjacent to a Freeway Viaduct. Details of the schedule, cost, shaft construction, TBM, tunnel construction, and installation methodology for the twin1.676 m (66 in) steel force main pipes and environmental constraints are provided.

Advances in tunneling technology, such as Earth Pressure Balance and Slurry Shield methods, have allowed the tunneling industry to construct tunnels in increasingly difficult ground conditions. In recent years, a number of civil projects including large diameter train tunnels, subway tunnels, water tunnels, sewer tunnels, and combined sewer tunnels have been successfully completed deep beneath the water table in soft ground conditions utilizing modern day Tunnel Boring Machines (TBM). With the increase in difficult ground conditions and higher water pressure comes the inherent need to utilize compressed air to gain access to the TBM?s cutterhead and cutting tools. This paper focuses on one such instance in Los Angeles, CA in which Traylor Brothers Inc. performed a cutterhead intervention by compressing the heading of an 11-foot diameter tunnel to approximately 3 bar gauge pressure. This paper focuses on the environmental controls used to manage ventilation, oxygen levels, temperature, and pressure, as well as discusses the problems associated with toxic gases and the increased risk of fire. The paper will also cover the physical science of the environment and the increased risks of performing basic job functions while in a compressed air heading.

Higher production, better safety standard, and potential for automation are some of the benefits of longwall mining. Today, longwall face advances at a faster rate exposing many diversified rock layers in a short period of time. It is now a serious challenge to cope with ground control problems such as roof falls, face and floor failure, and excessive shield loading as fast as possible to minimize production and monetary losses. In Illinois coal mines, the existence of weak floor strata blow the coal seam may pose additional problems related to floor heaving, shield base punching, and associated roof and face falls. In this study, the effects of weak floor on longwall ground control are analyzed using two-dimensional finite element models. A two-leg 635.6-ton (700-short-ton) yielding capacity shield is included in the models to evaluate the effects of different thickness and material properties of the weak floor. The study indicates that the thickness and material properties of weak floor have significant effects on shield loading, the distribution and intensity of front abutment stress, failure zones in the surrounding strata, roof-to-floor convergence, and floor punching by the shield base.

When done properly, benchmarking is an extremely effective tool for assessing ones performance, finding areas for improvement, and identifying strengths. However, when done improperly it can be an exercise in futility and result in incorrect observations and conclusions. A primary pitfall of benchmarking is collecting too much unreliable information. Surprisingly, the evolution of data storage and the ease of data collection are not helping with better data collection. Instead, it is just allowing one to collect more information, while at the same time inundating one with too much data. Additionally, the process of data collection can be tedious and is often times delegated to the least experienced individual resulting in unreliable and incorrect data collection and entry. Another primary pitfall of benchmarking is metrics that are cost based or poorly designed. Cost based metrics typically involve comparing unit mining costs ($/tonne). Between mines, unit mining costs are not very well correlated and have become further complicated by significant regional cost variations and mining inflation in the last years. Likewise, poorly designed metrics lead to poor benchmarking results. Examples of poorly defined metrics include: metrics that rely on information that is difficult to collect, metrics that are poorly defined, metrics which are too site focused, and metrics that measure low consequence practices.

