This study describes five fatal accidents so unusual in occurrence that the chain of circumstances bringing them about may never be duplicated. The intent of this publication is to illustrate that everyone working underground is subjected, at all times, to definite inherent mining hazards and that safety devices, though provided, often are ineffective or not used. It is hoped that a study of these fatal accidents in bituminous-coal mines will show workers and safety engineers that nothing can be taken for granted. Safety planning with continuous study can prevent these unusual accidents as well as the routine recurring types, which cause the majority of coal-mining injuries. The treacherous, fatalistic philosophy that "it can't happen to me" or that "when my time's up I'll get it regardless of what I do," still prevails among individuals and even safety engineers. The individual or safety man who believes in luck as applied to safety should not read any further, since this study was written for the sincere, safety-minded individual who accepts the prevention of all accidents as a challenge to his knowledge, initiative, persuasiveness, and salesmanship.

Multiple subject classification has increased the total number of entries to almost 5,000. Helium citations are listed by 23 major classifications. The number of subclassifications has been greatly increased to minimize the time required for literature search. The use of automatic data processing has been extended in compiling the bibliography.

The changing demographics of the mining industry make successful passing of knowledge and skills to the next generation critical. Training methods and technologies are being developed to meet this need. Twenty years ago classroom training was largely a one-way transfer of knowledge through instructor lectures. In contrast, instructors today can use virtual reality technologies to guide trainees as they fully participate in learning experiences. Trainees can interact with the virtual work environment, co-workers, and equipment without being exposed to workplace hazards. The speed of advancement in mine safety, health, and production training shows no sign of slowing in the next 20 years. Researchers at the National Institute for Occupational Safety and Health have studied and participated in the evolution of miner training. A look at training materials they have created in the past and those they are currently developing reveals changes in methods and technologies over time and also key characteristics which have remained constant. Possibilities for the future can he discussed in the framework of an implementation of virtual reality in new miner training sessions. The example computer-based training package teaches new miners to read mine maps and provides opportunity to practice navigation skills in a virtual mine. With this 'serious game', trainees experience the look and feel of moving through an underground coal mine while in the safety of their classroom. During field-testing in 2007, trainees and instructors responded very favourably to this new way of learning. The software was usable by mine trainers with even limited computer expertise and could be installed on the affordable computers they were using for their more traditional slide presentations. The Underground Coal Mine Map Reading Training's foundation on past successful development strategies and its acceptance by today's instructors and trainees provide valuable lessons for developing training materials, strategies, and technologies for mining's future.

The Bureau of Mines studied the presplitting technique used for controlling overbreakage in the walls of a vertical shaft at the U.S. Army Corps of Engineers Littleville Dam project near Huntington, Mass. The periphery of the shaft, the dimensions of which were 16-1/2 feet wide by 21 feet long and 52 feet deep, was presplit (cracked) in the solid to a depth of 48 feet prior to full-depth sinking. Presplitting prevented overbreak in a fairly soft schist and provided sound self-supporting walls which required very little scaling.

THIS REPORT covers the third in a series of five projects into which the Conowingo tunnel system has been divided for geographical and practical reasons. It outlines the work constituting Project No.3 that consists principally of the following: (a) To sink the No. 15 shaft and extend the Alden, Sugar Notch, Baltimore No.2, and Laflin shafts from their present depths to tunnel level; (b) to drive a tunnel off these shafts to connect with the western end of the tunnel in Project No.1 (Lackawanna) and to a point 1 mile south of the No. 15 shaft; (c) to construct an emergency pumping plant at the No. 15 shaft, complete with 3 centrifugal pumps of 200-cubic-foot-per-second (c. f. s.) capacity each, and a discharge-water shaft and a discharge tunnel for conducting the water from the pumping plant into the Susquehanna River; and (d) to construct a lateral tunnel 1 mile long off the main tunnel in the vicinity of the No. 15 shaft, to be known as the Wyoming lateral. The project, as outlined, is a self-contained drainage unit that will permit unwatering inundated mines and safeguarding active mines in the Wyoming Basin of the Northern field and also will handle the water in the Lackawanna Basin, thus permitting the temporary pumping plant provided in Project No.1 to be discontinued. The pumps installed in the temporary pumping plant provided in Project No.1 can then be moved to the emergency pumping plants recommended in Projects Nos. 2 and 3, to be installed as the third or standby unit.

