THE PLANNED MAINTENANCE PROGRAM All concentrators should start with a planned maintenance program from day one. Every- body thinks they have a program in place before start-up, but in reality, most have only a fraction of what is needed to avoid the "Post Start-up Disaster" (PSD) - two years for start-up is not that uncommon. An effective program has all of the following: (1) Good people. (2) Area Maintenance. (3) Good PM Checklists. (4) Daily Work Schedules. (5) Written Work Orders. (6) Equipment Histories. (7) Backlog Control. The installation of a workable program takes hard work, talent, and commitment. You have to know what you are doing (hard work will not replace talent). Good People Good people are available in any location if you make the effort. Always check references carefully (both superiors, peers and subordinates). Was the last concentrator they worked at successful? Was it clean? Find out! Success or failure follows people around. I know people who are experts on PSD because they have lived through it three or four times. You need some college trained people with hands-on experience. There are no secrets, you have to make the effort. If you make a mistake in hiring (and you probably will), make the tough decision necessary to rectify it quickly. Area Maintenance Area maintenance refers to organization of supervision such that one person is uniquely accountable for every inch of ground and piece of equipment. The New York City Transit System is celebrating its Diamond Jubilee this year. In 1904, 9.3 route miles of New York's first subway was placed in operation in lower Manhattan. Since that date the system has grown to 230 route miles and 458 stations (See Exhibit #1). The age of the system immediately provides a clue to the nature of our construction activity. While we must always plan for the future and expand the system in the direction of the future needs in New York, a substantial amount of time and money must be channeled towards the modernization and maintenance of the existing system which provides vital transportation for 3-1/2 million passengers a day. This paper however will be directed to our New Route's Construction and, specifically, to three rock tunnel and cut and cover sections presently under construction. Currently, New York is engaged in the construction of about 9 New Route miles of structure. Briefly, the 3 Sections we will discuss are as follows: Route 131-A Section 5A is a 4 track structure in Manhattan and includes the new Lexington Avenue Station. This 1240 foot long section is located between Park Avenue and 3rd Avenue and is a combined cut and cover and rock tunnel section under 63rd Street. Route 131-A Section 5B is an intricate 2423 foot long 4 track rock tunnel section extending from 3rd Avenue to the FDR Drive under 63rd Street in Manhattan.

Two 6.17 m (20 ft 3 in.) diameter raises, concrete-lined to 5.49 m (18 ft) inside diameter, were completed in 1978 at the Monterey Coal Company Number One Mine near Carlinville, Illinois, by Frontier-Kemper Constructors. These two raises are the largest in cross-section yet completed by full-face upreaming, and the success of this project is expected to increase the application of raise-boring to coal mine shaft construction. WHY 6.17 M (20 FT) DIAMETER ? Raise boring is well accepted in the metal mining industry for a variety of applications, and is gaining in acceptance in the civil works construction industry. The use of raise boring to construct vertical openings for coal mines has not been as widely accepted, even though coal mining requires, per mine, the greatest number of openings to the surface of any underground activity. Several factors have influenced the low utilization of raise boring in coal mining: 1. Coal mine shafts must often be of large diameter to pass the required high volumes of ventilation air at acceptably low water gage pressures. Cross-sectional areas of finished shafts exceeding 23.23 sq m (250 sq ft) are common. 2. The life of ventilation shafts is comparatively long, and the typical sedimentary strata overlying coal seams normally require that the shafts be lined. Shafts on the intake ventilation circuit must have permanent water-tight linings. 3. Unconsolidated overburden at the surface is often thick, and as reaming cannot be safely utilized in unconsolidated material,

