The The National Institute for Occupational Safety and Health (NIOSH) Pittsburgh Research Laboratory (PRL) is continuing to investigate the behavior of fully grouted roof bolts in the weak roof rock of the Safety Research Coal Mine (SRCM). This paper describes the results of three studies: • A series of 24 pull tests of bolts installed fully grouted and overcored to leave 12 inches of grouted bolt. The tests compared the pull-out performance of offset-head roof bolts with that of standard 5/8-inch bolts; • A second series of 24 tests that compared the pull-out loads obtained by bolts installed with 1 ft of resin (a Short Encapsulation Pull Test or SEPT) to that of fully grouted bolts overcored to leave only 12 inches of grouted bolt, and; • An investigation into the pressures generated during the installation of fully grouted resin bolts. All of the bolts installed and pulled were overcored and removed from the roof after the pull tests to allow inspection of the resin. The first study found that there was no significant difference in pullout load between the offset-head bolts and the standard bolts. The second study, comparing the SEPT bolts and the partially-overcored fully grouted bolts, indicated that SEPT can significantly underestimate bolt anchorage grip factor, suggesting that the SEPT is a conservative measure of actual bolt performance. Significant loss of resin to cracks in the roof was observed in the bolts overcored in the SRCM. The third study explored one possible explanation for the resin loss, high pressures generated during bolt installation. These tests employed bolts installed in strain gauged steel tubes. The results confirmed that significant pressures (greater than 4,000 psi) can be generated during bolt installation. The results of the pressure tests, combined with the observations of resin loss in bolts installed in the SRCM, suggest that under some circumstances resin loss and under-encapsulation could take place and lead to degraded bolt performance. Glove fingering was observed in all recovered bolts to various degrees. The limited comparative data suggested that the effect of glove fingering ranged from slight to moderate. Complete mixing of the resin was observed on all of the 40 bolts recovered (28 fully grouted and 12 SEPT). Australian and U.S. resins were compared and the Australian resins were observed to have much smaller catalyst compartment areas and lower resin viscosities. These differences are suggested as potential explanations for occasional resin mixing problems noted in Australian resin bolt installations.

The purpose of this report is to clarify the interpretation of some phases of previous progress reports5/ on the use or omission of stemming that have been brought to the attention of the authors by readers. As the stemming problem was approached from an entirely disinterested viewpoint and the data were presented impartially, results of investigations published in previous reports5/ were obtained under a definite set of conditions and procedure, and they may of may not be duplicated under different conditions and procedure in actual mining practice. Final conclusions regarding the use or omission of stemming in blasting have not been reached pending completion of all aspects of this investigation.

