This paper discusses a study performed to evaluate the feasibility of transferring formal system safety assessment technology to the mining industry. A representative listing of formal techniques was evaluated to select the most useful methods for the mining industry to implement. Criteria for selecting methods included the interests, needs, and requirements of mine safety officials. This paper describes one of the selected methods, preliminary hazards analysis (PHA), and illustrates the analysis procedure for a mining example. This study also includes an assessment of the adaptability of selected methods to a user-interactive computer system for use by mine safety personnel. This paper includes a description of a conceptual computerized PHA-type safety program that facilitates performance of the safety analysis and can accommodate updates and revisions as necessary.

INTRODUCTION THE Wisconsin zinc district, or the Upper Mississippi lead. and zinc district as it is also termed, lies in the southwestern corner of Wisconsin, and embraces adjacent portions of Illinois and Iowa. It includes Iowa, Grant, and Lafayette counties in Wisconsin, and Jo Davies county in Illinois. In Iowa, Dubuque formerly produced considerable lead, but there has been little mining there in recent years. The map, Fig. 1, shows the entire district, with roads, railways, streams, and the various towns, or "camps," around Which the mines. are clustered. The hatchwork indicates the areas iii which lead and zinc have been mined. The district is about 60 mi. (96 km.) north and south and about 40 mi. (64 km.) east and west. Not all of the territory is mineral bearing, and there are considerable areas that are believed to be barren. The grouping of the mines is patchy and irregular, though certain broad zones of mineralization extend almost the full width of the district, as from Mineral Point west through Mifflin and Livingston, and from Shullsburg west through New Diggings and Hazel Green. It is worth noting that the best mines of the district have, to date, been found within 10 mi. east or west of the fourth principal meridian line, thus lying within a long, narrow, north-and-south belt extending the length of the district. In a field as large as the Wisconsin, which has only within com-paratively recent time been worked intensively for zinc, the. chances are good for, opening new and important orebodies in parts of the district not yet, or insufficiently, prospected. However, the prospecting has been most successful around, or adjacent to, the old lead diggings of nearly a century ago, and the camps that were well known then are still centers of activity. In their order of importance they are: New Diggings, Benton, Mifflin-Livingston, Galena, Linden, Hazel Green, Shullsburg, and Highland.

Microscopic pyrites are present in all West Virginia coals. Sizes of these pyrites range from 0.1 µ 500 µ (1/2 mm) , and four types are noted: massive, patches, framboids, and isolated crystals. The relative and absolute abundances of the four types vary from seam to seam, and these distributions have a marked effect on the ease with which a seam may be cleaned of pyritic sulfur.

NEW YORKERS look forward to the third week of February as the time of the year when they can count on seeing their friends-from far and near gathered in the city for the four-day annual session of the Institute.., Indeed, the metropolitan district is so large that in engineer who lives in New, Jersey and has his office on the lower end of Manhattan island may easily pass a whole year without seeing an acquaintance who lives in Westchester and has an office near Forty-second Street. If the Institute served no other function than to provide a place where friendship may be renewed, ideas exchanged, and new acquaintances made it would still be worth while. But when about 30 technical sessions at which about 160 papers are read and discussed are compressed into four days the annual meeting is also entitled to high rank as an intensive course of post-graduate instruction in the newest development in mining and metallurgical technology. Last, but not least, it is the occasion of a good time for one and all, even if they do get more than a little behind on sleep before the end of the fourth day. Nine o'clock is the time set on Monday morning for the beginning of registration, but a half-hour before that a number have always 'put in an appearance, and this is fortunate, because by 10 o'clock four sessions had begun, those on mining methods, geophysics, a round table on the valuation of coal, and the session of the Local Section delegates. Three committees of the A. S. T. M. were also in simultaneous session, so the meeting may be fairly said to have started "in high."

