The Approach: Develop a battery-powered mine-rescue vehicle designed to carry rescue crews, rescued miners, and their support equipment. How It Works: The experimental vehicle, shown below, was designed and constructed by the Mine Safety Appliances Company under contract to the Bureau of Mines. ATV Manufacturing Company and Delco Electronics were subcontractors. A rechargeable, silver-zinc battery provides power to a 3-horsepower permissible motor, the speed of which is regulated by a solid-state controller. Three wheels on each side are independently driven by a chain and mechanical trans-mission.

Introduction The International Nickel Company have been pioneers in mine rescue work in Ontario. Following the fire in the Hollinger mine in the spring of 1928, the Company purchased ten self-contained oxygen breathing outfits, ten all-service gas masks, an oxygen high-pressure pump, life line, inhalator, ten cap-lamps with batteries, and a complete stock of spare and accessory parts. An instructor from the U.S. Bureau of Mines, Pittsburgh, came to the Creighton mine for one week and trained twenty men in the use and limitations of all equipment. Monthly training courses were commenced in the fall, and have continued to the present time. Of the original twenty men trained, ten are still on the Creighton mine rescue teams. In the meantime, and as an aftermath of the Hollinger fire, the Work-men's Compensation Board of Ontario and the Ontario Department of Mines established a central mine rescue station at Timmins. This was completed in the spring of 1930. It was then decided, in view of the great expansion in mine development in the Sudbury district, to build and equip a similar station to serve all the mines there, namely, Creighton, Frood, Garson, Falconbridge, Sudbury Basin, and Errington. An extensive survey was made and a central location selected opposite the Frood mine. The International Nickel Company donated a site, and the station was completed in the fall of 1930. The station is a two-storey brick building, 27 ft. by 47 ft., with a large garage adjoining it. The ground floor is used for equipment and for training the crews. On the second floor are living quarters for the superintendent and his family. The garage houses a closed truck, in which all necessary equipment can be taken to any desired point for training purposes or for actual mine rescue work, if and when necessary. The garage is also used as a gas room, where the men are trained in an irrespirable atmosphere. A gas-tight observation window between it and the training room enables the superintendent to see and direct the men undergoing training, weight exercisers and other devices being used.

