Most accidents are the result of risk taking. They can happen anywhere. Our coal industry categorizes accidents by type, occupation, commodity, age, every way you can imagine. We now know, how many were injured and when, for all the different types of accidents. We have ask ourselves many times, why are we having accidents, how can we stop the accidents and fatals in our industry? Present numbers and projections do not show that this will happen. Serious accidents can result in fatalities. Fatal accident can be prevented, and the coal industry can achieve zero fatalities. How soon this happens, depends on a great number of factors. I will discuss accidents and reasons for us having accidents in an attempt to get to the root causes. The historical perspective will be touched on, and the risk and analysis of accidents will be probed.

For the past six years, the author has made it a priority to attend the Prospectors and Developers Association of Canada’s (PDAC) annual conference. This most recent meeting took place March 10-13, 2002in Toronto, Ontario. In the three years since the author’s last PDAC report, the mining industry has undergone a severe trough. Some the industry’s woes included deeply discounted commodity prices, rampant layoffs, mergers, and overall avoidance by the investment community. However, at each of the PDAC conferences, the author has felt a heightened sense of excitement about the mining industry, more so than at other similar gatherings.

Due to improved, stringent safety regulations, many modern road tunnels are statistically safer, per kilometer, than above ground roadways. However, the public perception and reaction in the event of a serious tunnel fire is very different to that associated with a surface incident. Vehicle fires in road and rail tunnels are recognized to represent an immediate and extreme danger to the patrons of those systems. Unlike workers in the mining and civil construction industries, the general public are neither trained nor equipped to fight fires or evacuate under deteriorating smoke conditions. Accordingly, for the majority of modern transportation tunnels the ventilation infrastructure is sized on the requirement to control the combustion products from worst-case fire scenarios and to provide a tenable evacuation route for the tunnel users. It is common for these same emergency ventilation systems to be employed, often in a reduced capacity, for other operating modes such as the dilution of pollutants during congested traffic conditions. This is one of the fundamental differences to mine ventilation, where primary ventilation systems are not typically sized based on fire scenarios. This paper presents an overview of the subject of fires in vehicular tunnels. This includes a brief history of major transportation tunnel fires followed by a discussion on the resulting legislation and standards that govern the design and operation of tunnel emergency ventilation systems. A summary is provided on the common criteria, theory, and design techniques that are employed during tunnel emergency ventilation systems design. Information is provided on fire detection and suppression, with a discussion on the growing acceptance of wet suppression as a means to mitigate major fires in road tunnels worldwide.

A macroscopic approach integrated with microscopic analyses for wettability characterization is presented. Wetting depends on 1) Contact between the dust and the fluid droplets, 2) Wettability of the particles by the wetting fluid, and 3) Wettability rate (due to low contact times). The goal of this paper is to show that the wetting rate is a significant factor in dust control. Samples from the roof, three different horizons of the coal seam and the floor were pulverized to less than 25 µm. Experiments were conducted with known wetting fluids to correlate surface tension and equilibrium wettability. Further analyses were performed in terms of surface charge (zeta potential), particle size, mineral composition, elemental composition, and quartz content. The surface tension reduction was also achieved by the use of chemicals and/or altering the temperature of the water. Two methods were used to evaluate wettability rates: a) near real-time comparison of wettability rates, and b) comprehensive evaluation of the intrinsic wettability rates. The data obtained provides meaningful insights towards the design of engineering controls for an economical solution to dust control in coal mines.

Application of a magnetic field was shown to be effective in controlling iron losses in pilot-scale cationic silica flotation in the processing of magnetic taconite concentrates. The design of a magnetic field distribution device and batch flotation test results using a 1.42m3 (50 cu. ft.) WEMCO cell are reported. Major losses of iron units in froth products were in the -25µm (500 mesh) fraction, and the application of a magnetic field decreased the flotation of fine magnetite, thereby improving the selectivity of separation. The “as received” sample was sufficiently magnetized and no further benefit was gained by magnetizing treatment. Selectivity was somewhat adversely affected by demagnetizing the sample, though its effect was minimal due to the magnetic field. The device is simple in construction, low cost and may be installed readily in existing equipment.

The process of grinding mill quality assurance brings with it associated truths and myths. When the mill quality assurance process took its first great steps forward in the late 1970s and early 1980s, they were in response to the conditions existing, in our industry, at those times. In the late 1990s, we are faced with different conditions, and problems, and need fresh solutions. Yet the system developed for the early 1980s has produced no new approaches for the late 1990s, becoming instead more hardened and entrenched. This lack of development and actual resistance to research is now costing the industry significant amounts of money in late start-ups and useless scrap loss.

