The National Institute of Occupational Safety and Health (NIOSH) is dedicated to achieving "safety and health at work for all people -- through research and prevention." An important procedure in pursuing this goal is the NIOSH surveillance studies where the number of injuries are monitored according to demographic, employment, and injury characteristics. These numeric data are then used to identify injury risk factors, develop safety programs, and monitor implemented remediation methods. In essence, ongoing surveillance data form the bases for directing the resources of NIOSH in setting of research and prevention priorities, and in evaluating safety procedures. This surveillance techmque, which is based on the Public Health Service approach to identification and control of disease (1, 2, has been used quite successfully in dealing with relatively high number injuries such as hearing loss, musculoskeletal disorder, respiratory disorder, falls, etc. (2). However a problem arises when extending this paradigm to lower frequency traumatic events; particularly those often referred to as disasters (i.e., high impact events like mine explosions, airplane crashes, etc.). One can ask the question - there hasn't been a coal mine fire fatality in the U.S. for over ten years, to what extent should we be concerned with mine fire safety (resource-wise)? - The National Traumatic Occupational Fatalities (NTOF) system (4J which NIOSH uses to survey occupational fatalities annually, would suggest that underground mine fires would not be a high priority item in occupational safety. Yet, miners' (i.e., the exposed workers) do consider mine fires and explosions to be major safety concerns. I believe that we need to rethink our evaluation of work place disasters, what is worker exposure, and what are the underlying assumptions (both explicit and implicit) as to the impact that economic, societal and degree of hazard factors make on risk evaluation. The answers to all these problems, will require much broader considerations than those in current surveillance safety and health models. This paper discusses some facets of those considerations.

METHANE EMISSION PEAKS Methane emission from the coal at continuous miner faces varies considerably. Plotted on a chart, methane emissions consist of a series of peaks and valleys corresponding to the cutting cycle of the mining machine, with the methane concentration spiking as the machine cuts into the coal (Figure 3–1) [Kissell et al. 1974]. "In the land-pebble district of Florida a lively interest in the possibilities of obtaining increased recoveries of phosphate rock is evident. Research in several directions is being carried on by companies operating in the district to increase the yields obtained by the washing plants. The Bureau of Mines is cooperating with this research program to promote more efficient plant practice and to avoid unnecessary depletion of mineral resources.Washing practice, as carried on in the district at present for the recover of phosphate rock from the crude ores or matrix, is not a complicated process. Flow sheets of the several companies are similar, although the equipment varies with the preferences of the individual operators. For disintegrating the matrix log washers are favored, although revolving drums equipped with flights to break up lumps of clay are also used. Fine sand is removed by screening. Vibrating screens have become well established in the district, although stationary flat screens and trammels are also employed.The writer acknowledges with appreciation the cooperation of the phosphate companies in the land-pebble district; suggestions and criticism by Will. In these sizes the phosphate grains are accompanied by grains of sand - largely quartz - of the same size, and recovery of these phosphate grains as a commercial product is not possible by simple washing methods. Phosphate recoveries approaching complete extraction can only be effecte by applying metallurgical processes, either mechanical or chemical. As yet, most of these processes have not advanced much beyond the laboratory or perhaps the pilot-plant stage. Such processes may have some application to the beneficiation of phosphatic ores or washer rejects, but a discussion of them is not within the scope of this paper."

Provide ear protection which allows normal, unaltered hearing at low noise levels, yet protects the wearer from excessive noise. The Approach: A commercial earmuff with integral speakers has been adapted with a special amplifier so that low sound levels are not affected, but higher noise levels automatically reduced. How It Works: The discriminating earmuff developed by the Bureau of Mines has two modes of operation. In the first mode, where input sound levels are less than or equal to 83 decibels, the wearer will perceive noise at its actual level. For inputs in excess of 83 decibels, the second mode of operation begins and the sound perceived by the wearer is progressively attenuated, or reduced, as the input level is increased. A recognizable difference still exists between two adjacent sound levels enabling the wearer to differentiate between them.