MATERIAL used throughout this investigation was high-purity aluminum (99.998 pct). The 1/2-in. cubes were cut out of a cold-rolled slab and annealed at 550°C for 1 hr before deformation. The single crystal specimen was 1 in. in diam x 0.33 in. thick, cut from a cylindrical crystal produced by cooling at about 55°C per hr in a furnace which had a convenient gradient of about 10°C in every 4.5 in. vertical crucible length. Specific data on the ten specimens studied are summarized in table I. Three types of deformation were employed: (I) compression in a hand-operated hydraulic press; (2) impact with a 7-lb sledge hammer; and (3) rolling, using a wedge-shaped specimen' to obtain a gradient of deformations varying from 0 to 50 pct along the length of the wedge. Specimens were either deformed at room temperature, or first cooled in a freezing mixture of dry ice and ether at a temperature of —72°C and then deformed between steel blocks cooled with dry ice. The specimens deformed at the low temperature were ground and polished through 000 emery paper and wheels impregnated with alundum and alumina No. 1. During this entire procedure the specimens were dipped frequently in the freezing mixture of dry ice and ether in order to maintain constantly a frosty surface as insurance against overheating. All specimens were given a final electrolytic polish, using two different electrolytes* to insure that the surface effects observed were not char- acteristic of the polishing technique. With both electrolytes, the annealed specimens gave even surfaces with good polish. Several standard etchants were tried without success. The best results were obtained with an immersion etchant, developed in this connection, containing: hydrofluoric acid, 4 parts; nitric acid, 1 part; glycerine, 3 parts. Discussion of Results Typical strain markings have been observed within deformation bands, as shown in figs. 1, 2, and 4. By stereographic plotting of metallographic measurements, the strain markings within a deformation band in a single crystal of aluminum deformed by compression were found to be traces of (111) planes within a maximum scatter of 6". The edges of the deformation band were found to be traces of the (100) planes also within the maximum scatter of 6" (in agreement with previous workers). At 30 pct deformation, this scatter was considered quite normal because of the fragmentation and rotation of the crystal. This solution, based on the traces on one plane of observation is not unique, but as the planes were observed to be those normally and logically to be expected, further work in this regard was not felt to be necessary. The study of strain markings in a wedge-shaped specimen, rolled to obtain a gradient of deformation from 0 to 50 pct, revealed prominent strain markings at 25 pct reduction in thickness (fig.5). Faint and weak strain markings were visually observable up to a minimum reduction in thickness of about 12 pct. Since strain markings have been observed in other face-centered cubic metals after much smaller deformations;1, 2 and since the rate of recovery in aluminum is high, it is believed that, at lower deformations, self-recovery removes the localized strains at the slip interfaces, thus removing the source of the etching effects that give rise to strain markings. At high magnifications, strain markings sometimes seem to appear as very thin, light-banded re-gions bounded by a pair of dark edges. This so-