The recovery of natural gas (primarily methane) from coalbed deposits is an established and growing source for the U.S and accounts for over 7% of its total production. The gas from on-purpose degassing is often pipeline quality and requires only compression and dehydration before being admitted to the pipeline grid. However, the raw gas is sometimes contaminated with nitrogen (N2) and/or carbon dioxide (CO2) that can make the gas inadmissible to the transportation pipeline. In general, the N2 must be 4% or less and the CO2 should be 2% or less. In treating gas produced from virgin coal bed or from degassing operations where CO2 is present, amine systems are traditionally used for its removal. Engelhard’s Molecular Gate® adsorption system offers an attractive alternative due to its skid-mounted equipment, low pressure and unattended operation, cost and high on-stream reliability. Natural gas can also be sourced from the residual coal, after the coal has been mined. This source can be the GOB or gas remaining in abandoned coal mines. This source is almost universally contaminated, often with a combination of CO2 and N2. If the source is from air intrusion, the source can also be contaminated with O2. In treating CO2 / N2 contaminated feeds, the Molecular Gate adsorption system removes both of the impurities along with a level of O2 within a single processing step. To date, ten Molecular Gate units are underway with three systems having been installed for upgrading gas from abandoned coal mines. The technology consists of specialty adsorbents and advanced PSA processes that are jointly offered as fabricated packaged equipment by Engelhard Corporation and Guild Associates, Inc. Upgrading requirements, project challenges and project arrangements are discussed in this paper.

The importance of optimizing equipment replacement decisions becomes more critical each year due to the rising costs of buying new equipment versus maintaining old equipment. Replacing equipment when it is physically worn out, or replacing it too soon or too late relative to its Economic Life (when it is economically worn out), is very costly. After extensive literature review and research, Kennecott has developed its Equipment Replacement Guide (ERG) system which identifies, evaluates, and justifies the replacement of uneconomical equipment. The system considers such critical factors as inflation, decreasing work capability, increasing maintenance/repair and operating costs, taxes, time value of money, return on investment, salvage value, and investment tax credit. These factors are used in a system of equations which balance the costs of new equipment against the total operating costs of old equipment. The solution of the equations determines the Economic Life of equipment. Replacement of equipment at the determined Economic Life will earn the specified, required return on investment that is used in the calculation of the Economic Life.

Recent accelerated exploration efforts in southwest China have progressively quantified the vast phosphate resources in the Yunnan Province. To date, according to Chinese geologists, potential reserves in the province may be on the order of 4 Gt, which would equal the entire US reserve base. Development of this vast reserve is underway at two sites located 60-70 km southwest of Kunming, the provincial capital of Yunnan. The first, the Kunyang open-pit phosphate mine, produces an estimated 1.5 Mt/a of 22-30% P20,5. The other project located north of Kunyang is the Haikou mine, which the Chinese consider a possible 2.0-Mt/a operation. Because average ore grades run 3% less than those of Kunyang, beneficiation will be necessary. While the latter project has been initiated on a small scale, it is basically still on the drawing board. Brown and Root and the US Bureau of Mines are assisting on the project. Both operations are under the direction a regional administration which reports to the Ministry of Chemical Industries in Beijing. Combined production from both mines could represent some 70% of China's future productive capacity. Ore reserves in the region are estimated at 400 Mt by local officials. Geology of the District The phosphates are considered Cambrian in age and rest on a Precambrian dolomite. The Indian Ocean, Chinese geologists say, may have been the original source. The line of deposition stretches on a trend from Vietnam in the southwest, through Yunnan, into Sichuan Province to the northeast. Because the area is considered very stable from a geologic viewpoint, large quantities of phosphate have been deposited and large outcrops are known to occur everywhere in the province, according to Chinese sources. The Kunyang and Haikou ore bodies are thought to be two wings of an anticline with the former the southern portion, and the latter the northern portion. Each ore body strikes on an east-west axis. While there are many localized phosphate operations throughout the province, the Kunyang mine is the premium producer. Kunyang's Mining Development The Kunyang open-pit mine was opened in 1966. During the early years, operations were considered somewhat disorganized. Systematic mine planning and normal operations were not initiated until just two years ago, according to mine management. In 1979, Kunyang finally reached its design capacity of 1.5 Mt/a. The ore body consists of two phosphate beds separated by a band of shale. The top layer of phosphate varies from 4 to 8 m, averaging 6 m. The middle shale layer is 2 in. The third layer, which is not as rich as the top, is approximately 4 m. The Vulture mine near Wickenburg, Arizona was a major gold producer from 1863 to 1942, having yielded about 11t(12.1 st) Au and 8t (8.8st) Ag. Gold occurs as coarse native metal and electrum in quartz veins and also finely dis¬seminated within a quartz monzonite sill and its silicified wall rock. The sill is semi-conformable within a Proterozoic volcaniclastic-dominated sequence, all dipping north about 35°. Early mining focused on the vein-hosted gold, particularly in the immediate hanging wall and footwall of the sill. Later efforts included some open-pit mining of the outcropping sill and adjacent altered and mineralized rock. Two sets of post-mineral faults have complicated the orebody geometry. The sill is a 350 m (1148 ft) long apophysis from a quartz mozonite stock to the west of the mine area. The stock has been dated at 85 my and contains abundant lesser gold occurrences in its core and about its margins. The Proterozoic rocks are notably lean in precious metals except where altered in proximity to the Creataceous stock and sill. Circumstances dictate an interpretation of epigenetic mineralization related to the Laramide intrusion.