1. Introduction This document represents Part II of our final report on the development of a "Remote Sealing System for Extinguishing Coal Mine Fires. " It is a technical report which summarizes the results of system development and field demonstration. Part I of this two - part final report is an Operator's Manual for the application of the Remote Sealing System to a coal mine fire. 1.1 Background When coal mine fires go out of control and can no longer be fought directly, the standard fire fighting technique is to construct air-tight seals in the mine passages leading to the fire area so'--,that the oxygen supply to the fire is". cut off. Ultimately, it is desirable to isolate the fire zone in such a way that normal operations can resume in the remainder of the mine. The construction of seals in the- mine causes the atmosphere to pass through a toxic and explosive phase. Explosions, if they occur, are not only hazardous to the men working at the sealing operation, but can also undo much of the sealing work which has already been accomplished. Sealing of passageways surrounding the fire zone from within the mine is extremely difficult and dangerous. Methods for direct sealing are detailed in "Control of Mine Fires'. 1 An alternative to this approach is the technique of remote sealing. This involves drilling boreholes which intersect the mine passages at the locations where the desired seals are to be constructed. The seals are then constructed by pouring material down the borehole and filling a segment of the passage from bottom to top. During this operation, there are no personnel in the mine, all operations taking place on the ground surface.

The process of drilling and bolting the roof is currently one of the most dangerous jobs in underground mining, resulting in about 1,000 accidents with injuries each year in the United States. Researchers from the Spokane Research Laboratory of the National Institute for Occupational Safety and Health are studying the use of a drill monitoring system to estimate the strength of successive layers of rock and assess the integrity of a mine roof so that roof drill operators can be warned when a weak layer is being drilled. Measurements taken during drilling can be converted to suitably scaled features so that a neural network can classify mine roof strata in terms of relative strength. The feasibility of this concept has been demonstrated in the laboratory. The research project was undertaken in order to increase the safety of underground miners, especially those involved in roof bolting. The system should be applicable to the mobile drills used in underground mining and would likely find wider application as well.

To be successful , human exertion employed for the creation of value requires power and heat ; the ability to produce concentrated power in great quantities has resulted in the relatively recent industrial development that is so profoundly affecting the world . In our commercial history , we have advanced through several stages in the selection of the most suitable fuels , continuously striving to increase the flexibility and range of energy- transmission . The easy distribution of light , power and heat is the leading factor in the decentralization of industry which promises to correct some of the serious evils that have followed in the footsteps of our commercial development . The distribution of fuel in quantity in so convenient a form and with such an attractive reliability- factor as can be had with gas , meets the modern conception of efficient service . Natural gas has been used in this country for more than a century, but recently it has attained a prominence far in excess of anything known in its earlier history. We may consider that we are now getting into our real stride in the utilization of our natural- gas resources . : In a country like the United States , wherein we are accustomed to mammoth achievements , we may be inclined to accept casually the noteworthy technical and economic advances which have resulted in the expansion of natural- gas distribution . About three years ago , the transmission of natural gas a distance of more than 250 miles from its source was first accomplished , and this was really a noteworthy engineering feat . Today , this useful commodity is being transported twice that distance , and lines are being laid which will carry gas more than 900 miles from the points of occurrence . Let us not take such outstanding improvements too much as a matter of course ; the industry should be congratulated on its long steps forward .

"The wide publicity that is given to the discover; of new oil fields has led many people to believe that our supply of crude petroleum is practically in- exhaustible and that in order to keep pace with the demand for petroleum products, it is only necessary to drill a few more wells in some of the south-western states. It is not generally realized that, although many famous fields have been developed in Oklahoma, Kansas, Texas and Louisiana during the past few years, the production of crude oil in 1919 was not sufficient to meet the demand and it was necessary to import approximately 53,000,000 barrels from Mexico.Continued efforts are being made to recover a larger percentage of the oil underground by improvements in methods of drilling and in handling producing properties. Improvements are also being made in methods for handling the oil after it has reached the surface which will result in reducing losses through leakage and through evaporation. These methods for cutting down waste in production and storage, if universally adopted, will no doubt result in appreciable increases in our supply of crude oil."

This bulletin deals with the characteristic features of the principal explosives used in mining and engineering operations, and espe- cially with the tests that show the suitability of different classes of explosives for different kinds of work. The Bureau of Mines pub- lishes the report as one of a series dealing with tests of explosives and methods of reducing the risks involved in the use of explosives in mining operations. The application of explosives to engineering and mining opera- tions has increased rapidly in recent years. According to a pre- liminary report issued by the Bureau of the Census, 469,000,000 pounds of explosives was manufactured in this country during the year 1909, and the greater part of this production was used for industrial purposes. In large engineering projects and in mining operations requiring the use of explosives the selection of a suitable explosive from the many varieties offered for sale is of fundamental importance. Vari- ous considerations are involved in the selection of an explosive of proper class for the blasting to be done. Many explosives suitable for quarry work are unsuitable for use in deep mines or in close workings. In metal mining and in driving tunnels the character of the gases evolved by the explosive on detonation is an important consideration. An explosive for use in gaseous or dusty coal mines must be formulated and compounded so that its flame temperature and the height and duration of its flame are reduced enough to per- mit its being used with comparative safety. In wet workings or in submarine blasting explosives impervious to moisture are requisite. In extremely cold climates explosives that do not require thawing are desirable, provided they are equally good in other respects. An essential requirement of all explosives, especially of those for use in tropical countries, is that they shall remain stable without change in chemical or physical characteristics.

