The technology to enhance safety and control features for mine shaft hoists is being investigated by researchers at the U.S. Bureau of Mines (USBM). The objective of the research is to prevent injuries and fatalities related to hoist and elevator operations. Monitoring and controlling mine hoisting operations will allow hoist operators, inspectors, and maintenance personnel to increase their awareness of potentially dangerous situations and provide controls for safer operations. Process simulation and visualization software, real-time data sensing, and advanced programming techniques are being applied to this problem. Development of reliable and cost-effective sensors and data acquisition interfaces to monitor hoist loads and detect slack rope conditions is underway. Three-dimensional design software was used to visualize mine hoisting to enhance understanding of hoist operations. Quick Basic and Visual Basic (an object-oriented Windows interface) are being used for acquiring and processing hoisting data. Testing and evaluation of components are underway at USBM testing facilities. The resulting system will allow hoist personnel to assess critical operating parameters, such as hoist position, speed, acceleration, conveyance load, and rope tension.

Lower the respirable dust exposure of longwall shearer operators by use of a modified head to tail cutting sequence. Approach Modify the cutting sequence of double-drum shearer operations cutting unidirectionally head to tail, so that the tailgate drum is used to mine the entire coal face except when cutting out at the head. This approach would place the primary dust generating source, the cutting action of the drum, on the return air side of both shearer operators. How It Works Currently, 75% of the double-drum shearers in operation employ a unidirectional cutting sequence. On half of these operations, the primary cut is taken as the shearer travels from the headgate to tailgate ends of the face, with roof support to be advanced on the intake air side of the shearer. Typically, the leading (tailgate) drum is taking a full cut during the head to tail pass and the trailing (headgate) drum is cutting bottom coal. In this mode of operation, the dust generated by the trailing (headgate) drum can spread out into the walkway, and increase the respirable dust exposure of both head and taildrum operators. As an alternative, if the trailing drum is "free wheeling" or cutting minimal coal during the head to tail cut, and bottom coal taken by the tailgate drum during the tail to head clean-up pass, both operators are able to work on the intake air side of the primary dust generating source (drum) except when cutting out at the head. This modified cutting sequence would save wear on the conveyor and help to eliminate overloads, as well as reduce the respirable dust exposure of both shearer operators. This modifcation is only applicable to long-wail operations currently cutting unidirectional head to tail, and to those operations where this unidirectional cut would not produce operating constraints.

As part of its investigation of methods of increasing the efficiency of fuel resources, the Bureau of Mines is continuing the study of the general problems involved in the economic use of fuels in gas pro- ducers that were carried on by the technologic branch of the United States Geological Survey. In the course of this study many inci- dental problems of prime importance have demanded consideration. Among them is the determination, under practically constant con- ditions, of the duration of gas-producer tests necessary to reduce the possible error to a minimum. A discussion of this proper length of test period forms the first part of this bulletin. An attempt to dispel some of the uncertainties that arise in an investigation or to account for some of the minor phenomena often necessitates a systematic and careful study of certain chemical, ther- modynamic, or physical laws, or a combination of these, which in turn may result in information or conclusions of great value. Fur- thermore, seemingly unimportant observations often prove to bear directly upon the design, construction, or manipulation of the machine under test. In attempts to improve the gas producer considerable attention has been given by manufacturers to such questions as fixing or isolating the combustible components of the tar, utilizing the waste heat of the producer for the generation of the steam required by the plant, apply- ing the suction principle to bituminous coal and other tarry fuels, and many allied subjects. Little study, apparently, has been given to the physical-chemical conditions most suitable to the formation of hydro- gen and of carbon monoxide, with a view to regulating the percentages of these gases in generators using fuels other than anthracite coal or charcoal. An attempt was made to introduce such investigations when the operations of the government coal-testing plant began at the Louisi- ana Purchase Exposition in St. Louis in 1904, but owing to adverse conditions these problems were abandoned until the plant was trans- ferred to Norfolk in 1907. The delay in beginning this line of inves- tigations, together with the short testing season at Norfolk, limit the second part of this report to preliminary physical-chemical studies, which, however, were sufficiently significant to warrant careful ex- amination pending more complete researches, which are being carried on at the station located in Pittsburg.