Newmont Mining’s Ahafo Mine is located in Ghana approximately 300 km (190 miles) northwest of the capital city of Accra in the Brong-Ahafo region. The nearest city is Sunyani, located 45 km (28 miles) to the north. The Ahafo Mine will process ore from seven major pits that extend along a 40-km (25-mile) strike. Four pits are in the south and three pits are in the north. The resource is currently stated at 373 t (12 million oz). But it still has the potential for growth both from new discoveries and the development of underground operations.

Natural aggregates, primarily stone, sand and gravel, are recovered from widespread, naturally occurring mineral deposits and processed for use primarily in the construction industry. They are mined, crushed, sorted by size, and sold loose or in concrete products to build roads, houses, buildings and other structures. Smaller quantities are used in agriculture, cement manufacture, and chemical and metallurgical processes. The growth of cities, their buildings and the highways that connect them have created demand for enormous quantities of aggregates. Consumption has now reached 9 t (10 st) per person and continues to grow faster than population. The aggregates industry is the largest mining industry in the United States in terms of production by weight, by volume and by number of active operations. About 80 percent of the concrete pavement and more than 90 percent of the asphalt pavement is composed of aggregates.

The Ashlu Creek Hydro-electric project, located north of Squamish, BC, at the confluence of Ashlu Creek and the Squamish River, comprises a 49 MW run-of-river project and features a 4.08 m diameter, 4.4 km power tunnel and a 128 m deep drop shaft. The Ashlu Creek Hydro-electric project is being developed by Innergex under Ashlu Creek Investments Limited. Site civil works are being completed by Ledcor CMI of Vancouver with tunnel construction by Frontier Kemper Constructors ULC under a lump sum design-build contract. Site works got underway in September 2006 with the construction of the tunnel portal and laydown area. These site works required rock slope stabilization over a 50 m rock slope to secure the laydown area as well as the construction of a tail track muck tipping station and structural wall to facilitate TBM excavation and spoil removal. Starter tunnel construction commenced in early 2007 and TBM tunneling started in the late spring of 2007 and is planned to continue until early 2009. The power tunnel is being excavated using a 1,200 kW Wirth open-type hard rock TBM dressed with 31?17' cutters. The TBM featured two fixed mounted rock bolt drills behind the finger shield: these were removed and replaced with jacklegs to improve access. A probe drill at the end of the main beam is also present. The geology along the 4.4 km power tunnel comprises very strong and generally massive granitic bedrock with rock strength greater than 225 MPa. A unique feature of the project is that no subsurface investigations have been completed for the tunnel however extensive bedrock outcrops are present along the Ashlu Creek valley. A historical quarry exists approximately mid-way along the tunnel alignment that has exposed good quality granitic bedrock. TBM tunneling to date has achieved advance rates of 10?20 m/day with a maximum of 23 m and has encountered all of the anticipated six major fault zones where the installation of various standard high capacity tunnel support was successful to facilitate TBM advance through these sections.

Although production operations at mines, representing the extraction and removal of the mined material, often appear to receive the greatest amount of attention, the emphasis given to auxiliary operations is continually increasing. This is due to the changing economic, social, and environmental climate in which modern mines must operate. Auxiliary operations can best be described as those activities that are not directly related to the extraction and removal of the mined material, but must receive proper attention if recovery is to take place in a safe, efficient, and socially responsible manner. In fact, certain mining auxiliary operations, such as strata control and ventilation, are considered to be of such importance that they warrant separate sections in this Handbook. Judging from a mine-personnel standpoint, it is not uncommon for a mine to hire the same number or more individuals responsible for auxiliary operations as the number assigned to production.

