In the last few years major industrial companies have begun to realize the importance of Reliability as an enterprise strategy that, properly implemented, will improve financial performance. Typical results of an effective Enterprise Reliability Strategy include a 20% -50% reduction in maintenance cost accompanied by a 5% - 10%increase in real production capacity, with no capital investment in production equipment. One factor fueling the race to acquire and implement enterprise reliability is the pressure for companies to be increasingly cost-effective and competitive in global markets. In a recent study, Asset Productivity: The Next Wave, the Boston Consulting Group (BCG)stated that, “In order to compete for funds, companies must offer investor returns that are competitive with other opportunities. Alternative investments such as dot-com and others with non-traditional business models have raised the bar.” (APDF version of the study can be downloaded at www.bcg.com). BCG views asset productivity as the most powerful mechanism for increasing shareholder return. The author believes that the most effective way to increase Fixed Asset Productivity is to maximize asset utilization by increasing the Reliability of the Assets and the associated processes. Developing and implementing an ERS requires significant effort and knowledge. Most companies have difficulty finding a sufficient supply of these valuable commodities to implement an ERS Program internally. Many companies also struggle with figuring out how to get started and what direction to proceed.

The Mini-DiscTM cutter drill bit is a radical design departure from the traditional tri-cone rockdrill bit. Miniature disc type cutting tools as small as 3.25 inches (82.5 mm) diameter are fitted to a bit body to cut individual concentric kerfs. The action is the same as that commonly used on large tunnel boring machines but miniaturized. The high performance rate and efficiency of a disc cutter are the result of producing a large proportion rock chips rather than sand or powder. The Mini-DiscTM drill bit, referred to as the “Disc Bit” has other unique features as well. It can be re-greased between runs. It can be rebuilt in the field in about 15 minutes, cutters can be replaced and the Disc-Bit appears to require less down hole hydraulic power than conventional bits. Thoroughly tested in the laboratory, the bit has recently been undergoing field trials in well drilling, blast hole drilling and shaft drilling. As any engineer knows, the laboratory can not shake out all the problems but it eliminates the more glaring weaknesses. Performance results have been exhilarating, and a few mechanical problems were discovered and largely overcome. This paper is a progress report.

"This paper evaluates the history, construction and most importantly the performance of two remedial seepage cutoff walls installed in two major dams in the United States. Beaver Dam, on the White River in northwest Arkansas, was completed in 1966 and seepage through the karstic limestone under the embankment was apparent almost immediately upon filling of the reservoir. Several attempts to stop the seepage by grouting were unsuccessful. In 1994 a cutoff wall consisting of secant piles drilled with 32"" diameter down-the-hole hammers to a maximum depth of 185'?was completed and the seepage was reduced to a negligible amount. It has been over 20 years since the completion of the cutoff wall, and all indications are that the cutoff wall is performing as expected. Walter F. George Lock and Dam on the Chattahoochee River near Fort Gaines, Georgia, was completed in 1963 and suffered the same problem as Beaver Dam due to the limestone on which the dam was founded. Cutoff walls were installed in both embankments (1980s) and solved the problem in these areas, but under-seepage at the concrete lock, dam and spillway structures increased to the point that remediation was essential. The cutoff wall had to be installed through 110'?of water and another 100'?into rock upstream of an active dam that continued to generate electricity during the construction of the cutoff wall. A secant pile wall, in combination with a diaphragm wall, was completed in front of these structures in 2004. It has been over 10 years since the completion this cutoff wall and, again, the cutoff wall is performing as expected. The paper describes the construction of these cutoffs, but focuses on the long-term performance in terms of controlling seepage and piezometric levels.BEAVER DAMBeaver Dam is located on the White River, 609 miles above its mouth, in Carroll County, Northwest Arkansas. The construction of the dam and Beaver Reservoir was authorized on September 1954 by the Congressional Flood Control Act for purposes of water supply, flood control and hydroelectric power generation. The dam site was selected in 1957: construction began in November 1960 and was completed in June 1966. (FIG. 1)"