An extensive trial of laser-based survey and monitoring systems at Hillgrove Resources Kanmantoo Copper Mine in the Adelaide Hills, South Australia, has proved the capabilities of the system in monitoring rock movements on open pit walls. Initially, the trial involved deploying a Maptek I-Site 8820 laser scanner and the proof-of-concept Maptek Sentry software in the Nugent Pit to monitor ductile rock movements located up to 260 m away. The system was proven to be capable of monitoring various rock movements from 1.5 tonne rockfalls up to a 40 m high toppling wall failure. Data collected from smaller rockfalls was used to help build a database for Hillgrove’s rockfall management program. As part of the trial, the laser scanner was deployed to monitor a ramp movement event, as well as the toppling failure, adjacent to an IBIS slope stability radar. Both devices monitored the slump and the slowly developing toppling movements, with similar results. Although the radar was used as the successful ‘critical alarm’ device for the toppling failure, post-failure analysis showed the laser-based system to be capable of performing a similar role in the ductile rock regime; albeit over much shorter distances than the radar capability. A second laser scanner was deployed to monitor Kanmantoo’s Emily Pit in early December 2014, where it successfully provided critical alarm warnings for a block topple event. Sentry software, which takes advantage of Maptek I-Site 3D spatial imaging technology, was first released in November 2014 to meet industry demand for a cost effective, flexible system to detect movement. Sentry has now been re-launched as a fully transportable monitoring system. Deployed in a custom trailer, the system offers a power and communications module, cellular and Wi-Fi networking and a stable bollard for the I-Site laser scanner and Sentry software. This paper will present rock wall movement data from the Sentry trial and a subsequent two years of monitoring of the open pit walls at the Kanmantoo Copper Mine, at distances out to 750m. In addition, examples of remote geological mapping and small rockfall detection, using the laser scans, will be described. The Sentry system has proven to be beneficial for the continual improvement of slope monitoring and risk management at hard rock mines. "Roof bolts are the primary means of roof support in underground mines in the United States. Presently, post installation loads are measured on about 10% of the expansion anchor and combination bolts by using standard torque wrenches that Unit the accuracy of the measurements to about ±30%. No system has been available for measuring the loads generated on full-column resin grouted bolts. Mine roof bolts are long, slender, non-uniformly sized, roughly forged, and placed in a corrosive environment, creating obstacles for researchers trying to utilize ultrasonic reflection techniques to measure strain. A complete measurement system has now been developed by the U.S. Bureau of Mines, Denver Research Center. This system is being used successfully in determining loads measured on expansion anchor and combination bolting systems, examining the transfer mechanics of full-column, resin-grouted bolts, and measuring loads generated on bolts in an underground roadway support system.IntroductionMillions of roof bolts are used each year to provide structural support in underground U.S. metal, non-metal, and coal mines. Approximately 45% of the bolts used are expansion anchor, 45% are resin-assisted combination or full-column resin-grouted, and 10% are friction anchored. The effectiveness of these bolting types is dependent on a number of factors including rock density, bolt length, in situ stresses, rock properties, bolt pattern, installation tension, etc. When implementing expansion anchor and resin assisted combination bolts , installation and post-installation tension, or load, is a major determinant of support quality and subsequent behaviour of the reinforced roof. The method by which these systems carry roof loads is well understood and documented(1,2).The importance of these installation and long-term bolt tension measurements is well recognized and are required in the federal government regulations(3). A statistical sample of the bolts, lout of 4 during installation and lout of 10 inby the last open crosscut, is required to be measured using current torque wrench technology. The disadvantages of this system are the disturbance and weakening of the anchorage and limitations of accuracy to approximately ± 30%. The method by which resin-grouted bolting systems carry roof loads is much less understood, mainly because testing procedures used to evaluate point-anchor systems are not effective on resin-grouted bolts.Due to these problems, the U.S. Bureau of Mines designed a measurement technique that provided a highly accurate, low cost-per- bolt system that led to the development of ultrasonic pulse echo instrumentation. The original system was designed to measure the tension only on expansion anchor bolts. Recent developments and system modifications have permitted measurements on resin-assisted and full-column resin-grouted bolts. The use of these types of support systems has been increasing steadily in underground coal mines.The development of this instrumentation has provided laboratory and field results that help to explain the mechanics and relationships between the applied loads and the interaction between the grouted bolt system consisting of the bolt tendon, grout, and rock."

It is necessary to fully understand roof geological features in order to design a proper roof support system for underground coal mines. These geological features include: rock type, rock strength. rock layer interfaces, voids, cracks, and bed separations. Traditionally, engineers have relied heavily on the use of surface borehole log data to identify geological features. Since surface boreholes very expensive to be drilled in close spacing, log data are often insufficient to provide detailed information on the roof geology. Recent advances in control technology have made a large amount of roof bolter drilling data available to mine personnel. These data can be obtained during normal roof bolt installation cycle with little or no interruption in mining operations. This research project has developed several methods to identify geological features in a mine roof from measured drilling parameters. Power trend line analysis can be used to locate discontinuities and interfaces in mine roof. Power averaging can be applied to determine the relative strength of roof strata. Thrust trend line analysis can be used to locate fractures in mine roof. The results of these methods are combined to produce a mapped roof geology In a production environment, manually interpreting and managing collected drilling parameters is not practical since hundreds of holes will be drilled during a production day. A new software package, MRGIS, has been developed to allow mine engineers to make use of the large amount of roof drilling parameters for roof support design. An actual underground test site is presented to demonstrate the analysis and display capabilities of MRGIS.

The ground forces generated by a properly installed and tensioned mine roof truss assembly can provide permanent mine roof support, even in severe ground conditions, either by direct suspension of the rock loads within the potential failure zone above the mine opening or indirect reinforcement of the natural rock arch that tends to form within the immediate mine roof. Notable advancements in mine roof truss support systems hardware and design technology have occurred since the introduction of the in situ mine roof truss concept for permanent mine roof support in 1966. The systematic installation of mine roof truss support systems has proven to be a very effective method of mine roof control room and pillar mines under a wide variety of ground conditions, even in areas otherwise considered unmineable because of severe roof control problems. Properly designed mine roof truss systems have also been used for permanent support of mine slopes and haulage tunnels.

