The objectives of this paper are to introduce the International Code of Practice for Risk Management of Tunnel Works and to illustrate to the participants of the 13th Australian Tunnelling Conference 2008 how the insurance industry would like to see the future of tunnelling insurance. Tunnel construction projects have suffered large losses over the years as can be shown from various loss examples and a list of major losses over the last ten to 15 years. Since the consequences of insufficient risk control had to be borne by insurers, these losses have heightened the need for improved and adequate risk management measures so that insurance cover can also in future be provided for this hazardous industry sector. Observations are made as regards to the prevailing trends in the tunnelling industry, which contribute to the increase in risk and have adverse consequences for the insurance industry. To find a solution to this dilemma various options are examined with the suggestion of a Joint Code of Practice for Tunnel Works being agreed by all stakeholders, which would become essential to secure future insurance protection for such project work. The British Tunnelling Society has, in cooperation with the Association of British Insurers, developed such a code, which the International Tunnelling Insurance Group has now adopted for global application in international markets. The content and concept of this code is explained to establish a framework of understanding and applied risk management principles for tunnel works. This initiative also has the support of the International Tunnelling Association and the International Association of Engineering Insurers. The international application of this new code is demonstrated with a few examples, which also includes Australian projects. It is the aim that this code forms in future part of the insurance coverage for tunnelling works in order to put this risk segment on a more sustainable basis. For that reason the insurance industry is seeking the support of all stakeholders of tunnelling construction projects.

Over the period 2000 to 2003, roof falls have accounted for 4 to 14% of the fatalities in underground mining operations. The National Institute for Occupational Safety and Health (NIOSH) is conducting research to reduce the frequency, exposure, and risk of these events through an ongoing program of field and laboratory studies. One area of research involves the evaluation of polyurethane grouting technology that is commonly used to stabilize fractured mine roof strata. Concurrently, NIOSH is conducting work to evaluate the application of ground penetrating radar for advanced delineation of problematic mining areas. In this study, NIOSH partnered with Sub-Technical, Inc. who injected a polyurethane grout (also known as glue) into a roof area of NIOSH’s Safety Research Coal Mine. The mine roof area was scanned using ground penetrating radar technology before and after grout injection in an attempt to determine the extent of grout infiltration. A comparison of the pre-and post-grouting radar records showed a significant change at a mine roof depth of about four to five feet. The interpreted radar records were then compared with drill core information, borescope evaluation of roof-bolt holes in the study area, and underground observations. At this site, the interpretations of the radar records correlated with data obtained from the core holes, borescope evaluations and underground observations. This study showed that ground penetrating radar technology can be a useful tool for detecting changes in mine roof due to the injection of the grout.

Planning for the Macraes Extension project highlighted the need for additional tailings storage availability. The possible solution of developing new tailings impoundments in Northern Gully, in tum, led to concerns being expressed by the regUlatory authority over the assimilative capacity of Deepdell Creek. Philosophical differences over the appropriateness of applying a seepage model used to set standards in consents for the original project resulted in the company re-evaluating the whole monitoring system associated with tailings disposal and seepage control. Re-evaluation indicated problems arising from the existing in-stream water quality monitoring in respect of protection of the aquatic ecological system and verifiability of results. A new monitoring system was designed relocating the compliance monitoring point out of the stream and into the gully containing the tailings impoundments giving greater certainty of monitoring results to all parties and providing improved protection for the aquatic ecology of Deepdell Creek.

The development of information concerning mesothelioma in South Africa started with the 1959 finding of Dr. J. Christopher Wagner of cases of mesothelioma in persons living in the vicinity or working in crocidolite mines in Cape Province. After more than 40 years, the data from South Africa now provide important insights into the differences in mesothelioma risk associated with different asbestos fiber-types. Three case series reports and one analytical report were reviewed in this session. In total, information on 504 proven cases was presented. There were 28% of cases arising from occupational exposure in mining, 35% from non-mining occupations, 23% from environmental exposures; 14% where no exposure was established and 9% where the exposure was unknown. Among the 129 cases for which occupational exposure and asbestos fiber-type were established, 115 cases were associated with occupational exposure to Cape crocidolite; 1 case associated with occupational exposure to Transvaal crocidolite and 9 cases with occupational exposure to amosite (grunerite asbestos) and 4 cases with occupational exposure to mixed fiber-types. There were no cases associated with chrysotile asbestos. There were 33 cases of environmental mesothelioma linked to Cape crocidolite exposure and 2 to exposure to amosite and crocidolite but none to Transvaal amosite alone or to chrysotile. The evidence shows that amosite is capable of causing mesothelioma but that the major risk in South Africa is associated with Cape crocidolite.

