Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

It is estimated that substandard ground support installations and elements cost the Rio Tinto Copper Group millions of dollars between 2000 and 2014. These issues have also created significant safety and financial risks in Rio Tinto mines. As a result, Rio Tinto Copper has developed CaveCAD, an integrated system to collect, store and analyse all ground support-related data. The CaveCAD system stores geotechnical data and has the ability to automatically import data, collect data through tablet applications, report processed/analysed data and present this data through 3D visualisation software.The CaveCAD Ground Support module closes the design–implementation–verification loop by providing a method of verifying the compliance of all installed ground support, recording manufacturer specifications, recording quality control test compliance and rectifying any non-conformant areas. The module uses innovative technology to create value through cost saving, safety improvements and data collection efficiency. Cost savings will be achieved by a decreased need for excavation rehabilitation, less ‘waste’ due to faulty installation and quality assurance checks reducing the likelihood of substandard support units being used. Centennial Coal operates the Springvale and Angus Place longwall mining operations that extract coal beneath the Newnes Plateau in the Western Coalfields of New South Wales. Springvale and Angus Place mines have a history of difficult geotechnical conditions, where mining operations are conducted with a thick coal roof interbedded with numerous claystone units and relatively high levels of vertical and horizontal stress. The combination of a relatively high stress regime and weak laminated roof has led to numerous roof failures throughout the mine’s life.Following underground roadway development, the mine roadways can experience high levels of roof movement that can lead to roof falls. Two-point wire extensometers (known as ‘telltales’) are extensively used for roadway condition monitoring. Along with extensive other geological and geotechnical data, monitoring data is used to prepare geotechnical hazard plans that are then used in all areas of mine planning to allow for safe and efficient operation of the mine.This paper presents a case study of the use of a new software tool that allows time-scaled visualisation of monitoring data in the context of longwall and development face positions and installed strata support. It can be used in the identification of geotechnical risk factors and their interaction with the mining process to allow a more robust geotechnical hazard process. Following a detailed assessment of concen trator performance at Broken Hill South Limited which was reported in a previous paper, the fine grinding section was changed from a parallel arrangement of the two grinding circuits to a series arrangement, and the concentrator per- formance re-assessed. The change to series grinding had virtually no effect on the actual or potential capacity of the circuit to make a final product with 80% passing 100 mesh. However, the fresh flotation feed contained significantly less +150 mesh lead and +65 mesh zinc (material which was shown to float poorly), without an increase in the quantity of poor-floating ultra-fine lead and zinc. From the size-by-size flotation recovery data, it was shown that overall recoveries of both lead and zinc were approximately 1% higher than they would have been without this change in size distribution and it was concluded that this was a direct result of the change in grind- ing procedure.

Air blasts or wind blasts as they are termed in underground coal mining, occur in a number of types of underground mining where a void develops and the overlying or adjacent rock fails rapidly to allow the development of a high pressure air pulse. The effects of air blast vary widely from very low consequence to high consequence events causing multiple fatalities as occurred at North Parkes mine. The aim of this paper is to examine the phenomena of air blasts and the underlying parameters which determine the severity of the air blast event. A basic model under development is then detailed. The aim of the model is to determine the magnitude of the pressure pulse(s) developed during an air blast event. This, combined with a transient ventilation network analysis model will enable the airflows and associated pressure drops to be determined and allow the pressure pulse around the mine to be evaluated. The overall aim is to allow mining engineers and mine ventilation engineers to undertake a risk assessment of air blasts at individual mine sites, allowing working parameters to be changed to eliminate or reduce air blasts, or engineering controls to be developed to reduce the effects of air blasts.CITATION:Tuck, M A, 2013. Developed and built in 1982 for simultaneous bathy-metric and reflectivity mapping of the deep ocean by the Hawaii Institute of Geophysics, SeaMARC II was a breakthrough in sea-floor mapping technology. The design was developed cooperatively by industry and academia, and the system has served as a prototype for a new generation of mapping tools for operation in both shallow and deep water.Research programs using the SeaMARC II system have focussed on basic research into the geophysical evo-lution of the Pacific, ranging from intra-atoll mapping to surveys of the deep-sea trenches. During the past few years most work has been concentrated on detailed investigations of the morphology of mid-ocean spread-ing centers, seamounts, and the subduction zones which surround the Pacific. The detail provided by the high resolution, 10-km wide side-scan sonar images of fault and fold structures, combined with the swath of bathy-metric information obtained simultaneously, allow previ-ously unattainable analysis of seafloor morphology and are leading to development of detailed models for ocean crust genesis and deformation.