The authors investigated the nature, cause and treatment of 234 back injuries which occurred in 156 different situations in underground coal mines. A detailed interview was conducted with each injured miner as 137 variables were explored to provide a complete picture of the injury scenario. The miners worked at 23 mines in six states. An important finding of the study was that sudden movement, which occurred in 527 of the injuries, is a more frequent factor than heavy lifting in causing hack injuries. Along with sudden movement, other major injury causing factors were equipment seat design, lifting, cable handling, and slips/trips. No significant differences were found between the injury scenarios associated with lost time and non-lost time injuries; thus, both sets of scenarios should he used in doe loping back injury control strategies. Medical treatment and rehabilitation varied in content and quality. Dialogue between the medical, rehabilitation and mining communities is needed to develop an effective treatment strategy. Back injury control related training was found to be inadequate in most mines. Mine specific training for physical stress recognition and potential sudden torte release should he added to the training currently given. Based on the information collected in this study, recommendations were made regarding equipment design, mechanical assists and procedures for material handling, training, etc., which, if implemented, will assist in reducing the frequency of back injuries in underground coal mining.

The report of work accomplished on the United States Bureau of Mines' bauxite project in Pulaski and Saline Counties, Ark., is presented in this volume and in a series of Reports of Investigations immediately following this volume. This first volume covers the project as a whole, whereas the subsequent volumes contain maps of local areas and deposits developed, together with logs of the drill holes and descriptions of the separate deposits. The Arkansas bauxite project was initiated in December 1941 and continued for 4 years. The first 5 months were spent in studying the problems presented and in geophysical exploration, Core drilling began in May 1942 and ended in May 1945. The project was one of the larger and more important of those undertaken by the Bureau of Mines for the purpose of developing domestic sources of supply of critical and strategic minerals necessary for National defense and for the successful waging of modern mechanized warfare. Some of the results of the Bureau's bauxite exploratory work have been made available to the public through the medium of press releases, transactions of technical societies, and government publications. More information has been supplied to government agencies upon request and to land owners and mine operators of the deposits concerned in the form of maps showing locations of all holes drilled on the property and logs with chemical analyses of' all cores recovered. In October 1946, the factual data were made avail-able to .the public in manuscript form in an-"open file" at the office of the State geologist, R. B. Foxhall, Little Rock, Ark., and at the office of the. Bureau of Mines in Washington. For the purpose of providing a background for the presentation of the factual data obtained, the history of bauxite is reviewed briefly. Information relative to past production and reserves of bauxite in Arkansas also is given. A resume of the development of the industry in Arkansas, the exploration and mining methods employed, the various uses to which the bauxite is put, and a brief outline of the geology of bauxite in Arkansas complete the -background.