Such a module would be applicable to surface mines and road construction sites, both of which require personnel to work under and on top of steep and often unstable slopes.

The term "explosion-proof," as applied by the Bureau of Mines to an electric motor, refers to a motor inclosed by a casing so constructed that an explosion of a mixture of mine gas (methane) and air within the casing will not ignite a mixture of the same gas surrounding the motor. There are two classes of motors so constructed: First, a totally inclosed class built strong enough to withstand high internal pressures and so designed that the efficiency of all inclosing covers can be satisfactorily maintained; second, a class provided with relief openings or valves designed to relieve the pressure of an explosion within the motor casing and to cool any products of combustion dis- charged through the valves. A satisfactory motor of the first class is much more expensive to build than an equally safe motor of the second class. For this rea- son, attempts to make motors explosion-proof have been confined chiefly to motors of the second class.

Recently 30 men wore viewing the demonstration of a now portable electric arc-welding outfit. A few hours later 17 of the 30 men reported to the doctor for treatment. They were suffering from traumatic conjunctivitis. In two cases the gain was very severe and the symptoms were similar to those of iritis. orphine had to be administered to afford relief from pain. Only two men of the 30 wera not affected in some way from this exposure. These two men were thick-lensed orange-colored glasses. Several of the men wore orange-colored glasses with thin lenses, but the latter were not heavy enough to afford protection.against an exposure as long as took place. The distance of the eye from the are also in- fluences the possibility of injury. Conjunctivitis is an inflammation of the con- junctiva; the conjunctiva is the mucous membrane covering the inside of the eye- lids and part of the eyeball. Traumatic conjunctivitis is caused by foreign bodies in the eye, exposure of the eyes to high winds, dust, smoke, intense light from electric arc lamps, and from electric welding apparatus. In the instance mentioned above, the inflammation was due to the ultra-violet rays. The ultra-violet rays lie beyond the violet rays in the spectrum and are invisible to the eye. In some cases the effect is so severe that, in addition to conjunctivitis, an inflammation of the skin similar to sunburn is produced. The symptons of conjunctivitis caused by intense light or by the ultra- violet rays are marked photophobia (abnormal intolerance to light), excessive secretion of tears, intense starting of the lid, contraction of the pupil, some- times swelling of the lid, and small ulcers developing on the eyeball or cornea. Unless properly treated by a physician immediately, chronic inflammation of the conjunctiva, cornea, iris, or retina, and possibly blindness, may result. Under proper treatment most cases get wall in a few days. All treatments should be under the direction of a physician. That usually advised is to place ice packs on the patient's eyes three or four times daily. The pack should be left on from 15 minutes to an hour. The eyes should be irrigated with normal salt solution (a teaspoonful to a quart of sterile vater) or a saturated solution of boric acid several times daily. If there is a discharge of pus, a few drops of a 25 per cent solution of argyrol or a 5 per cent solution of protargol should bo placed in the eyes three to six times daily. The patient should be confined to a darkened room until his condition improves in order to avoid complications. These treatments will reduce the swelling, give the patient confort, and prevent the development of chronic conjunctivitis. In severe cases it may be necessary to administer morphine to relieve the pain.

Interest in training evaluation in the mining industry was piqued with the promulgation of Title 30, Part 46, of the Code of Federal Regulations. Under Part 46.3, a training plan is considered to be approved if it contains certain information, including “The evaluation procedures used to determine the effectiveness of training.” The present paper is a broad overview of training evaluation and is intended to give trainers and decision-makers a framework for planning or assessing training evaluation strategies. It discusses questions to consider when starting an evaluation plan, Kirkpatrick’s model of evaluation categories, and various ways to collect data. It does not provide detailed instructions on how to develop evaluation methodologies, but reviews topics to consider when creating an evaluation plan. Training evaluation is a term that has many different meanings: assessing the quality of a course, effectiveness of materials used, teaching style of an instructor, or the comfort of a classroom. An evaluation can be done informally over lunch or with highly structured data-gathering tools. It can produce results that are useful to trainers, program administrators, corporate decision-makers, or no one. The key to a worthwhile evaluation is clearly defining why the evaluation is being conducted. Once the purpose is defined, planning the evaluation strategy can begin. For an evaluation to be effective, it should be incorporated into the development of the training activity itself. The training evaluation worksheet at the end of this paper can be used to guide evaluation planning.