This chapter presents an overview of fatal and nonfatal injuries reported to MSHA during 1986-1995. Reportable injuries follow the criteria for reporting specified in 30 CFR 50. These criteria are listed in appendix A. The data used for these analyses came from the MSHA accident, injury, and illness data closeout files for the years 1986-1995. The MSHA data are described in appendix A and methods used in data analysis in appendix B. There may be slight discrepancies between the figures presented here and other presentations of MSHA data for the following reasons: (1) injuries to nonemployees have been excluded from analysis; and (2) injuries identified by MSHA after the files were closed out are not included. Within the MSHA data, there are differences in reporting requirements for operators and contractors. Contractors are required to report only those injuries that occur during certain categories of work (described in appendix A). For this reason, data on injuries among employees of operators and contractors in the MSHA database should not be considered directly comparable. As discussed in the "Introduction," the MSHA data on fatal injuries differ from the CFOI data (chapter 1) because of varying definitions used for the mining industry. As compared to the CFOI data, MSHA data exclude injuries that occurred off of mine property and include injuries occurring at mills and processing plants on mine property. Tables 4-1 through 4-9 and figure 4-1 provide comparisons of injury frequencies and rates among the commodities. These comparisons are followed by sections 4A through 4E, which provide more detailed tables and figures for each commodity separately. The method used to calculate injury rates is described in appendix B. Since denominator data for occupations (lob titles) were available for 1986 only, estimates of rates by occupation for the entire 10 year period are based on extrapolation from 1986 data. For this reason, estimates of injury by occupation should be interpreted with caution. Fatal Injuries A total of 1,078 fatal on-the-job injuries were reported to MSHA during the 10-year period 1986- 1995. The average annual fatal injury rate for all commodities combined during this period was 30.9 per 100,000 full-time equivalent workers. Mine operators accounted for 908 (84%) of the fatalities and had an average annual fatal injury rate of 28.2 per 100,000. While mine contractors comprised only 16% (n = 170) of the total number of fatalities, their average annual fatal injury rate was 2.3 times higher than that of mine operators (65.0 per 100,000). Table 4-1 presents the number and rates of fatal injuries by commodity for operators and contractors. Contractors in metal, nonmetal, stone, and sand and gravel are grouped together, since MSHA data on employment were not available for rate calculations for these commodities separately. The highest average annual fatality rate for the period, 66.5 per 100,000, occurred among metal and nonmetallic mineral mining contractors, followed closely by coal contractors (63.1 per 100,000). Among mine operators, the highest average annual fatal injury rate was in coal (38.9 per 100,000); the lowest was in nonmetal (14.3 per 100,000). Table 4-2 shows the average annual fatal injury rate by type of operation for each commodity for mine operators only. Of commodities with underground mines, all except stone had higher fatality rates at underground than at surface operations of these mines. The highest fatality rate for any operation occurred in surface operations at underground stone mines (124.5 fatal injuries per 100,000 workers). Stone had the second highest fatality rate for underground operations at underground mines (77.9 per 100,000). The highest rate for underground operations at underground mines occurred in metal (87.6 per 100,000). Table 4-3 shows the number and rate of fatal injuries by U.S. Bureau of the Census occupational groups (described in appendix D) for the 10 occupation-commodity groups with the highest fatal injury rates. These data should be interpreted with caution because the denominator used in calculating the rates was based on an extrapolation from data for the year 1986 only, as described in appendix B. Extractive occupations are represented in this group for three commodities: coal, metal, and sand and gravel. Construction trades are included for coal, sand and gravel, and nonmetal. The distribution of fatal injuries by commodity and work activity for mine operators is presented in table 4-4. A description of the work activity categories is in appendix C. Overall, the leading work activity at the time of injury was using or operating tools and machinery (26.4%) followed by constructing, repairing, and cleaning (24.0%), vehicular operations (18.8%), and materials handling (12.6%). The distribution varied by commodity. The largest percentage of fatal injuries occurred while operating tools and machinery in both coal and stone; during vehicular operations in metal; and during constructing, repairing, and cleaning activities in nonmetal and sand and gravel. Table 4-5 shows the distribution of fatal injuries by commodity and MSHA accident classification for mine operators. A description of the MSHA accident classifications appears in appendix E. In the course of the Bureau of Mines investigation relative to the causes, behavior, and control f anthracite-mine fires, attention was directed toward the correlation between the temperature of oxidation and the gaseous oxidation products in the range of temperatures between 100° and 350°C4/ These tests, made on a high-grade anthracite to determine the amount of oxygen consumed and the amounts of carbon dioxide, carbon monoxide, hydrogen and methane evolved when air was passed through samples of the anthracite at various elevated temperatures, showed that the gases evolved and their relative proportions vary with temperature, constitute a means for determining the probable temperature of the anthracite, and thus permit detection of incipient heating before a dangerous fire hazard exists. As the relationships mentioned are affected somewhat by the amount of previous oxidation of the coal, it was deemed desirable to know to what extent they might vary owing to this cause. It was also considered desirable to have a means for estimating the extent of oxidation from the gas analysis, as this would permit a more accurate estimate of probable temperature. Finally, it seemed desirable to know how much the composition of coal might vary over an extended period of oxidation, since this was previously suggested5/ as a possible means for estimating the probable maximum oxidation temperature to which a sample of coal had been subjected.

The Bureau of Mines determined the physical properties of southeastern Idaho phosphate fines as related to mineral composition, particle size and liberation, beneficiation, and potential use in structural clay products. The phosphate mineral in the fines was identified by unit-cell dimensions to be essentially the same as the phosphate mineral fluorapatite identified in the coarser sizes. The mean particle frequency as determined by electron-microscopy and pipet-analyses was between 0.5 and 0.2 micron. Liberation of the phosphate could not be definitely determined because of the weathered condition of the minerals and their extreme fineness. No beneficiation scheme was developed that would concentrate the phosphate into a marketable fertilizer product; therefore, forming and firing tests were made to demonstrate that the fines can be used to manufacture structural clay products.

This report is one of a series being prepared by the Bureau of Mines on exploration, development, mining methods, and costs at various open-pit mines in the United States. These reports have been prepared in cooperation with various mining companies. A similar program, conducted from 1928 to 1939, was generally conceded to have contributed to improving the efficiency of mining operations. The purpose of these reports is to promote development and conservation of our mineral resources by disseminating technological information on mining methods and practices. This report describes exploration, development, equipment, and mining by open-pit methods of the remainder of an ore body that had been previously worked by underground methods. The Tripp pit of the Consolidated Coppermines Corp. is located at Kimberly, Nev., a small mining company town having an elevation of 7,250 feet, 9 miles by road west of Ely, Nev. (fig. 1). The pit extends over the area formerly mined by underground methods from the Morris and Brooks shafts.

