Fatality prevention and zero harm are key focus areas for resource companies. However, they remain an ongoing challenge for high hazard industries, including the mining sector. As one example the latest International Council on Mining and Metals (ICMM) Safety Performance Report shows that, while there is generally a declining fatality trend since the organisation commenced publishing data in 2012, there were still 33 fatalities across member companies in 2022 (ICMM, 2023b). Every fatal incident brings significant pain and suffering to the colleagues, friends and family of those involved, however the failures and causal factors leading to each incident are frequently very familiar to people working in the industry, particularly those tasked with managing risk and safety. In the words of Dame Judith Hackitt, Grenfell Tower Fire Investigator, ‘there are no new accidents – just different people making the same mistakes because they haven’t learned’ (Hackitt, 2022). The recurring themes in terms of the nature of fatal hazards and their associated causes across the mining sector show that, despite improvements evidenced by the favourable declining fatality trends over the long-term (in general and outside of catastrophic multiple fatality events), changes are required to achieve fatality elimination and the aspiration of zero harm. This extended abstract and the accompanying presentation apply a mining frame of reference to the idea that there are no new accidents, examining how this applies within the industry and why the lessons from past failures are not always well understood or applied to prevent the reoccurrence of material unwanted events. Many contemporary mining and resources organisations have extensive internal incident history data and a wide variety of information on fatality incidents is publicly available, including the annual ICMM Safety Performance Report. This extended abstract and the accompanying presentation explore how this existing data can be used effectively to learn from the past and proposes practical ideas regarding strategies for successful knowledge transfer to the workforce as a key component of preventing history repeating and improving safety performance across the industry. As noted by ICMM, ‘sharing lessons from failure is vital to improving health and safety, but it’s not enough’ (ICMM, 2023a).

This paper reviews the various processes successfully in operation to treat refractory gold ores and describes in detail the alkaline pressure oxidation circuit operating at the Barrick Mercur Mine, Utah, U.S.A., on car- bonaceous/sulphide refractory ore and the acidic pressure oxidation circuit operating at the Barrick Goldstrike Mine, near Carlin, Nevada, U.S.A., on sulphide refractory ore. The autoclave process is important to Barrick for the extraction of gold. The Company has operated successfully a 750 ton per day unit at its Mercur property for the past two years and more recently has commissioned a 1500 ton per day unit at Goldstrike. The paper presents data relative to operating costs, materials of construc- tion for autoclaves and peripheral equipment, reagent requirements, and changes required during commissioning for both autoclave systems.

Organic complexing agents have received special attention over the past few decades in the search for reagents for improved flotation separation of minerals, especially non-sulphides. The majority of such reagents that have been proposed over the past few decades have remained largely a laboratory curiosity, except perhaps alkyl hydroxamates, because their selection is seldom based on commercial viability. In this regard, the choice of complexing agents such as oximes may be more realistic because of their widespread use in solvent extraction of copper. Our earlier studies have confirmed that they are also effective as collectors for copper oxide minerals, including chrysocolla which is the most difficult copper mineral to beneficiate. The flotation behaviour of chrysocolla for several structurally-related hydroxyoximes were studied as a function of pH and oxime concentration and the role of structural features examined. The structural changes studied included substitutions in both chelating and non-chelating parts of the molecule. The results indicated that, contrary to expectations, many of the structural changes in the basic molecule, salicylaldoxime, do not have a positive effect on collector efficiency. The reduction in collector efficiency is rationalised on the basis of possible steric factors affecting complexation and packing of the collector species on the mineral surface as well as effects due to changes in electron density brought about by substituents. Introduction of alkyl substituents on the oximic carbon of salicylaldoxime yielded the highest collector efficiency.