The rules of solution of oxygen from H2O-H2-He gas and of nitrogen from N2-H2 gas in shallow melts of liquid iron were measured at 1610o and 1600o C, respectiuely. Concentration profiles were detemined in the liquid iron. Tire rate data indicate that the solution process is controlled by diffusion in the iron melt. The diffusivities for oxygen and nitrogen in liquid iron, as calculated from the present data, are DFe-o = (12 ± 3) < 10-5 sq cm per sec and DFe-N = 11 ± 2) X 10-5 sq cm per sec at the temperatures employed. AN attempt was made by Shurygin and Kryukl to measure the diffusivity of oxygen in liquid iron. In their experiments a silica disc was rotated in liquid iron containing oxygen, and the rate of formation of liquid iron silicate was measured. Assuming that the rate of dissolution of silica is controlled by diffusion of oxygen in the iron, the oxygen diffusivity was computed from the rate data giving Dfe-0 = 6.1 X 5 sq cm per sec at 1600°C. Although this value seems to be of the right order of magnitude, there is no proof of the correctness of the assumptions involved in the interpretation of these rate data. The oxygen concentration in the iron at the iron-iron silicate interface was taken to be that in equilibrium with the silica-saturated silicate melt. That is, it was assumed that no concentration gradient existed in the liquid silicate. This is a questionable assumption, unless it is proved that the thickness of the silicate layer is very much smaller than that of the diffusion boundary layer in the iron. Furthermore, Shurygin et al.1 used the Levich equation2 to interpret their rate data. This equation was derived for mass transfer between a solid disc and a single-phase liquid. The hydrodynamic and diffusion boundary layers in the iron stirred by a disc, via coupling of the silicate melt, may be appreciably different from those predicted by Levich&apos;s derivations. In the present work the diffusivities of oxygen and nitrogen in liquid iron were measured at 1610" and 1600oC, respectively. EXPERIMENTAL METHOD Iron melts contained in high-purity gas-tight alumina crucibles were reacted with H2O-H2-He gas for the determination of the oxygen diffusivity and with N2-H2 gas for the determination of nitrogen diffusivity. At the end of the reaction period, the samples were quenched in a cold H2-He gas stream at the top of the furnace. Oxygen or nitrogen contents in the iron were determined by chemical analysis. Two different types of diffusion experiments were perforxed. To determine concentration profiles, a few rate measurements were made using 4-cm-deep melts. The solidified samples were sliced into discs and each disc was analyzed for oxygen or nitrogen. In another series of experiments, oxygen or nitrogen was diffused into shallow melts (about 0.5 to 1 cm in depth) and the total sample was analyzed to obtain an average concentration of the diffusate. In most experiments, 4- to 5-mm-ID alumina crucibles were used. Some experiments were also made in smaller (3 mm) and larger (7 mm) diam crucibles. This variation in diameter caused no difference in the reaction rate, within the limits of experimental uncertainty. To promote the establishment of a stable density profile in the melt, all the samples were suspended in the lower end of the hot zone so that the top of the melt was hotter by a few degrees. Molybdenum wire resistance heating was used. The reaction tube of the furnace was a gas-tight recrystal-lized alumina tube. In most experiments the furnace was heated by an ac power supply. To check the possibility of inductive stirring, some experiments were carried out in a dc operated furnace, with essentially the same results. The temperature of the furnace was controlled automatically in the usual manner. The temperature was measured with a Pt/Pt-10 pet Rh thermocouple and is estimated to be accurate within ±5°C. The iron used was prepared by melting and vacuum-carbon deoxidizing electrolytic "Plastiron" in a zir-conia crucible. The main impurities are: Si 0.004 pct P, S <0.002 pct Cr 0.005 pct N 0.001 pct Zr 0.002 pct O 0.003 pct Mn 0.004 pct C 0.002 pct The gas composition was controlled by constant pressure head capillary flowmeters. Oxygen was removed from the gas mixture by passing it through columns of platinized asbestos (450°C) and anhydrone. Selected H2O contents were obtained by passing the purified gas through oxalic acid dihydrate-anhydrous oxalic acid mixtures held at constant temperature in a water bath. Water vapor pressure data for the oxalic acid dihydrate-anhydrous oxalic acid equilibrium were taken from the 1iterature.3 The flow rate used was about 1.5 liters per min. The whole system was checked for tightness at regular intervals.

Y.-In recent years the metallurgical field of the copper industry has expanded greatly, the copper ores have become lean and diverse in character, and we are obliged to treat such ores on a very large scale. On the commercial side, the operating and consulting staffs of ,the great mining companies have been enlarged and organized, and no great capital expenditure is decided upon until it has been investigated and recommended by a highly organized force of specialists as a committee. I believe that in the scientific field and in the most valuable proceedings of our various societies the old idea of one-man point of view and one-man treatment of principle and detail together has been. splendidly developed and should continue, but for the advancement of our art we should deliberately discuss subjects among ourselves in order to do away with the idiosyncrasies of the individual and to gain a composite view representing the best thought of many men. In the development of new ideas leading to the introduction of new methods, the societies could enlarge their present great field of usefulness if such discussion be had, and in my opinion further great good could be done if a number of men of different qualifications and experience would join in preparing a joint book, each dealing with his particular problem, but consulting the other men. In this way individualism would be preserved and various processes would be described, and if properly reviewed the paper would be palpably capable of analysis, and the student would have at his command the associated efforts of a group of qualified engineers.

