Contemporary approaches to safety management appear to be failing short of meeting its mark in improving mine safety. This is evidenced by the high workers compensation, high incidence rates and fatalities. Evidence from high-risk and complex organisations points towards safety culture as being important in improving site safety. In more recent years resilience engineering has been touted as a new and innovative way of managing safety. This paper reviews and synthesises previous literature on safety culture and resilience engineering. It then highlights methods that can be used to measure safety culture and resilience engineering, and explores similarities and differences between complex organisations and gold mining to identify opportunities for more innovative approaches to improving safety in gold mining operations through safety culture and resilience engineering.

Alcoa operates two bauxite mines at Huntly and Willowdale, located within the jarrah forest south-east of Perth in Western Australia. Ore reserves are shallow but extensive, and mining pits averaging around 20 ha in size are distributed across a mining region in a mosaic pattern, linked to a centrally-located crusher by a radiating network of haulroads. A mining or crusher region is defined partly in terms of economic haul distances, and new regions are opened once the current regions’ reserves have been mined. Since commencing in 1973, the Huntly mine has constructed and progressively closed three previous mining regions and is presently in a transition from the fourth and current region of McCoy to the new region of Myara. At present rates of production, approximately 450 ha is cleared, mined and rehabilitated each year.The value of topsoil in returning a diverse mix of native jarrah forest plant species and supplying nutrients, organic matter and soil biota to rehabilitated areas has long been recognised. Managing the topsoil resource is therefore a critical element in the success of rehabilitation outcomes, but the distribution and dynamic nature of mining operations pose significant planning and logistical challenges. The regionalised approach to mining at Huntly also presents a range of issues for what are, in effect, soil management plans for episodic mine closure. The nature of mining and rehabilitation operations are described, together with the planning processes that Alcoa follows to quantify, track and report on soil movement. A case study of the Huntly mine demonstrates that an integrated operational and life-of-mine planning process is essential for successful and cost-effective mine rehabilitation.CITATION:Hackett, L and Grigg, A, 2014. Tailings storage facilities are geotechnically complicated structures that develop heterogeneous strength profiles and varied porewater conditions. The monitoring of porewater pressures is one of the only direct measurements that can be performed to monitor the performance of a facility throughout its life and is of particular importance in upstream facilities. Foundation underdrainage, continuous and staged tailings deposition, climate inputs (rain, snow, humidity, evaporation, etc), and stratified tailings permeabilities can complicate this critical measurement that introduces uncertainty to stability analyses. These difficult conditions manifest in vibrating wire piezometer measurements as very low or negative pressure readings, both of which are within the instrument’s range of accuracy. However, the data can be difficult to interpret, and a range of interpretations are adopted from optimistic low phreatic surfaces to conservative fully saturated tailings. Where tailings are potentially liquefiable, these very low readings introduce further uncertainty regarding the pore space saturation ratio and liquefaction potential above the estimated phreatic surface. This area of uncertainty is the vadose zone, and its affect on tailings behaviour is not well understood. The two facilities analysed in this paper are upstream, raised embankments with tailings classified as clayey, sandy silt that have been found to be potentially liquefiable. The vibrating wire piezometers (VWP) at both sites show very low or negative readings when they are initially installed near the surface and then go through cycles of re-saturation and de-saturation. Eventually the readings settle as the VWPs are buried deeper within the tailings. The instrumented sections within the tailings column do not show consistent readings that would be indicative of a hydrostatic phreatic surface at either site. Rather, they are highly variable and can be interpreted as either representing a high phreatic surface within the tailings or as a ‘perched’ phreatic zone within the tailings layers. How the VWP data is interpreted has a significant impact on how the facilities are raised and operated. The design outcome for Site 1, which adopts a lower phreatic surface, results in an embankment that meets factor of safety (FoS) requirements under residual strength conditions without any buttressing. Conversely, a conservative (high) phreatic surface has been adopted for Site 2 and has resulted in the facility requiring a significant buttress to meet FoS under residual strength conditions.

For the past 40 years, underground trucking using diesel powered or electric wheel driven vehicles, either conversions of surface trucks, or purpose built, has been the preferred means of ore and waste transport in underground mining. The relatively low capital cost, flexibility and requirement for minimal infrastructure means that trucking systems are selected in the early stages of a mining operation. An increase in the carrying capacity of trucking systems with dimensions to operate within existing development excavations is the solution to long term ore and waste underground haulage systems using trucks. The next generation of underground trucks will be low profile articulated trucks with carrying capacities up to 60 t.