A methodology has been developed and demonstrated to be effective in reclaiming surface-mined land for a forestry post-mining land use. The forestry reclamation approach (FRA) has been applied, almost exclusively, to flat or gently rolling surfaces. One of the primary steps in the FRA is to loosely grade the top 1.22 m (4 ft) of soil or substitute root growth medium to avoid excessive compaction. Concern has been expressed by regulators and the public that loose grading on steep slopes (i.e., greater than 23°) could lead to slope failures, which is one of the issues that the Surface Mining Control and Reclamation Act of 1977 (SMCRA) sought to correct. A field investigation was conducted at a steep-slope contour operation in eastern Kentucky to test the applicability of FRA in this setting. Final grading was performed according to FRA standards. The surface consisted of two distinct substitute rooting media. One was unweathered gray sandstone and the other was a combination of gray sandstone, brown sandstone, and topsoil. A variety of native hardwoods was planted following the FRA guidelines. The slope was instrumented with 70 survey monuments to monitor for mass movement. The reclaimed spoil was also characterized using bulk density and penetrometer resistance methods. The characteristics of both spoils are similar to those observed on flat to rolling surfaces where the FRA has been used and, after one year, approximately 70% of the trees had survived. During this first phase of investigation, there has been no major mass movement of the slope and slope stability analysis indicates that the application of the FRA has little impact on the overall stability of the slope. "INTRODUCTION Mine Ventilation has come a long way over the years. The main purpose of mine ventilation is to keep conditions in the mine safe for the worker so that the mine will be productive and profitable. Technological developments to the mine ventilation system itself and the development of mining techniques have helped improve the safety and productivity of the miners and the mine. Most of the developments to mine ventilation and to safety devices came as the result of serious mining disasters. Another effect of those mining disasters, especially in the late 19th century and after, was legislation to set specific safety standards for mines, especially in the underground coal mines. This paper will focus on the connection between mines disasters and the mine safety legislations promulgated because of those disasters. A BRIEF HISTORY OF MINE SAFETY LEGISLATION Mine legislation has a long history in the U.S. dating back well in the 1800’s. However, mine safety legislation is considerably newer. In 1891, the U.S. Congress passed the very first version of mine safety legislations setting up minimum safety standards. By today’s standards, the 1891 legislation was incredibly basic (7). It took a series of horrific mining disasters for Congress to take action a second time. In 1910, Congress established the Bureau of mines. The role of Bureau developed and changed over next 60 years. In 1995, Congress voted to disband it (27). Initially the Bureau was only charged with investigating and coming up with ways to reduce accidents in coal mines. It was not until 1941, when Congress formally allowed Bureau of Mines’ Inspectors into the mines. In 1947, the Congress enacted and adopted a set of federal safety regulations for the inspectors to follow (1). Finally, in 1952 Congress granted the Bureau of Mines the authority to inspect the mines and to issue citations and/or orders of withdrawals. This act was known as the 1952 Coal Act and the penalties were limited to civil penalties. It would take a couple more iterations of mine safety legislation for monetary and criminal penalties to be introduced (10). Up until 1966, mine safety legislation was directed primarily at underground coal mines and surface coal mines. Congress passed the Federal Metal and Nonmetallic Mine Safety Act of 1966, thus providing a set of standards for all mines to follow (1). The 1952 Coal Act was amended again to be the Federal Coal Mine Health and Safety Act of 1969. This amendment was the beginning of modern mine safety legislation (13). In 1973, Congress created what can be considered the first ‘version’ of the modern day MSHA (Mine Safety and Health Administration). The initial version was the Mining Enforcement and Safety Administrations or MESA. Congress created MESA to act as the enforcement agency, relieving the Bureau of Mines of that responsibility so they could focus on their initial purpose, the development of countries natural resources (14). As mining industry grew, mine safety and health became a greater concern and priority. More efforts were put in legislating the industry. The Federal Mine Safety and Health Act of 1977 provided the platform for today’s mine safety and health legislation. Under the Federal Mine Safety and Health Act of 1977, also known as the Mine Act, MSHA came into being (1)."

Traditional cycle theory would postulate that if you have a supply shortage relative to demand, the price will increase because buyers will incrementally increase the price they are willing to pay to ensure that their needs are fulfilled. As prices rise, more supply will be brought on and will eventually exceed demand. This supply excess will cause the price to decline as producers have to lower their prices to sell their goods. What is interesting is that cycles are simply volatility over time. Without the time factor, volatility simply goes up and down. When we add the time element, a cycle is created. This cycle can be big or small. In mining?s case, we are talking about a Super Cycle ? a big cycle. But a Su-per Cycle can be formed in two ways. It can form by either increasing the wavelength or the volatility. If these two elements are combined, a Super Cycle is formed. It is bigger than the normal cycle in terms of time duration and extent of volatility. A recent study (Fig. 1) conducted by Cuddington and Jarrett showed an interesting proof of Super Cycles and suggests that these cycles last between 15 and 30 years. So, what causes a Su-per Cycle? Is it a convergence of unrelated events or a consequence of related events? In a global community everything is related. Equilibrium theory would also take the position that a Super Cycle is a consequence of events.

