A recent headline in the Wall Street Journal declared, "trendy management schemes very often fail." Many of the management techniques addressed in the accompanying article are associated with quality management including reengineering, benchmarking, total quality management, and employee empowerment. The article made the point that executives, employees, and customers who are involved in such programs rate the effectiveness of these programs very low, and the programs often fail. Yet, we all know of companies that have made significant advances in their operations using these same quality management practices. Why do a few succeed while many fail? History has shown that the companies that successfully carry out quality management programs have two things in common: 1) Strong leadership committed to the success of the program. 2) A comprehensive plan for carrying out and maintaining the program. Stated another way -the reason many companies fail is the lack of committed leaders and inadequate planning. It is ironic that the title of the Wall Street Journal article is "The Best Laid Plans."

High-quality sample selection for metallurgical test work is essential to a geometallurgy study, but the large multidimensional dataset makes sample selection a daunting task, as classifying the dataset while respecting its heterogeneity is difficult. This paper presents a streamlined approach for sample selection, utilizing custom-built tools in Python to standardize the methodology, saving time and costs. This approach uses the cumulative sum method, principal component analysis, and k-means clustering method to elegantly cluster the data and select representative samples. A case study is used to demonstrate the effectiveness of the methodology by selecting 40 samples for flotation test work.

INTRODUCTION The decontamination of the former Shiprock uranium millsite and the interim stabilization of its tailings piles began in March 1974, and ended in June 1978. Personnel monitoring for whole body gamma ray exposure was conducted for selected individuals working at the mill site throughout most of this period. The millsite and exposure conditions are briefly described to develop a basis for interpretation of the exposure data. Millsite The Shiprock uranium millsite is located in the community of Shiprock, New Mexico on a bench overlooking the San Juan River (Figure 1). Both Shiprock and the millsite are located on the Navajo Indian Reservation. About 1.5 million tons of tailings were produced on the millsite during its operation from 1954 to 1963. The tailings were pulped with waste process fluids and water, and discharged to form two tailings piles. The piles (ponds, dams) were formed with earth starter dikes and the dikes were then raised using the separated sand fraction of the tailings. The slime fraction of the tailings were discharged into the diked impoundment. The mill was shut down and partially dismantled before the lease expired in 1973, and the remainder of the mill and tailings reverted back to the Navajo Tribe which was the lessor. Portions of the millsite were then occupied by the Navajo Engineering and Construction Authority (NECA), a tribal enterprise. In addition to its regular activities, NECA established an on-site training school for heavy equipment operators. The trainees principally used the south tailings pile as a practice area and spread the tailings into a 10 hectare row of raffinate ponds located along the southwest side of the millsite. Overall, the training activities inceased the effective area of the tailings piles from 20 to 35 hectares. Another 56 hectares were contaminated with wind and water eroded tailings. Exposure Conditions Persons decontaminating a millsite may be subject to radiation exposures from several sources. Only whole body gamma exposures are discussed in this report. These exposures are arbitrarily divided into three levels: high, intermediate, and low. The levels are related to areas in which the decontamination work was being done and have no connotation concerning imminent hazards. High exposures would result when persons were working with raw tailings, and intermediate exposures would occur when persons were working in the ore storage and mill yards. Low exposures would result from working in areas where surface contamination exists from wind and water erosion. [ ] The millsite and its contaminated environs were divided into major areas to facilitate the decontamination work (Figure 2). Copper has been mined in the Philippines since at least as early as the Ming dynasty (1368 A.D. to 1644 A. D.). The principal source areas of this early production were in Camarines Norte (Paracale), in the Mountain Province, which now boasts the largest number of Philippine porphyry copper deposits, in Masbate and in Surigao, The Spanish Era began in 1521. Although they were primarily interested in gold, the Spaniards operated several copper mines including the San Remigio mine on Panay and the Lepanto mine in northern Luzon, beginning in 1842 and 1865 respectively. Prior to World War II most of the Philippine copper production was supplied by these same two mines together with the Hixbar mine, Rapu Rapu Island which began operation in 1939. The Hixbar and San Remigio mines are cupriferous pyrite deposits and the Lepanto is a vein-¬form replacement deposit (Kinkel et al. ,I 1956). No significant copper production was derived from porphyry copper occurrences prior to world War II.