Each year at least one and in some years several explosions are reported as having resulted from firing of mudcapped shots in coal mines; undoubtedly a number of small or local explosions occur that are not reported; in addition there are numerous accidents, some of them fatal, attributable to this reprehensible blasting practice. It is surprising that so few explosions and other accidents from this cause occur since the practice of firing mudcapped, Even in gassy "adobe," "dobe," "sand blast," or "bulldozed" shots is widespread. mines where general blasting is safeguarded by being done with permissible explo- sives, rocks or bowlders frequently are blasted with mudcapped shots of dynamite; even if permissible explosives are used, this type of shooting is dangerous. The usual method of preparing a mudcapped shot is to place the explosive, either in loose or cartridge form, with the detonator or "cap" on top of a piece of rock or coal and cover it with several handfuls of moist clay, loam, sand, or When the shot is road dust; in many cases there is no covering of any kind. fired a dense cloud of dust, together with an intensively hot fleme, are thrown into suspension, and if the dust is explosive or ignitable an explosion or igni- tion is likely to result practically simultaneously. The presence of gas greatly increases the explosibility of the dust, as well as the probability of an ignition. Every shot, of any known explosive, fired in a coal mine without having the charge properly confined in a drill hole and tamped with incombustible stemming to the collar of the hole, is capable of initiating an explosion that may cause the death of every man in the mine. The U.S., Australian, and United Kingdom coal and the Canadian hard rock mining industries have long recognized the significance of high horizontal stresses as a factor affecting the stability of roof and rib conditions in underground mines. Recently, a growing segment of the U.S. underground stone mining industry has also begun to recognize that horizontal stresses occur in some of its more than 90 mines. Considering the typically high strength and massive nature of limestone, this fact is a revelation in itself. High horizontal stresses produce extensive and sudden rock failures and, in some cases, resulted in injuries to mine workers. Through the years diverse control strategies have been proposed and experimented with. Reorientation of mine entries to reduce stress concentrations have proven successful and are widely accepted in practice. Other solutions, like rock reinforcement, are poorly understood and less accepted in practice. It is the purpose of this National Institute for Occupational Safety and Health (NIOSH) study to develop a better fundamental understanding of these ground control strategies under high horizontal stress conditions through a series of field and laboratory studies. To this end a design technique is presented which provides stone miners with a method for making stability assessments. The consequences of widening rooms, changing geology and horizontal stresses, and different rock bolts on roof beam failures are discussed.

This paper presents one of a series of digests of foreign mining legisla- tion and court decisions that is being prepared in advance of a general report relative to the rights of American citizens to explore for minerals and to own and operate mines in various foreign countries. This interpretation of the laws of Switzerland has been prepared from the best information available in ashington, but is released subject to correction and amplification, if necessary, by the proper American diplomatic and consuler officers, to whom it is being referred through the courtesy of the Department of State. Sitzerland is a small country in which little mining is done, yet it has a voluminous body of mining legislation. There are two salt-mining districts in Switzerland; that at Bez, Canton of Vaud, belongs to the Canton but the deposit is worked by a private company; and those at Schweizerhalle, Rheinfelden and Ryburg are worked by a joint stock company formed by the Cantons interested. Iron ore is mined at Gonzen, Canton of St. Gallen, and large beds of gypsum are worked, mainly for domestic consumption. In general, however, although many kinds of minerals are said to have been found in Switzerland, mining operations are irreguler end relatively unimportant. Even during the world war only about 2,500 persons were employed in mining establishments and most of these were in under round stone quarries. On the other hand, the republic comprises 22 Cantons, of which 3 are divided, making 25 units, and in the absence of generel Federal legislation, each of these units virtually has its own distinct mining la Under these circumstances, it is obviously inadvisable to review the mining lars of Switzerland in the same detail as those of other countries which Lave been digested in this series. For the lars of individual Cantons, as well as for a more comprehensive summary of the Swiss legislation as a whole, the reader is referred to a recent German publication." It is now widely accepted that horizontal stress is present nearly everywhere underground. Often it is more severe than the vertical stress. It is a major cause of roof control problems in underground mines. The AHSEM computer program contains two modules to improve mine layouts for control of horizontal stress. For development, the user specifies the orientations of the maximum horizontal stress, the entries, and the crosscuts. Angled crosscuts may be used, and the angles may be different for the right- and left-hand crosscuts. AHSEM then shows the user how the stress condition would vary as the entry and crosscut orientations are changed. The second module is for longwall mining. Severe stress concentrations can make headgate conditions hazardous when the entries are improperly oriented. AHSEM shows whether a particular longwall orientation and extraction sequence places the headgate corner in a stress concentration or in stress relief. It also estimates the magnitude of the stress concentration.

