"This paper aims to demonstrate how a systemic approach can be applied to the analysis of the causes of accidents in South African mines. The accident analysis framework used was developed previously by the authors from the combination of the Mark III version of the Swiss Cheese model, Incident Cause Analysis Method (ICAM), the Nertney Wheel model, and safety management principles. Data on 91 accidents occurring from 2010 to 2012 at the site of a platinum mine in South Africa were used to populate the newly developed framework. The results obtained show that while routine violations (45% of all accidents analysed) were the most common form of human error, problems in the physical environment of workers were the most common workplace factor (39.6% of all accidents analysed). Furthermore, inadequate leadership was found to be the most common systemic factor responsible for accidents (51.6% of all accidents analysed). Some workplace factors were more commonly associated with particular unsafe acts than others, and some systemic factors were more associated with particular workplace factors than others. The outcome of this study demonstrates that systemic factors, rather than human errors and violations, are the chief causes of accidents in the mining sector. IntroductionThe mining industry is a very important sector of the South African national economy. A major factor threatening the sustainability of this industry is mining accidents, which frequently result in injuries or deaths, destruction of property, and pollution of the environment. In the past, mining accidents have led to the shutdown and threat of shutdown of mines (Ryan, 2008; Mail and Guardian, 2011). The country stands the risk of incurring significant losses if the mining industry continues to experience shutdowns. In 2012, the mining sector accounted for R262.7 billion (equivalent to US$32.83 billion) representing 8.3% of GDP directly, on a nominal basis (Chamber of Mines of South Africa, 2013).Mining safety is a global concern and has attracted significant international attention. This has precipitated various studies into different aspects of mining. Unfortunately, human error has been blamed for the majority of these accidents. A study by the US Bureau of Mines found that human error is the cause of almost 85% of all accidents (Patterson and Shappell, 2008). In Australia, it is the cause of two out of every three occupational accidents (Patterson and Shappell, 2008). Various mining companies maintain that they run efficient systems, and hence the behavioural problems of workers are to blame for most accidents. An understanding of accident causality could be a major step in the quest to reduce accidents. Only with a good understanding of the accident process can effective remedies can be designed."

In recent years, a great deal has been written and spoken, both in our country and overseas, about the dangers posed to the environment and the ecology by mining operations. In a cobalt precipitation plant, phosphoric acid is used to eliminate excess uranium from the pregnant leach solution, resulting in a cobalt hydroxide product that has little uranium contamination. However, the amount of phosphoric acid added must be monitored and controlled to avoid financial and sustainability impacts and to prevent negative impacts on the final product quality. One possible solution to this problem is the use of online residual phosphorus measurement techniques to regulate the addition. By continuously monitoring the residual phosphorus levels in the solution, the plant can adjust dosing in real time to maintain the target residual phosphorus level. As a result, the cobalt precipitation plant can improve phosphorus level control, lowering the risk of overdosing, and ensuring consistent product quality. This approach can also improve process efficiency, reduce operational costs, and have a positive environmental impact