This paper is one of a series being prepared by the Bureau of Mines on methods, practices, and costs in various mining districts in the United States. The paper describes mining and ore recovery methods common to the Batesville manganese district, Independence County, Ark., and gives the costs of producing merchantable ore by three operators whose methods were more or less typical of those of many other local manganese producers. The operating costs reported were those of Leonard Baxter at mines on Brooks Hill for the calendar year 1958 and Arkansas Mining and Exploration Corp. at Section 16 mine and the Searcy group of mines and Miller-McGee Corp. at the Diener mine during the first 6 months of 1959. Manganese are was mined in the district as early as 1849 and almost continuously thereafter until August 1959, when termination of the Government's domestic manganese purchase program resulted in complete cessation of mining activity. During those years, production came from more than 189 mines and properties, of which only a few produced as much as 30,000 tons of merchantable ore.

A study of joints in Marathon County, Wis., was made to determine the orientation of regional paleotectonic stresses and to develop a method of using optical diffraction analysis (ODA) in studying joint orientations and spacings. Standard geologic and petrofabric techniques employing the use of a geologic compass and a Schmidt net were used in gathering, describing, and reducing joint orientation data. Analysis of selected outcrops with ODA indicates that ODA can be applied to study both joint orientations and spacings. ODA is shown to be a rapid method that can be used to complement standard petrofabric techniques, especially in the study of joint spacings.

To keep the mineral and allied industries informed of trends in the causes of accidents and to point out the need for corrective measures, the Bureau of Mines collects, analyzes, and publishes, periodically health and safety statistics. This report includes data for 1961 on injury experience, with related employment statistics, at coal mines in the United Status. These data are presented in tabular form under the following headings; General Injury Experience Tables 1 to 6: Current data on number of injuries, degree of injury, frequency and severity rates, average severity, and related employment and production data by work location.

Data obtained from field and laboratory examinations were used by the Bureau of Mines to evaluate 82 silica deposits in the Pacific Northwest. A total of 94 field samples was taken from 64 deposits, and an additional 18 deposits were investigated that did not warrant sampling. Of the 37 deposits found to be of high quality, only 16 were large enough to be classified as industrial silica resources. Another 27 deposits did not meet quality requirements. Methods of simple low-cost beneficiation and rapid classification of a large number of samples were investigated in the laboratory. These tests indicated that high-intensity electromagnetic separation was equal or superior to either high-or low-intensity electrostatic separation. In general, the electromagnetic method gave higher recoveries and produced equal or better grade concentrates. A glass button method developed for estimating iron oxide content of silica samples proved to be highly satisfactory as a means of classifying field samples and evaluating beneficiation tests.