The three-dimensional finite element technique combined with the progressive failure approach was utilized to simulate stress distributions in a mine section which occur in response to a longwall mining operation. Due to the availability of the Cray supercomputer, a large-scale three-dimensional finite element model was developed for the analysis. The Hoek-Brown failure criterion, which describes a realistic behavior of rock masses, was used for the simulation of progressive failure. A comparison of calculated and monitored stresses in the mine has shown that the model gives a good representation of stresses in the mine structure.

Rare earths were not mined in the United States in 2008. However, processing of intermediate rare-earth concentrates continued throughout the year at the Mountain Pass Mine in California. Consumption of refined rare-earth products increased. U.S. rare earth imports were primarily from China, with lesser amounts, in decreasing order, from France, Hong Kong, Japan, Austria and Russia. The United States imported 100 percent of its supply of yttrium metal and compounds primarily from China. United States In 2008, lanthanide concentrates were processed by Chevron Mining Inc. (previously Molycorp Inc.) at its mine site at Mountain Pass, CA. Chevron Mining sold its entire holdings in the rare earths mine to Molycorp Minerals LLC in September and operations continued at the site through-out 2008. Major uses for these mineral commodities were in automotive catalytic converters, petroleum fl uid-cracking catalysts, permanent magnets, glass-polishing compounds, ceramics, metal-alloying additives, phosphors and recharge-able batteries. Three domestic companies produced and sold lanthanide intermediate concentrates and refined products. Chevron Mining sold its interests in the Mountain Pass Mine to Molycorp Minerals LLC on Sept. 30, 2008. Molycorp Minerals LLC is an investment company owned by Resource Capital Fund IV L.P., Pegasus Partners IV, LP, The Goldman Sachs Group Inc., Traxys North America LLC, Carint Group LLC and Mark Smith. The Molycorp operation previously mined the rare earth fluocarbonate mineral bastnäsite by hard-rock surface methods, and the mine remained on care and maintenance in 2008. Molycorp maintained significant stocks of bastnäsite concentrates, intermediate concentrates and high-purity rare earth compounds at its operations at Mountain Pass. In October 2007, Chevron Minerals completed phase one of its three-stage plan and restarted the rare earth separation plant at Mountain Pass. The company also processed intermediate rare earth concentrates from existing stocks during the first three quarters of 2008. Molycorp Minerals LLC maintained production in the last quarter of the year. The mine at Mountain Pass has been closed since 1998. Future plans are to restart processing of stockpiled bastnäsite concentrates followed by reopening of the mine.

A recent advance in slope dewatering-depressurization and stabilization is the use of vacuum assist horizontal drainage. (Brawner and Pakalnis) Horizontal drains have been used for several decades to reduce water pressures in slopes to improve stability. The benefit from vacuum assist arises from an increased hydraulic gradient caused by an induced negative atmospheric pressure. A research program included a laboratory model whereby the effects of vacuum drainage were evaluated. Three case studies are described where the technique has been successful. The first is in a rock slope where flows increased from 0.1 1/min. under gravity drainage up to 1.2 1/min, upon vacuum. The second study involves installation of 17 drains placed at a slightly downward gradient to intercept and de- pressurize aquifers under a major tar sand deposit in Canada. The third study was on a large open pit copper mine in British Columbia where flow increased about 30 per cent.