Researchers studying dust generation from blasthole drills developed a simple, quick fix that reduced respirable dust concentrations by more than 63% at the dump point. This device, which requires almost no maintenance, is small and inexpensive, and it will help operators maintain compliance to the dust standard. The thought process was that a reduction of respirable dust at any of the multiple sources on the drill, in this case the dust collector dump point, should reduce the total respirable dust generated by the drill. The U.S. Mine Safety & Health Administration (MSHA) has a coal respirable dust standard of 2 milligrams per cubic meter (mg/m3) for an 8-hour shift. MSHA and mine operators periodically collect personal dust samples to determine compliance with this dust standard. Drill operators have one of the highest exposure levels to respirable dust. This has been proven through many reviews of the MSHA dust sampling database. Prior to 1987, the highwall driller at surface coal mines had the most severe exposure to respirable silica dust, a trend that continues today based on MSHA's respirable dust sampling database. A review of the MSHA database for the years 1985 through 1992 shows that 81% of the respirable silica dust samples taken exceeded the permissible exposure limit for the highwall drill operator at surface coal mines. A more recent review of the MSHA database, for the years 1996 through 2000, shows that only 31% of the respirable silica dust samples taken exceeded the respirable silica dust limit for the highwall driller at surface coal mines. This suggests a substantial improvement in preventing the exposure of silica dust to the highwall drill operator. The drill operator, however, still tops the list of job occupations that have the highest exposure to respirable silica dust Respirable dust has been shown to be harmful to human health, especially when silica is present in the dust. It causes a disease known as silicosis, which can occur in three levels: chronic silicosis, which occurs after 10 years of exposure; accelerated silicosis, which occurs between 5 and 10 years of exposure; and acute silicosis, which can occur within a few weeks to five years of very high exposure to silica dust. Respirable dust consists of the dust particle size fraction whose median diameter is 4 micrometers (µm). The drilling machine generates high concentrations of respirable dust from several sources: drill table shroud leakage, dust leakage through the table bushing, dust discharge from the dust collector exhaust due to impaired filters, and dust entrained from the dust collector fines dumped onto the bench. The National Institute for Occupational Safety and Health's (NIOSH) Pittsburgh Research Lab (PRL) has been studying different methods to reduce respirable dust from these sources. During its testing, the institute identified a simple solution that reduces the amount of respirable dust generated at the collector dump point.

A flat slot is created in rock to cause stress relief effects in a pair of hydraulic borehole pressure cells (previously installed in drill holes above and below the slot ) , which act as sensing devices . This relief is annulled by repressuring the slot by means of a flat hydraulic cell . The slot pressure at annulment is taken to be the existing ground stress at right angles to the plane of the slot . A number of trial installations in the rib of a mine drift showed that for a valid determination of ground stress the sensing devices that respond to relief and repressuring must be within a specified zone close to the pressure transmitter ; otherwise the indicated ground stress may be excessive . By using an additional pair of borehole pressure cells , installed a few feet away , the existing rock stress can be redetermined at intervals , irrespective of a change of stress intensity subsequent to the initial installation . Based on these findings , equipment and procedures have been developed that make possible the installation and operation of a permanent rock stress monitoring station for observing mining - induced stress changes . The station consists of a 12 - in - wide hydraulic pressure cell cemented in a flat slot and four 2- by 8 - in flat hydraulic pressure cells in nearby drill holes . All drilling for the installation can be done with an air leg drill and conventional bits . Once the installation is completed , all required information is obtained by Bourdon pressure gage readings . The apparatus is simple , relatively inexpensive , and reliable under operating conditions in a wet mine . Unlike most methods of rock stress determination , this scheme permits periodic remeasurement of absolute rock pressure over several years , without additional drilling or instrumentation . [Investigation of mine land characteristics, evaluation of irrigation systems, and evaluations by the mining industry, determined that irrigation is a feasible method for aiding revegetation of coal mined lands in western arid and semi-arid regions. The various regions studied displayed a commonality of characteristics which enables, independent of the site or region, the recommendation of solid set irrigation. This Bureau of Mines report describes the evolution of modern-day blasting agent and slurry products and the state of the art of the usage of these products as it exists today. The steadily increasing consumption of blasting agents and slurries at the expense of cartridged high-explosive usage is related. Although safety is not covered in depth, reference is made to pertinent safety literature available. Product formulations, detonation reactions, and detonation products are discussed. Product terminology is defined and the significant differences between the relatively insensitive modern-day dry blasting agents and slurries and cap-sensitive high explosives are described. Blasting agent and slurry properties are discussed, as well as methods of determining and describing these properties, and specific types of products are also discussed with reference to their ingredients,-properties, and field applications. Increasing consumption of slurries, particularly in small-diameter boreholes, and the development of more cap-sensitive varieties of slurries are forecast although dry blasting agents should continue to dominate the market. The consumption of cartridged high explosives will level off or gradually decrease.