The tracer gas method of conducting ventilation surveys is a means of determining air quantity in underground mines with knowledge of the concentration of the tracer in the atmosphere and the flow rate at which the tracer is being released. The technique is useful in inaccessible areas of underground mines and areas where traditional point measurement of velocity is not practical. Sulfur hexafluoride (SF6) is the industry standard tracer(Kennedy 1987) used in underground mines because it is safe, stable, and not naturally occurring in the mine environment. The implementation of a second tracer will increase the versatility of the tracer gas technique allowing for simultaneous releases for the study of interrelated ventilation circuits, and for conducting multiple experiments in less time. This paper will detail the selection of perfluoromethylcyclohexane (PMCH) as a novel tracer with sensitivity, physical properties, and analysis characteristics similar to SF6. Methods for the release of liquid PMCH into underground mines will be recommended, and the vulnerabilities of sample loss due to condensation of vapor PMCH will be explored. Finally, the benefits and implications of using PMCH as a second tracer will be discussed.

For tunnels in horizontally bedded rock, geologic factors affect the selection and design of the initial supports, including the rock propensity toward parting along bedding and the resulting high potential for rock slabbing during excavation. To evaluate this ground behavior, numerical analyses and results with Mohr-Coulomb, Hoek-Brown, and Ubiquitous Joint constitutive models were used, using the Upper Rouge CSO tunnel in Detroit, Michigan as an example. The different rock models and other geologic considerations are shown to impact the selection of the tunnel initial support and design. This paper emphasizes the appropriate selection of an analytical model based on the site specific geology to achieve an optimum initial support design.

The creation and maintenance of a healthy working environment in surface mining operations require sustained efforts in pursuing advanced research initiatives toward the development and deployment of powered haulage technologies. This paper contains progress summary on a frontier research in truck dynamic control, vision and collision avoidance. This research is required to provide solutions to challenging problems regarding operator visibility in truck haulage in a surface mining environment. The paper focuses on truck control kinematics and dynamics, truck-haul road interactions, intelligent truck sensing, intelligent truck collision avoidance system, integrated system for situational awareness, augmented visualization and operator interface, and field testing and error variance modeling of developed technologies. A sensor suite of four subsystems, including front-side stereo-camera, front-side Doppler radar, close range sonar proximity sensors, and rear-side camera system have been recommended for developing a 360° truck operator vision to allow safe operations. This research initiative is significant because it will provide a strong basis for developing technologies to improve truck haulage safety in surface mines and construction sites.

One of the major problems of mineral exploration is the ability to reliably distinguish between uneconomic mineral deposits and economic mineralization. While the mining industry uses many geophysical methods to locate the mineral deposits, until recently, there was no reliable technology for mineral resources identification and characterization. The main goal of this project is to develop a new geophysical technique for subsurface material characterization, mineral exploration and mineral discrimination, based on electromagnetic methods. The proposed new technique is based on examining the spectral induced polarization effects in electromagnetic data using the modern distributed acquisition systems and advanced methods of 3-D modeling and inversion. The recovered parameters of the relaxation model will be used for the discrimination of different rocks, and in this way will provide an ability to distinguish between uneconomic mineral deposits and zones of economic mineralization using geophysical remote sensing technology (Zonge, 1976). The proposed research and development is undertaken using multi-partner collaboration between the University of Utah, four major mining companies, Kennecott Exploration, BHP Billiton World Exploration Inc., Placer Dome Inc., Phelps Dodge Exploration, and Zonge Engineering Inc., which is equipment manufacturer and service provider to the mining industry. The multi-partner collaboration between the academic institution, the University of Utah, and mining industry helps to identify the appropriate mining target and conduct the field experiment for practical demonstration of the developed new technology, which is an important part of the this project.

The purpose of this study is to develop a mathematical model to describe the oxidation of cyanide with SO2as proposed in the Inco process. This research employed a direct method for measuring the change in cyanide concentration with respect to time as effected by varying concentrations of SO2, copper and pH. Developing this model may be applicable in determining optimum conditions in a process well known and used in the mining industry. It was determined that the reaction is first-order with respect to cyanide, SO2and [Cu+2].The reaction order is 0.15, or zeroth order with respect to [H+] at pH range from 8.0 to 9.5.The activation energy, Ea, is 21.6 kJ/mol. Preliminary recommendations for the Upper Northwest Interceptor 1&2 specified installation of reinforced concrete pipe using pipe-jacking methods. However, a larger one-pass tunnel constructed using precast segmental lining would provide additional storage volume. The risk of encountering unexpected conditions or events and the resulting cost impacts could differ significantly for these two options. A probabilistic life-cycle cost analysis of these alternatives was performed. This paper discusses the construction alternatives considered, the risk assessment methodology used to evaluate these alternatives, the input parameters and risk factors used in this analysis, and the recommended project which is currently being constructed.