The use of wetlands to treat mine drainage has become increasingly common, particularly as more information is available on their operation and construction. However, the lifetime of each system is still, at best, an estimate. Treatment lifetime depends on the types of processes that provide the majority of the metal removal. Anaerobic systems (vertical flow systems) rely primarily on sulfate reducing bacteria, which reduce sulfate to sulfide and generate alkalinity. Metals can be precipitated as sulfides, and acidity is neutralized. These systems can remove over 90% of the metals and increase pH from around 4 to over 7. The bacteria will remain active as long as the system remains anaerobic and contains an adequate supply of sulfate and small chain organics. Although estimates of lifetime made on the total carbon in the system suggest that lifetime should exceed 20 years, data from field and laboratory studies show that the rate of sulfate reaction decreases with time, and that after several years rates decreased substantially. To maintain acceptable treatment, either additional organic material must be added or the total metal and acid load to the system would need to be reduced substantially. Aerobic systems (overland flow wetlands) can effectively remove metals from neutral mine drainage by reactions with organic material in the substrate. Over 90% of the copper and nickel have been removed in aerobic systems treating mine drainage in Minnesota. The primary removal processes in these systems include adsorption, ion exchange and complexation, which are finite; removal will cease unless new removal sites are generated. Two wetlands built in Minnesota have been successfully treating mine drainage for 12 years. At one of the wetlands, the annual production of new removal sites has been estimated to be equal to the annual metal input, and, as a result, metal removal should theoretically continue indefinitely.

Coal refuse slurries stored in tailings impoundments can potentially lead to environmental concerns such as effluent seepage, property devaluation, and in extreme cases, dam failures. Pressure filtration offers an opportunity to reduce the impoundment requirements by providing significantly drier solids. A full-factorial design was used to study the effects of temperature and pH on dewatering a refuse slurry obtained from a coal preparation facility in Pennsylvania, USA. The experimental results indicated a strong effect of temperature on cake moisture as the cake moisture after about 2800 s was 22% at 43 °C and only 36% at 10 °C for tests conducted at pH 3. ANOVA results confirmed the statistical significance of pH, temperature, and their interaction effect. Also, the results showed no significant differences between the tests conducted near neutral and alkaline pH values at low and ambient temperatures. On the other hand, Tukey's test indicated higher reductions in cake moisture at higher temperatures for lower pH values. This study affirms the need to monitor and control the pressure filtration process as throughput could significantly vary with changes in temperature and slurry conditions.

Increased competition in today’s global economy has put mineral industry under excessive pressure to effectively manage production assets to generate business wealth. Capital intensive industry (CII) organizations are seeking ways to manage skill resources, internal processes, and organizational networks with a view to reduce the cost of ownership of assets and increase efficiency of operations and service. The emerging concept of Enterprise Asset Management(EAM) is a collection of business processes that allow CII with right focus on return on investment on assets, to make right decisions on planning, acquisition, maintenance, operation, rehabilitation, and disposal of assets. Information technology (IT) can drive significant returns if the infrastructure covers seamlessly all business processes and all systems, like geographic information system, maintenance management, investment management, laboratory information management, and supervisory control and data acquisition system. The simple approach to IT in the past with disconnected silos of technology, information, and business processes cannot deliver the expected benefits under the modern business environment. Identification of business and application component is a prerequisite for the development of an asset management system that links effectively disciplined processes having characteristics of agility, speed, and integration. This paper discusses various issues and outlines the steps for developing an asset management roadmap in an organizsation. It also presents major business and application components of the system. The early part of the decade was marked by world-wide problems and economic difficulties. In 1980, President Jimmy Carter ordered the failed military mission to rescue hostages in the U.S. Embassy in Iran, Mt. Saint Helens, WA erupted killing 60 people and the Soviets and Mujaheddin guerrillas clashed in Afghanistan after the Soviet invasion. Later in the year, Ronald Reagan was elected president, defeating Carter. The first launch of a space shuttle (Columbia) occurred in 1981. Also in 1981, the first IBM-PC’s begin to roll off the lines. In 1982, Argentina invaded the Falkland Islands and Mexico’s economy was in collapse. In 1980, the U.S. unemployment rate stood at 7.1 percent, and by 1982 the rate increased to 9.7 percent. 1980 marked SME’s first full year in its new office building in Littleton, CO. The new facility was dedicated in November 1979. Claude L. Crowley was executive director of SME in 1980 and remained in that position throughout the decade. SME’s membership stood at about 26,000 throughout this period (1980-1982).

