"This work presents the potential for improving the flotation recovery of slow-floating sulphide minerals with the use of starvation dosages of a normal dodecyl (n-C12) trithiocarbonate (TTC) co-collector, together with a sodium isobutyl xanthate (SiBX) and dithiophosphate (DTP) collector mixture.At potentials below –150 mV (SHE), addition of nC12-TTC with SiBX improves the hydrophobicity of pyrrhotite, yielding captive bubble contact angles greater than those measured for SiBX or nC12-TTC alone, suggesting a low potential synergistic effect. This synergistic effect is further studied using Fourier transform infrared (FTIR) spectroscopy, the results indicating an increase in the surface concentration of the collector species when in a mixture. Thus, nC12-TTC with SiBX may act as an immobile surface anchor to which SiBX/SiBX2 molecules bond, increasing the localized concentration of collector species.Bench-scale flotation tests using mixtures of SiBX/DTP/nC12-TTC on a platinum group element (PGE)-bearing ore from the Bushveld Complex in South Africa confirm an improved metallurgical performance at very low substitutions (approx. 5 molar per cent) of SiBX. The improved recoveries for PGE, Cu, and Ni are correlated with improvements in the flotation kinetics of their slow-floating components. IntroductionShort-chain (less than C6) xanthates are very effective for the bulk flotation of sulphides (Fuerstenau, 1982), and early work (Leja, 1968; Gaudin, 1957; Plaksin and Bessonov, 1957; Taggart, Giudice, and Ziehl, 1934) indicated that the presence of oxygen in flotation slurries is necessary for the oxidation of the xanthate collector at the sulphide mineral surface to induce hydrophobicity. Fundamental research into the chemistry and adsorption mechanisms of xanthates over the years (Finkelstein and Poling, 1977; Woods, 1976; Winter and Woods, 1973) has revealed that for iron-bearing sulphide minerals, adsorption occurs predominantly through a charge transfer process. Oxidation of the adsorbed collector to the corresponding dimer takes place if the mixed potential of the system is greater than the reversible potential for dimer formation. In oxygenated slurries the rate of adsorption and oxidation of the collector depends greatly on the substrate surfaces. For chalcopyrite (Guler et al., 2005; Leppinen, 1990; Roos, Celis, and Sudrassono, 1990) and pentlandite (Hodgson and Agar, 1989) a two-step interaction with xanthate is proposed in which the chemisorbed xanthate is further oxidized to the dimer. The initial interaction of xanthate with pyrrhotite is through physisorption where xanthate physisorbs onto positive surface sites followed by oxidation to dixanthogen (Khan and Kellebek, 2004; Bozkurt, Xu, and Finch., 1998). This process is, however, slow and increased reaction time is needed to improve flotation recovery (Buswell and Nicol, 2002)."

The West Wits Line first began yielding gold in the mid-1930s and is among South Africa's leading gold-producing areas. Gold mining operations are carried out amidst the occurrence of seismic events which cause both damage to underground excavations and injury and loss of life to production personnel. The feasibility of establishing a system to minotor the seismicity on all the Gold Fields mines was investigated during the early 1980s. This led to the establishment of a centralized seismic network in 1981. The system was structured around three major objectives. It uses Perkin Elmer (now Concurrent Computer Corporation) 16 and 32 bit mini-computers. The hardware in use at both the central and remote sites is outlined, giving a good insight into the role of computers in the system. The system software reveals the fast flow of data through the network; it can be divided into three categories: (1) data logging and transmission - remote sites, (2) data capture and communications - central site, (3) applications and research - central site. Several examples are given which show how the system is measuring up to its objectives. These case studies also indicate how the computers are effective in representing the seismic data collected. It is concluded that the use of computers enables the pin-pointing of seismic events on the West Wits Line within about 3 -4 minutes of their occurrence. The choice of hardware is largely dependent on the objectives of system, and the Gold Fields Network has chosen mini-computers because of the speed of communication and the large amount of data and storage required for research applications.