With the proliferation of new sensor technologies, acquiring multiple data sets over the same ground is becoming cheaper and easier than ever. This new, higher resolution multivariate data provides a significant resource for exploration and resource geologists but comes with the added complexity of effectively integrating the various data sets in useful and meaningful ways to elucidate new geological understanding. Both geophysical data sets and hyperspectral data are extensively used for exploration targeting, providing different information at different resolutions and crustal scales. One of the biggest challenges comes from trying to effectively integrate data sets that record very different physical properties, across the different scales these data are captured at, in a way which can allow for a data-driven analysis of these combined data sets.

Eutrophication of water bodies is one of the most severe environmental problems worldwide. It is well known that the availability of phosphate, as the limiting nutrient, is crucial for increased primary production and eutrophication. Calcite exhibits positive surface charge in most natural waters and can therefore be used for phosphate immobilisation. In this study, sorption experiments were performed in order to test different calcite materials regarding their suitability for use in restoration of eutrophic lakes. These short-term experiments were performed under different hydrochemical conditions (pH, SICC, initial dissolved P concentration) and with varying calcite properties (specific surface area and amount) in order to predict the long-term stability of the Ca-P-bonds. Peculiar shapes of isotherms that could not be explained only by adsorption processes were detected in the individual experiments. Depending on the residual P-concentration in solution, the isotherms can be divided into three sections indicating different processes responsible for the P-fixation on calcite surfaces. Adsorption prevails in equilibrium with lowest residual P-concentrations. The inflection of the isotherm at moderate residual P-concentrations suggests that precipitation and transformation of a Ca-P compound becomes dominant. This is supported by characteristic Ca/P-ratios for different Ca-P-compounds (b-tri- calcium-phosphate, octa-calcium-phosphate). On the new surface, phosphate ions again are fixed by adsorption. The observed transformation of P-bonding is crucial for sustainable phosphate immobilisation on calcite surfaces. While pure adsorption processes are reversible, the development of a thermodynamically stable Ca-P-phase, like hydroxylapatite, will lead to a lasting immobilisation of phosphate.

The Mt Whaleback iron ore mine is located adjacent to Newman, approximately 1200 km north of Perth, in the Pilbara District of Western Australia. Approximately 15 per cent of the waste materials are in the zones with high concentrations of sulfide and carbonaceous minerals and are referred to as æreactive shaleÆ. The reactive shale has the potential to undergo self-heating and can react with the ammonium nitrate (AN) based explosives used in mining operations, resulting in spontaneous combustion and spontaneous explosion. The sulfide-rich shales also have the potential to produce acid rock drainage (ARD). In the present study, over 200 shale samples have been collected from different locations in the Mt Whaleback iron ore mine, including blasthole drill cuttings, fresh mine faces and waste dumps. These samples have been systematically characterised for their geochemical properties. Based on the characterisation results, a pH test was developed as a cost-effective, fast and efficient tool for the assessment of reactive ground in the mining industry. This paper summarises the results of the sample characterisations and discusses the key findings arising from the interpretation of results. These results have significant implications in the safe storage of the reactive shales and the prevention of spontaneous explosion, spontaneous combustion and ARD.

INTRODUCTIONDuring the past few years there has been considerable interest in developing the so-called direct reduction processes for treating iron oxides. The various methods proposed were recently reviewed by Scortecci and Palazzi (1960).In particular the application of fluidized bed techniques received considerable attention and, on first examination, this seems to be the ideal method 'of dealing with fine iron oxides. Several difficulties (e.g. product agglomeration) require the method to be investigated further before its potentialities can be fully assessed.PROPOSED FLUIDIZED BED REDUCTION PROCESSESBrassert and De Jahn (1951) were among the earliest to suggest the use of fluidized beds for the reduction of iron ore to metallic iron. The Toguraci low sulfidation epithermal gold vein deposits are located 2 km WSW from the Gosowong mine and form part of the Gosowong Goldfield. The goldfield lies within a Neogene magmatic arc on Halmahera Island, North Maluku Province, East Indonesia. The terrain is moderate to steep, rainforested, and poorly outcropping. Exploration and mining operations are being carried out within the PT Nusa Halmahera Minerals Contract of Work (exploration tenement) jointly owned by Newcrest Mining Ltd (82.5 per cent, operator) and PT Aneka Tambang Tbk. (17.5 per cent). First pass exploration of the Gosowong area began late 1992 resulting in the discovery of weakly developed porphyry copper mineralisation (named Bora) at Toguraci in 1993. Early in the evaluation of this porphyry an overprinting epithermal Au/Ag mineralisation event was recognised, however exploration focus remained on the Bora porphyry deposit for some time. The discovery of the nearby Gosowong epithermal gold deposit and the exploration of other prospects in the area further diverted attention and funds from Toguraci. It was not until year 2000 that Toguraci was seriously reevaluated as part of a detailed appraisal of the epithermal gold potential within a 5 km radius of the Gosowong gold mine. A fly-camp based drilling campaign using man-portable rigs was undertaken between August 2000 and March 2002. All exploration at Toguraci was then stopped pending government approvals to mine. These were finally issued in May 2003 and immediately drilling restarted and pre-mine approvals (eg the environmental impact study) were sought and granted. The first Toguraci gold bar was poured in February 2004. Exploration at Toguraci and elsewhere in the Gosowong Goldfield continues today.