INTRODUCTION Much of present fracture toughness testing con¬cerns chevron notched specimens (Newman Jr., 1984) since these, in principle, require neither fatigue precracking nor more registration than failure load to determine fracture toughness. Two such specimens which can be made from diamond drilled rock cores with minimal machining are the Short Rod (Barker, 1977) and the Chevron Edge Notch Round Bar in Bend¬ing or CENRBB Specimen (Ouchterlony, 1980). Even if the precracking property is especially valuable for a material like rock, practice has made the relative advantages of these chevron notched specimens much less apparent (Ouchterlony, 1982; Ouchterlony and Sun, 1983). Enough remains, however, to have the Commission on Testing Methods of the International Society for Rock Mechanics (ISRM) focus on them as standard configurations in a "Suggested Method for Determining Fracture Tough¬ness of Rock Material", which is in preparation. This paper presents material which supports the ISRM effort: 1. An experimental compliance calibration of the Chevron Bend (CENRBB or CB for short) and re¬sulting formulas. 2. Specimen tolerances, evaluation formulas, and loading rate requirements relating to both Short Rod and Chevron Bend Specimens of suggest¬ed standard proportions. 3. Some fracture toughness values obtained with the Chevron Bend Specimen. This material is mainly based on two reports (Ouchterlony, 1984 and 1985). Studies of the northeastern Mississippi bauxites were initiated primarily for the purpose of ascertaining the quality and quantity of the ore. Subsequently, this ore was found to have a rather dubious economic value as was previously suspected but never established in earlier economic reports (1, 2, 3). However, the quality of ore was found to be higher than previously thought. Even though the ore bodies were determined during the early stages of this study to be too thin, small and widespread for commercial production a continuation of the study was not abandoned. On the contrary, the present investigation was intensified concentrating less on the economic application and more on stratigraphy and genesis. Considering the ever increasing demand for aluminum products the northeastern Mississippi bauxite district which includes portions of Tippah, Benton, Pontotoc, and Union Counties may soon become, in spite of inferior quantities of ore, an attractive area for bauxite production. Presently, however, the meager quantity of ore discourages exploitation. On the other hand, these northeastern district bauxites are facies of more extensive units of lignite bearing high aluminum clays, and considering the advances in aluminum extraction from clay these clays may in the future beckon mining interest. The bauxite of the northeastern Mississippi district, mapped as a part of the Betheden Formation (4). has an outcrop trend along the county lines of Tippah and Benton Counties and the extreme western portion of Union and Pontotoc Counties. The need for a separate ventilation department within the management team was recognized by the management of U. S. Pipe and Foundry before the company was purchased by Jim Walter Corporation in January, 1970. Because the coal beds of the deep underground coal properties owned by Jim Walter Resources lay 1300 to 2200 feet below the surface, efficient mine ventilation practices are essential through shaft openings varying from 16 to 22 feet in diameter. The extremely high methane gas quantity indicated to be in the coal bed because of the depth of cover and documented by various U. S. Bureau of Mines tests and demonstrated by early mining proved the need to implement this early management decision. At U. S. Pipe and Foundry mine ventilation was a function of the Mining Engineering Department with ventilation personnel assigned to the mine receiving their day to day work assignments from the mine manager or mine foreman. At Jim Walter Resources, the General Manager - Ventilation reports to the Senior Vice President, Mining Engineering, who reports directly to the President, Mining Division, Figure 1 (Company Organization Chart). Position descriptions are static; however positions are dynamic and consequently the degree of emphasis of specific duties and responsibilities change depending not only upon the aptitude of the individual performing the job but also upon his ability to meet the needs of his supervisors; and ultimately upon his ability to react to the economic forces directing our society. The following describes the duties and responsibilities of the General Manager - Ventilation, Figure 2.

INTRODUCTION This paper report presents the general case history for the construction of the North Outfall Replacement Sewer (NORS) project. A 1982 City of Los Angeles Wastewater Facilities Plan recognized that a 19.3 km (12¬mile) section of the North Outfall Sewer (NOS), from Culver City to the Hyperion Treatment Plant, was deteriorating and had insufficient capacity. Built in the 1920s of unreinforced concrete and lined with vitreous clay tiles, the originally unventilated NOS suffered considerable corrosion by H2S gas. This caused roof tile and concrete to fall to the bottom of the pipe, which, over the years, reduced the 11.9 M3/S (420 cubic feet per second (cfs)) design capacity of the NOS to a current level of approximately 8.5 M3/S (300 cfs). This worsening condition, thereby contributing to the polluting of Santa Monica Bay. This situation culminated in the State of California Regional Water Quality Control Board, Los Angeles Region, issuing a Cease and Desist Order on January 27, 1986, which required the City of Los Angeles to eliminate such dry weather overflows through a number of prescribed actions. One of these was to construct the North Outfall Replacement Sewer (NORS) by April 1, 1993. PROJECT DESCRIPTION The NORS Project involved the construction of over eight miles of tunnel and related structures. It consisted of a main tunnel conduit to convey sewage to the Hyperion Treatment Plant and four diversion lines connecting the main tunnel to major existing

