The management of safety has undergone a number of radical changes in the past 100 years. The most important paradigm shifts occurred through the work of Heinrich, followed by the advent of the safety systems era and, more recently, the behavioural safety approaches that have become increasingly popular around the world. Each of these æapproachesÆ to safety has left an indelible mark on the way we target safety improvements, helping decide which interventions are deployed and how they will be measured. They have also resulted in a number of æmythsÆ about safety management, each of which will be discussed, analysed and challenged in this paper. This paper highlights the complex problem of æcomplacencyÆ, that has arisen out of the increased levels of regulation and consequent perceived protection that workers enjoy in the work place. It also looks at a phenomenon called æaccident migrationÆ which results from a superficial treatment of accidents with changes to rules and procedures. The most recent approach to safety is generally known as behaviour-based safety, which flowed from the human sciences and quality management era. This approach has had a dramatic impact on the way safety is managed around the world, but still falls short in a number of key aspects, most notably the area of risk awareness and risk understanding. Many workplace accidents occur simply because the risk is unidentified, underestimated, not understood or ignored. This paper proposes a new direction for safety, called competency-based safety. It is based on cognitive psychology aimed at combating the consequence of modern protection. A case study is presented detailing the deployment of this approach throughout Placer Dome.

A gold mine in southern China has transitioned from an open pit operation to a fully underground mine. An overhand mining method is used which requires the mined stopes to be filled with backfill having an unconfined compressive strength of at least 0.35MPa. Tailings from the process plant is deposited into separate flotation and Carbon in Leach (CIL) tailings storage facilities. The tailings has a high fines content and the thickener on site could only produce a dilute flotation tailings at a solids content of about 30 wt%. This resulted in two undesirable outcomes: the density of flotation tailings in the Tailings Storage Facility (TSF) was significantly lower than planned so that the TSF could not store the life of mine tailings and that the backfill product had to include a high percentage of manufactured aggregate and increased cement content to achieve the required strength. This paper presents results of a tailings study which identified that the flotation TSF would be capable of storing the remaining life of mine tailings if an amended backfill product made up of 100% of flotation tailings could be introduced. A 100% tailings backfill with the addition of 8% cement was developed by classifying the tailings and using the coarser fraction in the backfill. This backfill allowed 60% of the flotation tailings to be placed underground and only the remaining 40% in the flotation TSF. A new thickener was designed and installed to thicken the tailings and rheological testing of the tailings, using a closed loop pilot plant, provided parameters to upgrade the underground reticulation system to deliver cemented backfill to stopes throughout the mine and to deliver thickened tailings to the TSF. Iron from the blast furnace constitutes the starting point in steel production and any increase that can be made in iron production will bring about important repercussions in a long line of processes. Coke constitutes a major item in furnace operation and consumption at the Port Kembla Works of Australian Iron & Steel Ltd. has been approximately 1,850 lb. per ton of iron produced on ore practice, or 3,000 tons per day.For many years coke quality has been subject to much criticism by furnace operators throughout the world, sometimes with little real grounds. Whatever the merits of previous arguments, and they were no doubt two-sided, the development of bigger furnaces has focused attention on coke strength. At the same time a decrease in the quantity of good quality coking coals available to many plants has stimulated research in this field.Extensive coke research has been carried out overseas, and in Australia the.B.H.P. Organization is now actively doing likewise in its Central Research Laboratories at Shortland near Newcastle, N.S.W., and works research groups at Newcastle and Port Kembla Steelworks. Similar work has been initiated in the beehive coking industry. Such work is in its infancy and this paper will outline the direction of the activities at Port Kembla rather than propound specific solutions.At Port Kembla good quality coking coals are readily available from the IIIawarra coalfield. The seams generally mined for coking are Bulli and Wongawilli and to a much smaller extent Tongarra. Characteristics of these coals appear in Table 1.