"One of the underlying reasons for the establishment of the United States Bureau of Mines was the desire to promote greater safety in the mining and allied industries. Safety continues to be an important feature in the work of the bureau, and during the fiscal year 1929-30 the Bureau of Mines allotted $387,510 to the safety division out of a total appropriation of $2,249,670 (other than for helium).The safety division personnel consists of 22 engineers, 2 mine safety commissioners, 3 senior foreman miners, 18 foreman miners, 16 first-aid miners, 16 clerks, 10 cooks, and 3 laborers, a total of 90 persons. These safety workers are distributed through the 10 districts into which the United States has been divided for administration purposes. During the year 3 engineers, 1 senior foreman miner, 4 foreman miners, and 4 first-aid miners left the service. In general these men left the service to take up more lucrative positions.The 10 all-steel mine-rescue cars have headquarters as follows: Car 1, Reno, Nev.; Car 2, Raton, N. Mex.; Car 3, Pittsburgh, Pa.; Car 4, Terre Haute, Ind.; Car 5, Pineville, Ky.; Car 6, Pittsburg, Kans.; Car 7, Huntington, W. Va.; Car 8, Duluth, Minn.; Car 9, Butte, Mont.; Car 10, Des Moines, Iowa. There is also a mine-rescue car with headquarters at Juneau, Alaska. In addition, there have been established 11 mine rescue stations as follows: Birmingham, Ala.; Berkeley, Calif.; Seattle, Wash.; Evansville, Ind.; Jellico, Tenn.; McAlester, Okla.; Norton, Va.; Vincennes, Ind.; Wilkes Barre, Pa.; Salt, Lake City, Utah; and Pittsburgh, Pa.Every mine-rescue car and station is fully supplied with equipment and devices for use in case of mine fire or explosion, and at each station is an up-to-date automobile truck. At time of amine disaster the bureau's forces are immediately placed at the disposal of the authorities, State or company, with a view to giving such assistance as may be desired or available.The activities of the safety division at ordinary times are directed largely to securing and disseminating data on safety in mining. Among the numerous studies made, or being made, concerning safety and health in mining are those in connection with the causes and methods of prevention of mine explosions, explosibility of dust and gases, explosives and blasting, haulage, electricity, falls of persons or material, ventilation, dusts, high temperatures, timbering, lighting, machinery, and in fact practically anything which results or may result in acci¬dents of any kind in or around mines."