The recovery of iron from low-grade (31.6% Fe) Muro iron ore was investigated using reverse anionic flotation of quartz. Tests were performed to investigate the effect of pH, the effects of using distilled vs. tap water as the flotation medium and the effects of using cassava starch vs. dextrin for the depression of iron minerals. The optimum results were obtained around pH 10 and 11, and cassava starch was superior to dextrin in depressing iron oxides. Investigations at pH 11 using cassava starch and distilled water resulted in a concentrate grade of 69.4% Fe with a recovery of 84.4% Fe. Under the same conditions, the use o f dextrin produced a concentrate grade q f 65.9% Fe with a recovery of 78.6% Fe. Investigations at pH 10 using cassava starch and distilled water resulted in a concentrate grade of 60.2% Fe with a recovery of 83.9% Fe, and under these conditions the use of dextrin produced a concentrate grade of 56.1 % Fe with a recovery of 81.5% Fe. Flotation studies conducted at pH 11 using tap water and cassava starch resulted in a concentrate grade of 70.4% Fe with a recovery of 92.7% Fe. This demonstrated the superiority of tap water over distilled water in the reverse anionic flotation process. Investigating the effect of calcium ions on the activation and flotation of quartz with oleic acid revealed that a calcium ion concentration of 50 to 100 ppm was optimum.

Underground long-range planning and mine design is an iterative process beginning with a specified production level, quality criteria, and geology of the deposit. Given these factors, face equipment is selected, the number of units needed and production per unit are calculated, and the mine infrastructure needs are developed. The scheduling of units then follows, with the design of the various support systems coming next. The support systems needed to maintain production can be grouped as follows: (1) outby haulage of mined material, (2) supply and manpower haulage, (3) ventilation and associated environmental considerations, (4) drainage, fresh water, and related considerations, (5) rock mechanics, and (6) power, supply. Thus, long-range planning may be viewed as a three-tiered structure, with the data on required production, product quality, and geology at the upper level; the development of face production capacity and the scheduling of units sufficient to meet the needs of the first two at the second level; and the development of support operations for the production units at the third level. Changes at any level may affect not only the items on that tier but may also cause a change to the items on the tier above and below. For example, if the rock mechanics data indicate that desired opening sizes will not be stable, then this situation will affect not only the ventilation design but also the production rate and possibly the selection of equipment itself. These, in turn, may affect the scheduling of units and the number of units, ultimately influencing the entire third tier of systems.

The North Shore Inline Interceptor, Phase IA tunnel is a 8,550-meter (28,000-ft) interceptor tunnel currently being constructed by Traylor Bros./Frontier-Kemper, Joint Venture, using a 9.8-meter (32-ft - 4-in) diameter Robbins tunnel boring machine (TBM). This tunnel is being driven in Silurian age dolomite approximately 91.5 m (300 ft) below ground surface. Significant water inflows were encountered when the tunnel drive penetrated the thinly-bedded Waubakee Formation. Flow increases approaching 0.75 cu m per min per 30 m of tunnel (200 gpm per 100 ft) have occurred in this formation. After driving approximately 2,285 m (7,500 ft) of tunnel (200 m full face in the Waubakee Formation), total flows exceeded 12 cu m per min (3,200 gpm) with about 5.7 cu m per min (1,500 gpm) entering the tun¬nel from the 200 m (600 ft) of exposed Waubakee Formation. Additional water control measures consisting of increased pumping capacity, increased water treatment capacity, surface grouting ahead of the tunnel, grouting ahead of the tunnel face and panning to improve the working environment for installation of tunnel support, were implemented to deal with the large water inflows. Currently (January 24, 1989), 3,400 m (11,000 ft) of tunnel has been driven, total water inflows are in the range of 11.7 cu m per min (3,100 gpm) with only 6.8 cu m per min (1,800 gpm) inflow from about 1,220 m (4,000 ft) of tunnel driven full face through the Waubakee Formation.