Due mainly to conditions brought about by the war , the metal mining industry in this country is now passing through one of the most critical stages in its history , and is confronted by new problems upon the solution and adjustment of which depends , in a large measure , its future welfare and prosperity . The chief factors contributing to the cost of production are the cost of labor and supplies . It is conceded by our leading mine operators , managers , and engineers that any material reduction in the cost of labor or supplies , within possibly the next two or three years , cannot be anticipated , anl it is not thought likely that pre - war prices will ever again prevail . This being true , the mine operator must look in some cther direction to effect economies that will lower production costs . If this cannot be accomplished it will mean , in many instances , that mines and metallurgical works will have to be closed and abandoned , which will tend to disorganize the entire industry . How to lower production costs and still maintain the present high standard of wage , is the problem that is engaging the serious attention of mine operators and managers over the entire country . In As a solution to this problem it is thought that the most promising field of investigation lies in the direction of systematizing and standardizing mining practice , so as to effect economies in the use of both labor and materials . other words , to eliminate , in so far as possible , unproductive effort and waste and thereby secure a maximum of productive results , It is not the idea to speed up work or to demand a greater effort on the part of the workman , but rather so to systematize and standardize the various under ground operations that his energy will be directed into more useful and productive channels ,

This report describes three cases of carbon monoxide (CO) poisoning in a manhole, including one fatality, from CO migrating through soil after nearby use of explosives. A municipal sewer project involved installation of new pipes and manholes. Work of the general construction contractor was interrupted when a subcontractor detonated 265 pounds of nitroglycerin-based explosive 40-60 feet south of the manhole to break up underlying rock. A construction worker who descended into the manhole 45 minutes after the explosion collapsed within minutes, and two co-workers descended into the manhole to rescue him. One rescuer retrieved the unconscious worker, and the other rescuer died in the manhole. All workers had elevated carboxyhemoglobin levels. In the subsequent NIOSH investigation, air monitoring was conducted with real-time instruments, and air samples were collected in Tedlar bags. Laboratory analyses of the bag samples collected near the bottom showed 1905 parts per million (ppm) CO, 19.5% oxygen, and 3% carbon dioxide. Direct reading instruments showed progressively higher concentrations as the sensor was lowered into the manhole. Subsequent chamber tests on sample explosive yielded 27 liters CO per kilogram detonated. Based on this value, the surface blast at the construction site may have produced about 3,250 liters (114.8 cubic feet) CO. The CO in this incident most likely was released from the nearby explosion and migrated through soil and fractured rock into the manhole. The blasting and construction industries should be made aware of this previously unrecognized CO exposure hazard associated with surface blasting. The extent of CO exposure from explosives used in construction is not known, and additional information on the extent of CO exposure must be collected. In addition, confined space entry procedures (including monitoring confined space atmospheres before entry) should be observed; CO monitoring of confined spaces in the presence of blasting can prevent future incidents such as this one.