Today the roller screen has a wide range of applications in the steel-making industry. It is used for screening of green pellets before using the kiln. Since the appropriate size of a green pellet is -16+9 mm, there are different gaps between rollers that make green pellets classified in three categories: -9 mm (undersize), -16+9 mm (onsize) and +16mm (oversize). The efficiency of a roller screen is specified according to the weight percent of undersize in onsize as a fine material, and weight percent of onsize in oversize (loss). This efficiency is influenced by different operational parameters such as roller diameter, roller speed, and roller gap, material type of rollers and inclination of roller screen. This study investigates the effect of different material types of rollers in efficiency of Ardakan roller screens, located in the Yazd province of Iran. The rollers of the existing roller screens in this pelletising plant are made of polyurethane which according to this investigation, has a maximum lifetime of about six months. Operating the polyurethane rollers beyond its lifetime generates serious problems such as high wear of rollers, and consequently, material buildup on the roller surface which then significantly decreases the roller screen efficiency. So, after examining different roller materials, stainless steel 304 with higher surface roughness to help pellets move properly on the roller screen surface was selected as a solution to this problem. The new roller screen was constructed by Fakoor Meghnatis Spadana (FMS) and replaced the previous one. In this way, the maximum capacity of the roller screen was increased about 17% and the roller screen efficiency was increased about 15%. Body breakage usually involves breakage of particles by compression between two surfaces or impacted against a rigid surface, with the latter of interest in this current study. This mode of breakage has been identified to be the most elementary size reduction process and the most effective breakage process in comminution devices. It has been found in the literature that the progenies produced from this form of breakage possess a residual velocity at which they move, after the breakage event. In the pursuit of efficiently utilising applied comminution energy, a laboratory-scale breakage device has been developed to deliver precise specific impact breakage using the concept of a fast-moving hammer and a slow-moving particle to effect breakage. The residual velocities of the progenies from the breakage event may be used to generate secondary breakage without further addition of energy if the particle encounters a stiff obstacle such as a steel plate at right angles to its path. To measure the extent of secondary breakage, the steel wall of the device was lined with a rubber material to suppress the second set of breakage events. The performance of the new device is analysed, based on calculated t10 values and measured residual velocities of the moving daughter particles. Testing involving anvils showed a much finer PSD and higher t10 compared to the test involving rubber lining. This finding is corroborated by the measured residual velocities of the progenies. The resulting energies calculated from the measured residual velocities of the progeny were found to be approximately 42-88% of the primary specific impact energy. Dimension stone mining is Increasing because of growing demand from the building and construction industries. Diamond wire saw cutting is a popular dimension stone mining technique around the world. Estimation of the production cost for this method is critical for these operations. n this research a production cost model was developed for the mining of granite using a diamond wire saw. Cost and production data were collected from a granitic dolerite operation in Pakistan for a 4.5-year period, and analysed to identify the cost components. Fuel (34%), labour (15%), consumables (40%), and maintenance (5%) were found to be the four major cost components in this operation. A regression-based dimension stone production cost model incorporating statistically significant variables was developed. The data presented a strong correlation of production cost with fuel, labour, and consumables costs. Finally, a production cost model based on the consumption cost of diamond wire is presented. The model was verified using actual production cost data, which indicated 95% (R2 = 0.95) variance in the data between the predicted and actual production cost values. The model may help field engineers in computing their dimension stone operational cost during the cutting phase.

Although many sites are equipped with surveillance cameras and control systems, critical events often go unmonitored or under-analysed due to the lack of intelligent interpretation tools. Cameras typically act as passive recorders, requiring manual review by control room operators; a process that is labour-intensive, error-prone, and reactive. Vision AI is emerging as a transformative solution, combining computer vision and artificial intelligence to deliver real-time, actionable insights. This technology has evolved along two key phases: traditional object detection, and more recently, multimodal large language model integration. This paper presents solution architectures, deployment results, and key insights from real-world implementations across underground operations, open-pit truck-shovel operations, and smelter operations, demonstrating how Vision AI is reshaping mining operations to become safer, more efficient, and more intelligent.

It is well known that carbon management is essential for minimising soluble gold losses in carbonin- leach (CIL) and carbon-in-pulp (CIP) circuits. Even so, carbon measurements remain predominantly a manual operation with a low amount of control and automation around CIL and CIP reactors. Carbon inventory, i.e. knowing the carbon concentration in each reactor, is crucial for good carbon management. CarbonSense, a novel in-situ measurement system, was therefore developed for continuous monitoring of the carbon concentration in each reactor and paves the way for more advanced control in gold operations. The carbon management and optimisation system is based on Outotec® HSC Sim dynamic process simulations where each process component is modelled in detail. Generated models are validated with real data from a particular operation. The simulator will allow for the carbon control sequence to be realistically tested and the efficiency optimised by controlling the feed and discharge pumps in a hazard- and risk-free process environment. Furthermore, by combining CarbonSense and Outotec® gold courier (online soluble gold measurements) continuous feed-forward and feedback information can be obtained. The optimiser will then define the best running parameters based on the cost function of gold production versus intrinsic factors such as utility costs and adsorption time. Eventually, the application can be installed parallel with a real process island and it can be continuously calibrated with real-time process data. The dynamic model will then create future predictions for the entire process island. The example case presents a CIL circuit, which is simulated using Outotec® HSC Chemistry Sim and operated through Outotec® ACT (Advanced Control Tool) application. "SynopsisThe development of plasma technology at Necsa started in the early 1980s, when the applicability of high-temperature plasmas in the nuclear fuel cycle was investigated. Since 1995, this plasma expertise has expanded to other industrial applications, for example mineral beneficiation, nanotechnology, fluorocarbon production and waste treatment, all of which are also of relevance to the nuclear industry.Necsa has demonstrated the manufacture of plasma-dissociated zircon, zirconium metal powder, carbon nanotubes, silicon carbide (SiC), zirconium carbide (ZrC) and boron carbide (B4C) at the laboratory and pilot plant scale. These materials are commonly used in the nuclear industry. Zirconium alloys are used as fuel cladding material for nuclear fuel assemblies.Necsa manufactured the monomer tetrafluoroethylene (TFE), using 150 and 450 kW DC plasma systems, from which the polymer polytetrafluoroethylene (PTFE) was synthesized for use in filters and as seals in nuclear plants. With the nuclear renaissance at hand, it was demonstrated that plasma technology can be used to produce hydrofluoric acid (HF), which is used in the manufacture of fluorine gas (F2) for the production of uranium hexafluoride (UF6) directly from the mineral calcium fluoride (CaF2) without the use of sulphuric acid as in the conventional process. The recovery of valuable uranium from nuclear waste such as filters, oils, and solids with plasma processes will also be discussed. The destruction of low-level nuclear waste by a plasma gasification system can reduce the volume of this waste by several orders in magnitude, resulting in huge savings in the storage costs. Another product of plasma technology is the encapsulation process for nuclear waste and the production of vitrified product, which could be used as filler material for medium-level nuclear waste.IntroductionDuring the 1980s, a plasma research and development programme was launched at the erstwhile Uranium Enrichment Corporation of South Africa (UCOR) at Valindaba outside Pretoria. The main purpose of this programme was to develop alternative and more economical processes for the processing and manufacturing of uranium compounds used in the nuclear fuel cycle. The conversion of UO2 (as received from uranium mines) to UF6, which is needed for enrichment, conventionally involves several laborious, expensive, chemical processing steps. Plasma processes can eliminate several of the intermediate steps. Enriched UF6 is converted to UO2, which is pressed into pellets that are used in nuclear power plants. Plasma technology could also be used in a modified and optimized conversion process."