To reduce U.S. dependence on imports of titanium, the U.S. Bureau of Mines has investigated the recovery of titanium from perovskite (CaTi03). Domestic perovskite deposits represent a significant, but untapped, titanium resource. An acid sulfation method was developed that will extract about 97 pct of the titanium and the columbium and 70 to 90 pct of the rare-earth byproducts from perovskite concentrates. A flow-sheet for recovering titanium as pigment-grade titanium dioxide (Ti02) is discussed. Test work performed in developing each unit operation is described. The first three unit operations have been demonstrated using laboratory-scale continuous methods of operation: acid sulfation of perovskite concentrates, leaching sulfation residue to dissolve metal values, and precipitation of titanium oxysulfate monohydrate (TiOS04?HP) from the resulting leach flltrate. Batch testing of other unit operations was performed: TiOS04?H20 redissolution and iron reduction, precipitation of Ti02 by hydrolysis, and regenerating recoverable acid with rare-earth byproduct recovery. Cost evaluation results are provided, indicating that processing perovskite concentrates by acid sulfation may be an economically viable means of producing Ti02.

The U.S. Bureau of Mines has prepared this report on strategies for maintaining high job attendance among underground coal miners because high absenteeism is a threat to miners safety and seriously hampers productivity. A substantial number of research studies on the effectiveness of various strategies for reducing absenteeism among the employees of nonmining industries have been reported in the literature. These strategies have aimed at improving job attendance through one or more of the following: (1) improving employment procedures, (2) overcoming problems that adversely affect one's ability to attend work, and (3) increasing miners motivation to attend work. Many of these strategies appear applicable to the mining industry, and are reviewed in the first half of this report. The second half of this report describes how one could develop and implement a program for maintaining high attendance at underground coal mines. The steps include: measuring and evaluating attendance levels; formulating attendance goals and an absenteeism policy; developing and implementing an attendance promotion program; and periodically going through the preceding steps known as recycling.

THE BUREAU OF MINES was established in the public interest to conduct inquiries and scientific and technologic investigations concerning mining and the preparation, treatment, and utilization of mineral substances; to promote health and safety in the mineral industries; to conserve mineral resources and prevent their waste; to further economic development; to increase efficiency in the mining, metallurgical, quarrying, and other mineral industries; and to inquire into the economic conditions affecting these industries. The organic act of the Bureau, as amended by Congress and approved February 25, 1913, made it the province and duty of the Bureau to "disseminate information concerning these subjects 'in such manner as will best carry out the purposes of this Act.'" In accordance with that directive, the Bureau reports the findings of its research and investigations in its own series of publications and also in articles that appear in scientific, technical, and trade journals; in proceedings of conventions and seminars; in reference books; and in other non-Bureau sources. The number of these reports, the wide range of subjects they cover, and the variety of mediums in which they appear make the kind of list and index presented in this special publication both necessary and valuable. This issue describes Bureau reports and articles published during the period January 1, 1960 to December 31, 1964. It, supplements the 50-year list of Bureau publications issued from July 1, 1910, to January 1, 1960,2 and the 50-year list of articles by Bureau authors published outside the Bureau from July 1, 1910, to January 1, 1960.3 It supersedes the annual lists of Bureau publications and articles from January 1 to December 31, 1960,4 from January 1 to December 31, 1961,5 from January 1 to December 31, 1962,6 and from January 1 to December 31, 1963.7 The leading general and technical libraries of the United States maintain files of the Bureau's publications. This report reviews the current state of the art of remote control techniques for underground continuous miners. A delineation of aspects requiring additional data is presented. Also a summary and delineation are presented in overview form for the related section functions: roof control, haulage, ventilation, power supply and rock dusting. The automated remotely controlled continuous miner project is a long-range research effort. However, the program has been divided into intermediate steps resulting in incremental production increases throughout the program. Data are developed relating the impact of our proposed research and development accomplishments to potential productivity impact.