"Mine waste storages including tailings dams and waste rock dumps represent arguably the biggest risk on a mine site from both physical and geochemical viewpoints, related firstly to structural stability and the consequences of failure and secondly from potential on-going seepage or run-off of acid and metalliferous drainage (AMD). The risks are most likely to increase during operations and continue through the closure and post-closure phases, with the waste storages likely to be the most difficult features to resolve in the mine closure process. Contaminated water quality from tailings dams and waste rock dumps can require treatment "in perpetuity" at a cost of hundreds of millions of dollars. The problem is increasing with ever larger mine operations and waste generation volumes. This paper identifies leading practice methodologies in planning and operating mines and process plants to minimise the risks from tailings storages and how this might also affect waste rock management and even the design of mine excavations. These methodologies require a "big-picture" approach to develop an understanding of the overall issues and identify synergies between the different operational areas on a mine site that can reduce overall risk. With a conservative approach and realistic allowance for future costs it is possible to facilitate decisions at the mine planning stage that can reduce risk and save cost over the long term. This is in contrast to the common approach where each operational area is looking to minimise its own costs with a short-term view. Incorrect mine methodologies can be developed by allowing net present value (NPV) accounting methods to trivialise future closure issues.Methodologies discussed include: Integrated Waste Management - Significant structural improvements to tailings dams can be achieved if the structures can be incorporated into the waste rock dumps. Risk of AMD can also be reduced by utilising the tailings and waste rock effectively. This could go even further to consider the placement of wastes to limit mine void environmental issues; Process Optimisation - Water quality and the nature of potential tailings dam structures can be highly influenced by the geo-chemistry of the tailings. This can be modified by processing, where simple changes such as maintaining separate streams of geo-chemically different tailings rather than recombining them or by initiating further processing to remove problematic minerals such as sulphides; Enhancing Geophysical Properties - The geochemical performance of waste repositories can be greatly influenced by the geophysical properties of the stored materials, particularly the air-water characteristics, which are influenced by particle size, density and permeability. The geotechnical properties can be enhanced by strategic paddock dumping, blending and compaction.These methodologies are demonstrated by case histories.CITATION:Brett, D, 2013. Introduction Research has shown that when an emergency occurs, work-ers often do not get the information they need to take appropriate action. Important information about incidents is either not com-municated effectively or not at all to those affected by the event. The "Emergency Communication Triangle" safety talk focuses on the content of emergency warning messages. It presents a procedure using mental cues that can be used by senders and receivers of emergency warnings. The talk includes graphics for use during the presentation, as well as references for more information. Background Interviews with 48 miners who had escaped one of three different underground mine fires revealed that warning information provided to them was inadequate. Either insufficient information was provided to miners inby the fire areas or they failed to ask the questions needed to let them plan and execute an effective escape. Most of the miners who evacuated from these three mines did not have information that would allow them to make decisions about efficient escapes. The communication breakdown came from two directions: (1) the individuals providing the warning did not offer details about the situations even though some were known and (2) the individuals who received the warnings did not ask for clarification of the situation. The lack of information allowed miners to continue attempting to place cues into normal frameworks after they should have evaluated the situation as abnormal and threatening. The uncertainty created by the lack of information influenced their decision-making and therefore their actions. Approach Researchers at the NIOSH Pittsburgh Research Laboratory have developed a training intervention that can be used by senders and receivers of warnings during a mine emergency. This material was developed for use in short safety training sessions such as start-of-shift safety talks. The information can be presented in about 15 minutes. Although the illustrations were taken from the underground coal mining industry, the training can be tailored to any work setting by substituting appropriate examples. How It Works During the safety talk, miners learn about the six categories of critical information that should be provided during emergency communications: Who, Where, What, Miners, Event, and Response. • Who: When reporting an emergency or receiving a warning, the first thing a miner must do is identify himself or herself. This is important because people react differently based on who gives them information. • Where: Giving or finding out the location of the problem is important. Of the 48 miners interviewed about their escape, only 2 knew where the fire was located. However, this information was known by either the dispatcher or the person who discovered the fire. • What: Miners must tell or ask exactly what is happening. At one mine fire, miners near the phone heard the message to evacuate and went to gather the others on their crew. One worker shouted to a machine operator: "Come on down to the mantrip. We're going out." Because it was close to quitting time, the machine operator thought they were just leaving the section a little early and went through his normal end-of-shift routine, wasting valuable time. After giving or obtaining the three initial pieces of critical information, miners can then give or obtain details about the response in progress. • Miners: Is anyone hurt? Has everyone been accounted for? When and where was a missing person last seen?

The Bulli Seam, New South Wales, comprises medium to low volatile bituminous coal, generally of coking quality. It is worked over an area embracing 300 square miles.Variations in several factors are found.1. The seam is somewhat disturbed and dissected by various structural features.2. This structure, and present topography, have resulted in depths of cover from very shallow to approaching 2,000 ft in the presently worked areas.3. Wide variations of seam thickness occur.4. Gradations of rank occur from place to place.5. Variations in seam gas content and wide variations in seam gas composition are experienced.6. Variations in coal strength occur.These factors may contribute to the occurrence of stress and gas phenomena in mining exploitation.INTRODUCTIONThe Bulli Seam, the most extensively worked seam in Australia, is the uppermost seam of the Illawarra Coal Measures, which comprise the southern portion of the Sydney Coal Basin. Generally, the rank of the seam and its assemblage of types make it ideal for the production of hard metallurgical coke. The thickness and relatively flat dips render the seam suited to mechanized bord and pillar mining, and this method is used almost without exception. At the north-eastern corner of the field, stress and gas phenomena have occurred during mining.Various data have been assembled on physical, chemical and geological conditions of the seam which may influence the occurrence of stress and gas phenomena. This material is presented in an attempt to explain the location of such phenomena to date, and to enable variations in conditions to be considered when future possibilities are being assessed.