The concept of an in stream analysis probe was developed at The Zinc Corporation (now Z.C. Mines) in the late 1960's and commercialised quite rapidly, by Phillips and then AMDEL, as an alternative to the established centralised systems (Courier, etc.). The developments which have taken place since that time are reviewed and current procedures and performance are described. An audit of the system, recently undertaken by AMDEL, has highlighted areas where improvements can be made. In 1988 the ISA system will become an integral part of a substantially upgraded process control system.

Three soil cover test areas were constructed on waste rock backfilled into the Lichtenberg Pit, near Ronneburg Germany. A fourth, uncovered area was included as a control. Each test area measures 50 m by 65 m, and was constructed using full-scale equipment and construction practices. Extensive material sampling and testing was included as part of the construction. Devices were installed in each plot to monitor meteorology, infiltration, run-off, interflow, soil moisture, soil temperature, and soil pore gases. Boreholes were completed in each area and instrumented for temperature, pressure and pore gas sampling to a depth of approximately 60 m. Data from the monitoring instruments have been collected since July 2000, and have been comprehensively analysed and modelled on two occasions. The paper presents an overview of the test area design and instrumentation and the results of the two comprehensive data reviews. The reliability of the various monitoring methods is discussed and experience with several innovative methods is reviewed. The difficulties encountered in modelling the data set using common soil physical models are discussed.

Middle and Late Cenozoic fluvial conglomerates and gravel in northeastern Southland record the transport of alluvial gold from Central Otago into Southland, and subsequent reworking and concentration of this gold into economic placer deposits. A corridor of geochemically distinctive, Hg-free gold derived ultimately from veins in Torlesse Terrane schist in Otago extends through the Caples Terrane and into Southland. The initial repository in Southland was fluvial sediment of the East Southland Group, specifically weakly auriferous, quartzose delta plain strata of the Gore Lignite Measures (GLM), a downstream equivalent of Dunstan Formation in Central Otago. Miocene uplift along the northwest trending Old Man Range beheaded the parent river system, isolating GLM from its Central Otago source area and establishing the proto-Waikaia River system. Rising topography between Otago and Southland increased fluvial gradients, and initiated erosion of cover strata and a sequence of tectonically induced gold recycling events. Auriferous GLM were recycled from the hinterland into broad braided fluvial channel and floodplain deposits of the Waikaia Group (new), specifically the highly auriferous Waikaka Quartz Gravels and equivalent Waimumu Quartz Gravels downstream (WQG), superimposing more proximal depositional environments over parts of the former GLM delta plain. WQG were deformed, and locally cannibalised during the later stages of their deposition by reverse movement on north-northeast trending faults of the Dunsdale System (NW side up). Increased tempo of uplift in the hinterland (Old Man, Garvie, Umbrella, and Blue Mountain Ranges) in the Pliocene and early Quaternary resulted in further reworking of GLM and WkQG from the hinterland as well as basement erosion. Progressively more basement-rich detritus was deposited as a locally auriferous piedmont (Gore Piedmont Gravels - GPG) over earlier channel and delta plain (GLM, WQG) strata, extending from the Blue Mountains to Waikaia, and southward through the Gore Gap to beyond Mataura. Southward propagation of the north-northeast trending ranges, local warping, and uplift of the Murihiku Terrane in Pliocene and Quaternary progressively deflected GPG fluvial systems and ultimately constrained them to the present valleys. Pre-existing auriferous strata (GLM, WQG, GPG) were reworked and deposited with or without basement detritus as these fluvial systems migrated toward their present positions. Auriferous remnants of these transitional deposits are preserved in local structural depressions on the uplifted blocks that deflected the systems (eg King Solomon Mine), and on locally abandoned floodplain segments on the lowlands (Pebbly Hills-Gore Gap). The present drainage has continued to incise and cannibalise its own and earlier deposits during late Pleistocene-Recent uplift to form confined channel placers within the present valleys.

Gold deposits in Eastern China have characteristic of belted distribution. The concentrating belts of gold mineralization are about parallel to the island are belt in Western Pacific Ocean. Gold deposits are mainly concentrated in the aged metamorphic rock series on the upwarping zones and upwarping fault zones, and obviously controlled by the deep fractures and their deriva-tive secondary faults. Gold mineralization had genetic relation to the Mesozoic (especially Yanshan period) magmatic activity. The distribution and genesis of gold deposits can be explained by the assumption of wavy structure. The formation of wavy structure is related to the process of Pacific plate and India plate.