"Understanding the influence of feed material mineral assemblage on spiral separation is paramount for the successful and consistent operation of a spiral plant. Plant stability is required to maintain high recovery of valuable heavy minerals (VHM). Standard material analysis methods used in the heavy mineral industry to evaluate spiral separation behaviour are unable to quantify performance differences in the case of large variations in feed assemblage. This study illustrates a systematic approach to evaluate spiral separation performance. This technique was applied on a four-stage spiral plant. The performance information was used in a circuit simulation to clearly illustrate the importance of correct setting of the spiral splitter position and resulting spiral mass loading to ensure optimal circuit stability and maximum VHM recovery.IntroductionHeavy mineral sands deposits generally consist of naturally fine-grained, free-flowing sand due to the way in which the deposit was formed. The size of the heavy minerals market is roughly 8 Mt/a of zircon and titanium combined (Iluka, 2013). If an average combined feed grade of 5% zircon and titanium and an average combined overall product recovery of 80% are assumed, the resulting run-of-mine feed tons that would require primary spiral beneficiation is 200 Mt/a. The number of spirals required for the processing of this material, assuming 6500 operating hours per annum, an average of 2 t/h per start, and the same number of rougher spirals as other duty spirals, is over 30 000 spirals for the heavy mineral sands business alone. These numbers clearly illustrate the importance of the spiral in the heavy mineral industry.The aim of a spiral plant in the primary processing value chain of a heavy mineral process is to reject non-valuable quartz (a light mineral) from various valuable heavy minerals (VHM). The product from the spiral plant is a heavy mineral concentrate that has a reasonable grade (more than 90% heavy minerals) at a reasonable heavy mineral recovery (greater than 90%). This benchmark is fairly easy to achieve if all the mineral particles are well liberated (as in the case free-flowing sand), the feed is properly deslimed (-45 µm fraction removed to less than 5% of the solids mass), and the spirals are fed within design solids concentration (water to solids ratio) and design spiral load (dry tons per hour per start)."

Gold Fields is the world?s fourth largest gold producer with operations in South Africa, Australia, Ghana and South America. All of Gold Fields? South African Operations are located in the Witwatersrand Basin and are intermediate to deep level underground mines exploiting gold bearing, shallow dipping tabular orebodies, which have collectively produced over 50 kt (1,608 Moz) of gold over a period of more than 100 years. Although the use of Monte Carlo as a business tool was introduced in 1964, it has only been in the last 30 years that it has gained increasing acceptance within the mining industry. However, the application thereof has been mostly limited to determination of risk in project areas. This paper however, considers the possibility of using Monte Carlo simulations to asses the underlying ?Root Cause? criteria critical to the operations management of existing mining operations. Currently, working operations have a distinct advantage over project areas as they have a wealth of historical information at hand. This, in turn, can be utilized to model distributions and relationships that can be used in Monte Carlo simulations, the results of which can be analysed to determine areas of concern. Projects can then be set up to ameliorate the effects of these and in turn reduce the risk of not achieving the operational targets. It is here that it must be stated emphatically that it should be the ?root causes? and the associated relationships that need to be emplaced within the model in order to achieve results that reflect the true causal relationship. The paper outlines possible sources of ?root cause? relationships and considers the outcomes of these in a case study on a large South African Gold Mine.

Mineral processing efficiency depends on various factors and among them includes head grade fluctuations feeding the processing plant. Head grades can fluctuate beyond an admissible range, implying in recovery decrease for the mineral of interest. Grades feeding the processing plant depend on the geological domain which is being mined and on the mining schedule. Therefore, it is necessary to keep grade variability controlled. Blending piles are an efficient alternative to reduce grade fluctuations. The main factors influencing blending efficiency are the pile size or its mass and the number of layers used to pile up. Large piles are more efficient to reduce variability, however, there are economical and operational limits associated with them. This study investigates a methodology to analyse the ideal pile size to reach an acceptable variability given the in situ grade fluctuations. Geostatistical simulations provide the means to assess grade variability using an optimized mining schedule. These block grades are used to simulate any given pile throughout the mine`s life. The methodology was illustrated using a case study to control coal ash content at a mine supplying ROM coal to a power plant. Keywords: homogenization, piles, geostatistical simulation.