The Andacollo gold mine is located near the town of Andacollo (population 14,000 people) in Chile’s region four. It is about 56km (35 miles) by paved road from the coastal city of La Serena. Santiago is about480 km (300 miles) south. Site elevation varies between 1,000 and 1,280 m(3,300 and 4,200 ft). The climate at the Andacollo site is moderate but arid. Average annual rainfall is only 130mm (5 in.). Because there is little rain and the temperature varies only a few degrees (14 to 24 C or 57 to 75F), there are no problems with freezing, solution dilution or pad saturation. The climatic conditions are ideal for heap leaching.

The abandoned Belle Eldridge Mine near Deadwood, South Dakota, is on land controlled by the U.S. Bureau of Land Management (BLM) property. The mine produced primarily lead and zinc during the first half of the twentieth century. The BLM has expressed concern about acidic drainage as well as elevated arsenic levels in tailings and waste rock at the site. The mine site was evaluated for remediation in 1998 and 1999. Recent reclamation and remediation work included substantial regrading of waste rock, removal of material where metal loading were of concern, and re-routing of drainage from an adit. Trenching at the site shoed a greater volume than originally calculated during site characterization. Material was removed to a repository that was constructed at a nearby drainage divide. Because of physical hazards, the BLM has decided to remove some historic structures. However, a search of mine records and an inventory of archival materials were complete to preserve the history of the site.

The recovery of manganese from lower grade resources has been studied for a number of years. Of key importance of prior work, was the discovery that higher valent manganese resource can be readily leached with sulfurous acid. This eliminates the need for high temperature reduction roasting that is conventionally used in processing high grade material. High temperature roasting is energy intensive, emits a large amount of carbon dioxide, and would not be economical for processing lower grade material. While the prior work provides important information to extract manganese from lower grade resources, the production of unwanted by-products, and high water and energy usage has limited its commercial use. This paper describes the results from laboratory work that demonstrate solutions to problems, which to this point preclude commercial development. Preliminary mass and energy balances show that a significant opportunity exists in producing manganese with this hydrometallurgical scheme in a low cost, environmentally friendly manner. Emphasis of the work is on low water use, low overall energy use and economic destruction of unwanted by-products.

During the 1980’s, laboratories opened throughout the United States to serve the analysis needs of a rapidly growing asbestos abatement industry. The samples analyzed were either collected during the abatement of known asbestos-containing materials or were suspect asbestos-containing construction materials. Many asbestos analysts received minimal training and used analytical protocols that were industry specific. False positives (reporting a sample as containing asbestos when none is present) went unnoticed or had minor repercussions. The mindset of the industry was to err on the conservative side – it would be better to identify a non-asbestos fiber as asbestos than visa versa. With growing public awareness of the health hazards of asbestos, other materials began to be tested for asbestos content. Rock and ores began to be tested for naturally occurring asbestos by these laboratories using the same protocols or an in-house protocol. Analysts were given no additional training and accreditation, in most cases, it was not required. With no consistency in sample collection, testing protocol or expertise in analysis, significant discrepancies in final results can be expected. If we further complicate this scenario with misidentification of naturally occurring look-a-likes for asbestos, expensive false positives are likely. To avoid the repercussions of misidentification of naturally occurring chrysotile asbestos from look-a-like fibers in rock and ores, correct sampling and analysis protocols with more stringent scientific diagnostic criteria are needed.

Heat and humidity buildup withn refuge alternatives (RAs) may expose occupants to physiological hazards such as heat stress. The Mine Safety and Health Administration (MSHA) regulations require RAs in underground coal mines to provide a lifesustaining environment for miners trapped underground when escape is impossible. RAs are required to sustain life for 96 h while maintaining an apparent temperature (AT) below 95 °F (35 °C). The National Institute for Occupational Safety and Health (NIOSH) tested a 10-person tent-type RA, a 23-person tent-type RA, and a 6-person metal-type RA in its underground coal mine facilities to investigate the thermal environment over a 96-h period. The test results showed that mine air and mine strata temperatures surrounding an RA occupied by simulated miners (SMs) increased over the 96-h test period. The test results suggest that RA manufacturers should consider this increase in temperatures when calculating and evaluating RA components during surface and laboratory tests. The findings can equip stakeholders with additional considerations for calculating the interior heat and humidity temperature profiles for occupied RAs not tested in situ.