Employee safety and health is of paramount importance for SME and its members. Many SME members work daily in our Nation's mines or have a direct impact on others that work in the mines. SME supports the mission and purpose of the U.S. Mine Safety and Health Administration (MSHA), which is to prevent all injuries and disease from mining and to promote safe and healthful workplaces for the Nation's miners. SME supports the National Institute for Occupational Safety and Health and university research geared toward prevention of all occupational injuries and illnesses. SME maintains that laws and regulations governing the mining industry need to be based on sound scientific principles to ensure that workplace solutions can be readily implemented and that solutions to safety and health problems are fully realized.

This chapter deals with the Mine Safety and Health Administrations's (MSHA] concern and obligations with regard to asbestiform minerals. Asbestiform minerals are defined as the magical group of hydrated, silicate minerals that possess a fiberlike structure capable of being woven into cloth. Their unique combination of properties, such as resistance to heat and chemical attack, high tensile strength, and flexibility was recognized centuries before Christ. Pausanias, the early Greek geographer, speaks of golden lamps, made about 430 BC, with in- combustible wicks of Carpathian flax. The Ro- mans used asbestos as cremation clothes to con- serve the ashes of deceased persons of rank. Charlemagne had an asbestos table cloth that he used to pass through fire to clean (Lanza, 19381. Early recognition of the health hazards associated with exposure to asbestos was documented in the first century by Pliny the Elder, a Roman naturalist, and Strabo, a Greek geographer. Both wrote of slaves with sickness of the lungs, whose occupation was that of weaving asbestos into cloth. This concern for the health of asbestos workers disappeared over the ages. Today, with the use of modern technology, asbestos products have been greatly exploited. The asbestos output has increased over a hundredfold in the past 70 years. World production has jumped from 454 t/y (500 stpy) in 1900 to 4.5 Mt/y (5 million stpy) in 1978. An increase in health problems for asbestos workers accompanied this increased usage. However, it was not until the modern era that the association of asbestos with chronic respiratory disease was rediscovered. Reports were published at the turn of the century documenting the effects of asbestos exposure on textile workers. One such report in 1907 by H.M. Montague Murray described the post mortem findings on a 34-year- old carding machine operator. Murray de- scribed the lungs as showing extensive diffuse pulmonary fibrosis spicules of asbestos. Subsequently, more reports of pulmonary fibrosis following inhalation of asbestos were published, including an epidemiological study of asbestos miners in 1930 (Merewether and Price, 1930), at Thetford, Que., Canada. This study took place at the mine 52 years after large-scale mining of asbestos had begun. But, the cancer- producing potential of asbestos was not established until 1949 when a report by Merewether in England described an excess of cancer of the lung and pleura [the lining of the lungs) among individuals dying from asbestosis (Merewether, 1949). In the past decade much attention has been focused on asbestosis, mesothelioma, and bronchogenic carcinoma, as the three major biological effects associated with asbestos ex- posure. Parenchymal and pleural asbestosis is a fibrosis in the substance of the lung with thickening and often calcification of the membranes lining the thoracic cavity. Mesothelioma is a malignant tumor of the membranes lining the thoracic or abdominal cavities, and bronchogenic carcinoma is a cancer of the lung tissue itself. These diseases occur in various degrees and combinations. The severity and occurrence of these biological effects are directly influenced by conditions such as dose response relationships, intensity of exposure, duration of exposure, lapse of time since initial exposure, and cigarette smoking. Numerous studies indicate that the relationship between these conditions and the incidence of disease is proportional. Congress has recently concluded (Congressional Record, 1979) that, "exposure to asbestos fibers has been identified as significantly in- creasing the incidence of cancer and other severe or fatal diseases such as abestosis." Congress conveys its concerns for the safety and health of the nation's miners through the Federal Mine Safety and Health Act of 1977 in which it gave MSHA the authority to set safe