Mineral processing plants include different stages like crushing, grinding, classification and concentration, all of which have partially affect the final product. To identify and solve the related problems of each stage, process audition should be carried out continuously to find the major solutions of improving product and decreasing the maintenance and operating costs. The Iranian Gohar-Zamin iron ore beneficiation plant consists of equipment such as gyratory and cone crushers, high pressure grinding rolls (HPGR), ball mill, dry and wet double-deck vibrating screens, hydrocyclones cluster, magnetic drum separators, thickener and horizontal vacuum belt filters. The defects of initial design as well as current non-standard operating conditions of equipment were monitored and detected and the opportunities for process optimisation were presented as follows: establishing the control loop of power draw for cone crushers, the modification of feeding chute in cone crushers to minimise the non-uniform wear of parts, removing segregation in the fresh feed bin and screen’s circulating load chute in order to provide the proper feed to HPGR, reducing wear rate on HPGR rolls surfaces, decreasing the amount of rejected materials in screen, equalising the solid content and particle size distribution (PSD) of feed in all magnetic drum separators and equalising the PSD of hydrocyclones overflow. This paper provides a guideline for the mineral processing industry to focus its efforts on process design to deliver the efficiency with minimum capital costs. GEOSCANs on belt analysers have been widely utilised in iron ore for nearly two decades. Since the first installations in 2003, the availability of accurate and reliable grade data has become standardised across iron ore mines in the Northern Cape, where various streams are monitored continuously to ensure the optimal functioning of plants. Assmang Khumani has been at the forefront of technology adoption and has implemented 19 GEOSCANs at various locations throughout the plant, monitoring run-of-mine, stockyard feed, beneficiation plant feeds, and products. Over the years, collaborative advances in the technology have allowed for numerous benefits and gains to be made. In recent times, there has been a concerted effort on the development of analysis for phosphorus in the material flows, and based on Scantech's experience in the phosphate industry, this element has now been calibrated to achieve statistically acceptable and repeatable accuracies for real time analysis of this element on many of Khumani's GEOSCAN analysers. This paper provides an overview of GEOSCAN use at Khumani, while focusing on the development of phosphorus analysis. A case study of analysis results is presented for a range of analysers that have been calibrated for this element, as well as a discussion on the benefits obtained as a result of this analysis. "Reliable and accurate metal transfer information during silicon tapping from an EAF is invaluable for decision-making and process optimization, while obtaining such information safely is a necessity.Elkem Thamshavn Norway, a producer of metallurgical-grade silicon, identified inherent safety problems with gauging fill levels in ladles using dip bars while molten metal was continuously pouring from a rotating EAF. Operator safety was a paramount concern, and an alternative automatic method of controlling the tapped fill level needed to be found.It was also considered that further benefits could be achieved such as monitoring the tapping flow from the EAF in order to identify any clogging, as well as the potential to use the live feed from such a system to assist decision-making regarding work around the taphole area.Agellis Group AB, Sweden, a company with over 30 years of experience in gauging molten metal levels in the metals industry, was able to design a solution for Elkem Thamshavn that would provide the required real-time process data while enhancing operator safety.This paper shows how the Agellis AB EMLI-L system was successfully installed at Elkem Thamshavn’s EAF continuous tapping area, how ladle fill levels were constantly monitored, and how full ladles were replaced by empty ones without the need for operators to be in the tapping area to take level readings. The results were used to compare the accuracy and reliability of Elkem Thamshavn’s current measuring methods with that of the EMLI-L system. The paper also covers how problems such as how to connect, run, and protect cables in and around the EAF tapping area, as well as connector and cabling survivability during the three day refractory pre-bake period, were solved. Finally, the paper discusses the potential production improvements, maintenance requirements, and data integration attainable through the installation of the system, as well as the major safety improvements that have been achieved."