INTRODUCTION Virtually every sector of society in our country feels the impact of tunnels, either through the transportation of water, commodities, or people. The economic implications are staggering. The Committee on Rapid Excavation (1) reported that during the near future, 1968-1975, underground excavation activity will comprise 946 miles of tunnels (subways: 85 miles; underground urban highways: 123 miles; intercity highway tunnels: 210 miles; water supply tunnels: 528 miles) and 5,119 million tons of crude ore; metals: 874 million tons; nonmetals: 899 million tons; and coal: 3,346 million tons. It is estimated that the cumulative demand for underground excavation in the decade 1970-1979 will be 13 billion dollars for construction-oriented demand and 16 billion dollars for mining-oriented demand. The respective figures for the decade 1980-1989 are estimated at 22 billion dollars and 18 billion dollars. Of the many advantages claimed for tunnels (2, 3, 4, 5) the environmental considerations are becoming increasingly important. Transportation tunnels particularly for rapid transit systems, offer reductions in noise pollution and air pollution while improving aesthetics and optimizing land use (fewer problems of right-of-way and disruption of utilities). Considerable interest has been shown recently in sub-surface construction and mining (6, 7, 8). Recognizing the future demands for tunnels has led to considerable study of methods to improve rates of tunnel advance per day, safety, and the overall economics. The Federal coal Mine Health and Safety Act of 1969 has been criticized by those who say it goes too far in controlling the activity of private enterprise and by those who say it does not go far enough in eliminating fatalities and accidents in an accident prone industry. Possibly a better evaluation of this legislation lies somewhere in between. From the viewpoint of the coal industry it can be said that through the efforts of those engaged in mining, both the operator and the miner, the year 1969 marked a record year in mine safety with only 190 fatalities and a frequency of 0.84 per million man hours exposure. This outstanding achievement compared to any previous was the result of a determined effort on the part of a concerned industry to improve its reputation as being the most hazardous industry in the nation. After the effective date of the Act, March 31, 1970, the Bureau of Mines assumed complete control of all safety and health activities of the mining industry. Unfortunately, there was no improvement in the safety record of underground mines in 1970 with the total of 255 fatal injuries and a frequency of 1.04 for the year. The number of fatalities declined to 177 in 1971 but the number of underground coal mines had declined from 5,955 to 2,268 during the two year period. Meanwhile, surface mined coal increased from about 38% of total production in 1969 to over 50% in 1971, keeping the annual production of coal at a normal level but involving fewer men.