A 3-stage fluidizing bed process of chlorination, selective oxidation and sulfur chloride conversion was tested for processing of a chalcopyrite concentrate. It was found that the use of excess oxygen was essential to a stable operation in the oxidation reactor. The oxygen-excess gas will inevitably cause the oxidation of the sulfide concentrate in the chlorination and sulfur chloride conversion reactors. A new flowsheet is proposed to separate the chlorination and the selective oxidation steps with separate flow paths of the fluidizing gases, thereby preventing the passage of oxygen-excess gas in the chlorination and sulfur chloride conversion reactors. The feasibility of the proposed flowsheet was demonstrated in a laboratory-scale fluidizing bed setup.

In central Washington, six miles south of the small town of Vantage, the flow of the Columbia River is harnessed by the Wanapum Dam which is part of the Priest Rapids Hydroelectric Project owned and operated by Grant County Public Utility District (GCPUD). Extending more than 8,300-feet from bank to bank, the dam includes left and right earth embankments, left and right concrete gravity sections and a 10-unit powerhouse. Adjacent to the right embankment is a concrete gravity spillway featuring radial gates measuring 50-feet-wide and nearly 70-feet-high. In the early 1960’s when Wanapum Dam was built, these seven-story spillway gates were the highest in the world. In late February 2014, 50-years into its service life, during a routine maintenance inspection, observant workers noticed a slight misalignment in the roadway deck curbs and handrails located on top of the spillway. Further survey and inspection efforts uncovered a 65-foot-long by two- inch-wide crack that spanned the full length of Monolith 4. On discovering the extent of the damage, GCPUD initiated immediate drawdown of the reservoir to relieve pressure on the spillway monoliths. Wanapum Reservoir and the areas directly downstream of the dam were immediately closed, boat ramps and inlets were left completely dry, campsites were closed and boating activity was banned on the lake until further investigations and remedial procedures were in place and completed. An extensive root cause analysis ultimately led to the conclusion that errors in the initial design calculations and late changes to the final spillway design led to a condition that generated tensile stresses in the concrete at the upstream face, which over a period of 50-years resulted in the formation of a crack along a horizontal lift joint located about 85-feet below normal pool elevation. Further analysis showed that, although visible distress was found at Monolith 4, there was nothing preventing this issue from happening at other locations along the dam spillway. Although no visible cracking was found in the other monoliths, repair procedures for Monolith 4 would have to be replicated at the remaining monoliths.

New Zealand SteelÆs Waikato North Head mine, situated at the mouth of the Waikato River, supplies approximately 1.2 million tonnes per annum of titanomagnetite concentrate to the Glenbrook steel mill. The mine draws its fresh water supply from the Waikato River, abstracting approximately 38 000 cubic metres per day. During the concentration process of the ironsand, slimes are liberated into the plant wastewater. The plant wastewater, storm water and groundwater from the mining operation is treated and passed through settling ponds. Around 118 400 cubic metres per day of water are drawn from the recycle settling ponds for use in the concentration plant. Typically a daily average of up to 15 100 cubic metres of the treated water is discharged back to the Waikato River. Since the early 1990s significant improvements have been made to the water treatment at Waikato North Head mine and regularly the mine discharge water quality is similar or better than background river water quality.