Central Norseman Gold Corporation was established in 1935 and has been mining on the Norseman field in Western Australia since then. It is Australia’s longest continuously operating gold mining operation at nearly 80 years, although it has had its periodic challenges in the intervening years.Over the last 40 years, a series of open pit mines around the Dundas Goldfield have supplied lower cost mill feed to the Phoenix Mill at Norseman from time to time but this has largely come to an end for the foreseeable future. Now that underground ore sources are once again the principal mill feed, there are several challenges to be met for an ongoing profitable operation.During the last 20 years, there have been three different owners and this has affected the manner in which the operations were managed quite significantly. These differing approaches are contrasted and examined in some detail from both the operational and financial aspects.Planning of a downsized operation to ensure sustainable profitability in the future in a mining environment where there has rarely been more than two to four years of reserves ahead of the mill for planning purposes has been a significant challenge. The continuously growing coal mining industry of India presently includes shovel dumper combinations in open cast mines, sometimes with dragline mining of overburden, together with the whole range of longwall and bord and pillar underground mining, continuously undergoing fuller mechanisation. This transition from "low tech" to "high tech" systems is now monitored for reliability and provides bases for comparisons of downtime relative to age and make of equipment, whether imported or indigenous, including spare parts, as well as frequencies of particular failures to allow analysis and progressive improvement. Varying geologies complicate technology and equipment requirements, especially in underground mine,s with minimal technological standardisation, but now, reliability in South Eastern Coalfields Limited has been given more importance and superior technological back-up is being developed.

The green field site is one that has no history of mining and as such it presents the designer with a blank canvas, on which any number of drill and blasting options can be drafted. No matter which mining method is selected all technology and experience will need to be acquired from outside sources, applied quickly and produce the right results. North Limited, in joint venture with the Sumitomo Group (20 per cent), have developed AustraliaÆs first block cave mine at Northparkes in New South Wales, Australia. This innovative and large scale mining project was taken from a green field site in November 1993 to a fully commissioned block cave mine in December 1997. NorthparkesÆ Endeavour 26 mine is now producing at a rate of 3.9Mt per annum from a 27Mt ore block. Prior to 1993 no history of mining activity existed at Northparkes. The rock had never been blasted and many questions were raised about how it would behave. While it was clear that the Endeavour 26 (E26) orebody contained geotechnical properties appropriate to block caving, the blasting methods required to establish a block cave mine had never been used in Australia and no designs were available. The joint venture partners had no prior experience with block caving and it was immediately apparent that this blasting technology would have to be acquired from foreign sources and applied to the unique local conditions. Conceptual designs for the block cave drill and blast were first drafted in 1994, first issued for drilling in December 1995 and first blasted in January 1996. 470 km of drill metres and four million tonnes of rock were blasted to complete the undercutting project by September 1997. This paper will provide those developing new drill and blast projects with an inside view of how one small technical team solved many design challenges to successfully construct AustraliaÆs first block cave mine under strict quality controls and ambitious time constraints.

Truck/shovel operations constitute 50 - 60 per cent of the cost of open pit mining operations. Mining companies, therefore, are constantly seeking to close the gap between actual truck/shovel productive capacity and maximum potential truck/shovel capacity. This paper analyses the performance of a mixed truck/shovel fleet in an open cut coal mine in Australia. It analyses operating delays, and proposes a novel graphical means of determining truck-limited or shovel-limited operations. A goal seeking algorithm is advanced in order to maximise truck/shovel match factors (MF) and hence optimise fleet productivity.

Increased environmental regulations and pressures have created changing and new demands for tailings management strategies in a variety of mining applications. Critical among these items is the disposal of tailings as the pursuit of increased yields and lower costs have driven the application of different process technologies. With a number of technologies having a niche application focus, the belt filter press has been utilised for decades as a cost-effective and versatile option for dewatering different types of mineral tailings and other waste process streams. One of the critical advantages of the belt filter press is the technology’s ability to adapt and work in numerous plant designs, operating conditions and applications. As permitting and disposal options have become more stringent, plant designers, operators and equipment suppliers must develop comprehensive and site-specific economic, design and process evaluation techniques to ensure proper equipment selection and system design for belt filter presses or any other dewatering technologies. This paper will explore these techniques in relation to belt filter presses (with reference to other technologies) in a variety of different applications, along with other considerations to ensure a successful tailings dewatering system and disposal design. Summaries of equipment performance characteristics, operating and maintenance strategies and optimisation techniques will also be addressed in conjunction with the belt filter press’s adaptability to different plant designs and the resulting tailings characteristics.CITATION:Fenzel, M S, 2015. Australia is a leading producer of mineral sands and according to U.S. Bureau of Mines, produced 24, 53 and 64% of world's production of ilmenite, rutile and zircon respectively in 1983. The industry which is mainly export oriented earned in export income about $210 million in 1985. The dominant position which Australia occupies in world produc- tion of mineral sands is also maintained in many areas of research and development work on the processing and application of these minerals. Some of this research which is leading the world has been responsible for setting up industries to produce value-added and high-tech products such as synthetic rutile and partially stabilized zirconia (PSZ). The production of high purity zirconia is due to commence shortly while research in rare earths offers an exciting prospect for future Developments.

