Near misses recorded and reported by workers can provide awarenessto the potential causes of injury and prompt safety management initiatives. Although most companies require near-miss reporting, it is unclear what the value of these reports are, if any, and how they influence subsequent actions or controls to reduce on-the-job risks. Researchers at the National Institute for Occupational Safety and Health (NIOSH) conducted a case study with an aggregates company in which near-miss reports were analyzed at each of their locations over an entire quarter during the summer of 2018. Within that quarter, workers recorded 249 near misses. Of those, 167 were valid near misses that occurred at work. Researchers coded the reports using a qualitative 5 × 5 risk matrix. Of the 167 near misses, 19% were deemed low risk, 25% moderate risk, 30% high risk, and 26% critical risk. Several patterns in the near-miss incidents were documented, including classification of incidents and common corrective actions referenced (i.e., elimination/substitution, engineering control or redesign, work process/procedures, and personal protective equipment). The analysis provides insight into ways that risk communication and management programs can be improved to reengage workers and their situational awareness on the job.

In 2004, the engineers at the Deer Creek Mine located near Huntington, Utah initiated the ventilation planning to exploit a remote area of the mine?s reserves. This area was located under a mountain which would not allow for the incorporation of an additional shaft or portal access near the proposed longwall panels. With this constraint along with the desire to maintain a two entry gate road system and the expected condition of frequent coal wash outs a ventilation plan was established. Different options were examined that included; increasing the current exhaust fan power, adding another exhaust fan installation, converting the ventilation fans in the active portion of the mine to a forcing system, and modifying the ventilation system in the active area of the mine to a push-pull system. The push-pull ventilation system was ultimately selected. This paper details the design process of the ventilation system upgrade, the procurement process for the new fans, and the new fan startup testing.

INTRODUCTION There are many mine planning factors in sublevel caving, as in the other mining systems, which when varied can substantially alter mining costs and profit¬ability. In this chapter, the following four topics are addressed with regard to economic optimization of sub¬level caving: production planning, haulage level spacing, orepass spacing, and extraction cutoff. The costs used here are the right order of magnitude but since each mine is unique, they should be considered only as examples. The objective is to demonstrate the tech¬niques which can be used. In a real mine, the evalua¬tions must be done using actual costs and conditions. It is difficult to predict exactly the costs involved, and therefore it is valuable to perform a sensitivity analysis to evaluate the effect of making incorrect as¬sumptions. Since the mining industry is very capital intensive, the effect of the interest rate must also be studied. PRODUCTION PLAN FOR AN IRON ORE DEPOSIT Introduction The first and most important thing to do before de¬signing an underground mine is to establish a long-range production plan. Such a plan is necessary for all eco¬nomic evaluations and should provide information about the lifetime of the mine, how much ore and waste must be handled per year, how much development per year is required, etc. An example is given in the following section. Problem In an iron ore mine sublevel caving is used. The iron content is 42%, and the mine supplies a pelletizing plant with an annual capacity of 3 million t/a. The ore body is shown in Fig. 1. The length of the ore body is 1000 m and the width is 100 m. Each slice is 10 m high, and there are 10 m be¬tween crosscuts, each of which has an area of 20 m2. The density of the ore is 3.5 t/m3. The spacing between rings is 2 m, and the extraction is 100%. The iron con¬tent is 66% in the pellets and 6% in the tailings. The problem is to develop a detailed production plan. A typical sublevel caving sequence is shown in Fig. 2. Solution Amount of ore per meter of crosscut: 20 X 3.5 = 70 t. Number of crosscuts per slice= 100. Length of crosscuts per slice: 100 X 100 = 10 000. Ore from crosscuts per slice = 10 000 m X 20 m2 X 3.5 = 700 000 t. Number of sublevel caving rounds per slice: 10 000/2 = 5000. Area for each blast in sublevel caving: 10 X 10 - 20 = 80 m2. Amount of ore per blast: 80 m2 X 6 m X 3.5 = 560 t. Extraction: 100% (see extraction curve Fig. 12). Loaded ore per blast: 75% or 420 t. Loaded waste per blast: 25% or 140 t. Total: 560 t. Total amount of rock from each blast in the sublevel caving: Ore: 560 t of which 2 X 70 = 140 t from develop¬ment work and 420 t from sublevel caving. Waste: 140 t. Total: 700 t. The ore needed per year is 3 X (66-6)/42-6 5 mil¬lion t. Necessary number of blasts per year= 5,000,000/ 560 = 8929. Distance to develop per year = 8929 blasts per year X 2 m per blast 17 858 m. Total amount/year: Ore from development work 17 858 m x 70 t/m = 1.25 million t/a Ore from sublevel caving: 8929 blasts per year X 420 tons per blast = 3.75 million t/a 5.00 million t/a Waste rock dilution: 8929 blasts per year X 140 tons per blast 1.25 million t/a Total amount to hoist 6.25 million t/ a The amount of ore from development work will in¬crease a little if the horizontal drift is placed in the ore body and not in the footwall. In this example the ore loss in 20%, and the waste rock dilution is also 20%. After taking the ore loss into account, the lifetime for 100 m of the ore body will be:

This report describes Koppelman's work to develop the K-Fuel® Series C Process used to dry and stabilize Powder River Basin coal. It also contains summaries of the results of bench-scale and pilot plant tests conducted to prove the Series C Process. The process is being used by the KFx-Thermo Ecotek partnership for a 500,000 ton per year commercial plant that has been constructed near Gillette, Wyoming. The plant is presently undergoing shakedown.

The mining industry continues to experience a skills shortage and this is having an impact on the ability of mining companies to operate sustainably. Apart from a few exceptions, there is a dearth of geologists, mining engineers, metallurgists as well as environmental scientists and other professionals in the global mining industry. This relates to both quantity and quality. It makes good business sense for mining companies to employ local professionals rather than expensive expatriate staff. In some cases a mining company could employ more than 10 local mining engineers for the cost of one expatriate. However, in many countries local professionals are not always equipped professionally to work on mine sites. The result is that many companies are not operated optimally due to human resources gaps. In some cases, particularly in key areas such as mine planning, this threatens the sustainability of the operation or at the very least results in sub-optimal resource utilisation. This paper briefly presents some of the issues and strategies being implemented and that might be implemented to maintain a sustainable supply of quality minerals graduates to the industry in 2011 and beyond.

Over the past decade, a substantial effort has been made to improve the air quality inside enclosed cabs of both underground and surface mobile mining equipment to reduce respirable dust exposures to the equipment operators. As part of this effort, the National Institute for Occupational Safety and Health, (NIOSH) completed a comprehensive laboratory study which determined the significant factors for cab filtration and pressurization systems. From this information, a major underground mining equipment manufacturer designed a filtration and pressurization system which was incorporated into the enclosed cabs of its equipment. A long-term evaluation was performed on the effectiveness of this filtration and pressurization system to improve the air quality in the enclosed cabs of two different pieces of equipment, being a face drill and a roof-bolter machine, at an underground limestone mine. This long-term evaluation demonstrated a significant reduction in dust levels comparing outside to inside cab respirable dust concentrations. During this evaluation, a modification to remove one of the filters on the roof-bolter machine simplified the design without sacrificing the system?s efficiency. Testing using particle count instruments performed during non-production time periods on both pieces of equipment indicated Protection Factors greater than 100 when comparing respirable-sized dust particles inside the enclosed cab relative to outside.

Precipitated calcium carbonate is used as a filler and coating material in the manufacturing of paper because of its high brightness and light scattering characteristics. However, beneficiated calcite has similar potential, given the fact that the industrial specifications are already achieved. For paper industries, the requirement is that the desired product should be pure and have a brightness of 93 to 95 percent. A very-high-grade calcite deposit of around 92 percent is located in the north of Norway, which is used as the raw material for ground calcium carbonate (GCC) feed for the paper industries. There is a substantial amount of calcite resource, which has pyrite and pyrrhotite as major impurities. These sulfides, even when present in minor concentrations, impart color to the end product (paper). Removing these minerals is estimated to increase utilization of the deposit by 10  percent, which represents several million tonnes of calcite marble. Due to the requirement for environmentally friendly reagents and chemicals by the mineral industry, fatty di-amines are being tested for pyrite and pyrrhotite flotation. The interaction mechanism of diamine with the main components of the ore were studied and optimum conditions for removal of sulfide impurities are outlined.