As a result of commissioning No.5 B F, 2670m3 inner volume, in March 1972, the largest B F in Australia; new expertise in maintenance, furnace operating practices and design have been obtained. This experience has also been extended to the other furnaces. A series of operating problems were con- fronted; all parts of the furnace, including the hearth side walls and bottom,were adversely affected. In December 1977, serious hearth wall wear was detected at No.5 B F, and a potential hearth breakout was averted. Hearth wear in No.4 B F was discovered in March 1979, and emer- gency underhearth cooling was installed to reverse an extremely advanced hearth bottom erosion. Hearth behaviour of No.5 B F oampaign 1972-1978 and`No.4 B F campaign commencing 1972, has been studied and discussions on the causes, effects and measures undertaken to protect the hearths are presented in this paper.

The Fiji Islands are remnants of a once continuous Solomon-Vanuatu-Fiji-Tonga volcanic arc (the Vitiaz arc), located at the boundary between the Indian-Australian and Pacific plates. They record a history of (i) arc volcanism the late Eocene to late Miocene, (ii) an extensive magmatic response to arc rifting (5.5-3.OMa) during which volcanism was dominated by shoshonitic to tholeiitic basalts, and as rifting continued, (iii) a transition from arc to intraplate volcanism. The Hunter Ridge-Hunter Fracture Zone separates the inactive South Fiji Basin from the actively spreading North Fiji Basin, and has been considered to be a transform zone, almost parallel to the motion vector of the Australian plate. However rocks dredged during the 1993 `Alize' cruise, from along the northern part of the Hunter Ridge are typical arc tholeiites, and therefore demand that subduction has occurred beneath the Hunter Ridge sometime during the last û 5Ma.

Changes in the stress field around faults due to the excavation of underground space or the extraction of underground resources induce fault reactivation. To understand the mechanism of fault reactivation, it is important to observe the deformation and sliding processes of the asperities, which is regionally contacted areas at the fault planes. In the laboratory, triaxial compression tests are commonly conducted to reproduce the stress conditions around faults and study their mechanical behaviours. The conventional measurement devices for triaxial compression testing (eg load cells, local deformation transducers, and piezoelectric sensors) uses sensors to measure changes in the physical properties of rocks, but the direct visual information for the sample condition inside the pressure vessel is limited and can only be estimated from the sample after the experiment. We recently developed a direct visual measurement system to observe the slip behaviour of rocks from the inside of the pressure vessel of a triaxial test apparatus. This paper introduces this visual measurement system and presents some preliminary experimental results. The visual measurement system consists of a steel housing with a built-in commercially available digital camera. The triaxial compression tests were conducted to visually observe the sliding behaviour of the pre-cut specimens and to measure its behaviour through the digital image correlation (DIC) method. The captured images and the DIC results show the displacement field of the specimens for different confining pressure. We verified the sliding behaviour and the failure at/around the triangular asperity under at different confining pressure.