Formation of porphyry-style ore deposits starts with the metal extraction from magmas at so-called magmatic-hydrothermal transition (MHT). The processes and compositions at MHT are difficult to study as the record of them is poorly preserved. As the MHT is characterised by massive crystallisation of quartz, including quartz æeyesÆ and veins, their study can elucidate questions related to metal transport and deposition. Quartz grains from the porphyry deposits all over the world (Antapaccay, Peru; Batu Hijao, Indonesia; Climax, USA; Panguna, PNG; Far Southeast porphyry, Philippines; Rio Blanco, Chile; Omsukchan, Russia) were studied by optical, SEM-CL and BSE microscopy, electron microprobe and LA ICPMS. Quartz grains, even within each sample, have different shapes (rounded, egg-shaped, amoeboid or euhedral) and cathodoluminesence (CL) patterns (shape, number and width of bands in CL images). The outer layers of quartz are markedly different, which is unexpected if the grains had formed in the same conditions. Inflections and blurring of CL bands are often structurally associated with fractures that are commonly healed with secondary quartz and associated fluid inclusions. Our results show that quartz æeyesÆ could not crystallise from a silicate melt as normal phenocrysts. It is likely that in situ segregation of residual SiO2- and H2O-rich liquid and its solidification followed by coagulation (eg formation of silica-gel globules) took place in the magma (crystal mush) during MHT. Such globules could remain in a plastic state for a long time even at rather low temperatures. The solidification of globules occurred after bands of different composition had formed and led to fracturing of solidified and inflection of unconsolidated layers. All fluids (liquid and vapour) released from the original silicagel during solidification were trapped as inclusions (aqueous, salt-rich and sulfide). An EXTENDED ABSTRACT is available for download. Designing coal pillars to provide resistance against overburden loads has long been an aim of rock mechanics engineers. The need for accurate pillar strength models has become.more urgent as greater overburdens are encountered and pillar sizes grow larger. Current pillar design models differ widely in their predictions of the trend in pillar strength with increasing pillar width- to-height ratio. The goal of this paper is to evaluate current pillar strength theories, using a comprehensive data base of stress measurements from coal pillars. The stress measurements indicate that coal pillars maintain relatively high stresses near the ribline, and that the stress gradient within the yield zone can be approximated as a straight line. Several problems are identified for further research, including calibration procedures for different types of stress cells, strain softening behavior in the yield zone, and measurement of stresses in the cores of very wide pillars.

Discovery of the Vera-Nancy deposit in late-1995 followed 12 years of active exploration in and around the Pajingo Epithermal System, an andesite hosted veinfield covering an area of 150 square kilometres at the northern end of the Devono-Carboniferous Drummond Basin in North Queensland. Early exploration targeted open-pittable resources in the numerous auriferous veins and it was not until 1994 that exploration focus shifted to drilling to depths below about 100 metres from surface, which resulted in the Vera-Nancy discovery. That deeper drilling process is still at a very early stage of coverage of the epithermal system as a whole and substantial potential exists for additional discoveries.Drill-out of Vera-Nancy has to-date established a gold resource in excess of 1.4 million ounces. Mine development is proceeding via a twin decline system with first stoped production scheduled for August 1997.

When a major mineral find has been announced the general perception is that a battle between Aborigines and the mining company involved will inevitably occur.It is a fact that many miners in the past have not had a good relationship with Aborigines because of their indifferent attitudes towards them. They saw no common future and went their particular way. However, attitudes have changed significantly since Noonkanbah and both miners and Aborigines are seeking common ground in order to achieve mutual benefits from the establishment of a mine on or near Aboriginal land. There are many obstacles that both miners and Aborigines need to overcome inordertofindthiscommongroundtoachieveawin/win situation. The obstacles can be found in entrenched ideas and practices which need overhauling, not the least of which emanate from government policy and requirements.Without going into specifics, the Noonkanbah incident during the 1970's where Aborigines and their supporters confronted miners who were ordered by the Western Australian governmentto drill on Aboriginal land, polarized Aborigines and miners. Since this incident, many Aborigines, particularly those in the cities, have had the attitude that mining and Aboriginal culture are in direct conflict. Aboriginal opinion is generally shaped in the larger cities through mass media coverage of the opinions of urban Aborigines and their supporters. Through no fault of their own, these Aborigines were removedfromthe land of their ancestors and have notlived atraditional Aboriginal life-style for a number of generations and most have little knowledge of, nor have had involvement in, traditional Aboriginal lore and culture. For the most part these people live in awestern life-style and few still know their language. They generally have high profiles, often giving viewpoints that are wrongly considered "Aboriginal" and supposedly representing all Aborigines. This group is very different with their thought processes and values from those Aborigines who live on their own traditional lands with a traditional life-style, and practise their age old ceremonies. Their tribal values are based in their relationship with the land, expressed through complex religious ceremonial rites of song, dance and legend. Today this group is very much diminished in numbers but nevertheless in the region of the Gulf of Carpentaria there is a high proportion ofpeople who practise their lore, speak their own languages and pass on traditions to their children. It is well documented in the history of Australia that Aborigines have been decimated in numbers and whole tribes have been wiped out. Certainly in this region the experience has been no different.