Due to the perceived potential to extract the second economic manganese seam at its Black Rock Operations in the Northern Cape Province, South Africa, Assmang commissioned an investigation with respect to mining method and sequencing of the two orebodies in 2007. Based on this initial study into multiseam potential, conditions and impacts, the rock engineering aspect was expanded to determine conceptual rules to govern the extraction of both manganese seams using the normal bord-and-pillar layout implemented at this operation. This work included the theoretical analysis using basic pillar design practices and numerical modelling, including the use of Map3D, UDEC and Phase2 codes.In 2013, Assmang launched a large-scale investigation into the potential to increase mine output from all possible manganese sources at the operation. Included in this investigation was the requirement to confirm or modify the conceptual multiseam extraction rules to implementation. As part of this confirmation process, stress measurements, borehole camera audits and additional numerical modelling were performed at a particular site on the operation where a limited extraction of both seams had been undertaken. "The requirement for environmental approvals and associated environmental impact assessments (EIAs) is now commonly regarded as a key milestone and body of work during project development. EIAs can be undertaken at various phases during project development, generally either:\ during the scoping or prefeasibility studies, where limited project information is available and the mitigation and management measures of the EIA become a set of criteria and commitments that control project design during the feasibility study, as an iterative approach whereby mitigation and management measures are developed through an integrated process with the engineering team, and measures are included in the feasibility study cost model following the feasibility study, where detailed project design information is available, the cost model is complete, and the EIA is an assessment of a fixed project with limited opportunities for mitigation and management measures to be integrated into engineering design. This paper investigates the advantages of integrating EIA into life-of-mine planning during the feasibility study phase, using the Tampakan Copper-Gold Project (the Project) as a case study. The Tampakan Project is located in the Philippines on the southern island of Mindanao. The Project involves the proposed development of three main areas of infrastructure:a large-scale, open pit copper-gold mine and associated mine site infrastructure (including tailings and mine waste storages, fresh water dam, concentrator, etc) a power station, port and filter plant (PPFP), located approximately 60 km from the mine a pipeline and transmission line linking the mine and PPFP sites.Examples of engineering measures developed through an iterative process to mitigate and manage predicted environmental and social impacts include design of water management infrastructure, modification of the disturbance footprint (eg roads realigned to avoid critical habitat) and realignment of project infrastructure to minimise impacted water catchments.CITATION:Blunt, J and Jones, J, 2012. The key to the successful debt funding of a new mining project, whether in economically established or developing nations, is the submission of a comprehensive feasibility study. The feasibility study forms the basis from which the resource banker can construct financial models which govern the amount of available funds, the conditions which control the supply of such funds and the time period over which the loan may be repaid. The bank will wish to ensure that the feasibility study is the best representation and confirmation of reality possible because this model is the basis of the loan security package. Thus the study will be vigorously interrogated by independent experts in all its elements including the ore reserve, mineability, metallurgy, operating and capital cost profile, environmental factors and management.

Selective mining played a major role in saving a mine and a communityÆs existence as well as creating a working culture that is the model for all KumbaÆs mines. The successes that resulted from selective mining led to receiving KumbaÆs highest award (it was the Gold Medal Breakthrough project of 2003) as well as the productivity award at the National Productivity Institute award ceremony in 2004. Kumba was established in November 2001 (previously Iscor Mining). The new company identified Mineral Resource Management (MRM) as one of its key drivers and an initiative was launched to define MRM in the Kumba way. Best MRM practices at the Kumba mines were included in the definition. Selective mining was born out of this initiative. One of the key principles of good MRM practice was identified as: æThere must be a balance in the operational processÆ. This principle was applied with great success at Thabazimbi Iron Ore mine in the Limpopo province, South Africa. The emphasis of this paper will be to show how value can be unlocked throughout the whole mining value chain. In the past (up to 2001) the mine went through a number of crisis situations: planned production performances were not reached, the stripping ratio was extremely high with two new pits, the product beds were not of constant quality, slope failures were big risks, flooding could and did cut off half of the ore supply, and restructuring demoralised staff.This paper focuses on: selective mining throughput drivers, and selective mining and its role in achieving this balance.The results were dramatic, with huge improvements in production results, stockpiles, plant yields and product bed specifications. The mine was brought into balance and a self feeding loop was created with positive spin-offs up to this day.