Metallurgical simulation and evaluation of the resource efficiency of whole production processes are of key importance for impact assessment, in terms of environment and resources. Exergy is a measure of resource consumption, taking into account energy, raw materials, water, and chemicals; which allows for the evaluation of different technologies to find the most resource efficient and environmentally friendly process. This paper compares the sustainability for production of rare earth magnets from primary and secondary resources through an exergetic evaluation of the production process, in addition to traditional life cycle assessment (LCA). The methodology makes use of simulation tools for tracking material flow and resources used, based on thermodynamic laws, from ore exploitation to magnet production and recycling. It highlights areas with the greatest potential for improvements in terms of resource consumption and environmental impact generation, having a thermodynamic-validated flowsheet for primary and secondary production of NdFeB magnets, present in several low-carbon technologies, such as wind turbines, electric vehicles, and cooling systems. It demonstrates the minimum exergy destruction for magnet production, through an irreversibility breakdown chart, showing where irreversible material and energy losses occur. Results show that CO2-equivalent emissions for producing NdFeB permanent magnets from primary resources could be twice as high when compared to magnets produced from recycling technologies. The theoretical maximum exergetic efficiency for primary production is 57.61%, whereas for permanent magnets produced from secondary resources, the exergetic efficiency reaches up to 82.89%. Two magnet recycling technologies were evaluated, i.e. in acidic medium and via hydrogen decrepitation, and it was observed that their exergetic efficiency and environmental impact performed better than production from primary resources. The categorization of artisanal and small-scale mining has a significant impact on perception and understanding of the sector, particularly from a livelihood perspective. Several developing countries classify different types of artisanal and small-scale mining based on legal status. Given the nature such activities, the tragedy of the commons theory has been applied to understand the inevitable environmental, economic, sociocultural, safety, and health negative externalities – which are products of informality and illegality. Consequently, formalization is a panacea to sustainability. This article aims to offer a conceptual organization and foundational knowledge for characterization of artisanal and small-scale mining. It seeks to identify existing literature on the subject to avoid replication and acknowledge other scholars. Moreover, the article aims to place the study within the context of existing literature, establishing relationships with other studies and why the research is necessary. This paper pinpoints gaps left from other studies in so far as the characterization of artisanal and small-scale mining is concerned. Thus, reviewing literature abetted in finding similarities and differences on how artisanal and small-scale mining is defined from a scholarly and legal basis. The paper ascertains major claims made by earlier research on this topic, such as environmental impacts, the lack of adequate regulatory framework, and presents querying and probing questions lacking from other studies.

The value realised by an open pit mine depends on the quality and integrity of the mine planning process as well as the level of execution against these mine plans. When managing the execution against the mine plan, how well the mine plan is executed spatially, is of critical importance. The short-term mine plan is the plan that is physically executed on most open pit mines. At the same time, the spatial mine-to-plan compliance is typically reconciled against the annual business plan. Short-term mine planning, therefore, plays an important role in the effectiveness of the spatial mine-to-plan compliance reconciliation process by providing operational teams with detailed designs and schedules, while ensuring that mining execution is spatially aligned with the business plan. The Sishen open pit iron ore mine (Sishen) strives to continuously improve spatial compliance to the business plan. Sishen enhanced the role that short-term mine planning plays in enabling the forward-looking component of the spatial mine-to-plan compliance reconciliation process. The enhanced short-term mine planning process focuses on detailed tactical sequence designs per mining pushback, the health of value chain buffers, spatial plan-to-plan reconciliation, and the associated management routines. This led to improved spatial mine-to-plan compliance to the business plan from 71% to 94% over the four-year period from 2020 to 2023 inclusive. These results indicate that the application of short-term mine planning as part of an integrated spatial mine-to-plan compliance process at open pit mines can contribute positively to improving the level of spatial execution against the business plan. This paper presents the enhancements made to the short-term mine planning process, which other open pit mining operations can consider to improve spatial mine-to-plan compliance.