New developments in the non-invasive and real-time measurement of the velocity profile of slurry flow in horizontal pipes will be described. This information can be used to determine the approach and onset of solid deposition on the bottom of the pipe. Having this information in real time can enable operation at lower velocities or higher solids concentration or both while avoiding solids deposition or plugging and their associated operational costs. In addition, advances have been made in the measurement and trending of pipewear on slurry lines. In contrast to manual methods, CiDRA?s system uses a permanently or semi-permanently installed ring of conformable ultrasonic transducers clamped onto the outside of the pipe. These transducers are used to measure the thickness of the pipe under their respective locations. This results in better repeatability, accuracy, and failure prediction, along with reduced labor costs. The benefit is significantly improved pipe wear monitoring in pipelines with abrasive solids. This provides an improvement in the ability to insure safe operation and avoidance of costly operational and environmental damage due to leaks caused by pipe wear.

Introduction The term kaolin is derived from kauling, a Chinese word meaning high ridge. This was the name of a hill in China where kaolin was first mined several hundred years ago (Murray, 1976). Other common names used for kaolin include clay and China Clay. Clay technically has a broader meaning and could refer to bentonite and ball clay as well as kaolin. China clay, as a descriptive term, apparently results from kaolin's origin. But it could also be due to a significant historical use in ceramic applications. The terms kaolin and clay will be used interchangeably in this analysis because of the common practice of using these terms in the paper industry. Major reasons for kaolin's wide-spread industrial usages center on kaolinite's physical features of good color, low viscosity, and inertness. Kaolin is described as a hydrous aluminum silicate with an approximate composition of 2H2O • A12O3 . 2SiO2. A new version of the product is calcined clay with modified hydration content. It is gaining widespread use. Components of any particular kaolin, in addition to alumina and silica, are viewed as impurities. The more values deposits and, hence, higher priced kaolin products are those of the highest purity. Kaolin is the most widely used mineral in paper manufacturing throughout the world. Key production centers are located in England and the US. Most of the Tatter's production is located in Georgia. The key markets for Georgia kaolin are paper markets - both paper coating and paper filling. These paper markets extend throughout the world, not just to domestic markets. In fact, export markets are second in size only to domestic paper markets for Georgia's producers of kaolin. Just as the paper markets are important for kaolin producers, kaolin is an important raw material for papermakers. Compared to the other three major pigments - titanium dioxide, calcium carbonate, and talc - kaolin's 1984 tonnage for papermaking represented 85% of the total. Even though titanium dioxide's unit value is much higher, kaolin's estimated total 1984 value still represented 51% of total dollars spent by papermakers for the four pigments. Kaolin grades- are briefly defined using particle size and brightness as principal means of classification. Likewise, grades of papers using kaolin are defined using the American Paper Institute's guidelines. From these definitions, associated kaolin grades used in particular grades of paper are reviewed. Special emphasis is placed on how qualities desired in certain papers are achieved through the use of specialized kaolin grades. A damaged outlet pipe beneath Lenihan Dam has restricted reservoir discharges, severely constraining the owner?s groundwater recharge program. The solution, a new outlet pipeline through the dam?s right abutment, includes an approximately12.2-m-deep (40-ft-deep) shaft and 618.7-m-long, 4.3-m-diameter (2,030-ft long, 14-ft-diameter) tunnel mined in Franciscan melange composed of hard blocks of rock and soft clayey matrix. Alternating road header with drill and blast operations and using steel ribs and shotcrete have proven a flexible combination for handling the variable ground conditions. Excavating the shaft and connecting it to the tunnel require lowering the reservoir for five months, during which time careful construction sequencing and use of a tunnel plug eliminate the risk of an uncontrolled discharge.