My first paper dealing with the theories of the hardening of steel by rapid cooling was published in the Transactions of this Institute in 1896— 30 years ago-under the title "The Microstructure of Steel and the Current Theories of Hardening," hence, also, the title of the present paper. It was extensively discussed and was, I believe, one of the starting points of many subsequent papers and discussions of the same subject, dis-cusssions which at times became almost violent and which led to the formation of different schools such as the "Carbonists" and the "Allo-tropists." Some of the older members of the Institute, who took part in the debate or who were interested observers, will, I am sure, have kept a vivid remembrance of it. The controversy gradually quieted down, apparently for lack of ammunition, without any side acknowledging defeat. Recently, however, interest in it ha been revived through the claims of some that these vexed questions had at last been solved. In an effort to ascertain if possible the prevailing view of those best qualified to express an opinion a questionnaire was submitted to the following gentlemen: J. 0. This paper supplements one appearing in Vol. 139 of the "Transactions," page 87 (1940). Because that paper will be issued soon, only a summary is given herein.

This paper describes the installation and operation of a Texas Nuclear on-line silica analyzer (NOLA) coupled with a Leeds and Northrup Microtrac particle-size monitor (Microtrac) at U.S. Steel&apos;s Minntac taconite concentrator. An automatic sampling system provides the NOLA with samples of final concentrate. After the sample is analyzed for silica by the NOLA, it is pumped to the Microtrac for size and surface area measurement. A sample is analyzed every six minutes, providing the control room with the silica concentration, the percent passing 270, 400, and 500 mesh, and an equivalent Blaine surface area number. When fed a slurry of final concentrate containing between 20 and 70% solids and 5 to 6% silica, the NOLA analysis is within ±0.2% of the wet chemistry silica. The agreement between the Microtrac output and standard screen analyses is ±1.2% at 270 mesh; ±1.5% at 400 mesh, and ±2% at 500 mesh. The poorer disagreement at the finer sizes is mainly due to inaccuracies in screening at 400 and 500 mesh. The agreement between Microtrac surface area a d laboratory Blaine surface area is about 9 ±80 cm 2/g. These instruments provide rapid and reliable feedback information to the concentrator and feed forward information to the agglomerator.

The paragraphs quoted, regarding Mr. Hoover&apos;s appointment as chairman of the newly organized food board of the United States, are taken from the editorial page, of the New York Times for April 13: ?The country is fortunate in commanding the services of Herbert C. Hoover as Chairman of the newly established Food Board. This is n, problem of first importance. Few realize the extent to which American food stuffs will enter into the decision of the war, and it is doubtful if it has occurred to many minds as yet that we shall have to deny ourselves what we have regarded as necessaries in order to keep our European allies supplied. " Mr. Hoover is one of the leading mining engineers of the country. At the outbreak of the war he put his private affairs aside to become the head of the American Commission for Relief in Belgium. He has had an experience that perhaps no other man has ever had, covering the raising of money and the purchase and distribution of supplies on a large scale; He has been brought into close relations with the neutrals and the belligerents on both sides. By them all he is recognized is one of the best executives brought to the fore by the mar. Mr. Hoover is in a position now to place abilities and experience at the service of his country. He will come back with comparative data that will enable him to measure resources and deficiencies accurately. This appointment gives renewed assurance of our Government&apos;s grasp of the problems of war and its resolve to apply, business principles to their solution."

IN view of what has happened in - the past three years, it seems incredible that industrial corporations continue to write to engineering and mines schools for "promising members of the graduating class," or, indeed, that they even send representatives to the colleges to interview engineering graduates. Certainly, by this time, every company should be aware of the fact that recent engineering graduates are almost nonexistent and the number is not likely to increase within the next several years. At the February, 1945, annual meeting of the Local Section delegates of the A.I.M.E. in New York, W. B. Plank, professor of mining engineering at Lafayette College, summarized well the situation of the colleges by titling his talk "The Lost Generation of Engineers."