Researchers from the National Institute for Occupational Safety and Health (NIOSH), in cooperation with mine operators, conducted a study of heat stress exposures among mine rescue workers in underground mines. Mine rescue workers face extreme heat loads as they enter hot and poorly ventilated environments, particularly when they are wearing breathing apparatuses. The burden of wearing a closed-circuit breathing apparatus (CCBA), the inability to drink fluids for extended periods of time, and the potential for ventilation disruptions and fires combine to increase the risk of heat illness during a response to an emergency. In this research, ambient environmental conditions and heat strain indicators were measured using conventional ventilation monitoring tools during mine rescue training exercises. Heat strain was measured with an ingestible temperature-sensing pill that measured core temperature continuously. In addition, a heart-rate monitoring chest strap was used to indicate physical strain resulting from physical activity and heat. Both core temperature and heart rate data were trans-mitted to a remote recorder worn by a rescuer and time-stamped. Team activities were also observed to determine the contribution of work rate to a person’s total heat load. The effectiveness of several engineering controls, such as cooling the air inhaled by rescuers through an apparatus, wearing cooling vests, and supplying water through the CCBA masks, were investigated. Administrative controls, such as limiting the duration of exposure and controlling the work rate through forced-rest regimens, were also evaluated. Results to date are discussed, and tools for evaluating team condition and estimating the length of time it would be safe for rescuers to work are proposed.

Mine-to-mill improvements can be initiated at any stage, but it is our assertion that drilling accuracy is a practical anchor in any operating mine or quarry for a structured continuous improvement process. This paper will: 1. Define deviations from plan and provide some best practices for maintaining drilling accuracy. 2. Introduce tools and methods to monitor and calculate consequences of accurate versus inaccurate drilling. 3. Demonstrate the use of these tools and methods and their impact on one cost item, the mitigation of oversized material by a hydraulic hammer. 4. Provide recommendations for implementing improvements.

The United States began exporting coal in the late 1800'5, at first in very small quantities to Canada and beginning in 1897 and 1896, to the east coast of South America. Shipments to European countries began on a very limited basis in 1902 and these shipments increased gradually until World War One when shipments overseas increased from the 8 million tons per year level to 22 million tons. After the war exports dropped as fast as they had increased and, with the exception of exports to Canada, remained at very low levels until just after World War 11 when the United States again emerged as a major coal exporter. World War II left the coal industries of Europe in ruin. As the United States was the only country able to supply the large quantities of coal at reasonable prices needed for reconstruction of the European economy, the U. S. coal exports overseas expanded rapidly to 42.5 million tons in 1947. Additionally, we shipped 25 -2 million tons of coal to Canada In that year for a total export of 68.7 million tons. Unfortunately, that level was not reached again until 1957 when exports reached an all-time high of 76 .4 million tons- -a high I might add that has not been reached since.