The ongoing Overpeck Valley Parallel Sewer Project of Bergen County Utilities Authority (BCUA), New Jersey, is a US $65 million CSO improvement program to meet a State DEP consent order by 2010. One major program component includes a total of 1.8 km (1.2 mile) long, 1,800 mm (72 in) diameter reinforced concrete pipe (RCP) installed by a Herrenknecht AVND1800AB MTBM. The alignment includes drives through 850 m (2,788 ft) of extremely soft glaciolacustrine varved clay, 980 m (3,214 ft) of loose to medium dense mixed soils containing silty sands, gravels, and boulders, and crosses under the New Jersey Turnpike, US Highway Route 46, and a CSX Railroad Intermodal Yard and a branch mainline. Two types of MTBM cutterheads are being used, one configured for the weak varved clay, and one for the mixed soil conditions. The MTBM is also equipped with a double-steering system consisting of a standard front steering joint and a second trailing steering joint to assist grade control should adequate steering not be achieved by the front primary steering in extremely weak soil conditions. This paper discusses the significant engineering aspects and challenges related to site settings, ground characterization, key MTBM features and performance, and required ground treatment.

Introduction Atlas Consolidated Mining and Development Corp. (ACMDC) started gold production at its Masbate gold mine early in 1980. This involved an open-pit mine and a 3.5-k/d (3850-stpd) carbon-in-pulp processing facility (Pizzaro and Antonio, 1981). When this operation started, the mine began stockpiling sub-grade milling material averaging 1 g/t (0.029 oz per st) gold. Furthermore, geologists estimated that there were additional low grade reserves within the ultimate pit limits. These amounted to 2.4 Mt (2.6 million st) with an average grade of 1.1 g/t (0.03 oz per st) gold. The success of low cost heap leaching prompted an investigation to determine if this tech¬nique would be applicable for the low grade Masbate ore. The findings were favorable and a series of laboratory tests were undertaken. These tests indicated the amenability of this subgrade milling ore to heap leaching. Depending on ore size, gold extractions of more than 60% were obtained. To further define operating parameters for a commercial heap leaching plant, a 400-t (440-st) batch pilot plant was built and operated. These test results were used to design the first commercial heap leaching plant in the Philippines. One design concern was the control of solutions during the rainy season. Due to the material's low grade, every effort was made to minimize capital and operating costs. To this end, surplus mining equipment from the copper operations of ACMDC and used machinery in the crushing circuit were considered. Location: The Masbate gold heap leaching operations are located on the northeastern part of Masbate Island, Philippines. This is about 480 km (300 miles) south-east of Manila. Climate: The climate is moderate with two distinct periods. The dry season runs from January through June, and the rainy season extends from July through December. During the wet season, the rainfall at the mine site averages 300 mm/m (11.8 in. per month) for a total of about 2000 mm/year (78 in. per year). However, heavy downpours in excess of 100 mm/h (4 in. per hour) can occur. Geology: Four distinct rock types have been identified in the pit area. They are tuffaceous agglomerates, andesitic agglomerates, metasedimentary rocks, and hornblende augite andesitic porphyry. The tuffaceous agglomerate occupies the northern and western flanks of the mine. Andesitic agglomerates are localized distinctly in the central portion of the volcanic plug within the mine site. The eastern boundary of the mine is occupied by metasedimentary rocks, whereas, the hornblende augite andesitic porphyry occurs as an intrusive within the andesitic agglomerate and tuffaceous agglomerate. Some areas of the pit contain high proportions of clay minerals, requiring an agglomeration step in the flowsheet. This ensures satisfactory percolation through the heaps. INTRODUCTION Two promising innovative tunnel support systems are being developed by the Civil Engineering Department of the University of Illinois. One of these is a lining which can be placed by a slipform located immediately behind and contiguous with a tunneling machine. Recent advancements in construction materials appear to provide the breakthrough required to make such a system feasible and practical. The lining material which permits this construction scheme is wire fiber reinforced concrete made with high, very early strength regulated-set cement. Parts I and II of this paper relate to this slipformed liner. Part I discusses the material and equipment required to install the liner. The "Extruded Liner System" (ELS) has been devised to place the lining. This ELS concept is briefly described and a tentative development program is outlined. In Part II, the physical properties and practical construction aspects of the liner material are discussed and a program for structural testing of 10-ft diameter prototype tunnel supports is presented. Part III contains descriptions of improvements to conventional steel-rib tunnel supports. It is shown that square tubular box sections are structurally superior to wide flange sections for complex loading conditions often encountered in rock tunnels. Analytical studies and tests on connections have been performed to evaluate their structural behavior.