For three years The Hudson Coal Co. has used egg anthracite instead of coke in its foundry cupola. It has long passed the stage of being told it cannot be done—the metal would be cold, of poor quality, or the speed of operation would be low. From observation, from use and from test. we know that our metal is about average; the cast iron invokes no adverse comment from the machine shop and the speed suits our foundry requirements. We have there-iore resolved into practice anthracite combustion processes in an industrial furnace formerly using coke, under conditions suitable to our needs. When it comes to translating what we know into use in another cupola, however, where operating conditions may be different, or to explaining why, with seemingly the same conditions in our own cupola, we have attained sometimes a temperature of 2800° in the metal and at another time 2600°, or a speed of 5 tons per hour against 2 35 tons per hour, we cannot do it. We cannot do this because there seems to be an unbridged gap between laboratory ideas on combustion and practical ideas on com-bustiqn, which we have been unable to cross in terms understandable to us as practical consumers. This gap or bridge centers around the problem of how combustion actually takes place; and we believe it is controlled by two factors: (I) the character of the surface of the fuel and (2) the space provided and needed within which the oxygen of the air is allowed to or can act on that surface. It has long been contended on behalf of coke that porosity is an important factor in combustion, although, admittedly, there are two schools of opposing thought on this question, as revealed in technical literature. It has been understood generally that a light, porous coke is the best metallurgical fuel because there are more square inches of carbon exposed to the blast of air, therefore more useful carbon-dioxide reactions can take place in the given space between the entrance of the air and the point where useful work is done. It is now our general idea that the heavier and the harder the coal, the better the metallurgical fuel, provided the surface of that fuel is suitable for fast reaction with the oxygen. Time and Space A cupola is loaded in layers of fuel and iron. The horizontal line where the first charge of iron rests on the coal is the top of the bed coal. With us that is 22 in., usually, above the tuyeres where the air enters. This means 600 lb. of coal. Every subsequent coal layer is 100 to 125 lb. of coal, and that 100 to 125 lb. of coal melts 1000 lb. of iron at around 2700° in the ladle. This quantity of coal represents a 3-in. layer spread across the cupola. The question arises: If this 3 in. of coal will melt 1000 lb. of metal, why was it necessary to use 600 lb., or 22 in., of coal beneath and between the first charge of iron and the air entrance?