An ilmenite concentrate was reduced in the solid state with carbon under argon atmosphere at temperatures between 1000oC and 1400oC. The ilmenite concentrate and 50% excess carbon were reacted to achieve the highest attainable degree of reduction at each temperature and the reaction products were subjected to a series of analytical work by optical microscopy, X-ray diffraction analysis and SEM ( EDS) analysis. The ilmenite remained unaltered at 1000oC and had a slight change in its composition at 1050oC. A secondary oxide phase containing metallic nuclei was detectable at 12% reduction at 1100oC. The reduction of TixOy took place in a sequence changing the y/x ratio from 2.0 to a value less than 1.0 at 84% reduction at 1400oC. Metallic phase consisted of Fe and C initially and started to include Ti at the later stages. Complete reduction of TixOy to metallic Ti was not possible at these temperatures.

If one can embrace the medical analogue, much of the mining industry currently suffers from a massive bacterial infection. When pyrite-bearing or sulfide-bearing rock formations, tailings, or mine wastes are infected by Acidithiobacillus ferro-oxidans, the likelihood of forming acid rock drainage (ARD) is almost guaranteed. The ?pharmacy? of antibiotics available is extensive, ranging from solid alkaline amendments like limestone to liquid ?medicines? such as sodium lauryl sulfate, sodium thiocyanate, waste milk, and bipolar lipids. Unfortunately, the ?geo-medical? teams of geochemists, microbiologists, engineers, and mine managers lack the tools to surgically apply these active ingredients where they are needed most with a minimum of waste. Distribution of fine grained limestone on the surface of an acidic mine waste dump is analogous to applying a bandage soaked in antacid to treat an upset stomach. The implementation of up-to-date best management practices has not healed the patient; an equivalent combination of hypodermic needle, cyber knife, and arthroscopic probe is clearly needed. Using an engineered, flow-able or pumpable foam or pHoam? as the medicinally analogous ?dextrose delivery solution? for solid and/or liquid ?geo-antibiotics?, the authors have combined off-the-shelf technologies that have been previously applied in solving geotechnical problems in the mining industry. A patent for the innovative process is pending. This paper discusses method concepts and the advantages it could provide over conventional BMPs.

Four companies mined ball clay in four states during 2005. They included H.C. Spinks Clay (owned by Franklin Minerals), Kentucky-Tennessee Clay (owned by Imerys), Old Hickory Clay and Unimin. Production was estimated to be 1.32 Mt (1.45 million st) valued at $53.3 million, based on the results of a preliminary survey of the ball clay industry. Production in 2004 was 1.22 Mt (1.34 million st) valued at $54.1 million. Tennessee was the leading ball clay producer state with 61 percent of domestic production, followed by Texas, Mississippi and Kentucky.

Introduction Rockfalls occur when rocks or boulders detach from mountains or hills and tumble down. They can pose significant hazards to infrastructure such as highways, buildings, and mine open pits, and they sometimes result in personal injury or death. Prediction of rockfalls is a difficult task. Slopes that are at risk of rockfall have highly variable geometries. The location, mass, and shape of the rocks that might dislodge and fall are uncertain. Moreover, the materials that make up a slope can vary considerably from one section to another. Furthermore, the relevant material properties are usually not well known. Performing probabilistic simulations of rockfalls, combined with proper statistical analyses, has proven to be an effective and acceptable method for dealing with these difficulties [1, 2, 3]. The movements of a falling rock can be classified as freefall, bouncing, sliding, toppling, or rolling. The falling trajectory is controlled mainly by the geometry of the slope, the block shape, and the energy dissipated at each contact of the block with the slope. A rockfall model is evaluated based on its ability to efficiently predict the velocity, frequency, height of bounce, and run-out distance of falling rocks. With this information, the engineer can design remedial measures such as restraining nets and ditches. Most of the existing rockfall simulators are based on particle models that consider the falling rock as an infinitesimal particle with a mass, called the lumped mass or stereomechanical models [1, 2, 3, 4]. Other models involve hybrid methods relying on simplified assumptions. The most common hybrid methods are based on contact searching for the movement of a dimensionless object and incorporate some aspects of rigid body impact for bouncing [5, 6, 7]. There are also some rigid body models in the literature that employ simplified versions of rigid body models [8, 9]. This study applies a more sophisticated rigid body theory based on rigid body impact mechanics (RBIM), which was developed by Stronge [10, 11, 12]. The new theory captures all modes of rigid body movement. Due to the importance of the rock geometry, single impacts of ellipsoidal and prismatic rocks with rectangular cross sections are investigated. The effect of rock slenderness on rebound velocities and energies is also investigated for ellipsoidal rocks. The correspondence between the model parameters and the data from the rockfall literature data will be discussed.