THIS BULLETIN analyzes and summarizes research at the Bureau of Mines dealing with the preparation, properties, and reactions of iron carbides in the period 1948-60, part of which was sponsored by Wright Air Development Laboratories. The objective was to develop better catalysts for the synthesis of methane and higher hydrocarbons from hydrogen and carbon monoxide. The investigations resulted in the first adequate characterization of epsilon iron carbide, which later proved to be an important intermediate in the tempering of martensite in steel. At temperatures near 170° C, the carbon in carbon monoxide reacts nearly quantitatively with finely divided alpha iron to form an iron carbide in which the metal atoms occupy the positions of the close packed hexagonal arrangement with lattice parameters of ao=2.754 A and co=4.349 A. The carbide has a composition within 2 atomic percent of that required for the formula Fe2C, a Curie point of about 370° C, a specific magnetization of 160.4 ergs per gauss-gram Fe at 50° C, and a ferromagnetic moment of 1.70 Bohr magnetons. Epsilon iron carbide can be converted to chi iron carbide by thermal treatment at 343° C for about 70 hours. The chi iron carbide so formed has a Curie point of 247° C, a specific magnetization of 142.5 ergs per gauss-gram Fe at 50° C, a ferromagnetic moment of 1.72 Bohr magnetons per iron atom, and a paramagnetic moment of 5.5 Bohr rnagnetons per iron atom. Direct synthesis of this carbide from carbon monoxide at 240° C indicates a composition within 7 atomic percent of that required for Fe2C. Chi iron carbide can be converted to cementite at 550° C in about 315 minutes. The theta iron carbide so prepared has a Curie point of 210° C, a specific magnetization of 147.3 ergs per gauss gram Fe at 50° C, a ferromagnetic moment of 1.72 Bohr magnetons per iron atom, and a paramagnetic moment of 3.9 Bohr magnetons per iron atom. Pure chi iron carbide reacts to form theta iron carbide by the elimination of free carbon. In the presence of an excess of free iron in intimate contact with the carbide, chi iron carbide reacts with the free iron to form theta iron carbide. The second reaction proceeds at an appreciable rate at 320° C and can be used to snythesize finely divided theta iron carbide with no free-carbon impurity at low temperatures. The ease of the reaction in the presence of free iron suggests that chi iron carbide is an intermediate in the tempering of epsilon iron carbide to theta iron carbide as it proceeds in steels. Experiments on carbon steel show that the magnetic moment of the iron atoms in martensite is about 2.2 Bohr magnetons per atom, a value approximately the same as that of the iron atoms in alpha iron. The Curie points of epsilon iron carbide, chi iron carbide, and theta iron carbide were found -in various specimens of tempered steel to be 10° to 20° C lower than normal values. This shift is probably the result of manganese in solid solution. Preparations of various compositions of epsilon iron carbonitrides show many effects of extensive isomorphism between epsilon iron carbide and epsilon iron nitride. The action of carbon monoxide on finely divided alpha iron at 400° C results in free carbon deposits containing filaments about 0.1 micron in diameter and several times as long. The filaments are characterized by nuclei of dense material located midway from each end of the filament. Although carbiding to near completion at 240° C over periods of about 300 hours produces a preparation containing no appreciable percentage of epsilon iron carbide, very brief periods of carbiding of the order of a minute result in the formation of detectable epsilon iron carbide even at temperatures as high as 325° C. The data indicate that epsilon iron carbide is an intermediate in the formation of chi iron carbide during its formation from carbon monoxide in the gas phase. Attempts to convert cementite to chi iron carbide by cold work increased the Curie point but did not change the X-ray powder diffraction pattern.

It was the purpose of this contract to conduct research in several different areas of instrumentation used to determine the quantity of respirable dust in the mining atmosphere. One area of research was in evaluation of instruments including several respirable dust monitors, several types of photometers, and a QCM cascade impactor. An instrument evaluation chamber was developed to aid in evaluating the instruments in a dust atmosphere. Another area of research was investigations into new instrumentation concepts, such as a virtual impactor/photometer instrument, a new optical device which can be used as either an optical particle counter or photometer, a new personal respirable sampler, which employs an impactor instead of a cyclone as a preclassifier, and the development of a new cascade impactor, which deposits the particles uniformly on the impaction plate. Another area of research included the use of many of these instruments in two field studies, including a silica sand plant and a tungsten mine.

This report presents the results of a three-phase investigation of the feasibility of using cemented coal waste in active underground coal mines to prevent mine subsidence. Coal caste was broadly defines to include waste from both coal preparation plants and coal-fired power plants. Underground systems with potential for cemented backfill are determined through identification of current mining methods and available technologies for cemented backfilling and consideration of materials and systems constraints upon mining and backfilling integration technical concerns including operation, health and safety, materials utilization, environmental impact, applicability and techni¬cal status are assessed for six (6) systems which were identified as potentially feasible. Two (2) of these systems are conceptualized in a hypothetical case history and developed to further assess their operational and economic feasibility. Comparisons of costs among both systems and with costs of surface disposal are presented. Also presented are the conclusions reached concerning the present feasibility of using cemented backfill in active coal mines and potential avenues for further research. Appendices contain further consideration for design of cemented mixtures to be used in backfilling, the results of laboratory testing of mixtures rich varying concentrations c: waste products and cementing agents, and an annealed bibliography

Infrared and displacement studies were made of an unsupported roof in Kaiser Steel Corp.'s York Canyon coal mine, Raton, N. Mex. Monitoring of the roof continued until the roof over the area was caved. Loose slabs in the roof, about 6 inches thick, were detected by means of associated thermal anomalies, using remote-sensing infrared instruments. Displacement measurements taken during the study indicate that roof movement was cyclic in nature; the cycles appear to have been related to the mine work cycle (work days to idle days).