The routine use of gravity gradiometry for the detection of orebodies with no outcrop is now being undertaken. The commodities and orebody styles range from banded iron ore formations to volcanogenic massive sulfide (VMS) style polymetallic deposits. This recent innovation of flying gravity gradiometry was pioneered by BHP Billiton and Lockheed Martin with the Falcon technology (Lee, 2001), and now two additional systems, also using Lockheed Martin instruments, are deployed. In South Africa, magnetic gradiometry is being developed and deployed with the same objectives in mind. In this case, a system from Institute of Photonic Technology (IPHT) is being used. The detectable wavelengths from such systems have decreased to less than 200 m in the last year, and it is expected in 2011 that wavelengths to less than 100 m, with anomaly signal strengths of less than 2 Eotvos for gravity gradiometry will be viable. The key to these advances lies in the improvements of processing software and geological modelling. In particular, methods using all the tensor components are starting to be routinely deployed to filter, level and grid the observed signal. These methods do, however, require full insight and understanding of the reference coordinate systems used by the various instrument designers and survey operators. The digital terrain models need also to be observed to an accuracy of better than 20 m cell sizes, using such techniques as Lidar. Then the terrain effects can be modelled. The topographic surface represents the largest and most proximal density contrast encountered in an airborne survey. Hence terrain effects can have significant impact on airborne gravity gradiometry data. A decorrelation of the terrain effect with the observed signal requires a method of best fitting with a regional surface density. A novel technique has been developed for this purpose. This technique uses a tensor algebra to estimate a density between 1 - 4 g/cc that explains the average observed signal. Armed with this result, the anomalous buried bodies can be revealed by subtraction.

The aim of this paper is to present an innovative method to help delineate potential target volumes for haematite-goethite mineralisation as opposed to more magnetic ores. This is achieved through interpretation and modelling (2.5D and 3D inversions) of ground magnetics and gravity surveys over the Koolanooka South prospect in Western Australia. The targets are assumed to be dense and weakly magnetic and hosted within banded iron formation (BIF) horizons. Geophysical interpretation (2D map) was performed using a variety of processed magnetic and gravity images (Betts et al, 2007) to determine the distribution of lithologies, to understand the structural architecture and to map out any anomalous areas of excess density (possibly indicating goethite alteration). The created geological map is used to build geological cross-sections (geometrically consistent) that were modelled (in 2.5 D) against both potential field datasets. The crosssections are consequently used as the framework to build a 3D geological model. The model defines 3D volumes of rocks including several BIF horizons. The distribution of densities and magnetic susceptibilities of the BIFs is inverted within the GOCAD«-VPmg environment to reduce the misfit between the response of the model and the observed gravity and magnetics. By combining the results of both petrophysical property distribution it is possible to map out (using classical GIS type queries in 3D) volumes of higher density and lower magnetic susceptibility.

This paper is based on studies made intermittently from 1937 to 1950. Its object is to describe the surface geology and the tin lodes, and to relate the lodes to the structure of the field.PREVIOUS REFERENCESR L. Jack made his first report on the field in 1880, but his principal report was made in 1883. W. E. Cameron described the Great Northern lodes in 1904. Numerous later reports on various mines were made by L. C. Ball, J. H. Reid and C. C. Morton, the most important of which were on the Black King, Good Friday, St. Patrick and the Great Northern Gully Shaft.GENERAL GEOLOGYJensen maps the Herberton area as part of a granite massif intruding rocks of two ages, the Younger Metamorphics of Silurian-Devonian age, and the Older Metamorphics of Older Palaeozoic age. The distinction is apparently based on lithological data. The Older Metamorphic rocks occur as inliers in the granite massif to the west and east of Herberton (Fig. 1).The area described consists of granite traversed by numerous dykes. West of the area, approaching the Older Metamorphic rocks, the granite becomes finer grained as if chilled towards its contact, and the dykes more numerous. To the east, passing from the Nova Scotia mine to Mount Lavery there are numerous dyke rocks, but the outcrops are poor and knowledge of the geology indefinite. The area is the main tin-producing area. To the west there are comparatively small tin lodes in the granite, and the "Copper Firing Line" occurs in the Older Metamorphic...