The following notes have been compiled to assist authors in the preparation of papers for presentation to the Institute and for publication in the Journal. STANDARDS FOR ACCEPTANCE To merit consideration papers should be of sufficient high standard and contain matter that is new, interpretations that are novel or of new significance and conclusions that cast a fresh light on old ideas. Their publication should be of real interest to technical men and of benefit to mining and industry. Authors must realize that because a mine shaft is new or the mine itself is newly established, this in itself does not justify a paper unless significantly new techniques or processes were involved in the opening-up procedure. A few well selected diagrams and illustrations are often more pertinent than an amorphous mass of less well chosen material. Over-statement and dogmatism are jarring and have no place in technical writing. The amount of textbook material included in a contribution should be the minimum essential to the argument. The length of a paper is not the criterion of its worth and it should be as brief and concise as possible, consistent with the lucid presentation of the subject. Avoid the use of the first person, be objective and do not include irrelevant or extraneous matter. Papers should be submitted at least three months prior to the intended date of presentation. The text should be typewritten, double-spaced, on one side only of foolscap paper, leaving a left-hand margin of 11-inches, and should be submitted in duplicate to facilitate the work of the referees and editors. Galley proofs are sent to the authors for the correction of printers' errors and not for the purpose of making alterations and additions which may be expensive. Should an author make alterations which are considered excessive, he may be required to pay for them. Words to be printed in italics should be underlined singly. For small capitals they are to be underlined DOUBLY and for large capitals TREBLY. Abbreviations and symbols are laid down in British Standard 1991 and proof correction symbols in British Standard 1219C. Abbreviations are the same for the singular and plural, e.g. ft for foot and feet, lb for pound and pounds. Percentages are written in the text as per cent; the symbol % is restricted to tables. Likewise ft and in. should be used, x' y" only being permissible in diagrams and plans. Drawings and diagrams are to be in black India ink and should be about 6 in. wide. Numbering of tables should be in Roman numerals: I, II, etc. and figures in Arabic numerals: Fig. 1, Fig. 2, etc. Photographs should be black and white glossy prints. As a guide to the printer the author should indicate by means of notes in the margin of the typescript where drawings and diagrams, etc. are to appear in the text. When submitting graphical representations avoid a fine grid if possible. Curves should be in heavy line to stand out. Lettering too should be bold as a reduction in size is often involved in the printing process. SUMMARY ABSTRACT OR SYNOPSIS It is most important that the summary should provide a clear outline of the contents of the paper, the results obtained and the author's conclusions. It should be written concisely and in normal rather than abbreviated English and should not exceed 250 words. While the emphasis is on brevity this should not be laboured to the extent of leaving out important matter or impairing intelligibility. Summaries simplify the task of abstractors and therefore should present a balanced and complete picture. It is preferable to use standard rather than proprietary terms. FOOTNOTESAND REFERENCES Footnotes should be resorted to only when they are indispensable. In the typescript they should appear immediately below the line to which they refer and not at the foot of the page. References should be indicated by super-script, thus . . .1 . . . 2. Do not use the word Bibliography. When authors cite publications of other societies or technical and trade journals, titles should be abbreviated in accordance with the standards adopted by this Journal.

Sheryl Sandberg, the COO of Facebook, published a book titled 'Lean In', where she invites women to sit at the table. In summary, to 'sit at the table' entails not only embracing opportunity, but to actively seek out opportunities in leadership positions. Sandberg points out that, unfortunately, apart from institutional obstacles, a woman's own perceived limitations have a negative impact on her leadership ambitions. To create an environment where diversity will flourish, where everyone has a seat at the table and everyone has a voice, safety must be established first. To establish safety within a diverse team, there is power in relationships and connections. This study will first and foremost review theories in literature as to why diversity and inclusion are so important and the value that diversity brings to our industry. The study also recommends a few focus areas for improving diversity and inclusion in the industry.