The Kibali Gold Mine is currently comprised of several open pits, a 3.5 Mtpa underground mine and a 7.5 Mtpa processing plant. Development of the underground operation commenced in December 2012, with initial access established through twin surface declines, followed by the commencement of a blind sink 8 m diameter rock hoist shaft in March 2013. Stope production started in December 2014 with ore and waste trucked to surface. The 755 m deep blind shaft sink was completed in August 2015 together with the cutting and equipping of three main station levels. Two of these levels provide access for the establishment of the underground materials handling system as well as the main dewatering system for the mine. This paper covers the process of determining the optimal location of the Kibali hoisting shaft relative to project-specific critical factors, and then focuses on the design and implementation journey associated with the off-shaft development that essentially encompasses the Material Handling System (MHS) for the mine. The journey starts from the findings of the previous owners Feasibility Study (FS), a subsequent Optimised FS, followed by detailed engineering through to implementation. It was identified during implementation that a significant change in the designed off-shaft layout was required following detailed underground geotechnical and hydrogeological investigations. In addition, the paper provides details on the MHS performance since commissioning to steady state, and on key learnings relating to the overall project process.

Mining in Western Australia places significant economic and environmental responsibility on Government and the mining industry. Groundwater abstraction is regulated by the Department of Water. The Department has recently issued a guideline for Pilbara mines, and this paper focuses on the Pilbara for its examples and studies. The guideline is intended to form the basis for developing a statewide policy, consistent with reporting and assessment policy.With rapid expansion of the mining industry between 2005 and 2008, the groundwater resources in the Pilbara region of the State are under increasing pressure. Significant new management and regulatory issues arising recently include: cumulative impact assessment; disposal of dewatering excess by discharge, sharing or reinjection; life-of-mine approvals and closure planning; closure water quality including metals concentration in pit lakes; and in situ leaching mining methods and their impacts on regional water resources (and change of State Government Policy with respect to uranium mining).

With the recent rapid development of Chinese steel industry, the world iron ore resources of good quality are depleting rapidly. As a result, iron ore deposits of lower quality have been or are being developed to meet the market demand and are becoming increasingly important to iron and steel companies. As concentrates have advantages of high Fe grade and less harmful impurities, they are ideal sweeteners to maintain sinter grade and quality when high proportions of low-grade ores are used in the sinter blends. However, due to their fine nature, addition of concentrates will inevitably increase the proportion of adhering fines and reduced the proportion of nucleus particles of the sinter blends, which can adversely affect the green bed permeability and sintering productivity. In the present study, lab scale sintering tests were carried out under the conditions similar to the sintering process to examine the effect of concentrates on the high temperature sintering characteristics of sinter blends and the structure and strength of the resultant sinter analogues.

In 1983, open cut coal mining operations were suspended at the Tent Mountain coalmine, located in Alberta, Canada in an area called Crowsnest Pass. Montem Resources (‘Montem’), an Australian mining company, purchased the asset in 2016. They conducted several drilling campaigns and undertook environmental monitoring with the intent to restart operations. In 2020, Montem completed a Definitive Feasibility Study, which indicated a mine life of more than 14 years.

Separating Particles by Horizontal Deflection in a Paramagnetic Fluid This paper describes the horizontal deflection behavior of particles in paramagnetic fluids under a high-gradient superconducting magnetic field. Movements of single particle were observed experimentally and indicated that they could be deflected to different positions. Based on these results, the behavior of groups of particles from ore samples was examined in the superconducting magnetic. It was found that these particles could be deflected and settled at a certain position due to the combination of gravity and magneto-Archimedes forces as well as a lateral buoyancy (displacement) force. Mineral particles with different susceptibilities and/or densities could be deflected to different positions, thus producing groupings of similar types of particles. The work described here, although in its infancy, could form the basis of new approach to separating particles based on a combination of susceptibility and density.

Myuna Colliery long term ventilation requirements were included as part of the overall planning of colliery construction and underground development. Ventilation, during the simultaneous sinking of two drifts and the downcast shaft, was accomplished by established methods of ducted exhaust and blower fans. Drivages for the 3 seam interconnections of shaft and drifts were ventilated by extension and duplication of the exhaust systems. Ventilation, for the below seam extension of the belt drift, construction of between seam and below seam bins, and the completion of a circular between seam drift, required a step by step changeover to a temporary main ventilation exhaust fan mounted adjacent to the top of the completed downcast shaft. The second, permanent upcast, shaft was commenced and colliery production started one seam at a time, using the transport drift, while the underground bins, belt and between seam drifts were completed. The uocast shaft was completed and a connection made in the upper seam, completing all contract mining and allowing the main fan to be erected.