Middleburg Ferrochrome (MFC), a company in Samancor Chrome, commissioned a new ferrochrome smelter in March 2009. This is a 60 MW DC-arc furnace with a DC power supply for the smelting of Transvaal chromite ore fines. With the benefit of 12 years' experience with this technology while operating a 44 MW DC-arc furnace, MFC mitigated all known challenges and considered up-scaling risk in the new plant, which had a rapid ramp-up to full power in 02 of 2009. Good operating practices, as made possible by robust equipment selection, including a thyristor rectifier set of proven technology, and operation with optimal slag chemistry, leads to a safe and effective plant operation. With the optimized furnace design and a correctly sized power conditioning system, using only standard (static) harmonic filters and a correctly sized smoothing reactor, the arc furnace is not a 'dirty load' for the utility, staying well within IEC limits for power quality. A patented arc compensation system is installed and allows for full arc position control (to compensate for magnetic field induced arc deflection) so that a symmetrical crucible is maintained, significantly benefiting refractory life. The principal benefits gained from the MFC process is the fact that ore fines are processed, without metallurgical coke, and higher chrome recoveries are achieved compared to other smelting technologies when using a DC open arc for smelting. "The working environment for ground support installation in mines has improved during the last 20–30 years, with more mechanized equipment for installation of ground support elements such as bolts, cable bolts, and screens. Ground support installation productivity has, however, not followed the same development curve, remaining more or less constant. In some cases, for example the mechanized installation of bolts, productivity has even dropped. One reason for this is that modern mechanized bolt rigs are complex. In this paper we evaluate manual and mechanized ground support systems, propose a way to measure the productivity of bolt rigs, and make relevant comparisons between different mines and equipment. Some productivity measures for rock reinforcement are suggested, using productivity results from eight case study mines.IntroductionTo improve safety for operators and equipment underground, ground support systems (bolts, screens, etc.) are installed in mines to stabilize the rock mass (Hoek, Kaiser, and Bawden, 1995). Important questions to ask when selecting a ground support system include where ground stability problems are located, why they have occurred, how problems should be addressed, and when and how the ground support system should be installed (Hadjigeorgiou and Potvin, 2011). The performance of bolting and screening operations in underground mines is important for both safety and productivity reasons. With greater mine depths and an increasing focus on safety, ground support has become a significant bottleneck in the production process of many mines.Many factors play a role in establishing safe and stable underground structures. Equally, there are several factors to consider in productivity, including the mining method, organization, logistics, mechanization level, and procedures. Today, most mines in Scandinavia have changed from manual to mechanized bolting, as this improves operator safety and arguably boosts net capacity. Internationally, however, manual installation of bolts remains common, with some claiming that manual installation is more productive (Bawden, 2011)."