Arising from prospecting success, the underlying worth of the shares of a publicly listed mining exploration company can increase many-fold. Prompt and accurate reporting during the exploration stage is therefore essential. While there is scope for varying degrees of optimism or caution in statements about the future economic prospectivity of a mineral deposit which has been discovered, the directors of a mining exploration company have both a legal and ethical obligation to keep their shareholders and the general public fully informed as to the state of progress of, and significant factual results arising from its exploration efforts. Suggestions are made as to the desirable form and content of exploration reports up to and including the time when the orebody has been delineated in terms of assured tonnage and grade. The benefits which will flow from this greater disclosure are discussed.

The high sulfidation Purnama Au-Ag deposit located on the Indonesian island of Sumatra entered production in mid-2012 following a two-year program of construction and development. Discovered in 1997 following a regional stream sediment geochemical sampling program by the Normandy Anglo Asia joint venture, it is the first and largest deposit in G-Resource’s Martabe Project to enter production and is the centrepiece of a series of six deposits totalling 8.1 Moz Au and 73.8 Moz Ag in Mineral Resources (G-Resources, 2013a).In 2011 investigation into the controls of high-grade mineralisation in isolated parts of the deposit identified a potentially significant mineralisation style which had not been previously recognised or accommodated in the resource model. An extensive investigation of core and logging data followed to confirm the distribution of the somewhat unusual mineralisation style – a sedimentary-like cavity infill of very high-grade auriferous pyrite within the steep hydrothermal breccia feeders to the system. With the bulk of mineralisation located within the adjacent volcanic sequence and sandy-matrix phreatomagmatic breccia surrounding the feeders, the lack of constraint for high-grade intersections in the hydrothermal breccia zones had potential to significantly impact the mine plan in the lower sections of the planned pit through overestimation of local gold grade. Combined with new data emerging from grade control drilling and infill resource drilling in the pit, a program of reinterpretation of the deposit was initiated to update the geological and alteration framework underpinning the resource model. Some fundamental aspects of recent single-particle fracture studies are reviewed. Findings from this research shed new light on the utilisation of energy during comminution operations and provide useful pointers to new research directions that could lead to significantly better comminution machines. Two methods for quantitatively predicting mineral liberation during comminution are reviewed. These methods have been developed into useful practical procedures, but they are almost unknown in the mineral processing community. Their use should promote the consideration of mineral liberation as an important variable to be considered during the design and operating control of comminution circuits.

The use of monitors to assess equipment performance is carried out on a routine basis in many industries. The benefits gained through process control in knowing exactly how the equipment is working far outweigh the initial capital costs of the monitor. In the mining industry several dragline monitors have been developed and installed on equipment through out the world, but to date none has been fully accepted by the industry. This paper describes the use of a sophisticated monitoring system which incorporates an onboard processor to assess both dragline and blast performance. The results of a case study are presented and an indication of the potential of the monitoring system to optimise mine production is given.

The increased international focus on sustainable development and cost improvement, coupled with the unpredictability and adverse impact of droughts in Australia and globally, has highlighted the critical need for responsible management of water within the minerals industry. The Rio Tinto Technology Group recently initiated a priority program to examine how water management practices could be improved within the GroupÆs spheres of activity. Rio Tinto Technical Services (TS), the technical services consulting group of the Technology Group, has for many years been undertaking water management reviews and assisting group operations with the development of responsible water management strategies and plans. Rio Tinto therefore requested that Rio Tinto Technical Services develop a world-class approach to mine water management that will drive improvement in strategic assurance and operational efficiency of water at the companyÆs managed operations across the world. The initiative, known as æExcellence in Water ManagementÆ, includes the selection and use of state-of-the-art management, technical tools and methodologies to strive for best practice. The Rio Tinto Technical Services approach facilitates a consistent and objective assessment of performance across the entire spectrum of complex water management issues within a mining operation, from initial exploration through to decommissioning and close-out of the operation. Based on this rigorous assessment, detailed action plans can be developed, prioritised and scheduled for subsequent implementation. This paper presents the progress and experience gained to date in the æExcellence in Water ManagementÆ program, demonstrated by means of a pilot study conducted at Northparkes Mines in New South Wales.