Ball clay production in 1991 amounted to about 800 kt (880,000 st), about the same as in 1990. 1992 estimates show a 5% decrease to 750 kt (827,000 st). This decrease continued a trend of reduced production that began after the peak of 900 kt (1 million st) that occurred in 1988. Use of ball clay in sanitary ware was about 2% less in 1992, although levels of use in floors and wall tile, dinner ware and pottery remained constant. Filler, extender and binder applications continued to be important end uses, accounting for about 17% of total market. Refractory uses maintained their market share of about 17 kt (18,700 st). Use of clay liners in toxic and waste dumps is an application widely found in Germany. However, its use has not been fully exploited in the United States. This could represent an opportunity for domestic clay producers. Tennessee remained the leading source of ball clays. Other producers were Indiana, Kentucky, Mississippi and Texas. Tennessee's production, however, decreased about 3% in tonnage produced and its value dropped from 68% to 65% of total production. The ceramics division of Southern Clay Products, Inc., was bought from LaPorte plc by United Clays of Texas, Inc., a US subsidiary of Watts Blake Bearne & Co. The purchase included ball clay reserves at Troup, TX. The acquisition makes United Clays the second largest ball clay producer in the United Sates. Introduction The Gold Quarry gold deposit is the largest bulk tonnage epithermal gold deposit in northeastern Nevada to be developed by Newmont Gold Co., formerly known as Carlin Gold Mining Co. Exploration and development of the deposit extended over a period of nearly 50 years before bulk tonnage mining became economically feasible. Proven reserves at Gold Quarry are currently estimated at more than 255 t (8.2 million oz.) of gold, accounting for more than 40% of the total Carlin Trend mineable reserves for 1986. Milling began in late 1985; dump leaching of lower grade ores began in May of 1986. The mill has exceeded the initial design throughput rate of 6.3 kt/d (7000 stpd) and is now processing more than 8 kt/d (9000 stpd). The dump leach facility will process more than 7.3 Mt (8.1 million st) of lower grade ores in 1986. The Gold Quarry ore body is to be developed by an open pit that will eventually be 1830 m (6000 ft) long, 1220 m (4000 ft) wide and 300 m (1000 ft) deep. The mine currently produces 77 kt/d (85,000 stpd) on a 24-hour-a-day, seven-day workweek. Planned pit production will increase to nearly 168 kt/d (185,000 stpd) in 1988. Production equipment currently includes one Demag H-/285, three Komatsu PC-1500 hydraulic shovels, two Cat 992C loaders and 21 90 to 135 t (100 to 150 st) haul trucks. A portion of the Massachusetts Bay Transportation Authority's Red Line Extension through Somerville, Massachusetts consists of 1,553cm (5,092 LF) of hard rock tunneling 6.7m (22 ft.) diameter horeshoe, 148m (485 LF) of mixed face tunneling, 7.2m (23 ft.) diameter circular, beneath an active railroad in a heavily commercialized business district. This paper will deal with the excavation of the mixed face area without the use of a shield. It will address the special excavation and support problems encountered during tunneling operations using the floating crown bar and modified spiling methods. Topics covered will include excavation and support methods, cement grouting, compaction grouting, and instrumentation. PROJECT DESCRIPTION The Davis Square to Porter Square Tunnels and Shafts Somerville/ Cambridge, Massachusetts is part of the Massachusetts Bay Transportation Authority's (MBTA) 5.1km (17,000 ft.) Red Line Extension. This extension runs from Harvard Square, Cambridge, to Alewife Brook Parkway, Cambridge through Somerville. This portion (MBTA Contract #091-106) required the excavation and concreting of 1,701m (5,580 LF) of single track twin tunnels consisting of 1,553m (5,092 LF) of conventional hard rock tunneling under 56 residential properties and 148m (485 LF) of mixed face tunneling beneath an active railroad in a heavily commercialized business district. The mined diameter of the mixed face tunnel is 7.16m (23.5 ft.) Access to the tunnel was via the Summer Street vent shaft, and the two Grove Street access shafts where all tunnel excavated materials were removed.

INTRODUCTION The Newtown Creek Pollution Control Project (P.W. 53) is a major system of facilities designed to collect wastewater from large areas of the Boroughs of Manhattan, Brooklyn and Queens, then channel it to a treatment plant where the wastewater is processed and discharged. The North Branch interceptor collects wastewater along the east side of Manhattan and transports it downstream to a Junction Chamber at Avenue "D" near East 13th Street. The South Branch interceptor similarly collects wastewater from the southern part of Manhattan Island on both east and west sides of Manhattan and transports it to the same Junction Chamber from where both flows are delivered to the adjacent Manhattan Pumping Station which pumps the wastewater under the East River to a treatment plant in Brooklyn. This system is shown in Figure 1. Construction of the Manhattan Pumping Station began in the Summer of 1966 and was delayed by a number of mishaps. In the vicinity of the North and South Interceptors, approximately 3,060 cubic meters (4,000 cubic yards) of soil flowed into the excavation: 765 cubic meters (1,000 cubic yards) at the North Branch Interceptor, and approximately 2,300 cubic meters (3,000 cubic yards) at the South Interceptor. These movements were accompanied by surface subsidence, collapse, or both. On June 4, 1973, while pumping station construction was underway, approximately five to seven cubic yards of sand entered the junction chamber through a temporary bulkhead valve in the North Branch Interceptor.

These encompass all adverse effects of oil shale mining, processing, and ancillary developments on the environment. Specific problems concern: land, vegetation, wildlife, water, air, population growth, aesthetics and public emotion; and fall into four categories of: land, water, air and emotion. Land includes excavations, spent shale disposal, loss of vegetation and impact on wildlife and domestic grazing, plant sites, buildings and roads, necessary urbanization and the total aesthetic or visual Impact. Water considerations are: requirements for Industrial and domestic supplies, adverse effect of underground water on mining, avoidance of pollution or adverse effect on surface or near-surface water, and the utilization or safe disposal of liquids. Air problems include dust, smoke and stack gas, Public emotional problems include reaction to the above problems, and misconceptions of oil shale country and an oil shale Industry. Government and industry are aware of the problems, and are seeking solutions. Last year, the three affected states studied oil shale environmental problems and solution costs. Colorado estimated environmental costs would add approximately 7% to construction and 7% to operation. The report suggested research in: (1) Water availability and impact; (2) Disturbed lands revegetation and rehabilitation, including spent shale; (3) Environmental inventory and impact; and (4) Regional development and land use planning. Governor Love appointed a 47-person committee representing Industry, government and environmentalists to attack the problems. Research pro-posals were prepared to focus on each problem. $715,000 state, federal and industry money is being committed. Results should provide data for industrial solutions and equitable agency administration.