A unique partnership between the Colorado Mine Safety and Training Program (CMSTP), the Colorado Rock Products Association (CRPA) and the Higher Education and Advanced Technology (HEAT)Center resulted in a video CD-ROM training program for new miners entering the aggregate industry. Cooperation from everyone involved, including the U.S. Mine Safety and Health Administration (MSHA),made this the product against which other training materials in the mining industry will be measured in the 21stcentury. This product’s versatility and wide application make it a tool that is as critical to a mine’s success as any earthmoving machine. The state and federal grant money involved allows this product to be affordable to small operators and independent contractors. The quality of this two CD-ROM set rivals any commercially made CD-ROM training.

A novel dry and wet air dust filter concept was conceived and demonstrated successfully at two mines. A steel-wool and a fiberglass filter, suitable for underground use were identified through laboratory testing and demonstrated in the mine. The steel wool filter was operated with and without water sprays while the fiberglass filter was only operated in the dry mode. The dry steel wool filter provided a dust capture of 37% at a negligible pressure drop. When fine misting water sprays were used on the steel wool filter, the dust capture increased to 51% again with a negligible pressure drop. The filter screen also remained clean and no change in air flow characteristics was observed as a function of time. The dry fiberglass filter however did not perform as well achieving only 11% dust capture while introducing a significant resistance to air flow. This technology is seen to have several applications in underground coal mines.

The gold content of porphyry copper deposits ranges from less than 0.05 g/t to more than I g/t ((0.0017 to 0.035 oz per st) and the average is about 0.05 g/t (0.0017 oz per st) gold. Perhaps the highest grade mineralization found near a porphyry cop¬per deposit is the Ok Tedi Gold Cap ore body whose grade falls in the 2 to 3 g/t (0.07 to 0.1 oz per st) range. It is difficult to define a gold-rich por¬phyry copper deposit. All porphyry copper ore bodies contain some gold, but there seems to be a fairly sharp break in the gold content at about 0.1 g/t (0.003 oz per st) gold. There is also a problem in identifying dis¬seminated gold deposits that may have an indirect relationship to porphyry copper systems. This relationship is clear cut in deposits such as the Ok Tedi Gold Cap, the El Hueso deposit, the Fortitude and West de¬posits at Battle Mountain, NV, and the Sur¬prise deposit at Ely, NV. There are many other bulk disseminated, epithermal-type gold deposits - McCoy, NV, for example - that might be related to a porphyry system but where this relationship is remains very uncertain. In such places as the Goldfields and Tonapah district in Ne¬vada and in the El Indio, Chile district, there may be a vertically stacked ore deposit rela¬tionship between near surface epithermal deposits and disseminated molybdenite and chalcopyrite enargite mineralization at depth that may represent a deep porphyry system. This, however, has not been positively demonstrated. Most of the bulk epithermal gold deposits are probably not related to porphyry copper systems. Exceptions would probably be Ok Tedi, El Indio, El Hueso, Battle Mountain, and Lepanto where vertical or lateral zoning relationships are more apparent. This author believes that porphyry copper systems are related to a relatively unique type of intrusion that is probably distinct from, for example, the McCoy intrusivebody. For that reason, deposits such as McCoy, Chimney Creek, and the like will not be included in the definition of gold-rich por¬phyry copper deposits for this article. The interpretation of the origin of several of the deposits previously mentioned are subject to an unusual amount of contro¬versy. The Buick mine, located 195 kilometers (120 miles) southwest of St. Louis in the Viburnum Trend, is owned and operated by Homestake Mining Company. The mine produces lead and zinc ore from a room and pillar mining method at a depth of approximately 335 m (1 100 ft). The Buick orebody typically averages 60 to 120 m (200 to 400 ft) in width with ore thicknesses ranging from 2.4 to 36.6 m (8 to 120 ft). The mine is divided in two; the halves are referred to as the North mine and South mine orebodies. The North mine orebody contains the higher grade ore; however, the developed reserves for the South mine orebody comprise about 60 percent of the total developed reserves for the Buick property. Existing pillars represent one-third of the total developed reserves.