The development of a cyanidation route for the treatment of gold-plant gravity concentrates is described. This work was carried out as a natural consequence of earlier improvements to the recovery of gold in the gravity sections of existing gold plants. Concern about the environmental aspects of the conventional amalgamation process was further motivation in the development of an alternative procedure. It was found that the partial pressure of oxygen in solution was the most critical reactionrate determining factor. The rate of gold dissolution was shown to be controlled by the reaction rate below approximately 25°C and by diffusion above that temperature. Thus, the efficiency of pulp agitation and oxygen partial pressure permitted a high degree of control over the rate of gold dissolution. The use of oxygen rather than air significantly increased the dissolution rate of both gold and silver. The solubilities of sodium, calcium, and potassium aurocyanide complexes under simulated reaction conditions were studied, and it was established that the values for the sodium and calcium complexes were limited, requiring the use of lower pulp densities and higher reaction temperatures for the treatment of very high-grade concentrates. The presence of excessive amounts of tramp iron in such concentrates of up to 25 per cent by mass was found to result in poor gold dissolution owing to the cementation of gold under certain reaction conditions. The maintenance of high oxygen partial pressures, together with high cyanide concentrations, high pH levels, and the presence of calcium in the system, all aided in passivating the tramp iron. Copper, gold, and iron were shown to be the main cyanide-consuming elements. Optimum gold dissolution was obtained in a closed bench-scale reactor incorporating a flotation-machine type of agitator for effective pulp agitation and oxygenation, the oxygen being recirculated. The reactor was designed to operate under a very small positive pressure. After reaction times of 2 to 6 hours at temperatures in the region of 30°C, both the gold and the silver dissolution averaged 99,S per cent. In the case of very high-grade concentrates, it was shown that gold could be floated in the same reactor by drawing off the flotation concentrate through a port in the reactor. A flotation recovery of 87,4 per cent of the gold in 3,9 per cent by mass was obtained. The recovery of osmiridium from cyanide-leach residues and the electrowinning of gold from the pregnant liquors were also considered as processing aspects in the treatment of gold-plant belt concentrates. Final table tailings with a gold concentration of 10to 12 gft were obtained. The results of the investigation strongly suggested that an alternative cyanidation route for the treatment of gold-plant concentrates was technically feasible, provided the system included effective agitation and oxygenation. In the case of lower-grade concentrates, cyanidation followed by the electrowinning of gold appears to be the simplest route, while, for high-grade concentrates, flotation and smelting of the concentrate followed by cyanidation of the flotation tailings are suggested. The use of oxygen rather than air for sparging the pulp would depend largely on the grade of material to be treated. A preliminary comparison of the costs related to the amalgamation and cyanidation processes for the recovery of gold from such concentrates indicated very similar capital and operating costs. It was concluded that other considerations, e.g. toxicity, safety, and lower labour requirements, were more important than such small cost differences. Furthermore, the 'opening' of the gold-plant gravity circuit, which would be made possible by a cyanidation route, would further reduce the operating costs because of the subsequent decrease in the tonnage to be treated.

The mining industry in South Africa faces real challenges with regard to the reduction of safety-related incidents. The purpose of this paper is to highlight the need for alternative interventions in addition to other safety improvement strategies already employed on mines. It furthermore supports an existing paradigm: incident investigations. An investigation into fatalities in the South African mining industry shows that although great progress has been achieved through the reduction in fatalities in all sectors of the industry over the last few years, more can and should be done to achieve status of ?zero harm?. Fatalities and lost time injuries on mines result not only in large financial losses, but also in disruptions of mining operations in general through Section 54 stoppages to resolve the situation. The approach of the paper is therefore not to try and prove whether there is high fatality rate and/or lost time injuries, but in what way incident reconstruction simulations (IRS) can have an impact in the future prevention of such incidents. The challenge therefore lies in the utilization of pro-active systems that would show employees the possible outcomes of being exposed to certain hazards. In real life, these incidents cannot be re-enacted and in most cases we can only learn through the previous mistakes of others. A virtual reality (VR) education and training system will enable employees to be ?exposed? to different hazards in a safe environment, and simulation, in a ?forgiving? environment, of the possible consequences of unsafe acts. According to the Occupational Safety and Health Act (OSHA), unsafe acts and unsafe conditions are indirect causes of safety-related incidents, but there are also direct causes; for example, unplanned release of energy and/or hazardous material, and basic causes, for example, management safety policies, personal factors, and environmental factors. The administrative workload associated with any incident investigation places a large burden on the investigation team, and in most cases other employees are informed through vague administrative processes what the ?real cause? for the incident/accident was. New preventative strategies are often put in place without showing the employees what the real causes of the incident/accident were. This can be mitigated through the use of interactive VR simulations/animations of the hazards that they are exposed to and how to deal with them. If VR interventions are not used as proposed by this paper, the real causes and consequences of almost all fatalities and lost time injuries may never be fully understood. This paper also emphasizes what potential impact IRS could have on the prevention of future mine incidents as part and parcel of a pro-active risk prevention strategy. The intention is therefore not to replace any other risk prevention strategies, but to visually enhance the reality of the incident. This work evaluates the deformation behaviour, at warm working temperatures, of green particle-reinforced aluminium composites produced by powder blending in a high-energy ball mill. The work focuses on metal matrix composites (MMCs) based on the 2124-Al alloy, reinforced with 10 or 15 vol.% SiC and metal matrix nanocomposites (MMNCs) based on the 2124-Al alloy, reinforced with 5 or 10 vol.% Al2O3. Three batches for each powder were blended and powder properties such as particle size distribution (PSD) and shape were consistent after blending. It was observed that a more uniform distribution of the reinforcement phase in the aluminium alloy matrix was achieved in 2124-Al/Al2O3 than in 2124- Al/SiC composites. The powders (unreinforced 2124-Al and blended) were initially over-aged at 350°C for 2 hours to reverse any natural ageing that may have occurred prior to use. The over-ageing was incorporated to improve compressibility of the powders with the aim of achieving green compacts with higher integrity. Uniaxial compression tests performed at ambient temperature on a Gleeble® 3500 thermomechanical simulator were unsuccessful as the green compacts fragmented. Engineering stress-strain curves showed that green compacts of unreinforced 2124-Al, 10%SiC MMC and 5%Al2O3 MMNC deformed in a similar manner at ambient temperature and had the same compressive fracture stress of approximately 170 MPa. When the deformation temperature was increased from ambient to warm working temperatures (170–280°C) it was observed that electrical resistance heating (the heating mode of the Gleeble®) of unreinforced Al alloy, MMC and MMNC green compacts did not occur. This was attributed to the high electrical conductivity of aluminium, which resulted in poor heat generation due to the low electrical resistance in the samples. It was presumed that the small sample size (d=8 mm, h=12 mm) also caused rapid heat loss. After further experimentation, the green compacts were heated successfully by insulating the samples to retain heat. It was found that at 280°C, increasing the soaking time from 6 to 20 minutes decreased flow stress and improved plastic flow in the 2124-Al/10%SiC green compact.

The Barrick North Mara Mine and the Advanced Systems Group at SGS Minerals collaborated on the implementation of a grinding expert system for the control and optimisation of a single semi autogenous (SAG) mill. Over the last 5 years, grinding expert control has gained wide acceptance by mineral processors. In this paper, two key factors for success are highlighted: the adaptability of the software tools and the interaction with site personnel. Created with SGS?s MET expert console (MEC) toolkit, and running on Gensym?s G2 real time inference software, the North Mara expert logic has been set up to keep a steady and on spec leaching feed slurry while striving to maximize throughput. Leach feed stability was achieved by standardizing operating practices across shifts; a result of successful collaboration with site personnel in developing a consistent control strategy. The strategy maintains the SAG load and power draw in an optimised range by manipulating SAG feed rates and inlet water flow. The project life-cycle was slightly longer than average due to site specific constraints. One of these was the high turnover rate of site?s workforce during the project, coupled with the availability of key staff on site due to the fly in fly out work roster. These constraints called for different, and innovative ways of project commissioning and hand-over. This paper describes details of the control solution, its implementation as well as results and conclusions.