The Mine Safety and Health Administration?s (MSHA) Roof Control Division (RCD) collects a variety of geological and engineering information pertaining to mining conditions in surface and underground mines throughout the United States. Recently, lineament analyses, trends of geologic structures, and inferred stress directions based on mapping at mines with problematic ground conditions, have been integrated into a GIS. The incorporation of different kinds of information into the same geographic coordinates and map projection, placed on digital geologic maps, allows site specific data to be evaluated in a regional geospatial context, identifying trends in the orientation of horizontal stress or patterns of structural geologic features. As more information is collected, the GIS will evolve as a tool for district- and basin-wide evaluations of controls on ground instability, and may evolve into a predictive tool for some kinds of geologic hazards. In addition to the geologic and geotechnical evaluations, the GIS also serves as a searchable database of previous mine investigations. The database can be searched for a variety of criteria, and the results displayed in real geographic coordinates to find the locations of previously visited mines or to evaluate mine fatality and injury data. This not only allows old reports to be found, but also places the information in a geospatial context with respect to neighboring mines or geographic areas. The GIS also has the capability to serve as a digital mine map repository, where maps can be stored in digital, georeferenced format for display in relation to each other and various themes of interest. This allows evaluation of multiple seam mining interactions and proximity to other mines, and on a national level could serve as an abandoned mine map database.

The iron mines of the district have the best safety record for that industry in the country as a whole, while the copper mines of the district rank second in two of the four years recorded (Table 1). In Utah copper comes mainly from open-pit mines, which enables that state to lead easily in safety. All of the Michigan production comes from deep under-ground workings. About 60 per cent of the iron miners in St. Louis County, Minnesota, are employed at open-pit mines; this favorably affects the combined open-pit and underground accident-frequency rate, as this county pro-vides most of the Minnesota-mined iron ore. In Michigan, however, there are few open-pit mines, the bulk of the production coming from underground mines, while all of the Wisconsin production, just across the state line from Michigan, is from underground mines. In spite of the fact that fewer mines in Michigan use the safer open-pit method, the iron-mining industry of that state maintained a consistently low record from 1931 to 1934; however, it was many points behind its nearest com petitor, Minnesota, with its much greater production from open-pit mines.

Safety nets have been used selectively in several South African mines over the years. Recently, however, an increased emphasis on mine safety has seen a resurgence of their utilization underground. This renewed interest has, however, also raised questions about the design and performance of these nets and their practical applications underground; these are the subjects of this paper. Generally, mine safety nets are used to protect personnel from falling rock in temporarily supported development ends, tunnels, ASGs and stopes. These diverse applications each require specific net designs, resulting in a wide product range. As a rule, however, mine safety nets need to carry specified static or dynamic loads with the least possible deformation. Because of the application-specific characteristics of mine safety nets, there is no South African national standard for them. Consequently, neither the SABS, nor the CSIR, performs standard tests on these components. In 2007, therefore, SIMRAC made available their Savuka facility for the testing of mine safety nets; the Savuka drop test became the accepted industry test standard for mine safety nets. From late 2007 to mid 2009, Norsenet, in collaboration with SRK Consulting, the designated operators of the site, conducted approximately 200 tests, which formed the core of Norsenet's database of safety net performance under various loading conditions. The test specimens comprised a wide range of net materials and config¬urations; the materials included: high density polyethylene, polypropylene, polyester, nylon, as well as a blend of polyester and polypropylene used primarily for reinforcement ropes. Net configurations encompassed the following parameters: net type, i.e., knotted nets or sewn nets; net strand type, i.e., braids, twines, or webbing; strand size; mesh size; mesh orientation, i.e., parallel or oblique to the net edge; net edge reinforcement, i.e., knotted, tied, with or without reinforcement ropes; internal reinforcement ropes, i.e., diagonal ropes and/or orthogonal ropes; as well as net attachment options, including loops and chainlinks. From the number of variables listed above, and the limited number of tests available, it can be appreciated that the findings of the test series required careful scrutiny. Nevertheless, principal performance factors clearly emerged: besides the obvious net mesh variables, such as net strand material, strand diameter and mesh size, it was the configuration of the reinforcement ropes and their attachment to the net panel which had the strongest influence on net performance. All the structural elements of a safety net need to work together to transfer the imposed load from the net panel, through the reinforcement ropes, to the net attachment points. In this load transference chain, the weakest link limits the strength of the structure. From this development work, Norsenet has gained valuable expertise to custom-design and manufacture nets to clients' specifications. The examples presented in the paper will also show the experience acquired in the installation of nets underground. Future work will include the establishment of a state-of-the-art net testing facility at Norsenet's factory in Vredenburg.. Coal mining as an occupation, is hazardous under the most, ideal conditions. Confronted with this inescapable fact, every possible emphasis is placed by the Phelps Dodge Corporation, at the Dawson mines upon the employment of every means available for safeguarding its employes from the dangers attending their vocation. Accidents have a human aspect and a direct bearing upon the well-being of the workmen themselves. They cause a loss of wages, suffering and often privation of the employe and those dependent upon him. In case of death, his children are often forced into the wage earning class without preparation to fight the battles of life, with the result that the efficiency of the next generation is lowered. Dr. Jones, formerly of the war industries board, is responsible for the following statement. "Accident prevention as it is now conducted is principally a matter of education and not merely one of protection against physical hazards as frequently believed. In fact, the last ten years experience in accident prevention has demonstrated conclusively two things: 1. That three-fourths of all deaths and serious injuries in industry can be prevented. 2. In the industries doing the most efficient work, two-thirds have been accomplished thru organization and education as against one-third accomplished by means of mechanical safeguards or equipment. Experience in the past decade has conclusively proven that the work of accident prevention is not a "one man job" that satisfactory results can be secured only thru the highest measure of co-operation between the employer and employe, and this by means of organization. The employer must be vitally interested in the work if he expects to educate his men to share the responsibility with him. The men must be given a part to perform in it, if their interest is to be aroused and maintained. It is only thru organization that this is possible. In this connection it is interesting to note the results obtained by the Old Dominion Co. of Globe, Ariz., in reducing accidents after the organization of their safety department. The O. D. Co. are pioneers in the adoption of the workmens safety committees and and the results obtained by this, company after their safety work was well organized are truly wonderful. For the years 1913 to 1916, inclusive, at which time the safety work was not well organized, there were a total of 18 fatal accidents, the years 1917 to 1920, inclusive, was a period of intensive safety work, during which time there were only three fatal accidents, none having occured during 1919.