Before proceeding I wish to say that the excellent diagrams by which my paper is illustrated are the work of Mr. C. H. Barnes, Constructing Engineer of the Kalgurli Gold Mines Limited, and I am also much indebted to Mr. F. A. Moss, manager of the mine and for the privilege piIling information not previously made public.The limited time at my disposal has coolled to, curtail the scope of my paper to a few practices in ore treatment which are being worked on the Kalgurli Mine, but which I believe are of more than ordinary interest to thp~e Goncerned in modern treatment of gold ores. As an introduction to my subject it is, necessary that I should give a brief outline of the process by whiGh the gold is won from the complex sulpho-telluride ore of the Kalguili mine.My business is not to enter into any discussion as to the relative merits of the two rival sulpho-telluride ore treatment processes at work on the Kalgoorlie field, viz, the treatment of the unroasted ore after shining by solutions of bromocyanide, as against the treatment of the ore-after roasting by solutions of potassium or sodium cyanide either in filter presses or percolating sand vats. Even if reliable costs and percentages of extractions were obtainable in each case, it would not constitute an infallible basis of comparison, as the larger plants would necessarily have an advantage in cost per ton as against the small plants, and the other conditions obtaining are in many Cases dissimilar.

A flow sheet has been developed to treat a high tonnage highly preg-robbing gold ore. The ore is low grade and fine grained with a high proportion of very fi ne free gold. The key components of the flow sheet include gravity, flotation, intensive cyanidation with direct electrowinning, and carbon-in-leach of the flotation concentrate. The circuit produces a low overall mass pull to concentrate allowing economic intensive treatment of large tonnage low-grade material. Efficient recovery of fine gravity gold using a Falcon concentrator in both the milling and flotation circuits, and subsequent intensive cyanidation in a Gekko InLine Leach Reactor (ILR) is critical to the process with over 70 per cent gold recovery expected in this circuit. Grind size control was also found to be important in controlling the activity of the preg-robbers in the carbon in leach circuit and also in the solid liquid separation performance. Tests at the large-scale demonstration plant have been conducted to demonstrate the effectiveness of this flow sheet. A large low-grade deposit is currently under development with the aim of developing a 10 Mt/a treatment plant with the subsequent addition of equivalent sized process lines in multiples of 10 Mt/a. The economic development of this orebody is critically dependant on understanding and exploiting the properties of the ore. In particular it is critical to minimise the effect of preg-robbers by recovering free gold with Falcon concentrators and InLine Leach Reactors. Recovery of the remaining gold by flotation into a small mass yield allows this fraction of gold to be treated by CIL leaching utilising high carbon levels. It is critical is to ensure operation at the optimal grind size to maximise liberation of the free gold and minimise pregrobber activity. The ore has been identified as preg-robbing with low recoveries obtained if treated using conventional cyanidation and carbon absorption. The mineralogy consists of predominantly fi ne grained free gold with minor sulfides including pyrite and arsenopyrite. With conventional cyanide leaching in the absence of carbon, gold recovery is less than 50 per cent. Direct cyanide leaching of the ore in the presence of activated carbon results in an improved but still uneconomic gold recovery. The gravity recoverable gold (GRG - as defined by Andre La Plante) content of the ore has been measured to be 70 per cent to 85 per cent in various tests with liberation grind size of approximately 106 ¦m. This indicates gold is predominantly free and suitable for recovery by batch centrifugal concentrators into a low mass yield. This property of the ore presents the opportunity to maximise the recovery of gold by separating it from the majority of the preg-robbers prior to leaching. Due to the very fine gold particle sizes and the necessity to maximise recovery of this fine fraction Falcon concentrators were selected for the duty, with their proven high recoveries at very fine gold sizes. The gravity recovery in the milling circuit is designed to be in the range 55 to 65 per cent. This has been achieved in the existing pilot plant using a Falcon operating on ball mill discharge within the mill