A study of the accessory opaque iron-titanium oxide minerals of the Oiles Complex, central Australia, was an Important complement to an investigation of the magnetic properties of the Complex. These minerals constitute no more than 10 per cent of the rocks sampled, and generally less than 1 per cent of the ultramafic rocks ("magnetite" concentrations were not sampled for magnetic investigation).Titanomagnetite is the main opaque mineral present, although ilmenite exsolution has been widely developed. High-temperature oxidation of these produced mainly titanohematite, pseudobrookite and rutile. Both exsolution and high-temperature oxidation developed along specific crystallographic directions (along {111}), and in Irregular patches. Low-temperature oxidation is generally not well-developed, indicating the small degree of weathering in the rocks sampled.Although the Oiles Complex is layered, no apparent overall preferred orientation of the iron-titanium oxide minerals was observed, other than crystallographically controlled alignment of very fine grains within pyroxene crystals. Other textural features of the opaque oxides are restricted to symplectic intergrowths mainly with the ferromagnesian silicates.

Hydro-Jex© releaching was undertaken during the summer of 2010 at the Valley Leach Facility (VLF) at AngloGold Ashanti?s Cripple Creek & Victor Gold Mine. Several advancements to the technique were made in response to the technical design and operational challenges posed by the unique site conditions. New geophysical resistivity monitoring techniques were developed to monitor solution migration during the injections. These developments are presented along with discussion of gold production resulting from the project.

"This paper discusses the project history, process design and layout of the Rainy River Gold Project mill. The circuit consists of a 55 x 83 primary crusher, 15 MW SAG mill and 15 MW ball mill. Both mills are twin pinion with LSS and VFD drives. A pebble crusher is located in the process plant with bypass and stockpile options. Gravity concentration followed by 30 hours of leach residence, carousel CIP, carbon recovery, cyanide destruction and tails disposal in a 1,428 hectare tailings management area (TMA).The Rainy River Gold Project is located approximately 65 km northwest of Fort Frances in north western Ontario. The project is scheduled to be commissioned and reach commercial production in mid-2017. Expected cost is $US877M and is comprised of an open pit mine, process plant, tailings management area and supporting infrastructure. "Certain mining methods (eg. Cut-and-fill) result in the contamination of run-of-mine ore with shotcrete. This shotcrete remains mixed with the ore throughout processing. Experimentation performed by Falconbridge Limited revealed that underground contamination of ore by shotcrete (eg. as little as 1% by weight) has led to ill-effects on the nickel recoveries from the flotation process in the mill. The nickel recoveries of contaminated samples were lower than tests without the shotcrete Contamination.To optimize flotation circuits it is important to know why the contamination has created these effects and also to find a method to abate or remove them. Laboratory testing showed that the age of the shotcrete was a factor in it’s effect on nickel recoveries. Younger aged shotcrete gave nickel recoveries lower than baseline nickel recoveries with no contamination. Older shotcrete gave nickel recoveries similar to the baseline.Literature on other cementitious material contaminations on ore revealed that a possible means of remediating this problem is the use of acid in the flotation. The initial pH in the flotation pulp was designed to be pH 9.2, but with the young shotcrete contamination the pH was in the neighbourhood of 10.5. Acid addition may act as a means of lowering the pH to the optimal operational value. Laboratory test work showed that acid addition brought nickel recoveries closer to the baseline."

The North Shore Connector (NSC) project will extend the Allegheny County Port Authority?s Light Rail Transit System from downtown Pittsburgh below the Allegheny River to the North Shore via twin bore tunnels. Jet grouting, diaphragm walls, and soil mixing were selected as cost-effective technically viable ground improvement and support of excavation techniques. Jet grouting was specified to provide break-out (egress) and break-in (ingress) block sections at the tunnel boring machine (TBM) launching and receiving pits. Jet grout treatment was also used adjacent to buildings with shallow foundations and below retaining walls along the tunnel alignment. A new method for measuring jet grout column diameter based on electrical resistivity was employed on the project. New equipment was introduced in the U.S for constructing soil mix temporary support of excavation using a soil-mix technique termed Cutter Soil Mixing (CSM) due to the use of two vertical cutting wheels that have evolved from diaphragm wall trench cutter technology. The system allows for continuous water tight soil mix panel construction including steel beams. This paper highlights some applications of jet grouting and various support of excavation systems (e.g. slurry walls, cement bentonite walls and CSM walls), including new innovative techniques (e.g. Cyljet and CSM) used under the constraints of an urban environment.