Mine dewatering has long been a problem with mines and some of the shallowest mines have the biggest problems. Machinery was developed by the ancients for raising water by rope buckets with treadmills or by horse whim. From the 1700's on into the early part of this century, massive cornish pumps or equivalent were the vogue powered by steam. One Australian mine pumping system was claimed to be the largest in the world just after the turn of the century. It was installed at Beaconsfield in Tasmania and the mine, like so many other mines of the period, were beaten by problems and cost associated with mine dewatering. The owners of that mine are looking at the feasibility of dewatering using surface drilled wells. Much simpler and smaller pumps are the order now and it is feasible to use boreholes instead of shafts to contain the pumps or else to use the holes to convey discharge waters to the surface from pumps located in drives or stopes.

Mere recognition of the need for technic- ally strong human resources in the'extractive metallurgical industries has not prevented the decline in the education base for broad spec- trum metallurgists. The ability of chemical engineers to perform very effectively in many extractive metallurgical positions does not remove from the need for a general metallurgic- al education system. The intrusion of chemical engineers into extractive metallurgy is only symptomatic of the problems facing the minerals industry and its educators. The prospect of a career in the minerals base industries is unattractive to potential students by virtue of perceived inadequacies in the professional basis of the industry and ignorance of its sig- nificance to Australia's future, as assisted by excessive secrecy and a low profile public Image.

For a multitude of years fine iron ore, -1 mm, across the world has been deposited as discard material in tailings facilities. This contributes to environmental problems, decreases plant yield and increases the cost of processing. Fine iron ore recovery processes are often deemed unfeasible or uneconomical, due to the complexity of the reclamation thereof in comparison with high-grade ores. However, recent advances in the recovery of fine iron ore using the Optima Classifier® has shown great promise. The Optima Classifier® is a new hindered settling separator that makes use of an upward current of water for density separation. It’s unique design combines three different separating zones for increased efficiency and recovery. In this study the potential of the Optima Classifier® to produce a +63 per cent Fe concentrate from -1 mm feed material was evaluated from a standalone and process perspective. It was found that a +63 per cent Fe product can be produced from a single stage, using the Optima Classifier®. Fine iron ore feed with an average head grade of 50 per cent Fe was upgraded to average concentrate grades of 63.2 per cent Fe. Displaying upgrade ratios of up to 1.26 in a single stage. A three-stage, rougher, cleaner and scavenger system did however show the best performance with recoveries of up to 70 per cent, yielding 56 per cent of a 63 per cent Fe product. Furthermore, the dewatering of the product and tailings was evaluated using the UltraG ultra-fine dewatering screen ensuring dry, stackable material. The tailings could be used in codisposal, while loads on thickeners and filter presses are significantly reduced. The high throughput of the Optima Classifier® ensures a small plant footprint, minimal civil and structural requirements and low operating utility consumption such as electricity, minimising the carbon footprint of a fines beneficiation plant. The Optima Classifier® in combination with the UltraG dewatering technology has provided a platform for economical recovery of fine iron ore, both current arising and historical. The hot-dip galvanising industry is the largest consumer of zinc, with over 40 per cent of the world’s total annual production of zinc used for galvanised steel. The dross generated in the hot-dip galvanising coating process is a valuable co-product, since it contains high quantities of recyclable metal. A new method to recover the metal from the industrial galvanising dross was proposed using supergravity separation. During the hot-dip galvanising process, the presence of zinc dross has a serious impact on the surface quality of the hot-dip galvanised sheet, forming zinc dross defects. To reduce the impact of zinc dross, the zinc dross on the surface of the zinc pool needs to be manually or mechanically removed at regular intervals. However, during the dross removal process, a large amount of metal zinc is inevitably carried out, resulting in low zinc utilisation rate and high zinc loss rate. This article proposes an efficient and low-cost technology for the recycling and utilisation of hotdip galvanised dross, which uses super-gravity technology to recover hot-dip galvanised dross online. The metal in the hot-dip galvanised dross returns to the zinc pot in situ, while a small part of the dross remains in the super-gravity equipment, which can reduce the amount of zinc dross by more than 60 per cent and achieve a reduction in zinc loss from 17 per cent to less than 7 per cent.

The unique characteristics of the Target orebody and modernistic approach to mining present significant challenges and a concomitant high potential for new opportunities. The break-away from the traditional labour intensive mining methods improves both safety and productivity significantly. The intention of the paper is to illustrate the application of well known mining methods to the particular requirements of a complex orebody at depth. The evolutionary process followed to establish the final mining method design provides insight behind the finished product, and the modern approach to the planning and service principles equips the reader with a suite of mining engineering ideas.