The Triton lode is a schistose feldspar biotite chlorite rock with quartz, carbonate, pyrite and some pyrrhotite. Its more chloritic sections strongly resemble the enclosing greenstones, its more feldspathic portions are indistinguishable from the flne-grained phases of the porphyry dyke into which the southern extremity of the ore body merges. The ore body' has a length of 880 ft. and an average width of I2t ft.; in places this widens to as much as 30 ft.The average dip is 85° west and pitch of the ore shoot 75° south.METHOD OF SAMPLING.With the exception of a limited amount of channel sampling (a reference to which is made elsewhere in this paper) all the sampling is by chip sample.The samplers use a hammer weighing approximately 11 lb. and chip directly into the sample bags. Sampling is generally carried out over 3-ft. sections and portions thereof to the nearest half foot-thus a face 8t ft. wide is divided up into two sections of 3 ft. each and one section of 2 ft.-no measurements of less than 4 ft. are used and calculations are all in feet and dwt. per long ton.In the oxidised stopes wp.ere the lode is much narrower, .i.e., 6 ft., sampling is over 2-ft. sections. A mixed-integer formulation of the mine scheduling problem is discussed and applied for the purpose of optimizing both the mine production sequencing and the mill blending and processing problems simultaneously. Previous attempts at solving these problems separately, although successful, fail to take into account the interrelationships between these problems. Project structure serves as the outline of this paper, which is directed towards a mining engineering audience. Generalized in its organization, this concept is useful for long, intermediate and short range scheduling. Actual applications are mentioned which show that the concept is also applicable to mining a variety of ores.

Mine seals are necessary in nearly every mine to isolate mined-out areas from the ventilation network. There is a large number of seals in active mines and that number continually increases as the development spreads across the reserves. The recent accident caused by the failure of seals in the Sago mine resulted in new regulations. These regulations have been set based on a principle of eliminating the risk of failure, rather than the sound engineering and understanding of the conditions that may cause seal failure during an explosion of methane in sealed areas of the mine including the probability of such occurrences. This paper discusses the following subjects related to mine seals: The potential causes of methane explosions and the dynamic effects of pressure wave propagation. Seal materials and methods of construction in view of the interaction of the seal with the host rock environment as well as requirements for gathering geotechnical data. The design of seal dimensions is based on loading conditions, therefore the probable ranges of such loading is reviewed. There is also discussion of possible methods for shielding of seals from pressure pulses following an explosion. Particular seal?s types are examined in reference to the conditions at the seals location. A novel design is presented that will be a low- cost solution for the sealing of mined out areas. Recommendations are given for further research leading to a rational engineering system for design of new seals in coal mines.