Dry coal separation technology has particular advantages in the applications in arid and cold regions, or low rank coal separation. The hazardous pollutants in low rank coals can not be efficiently removed by conventional wet coal separation processes because a considerable amount of porose and easily friable low rank coals suffer serious particle size degradation problems in water. In this paper, a novel dry coal separation method with a vibrated air-fluidized bed of magnetic fly ash and magnetite is put forward for dry coal beneficiation. The magnetic fly ash used in this study was derived from fly ash obtained as a byproduct of coal combustion. The vibration?s effect on the uniformity and stability of a magnetic fly ash air-fluidized bed was studied. The 6×0.5 mm dry coal separation results with a vibrated air-fluidized bed of magnetic fly ash and magnetite showed that the probable error E value of separation is 0.06-0.08 t/m3.

The complex nature of the carboniferous measures prevailing in the U.K. has dictated the types of mining required to exploit the coal seams. Most mines in Britain are referred to as deep mines in the sense that they operate at depths of 1800 - 3000 feet. The main system of coal transport to the surface is by vertical shafts which intersect the workable seams which at times number between 4 and 5. As mining has been practiced in most of the major coalfields in the U.K. for well over 100 years, with the initial extraction of the shallower and more profitable seams, the present management have been left a legacy of indiscriminate extraction which severely hampers their efforts to maximise the mine layouts. There is no single standard design of mining layout capable of meeting the wide variety of conditions which exist in the British coalfields. Mining layouts are subject to fixed and variable parameters. In discussing multi-seam working, the main parameter must be the effect of old workings above and below the seam to be extracted. There is a distinct lack of reliable information which would enable layouts to be designed to completely avoid or control these destructive forces. The guidelines are, therefore, mostly empirical.