The United States produces all of the fuller's earth used for re- fining petroleum within its borders. On the other hand, most of the fuller's earth used in bleaching edible oils has been imported from England. Recently a few of the refiners of cottonseed oil have begun to utilize domestic fuller's earth, whereas others have been unable to substitute it successfully for English earth in their practice. Many samples of American earth distinctly superior in bleaching power to the English earth have come to the attention of the Bureau of Mines. For these reasons an investigation of the mining, preparation, and use of fuller's earth in this country, especially in its application to edible oils, was conducted in order to ascertain why our own raw material has been deemed inapplicable to our needs. During the calendar year 1912, according to figures of the United States Geological Survey," there were imported into the United States 1,970 tons of unground fuller's earth, valued at $11,619, and 17,139 tons of ground fuller's earth, valued at $133,718, these values being based on the wholesale market price at the port of origin. The addition of transportation charges, commissions, etc., make the price to the American refiner $14.50 to $16 per ton. Accord- ing to the latest figures compiled by the United States Geological Survey, the United States in 1912 produced 32,715 tons of fuller's earth, valued at $305,522, or $9.34 per ton, at the mine. Most of this domestic production was from three plants in Florida and one in southwestern Georgia and was used almost wholly for decolorizing petroleum. A small amount from other localities was used in the refining of edible oils. No figures definitely differentiating fuller's earth from other clays are kept in regard to our exports, but it is certain that several thousand tons of domestic earth was exported to Germany, and it is also true that German refiners of edible oils have

"The Workman's Compensation Law of Indiana, enacted in 1915, amended in 1917, 1919, 1923, and 1927, and in 1929 named the Indiana Workmen's Compensation Act of 1929, provides for a weekly compensation for disability due to accidents of 55 per cent of the average weekly wage of the employee, to start on the eighth day of disability. Compensation is computed from a maximum and minimum weekly wage of $30 and $16, respectively. Thus, the maximum weekly compensation is $16.50 and the minimum $8.80; the total compensation paid is not to exceed $5,000.In addition to the compensation the employer is required to pay all medical, surgical, hospital, and nurse fees for a period of 30 days and may be required to pay such fees for an additional 30 days at the direction of the Industrial Board of Indiana. In the case of fatalities the employer must contribute $100 toward funeral expenses.The law also stipulates that ""the employer shall keep a record of all injuries, fatal or otherwise, received by his employees in the course of their employment,"" also that the employer shall report within one week of their occurrence all accidents that cause either the death of the employee or the loss of more than one day's work. However, no standard form is furnished or required by the industrial board on which the employer shall keep a record of accidents, neither is he required to report the time of return to work of the injured employee unless the period of disability exceeds seven days and the accident thus becomes compensable. Hence, no record is kept of noncompensable accidents other than the first report sent to the industrial board by the employer.If the employer were required to keep a complete and concise record of all accidents, in duplicate, on a special form either furnished or required by the board, giving all of the essential data in regard to the accidents, and if he were required to submit a copy of this record to the board at the end of the fiscal year, the data thus assembled would not only be of material assistance to the board, particularly in making up its annual report, but would also furnish information of much value in the study of accident causes and what may be done to remove them.Those industries that have made marked progress in reducing accidents keep such records, as it is only through a full knowledge of how accidents occur that intelligent steps can be taken to prevent their recurrence. Unfortunately, the mining industry as a whole in Indiana has not kept pace with many other industries in reducing accidents, and it is urged as an aid to improvement that a thorough and complete record of mining accidents be kept."