A simple physical model has been developed to fit carrier-removal data in silicon irradiated near room temperature with reactor spectrum neutrons. Commonly observed donor and acceptor defect energy levels are assumed to be introduced linearly with neutron fluence. The donor levels (in ev) are Ev + 0.16, Ev + 0.27, and Ev + 0.31 and the acceptor levels are Ec - 0.55, Ec - 0.40, and Ec - 0.1 7, where Ev and Ec are the valence and conduction band energies, respectively. The introduction rates of each level are adjusted to fit literature initial carrier-removal rate data. When normalized with respect to the Ev +0.27 level, the relative values of introduction rates are 5.3, 1.0, 3.1, 1.0, 2.0, and 20.0, respectively for the six levels indicated above. To fit p-f (hole concentration vs neutron fluence) and n-f (electron concentration us neutron fluence) data, the introduction rates are multiplied by a factor which preserves the relative values given above. This factor depends upon irradiation temperature, reactor energy spectrum, neutron fluence calibration, and oxygen content of silicon. An extensive study of the effect of neutrons on carrier-removal in silicon irradiated with reactor spectrum neutrons (E > 10 kev) has been given by Stein and Gereth1 (SG) and Curtis, Bass, and Germano&apos; (CBG). They measured initial carrier-removal rates for both p- and n-type silicon over an impurity range typical of silicon devices. In this work, we attempt to fit a simple theory to this data to establish a usable relationship between hole and electron concentration, p and n, respectively, and neutron fluence f. The p-f and n-f relations are needed to assist in the design of neutron tolerant silicon devices and are needed to clarify presently used empirical resistivity-fluence relationships.3 Neutron damage in silicon produces a variety of defects ranging from simple point defects to defect clusters. For the purpose of this treatment, we assume that simple point defects dominate carrier-removal effects. In contrast to this view, stein4 has proposed that defect clusters are responsible for a significant portion of carrier-removal effects. In the following section, it is shown that the carrier-removal effect in n-type silicon with an electron concentration less than 1015 cm-3 can be explained adequately by assuming that the divacancy is the dominant defect and that its introduction rate is independent of the electron concentration. For electron concentrations greater than 1015 cm-= an additional acceptor defect center is needed, and for simplicity the A-center (vacancy-oxygen pair) has been chosen. Although the E-center (vacancy-phosphorus pair) can account for some of the results, the A-center was chosen because the E-center requires a more involved treatment which the presently available data do not justify. In p-type silicon three radiation-induced donor levels are assumed, namely the divacancy and two other centers of unspecified nature located at Ev + 0.16 ev and Ev to 0.31 ev. The donor divacancy at Ev + 0.27 ev is assumed to be introduced at the same rate in p-type as in n-type. However, this rate is too low to fit p-type initial carrier-removal data. The dominant centers in p-type silicon are assumed to be the Ev + 0.16 ev and Ev + 0.31 ev levels where the latter is not the divacancy. The introduction rates are chosen to fit initial carrier-removal rate data. Assuming that the introduction rates are independent of Fermi level, the ratio between them is fixed for subsequent p-f and n-f calculations. Using the same ratios, the initial carrier-removal rate data1,2 as well as p-f and n-f data1,5 can be fit provided the absolute value of the introduction rates are adjusted to account for irradiation temperature, reactor energy spectrum, neutron fluence calculation, and the oxygen content of silicon. THEORETICAL ANALYSIS This analysis is basically the same as that used by Hi116 to analyze electron damage in silicon except we express the degree to which an impurity level is ionized not in terms of the Fermi level, but in terms of carrier concentration. Landis and pearson7 have used the latter approach to analyze y-damage in silicon. Neutron-induced defects responsible for carrier-removal at room temperature are assumed to be simple point defects with no interaction between defects so that they may be represented by discrete energy levels. It is also assumed that no constituent of a defect complex is used up and defects stabilize shortly after irradiation. Defects are assumed to be introduced linearly with fluence according to the product Rtf where Rt is the defect introduction rate and f the neutron fluence. Taking into account the ionization of defects according to Fermi statistics, and considering charge neutrality where minority carriers are neglected, the n-f relation is where no is the preirradiation electron concentration. The parameter Nt is the electron concentration at which the ionized defect concentration is one-half the total defect concentration (Rtf) or where Et is the defect energy level. For silicon at 300°K, ni = 1.45 X 1010 cm-3 and Ei = Ev+ 0.542 ev which was determined using Ec — Ev = 1.11 ev and me* = 1.07 mo and mh* = 0.558m0. RAPID progress has been made in the automatic control of open-hearth furnaces in the past few years and many firms today\supply such control apparatus. It is somewhat surprising that so little was heard about open-hearth control up to about, 1928, and that thereafter the practice of automatically controlling open-hearth furnaces spread very rapidly. Before discussing the reasons for this it may be advisable to list the quantities that can be controlled. One of the easiest quantities to be controlled in the open-hearth furnace is pressure. The reasons for controlling it are plain. If there is excess pressure in the furnace, products of combustion leak out around .the .door and through all the cracks in the brick-work. The stability of the piers between the doors is seriously endangered, and front wall repairs become frequent. Furthermore, hot gases are lost that might be usefully employed in heating the checkerwork. If there is a slight vacuum in the furnace, cold air entering through the doors and cracks cools the interior, slowing down the reactions and making it impossible to secure the highest degree of preheat from the checkerwork.

ALTHOUGH the application of light metals in war materiel increased during the year, based on the number of uses, the trend in aluminum and magnesium production in 1944 was characterized by a sharp decline in the production of both metals accompanied by a further accumulation of large Government reserve stocks. Several primary reduction plants were curtailed to prevent building excessive inventories. By the end of the year, however, a lively revival of war production took place when resistance stiffened on the western front in Europe. Production of primary aluminum ingot was reduced from over 90,000 tons per month at the start of the year to 45.000 tons per month by the end of the year.