INTRODUCTION Newmont Gold Company's Rain operations are located in Elko County, Nevada, approximately 9 miles southeast of the Town of Carlin.The property was submitted to Newmont Exploration Limited in 1979 and underwent development drilling over the course of the next four years, culminating in the completion of a project feasibility study in October 1986. Initial project scope included development of an open pit mine, an 815,000 DSTPY heap leach, a 572,000 DSTPY mill (Mill No. 3), and associated support facilities. Approval to proceed was given by Newmont Gold Company management in June 1987. Detailed engineering began in early July, followed immediately thereafter by procurement of major equipment components through various second hand sources. PROJECT DESIGN Keep It Simple In early August, Newmont Mining Corporation, parent company of Newmont Gold Company, became the takeover target of an "investor" group headed by T. Boone Pickens. Details of the ensuing events have been well documented by the press; suffice to say their impact on this project was material and required immediate flowsheet changes. Project design throughputs were increased to 1,000,000 DSTPY and 760,000 DSTPY for the heap leach and mill, respectively, in mid- September. These changes caused major project headaches as all general arrangement drawings had been approved and structural steel as well as concrete contracts were in place. Additionally, ground breaking was to commence in two weeks time! Rain project management assembled shortly after the announced changes, to devise an engineering and construction strategy capable of handling these new business conditions. The project had been justified using a gold price of $325 per ounce with a forecasted overall cash cost per ounce of $230. Hence, strict control of capital expenditures was necessary to maintain development economics. Mill start-up had also been moved forward one month, to August 1, 1988. To meet an already ambitious schedule, construction techniques needed to be simple and proven as most of the facilities were planned for erection during winter at a site with design criteria of -40F and 90 mile per hour winds. Perhaps the most critical factor requiring consideration was that of personnel. Newmont Gold's revised business plan called for corporate-wide production increases from 587,000 ounces per year in 1987, to 1,400,000 ounces by mid- 1989. Labor requirements were forecasted to grow from 900 employees to over 2,100. This fact, in combination with the rapid growth of Nevada's gold industry, led to the expectation that turnover of operating personnel would be extremely high. It was concluded that, to meet cost, time, and operational goals, the necessary strategy could be succinctly put - "Keep It Simple." "Organic smoothing additives are used to produce dense, thick, high-quality cathodes in copper electrowinning. Glue had been used traditionally and is still used in operations that do not employ solvent extraction to produce the electrowinning electrolyte. With the advent of solvent extraction–electrowinning, guar started to be used and remained in use for many years as it does not affect the phase disengagement in solvent extraction. Recently, however, guar has been replaced by less-expensive saccharide- and polyacrylamide-based additives. This paper is the first of a series of four papers that examine saccharides and polyacrylamides as new additives for copper electrowinning. A review of previous research related to copper electrowinning additives is presented here, along with a selected review of additives in other copper electrodeposition systems.IntroductionThe recovery of copper from solution by electrowinning was first practiced in the early 1900s, but it was the combination of solvent extraction with electrowinning (SX-EW) in March 1968 at the Ranchers Bluebird Mine (Kordosky, 1992) that changed the landscape for the electrowinning process. Most copper electrowinning capacity is now associated with solvent extraction, though some copper is still won directly from leach solutions (Robinson et al., 2013). The ability to produce high-purity copper cathodes from SX-EW has taken the continued improvement of SX-EW technology over the past more than 40 years.One of the technologies that has seen improvement is that of the organic smoothing agents used during the electrodeposition of copper in the tankhouse. These organic additives are added to the acidic copper sulfate electrolyte during electrowinning to improve the smoothness, brightness and density of the copper cathodes. The organic additives that are presently in use are different from those used in 1999 (Jenkins et al., 1999). Over the past decade, many tankhouses have replaced the use of guar-based organic additives with those derived from other saccharides. Until now, there has been no comprehensive publication in the open literature on the behavior of these additives during copper electrowinning."

The MIDREX® Direct Reduction Process was developed during the 1960s by the Midland-Ross Corporation, which had pioneered shaft furnace technology and stoichiometric gas reforming for use in processing minerals. In 1969, the first MIDREX® Direct Reduction Plant was built in Portland, OR. Since then, 49 additional modules have been sold in 18 countries. For the last ten years, over 60 percent of the world's direct reduced iron has been produced in MIDREX Plants. During the quarter century since commercialization, many technical improvements have been made, including larger shaft furnaces, in-situ reforming, greater heat recovery, improved catalysts, and hot briquetting. Midrex continues to enhance the technology in the areas of economy-of-scale, energy efficiency, alternate fuels, and raw material flexibility.

"Metallurgical process development in greenfield projects must define the comminution flowsheet and establish the equipment sizes that are required to deliver the project design criteria. These are critical steps that heavily influence the financial performance of the project, requiring a detailed understanding of the ore properties of the project resources and the best practice methods to size comminution machines. This paper reviews current practices for determining the ore characteristics relevant to comminution circuit design. Particular attention is given to the design of sampling and testing programs, and the methods commonly applied to size crushers and mills for conventional size reduction applications in mineral processing. IntroductionMetallurgical process development in greenfield projects requires the selection of a comminution flow sheet and must establish the equipment sizes required to deliver the project design criteria. This necessitates a detailed understanding of the ore properties that define the project resource. For brownfield projects the same objectives may apply for plant expansions, or the project objective may be simply to define the capacity and operating cost of an existing facility for a new ore source. The process development phase in either instance will typically comprise the following key activities:• Geometallurgical classification and sampling of the ore deposit or deposits.• Determination of comminution energy requirements.• Benchmarking similar projects.• Identification of flow sheet options.• Equipment selection and sizing.This paper is concerned specifically with process development testing and will as such focus on the first two activities on the list: sampling and testing of the deposit. The discussion will focus on testwork programs to support the design of comminution circuits commonly in use in mineral processing today, specifically crusher, tumbling mill and stirred mill technologies.It is important that any testwork program is designed giving consideration to the types of flow sheets that are viable for the project, and the data that will need to be generated to support the final equipment sizing. Designing a testwork program to only provide information about one particular type of flow sheet will not allow sufficient information to be generated for a rigorous trade-off analysis of options, and may therefore preclude financial optimization of the project. Testing programs should be designed to collect sufficient data to ensure that any backtracking at a later stage to fill in missing information is minimal."