OWNERSHIP OF ORE-CONSTRUCTION OF CONTRACT. A contract between the Republic Mines Corporation and the Quilp Gold Mining Co. provided that upon forfeiture of its rights under a contract the Republic Mines Corporation was required to give a good and sufficient deed conveying to the Quilp Gold Mining Co. all ore bodies of every description lying east of the west line of the Quilp Mining Co. claim extended downward vertically and lying south of the north end line of the Quilp claim extended downward vertically and which may apex within the lines of the Surprise, an adjoining mining claim. It must be presumed that the contracting parties under such circumstances had in view their statutory rights as provided by the mining statutes of the United States, and that both were familiar with the provisions as to extra- lateral rights granted by the statute to locators of mining claims. This contract means to include all ore bodies of every description lying east (or easterly) of the west (or westerly) line of the Quilp claim extended downward vertically and south (or southerly) of the north end line of the Quilp claim extended downward vertically within the Quilp claim excepting the part that was overlapped by the Surprise claim, owned by the Republic Mines Corporation. Quilp Gold Min. Co. v. Republic Mines Corp. (Washington), 165 Pacific 57, p. 61. PURCHASE OF GAS-REFUSAL TO DELIVER SALE TO OTHERS. A gas company that had entered into a contract to purchase a quantity of natural gas refused to receive and accept the gas that was tendered on the ground that it did not conform to the requirements of the contract; subsequently the company refused to sign a writing by which it was asked to consent to the sale of such gas to other persons. But the refusal of the purchasing company to sign such a writing giving its consent or its refusal to consent to such sale did. not affect the rights of the seller to dispose of the gas elsewhere and to other persons. OWNERSHIP OF MINERALS IN HIGHWAY. All mineral deposits within the limits of a highway belong to the owner of the fee and he is entitled to remove them so long as he does not interfere with the public use of the highway. City of Scranton v. Scranton Coal Co. (Pennsylvania), 100 Atlantic 813, p. 819. SALE AND CONVEYANCE. SALE AND CONVEYANCE EXCEPTION AND RESERVATION. The owner of an undivided interest in oil and gas lands conveyed the same to certain named grantees by deed containing a clause excepting and reserving all the oil and gas produced from the undi- vided interest in the land described. The language is not apt for the creation of either an exception or a reservation, but is rather that of a layman unfamiliar with legal distinctions. Strictly and literally construed, it affords a basis for a contention that that which was excepted and reserved was not realty in any form or anything inci- dent thereto, but personalty produced from the land. Immediately following the description of the tract, the grantor excepted and reserved something from the land described. What he excepted and reserved, he excepted and reserved then, and it was from his interest in the land. The clause expressed according to the evident intention of the grantor would read: "Excepting and reserving from the above all oil or gas which may hereafter be produced therefrom." No other construction can reasonably be placed upon the language used. The carving out of a separate estate in the oil and gas in land is a common occurrence in oil and gas producing fields. Mandle v. Gharing (Pennsylvania), 100 Atlantic 535, p. 536. RESERVATION CONSTRUED AS AN EXCEPTION. Where apt words of reservation are used in a deed they will be construed as an exception where this is the evident intention of the parties to the instrument. Although an exception and a reservation differ in legal effect, yet in their creation there is no magic in words, and if the meaning is clear either expression will operate for the purpose designed. The technical distinction between an exception and a reservation is frequently disregarded in the ordinary use of the word, and whether the language used creates an exception or a reservation must be determined from the intention of the parties ascertained from the entire instrument.

The question "Why can not American kaolins be substituted for English china clay?" has been asked time and time again, and more often since the beginning of the present war in Europe, which has threatened to cut off the supply of European clays imported from abroad. The answer to this question has invariably been, that the domestic kaolins are not as pure and uniform in composition as the imported clays. This demand for white-burning American clays is of such economic importance that it was considered advisable to determine whether some of our vast deposits of impure white clays could not be refined sufficiently to permit their being substituted for the foreign materials. Up to the present time mechanical principles only have been applied in the refining of kaolins, but in order to keep pace with the increasing requirements for better quality and uniformity of product, the application of the principles of colloidal chemistry is necessary. The investigation described in this report was carried on to deter- mine the practicability of applying technical control of clay disperse systems to the refining of kaolins and the utilization of the prepared clay in the manufacture of vitreous china and wall tile. It is hoped that this report will prove a stimulus to the clay- washing industry; will point out a method of refining certain Amer- ican clays, and will lead to the substitution of domestic kaolins for imported china clays by manufacturers of high-grade ceramic wares.