"The Generic Mineral Technology Center for Respirable Dust (GTCRD) was created at The Pennsylvania State University and West Virginia University by a congressional initiative in 1983 responding to recommendations in the 1980 National Academy of Sciences study on the Measurement and Control of Dust in Mines. Many of these recommendations dealt with the need to initiate focused fundamental research for further professional progress in dust control. Long-term, sustained support was emphasized to develop expertise in respirable dust research. In creating this Center, Penn State and West Virginia University were specifically recognized for their expertise in and facilities for research on coal mining and medicine. Today, the Center includes the Massachusetts Institute of Technology, the University of Minnesota, and the Michigan Technology University. The Respirable Dust Center is the only organization that coordinates and integrates the medical, scientific, and engineering aspects of respirable dust disease. The Center works with the U.S. Bureau of Mines (USBM), Mine Safety and Health Administration (MSHA), the National Institute for Occupational Safety and Health (NIOSH), the United Mine Workers of America (UMWA), and the U.S. coal industry in defining the research priorities for the programs of the Center to enhance research synergism.The Center's mission is to enhance the health, safety and productivity of miners by advancing the fundamental understanding of all aspects of respirable dust associated with mining and milling and the interaction of dust and lungs to reduce the incidence and severity of respirable dust disease.All Dust Center activities are focused to be responsive to this mission, which is shared by the several federal agencies closely associated with the GTCRD. While significant progress has made mine atmospheres cleaner and healthier, respirable dust-related disease remains the nation's most pernicious industrial hazard.The goals of the GTCRD were formulated with recognition of the diverse scientific, engineering, and medical aspects of respirable dust, in addition to a need to enhance researcher interaction, as well as education and technology transfer efforts, in the respirable dust area. These goals reflect the needs of the mining industry as determined by the National Academy of Sciences Committee on Measurement and Control of Respirable Dust study done in 1980. The report of this committee encouraged the Bureau of Mines to redirect its research efforts ""...toward obtaining fundamental understanding of the origin, transport and characteristics of respirable coal mine dust."" The GTCRD's goals, detailed in chapter three of this document, were formulated to mesh with NAS recommendations."

During the period of 10 years from January 1, 1908, to January 1, 1918, approximately 12,850,000 barrels of oil and 5,024,506,000 cubic feet of gas were destroyed by fire in the United States, entailing a total estimated property loss of $25,254,200. During this period 503 fires were reported. As is shown in Plates I and II, 310 of these fires were caused by lightning and 193 by all other causes. The losses from the fires caused by lightning were estimated to be $11, 148,000, and from those due to all other causes $14,106,200. Directly and indirectly, the fires resulted in the deaths of nearly 150 persons and were responsible for almost as many more being permanently disabled. For the past three years the Bureau of Mines has been conducting investigations to determine the nature and the specific causes of these fires, with a view to suggesting means whereby they may be suc- cessfully combatted and, in some places, perhaps eliminated entirely. As the subject of oil and gas fires is necessarily broad, no attempt is made in this bulletin to treat it exhaustively; rather the bulletin aims to point out what has been done by operators in the past, and to describe various fire-prevention methods and fire-fighting appa- ratus which are being used or adopted by many of the larger oil companies. These methods and apparatus, it is believed, if uni- versally employed by operators, will largely decrease the present enormous annual losses. In the discussion of oil-well fires, the methods used in extinguishing several of the larger and more perplexing ones are described. As the right procedure for extinguishing any oil-well fire must be de- termined largely by local conditions, it is not probable that all of the methods used at some one fire, as described, will be applicable to a future fire. However, the operator, from these descriptions, may at least gain suggestions that will greatly assist him in fighting similar fires.

This information circular presents data for 1960 on individual coke plants in the United States. All oven-coke plants in existence, whether active or idle at the close of the year, are listed in this report. With two exceptions, only beehive plants on which reports were submitted to the Bureau of Mines are included. Although the Bureau was unable to obtain reports from these two active plants in 1960, the number of ovens and annual coke capacity are estimates, based on previous reports filed with the Bureau of Mines. Accordingly, there were 73 oven-coke plants in existence at the end of 1960 of which 70 were active. Reports were received on 44 beehive plants of which only 13 were in operation at year end. Oven-coke plants are listed alphabetically by States in table 1 which shows the name and address of the company, location of plant, classification or type of ownership, number and type of ovens, annual coke capacity, and coal-chemical materials produced. The terms "furnace" and "merchant" plants used in this and all other reports of the Bureau of Mines designate the type of ownership. Furnace plants are those owned by iron and steel companies whose main business is production of coke for use in their own blast furnaces. Merchant plants are those associated with chemical companies, gas utilities, as well as those that produce for sale on the open market metallurgical, industrial, and residential-heating grades of coke and coal-chemical materials. The annual coke capacity shown for each plant in this table is based on the minimum coking time necessary to produce coke with qualities suitable for its intended use. Therefore, the annual capacity of a plant may change from year to year, depending on age and condition of ovens, the character and quality of coal carbonized, the grade of coke required, and other factors. For example, annual coke capacity of a coke plant primarily making foundry coke with longer coking cycles would be lower than one making blast-furnace coke. Also some ovens must be operated at slightly lower than normal flue temperature because of the condition of the refractories and maximum capacity would be somewhat lower than designed capacity. For this reason, the capacity reported to the Bureau of Mines may differ from the designed or rated capacity estimated by the coke-oven builders at the time of construction.