"The behaviour of rocks depends on their material properties, and therefore it is important that these properties are determined by relevant and reliable laboratory testing methods. The ISRM Suggested Methods are very important documents in this regard, and a draft suggested method exists for the determination of the ’complete stress-strain curve for intact rock in uniaxial compression’. The method specifies the required geometry of specimens and the required testing procedures. The research described in this paper involved the determination of complete stress-strain curves for two materials, one exhibiting the Class I characteristic, and the other the Class II characteristic. To obtain a better understanding of the deformation behaviours in the post-peak region, tests were carried out on specimens of both material types with different length-to-diameter ratios. The results showed that the geometries of the test specimens influence the stressstrain curves in the post-peak region. The implication is, therefore, that the numerical values obtained for the slopes of the post-peak graphs are not material constants, but are geometry-dependent. Explanations for this behaviour are given. These findings have important implications regarding the use of post-peak data, for example for blast fragmentation considerations, and for evaluation of rockbursting potential. The research findings are based on a limited amount of laboratory testing, and should be confirmed by further testing. IntroductionThe most common approach for studying the behaviour of rock is by compression testing of cylindrical specimens in the laboratory. Rocks respond to forces by deforming and eventually breaking. The strength and the deformation characteristics are derived from the stressstrain curves produced. Standard shapes and sizes for the test specimens and standard loading rates are recommended in the Suggested Methods of the International Society for Rock Mechanics (ISRM) and the American Society for Testing and Materials (ASTM) standards. In spite of these standards, there remain certain aspects of testing which require further attention. The research described in this paper deals with deformation and failure behaviour of rock materials using stiff, servocontrolled testing equipment. The particular objective was to test the premise that the behaviour of cylindrical rock specimens in the post-peak region of the stress-strain curves does not reflect only a material characteristic, but is influenced by the height-to-diameter ratio (the geometry) of the rock specimen. An additional objective was to investigate the effect of the stiffness of the test equipment on the outcomes of tests."

The Paradela dam has a spillway that makes it possible to discharge a maximum flow of 400 m3/s, the water discharges being on the river Sela, about 2km upstream the river Cávado. After the first use of the spillway, some landslides have occurred on the right bank of the river Sela as a result of the erosion generated on its base about one hundred meters near some houses of a small village. This paper presents a study performed for the slope stability where finite element results are taken into account. A monitoring program was established that essentially consisted of measuring sub-horizontal displacements in points located in boreholes at different depths and also phreatic levels. Some monitoring results are presented. Le barrage de Paradela a une décharge d’eau avec un débit maximal de 400 m3/s sur le fleuve Sela dans un endroit situé environ 2km sur l'amont du fleuve Cávado. Après la première utilisation de cette décharge, quelques glissements se sont vérifiés sur le versant droit du fleuve Sela résultant de l’érosion générée sur sa base dans une zone à 100 mètres de quelques maisons d’un petit village. Ce travail présente une étude réalisée concernant la stabilité du versant avec la considération des résultats des éléments finis. Un programme d’auscultation a été mis en place pour mesurer les déplacements sub-horizontales sur des points situés en forages à des différentes profondeurs. Quelques résultats de cette monitorization sont présentés. Der Ueberlauf des Padela Dammes ermoeglicht das Ablassen einer max. Wassermenge on 400 m3/s in den Sela Fluss, ca. 2 km flussaufwaerts des Cávado Flusses. Nach Inbetriebnahme des Ueberlaufes traten in ca. hundert Meter Entfernung zur Wohnbebauung eine Dorfes Erdrutschungen am rechten Ufer des Sela Flusses auf, die auf Erosionen am Flussbett zurückzuführen waren. In der vorliegenden Studie wurde die Boeschungsstabilitaet unter Beruecksichtigung von Berechnungsergebnissen der Finiten Element Methode untersucht. Es wurde ein essprogramm ausgearbeiten, das im Wesentlichen auf Messungen der Untergrundverformungen in verschiedenen Bohrlochteufen sowie auf Grundwassermessungen beruht. Im Folgenden sind einige Messergebnisse dargestellt.