The computer program has been developed in conjunction with the rest of the project to evaluate evacuation capabilities of the mines and develop guidelines for the escape systems. There are several problems associated with predicting evacuation performance of a mine. First, the mine has many interconnected passages, shafts, hoists and a number of miners in varying positions. To be able to predict evacuation time requires processing a hugh amount of data. A simplistic approach of determining the miner farthest from the exit and calculating evacuation time for him knowing his walking speeds and hoisting times is not realistic, since the waiting time for the hoist is neglected. The second problem is being able to repeat the calculations for a number of configurations. It may be possible to spend enough time to hand calculate the evacuation parameters for a single mine for a single configuration, but to evaluate capabilities of the evacuation

During the nine years that have elapsed since the Bureau of Mines was established in 1910, the work of the bureau has included many investigations that have proved of high value to the Nation. Eleven mining experiment stations established by act of Congress have greatly increased the bureau's usefulness; being situated in the mining fields, these stations are closely in touch with industrial needs, and the results of their research and educational work are more directly available to those engaged in the mining, metallurgical, and mineral industries. In the United States mining is second in importance to agriculture only. The Federal Government annually appropriates large sums in the interest of agriculture, whereas the appropriations for mining are relatively small. But the nation-wide popularity of Government assistance to agriculture should not overshadow the need of Federal assistance to the mineral industry. With scientific management farms can be made to produce indefinitely without any depletion of the potential resources of the soil, but the mineral wealth of a country is a fixed quantity, and every year's production brings nearer its ulti- mate exhaustion. As the deposits of the richer or more readily available ores, espe- cially those in the West, have been depleted, development of methods for mining and treating the leaner or more inaccessible ores has lagged, so that meeting the increasing demands for certain metals is becoming more and more difficult. In some branches of mining the small inde- pendent operator is greatly handicapped. He has to rely on custom mills and smelters for the profit or loss of his product, and because of relatively inefficient methods of milling or smelting he may receive no return for much of the ore he mines. Even though the small operator thinks he knows of improved methods of obtaining the metals from the ores, he can not afford to install the necessary machinery or to carry on experiments that may or may not prove successful. In view of the increasing need of metals and the impossibility of renewing the ore supplies, efforts should be directed toward developing the most economical and efficient methods in mining and metal- lurgy. The great mining companies carry on independent research work, some of them on a large scale; but as such work is usually undertaken for gainful purposes, these companies do not feel under any obligation to give the results of their investigations to the world. Clearly the solution of the difficulty is to place in the mining districts trained mining and metallurgical engineers who will investigate and solve the problems for the benefit of the whole industry. It is with this purpose in view that experiment stations of the Bureau of Mines have been established to investigate economy, efficiency, and safety in mining, and to make public the results of the investigations. The Federal Government can be of most help in developing the mineral resources of the country when its forces are brought in direct contact with these resources. Mining is as old as civilization, but only within a generation has this country busied itself with any other problem of mining than the amount of mineral products that could be put upon the market. Little concern was felt if much of the metal in an ore was wasted, if the miner lived in unsanitary surround- ings and died of preventable disease, or if accidents occurred fre- quently and from causes that could easily be controlled. Now we realize that the waste of any resource is inexcusable. The mineral wealth of the country can be mined but once. The task before the Nation is to recover as much metal as possible from the ore in the ground, and to do this in the most economical way without waste of human life. This task is of such magnitude as to warrant aid from the Government through experiment stations. In its efforts to make the work of these stations of most benefit to the mineral industry the Bureau of Mines seeks the cooperation of State authorities, of mine operators, of miners' organizations, and of every person interested. These experiment stations also serve an educational purpose. Gen- erally speaking, mining is not a preferred occupation. The miner should have reason to take greater interest in his work; he should feel that the experiments the Government is making affect him vitally by enabling him to increase his production and by making his work safer. The mine operator should realize that the Government stands ready to aid him, and he should know that he can bring his own problems to a Government organization that is trained and equipped to solve them. The surest method of accomplishing these results is to carry the work of the Bureau of Mines into the mining regions through the establishment of experiment stations easily accessible both to the mine operator and to the miner. Many miners in this country do not read English, but they can understand motion pictures of mining methods; they can learn how lives can be saved in case of accident; and they can appreciate the larger results gained by modern methods and equipment. The first steps in the process of mine evacuation are the recognition of a problem and an attempt to communicate the problem to miners who may be affected. This chapter will focus on the way that a problem, fire, came to the attention of mine personnel and the messages that were sent to miners in the affected areas. The concept of information uncertainty, which was introduced in chapter 4, will be discussed as it influences problem perception and diagnosis. Sociotechnical and interpersonal communications will be explored and suggestions for improving these systems will be offered. It might seem that the first indication of a dangerous fire would motivate individuals to take self-protective action, to evacuate the affected area or structure, and to provide clear warning to others who are in danger. Research has actually shown that in most situations this does not occur. Instead, time is taken to gather more information, confirm information that is provided, and consider possible alternative explanations that could account for the given circumstances [Canter 1990; Scanlon 1979; Bickman et al. 1977]. This process of confirmation can lead to the loss of critical time. Canter [1990] summarizes the problem in his book, which reports studies of a number of fire events: "As discussed throughout this book, ambiguity and confusion, incoherent instructions and time-wasting actions, lack of appropriate instructions and misunderstanding of the nature of the event that is unfolding, are all hallmarks of fires and emergencies that kill people." In this chapter, the detection of each mine fire and the communication of warning to endangered miners will be reviewed with an emphasis on how those processes were affected by the availability and use of information. Information can become available through a variety of mechanisms during an emergency situation. First, cues may be taken directly from the environment. Smoke is an obvious example. Secondly, mechanical devices, such as smoke detectors, may provide warning messages. A third source of information is interpersonal communications. These can occur face-to-face or through some mechanical device such as a telephone. With all three methods there is a possibility of miscommunication, misunderstanding, and misinterpretation. All of these means of communication were used with varying degrees of success in the three mine fires analyzed here. A fire, like any nonroutine situation, engenders uncertainty about a diagnosis and understanding of the problem [Mead 1938]. This uncertainty leads to delays in realization of the seriousness of the situation and therefore in the proper response to it. Delay in action is an important concern in any fire setting, but is even more at issue in underground coal mine fires. Mine fires are qualitatively different from structural blazes: workers' escapeways may be miles long; the seam height at many operations is so low that it is impossible to walk upright; access to underground workings is always limited to a few (sometimes only two)