Dilution reduction is one of the areas that offer metal mines an opportunity to increase revenue and reduce costs, resulting in enhanced profits. Effective dilution control programmes require interaction on a multi-disciplinary level. This paper outlines recent initiatives undertaken at Konkola Mine to reduce dilution and increase revenue. The increase in revenue arising from reduced dilution and high factors between in situ and delivered to mill grades were quantified and amount to close to U$ 1.0 million per one per cent improvement in grade factor at a 75c/lb copper price. Sources of dilution from open and in?stope mining methods practised at the mine were identified and a multi-disciplinary dilution control forum set up to control these. Among the major achievements were the introduction of low dilution in-stope mining methods; the introduction of Gemcom geological and mining planning software to replace traditional pencil and paper methods, leading to better in situ estimation and production planning; enhanced control in open stopes through dilution awareness among mining production personnel; location of critical secondary development ends in correct positions; enhanced blast hole control; tracking of development waste by metal tracers and monthly tally; implementation of a cavity monitoring system to check over-breaks in open stopes; and setting up of a stope history database. The results from the cavity monitoring system showed that stopes with over-breaks of 5 metres or less could be controlled to break along the hangingwall contact.

Most of the papers designated as being within the topic of underground storage at this Symposium are concerned with aspects of radioactive waste isolation. Although this is, unquestionably, an important international application of underground storage, and one involving a considerable number of research investigations, the subject of underground storage is indeed much broader. Several major international conferences have been devoted exclusively to it. Applications include storage, under a range of temperatures and pressures, of oil and liquefied petroleum products, compressed air, heat, foodstuffs of various kinds, hazardous and 'nuisance' industrial by-products, and a variety of special applications, both civilian and military - some in use, others proposed. Storage may be in caverns excavated specifically for the purpose, exhausted mines may be adapted for storage or, in some cases, the material to be stored may be injected under pressure into the pore spaces of porous-permeable formations at depth. A variety of rock types and geological environments are used, e.g. excavations in salted or domal salt, where long-term creep closure of the openings is a concern; massive granite, where fluid loss through fissures may be important. Cavities may have spans of several tens of meters, and there is interest in achieving still larger excavations. As would be expected, this breadth of underground storage applications introduces a similarly broad range of questions in rock mechanics and rock engineering. The report will review these questions and recent developments in rock mechanics research, computational procedures, and design applications - and will also discuss the opportunities for a still greater range of applications of underground storage to current industrial and social problems.

The problem of estimating the precision of systematic samples from a mineral deposit is attacked by interpolating the quality, or other attribute measured, by using Fourier approximation. Such approximation using trigonometric functions yields a particularly simple expression for the variance of the mean value of the attribute sampled. The increasing volume of publication in which the science of statistics has been applied to the problem of predicting the quality of an ore deposit from sets of samples taken from it might lead one to believe that engineering judgment can be replaced by statistics. There is, in fact, an implicit application of the principle of indifference or of "no advance knowledge" which has been discredited in modern statistical theory and practice. These pages will show that it is not always advantageous, efficient, or safe to apply this principle. This problem will be dealt with by using series of orthonormal functions to describe the distribution of the quality of ore in space as contrasted with the more commonly used distribution of quality as a frequency function. Mining engineers are rarely well acquainted with the use of such series or of the functions comprising them. These uses are commonplace in the fields of communication and information theory and it is not surprising that applications taken from these fields should be found appropriate for the analysis of the information contained in a set of samples from an ore deposit. It is hoped that presentation of the following basic, but incomplete, results will generate wider interest in this method and a fuller development of it as well as of the necessary computational techniques. FUNDAMENTAL PREMISES Random variables in the strictest sense are not very common in mine sampling. The sets of samples are almost always taken systematically and are, hence, not subject to all the laws governing random events. Therefore, unless there is indeed random experimental error in the determinations of quality, the treatment of systematic samples of n members as though they were random samples of n members often leads to calculation of confidence limits for the mean quality which are larger than they should be with the result that unnecessary additional sampling is done. The reason for this will emerge in the following discussion leading to Eq. 24 and its generalization. In order to inquire into such questions on a rigorous basis and at the same time avoid arithmetic complications that would obscure the concepts involved, a one dimensional distribution of quality will be assumed.