The mine plan for an underground longwall mine plan usually requires a longwall panel conveyor that begins operation at its maximum length and gradually shortens over the life of the panel. Current mine plans around the United States require initial panel conveyor lengths of up to 4500 m (15,000 ft) and capacities of up to 3.2 kt/h (3500 stph). Because these conveyors are constantly being moved from panel to panel, their components must be as compact and light as possible, and the design must be flexible enough to be modified as new terrain and conditions are met. In the past, when drive horsepowers or belt tensions were exceeded due to conveyor length, a new flight would be added, including additional belt storage units. In some cases, this led to as many as three flights installed in a single longwall panel. As the longwall retreated through each flight, mining was required to stop during removal of the drives and storage units. This sometimes meant several days of lost production. Intermediate drive technology was developed to eliminate this excessive downtime and still keep conveyor belts, motors, pulleys and other components as small as possible. With intermediate drives placed along the carrying side of the conveyor, belt and motor sizes are kept as small as possible, and only one storage unit is needed. Most importantly, as the longwall retreats through a booster drive station, it can generally be removed in a regular downtime slot and no additional production time is lost.

Over the past several years, computerized financial models have been an important part of Newmont's strategic planning process. Two applications will be discussed in this paper: a general purpose financial model of Newmont, and a detailed operational/financial model of Magma Copper Company. Newmont?s general purpose financial model was developed using Lotus 1-2-3. It was designed to aid management in assessing the financial consequences of investment and financing decisions associated with long-term strategic planning by simulating the consolidated effect from individual transactions. Not only is the model capable of assessing the sensitivity of specific decisions to changes in the external economic environment (i. e., commodity prices, interest rates, inflation and exchange rates), it can also evaluate the effect from operating and financial decisions, including changing production levels and cost structures at individual operating units as well as simulating several types of transactions, including equity and debt issues, acquisitions and divestitures. The analysis of Magma Copper Company required the development of a detailed operating and financial model of the Company. The model was developed using IFPS in the corporation?s UNISYS (then Burroughs) B7900 mainframe computer and included separate pro forma financial statements for each of San Manuel-Pinto Valley's mines and concentrators, as well as Magma's smelting and refining operations. The outcome of this analysis was the decision to merge Magma Copper and Pinto Valley Copper Corporation, and to recapitalize the new corporation in order to provide funds for a retrofit and expansion of its Arizona smelter and expansions of its refinery and SXEW facilities.

INTRODUCTION The discovery of oil and gas reserves in the North American Arctic has led to substantial thought being given to their development. Systems adequate for exploration are not always, or even often, suitable for permanent installations. Combinations of offshore pipeline, drilling ships, platforms, sub-sea systems, sand islands and -ice island bases have been proposed. However, conditions in the Arctic are such that these methods may be difficult or impossible to implement. Problems exist with (1) the freezing of the Arctic Ocean surface for 9 - 10 months per year, (2) the movement of sea ice, (3) scouring of bottom sediments by ice islands and icebergs to great depths below the ocean bottom and (4) deep water channels. Any method which can avoid the problems associated with surface construction in these very adverse conditions merits detailed examination. One of the proposed solutions is to tunnel from a convenient or available access point to a point within the field, construct a chamber large enough to accommodate the drilling equipment and drill the development wells in a protected environment. Service and delivery lines would use the access tunnel and further tunnels and chambers would be constructed in a radiating pattern to develop the whole field. In order to determine the feasibility of underground construction in the North American Arctic, it is first necessary to understand the geologic and environmental factors which would affect such construction, to be knowledgeable concerning the problems and then to consider the techniques appropriate to the conditions which will be encountered, bearing in mind the need to minimize both con-