Homans [1950] offered a working definition of group that is useful for this research: "We mean by a group a number of persons who communicate with one another often over a span of time, and who are few enough so that each person is able to communicate with all the others, not at secondhand, through other people, but face-to-face." This chapter begins with a discussion of the group concept in reference to the hypothesized escape groups mentioned earlier. It then moves to an examination of analysis techniques used by the authors. Finally, the subjects themselves are profiled according to their demographic characteristics. The Nature of Groups Warriner [1956] took a realist approach to understanding groups: "(1) the group is just as real as the person, but (2) both are abstract, analytical units, not concrete entities, and (3) the group is understandable and explicable solely in terms of distinctly social processes and factors, not by reference to individual psychology." Warriner's realist position merely holds that "group" occupies a different domain in which it is no more or less concrete than "person." At this group level the unit of analysis will be those relations that indicate social rather than individual behavior. It is possible to investigate these group properties empirically-if a researcher avoids confusing conceptual entities with concrete ones. Most people seem to accept that group attributes must somehow be inferred, but think personal attributes will be directly manifested, requiring little or no interpretation [Snizek 1979]. In other words, nobody would equate physical components of an underground working section with the actual work group, yet social scientists (as well as laypeople) very often confuse real individuals with notions of the person. In actuality, neither groups nor persons are directly disclosed to the senses; both are inferred by experience and observation. One can "see" a group just as clearly as one can "see" a person, given the proper perspective from which to do so. It is necessary, in developing this perspective, to begin with a sound definition of the thing being examined. Besides communication, or more generally, social interaction, Homans [1950] included three other components of group makeup. "Sentiment" is characterized as the feelings people tend to form about one another when they interact often. These feelings include not only friendliness and dislike, but attitudes such as approval or disapproval. A "norm" is an idea, held in common by group members, that specifies how people ought to behave in given circumstances. In lay terms, norms are simply those informal rules individuals abide by in order to get along together in social situations. Finally, "activity" refers to those things persons do with others. In work groups, as an example, many of the activities are cooperative and goal-directed.