The Bureau of Mines inventoried the land in southwestern Alaska to classify ownership and availability and compared this land with known mineral terranes and mineral deposits. This report discusses and quantifies the potential for favorable location of mining claims for metalliferous (locatable) mineral deposits. Of the nearly 24 million acres that constitute the study area, approximately 3.6 million acres or 17% are available for mineral location, 3.4 million acres or 14% are available with restrictions, and 17 million acres or 71% are unavailable. Mineral terranes are known to underlie about 7.8 million acres or 33% of the study area; 3 million acres or 38% occur on land available for mineral location, 1.3 million acres or 17% occur on land available with restrictions, and 3.5 million acres or 45% occur on unavailable land. The State of Alaska has opened over 91 % of its land in the study area to mineral location; in comparison, the Federal Government allows mineral location on 0.07% of the land under its jurisdiction. There are 117 known mineral deposits or occurrences within the study area, including 77 lode deposits and 40 placer deposits. Of the 41 lode deposits occurring within a mineral terrane, 2 occur on available land.

The U.S. Bureau of Mines is conducting research on guidance systems for computerized haulage in underground mines. This report describes the process of component selection, installation and testing, perception algorithm development, and steering control algorithm development, to produce a real-time steering control system for an articulated haul truck for underground mining. Ultrasonic transducers were mounted on the truck to provide perception data by measuring the distance to the ribs. Linear potentiometers mounted inside the steering cylinders measured the steering angles. Algorithms, implemented on an inexpensive, robust onboard computer system, analyzed the data and controlled the steering via an electro-hydraulic proportional valve. The steering algorithm presented follows a zone defined by the relative position between the truck and a rib. Experimental verification of the ability to follow the zone in a test corridor on the Colorado School of Mines campus is presented. The automatic steering system successfully negotiated straight sections, curves, and the transition zones between straight and curves with the truck in first and second gears. The resulting real-time automatic-steering system can be applied to many underground articulated machines.

INTERNATIONAL conferences on safety in mines originated in 1912 when, in response to an invitation issued by the United States, representatives of Austria-Hungary, Belgium, France, and Germany participated in a conference held in Pittsburgh, Pa The record shows that a permanent organization was proposed to be developed through. diplomatic channels. However, the advent of war 2 years later prevented this proposal from coming to fruition. No further cooperation between the Bureau of Mines and a group having: similar interests in a foreign country took place until 1924, when formal cooperation with the Safety in Mines Research Board of Great Britain was established. Interchange of personnel and results of research under that arrangement have resulted in a number of publications in both countries and more rapid advance in knowledge than could have been obtained through the same efforts exerted independently.

Humankind's interest in measurement of human physical strength probably dates to the first humans. At that time, life was truly a struggle in which the fittest survived. To a great extent, fittest meant strongest. Interestingly, current interest in human physical strength in the workplace stems from 1970-1980s vintage research demonstrating that persons with adequate physical strength are less likely to be injured on physically demanding jobs. Survival in many modem workplaces may still be a case of survival of the strongest. There is, however, a flip side to this issue - that persons with limited strength are more likely to be injured on "hard" jobs. To address this problem, we can apply what we know about physical strength to job design. "Hard" jobs can be redesigned to be within the physical strength capability of most people. Since physical strength is important to these jobs, we must find ways to quantify it through testing. This publication concerns human physical strength testing. Its purpose is not to recommend any particular type of testing, but rather to describe the types of testing available and their uses. It is up to each individual user of strength testing to decide which testing technique is most appropriate for his or her particular application. This booklet discusses four types of strength testing: isometric, isoinertial, psychophysical, and isokinetic. Human Strength Before describing the different types of strength measurement, we must define the term "strength" and explain the concept of strength measurement. Strength is defined as the capacity to produce force or torque with voluntary muscle contraction. Maximum strength is defined as the capacity to produce force or torque with a maximum voluntary muscle contraction.(1,2) These definitions include some key words that must be explained. A voluntary muscle contraction is "voluntary." When a person's physical strength is measured, only the effort the person willingly puts forth at the time is measured. Thus, when we test a person's "maximum strength," we are not measuring his or her actual maximum, but some lesser value representing what he or she is comfortable expressing at the time with the existing equipment and environmental conditions. Interestingly, when researchers startled persons being tested (e.g., by setting off a starter's pistol behind them), they have found significant increases in measured strength.(3) It has been hypothesized that the lower strength displayed during normal testing provides a margin of safety against overloading and damaging muscle tissue. The test equipment and the tested person's familiarity with the process also influence the "voluntary" strength output. The interface between the tested person and the test equipment is particularly important. A poorly designed interface induces localized tissue pressures that vary from uncomfortable to painful. In this situation, testers are measuring voluntary discomfort tolerance - not strength. It is important for strength researchers to keep the "voluntary" nature of their data in mind when they are designing their equipment and protocols.