"The demand for barite has increased greatly in the last 10 years, especially during World War II. Depletion of oil reserves and subsequent increase in deep drilling created a heavy demand by the oil industry for barite to be used as drilling mud. The great increase in demand for barite gave impetus to the development of a, deposit of impure barite near, Magnet Cove in Hot Spring County, Ark.The critical condition of the national barite supply early in 1944 led the War Production Board to recommend that the Bureau of Mines attempt to prove the existence of a large reserve tonnage of barite in the Magnet Cove area and arch for new deposits. To carry out this program, the Bureau explored on newly reported. Barite deposits in southern Montgomery County, Ark. The Gap Mountain, Fancy Hill, and Sulphur Mountain barite deposits were investigated. Eight diamond core-drill holes and 1,336 linear feet of surface trenching were completed, and barite samples obtained were analyzed, A study of the Montgomery County barnse district indicates that the lenticular bodies of barite replace a favorable host rock near the base of the Stanley shale formation. Barite is exposed along the side of anticlinal mountains where erosion has truncated the beds on the limbs of the associated synclines. An appreciable amount of barite suitable for commercial use has been indicated by trenching and core drilling. The Gap Mountain and Fancy Hill deposits are controlled by the Magnet Cove Barium Corp. and Baroid Sales Division of National Load Co., respectively. The ore is similar to that of the Magnet Cove area and is amenable to concentration by a flotation process. Mining of these deposits had not been begun by the time the Bureau completed its exploration project."

Secondary aluminum dross, which has been exposed for several years to atmospheric conditions, was characterized using chemical analysis, X-ray diffraction, optical and scanning electronic microscopy, and energy-dispersive spectroscopy. Based on the characterization, a method to purify the solids is presented. This method starts with the washing of the dross in boiling water to eliminate the soluble compounds like Na and K, followed by the physical separation of the metallic aluminum by milling and screening. Finally, an acid lixiviation process is applied using three different acid solutions, HCl, HNO3, and H2SO4. The best results were obtained using HCl. The main insoluble components of the dross after lixiviation were Al, SiO2, and Al2O3. MgAl2O4, AlN, Si, and Na2Al22O3·2H20 species were also found. Based on the alumina content, the solid product could be suitable as a raw material in the manufacture of castable refractories or metal matrix composites.

"In 1991, local prospector C. “Mickey” Clement recovered at least three alluvial diamonds from the Michipicoten River. Two of these stones were sent to the Department of Mineralogy, Royal Ontario Museum, where they were identified as industrial-grade diamonds with weights of 1.05 and 1.13 carats. Both stones were described as frosted and graphiteinclusion riddled.In 1995, prospector Sandor Surmacz and geologist Marcelle Hauseux discovered diamonds in bedrock approximately 20 km north of the Michipicoten River. An 18.1 kg bulk sample of a xenolith-rich lamprophyre, taken from an outcrop at the side of the Trans Canada Highway north of Wawa, yielded 1 macro diamond and 5 micro diamonds. All but one was gem quality (Sage, 2000).Since then, more than 36 occurrences of diamonds in bedrock have been reported in an area approximately 30 km2 in size, centred 20 km north of the town of Wawa. The occurrences are hosted within a sequence of unusual, Archean-aged, heterolithic breccias previously considered to have no economic significance. Ten kimberlite occurrences also have been confirmed in the Michipicoten greenstone belt in the last five years. Two of these kimberlites are diamondiferous.To date, expenditures on property acquisition and diamond exploration in the Wawa are estimated at CDN$120 M.This paper summarizes the geological observations made on these occurrences and describes the relationship between the occurrences and the regional geology."

As a discipline, geophysics covers an enormous range of scales and applications - from planetary geophysics through to the monitoring of acoustic emissions of rock samples. Although it is not a core discipline within the mining industry, geophysics has a useful role in providing information on the nature of the rock mass and the way it is responding to the mining process. For coal mining purposes, geophysical studies mainly seek to - delineate boundaries between the coal and the various layers within the surrounding strata - determine the physical properties of the rocks in-si tu, and - monitor the response of the rocks to the mining process Some of these applications may be associated primarily with mine planning, but once a mine is underway, most geophysical results can be used for the purposes of ground control. For example, insights into the nature of a geological fault mapped by a 3D seismic reflection survey will be relevant to determining support requirements while mining through it. In some instances there are parallels in the use of geophysics by the petroleum industry. Seismic reflection and wireline logging techniques have mainly been developed for petroleum exploration where their on-going development continues to provide spin-offs for the coal industry. The 3D seismic survey techniques that we now routinely employ were in their infancy 25 years ago and, almost certainly, the high quality borehole scanning tools that are now available would not have been developed without the need for petroleum well evaluation. Mining has also led to the development of specific techniques. An example is microseismic monitoring for the management of roof conditions during longwalling, and also the prediction of rock and coal bursts. The use and success of this technique have grown greatly over recent years. Another example lies with the use of geophysical wireline logs to predict rock strength and in rock mass classification. Coal seams also provide an unusual geophysical situation in that they form seismic and electromagnetic waveguides. The in-seam seismic and radio imaging methods have been developed to exploit these geological properties. In this paper I address the development and application of geophysical methods such as these with a particular emphasis on their role in ground control. We will see that there have been many developments over the past 25 years but we should also think about the future and how geophysics needs to be developed. From a miner?s perspective, one obvious area of need is in the detection of voids in the vicinity of current mine workings, be they old workings or karsts within nearby carbonate beds.