The National Institute of Occupational Safety and Health (NIOSH) is dedicated to achieving "safety and health at work for all people -- through research and prevention." An important procedure in pursuing this goal is the NIOSH surveillance studies where the number of injuries are monitored according to demographic, employment, and injury characteristics. These numeric data are then used to identify injury risk factors, develop safety programs, and monitor implemented remediation methods. In essence, ongoing surveillance data form the bases for directing the resources of NIOSH in setting of research and prevention priorities, and in evaluating safety procedures. This surveillance techmque, which is based on the Public Health Service approach to identification and control of disease (1, 2, has been used quite successfully in dealing with relatively high number injuries such as hearing loss, musculoskeletal disorder, respiratory disorder, falls, etc. (2). However a problem arises when extending this paradigm to lower frequency traumatic events; particularly those often referred to as disasters (i.e., high impact events like mine explosions, airplane crashes, etc.). One can ask the question - there hasn't been a coal mine fire fatality in the U.S. for over ten years, to what extent should we be concerned with mine fire safety (resource-wise)? - The National Traumatic Occupational Fatalities (NTOF) system (4J which NIOSH uses to survey occupational fatalities annually, would suggest that underground mine fires would not be a high priority item in occupational safety. Yet, miners' (i.e., the exposed workers) do consider mine fires and explosions to be major safety concerns. I believe that we need to rethink our evaluation of work place disasters, what is worker exposure, and what are the underlying assumptions (both explicit and implicit) as to the impact that economic, societal and degree of hazard factors make on risk evaluation. The answers to all these problems, will require much broader considerations than those in current surveillance safety and health models. This paper discusses some facets of those considerations.

Increase the safety of operators of front-end loaders by providing a means of monitoring the machine's stability. Approach Through a research contract, the Bureau of Mines developed a front-end loader stability indicator which provides the machine operator with a reliable, easily interpreted display assessing the stability of the machine. How It Works Statistics show that front-end loader accidents are the largest single category of machinery-related accidents in the surface mining industry. Many of the fatalities resulting from these accidents occur when front-end loaders rollover and either crush the operator who is trapped within the cab or strike the operator who is trying to jump to safety. Although rollover protective structures (ROPS) are required by law on front-end loaders to protect operators in the case of a rollover, these do not prevent the actual rollover from occurring. In fact, the operator has no way of evaluating the stability of the machine except by using personal judgment. The Bureau of Mines responded to this problem by developing a front-end loader stability indicator whose primary function is to Indicate to the operator when the machine is approaching an unstable situation which may trigger a rollover. The device bases Its interpretation on measurement of the normal loads on the machine's wheels. Strain gage instrumentation located on the machine is used to monitor the magnitude and change of forces acting normal to the front-end loader's wheels. These forces are direct indicators of the machine's center of gravity relative to its position on the ground. The strain gages are connected to electronic circuitry which amplifies and processes the signals. Stability or instability of the loader is relayed to the operator through a display of green, amber and red lights.

Approach A small lightweight pagerphone, having crocodile clips which pierce the phone line insulation, was developed. How It Works The Portable Telephone/Intercom is temporarily clipped onto an existing pagerphone line at any location, and operated like an intercom. It is riot necessary to strip the insulation from the pagerphone line to make the connection. In addition to the clips for connecting to pagerphone line, the Portable Telephone/Intercom consists of a small belt-carried control box, an ear-phone, and a bone-conduction microphone which is attached to the webbing in a miner's helmet.