recirculating load. Intensive leaching in an InLine Leach Reactor is essential to recover the gold from the gravity concentrates. Tabling is not a preferred option since recovery is lower than intensive cyanidation and any gold losses to table tails will report to CIL where recovery is reduced by the presence of preg-robbers. Furthermore, tabling recovery from the gravity concentrates is particularly poor due to the fine particle size of the free gold. ILR recoveries of 98 per cent were demonstrated in the pilot plant using an ILR100BA and leach conditions were investigated in the lab using pilot plant generated Falcon concentrate. Recovery of gold from the pregnant solution was achieved with direct electrowinning at pilot and efficiency and sizing parameters were determined at lab scale. To further increase free gold recovery Falcon concentrators will be installed in the flotation circuit. Location of the Falcons in the circuit is not finalised but installation on the rougher concentrate stream is favoured since it will allow multiple pass recovery. This is expected to recovery a further ten to 20 per cent of the gold, as indicated by lab tests on pilot plant generated material. The gravity concentrate from the flotation circuit will also be leached using InLine Leach Reactors. Initial intensive cyanidation tests carried out on lab generated flotation concentrates indicated higher recoveries than CIL can be obtained even though the concentrates contain significant levels of preg-robbing material. This can be explained by the high cyanide, high pH leaching conditions used in the ILR which alters the absorption equilibrium of the preg-robbers. Further optimisation of the leach conditions will be undertaken once more concentrate becomes available. Recovery of the remaining gold content is achieved by flotation into a small mass yield, however the preg-robbers are also recovered into this concentrate. Due to the presence of the preg-robbers gold leaching must be carried out in the presence of high levels of activated carbon in a CIL system. The recovery has been shown to be strongly dependant on the carbon loading and the concentrate particle size. The carbon competes against the preg-robbers for dissolved gold while excessive grinding increases the surface area and activity of the preg-robbers. This has been demonstrated in test work however the economic optimum leaching conditions and carbon loading have yet to be determined. The grind size is critical to the economics of the project, not only due to its effect on gold recovery but also due to its effect on the major operating and capital costs at both the front and back end of the process. It is critical to grind enough to obtain both gravity free gold and flotation liberation at low mass yields however over-grinding results in large increases in mill size and power at the front end and filtration or thickening costs at the tails. Because these unit operations treat essentially all the plant feed even small changes can result in major impacts on project economics. The viability of this project is greatly enhanced by the high recovery of gold into a small mass yield. In particular the production and intensive leaching of the free gold gravity concentrates will allow over 70 per cent of the gold to be recovered efficiently with minimal impact from preg-robbers. By using flotation the mass of the remaining fine preg-robber contaminated material, to be leached in CIL, is greatly reduced allowing this difficult material to be economically treated. Zinc and lead sulphide flotation con centrates containing from 0.02 to 0.20% Cd are roasted on an updraught sinter machine at the Cockle Creek Works of Sulphide Corp- oration Pty. Limited to provide sinter for simultaneous smelting of zinc and lead in an Imperial Smelting Furnace (I.S.F.). The major portion (about 85%) of cad- mium in concentrate feed to the Sinter Plant is eliminated with sinter fume and is rec- overed by weak acid leaching in the Cadmium Plant'followed by counter-current decantat- ion, ion exchange concentration, precipitat- ion of cadmium sponge and partial refining to caustic refined cadmium metal. Cadmium not eliminated during sintering is volatilised in the I.S.F. and subsequently recovered as zinc alloy by distillation in the zinc ref- ining columns of the Zinc Refluxer. The partially refined cadmium metal and cadmium- zinc alloy are blended as feed to the cadm- ium refining column where separation and recovery of cadmium is effected. Condensate from the column is further refined under caustic soda flux and cast into anode shapes. Production is 200 t.p.a. at 85% recovery of cadmium in concentrates.