"PROJECT OVERVIEWThe Liantang/Heung Yuen Wai Boundary Control Point (BCP) and Associated Works will be the seventhland crossing between Shenzhen and the Hong Kong Special Administrative Region (HKSAR) of China.The project includes site formation of about 23 hectares of land for the development of the BCP and anapproximately 11-kilometre (km) long dual two-lane trunk road (the “Connecting Road”), connecting theBCP with the existing Fanling Highway. The route alignment is within a relatively rural area of HongKong, with very limited similar infrastructure works carried out nearby. It passes beneath bothmountainous terrain, and alluvial plains, and falls within an area of several major regional faults andassociated metamorphism, resulting in a complex variety of ground conditions.ALIGNMENTThe Civil Engineering and Development Department of the Government of the Hong Kong SpecialAdministrative Region carried out extensive public engagement in selecting the alignment of theConnecting Road. Over 70 consultation forums with public bodies, including the District Councils, RuralCommittees, local representatives, etc. were held during the alignment design process. In order toalleviate the concerns of various nearby villages, and to minimize land clearance and environmentalproblems, the original alignment of 10 km long (3.5 km of tunnels and 6.5 km of viaducts) was revised toits final alignment of 11 km long which comprises 5.7 km of tunnels, 4.3 km of viaducts and 1.0 km of atgrade roads.The finally adopted alignment commences at the southern end with an interchange with the existingFanling Highway, part of Route 9, which is a dual three lane expressway currently being widened to dual4 lane. Alongside the expressway is the main rail connection to the mainland of China, and theinterchange viaducts connecting to the southern portal of the Lung Shan tunnel must cross over thisrailway. The southern portal is located in the foothills of Lung Shan, and the 4.8km long tunnel is alignedin a north-northeasterly direction. The southern half passes under mountainous terrain of Lung Shan,and then crosses a valley containing the Kwan Tei river. The northern section comprises low hillsseparated by an alluvial plain and Tan Chuk Hang village area. At the northern end, the tunnel turns tothe north-west to connect with Sha Tau Kok road, an existing single carriageway road, with a newroundabout interchange. Mid-way along this tunnel there will be a ventilation building, connected to the main tunnel by a 330m long ventilation adit. To the north-western side of this interchange is the900m long Cheung Shan tunnel, and after this the remaining 4.3km of the highway connecting with theBoundary Control Point is to be constructed as an elevated viaduct. Introduction Fire assaying is an ancient method of determining the amount of gold and silver in geological materials. Today's commercial, mining, school, and private fire assaying laboratories still use this basic assaying method. One of the primary reagents used to collect gold and silver from the ore is litharge (PbO), or lead oxide. The Federal Occupational Safety and Health Administration's (OSHA) and the Nevada Division of Occupational Safety and Health's (OSH) have a lead standard. It states that a person exposed to airborne lead at concentrations greater than 50 µg/m3 (1.4 µg per cu ft) of air over an eight-hour period has gone beyond the permissible limit for health safety. It is not uncommon to get airborne lead concentrations of 1400 µg/m3 (40 µg per cu ft) in the flux/sampling area or 900 µg/m3 (25 µg per cu ft) around the gas/ electric furnaces during the fusion and cupelling phases. Therefore, any individual working in these areas for more than 30 days per year without proper protection is a likely candidate for lead poisoning. Lead In A Human Lead enters the body as a toxic substance by breathing it into the lungs and upper respiratory tract and by getting lead particles into the digestive system through ingestion. Smoking, eating, drinking, chewing tobacco and gum, or wiping a lead-contaminated hand across the mouth are all potential ways forgetting lead into the body. Lead taken in large quantities can actually kill a person in a matter of days. This is rare, however. The real hazard of lead overexposure is for the unsuspecting person who works in an unsafe lead environment for months or years. This individual can slowly receive damage, sometimes permanent, to the central nervous system, gastrointestinal tract, kidneys, blood, gums, and reproductive system. Safe Blood-Lead Level The Lead Standard states that the maximum safe blood-lead level is 40 µg/100 g (40 µg per 3.5 oz) of whole blood. It recommends 30 µg/100 g (30 µg per 3.5 oz) if a person is of child-bearing age. People not working around lead products normally have a blood level of less than 10 µg/100 g (10 µg per 3.5 oz). A person not adequately protected can exceed 40 µg/100 g (40 µg per 3.5 oz) in a few weeks by fire assaying only 100 samples per day per week. Studies show that some individuals have developed lead-related diseases with blood-lead levels less than 80 µg/m3 (2.3 µg per cu ft). Lead Poisoning Symptoms There is a long list of symptoms associated with lead poisoning. The most common are: a prolonged, sweet, metallic taste in the mouth; loss of appetite; anxiety and irritability; weakness and dizziness; nausea and tremors; and insomnia. Preventive Measures Based on the lead standard, a number of preventive measures can be taken to ensure that fire assay laboratory personnel frequently exposed to lead are protected against overexposure. First, the actual lead concentration within the assaying area should be determined. In Nevada, one can contact the Department of Industrial Relations, Division of Occupational Safety and Health for assistance in monitoring any laboratory. The agency will also recommend ways to protect against lead. This agency does the monitoring and provides an educational class on lead safety. This type of agency can be found in other states. Commercial consulting firms are also available, but they are expensive. Regardless of the technique used to determine lead concentrations, it is most likely that the average fire assay area exceeds the 50 µg/m3 (1.4 µg per cu ft) lead concentration limit. The softwall mining system is a novel, new method of mining mineral deposits in soft, shallow, nonrock ground conditions. This method can mine the ore layer without substantially disturbing the existing overburden. The softwall mining system consists of machinery that is hydraulically controlled and powered, that can be buried (or used on the surface) to extract the ore or mineral from a layered deposit. The method is essentially an application of the longwall mining concept, modified for loose overburden and ore conditions, combined with hydraulic mining.