During the past five years the use of all-hydraulic drilling equipment has increased rapidly, and we have now reached a point where the hydraulic drill has become an alternative to be seriously considered for every drilling operation. The hydraulic drills have many advantages over pneumatic drills, but they also have certain characteristics, which makes a potential user hesitate before making the switch. And when the decision to use hydraulic drills has been made, the user then faces another choice of equal difficulty: the choice between different hydraulic systems. This paper discusses some of the factors to be taken into account when considering hydraulic drilling equipment for rapid tunneling in hard to medium hard rock. THE CHOICE BETWEEN HYDRAULIC AND PNEUMATIC EQUIPMENT The choice between a hydraulic and a pneumatic drill is much more than just that: It is a choice between two different systems. The choice is usually based on the productivity, economy or ergonomy of the system. However, also other factors should be considered, such as service capabilities and training requirements. And finally the choice will be influenced by the need to acquire know-how in the new technology. Abstract. Dynamic experiments were performed in a continuous open-circuit 40x40 cm ball mill using a pseudo-random binary sequence of the feed rate and measuring the variations of the discharge particle-size distribution. The impulse response is calculated by a cross-correlation technique. Then a model involving a discrete transfer function and a time-series stochastic equation is calibrated using maximum likelihood methods. Based on this empirical model. a minimum variance controller with constrained input is designed and its performances are compared. by simulation, to the performances of a conventional PID regulator.

A new coal boom is under way in western Canada. As recently as the mid-1960's, the coal industry in western Canada was considered stagnant and, by some people, dying. But Japan's growing need for coking coal, which became more pronounced in 1966, sparked great interest in western Canadian reserves.' As a result of a rapidly expanding export market and growth in domestic demand, Canadian coal production spurted to 16.6 million tons in 1970. Production in 1965 was 11.5 million tons. Today, about 60 percent of Canada's output comes from western Canada, but by 1975, 87 percent (23.1 million tons) is expected to come from this area, and 96 percent (68.3 million tons) by 1990. Opening, developing and operating new coal mines to meet the needs of a rapidly expanding market for western Canadian metallurgical coals pose some unique and difficult problems. Covering nearly every aspect of opening properties in remote areas, the problems are met in exploration, transportation, construction, mine operations, coal preparation, manpower and ecology. The Folsom Area Stormwater Improvement Project is part of SFPUC’s flood resilience efforts under the Sewer System Improvement Program. This 4,000-foot-long, 12-footinside- diameter tunnel will help reduce the risk of flooding in San Francisco’s low-lying Inner Mission neighborhood. The project is in a high seismic hazard zone and extends through a complex geologic corridor that includes full-face soft, compressible Bay Mud, Colma Sands, and full-face rock with several stretches of mixed-face conditions. Additional challenges include removal of 99 piles beneath an existing box sewer, tunneling under commuter rail tracks, and an underground tie-in. This paper examines these challenges and how they are being addressed.

The state of Arizona produced 253,276.2 metric tons (279,000 st) of crude gypsum during 1986. This represents approximately 1.7% of the total U.S. production. Four mines provided Arizona's gypsum, three of these mines are in the San Pedro Valley. The uses of the gypsum produced in Arizona are for manufacture of cement, agricultural uses, and wallboard manufacture. The gypsum is used as a retarder in the manufacture of Portland cement. The use in agriculture is treatment of alkali soils to allow water penetration. Other deposits in the San Pedro Valley have production potential, providing there is sufficient market for additional wallboard. The author completed an evaluation of one property earlier this year.