In turn , a feeling among employees that the company really has their interests at heart has a wonderful psychological effect . It is bound to bring forth more and better work and to attract and keep workmen of the better class . There is keen competition between the various branches of this corporation for a trophy that is awarded each year to the one having the best accident record . This trophy was won by the Nacożari branch of the Moctezuma Copper Co. in Mexico in 1926 and 1927 , and the Morenci branch won the trophy in 1928 and 1929 .

Prier to the date of this contract, Ocean Energy had developed and demonstrated a high efficiency fluorescent caplamp which operated from a standard 4 volt miner's caplamp battery. This unit produced about five times the light output of the standard incandescent miner's caplamp. The Company believed that the major market for this unique caplamp was in the industrial and municipal applications. Because of the additional safety requirements for coal mining use, and in conjunction with the much smaller market in the mining industry, the additional expense and difficulty of modifying this caplamp to comply with the Federal and State coal mine regulations were not considered at that time. Recognizing the potential of this lamp for improving illumination conditions for underground coal mining, the US Bureau of Mines contracted with Ocean Energy to make the required modifications to this lamp to bring it into compliance with the Federal and State of Pennsylvania regulations, and thereby, make this lamp available to the anthracite and bituminous coal mining industry. At the outset of the contract, the injection molds for the caplamp housing were being completed, and only prototype, hand fabricated, units were available for tests. The Bureau evaluated these prototype units and formulated a performance specification for the miner's version. Light level and distribution, weight, size, angle of tilt, and operating duration were all specified. In addition, it was required that the caplamp be com¬patible with existin charging equipment at the mines and that the final design be in full compliance with MESA Schedule 6D and he ultimately approved by MESA, Approval and Testing, as well as by the Pennsylvania Bureau of Deep Mine Safety. The scope of this contract covered the modifications required to achieve these objectives. It was agreed by the Bureau that all rights to the lamp, including design, material combinations, circuitry, etc. would remain with Ocean Energy. The charger/adapter system and components, which were developed under contract funds, are disclosed hero and become public knowledge and property. Patent applications have been filed by Ocean Energy to protect the mechanical and electrical design aspects of the lamp. It should be noted that this report contains details, designs and data which may be covered by one or more Ocean Energy patent applications, and disclosure herein is made only to provide better understanding of the contract results.

Researchers at the National Institute for Occupational Safety and Health are investigating technologies that could be used to detect objects, small vehicles, and pedestrian workers that may be in the blind areas of haulage equipment used in mining and construction. A detection system that warns the equipment operator that there is an obstacle nearby could prevent collisions and save many lives each year. One popular technology for collision warning systems is radar. Several different types of radar have been tested in the laboratory and on mining equipment. Early in the study, questions arose concerning the best way to test radar systems. Many factors affect the performance of radar, including the size, shape, and composition of the object that is to be detected; the height of the radar antenna(s); and the relative motion of the radar system and/or object. This report discusses several different test procedures and test targets and recommends methods to determine how effective a radar system will be in detecting a person near heavy equipment.

As part of a cooperative effort between the Bureau of Mines and the Florida Institute of Phosphate Research (FIPR), with input from the phosphate industry, the conversion of phosphogypsum to sulfur is being investigated. The proposed process incorporates the thermal reduction of phosphogypsum to calcium sulfide (CaS) and a hydrometallurgical treatment to convert calcium sulfide to sulfur (S°). The research described herein is focused on the latter half of the process. It was demonstrated that calcium sulfide could be converted to ammonium bisulfide (NH4SH), and that ammonium bisulfide could be oxidized by air in the presence of a catalyst, to elemental sulfur, which was adsorbed on the catalyst. Two methods for recovering sulfur from the catalyst were developed: (1) thermal treatment to volatilize sulfur, and (2) leaching the sulfur from the catalyst with liquid anhydrous ammonia. Over 90 pct of the sulfur adsorbed on activated carbon (the catalyst) was recovered by both methods. Based upon the encouraging laboratory investigations, a hydrometallurgical scheme was proposed for the overall conversion of calcium sulfide to elemental sulfur.