"Portions of the Icy Cape placer Au district have been described and exploited since the early 1900s. Industrial studies of Au-bearing garnetrich intervals established a 3.5 million troy ounce resource in the western half of the district. This estimate is not compliant with CIM and SME definition standards for Mineral Resources and Mineral Reserves. We note high variability in the textures of sediments and their heavy mineral (HM) content and composition in the district, probably as a function of the depositional environment. Sediments are medium- to fine-grained sands with a valuable HM assemblage that averages 26.0% in a range of 1.1– 72.6%. Valuable heavy minerals (VHMs) garnet, magnetite, ilmenite, rutile, and zircon, but excluding Au, average 7.9% in a range of 1.0–37.6% of the HM assemblage. Trace quantities of uraninite, thorite, monazite, xenotime, and others are noted. Garnets have an average Y content of 726 ppm with a range of 0–2150 ppm. Energy-dispersive X-ray fluorescence confirms compositions commensurate with Ca, Al, and Mg silicates. We provide empirical analytical data and note high variability in the textures, HM content, and compositions as a probable function of depositional environment. The objective of this study was to develop, install, and test a small underground mobile refrigeration plant (MRP) to deal with some of the real problems associated with mine cooling in an operating mine. The requirement for cooling the underground environment is discussed with particular emphasis on the need for this method of cooling, with the concomitant benefits. The research investigated current methods of cooling and reasons for previous failures in MRP. Both static and dynamic simulations were conducted to increase the confidence level under operating conditions. Implementation and testing, resulted in ?lessons learnt? requiring modifications, which are documented. Actual results have been recorded. These results have proved that significant cooling via MRP is feasible. Main benefits include positional efficiency, cost per kilowatt of cooling and cooling opportunities for remote hot areas of a mine. Finally, a proven technology is now available for large-scale implementation in the mining industry. Now the ventilation engineer has another system of cooling, which can be utilized in the quest to create an occupational environment, which meets the physical and mental health requirements of the worker.

The paper covers many literature reports of processes using nitrogenous species as an oxidant for processing refractory gold ores, especially development in the pretreatment of refractory gold minerals by nitric acid. These processes include the high-pressure and the atmospheric-pressure oxidation methods. High-pressure oxidation processes are difficult to carry out, because they require high-capital cost autoclaves. Atmospheric-pressure oxidation processes are easier to manipulate, but complete absorption and reuse of NOx is difficult or multiple-stage absorption must be established for its reuse. In May of 1980, a mine operated by Belmoral Mines Ltd experienced a failed crown pillar, resulting in the death of eight miners and putting sixteen others in harm?s way. The Ferderber mine was located in the Val d?Or region of the province of Quebec in Canada, and had been under development for 19 months and production for 9 months, since the opening of the portal. The crown pillar separating the 1?7 level from overlying overburden ultimately failed, permitting an estimated 1.5 million cubic feet of liquefied sediment to gain entry into the mine. This paper back-analyses the cave-in by applying decision error theory, in an examination of the organizational culture and the collective decisions contributing to the cave-in disaster. A case will be made for a greater understanding of mining methodology, geomechanics and risk assessment when operating in such challenging geologic conditions. Boundary condition and decision (BCD) analysis will be introduced as an innovative new method for the analysis of mine accidents and incidents. It will also be shown that BCD analysis can be applied to any event in an enterprise for which standards, norms or legislation exists. A cognitive profile will be presented that provides insight into the safety ethos existing within the Ferderber mine organization at the time of the tragedy.