A narrow (200-300m) zone of amphibolite facies metamorphic rocks underlies the Papuan ophiolite near the Musa-Kumusi divide, SW of Popondetta, Oro Province. The amphibolite rocks are bounded up-section, to the NE, by peridotite and gabbro of the ophiolite; and down-section, to the SW, by mafic greenschist, which in turn overlies blueschist facies mafic schists of the Emo Metamorphics.Amphibole compositions within the narrow contact zone show a strong chemical zonation which reflects a steep thermal gradient, with maximum temperatures >800¦C at the peridotite contact. Queensland hosts vast deposits of brown and black coal, significant accumulations ranging in age from Permian to early Tertiary. The great diversity of coal resources in the State to some extent reflects the proximity of Queensland to plate-marginal tectonic processes through much of the Phanerozoic eon. Coal is contained within sedimentary basin fills of four fundamental types; 1. extensional, 2. foreland, 3. strike-slip, and 4. intracratonic. Thick (10 m+) though laterally restricted coal bodies are characteristic of extensional and strike-slip basins, whereas coals of more modest thickness but much greater lateral extent are typical of foreland and intracratonic basins.

Close circuiting autogenous and semi-autogenous mills is an energy efficient way of grinding and a number of operations have utilised this in the design of their comminution circuits. Owing to the large flow of slurry through the mill that results from operating this way the grates and pulp lifters have to be designed to cope. However, the literature has little or no information on how grate and pulp lifter size and design are related to their ability to remove slurry from the mill. Over the last few years the JKMRC has been researching this area and has developed relationships between grate/pulp lifter design and flowrate capacity. At the same time it has determined the major causes of pulp lifter inefficiency and a solution to preventing this inefficiency (the Twin Chamber Pulp Lifter). This paper describes the models that have been developed through this program and uses industrial scale data to illustrate their accuracy. Results from the industrial trials of the Twin Chamber Pulp Lifter are also presented.

Ventilation and the environmental climate in the Campiano mine is the subject of this re- search. The mine is located in Southern Tuscany (Central Italy), a region where geothermal gra- dients are very high (three times the average gradient of the earth). The research produced interesting indications as to the influence of surface climatic conditions on the environmental parameters inside the mine. A close correlation has been found between dry-bulb temperature and humidity ratio of the air in the mine for dif- ferent metereological conditions and for differ- ent mass flow rate values. Variations in air enthalpy, caused only by the heat exchange with the rock walls during the flowing of the air through the mine, depends on the surface temperature values. Statistical analyses have been used to ex- press air enthalpy variations on surface tempe- ratures, on the variations of the humidity ratio that the air undergoes inside the mine and on the mass flora rate. The application in practice of the results will permit the identification of correct mea- sures to be adopted so that optimal microcli- matic conditions are produced in the mine.

Mining is taking place at increasing depths of cover on the Southern Coalfield beneath pre- dominantly massive sandstone strata. Within the next twenty years, there will be longwall mining at depths greater than 600 m. As the Sydney Metropolitan area continues to expand and as coal mining approaches these urban areas subsidence will continue to become an increas- ingly important aspect of coal mining research. Investigations commenced in the mid 1960's and it is now possible to predict subsidence in the Southern Coalfield with reasonable con- fidence. The work is showing that the characteristics of the subsidence troughs over the deeper mining at Appin differ from the characteristics of mine subsidence at less cover. The longwall mining at Appin Colliery has resulted in a maximum measured subsidence of 730 mm. The maximum strains were small because of the large radii of curvature of the subsidence profiles. There was no damage to any of the buildings in the township of Appin.

Complex ores, transportation problems, and the Philippine Insurrection halted operations. Ultimate success went to American firms, though Frank Karuth's Philippines Mineral Syndicate had pioneered the field.

Reliability is also most important, and indeed is fundamental, to successful coal exporting.Coal preparation, blending, distribution, marketing and utilisation are areas of the coal industry in which reliability can be critical to coal exporting. Major elements of reliability which impact on coal exporting include coal quality, supply performance, -coal ownership changes, governmental regulations and policies, market reputations and differences in export markets.