The paper outlines the development of a solvent extraction process for the recovery of scandium, yttrium, thorium, and heavy lanthanides from waste sulphate liquor at the Port Pirie uranium treatment plant. A summary is given of known solvents for these elements, including the recently developed alkyl-phosphoric acid esters which are applicable to sulphate solutions.A kerosene solution of di- (2'-ethylhexyl) phosphoric acid (EHPA) has been suggested by some workers as a suitable solvent for fractionation of the lanthanides. Experimental work with this solvent showed that it could be used for initial recovery of the above elements from low concentrations in the complex sulphate liquor. In particular, scandium could be recovered as a high grade product by extraction with 0·lM EHPA, followed by a purification strip with 9N sulphuric acid, and final stripping of scandium with sodium hydroxide solution.To enable alkaline stripping to be used successfully, the kerosene solution of the reagent was modified by the addition of a small proportion of nonanol. Experimental work included laboratory scale batch tests and small pilot scale continuous tests.A proposed extension of the process for the separate recovery of individual heavy lanthanides, yttrium, and thorium is described.INTRODUCTIONAt the Port Pirie uranium treatment plant of the South Australian Government, large volumes of "barren liquor", free from 'uranium, are at present run to waste. This "barren liquor" results from sulphuric acid leaching of Radium,Hill uranium concentrate, followed by separation of solid residues and removal of uranium from the clear solution by ion exchange. Ore and waste transports form an integral part of any mining operation and represents a substantial portion of the mine´s total operating cost. Mine layout, truck fleet and size, production rates, haulage distances, shift schedules, maintenance strategies etc. are all important and not at all static variables. They all change during the life of a mine and interact in a dynamic way. It is therefore essential for the successful mine management to constantly analyse and optimise the mine's transport systems. Önnerlöv has recently conducted a life-of-mine haulage simulation project for Zinkgruvan Mining, a medium sized, underground zinc producer, located approximately 60 kilometers south of Örebro, Sweden. The main objective of the project was to analyse and optimise the investments required to provide ore and waste haulage under changing conditions over an +10- years period: Deepening of the mine, increased production rates, changed mine layout, changed truck fleet etc. A large number of scenarios has been tested in the simulation model in order to identify the "lowest cost per ton" solution. This paper comprises an introduction of Zinkgruvan Mining and a brief background to the actual project, a description of the simulation model, project conclusions and some useful hands-on tips for mining engineers and project managers how to run a successful simulation project.

This paper describes the research and development programme to develop a prototype underground auger mining system aimed at developing and demonstrating an underground auger mining system able to provide economic access to thinner (<1.8 m) coal reserves of South Africa. Data concerning the performance of modern coal augers under South African coal mining conditions were gained from a proven surface auger which operated across a range of coal types. These data were analysed and considered in conjunction with historical information regarding previous attempts at building underground augers dating back to the 1950s. Drawing on these data a prototype underground auger mining system, the BryDet BUA 600 underground coal recovery auger, was designed and manufactured by the BryDet Development Corporation in collaboration with Eskom and Cutting Edge Technology Pty Ltd with support from Anglo Coal and the Ingwe Coal Corporation Ltd. The prototype BryDet BUA 600 underground coal recovery auger commenced trials in March 2000 at the Matla Mine near Witbank in South Africa. The initial trials were undertaken in an open cut to facilitate the commissioning and training associated with a prototype mining machine. A total of 30 holes was augered in the box cut before the machine moved underground. The prototype BUA 600 first achieved the target penetration depth of 80 m in its third hole. The prototype BUA 600 is a new underground mining system which has passed early testing with ?flying colours? and could provide access to extensive underground coal reserves unmineable by conventional mining techniques both in South Africa and elsewhere around the world.