"Portions of the Icy Cape placer Au district have been described and exploited since the early 1900s. Industrial studies of Au-bearing garnetrich intervals established a 3.5 million troy ounce resource in the western half of the district. This estimate is not compliant with CIM and SME definition standards for Mineral Resources and Mineral Reserves. We note high variability in the textures of sediments and their heavy mineral (HM) content and composition in the district, probably as a function of the depositional environment. Sediments are medium- to fine-grained sands with a valuable HM assemblage that averages 26.0% in a range of 1.1– 72.6%. Valuable heavy minerals (VHMs) garnet, magnetite, ilmenite, rutile, and zircon, but excluding Au, average 7.9% in a range of 1.0–37.6% of the HM assemblage. Trace quantities of uraninite, thorite, monazite, xenotime, and others are noted. Garnets have an average Y content of 726 ppm with a range of 0–2150 ppm. Energy-dispersive X-ray fluorescence confirms compositions commensurate with Ca, Al, and Mg silicates. We provide empirical analytical data and note high variability in the textures, HM content, and compositions as a probable function of depositional environment. Previously, the grade of ore at the shaft head was taken to be equivalent to the grade measured in the faces by means of chip sampling. The tonnage mined from the stopes is determined from survey volumetric measurements. Estimates of shaft head grade and tons include the grades and tons of waste, development ore, sweepings from old areas, and other sources, not all of which are sampled. The tonnage of ore delivered at the shaft is often estimated using skip factors. These factors are subject to manipulation and not all the skips are filled to the same level. Consequently, both the grade and tonnage of ore delivered at shaft head are subject to a measure of uncertainty. For many years, these uncertainties have led to disputes between shaft managers and metallurgists over the reconciliation between shaft tons and grade and plant tons and grade. These problems are compounded when several shafts feed a central processing plant. Shaft bonuses and a shaft?s profitability are affected by poor gold allocation methods. Without measurement of the grade and tonnage from all shafts, a poorly performing shaft could benefit at the expense of the other shafts. In order to improve the measurement of grades and tonnage at the shaft head, which is the custody transfer point between the mine and surface operations, South African mining companies have developed a methodology to measure the mass of ore from each shaft using rail weighing equipment, and the run-of-mine grades delivered from each shaft using cross-stream (go-belt) sampling. Harmony Gold Mining Company has implemented the rail weighing and go-belt sampling methods for their shafts. This paper deals with the statistical tests on the procedures and control measures that are in place to establish confidence in the gold allocations. The metal accounting system for examining the allocations follows the first principle of the AMIRA P754 Metal Accounting Code of Practice namely ?The metal accounting system must be based on accurate measurements of mass and metal content. It must be based on a full check-in check-out system?. The check-in is mass and grade of run-of-mine ore and the check-outs are the plant input grades and the recovery and residue from the plant. The balances between check-in and check-out are the subject of the statistical analyses, which have been conducted using the classical statistical tests and procedures that are generally used in all fields of endeavour. It is concluded that the gold allocation procedures that rely on go-belt sampling and rail weighing of ore from shafts has been successfully implemented, and that the system, which has been in operation at Harmony since 1993, has won general acceptance from stakeholders, with the gold allocations being perceived as being fair to all. Apart from the on-mine issues of bonuses and fair allocations, accurate metal accounting assists in the broader issues of generating accurate production figures for management. An added benefit of weighing and sampling ore at the shaft head has been that shafts now have daily moving average grades, which are used for grade control. The go-belt sample at the shaft head is the first accurate sample of production in the value chain from face sampling to gold bullion. Gy's formula for the variance of the fundamental sampling error (FSE) is based on a model that assumes independent selections of particles. A generalization of this model, which can theoretically lead to more realistic representation of actual sampling processes, was, for the first time, experimentally verified by repeated sampling from a well-mixed batch consisting of particles of zirconium silicate of two different sizes. The predictions for the sampling variance made in Gy?s theory and in the more general model were compared with the experimentally observed sampling variance. During the experiment performed here, the variance prediction made in the theory of Gy ranges from 0.96 × 10?7(g2/g2) to 0.95 × 10?7(g2/g2), which is approximately a factor 60 lower than the experimentally observed variance (5.76 × 10?6(g2/g2)). The difference between the Gy prediction and the experimentally observed variance cannot be fully explained by the uncertainty in the experimentally obtained variance due to the finite number of degrees of freedom. Therefore, the difference is likely to be a significant effect. This shows that, under the experimental conditions, Gy?s formula for the variance of the FSE probably undervalues the sampling variance, unless an empirical correction factor for grouping and segregation is applied in the theory of Gy. The new theory resulted in a variance prediction of 6.00 × 10?6(g2/g2), which is much closer to the observed variance than the Gy prediction. This indicates that the generalization of Gy?s theory offers an outlook for providing a better variance prediction than the Gy predictor in the case of dependencies between particle selections.
